5. Intrapartum maternal anaphylaxis to tramadol: a case report

Background: Tramadol causing intrapartum maternal anaphylaxis is unknown. Case: A 29-year-old, primigravida at 39+4 weeks pregnancy had an anaphylactic reaction to injection tramadol. She was intubated, caesarean section done and health baby and mother were discharged. Conclusion: Staffs have to be aware of this uncommon anaphylactic reaction to tramadol.

Open Source Software (OSS) is updated regularly to meet the requirements posed by the customers. The source code of OSS undergoes frequent change to diffuse new features and update existing features in the system, providing a user friendly interface. The source code changes for fixing bugs and meeting user end requirements again affects the complexity of the code change and creates bugs in the software which are accountable to the next release of software. In this paper, the complexity of code changes in various Bugzilla open source software releases, from version 2.0 on 19th Sep, 1998, to 5.0.1 on 10th Sep, 2015, bugs in each software version release, and the time of release of each software version are considered, and the data used to predict the next release time. The Shannon entropy measure is used to quantify the code change process in terms of entropy for each software release. Observed code changes are utilized to quantify them into entropy units and are further used to predict the next release time. A neural network-based regression model is used to predict the next release time. The performance is compared with the R measure calculated using the multi linear regression model, and a goodness of fit curve is produced.

The main contribution of this paper is decomposition/separation of the compositie induction motors load from measurement at a system bus. In power system transmission buses load is represented by static and dynamic loads. The induction motor is considered as the main dynamic loads and in the practice for major transmission buses there will be many and various induction motors contributing. Particularly at an industrial bus most of the load is dynamic types. Rather than traing to extract models of many machines this paper seeks to identify three groups of induction motors to represent the dynamic loads. Three groups of induction motors used to characterize the load. These are the small groups (4kw to 11kw), the medium groups (15kw to 180kw) and the large groups (above 630kw). At first these groups with different percentage contribution of each group is composite. After that from the composite models, each motor percentage contribution is decomposed by using the least square algorithms. In power system commercial and the residential buses static loads percentage is higher than the dynamic loads percentage. To apply this theory to other types of buses such as residential and commerical it is good practice to represent the total load as a combination of composite motor loads, constant impedence loads and constant power loads. To validate the theory, the 24hrs of Sydney West data is decomposed according to the three groups of motor models.

