2. Masculinity, health beliefs and implications for health policy among men in Trinidad and Tobago

Abstract

Background: Over the last three decades, men's health has emerged as an area of research interest. The relationship between male socialization, the performance of masculinity and health behaviour has been well established. Within the Caribbean, however, there is a gap in the literature on how masculinity affects health beliefs and behaviour. Aim: This study explored men's accounts of "doing" masculinity and the implications for health beliefs and behaviours in order to make recommendations for developing male sensitive health services. Methodology: Men between the ages of 19-60 years in Trinidad and Tobago participated in this study during the period August 2011 to January 2012. Fourteen focus group discussions were conducted among men from various socio-demographic groups and twelve semi structured interviews with men involved in clinical and social programs targeting males. Data were coded thematically using inductive and deductive analysis. Findings: This study is a contribution to the literature on masculinity, in general, and masculinity and health in the Caribbean, in particular. Men reported three core elements for performing masculinity: being a provider, being a leader and being heterosexually promiscuous. However, these expectations could not be realized fully in their personal experiences. Men were expected to take risks and show disregard for their own health and yet perceived being healthy as essential for 'being a man'. Views about prostate cancer screening were an example of this tension: men wanted to be healthy, were positive about screening in principle but unwilling to access services. Implications for policy/practice: Men were willing to talk openly about their health risks, suggesting that they might be willing to access services perceived as sensitive to their needs. Health information and services must be developed and implemented with men in mind to encourage greater participation in health service delivery.

Abstract

Long-term care (LTC) reflects a growing emphasis on person-centered care (PCC), with services oriented around individuals' needs and preferences. Addressing contextual and cultural differences across countries offers important insight into factors that facilitate or hinder application of PCC practices within and across countries. This article takes an international lens to consider country-specific contexts of LTC, describing preliminary steps to develop common data elements that capture contextual differences across LTC settings globally. Through an iterative series of online, telephone, and in-person sessions, we engaged in in-depth discussions with 11 colleague experts in residential LTC and coauthors from six countries (China and Hong Kong, England, Sweden, Thailand, Trinidad and Tobago, and the United States). Our discussions yielded rich narrative describing a vast range in types of LTC settings, leading to our development of a working definition of residential LTC. Scope of services, funding, ownership, and regulations varied greatly across countries and across different residential LTC settings within countries. Moving forward, we recommend expanding our activities to countries that reflect different stages of residential LTC development. Our goal is to contribute to a larger initiative underway by the WE-THRIVE consortium to establish a global research measurement infrastructure that advances PCC internationally.

Abstract

Background: Although cancer prevention strategies have resulted in a dramatic reduction in the overall cervical cancer burden in developed countries, among women with HIV the problem remains unabated. Several studies have identified limited knowledge as a major contributory factor for cervical cancer especially in vulnerable populations like women with HIV. Objective: To determine the knowledge and attitudes of HPV, Pap smear testing and HPV vaccination among women with HIV utilizing the health belief model as a guiding framework for the study. Methods: 50 HIV positive women in the 21-64 age groups were purposively sampled from the HIV clinic in San Fernando General Hospital, South Trinidad. Data were collected using an administered questionnaire with a 100% response rate after receiving ethical approval. Results: Respondents were generally knowledgeable about Pap Smear testing but had limited knowledge about HPV and HPV vaccination. Most respondents were unaware that HPV infection was a risk factor for cervical cancer. Many respondents knew that the purpose of pap testing and the recommended Pap testing guidelines for women with HIV. Although most respondents 43 (86%) considered Pap Smear testing to be important it was not a regular practice. The main reasons for not having regular Pap test include the procedure is embarrassing and painful, fear of a cervical cancer diagnosis, reluctance to be examined by male doctor and not being asked to do Pap test by healthcare professionals. Respondents also had a negative attitude when asked if they would recommend the vaccine to friends and relatives. Conclusion: While women with HIV may be knowledgeable of the risk associated with cervical cancer, this does not necessarily translate into effective screening behaviours. The findings of this study demonstrate the need for an organized and integrated cervical cancer screening program for women with HIV as an integral component of management.

Abstract

In 2013, the SickKids-Caribbean Initiative (SCI) was formalised among The Hospital for Sick Children in Toronto, Canada, the University of the West Indies, and Ministries of Health in six Caribbean countries (Barbados, The Bahamas, Jamaica, St. Lucia, St. Vincent and the Grenadines, and Trinidad and Tobago). The aim was to improve the outcomes and quality of life of children (<18 years) with cancer and blood disorders in the partner countries. Core activities included filling a human resource gap by training paediatric haematologists/oncologists and specialised registered nurses; improving capacity to diagnose and treat diverse haematology/oncology cases; developing and maintaining paediatric oncology databases; creating ongoing advocacy activities with international agencies, decision makers, and civil society; and establishing an integrated administration, management, and funding structure.
