Introduction: A retrospective data analysis was performed on 17 eyes from 13 horses which underwent a sub-Tenon's injection to facilitate phacoemulsification or pars plana vitrectomy under general anesthesia between 2018 and 2022. All procedures were performed by the same veterinary ophthalmologist. Seven eyes received a sub-Tenon's dose of 7 ml lidocaine (XylocainÒ, 2 % lidocaine hydrochloride, Germany, Aspen Germany GmbH), and 10 eyes received 7 ml mepivacaine (MepinaestÒ purum 2 %, mepivacaine hydrochloride, Switzerland, Gebro Pharma GmbH). Statistical analysis compared onset and duration of globe centralization and pupil mydriasis between the two groups. Intraoperative and postoperative complications were also assessed. Mepivacaine had a significantly later onset of action regarding globe centration and mydriasis (8,9 minutes vs. 6 minutes), but also a significantly longer duration of globe centration than lidocaine (31,5 minutes vs. 15,6 minutes). There were no statistically relevant differences between solutions regarding duration of pupil dilation (40,4 minutes for 2 % lidocaine vs. 69,2 minutes for 2 % mepivacaine). Chemosis occurred in all 17 eyes. Surgical complications included corneal epithelial defects (5), retinal detachment (5), lens opacification (5), temporary blindness during recovery (3) and glaucoma (2). Sub-Tenon's anesthesia is a feasible alternative to systemic neuromuscular blockade and retrobulbar block anesthesia for surgical procedures on the equine globe. A controlled prospective in vivo study is needed to further evaluate effects and risks.

Background: The occurrence of cataracts in rabbits is a common reason for consultation in veterinary ophthalmology., Case Description: A 6-year-old female Lop rabbit ( Oryctolagus cuniculus ) was referred with a 3-month history of bilateral cataracts. The owner noted that the rabbit was frequently bumping into obstacles. Physical examination was unremarkable. Routine hemogram and biochemistry panel were within usual values, and Encephalitozoon cuniculi were not detected by serology. A complete ophthalmological examination revealed bilateral hypermature cataracts causing total blindness. Bilateral tonometry showed normal intraocular pressures. Electroretinography (ERG) under general anesthesia revealed strong photoreceptor responses for each eye, and ocular ultrasound was normal except for bilateral hypermature cataracts. Following ERG, during the same anesthesia session, conventional phacoemulsification followed by acrylic intraocular lens (IOL; +58D) implantation was performed uneventfully in both eyes. Follow-up examinations were carried out for 1 year. Except for immediate post-operative bilateral corneal ulcers that were quickly cured with medical treatment, no other post-operative complications were observed. The 1-year follow-up examination revealed centered IOLs without posterior capsular opacity, enabling the observation of normal fundus in both eyes. The owner confirmed the rabbit had normal autonomy and moved with accuracy, as observed during follow-up visits and on videos provided by the owner., Conclusion: To the best of our knowledge, this is the first described case of successful bilateral phacoemulsification and bilateral intraocular lens implantation with prior electroretinographic evaluation of the retinal function in a pet rabbit with cataracts., Competing Interests: The authors declare no conflict of interest.

Objective: To evaluate refractive state outcomes following phacoemulsification and implantation of 3 different intraocular lenses (IOLs)., Animals: A prospective, randomized, controlled study was conducted on 43 client-owned dogs undergoing phacoemulsification with IOL implantation., Methods: Eyes were randomized to receive either an-vision Fo-X (n = 26), an-vision MD8 (18), or I-MED I-LENS (24) IOL. Refraction was measured 1 week, 1 month, and 3 months postoperatively using streak retinoscopy by 2 examiners masked to each other's results., Results: Postoperative refractive outcomes were highly correlated and not significantly different between 2 examiners for all time points (r = 0.97, 0.98, and 1.00; P = .76, .94, and .98, respectively). One week postoperatively, the refractive errors (mean ± SD) for Fo-X, MD8, and I-LENS were -0.14 ± 2.02 diopters (D), 0.97 ± 2.01 D, and 0.15 ± 2.55 D, respectively. One month postoperatively, the refractive errors were 0.35 ± 2.04 D, 0.06 ± 2.41 D, and -0.82 ± 2.20 D, respectively. Three months postoperatively, the refractive errors were -0.16 ± 2.67 D, 1.60 ± 2.99 D, and 0.59 ± 1.51 D, respectively. There were no significant differences in refractive error outcomes between Fo-X, MD8, and I-LENS at 1 week, 1 month, and 3 months postoperatively (P = .16; F(df=2,66)- = 1.89). However, the Fo-X was the only IOL to yield nearly emmetropic outcomes (±0.50 D) at all 3 time points., Clinical Relevance: The postoperative refractive states of dogs were not statistically different when comparing 3 types of IOLs at 3 postoperative time points, though the Fo-X was the only IOL to yield nearly emmetropic outcomes at all 3 time points.

Background: Long-term comparisons of phacoemulsification with topical medication are limited in canine diabetic cataracts., Objectives: To compare outcomes of eyes submitted to phacoemulsification with those of topical medication for canine diabetic cataracts and identify risk factors for complications., Methods: Through medical records review, 150 eyes (76 dogs) with diabetic cataracts were included; 58 eyes (31 dogs) underwent phacoemulsification (phaco-group) and 92 eyes (48 dogs) received ophthalmic solution alone (medication-group). The medication-group was divided into owner-led and vet-led groups depending on who elected not to perform surgery. Comparisons involved time-to-complications, vision, and the number and type of ophthalmic solutions administered. The association between complications and pretreatment clinical findings was investigated., Results: No difference was found in complication risk between the phaco and owner-led medication groups. Conversely, the vet-led medication-group had a higher complication risk than the other groups. At the last follow-up, 94.8% of the phaco-group had vision, whereas 7.6% of the medication-group restored some visual axis. Poor glycemic control in the medication-group and younger age in the phaco-group increased complication risk. At 1-year post-treatment, the average number of ophthalmic solutions administered was 1.7 and 2.6 in the phaco and medication groups, respectively. The medication-group used anti-inflammatories the most throughout the follow-up, whereas the phaco-group used anti-inflammatories the most until 1-year post-treatment and lacrimostimulants at 1.5-year post-treatment., Conclusions: For canine diabetic cataracts, phacoemulsification is recommended because it is superior to topical management alone in terms of maintaining vision and reducing the number of ophthalmic solutions required in the long term., Competing Interests: The authors declare no conflicts of interest., (© 2023 The Korean Society of Veterinary Science.)

