Objective: The study aims to determine the daily workload and to analyze the content of workload, to describe the service profile of FPs working in primary care in Turkey. Methods: The study design is descriptive and cross-sectional. A total of 28 FPs from 17 different cities were enrolled into the study. Two surveys, one inquiring the demographic properties of FPs participated and a second encounter form with 43 items inquiring all aspects of FPs’ workload were filled by the participants. Results: A total of 1,215 visits were reported. The average daily workload of FPs participating was 45.7± 16.8 visits. 92.5% of all patients were family physicians’ own registered patients while 7.5% of them were guests. A total of 1,610 RFEs were reported. We categorized them as prescription requests (451, 28%), clinical complaints (447, 27.8%), preventive medicine services (436, 27%), administrative reasons (161, %10) and other reasons (115, 7.2%). Essential hypertension and diseases of musculoskeletal system were the most common diagnoses in prescription requests. Sore throat and cough were the top two clinical complaints. Conclusions: FPs in Turkey are under severe pressure. Prescription requests account for an important percentage of this workload. Measures should be taken to lessen this burden.

Family practice’s contextual, scientific, and comprehensive core competencies need having an altruistic consciousness. As self transcendence stands on the top of modified maslow’s hierarchy of needs, primary care also need to be altrustic.

Aim. Our aim was to detect older patients who were prescribed inappropriate drugs according to START/STOPP criteria in primary care. Materials and Method. Patients aged over 65, admitted to health center no. 5 in Afyon, were included. The files of the subjects were surveyed retrospectively for the final one year in the digital environment, using the Family Medicine Information System. The files surveyed allowed us to list the drugs they used in the past year and to detect inappropriate drug use. Results. The number of patients that took part in this study was 325 (average age: years). We found that, among these participants, 48 patients (14.8%) were using drugs inappropriately according to STOPP criteria. Conclusion. Further focus on avoiding inappropriate drug use will allow clinicians and other health professionals to reduce side effects and other complications. In patients aged over 65, there is a need to attach particular importance to inappropriate drug use, drug interactions, and avoidance of side effects.

ÖzetBaş dönmesinin Amerika'da birinci basamakta yıllık insidansı %1.7, buna rağmen 2. ve 3. basamaktaki yıllık prevelansı %17'dir. Dört çeşit baş dönmesi vardır: vertigo, sersemlik, presenkop ve dengesizlik. Birinci basamakta prevelansı çeşitli çalışmalarda değişik oranlarda bildirilse de (%28, %29, %32), en yüksek oranı vertigo oluşturmaktadır. Presenkop, bilinç kaybının yaklaştığının hissidir. Dengesizlik ya da postural kararsızlık, kesin olarak hareketle ilişkili olmayan bir Vertigonun periferal ve santral sebepleri genellikle öykü ile ayırdedilebilir. Vertigolu birinci basamak hastalarının %93'ü Benign Paroksismal Pozisyonel Vertigo (BPPV), akut vestibüler nörinitis veya Meniere hastalığına sahiptir.

All people have pruritus even once in their lives. Acute pruritus, like insect bite, is having pruritus during a limited duration changing from seconds to weeks. Chronic pruritus is a kind of pruritus with a longer duration than 6 weeks. Chronic pruritus is a worldwide problem. It is encountered in all ages and a has great effect to quality of life. Other classifications exist other than acute or chronic pruritus. International Forum for the Study of Itch (IFSI)classifies itch into three groups: pruritus in diseased skin(group 1), pruritus in non-diseased skin(group 2), pruritus secondary to scratching lesions (group 3). Besides, it is classified into six specific groups according to the underlying disease: dermatologic, systemic, neurologic, psychogenic, mixed and others., Tüm insanlar hayatlarında bir kez de olsa pruritusu yaşamışlardır. Akut kaşıntı, böcek ısırması gibi, saniyelerden haftaya kadar değişen sınırlı süre kaşıntı yaşanmasıyken, kronik kaşıntı ise 6 haftadan uzun kaşıntılara verilen addır. Kronik pruritus dünya genelinde bir problemdir. Her yaşta görülebilir ve yaşam kalitesine büyük etkisi vardır. Kaşıntı için akut ve kronikten başka sınıflamalar da vardır. IFSI (International Forum for the Study of Itch: Kaşıntı Çalışması Uluslararası Forumu) sınıflaması kaşıntılı bireyleri 3 gruba ayırır; hastalıklı deride kaşıntısı olanlar (grup 1), hastalık olmayan deride kaşıntısı olanlar (grup 2), deri kaşıma lezyonlarına sekonder kaşıntısı olanlar (grup 3). Ayrıca altta yatan kaşıntılı hastalıklara göre de 6 spesifik kategoriye ayırır: dermatolojik, sistemik, nörolojik, psikojenik, miks ve diğer pruritus.