A patient at her 38+ wks of pregnancy as a ultrasonographically diagnosed case of conjoined twin admitted in our department with ruptured membrane. Two live female babies joined at the chest were delivered by caesarean section. The parents refused a separation operation and the mother and the babies were discharged from hospital at 6th post natal day. A review of the literature suggests that early diagnosis by a combination of ultrasound and MRI is essential for management as it provides prognosis for viability and process of surgical separation and also the opportunity for early counseling of parents and termination if indicated. Embaràs i malaltia cardíaca; Resultats de l’embaràs; Transposició de les grans artèries Embarazo y enfermedad cardíaca; Resultados del embarazo; Transposición de las grandes arterias Pregnancy and cardiac disease; Pregnancy outcomes, Transposition of the great arteries Background In the past 3 decades, the arterial switch procedure has replaced the atrial switch procedure as treatment of choice for transposition of the great arteries. Although survival is superior after the arterial switch procedure, data on pregnancy outcomes are scarce and transposition of the great arteries after arterial switch is not yet included in the modified World Health Organization classification of maternal cardiovascular risk. Methods and Results The ROPAC (Registry of Pregnancy and Cardiac disease) is an international prospective registry of pregnant women with cardiac disease, part of the European Society of Cardiology EURObservational Research Programme. Pregnancy outcomes in all women after an arterial switch procedure for transposition of the great arteries are described. The primary end point was a major adverse cardiovascular event, defined as combined end point of maternal death, supraventricular or ventricular arrhythmias requiring treatment, heart failure, aortic dissection, endocarditis, ischemic coronary events, and thromboembolic events. Altogether, 41 pregnant women (mean age, 26.7±3.9 years) were included, and there was no maternal mortality. A major adverse cardiovascular event occurred in 2 women (4.9%): heart failure in one (2.4%) and ventricular tachycardia in another (2.4%). One woman experienced fetal loss, whereas no neonatal mortality was observed. Conclusions Women after an arterial switch procedure for transposition of the great arteries tolerate pregnancy well, with a favorable maternal and fetal outcome. During counseling, most women should be reassured that the risk of pregnancy is low. Classification as modified World Health Organization risk class II seems appropriate. ObjectiveCardiac disease is a major cause of maternal mortality. Data regarding pregnancy outcomes in women with a systemic right ventricle (sRV) are scarce. We studied pregnancy outcomes in women with an sRV after the atrial switch procedure for transposition of the great arteries (TGA) or congenitally corrected TGA (CCTGA).MethodsThe ESC EORP Registry of Pregnancy and Cardiac Disease is an international prospective registry of pregnant women with cardiac disease. Pregnancy outcomes (maternal/fetal) in all women with an sRV are described. The primary end point was a major adverse cardiac event (MACE) defined as maternal death, supraventricular or ventricular arrhythmias requiring treatment, heart failure, aortic dissection, endocarditis, ischaemic coronary event and other thromboembolic events.ResultsAltogether, 162 women with an sRV (TGA n=121, CCTGA n=41, mean age 28.8±4.6 years) were included. No maternal mortality occurred. In 26 women, at least one MACE occurred, heart failure in 16 (9.8%), arrhythmias (atrial 5, ventricular 6) in 11 (6.7%) and others in 4 (2.5%). Prepregnancy signs of heart failure as well as an sRV ejection fraction ConclusionThe majority of women with an sRV tolerated pregnancy well with a favourable maternal and fetal outcome. Heart failure and arrhythmias were the most common MACE.

BackgroundCardiovascular disease is the leading cause of death during pregnancy with thoracic aortic dissection being one of the main causes. Thoracic aortic disease is commonly related to hereditary disorders and congenital heart malformations such as bicuspid aortic valve (BAV). Pregnancy is considered a high risk period in women with underlying aortopathy.MethodsThe ESC EORP Registry Of Pregnancy And Cardiac disease (ROPAC) is a prospective global registry that enrolled 5739 women with pre-existing cardiac disease. With this analysis, we aim to study the maternal and fetal outcome of pregnancy in women with thoracic aortic disease.ResultsThoracic aortic disease was reported in 189 women (3.3%). Half of them were patients with Marfan syndrome (MFS), 26% had a BAV, 8% Turner syndrome, 2% vascular Ehlers-Danlos syndrome and 11% had no underlying genetic defect or associated congenital heart defect. Aortic dilatation was reported in 58% of patients and 6% had a history of aortic dissection. Four patients, of whom three were patients with MFS, had an acute aortic dissection (three type A and one type B aortic dissection) without maternal or fetal mortality. No complications occurred in women with a history of aortic dissection. There was no significant difference in median fetal birth weight if treated with a beta-blocker or not (2960 g (2358–3390 g) vs 3270 g (2750–3570 g), p value 0.25).ConclusionThis ancillary analysis provides the largest prospective data review on pregnancy risk for patients with thoracic aortic disease. Overall pregnancy outcomes in women with thoracic aortic disease followed according to current guidelines are good.

20. Adsorption Behavior of Methylene Blue Cationic Dye in Aqueous Solution Using Polypyrrole-Polyethylenimine Nano-Adsorbent.