Background: Posterior capsular opacification (PCO) is a cause of decreased vision after canine cataract surgery. PCO can appear in the months following surgery but can also be present at the time of surgery., Aims: To describe the intraoperative management of marked axial PCO during canine cataract surgery through a retrospective case series study of six cases., Methods: Six dogs (six eyes) with cataracts were included in this study. A complete ophthalmologic examination including electroretinography and ocular ultrasound was performed. After conventional phacoemulsification, a marked PCO obstructing the visual axis was found in five cases, previously diagnosed by ultrasound in the sixth. An axial posterior capsulorhexis (APC) was performed in all cases, before or after implantation of the intraocular lens (IOL). For the four cases implanted before APC, the posterior capsule (PC) was visualized either by positioning the IOL laterally in the bag with viscoelastic or through the optic of the implant. After perforation of the PC with a 30-Gauge needle, APC was initiated with Vannas curved scissors, then finalized with Utrata forceps to obtain a circular axial opening (3 mm diameter). In each case, a moderate anterior vitrectomy was performed through the APC (under the IOL when initially placed), then the IOL centered and the viscoelastic was removed., Results: Six dogs (Beagle, German Shepherd, Cavalier King Carles, French pointing dog, American bully, Beagle Harrier) aged 11 to 94 months (mean 51.8) were included. The mean follow-up period was 15.5 months (range 10-22). Visual function with capsular axial transparency and well-centered IOL, without complications during the follow-up period, was preserved for each eye., Conclusion: APC combined with moderate anterior vitrectomy appears to be effective in the treatment of marked axial PCO obstructing the visual axis during canine cataract surgery., Competing Interests: The author declares no conflict of interest.

Background: Long-term comparative data of phacoemulsification combined with endoscopic cyclophotocoagulation (phaco-ECP) versus phacoemulsification (phaco) alone in dogs are rare., Objectives: To investigate the effects of ECP on postoperative intraocular pressure (IOP) and complications after phaco in dogs with normal IOP., Methods: Medical records of IOP, conjunctival hyperemia, corneal edema, aqueous flare, posterior synechia, intraocular fibrin, and posterior capsule opacification (PCO) formation in 15 canine eyes that underwent phaco-ECP and 36 eyes that underwent phaco alone were evaluated retrospectively. ECP was applied when either the iridocorneal angle or the ciliary cleft was narrow or closed., Results: The IOP of the phaco-ECP group persisted within the normal range postoperatively. The phaco-ECP group had a shorter period of dorzolamide use than did the phaco group. PCO was formed earlier in the phaco-ECP group than in the phaco group. The phaco-ECP group showed more severe corneal edema than the phaco group at every follow-up visit. Posterior synechia was more severe in the phaco-ECP group than in the phaco group from two weeks until the last follow-up., Conclusions: Although ECP might cause more postoperative complications such as corneal edema and posterior synechia, it could effectively reduce the incidence of IOP increase after phaco in dogs with a high risk of postoperative glaucoma., Competing Interests: The authors declare no conflicts of interest., (© 2023 The Korean Society of Veterinary Science.)

A 4-year-old male Canada lynx (Lynx canadensis) was referred to the ophthalmology service at the University of Saskatchewan with a 7-month history of cataract and chronic phacoclastic uveitis secondary to penetrating trauma from a lynx claw. Ophthalmic examination of the right eye revealed a corneal scar, marked aqueous flare, extensive fibrovascular membranes extending from the iris to the lens, anterior and posterior synechiae, immature cataract, and anterior vitritis; the fundus was not visible. Phacoemulsification surgery and intraocular lens implantation using a custom lens of D+46 and 14 mm (An-vision, West Jordan, Utah, USA) was performed. Post-operative medications included sub-conjunctival injections of atropine, cefazolin, and triamcinolone, and oral doxycycline and prednisolone. At the 5-month follow-up, the uveitis was controlled, and a normal fundus was visualized; at 21 mo, the eye remained comfortable and visual. This is the first case report to describe phacoemulsification in a wild felid as a treatment for a traumatic cataract and severe phacoclastic uveitis. Key clinical message: Despite chronic phacoclastic uveitis, phacoemulsification surgery can provide a positive outcome for mature wild felids with traumatic lens rupture, even when topical treatment cannot be administered., (Copyright and/or publishing rights held by the Canadian Veterinary Medical Association.)

Case Description: A 7-month-old hand-reared female Bennett's wallaby (Macropus rufogriseus) was evaluated for bilateral ocular opacity of 3 months' duration., Clinical Findings: On physical examination, the wallaby was in good overall condition. An ophthalmic examination revealed mature cataracts in both eyes (OU). The cataracts were suspected to have a nutritional origin as it has been described in hand-reared macropods. Results of tonometry were normal OU. Results of CBC and serum biochemistry were unremarkable. The wallaby was premedicated with medetomidine and ketamine hydrochloride. Ocular ultrasonography performed while the wallaby was anesthetized revealed a thickened and opaque lens, mildly heterogeneous vitreous humor, and no sign of retinal detachment OU. An electroretinogram confirmed functional retinae OU., Treatment and Outcome: Surgery was considered necessary for welfare reasons. Phacoemulsification and vitrectomy were performed without intraoperative complication. The wallaby recovered uneventfully, and systemic NSAID, topical corticosteroid, and systemic and topical antimicrobial therapy were prescribed. One year later, the animal had vision OU. Physical examination did not show any vision-threatening postoperative complications., Clinical Relevance: The etiology of nutritional cataracts is not fully understood in macropods, but this condition is rather common in these species. To our knowledge, this is the first detailed report of a successful bilateral phacoemulsification and vitrectomy in a Bennett's wallaby.

Objective: To calculate the necessary pseudophakic intraocular lens (IOL) power to approximate emmetropia in adult tigers., Animals: 17 clinically normal adult tigers., Procedures: 33 eyes of 17 clinically normal adult tigers underwent routine ophthalmic examination and B-scan ultrasonography while anesthetized for unrelated procedures. Specific ultrasound data (globe measurements and corneal curvature) and estimated postoperative IOL positions were utilized to calculate predicted IOL power by use of Retzlaff and Binkhorst theoretical formulas. Applanation tonometry and refraction were also performed., Results: Mean ± SD axial globe length was 29.36 ± 0.82 mm, preoperative anterior chamber depth was 7.00 ± 0.74 mm, and crystalline lens thickness was 8.72 ± 0.56 mm. Mean net refractive error (n = 33 eyes) was +0.27 ± 0.30 diopters (D). By use of the Retzlaff formula, mean predicted IOL power for the postoperative anterior chamber depth (PACD), PACD - 2 mm, and PACD + 2 mm was 43.72 ± 4.84 D, 37.62 ± 4.19 D, and 51.57 ± 5.72 D, respectively. By use of the Binkhorst equation, these values were 45.11 ± 4.91 D, 38.84 ± 4.25 D, and 53.18 ± 5.81 D, respectively. Mean intraocular pressure for all eyes was 14.7 ± 2.69 mm Hg., Clinical Relevance: The calculated tiger IOL was lower than reported values for adult domestic felids. Further studies evaluating actual PACD and pseudophakic refraction would help determine the appropriate IOL power to achieve emmetropia in this species.

Background: Cataracts are the major cause of visual impairment in animals which can be curable by surgical treatment. Phacoemulsification is the standard technique for cataract treatment that is applied to almost all species with a high success rate., Case Description: A 2-year-old intact female orangutan ( Pongo pygmaeus ) was presented for the study having bilateral opacity of the lenses, for 2 weeks. Ophthalmic examination revealed mature cataract OU. Ocular biometry measurements using A-scan ultrasonography for appropriate intraocular lens (IOL) refractive power calculation were carried out. Electroretinography was applied to ensure retinal function is intact. The orangutan underwent phacoemulsification OU and +24 diopter IOL implantation OS to restore vision. IOL implantation was not carried out OD because of a posterior capsular tear. Retinoscopy after 3 weeks postoperatively revealed +2.0 diopters OS. The outcome of the cataract surgery was successful during 3 years follow-up. The orangutan lived with other orangutans and was alert with normal behavior such as catching food, climbing trees, and swinging hand over hand from one branch to another., Conclusion: Cataract surgery with phacoemulsification OU and adjusted IOL implantation OD was successful with few complications in this orangutan. A twenty-year-old male Asiatic black bear (Ursus thibetanus) presented at the Rakuno Gakuen University Animal Medical Center with a 10-year history of bilateral blindness and cataracts. Surgical treatment of bilateral cataracts by extracapsular lensextraction using phacoemulsification and aspiration (PEA) was performed under general anesthesia. An anterior capsulectomy was performed using micro iris scissors and micro anterior lens capsule forceps. The cataract was removed with PEA using the two-handed technique. After surgery, systemic corticosteroids, anti-inflammatory drugs and antibiotics were administered. After cataract removal, the bear had recovered vision, and good quality vision has been maintained to date (15 months). PEA can be a safe and effective treatment for cataracts that impair vision in bears.

An 18-month-old intact female lioness (Panthera leo) was referred to the Clinica Veterinaria Roma Sud for evaluation of bilateral cataracts. Phacoemulsification and implantation of +30 diopter intraocular lens (IOL) were performed bilaterally. Seven years after surgery, the IOL remained centrally positioned and the patient had normal activity.

OBJECTIVE To characterize and determine the incidence of acute-onset (ie, developing ≤ 6 weeks after surgery) postoperative infectious and sterile endophthalmitis in dogs following elective cataract surgery. DESIGN Retrospective case series. ANIMALS 2,630 eyes of 1,447 dogs that underwent elective unilateral or bilateral cataract surgery by phacoemulsification at Cornell University from 1995 through 2015. PROCEDURES Medical records were reviewed to collect and summarize data regarding dog signalment, clinical findings, diagnostic test results, surgery characteristics, eye or eyes affected, concurrent major systemic diseases, treatments, and clinical outcome. RESULTS Infectious endophthalmitis developed in 4 eyes of 4 dogs during the follow-up period, representing 0.15% of eyes and 0.28% of dogs that underwent surgery. Unilateral sterile endophthalmitis developed in 3 (0.11%) eyes of 3 (0.21%) dogs. All cases of infectious endophthalmitis were unilateral and in pseudophakic eyes and followed bilateral cataract surgeries. Clinical signs consistent with infectious endophthalmitis developed a median of 18 days after surgery and included marked and progressive hypopyon; Staphylococcus or Streptococcus spp were recovered from aqueous and vitreous humor samples. All eyes with infectious endophthalmitis responded poorly to medical treatment and were enucleated. In 2 eyes with infectious endophthalmitis, corneal incision nonunion with epithelial downgrowth was identified histologically and postulated as the route of bacterial entry into the globe. CONCLUSIONS AND CLINICAL RELEVANCE Bacterial endophthalmitis following elective phacoemulsification was uncommon in the dogs of this study. Introduction of bacteria into the eye may occur during surgery or in the postoperative period from corneal incisions that fail to heal normally.