- Abstract
In this work, a polypyrrole-polyethyleneimine (PPy-PEI) nano-adsorbent was successfully synthesized for the removal of methylene blue (MB) from an aqueous solution. Synthetic dyes are among the most prevalent environmental contaminants. A new conducting polymer-based adsorbent called (PPy-PEI) was successfully produced using ammonium persulfate as an oxidant. The PEI hyper-branched polymer with terminal amino groups was added to the PPy adsorbent to provide more effective chelating sites for dyes. An efficient dye removal from an aqueous solution was demonstrated using a batch equilibrium technique that included a polyethyleneimine nano-adsorbent (PPy-PEI). The best adsorption parameters were measured at a 0.35 g dosage of adsorbent at a pH of 6.2 and a contact period of 40 min at room temperature. The produced PPy-PEI nano-adsorbent has an average particle size of 25-60 nm and a BET surface area of 17 m
2 /g. The results revealed that PPy-PEI nano-composite was synthesized, and adsorption was accomplished in the minimum amount of time. The maximum monolayer power, qmax, for MB was calculated using the isothermal adsorption data, which matched the Langmuir isotherm model, and the kinetic adsorption data, which more closely fitted the Langmuir pseudo-second-order kinetic model. The Langmuir model was used to calculate the maximum monolayer capacity, or qmax, for MB, which was found to be 183.3 mg g-1 . 21. Association Between Fear of COVID-19 and Emotional Distress in Nurses With Mediating Role of Socio-Demographic Features.

Objectives: To determine the predictive association between fear of COVID-19 and emotional distress (depression, anxiety, and stress) in frontline and non-frontline nurses. To explore the mediating role of socio-demographic features. Methods: Correlational cross-sectional research design was implied. A total of 500 on-duty male and female, frontline and non-frontline, nurses were included from five major hospitals in Gujrat (Aziz Bhatti Shaheed Hospital, City Hospital, Doctors Hospital, Akram Hospital, and Gujrat Hospital). Fear of COVID-19 scale and the Urdu version of depression, anxiety, and stress scale - 21 (DASS-21) were used to measure variables of interest. Descriptive statistics, structural equation modeling (SEM), linear regression, and t -test were carried out using Statistical Package for Social Sciences (SPSS) 21. Result: Structural equation modeling (SEM) revealed a significant predictive link between fear of COVID-19 and depression, anxiety, and stress (goodness of model fit; NFI = 0.93, GFI = 0.914, AGFI = 0.93, CFI = 0.936, and IFI = 0.936). Furthermore, a significant mediating effect of certain demographic features was discovered by SEM (CMIN/DF = 1.11, NFI = 0.94, TLI = 0.98, GFI = 0.08, AGFI = 0.93, RMSEA = 0.029, CFI = 0.99, and IFI = 0.99). Results of linear regression analysis also revealed a momentous predictive association between fear of COVID-19 and emotional distress ( R = 0.860). In comparative analysis, the results of t-test explored the statistical significant difference in fear of COVID-19 and emotional distress between frontline (mean = 25.775, 36.147 and SD = 1.75, 2.23) and non-frontline nurses (mean = 21.702, 27.353 and SD = 4.607, 10.212), with t