CASE DESCRIPTION A 10-year-old sexually intact male client-owned Texas rat snake (Elaphe obsoleta lindheimeri) was referred for evaluation because of a 5-month history of progressive bilateral ocular opacities and abnormal behavior. CLINICAL FINDINGS On ophthalmic examination, the snake had bilateral mature cataracts and uveal cysts. No additional ophthalmic or physical abnormalities were detected. Results of CBC, serum biochemical analysis, and ocular ultrasonography were unremarkable. TREATMENT AND OUTCOME Bilateral spectaculotomy was performed, followed by bilateral phacoemulsification and uveal cyst aspiration, without complication. Histologic evaluation of the phacoemulsified lens material revealed only nonspecific findings associated with cataractogenesis. Vision was restored and the abnormal behaviors resolved after cataract surgery. Long-term follow-up examination performed 60 weeks after surgery revealed no additional ocular or physical abnormalities. CLINICAL RELEVANCE The ocular anatomic and physiologic characteristics of snakes can pose intraoperative and postoperative challenges to phacoemulsification, but the outcome achieved for this surgical case suggested that successful cataract surgery is possible in these species. This case further demonstrated that cataracts may be associated with reversible behavioral abnormalities in captive snakes.

This study was performed to evaluate changes in intraocular pressure (IOP) during standard coaxial phacoemulsification using 4 different bottle heights (BHs) and 2 different incision sizes. Coaxial phacoemulsification was performed with a venturi-based machine in 8 enucleated canine eyes through 3.0 and 3.2 mm clear corneal incisions (CCIs). A pressure transducer inserted in the peripheral cornea monitored the IOP in real-time. The surgery was subdivided into 4 stages: sculpt-segment removal, irrigation/aspiration, capsular polishing and viscoelastic removal. The mean IOP and the difference between the maximum and minimum IOPs were calculated at each stage and compared. The ultrasound time and volume of irrigation fluid used were recorded. The mean IOP increased with an elevation in the BH. The mean IOP in the irrigation/aspiration stage was significantly higher than that in the sculpt-segment removal stage at the same BH. The difference between the maximum and minimum IOP at each stage was greater in the 3.2 mm than the 3.0 mm CCIs, although the mean IOP was lower with the 3.2 mm than the 3.0 mm CCIs. The ultrasound time and irrigation fluid volume were greater with the 3.2 mm than the 3.0 mm CCIs. Therefore, fluidic parameters during each stage could be reassessed and adjusted to reduce complications arising from an elevated IOP. Phacoemulsification with 3.0 mm CCIs at a lower BH might lead to less stress on the eye from IOP fluctuations, ultrasound energy and irrigation fluid.

Objective: To evaluate the effects of peribulbar anesthesia (sub-Tenon injection of lidocaine hydrochloride) on akinesia of extraocular muscles, mydriasis, and intraoperative and postoperative analgesia in dogs undergoing phacoemulsification., Animals: 14 Beagles with ophthalmically normal eyes., Procedures: A blinded randomized controlled trial was performed. Dogs were anesthetized and assigned to 2 treatments: concurrent sub-Tenon injection of 2% lidocaine hydrochloride solution (2 mL) and IV injection of saline (0.9% NaCl) solution (0.02 mL/kg; lidocaine group [n = 7]) or concurrent sub-Tenon injection of saline solution (2 mL) and IV injection of 0.2 mg of atracurium/kg (0.02 mL/kg; control group [7]). Pupils were dilated by topical application of a combined tropicamide and phenylephrine ophthalmic solution. Ten minutes after the injections, pupil diameter was measured and phacoemulsification was performed. End-tidal isoflurane concentration was used to evaluate intraoperative pain. Subjective pain scores were recorded during the postoperative period., Results: Akinesia was induced and maintained throughout the surgery in all eyes. Mean ± SD pupil diameter was significantly greater in the lidocaine group (13.7 ± 0.7 mm) than in the control group (12.2 ± 0.8 mm). Isoflurane requirements were significantly lower in the lidocaine group than the control group. However, postoperative pain scores were not significantly different between the groups., Conclusions and Clinical Relevance: Sub-Tenon injection of lidocaine was an effective method for inducing akinesia of extraocular muscles, mydriasis, and intraoperative analgesia for phacoemulsification in dogs. Therefore, this could be another option for surgical field exposure and pain management during phacoemulsification in dogs.

It is suggested that topical application of opioids may provide localized analgesia without delay in corneal wound healing. This study was designed to evaluate the effect of topical application of 0.8% nalbuphine on post-operative ocular pain in dogs. Twelve eyes from 11 dogs undergoing phacoemulsification cataract surgery were divided into a nalbuphine group (n=6) and saline group (n=6). Postoperatively, the nalbuphine group received 0.1 ml of topical 0.8% alkalinized nalbuphine (pH 5.6) every 8 hr, and the saline group received 0.1 ml of topical saline (pH 5.9) as a placebo. All dogs received systemic postoperative pain managements with oral tramadol (4 mg/kg) and prednisolone (0.5 mg/kg) every 8 hr. All dogs received pre- and post-ophthalmic examinations. Pain was scored in the dogs using a pain scoring system modified from the University of Melbourne pain scale at 15, 30 and 60 min following the topical treatment on days 1 and 2 (24 and 48 hr after surgery). Eye blink frequency and corneal touch threshold (CTT) were recorded at the same time. There was no statistical difference in the pain score between groups. Significant decreases in CTT, blepharospasm and eye blink frequency were observed after the topical nalbuphine treatment. This indicated that topical application of 0.8% nalbuphine solution can produce a rapid reduction of corneal discomfort in dogs.

Objective: To determine whether differences exist in the calculated intraocular lens (IOL) strengths of a population of adult horses and to assess the association between calculated IOL strength and horse height, body weight, and age, and between calculated IOL strength and corneal diameter., Animals: 28 clinically normal adult horses (56 eyes)., Procedures: Axial globe lengths and anterior chamber depths were measured ultrasonographically. Corneal curvatures were determined with a modified photokeratometer and brightness-mode ultrasonographic images. Data were used in the Binkhorst equation to calculate the predicted IOL strength for each eye. The calculated IOL strengths were compared with a repeated-measures ANOVA. Corneal curvature values (photokeratometer vs brightness-mode ultrasonographic images) were compared with a paired t test. Coefficients of determination were used to measure associations., Results: Calculated IOL strengths (range, 15.4 to 30.1 diopters) differed significantly among horses. There was a significant difference in the corneal curvatures as determined via the 2 methods. Weak associations were found between calculated IOL strength and horse height and between calculated IOL strength and vertical corneal diameter., Conclusions and Clinical Relevance: Calculated IOL strength differed significantly among horses. Because only weak associations were detected between calculated IOL strength and horse height and vertical corneal diameter, these factors would not serve as reliable indicators for selection of the IOL strength for a specific horse.

Objective: To determine whether an applanation tonometer and rebound tonometer can be used to detect similar intraocular pressure (IOP) measurements in eyes of dogs undergoing phacoemulsification., Animals: 24 dogs (40 eyes) undergoing elective phacoemulsification., Procedures: IOP measurements were obtained from each eye by use of both the rebound tonometer and applanation tonometer. Central corneal thickness was measured by use of an ultrasonic pachymeter 3 hours before surgery and 2 and 24 hours after surgery. Statistical analysis was performed by use of paired t tests., Results: Mean ± SD IOP 3 hours before surgery, 2 hours after surgery, and 24 hours after surgery was 11.9 ± 4.7 mm Hg, 15.5 ± 11.7 mm Hg, and 10.9 ± 6.7 mm Hg, respectively, as measured with the rebound tonometer and 12.2 ± 5.3 mm Hg, 15.7 ± 12.5 mm Hg, and 12.4 ± 5.4 mm Hg, respectively, as measured with the applanation tonometer. Measured IOP did not differ significantly between the 2 tonometers 3 hours before surgery and 2 hours after surgery, but measured IOP differed significantly between the tonometers 24 hours after surgery., Conclusions and Clinical Relevance: Use of a rebound tonometer underestimated IOP, relative to results for use of an applanation tonometer, by 1.65 mm Hg in eyes 24 hours after phacoemulsification. Caution should be used when IOP measurements obtained with a rebound tonometer are in the high part of the reference range, and verification of these values with an applanation tonometer would be advised.

Objective: To compare the effects of 2 preoperative anti-inflammatory regimens on intraocular inflammation following phacoemulsification., Design: Randomized controlled trial, Animals: 21 dogs with immature cataracts., Procedures: All dogs had cataract surgery via phacoemulsification, and most received prosthetic intraocular lenses. Dogs were randomly divided into 2 groups. Group A dogs were treated topically with prednisolone acetate for 7 days prior to surgery, whereas prednisolone acetate treatment commenced the evening prior to surgery in group B dogs. Postoperative care was identical for both groups. Blood-aqueous barrier breakdown was quantified by use of anterior chamber fluorophotometry, with fluorescein entry into the anterior chamber measured 2 and 9 days after surgery compared with baseline scans obtained prior to surgery. Ophthalmic examinations were performed before surgery and 1 day, 9 days, 3 weeks, 7 weeks, 3 months, and 6 months after surgery. A subjective inflammation score was established at each examination. Intraocular pressures were measured 4 and 8 hours after surgery and at each follow-up examination., Results: There was no difference in the extent of blood-aqueous barrier disruption between the groups at 2 or 9 days after surgery. Subjective inflammation scores were also similar at most time points. Dogs in group A developed postoperative ocular hypertension at a higher frequency (60%) than did those in group B (18%)., Conclusions and Clinical Relevance: In dogs that underwent cataract surgery via phacoemulsification, a full week of topical prednisolone acetate treatment prior to surgery did not decrease postoperative inflammation, compared with commencement of topical prednisolone acetate treatment the evening prior to surgery, and was associated with a greater incidence of postoperative ocular hypertension.

Outcomes for 77 cataractous eyes were compared after each eye underwent no treatment, topical medical treatment only, or phacoemulsification with intraocular lens implantation. Median follow-up time for all dogs was 2.3 y. Failure occurred in all untreated eyes and the rate of failure was 65 and 255 times higher than in medically and surgically treated eyes, respectively. The failure rate was 4 times higher in dogs receiving only medical treatment compared with dogs that received surgery. Across all groups, the success rate for mature and hypermature cataracts was lower than for immature cataracts. Regardless of cataract stage, the chance of success was higher for eyes undergoing phacoemulsification than for eyes that received medical management only. Results of this study support prompt referral for phacoemulsification when cataracts are diagnosed in dogs or, if referral is not possible, topical anti-inflammatory therapy.