Conclusion: Managing the mediating effect of demographic characteristics and reducing the fear of COVID-19 can help nurses to overcome emotional distress, such as depression, anxiety, and stress. Nanoparticles (nanoparticles) have received much attention in biological application because of their unique physicochemical properties. The metal- and metal oxide-supported nanomaterials have shown significant therapeutic effect in medical science. The mechanisms related to the interaction of nanoparticles with animal and plant cells can be used to establish its significant role and to improve their activity in health and medical applications. Various attempts have been made to discuss the antibiotic resistance and antimicrobial activity of metal-supported nanoparticles. Despite all these developments, there is still a need to investigate their performance to overcome modern challenges. In this regard, the present review examines the role of various types of metal-supported nanomaterials in different areas such as antibacterial, antifungal, anticancer, and so on. - Abstract
Parabens, the alkyl esters of p-hydroxybenzoic acid such as methylparaben (MeP), ethylparaben (EtP), propylparaben (PrP), butylparaben (BuP) are used as a preservative in food, personal care products (PCPs), and pharmaceuticals, due to their antimicrobial properties. Parabens are continuously released into the environment, during washout of PCPs, disposal of industrial waste from the pharmaceutical and paper industries. Parabens have been detected in the indoor dust, wastewater stream, surface water of rivers, and the marine system. Recent eco-toxicological data and the environmental presence of parabens, has raised concerns regarding the safety and health of environment/humans. Thus, to further understand the toxicity of parabens, the present study was carried out in the soil nematode and well established biological model organism Caenorhabditis elegans. In the present study, LC
50 of MeP, EtP, PrP and BuP for 72 h exposures from L1 larva to adult stage was found to be 278.1, 217.8, 169.2, and 131.88 μg/ml, respectively. Further exposure to 1/5th of LC50 of parabens yielded an internal concentration ranging from 1.67 to 2.83 μg/g dry weight of the organism. The toxicity of parabens on the survival, growth, behavior, and reproduction of the C. elegans was found in the order of BuP > PrP > EtP > MeP. Worms exposed to parabens show significant down-regulation of vitellogenin genes, high levels of reactive oxygen species and anti-oxidant transcripts, the latter being concordant with nuclear localization of DAF-16 and up-regulation of HSF-1 and SKN-1/Nrf. - Abstract
Background: With advances in reverse vaccinology approaches, a progressive improvement has been observed in the prediction of putative vaccine candidates. Reverse vaccinology has changed the way of discovery and provides a mean to propose target identification in reduced time and labour. In this regard, high throughput genomic sequencing technologies and supporting bioinformatics tools have greatly facilitated the prompt analysis of pathogens, where various predicted candidates have been found effective against certain infections and diseases. A pipeline, VacSol, is designed here based on a similar approach to predict putative vaccine candidates both rapidly and efficiently., Results: VacSol, a new pipeline introduced here, is a highly scalable, multi-mode, and configurable software designed to automate the high throughput in silico vaccine candidate prediction process for the identification of putative vaccine candidates against the proteome of bacterial pathogens. Vaccine candidates are screened using integrated, well-known and robust algorithms/tools for proteome analysis, and the results from the VacSol software are presented in five different formats by taking proteome sequence as input in FASTA file format. The utility of VacSol is tested and compared with published data and using the Helicobacter pylori 26695 reference strain as a benchmark., Conclusion: VacSol rapidly and efficiently screens the whole bacterial pathogen proteome to identify a few predicted putative vaccine candidate proteins. This pipeline has the potential to save computational costs and time by efficiently reducing false positive candidate hits. - Abstract
We planned to investigate the rates of Caesarean Section (CS), potential Vaginal Births After previous Caesarean Section (VBAC), and successful VBAC in a secondary care hospital. We conducted an analytical retrospective study at Aga Khan Hospital for Women, Karimabad, Karachi, from October 2011 to September 2012. Data related to total deliveries, Lower Segment CS (LSCS), attempted VBAC and successful VBAC was retrieved from medical records. Total number of deliveries were 3266. Of these, 1021(31.26%) deliveries were conducted by CS. A total of 365(11.1%) had a previous history of one CS and VBAC trial was given to 33(9%) of these pregnancies. The success rate of VBAC was 21 (63.6%). Our results highlight that despite having limited resources, our rates of CS, VBAC trials and successful VBACs were within reasonable limits when compared with international rates.

- Abstract
The percentage of overweight and obese person has increased markedly since several decays. Obesity is associated with increased risked factor for many diseases such as, diabetes, heart complications, arthritis and certain types of cancer. Feeding behavior is in controlled by a major interaction between central nervous system and many organs of the body. The role of serotonin (5-HT) in feeding behavior is well recognized. The aim of present study was to evaluate the effect of Anethum graveolens seeds aqueous extract (AGAE) on food intake, body weight and serotonin metabolism in over weight rats. Five weeks oral administration of AGAE shows significant decrease in body weight, food intake and significant increase in whole brain 5-HT, 5-HIAA and tryptophan level in brain and plasma of experimental animals. Increased level of 5-HT induced satiety and suppressed food intake and result is the reduction in body weight.

Use of the herbal drugs increasing all over the world due to its minimum side effect. Nigella sativa black seeds used in folk medicine for the promotion of good health and for the treatment of many diseases .The present study is designed to investigate the neurochemical and behavioral effect of aqueous extract of Nigella sativa L. seeds in rats. Neurochemical studies were performed for DA and DOPAC levels in whole rats' brain. Locomotive behavior was observed in novel environment and familiar environment. Elevated plus maze and light dark behavioral modules were used to monitor anxiety in rats. The oral administration of AENS for six weeks increased time spent in open arm of elevated plus maze and light compartment in light dark box. Increased locomotors activity in novel environment (open field) was noticed suggesting that increased in DA level may be related to increased locomotive activity in rats.

Objective: To determine the effect of buspirone and propranolol on restraint stress-induced behavioral deficits in a learned helplessness model., Design: Experimental study., Place and Duration of Study: The experiment was performed in the Department of Biochemistry, University of Karachi and completed during a period of three months (3 June 2004 to 17 August 2004)., Subjects and Methods: The study was conducted on 36 male albino Wistar rats. Animals injected with saline, buspirone (0.2 mg/kg) and propranolol (1 mg/kg) for 2 weeks were exposed to an episode of 2 hours restraint stress. Cumulative food intake and body weight changes were monitored over a period of 24 hours. Exploratory activity in the lit area of light dark box was monitored 24 hours after the termination of restraint period., Results: Single (2 hours) restraint stress decreased 24 hours cumulative food intake and growth rate. Exploratory activity in the lit compartment of a light-dark box also decreased. Prior administration of buspirone as well as propranolol attenuated restraint-induced deficits of food intake and growth rate. Administration of buspirone and propranolol increased lit area exploration in unrestrained animals and attenuated restraint-induced deficits of exploration., Conclusion: The results show that anorexiogenic and anxiogenic effects of restraint stress were smaller in animals injected with buspirone or propranolol. Buspirone and propranolol could attenuate stress-induced behavioral deficits.

- Abstract
Anorexia or loss of appetite, one of the most typical symptoms observed in experimental and human cirrhosis, has been proposed to be associated with altered brain serotonin (5-HT) metabolism. In order to evaluate this hypothesis, brain 5-HT, its precursor tryptophan (TRP) and its metabolite 5-hydroxyindole-acetic acid (5-HIAA) were measured in brains of rats with thioacetamide (TAA)-induced liver cirrhosis. Thioacetamide at a dose of 500 mg/l in drinking water was administered for 6 weeks and during this period food intake was carefully measured in order to monitor the loss of appetite or decrease in food intake observed in cirrhosis. Concentrations of brain TRP, 5-HT and 5-HIAA were measured by HPLC with electrochemical detection. In TAA-treated rats, concentrations of 5-HT, TRP and 5-HIAA were increased in brain (44%, 33% and 36% of controls, p < 0.01). In plasma and liver of cirrhotic rats, TRP levels were increased (195% and 43%; p < 0.01). Plasma glucose and albumin levels were decreased (50%; p < 0.01 and 31%). Food intake, growth rate and locomotor activity of TAA-treated rats also decreased (73%, 22% and 73% of controls; p < 0.01). The results of this study show that brain 5-HT concentration in rats is increased in TAA-treated rats and it may, therefore, play an important role in the pathogenesis of anorexia associated with TAA-induced cirrhosis.