Objective: To determine whether dogs that received eyedrops containing phenylephrine and scopolamine would have a higher mean arterial blood pressure (MAP) when anesthetized than would dogs that did not receive the eyedrops., Animals: 37 nondiabetic and 29 diabetic dogs anesthetized for phacoemulsification and 15 nondiabetic dogs anesthetized for corneal ulcer repair (control dogs)., Procedures: Medical records were reviewed to identify study dogs. Dogs undergoing phacoemulsification received 2 types of eyedrops (10% phenylephrine hydrochloride and 0.3% scopolamine hydrobromide) 4 times during a 2-hour period prior to the procedure. Control dogs did not receive these eyedrops. Heart rate and MAP were measured before surgery in all dogs 10 and 5 minutes before, at the time of (t0), and 5 (t5) and 10 (t10) minutes after atracurium administration., Results: MAP was greater in the 2 groups that received the eyedrops than in the control group at t0 and t5; at t10, it was greater only for the nondiabetic dogs that received eyedrops. Nine nondiabetic dogs and 1 diabetic dog anesthetized for phacoemulsification had at least 1 MAP value>131 mm Hg; 73% of MAP values>131 mm Hg were detected within 10 minutes after atracurium administration. At no time did a control dog have an MAP value>131 mm Hg., Conclusions and Clinical Relevance: Anesthetized dogs pretreated with eyedrops containing phenylephrine and scopolamine had higher MAP values than dogs that did not receive the eyedrops, suggesting the drops caused hypertension. Atracurium may interact with the eyedrops and contribute to the hypertension.

Objective: To evaluate the intraoperative and postoperative analgesic effects of intracameral lidocaine hydrochloride injection in dogs undergoing phacoemulsification., Animals: 12 healthy Beagles with healthy eyes., Procedures: Dogs were randomly assigned to receive 1 of 2 intracameral injections: 2% lidocaine hydrochloride solution (0.3 mL) or an equivalent amount of balanced salt solution (BSS). All dogs were treated with acepromazine (0.05 mg/kg, i.v.) and cefazolin (30 mg/kg, i.v.), and tropicamide drops were topically applied to the eyes. Anesthesia was induced with propofol and maintained with isoflurane. The initial end-tidal isoflurane concentration was maintained at 1.2%. Heart rate, respiratory rate, arterial blood pressure, esophageal temperature, inspired and end-tidal isoflurane concentrations, and oxygen saturation were recorded every 5 minutes. The allocated agent was injected intracamerally after aspiration of the same volume of aqueous humor. Ten minutes after injection, phacoemulsification was performed. After surgery began, the isoflurane concentration was adjusted according to heart rate and mean arterial blood pressure. Pain scores were recorded before surgery and at 0.5, 1, 1.5, 2, 2.5, 3, 3.5, 4, 6, 8, 16, and 24 hours after extubation., Results: Isoflurane requirements were significantly higher in the BSS group than in the lidocaine group. Mean +/- SD time to administration of supplementary analgesia was significantly shorter in the BSS group (1.4 +/- 1.2 hours) than in the lidocaine group (4.9 +/- 1.2 hours)., Conclusions and Clinical Relevance: Intracameral lidocaine injection had significant analgesic effects in dogs undergoing cataract surgery. Results of this study suggest the value of intracameral lidocaine injection as an analgesic for intraocular surgery in dogs.

A 1.5-year-old, spayed, female dachshund was presented with a cataract and lens-induced uveitis in the left eye. The cataract progressed from immature to hypermature in 4 months. Phacoemulsification and intraocular lens implantation was performed and the dog remains visual in the left eye 1 year post-surgery.

Objective: To compare the iridocorneal angle (ICA) and angle opening distance (AOD) in dogs with cataractous and noncataractous lenses; evaluate cataractous eyes ultrasono-graphically for association of postoperative ocular hypertension (POH) with the ICA, AOD, and postoperative echogenic anterior chamber debris; and evaluate intraobserver reliability associated with ICA and AOD measurements., Animals: 56 dogs with 102 cataracts, and 23 clinically normal dogs., Procedures: Ultrasound biomicroscopy was performed on 102 eyes of 56 dogs before and after cataract surgery and on 46 nondilated and dilated eyes of 23 clinically normal dogs. Cataract stage, ICA, AOD, and association with POH were assessed., Results: Cataract stage and ICA or AOD were not significantly associated; however, ICA and AOD typically decreased with increasing cataract maturity. Before and after pupillary dilation, AODs were significantly smaller in cataractous eyes than in noncataractous eyes. Before surgery, ICA and AOD in eyes without pupillary dilation were significantly associated with POH. At > 13 degrees , odds of developing POH increased by 11% for each degree increase in the ICA. Postoperative anterior chamber debris was not associated with POH. Coefficient of variation for repeated measurements was 10% for the ICA and 9.5% for the AOD, suggesting good intraobserver reliability., Conclusions and Clinical Relevance: In this study, dogs with larger ICA and AOD measurements before surgery were at greater risk of developing POH. This information may be useful for future studies to determine whether preventative treatment for POH administered prior to surgery may be beneficial.

Case Description: A great horned owl of estimated age < 1 year that was captured by wildlife rehabilitators was evaluated because of suspected cataracts., Clinical Findings: Nuclear and incomplete cortical cataracts were evident in both eyes. Ocular ultrasonography revealed no evidence of retinal detachment, and electroretinography revealed normal retinal function., Treatment and Outcome: For visual rehabilitation, cataract surgery was planned and intraocular lens design was determined on the basis of values obtained from the schematic eye, which is a mathematical model representing a normal eye for a species. Cataract surgery and intraocular lens placement were performed in both eyes. After surgery, refraction was within -0.75 diopters in the right eye and -0.25 diopters in the left eye. Visual rehabilitation was evident on the basis of improved tracking and feeding behavior, and the owl was eventually released into the wild., Clinical Relevance: In raptors with substantial visual compromise, euthanasia or placement in a teaching facility is a typical outcome because release of such a bird is unacceptable. Successful intraocular lens implantation for visual rehabilitation and successful release into the wild are achievable.

Objective: To determine the immediately antecedent cause of secondary glaucoma and the prevalence of secondary glaucoma with anterior uveitis or lens dislocation in dogs., Design: Retrospective case series., Animals: 156 dogs with secondary glaucoma., Procedures: Cause of glaucoma was determined from records. Breed, age, sex, and neuter status of all dogs with secondary glaucoma were compared with the general hospital population. The prevalence of secondary glaucoma in dogs with a primary diagnosis of lens dislocation or anterior uveitis during the same period was determined., Results: Secondary glaucoma was diagnosed in 156 of 2,257 (6.9%) dogs examined because of ophthalmic disease and was bilateral in 33 (21.2%) of those dogs. In 31 (94%) bilaterally affected dogs, the antecedent cause was the same in both eyes. Common causes of secondary glaucoma were non-surgical anterior uveitis (44.9%), anterior uveitis associated with prior phacoemulsification (15.8%), and lens dislocation (15.2%). Parson Russell Terriers, Poodles, Boston Terriers, Cocker Spaniels, Rhodesian Ridgebacks, and Australian Cattle Dogs had diagnoses of secondary glaucoma more often than expected, compared with the reference population. Age, sex, neuter status, and laterality were not associated with secondary glaucoma. The prevalence of secondary glaucoma in dogs with lens dislocation or uveitis was 15% or 17%, respectively., Conclusions and Clinical Relevance: Glaucoma develops secondary to many intraocular diseases, particularly uveitis and lens dislocation. Diagnosis of these diseases should prompt frequent monitoring of intraocular pressure, regardless of signalment.

This study evaluated the surgical outcome and complications of phacoemulsification and the implantation of an acryl foldable intraocular lens (IOL) with a squared edge in dogs with cataracts. Thirty-two eyes from 26 dogs were examined. The mean follow up period was 75.9 days ranging from 23 to 226 days. The complications after phacoemulsification were posterior capsular opacity (PCO) around the IOL (n = 11), ocular hypertension (n = 4), focal posterior synechia (n = 4), hyphema (n = 3) and corneal ulcer (n = 2). The complications associated with the IOL were decenteration of the optic (n = 2) and ventral haptic displacement (n = 1). Most cases of PCO were found only around the margin of the IOL, and all eyes had vision during the observation period. In conclusion, the implantation of an acryl-foldable lens with a squared edge at the time of phacoemulsification is an effective method for preserving the central visual field of dogs with cataract.

Objective: To determine common postoperative complications and risk factors for development of postoperative glaucoma or failure to preserve vision after phacoemulsification for cataract removal in dogs. Design-Retrospective case series., Animals: 172 dogs (290 eyes) that underwent phacoemulsification surgery for cataract removal., Procedure: Medical records were reviewed for postoperative complications; prevalence rates for each complication were calculated for follow-up periods of 3 months, > 3 to 6 months, > 6 months to 1 year, > 1 to 2 years, > 2 to 3 years, > 3 to 4 years, and > 4 years. Odds ratios for breed, age, sex, cataract hypermaturity, lens-induced uveitis, and diabetes mellitus were determined with respect to glaucoma and failure (ie, blindness, enucleation, or evisceration)., Results: The most common complication was mild posterior capsule opacification. Retinal detachment was uncommon (1% to 2%) for all time periods. Prevalence of glaucoma increased with time, although it remained < 10% until after the 1-year follow-up period. Boston Terriers, Cocker Spaniels, Cocker Spaniel-Poodle crosses, and Shih Tzus had increased risk of developing glaucoma. Eyes with hypermature cataracts were more likely to develop glaucoma. Prevalence of failure increased with time, although it remained < 10% until after the 3-year follow-up period. Cocker Spaniel-Poodle cross and Shih Tzus were more likely to have failure., Conclusions and Clinical Relevance: Evaluation of breed and cataract hypermaturity may aid in the selection of patients. The increasing prevalence of postoperative complications with time indicated that longterm monitoring is warranted.

Objective: To determine prevalence of retinal hemorrhages and microaneurysms in dogs with diabetes mellitus following cataract extraction by means of phacoemulsification and identify potential risk factors., Design: Retrospective study., Procedure: Medical records of dogs undergoing phacoemulsification between 1993 and 2003 were reviewed, and information was recorded on signalment, history, physical examination findings, ophthalmic examination findings, results of laboratory testing, electroretinographic findings, and surgical findings. Glycemic control was classified as poor, intermediate, or good on the basis of baseline blood glucose concentration, perioperative body weight loss, daily insulin dosage, and presence of glucosuria and ketonuria. Data from diabetic and nondiabetic dogs were analyzed to determine prevalence and risk factors for development of retinal hemorrhages or microaneurysms following phacoemulsification., Results: 11 of the 52 (21%) dogs with diabetes mellitus developed ophthalmoscopic signs of retinal hemorrhages or microaneurysms, compared with 1 of the 174 (0.6%) nondiabetic dogs. Median time from onset of diabetes mellitus to diagnosis of retinopathy was 1.4 years (range, 0.5 to 3.2 years). No risk factors for development of retinopathy were identified., Conclusions and Clinical Relevance: Results suggest that retinal hemorrhages and microaneurysms may be more common and develop earlier in diabetic dogs than previously reported. This may affect treatment, as diabetic dogs survive longer with improved glycemic control.