31. Neurochemical and behavioural effects of long term intake of tea.

- Abstract
Tea consumption in many cases is the main source of caffeine intake in humans. In the present study neurochemical and behavioural effects of long-term tea intake are monitored in rats. Long-term tea administration did not alter plasma tryptophan (TRP) but significantly attenuated brain TRP and 5-hydroxytryptamine (5-HT, serotonin) levels. Brain 5-hydroxyindole acetic acid (5-HIAA) was comparable in both tea-treated and control rats. An increase in home cage activity was observed after one week in rats taking tea as sole source of liquid, whereas no change on the activity was observed in an open field. Caffeinism has been associated with depression. The decreases of brain monoamine metabolism observed in present study are discussed as lowering of mood observed in tea or coffee consumers.

32. The neurochemical profile of long term oral administration of moclobemide.

- Abstract
Moclobemide, a benzamide derivative, predominantly inhibits the A form of monoamine oxidase (MAO) and its MAO binding is reversible. Acute administration of moclobemide has been shown to induce an increase in brain levels of monoamines and a concomitant decrease in their respective metabolite. In the present study, the drug was administered to rats orally in drinking water at doses of 0.5-1.0 mg/day/rat of an average weight of 250 g for three weeks. This was equivalent to the recommended human dose of 150-300 mg/day. The drug administration did not alter food intake, growth rate and activity of rats. Brain levels of 5-hydroxytryptamine (5-HT) and dopamine (DA) increased. However, increases in 5-hydroxyindoleacetic acid (5-HIAA), dihydroxyphenylacetic acid (DOPAC) and homovanillic acid (HVA) as reported in acute studies were not observed following chronic drug administration in the present study in addition, an increase in brain levels of tryptophan also occurred. Neurochemical profile of long-term moclobemide administration is explainable in terms of an inhibition of MAO activity, increased availability of 5-HT precursor tryptophan and decreased egress of monoamine metabolites.

- Abstract
We report two cases of patients with intracranial tumors that share features with lipomas and teratomas. Although rare reports of lipomas with "hypertrophic nerves" and "teratoid tumors" have been recorded, these two cases are unusual because they contain mature neuroectoderm (choroid plexus, peripheral nerve) and mesoderm (skeletal muscle). The findings are discussed and modern classification schemes are presented. We believe that the cases are examples of a transition between lipoma and teratoma.

- Abstract
We describe a malignant lymphoma arising in the kidney that exhibited clinical and histologic features of low-grade B-cell lymphoma of mucosa-associated lymphoid tissue (MALT). Clinically, the neoplasm involved the kidney and one perirenal lymph node (stage IIE) and did not recur or disseminate following nephrectomy, lymphadenectomy, and local radiation therapy. Histologically, the neoplasm recapitulated the features of low-grade B-cell lymphomas of MALT at other sites. The neoplastic cells resembled centrocyte-like cells, some of which formed lymphoepithelial lesions with renal tubules, and reactive lymphoid follicles were scattered within the neoplasm. The neoplastic cells expressed monotypic cytoplasmic Ig kappa. Low-grade B-cell MALT lymphomas arising in the kidney are rare. Their occurrence further demonstrates the diversity of anatomical sites that may be involved by MALT lymphomas. In addition, this neoplasm also had histologic and cytologic features resembling those of monocytoid B-cell lymphoma, supporting the hypothesis that low-grade B-cell lymphomas of MALT and monocytoid B-cell lymphomas are closely related and may be two morphological manifestations of the same neoplasm.

- Abstract
Benign fibrous tumor of the tunica vaginalis testis is an uncommon lesion of unknown pathogenesis and histogenesis, frequently described as fibrous pseudotumor. We describe a case of a fibrous tumor of the tunica vaginalis testis studied with light, immunocytochemical, and electron microscopy in a 64-year-old man who also developed a metachronous renal cell carcinoma. To our knowledge, this is the first such case to be presented. We describe the cell of origin of this rare entity and its similarity to fibrous tumors of pleura and extrapleural sites, with a review of the English-language literature and emphasize that not all intrascrotal tumors are malignant.