Objective: To determine factors contributing to glaucoma after lens extraction via phacoemulsification in dogs., Design: Retrospective study., Animals: 22 dogs (29 eyes) with glaucoma and 21 dogs (30 eyes) without glaucoma after phacoemulsification., Procedure: Medical record review., Results: Eyes at increased risk for glaucoma included those of Boston Terriers, those with uveal or retinal abnormalities before surgery, and those with intraoperative intraocular hemorrhage. Significant differences between groups were not detected for incidence of preoperative lens-induced uveitis, presence of an intraocular lens, or frequency of an acute postoperative increase in intraocular pressure. Glaucoma developed (mean +/- SD) 12.8+/-14.1 months (median, 10 months; range, 0.25 to 55 months) after surgery. Eighteen of 29 (62%) eyes with potential for vision after onset of glaucoma retained vision for a mean of 16.5+/-12.8 months (median, 10.8 months; range, 1.5 to 37 months) after glaucoma was diagnosed. Most of these eyes still had vision at the conclusion of the study period., Conclusions and Clinical Relevance: Risk factors identified by this study will aid in preoperative counseling of clients and refining selection criteria for candidates for phacoemulsification. Careful follow-up for the remainder of the dog's life after surgery may improve long-term success rates by permitting early intervention before intraocular pressure increases substantially and vision is irreversibly lost. Surgery for cataracts may still be worthwhile in dogs with increased risk of glaucoma, especially if elderly, because of the lengthy period to onset of glaucoma after surgery and the beneficial effects of treatment after glaucoma develops.

A seven-month-old, male Shih Tzu dog weighing 3.7 kg had an immature cataract in its left eye. A biomicroscopic examination revealed numerous vacuolations in the posterior cortices with nucleus cataracts, covered by an intact anterior lens capsule. The changes observed by ocular sonographic examination (OSG) of the left eye were hyperechoic, and a funnel-cone shape was observed posteriorly with cortex hyperechogenicity in the lens. The left eye was diagnosed as having a posterior lenticonus with congenital cataract. Phacoemulsification was performed on the left eye as diagnostic treatment of the posterior lenticonus and cataract. Postoperative OSG on the left eye revealed a V-shaped linear echo that was indicative of a posterior capsule of the lens. Moreover, it was confirmed that hyperechoic cataract material inside the lens had disappeared.

A 1-piece, tripod-shaped, acrylic intraocular lens was implanted in the posterior chamber after phacoemulsification to remove cataracts in 20 eyes of 16 dogs. Biomicroscopy and intraocular pressure measurements were performed at 1, 8, 30, and 90 days after surgery; retinoscopy was performed at day 30. Complications were not encountered during implantation of the lens. All dogs had mild ocular inflammation characterized predominantly by aqueous flare and mild corneal edema at day 1, which resolved completely by day 8; 3 eyes had mild hyphema at day 1, which resolved by day 8. Three eyes developed fibrin in the anterior chamber, which resolved by day 30. Two eyes developed distorted pupils caused by adherence of vitreous humor or fibrin to the incision site. Postoperative intraocular pressure ranged from 7 to 21 mm Hg. Implantation resulted in 9 emmotropic eyes, 5 myopic eyes, and 6 hyperopic eyes. Maintenance of central lens position was excellent in 19 eyes; in 1 eyes, a disclocated superior haptic at day 2 was easily repositioned.

Objective: To determine whether intracameral injection of carbachol at the completion of phacoemulsification in dogs would prevent the increase in intraocular pressure (i.o.p) that can develop during the first 24 hours after surgery., Design: Randomized controlled trial., Animals: 32 adult dogs undergoing elective unilateral or bilateral phacoemulsification., Procedure: Dogs were randomly assigned to 1 of 4 groups with 8 dogs/group: phacoemulsification and intracameral administration of 0.5 ml of 0.01% carbachol at the end of surgery; phacoemulsification, intraocular lens implantation, and intracameral administration of 0.5 ml of 0.01% carbachol; phacoemulsification and intracameral administration of 0.5 ml of balanced salt solution; and phacoemulsification, intraocular lens implantation, and intracameral administration of 0.5 ml of balanced salt solution. Intraocular pressure was measured at 3 and 6 hours and the morning after surgery. Aqueous flare was also measured 6 hours and the morning after surgery., Results: None of the dogs treated with carbachol developed postoperative ocular hypertension (i.e., IOP > 27 mm of Hg), whereas 12 of 16 control dogs had ocular hypertension 3 hours after surgery. Intraocular pressure 3 hours after surgery was not significantly associated with phacoemulsification time or phacoemulsification power or with whether the dog received an intraocular lens implant. Severity of aqueous flare was similar for treated and control dogs., Clinical Implications: Results suggested that intracameral administration of 0.01% carbachol at the end of surgery was a safe and efficacious method of preventing the postoperative increase in IOP associated with phacoemulsification in dogs.

Objective: To document the incidence of postoperative ocular hypertension (POH) after cataract surgery in dogs., Design: Retrospective analysis of medical records., Sample Population: 88 dogs that had had cataract surgery., Procedure: The effect of several categorical variables on the development of POH was evaluated statistically. Postoperative ocular hypertension was defined as intraocular pressure > 25 and > 30 mm of Hg., Results: The incidence of POH > 25 mm of Hg was 48.9%; > 30 mm of Hg, 33.8%; > 40 mm of Hg, 20.1%; and > 50 mm of Hg, 5.8%. Mean onset of POH > 25 mm of Hg was 4.9 hours. The incidence of POH was not affected by the type of surgery. Eyes that had phacoemulsification developed POH significantly more rapidly (mean, 3.9 hours), compared with those that had extracapsular lens extraction (8.4 hours). Mean phacoemulsification duration was greater in eyes that developed POH, and older dogs were more likely to develop POH. Development of POH was not correlated with sex, stage of cataract, type of surgical procedure performed, intraocular lens placement, preoperative lens-induced uveitis, or posterior lens capsule tears and vitrectomy. However, eyes that received intraocular lens implants developed POH more rapidly, compared with eyes without implants., Clinical Implications: The high incidence and early onset of POH after cataract surgery suggests that routine use of antiglaucoma medications in the first 12 hours after surgery is warranted.
