O método de análise utilizado foi fundamentalmente misto, pautado em análise exploratória, testes estatísticos, bem como na classificação das estratégias a partir de um sistema categórico de habilidades e da Taxonomia SOLO. Tivemos a participação de cerca de 341 estudantes de 12 turmas do Ensino Médio Integrado de uma escola pública federal, em dois campi. A tese é em formato multipaper e contêm quatro artigos. O primeiro artigo teve como objetivo investigar quais foram as estratégias utilizadas pelos estudantes ao realizarem a prática experimental, quando apresentada de natureza tradicional e investigativa. Os resultados mostraram que habilidades de duas naturezas foram mobilizadas pelos estudantes no momento de resolução da/o tarefa/desafio: procedimental e interpretativa. Para a habilidade procedimental identificamos cinco tipos específicos de categorias, inferidas a partir das respostas dos estudantes: habilidade em executar medidas (EXM); habilidade em fazer cálculo de média (FCM); habilidade em resolver equações matemáticas (EQM); habilidade em aplicar regras matemáticas (ARM) e a categoria própria da atividade investigativa: habilidade em propor a solução do problema (PSP). Já para a interpretativa, identificamos a categoria habilidade de verificar (examinar) a validade da resposta encontrada (VRE). Os resultados apontam que a habilidade procedimental, para a amostra geral de respondentes, é significativamente diferente da habilidade interpretativa, sendo maior essa última. Quando realizamos a análise de acordo com a natureza da atividade respondida, identificamos que a atividade tradicional demandou habilidades do tipo procedimental e interpretativa de maneira adequada. Já a atividade investigativa demandou mais a mobilização das habilidades procedimentais EQM e EXM, e a habilidade interpretativa VRE. Testes estatísticos mostraram que existem diferenças significativas entre as habilidades mobilizadas pela atividade tradicional e a atividade investigativa. O segundo artigo teve como objetivo investigar quais as estratégias utilizadas pelos estudantes ao realizarem a prática experimental, quando apresentada de natureza tradicional e investigativa em dois ambientes distintos: material/virtual. Dessa forma fizemos a comparação geral em relação ao ambiente em que a atividade foi realizada. Testes estatísticos mostraram que o ambiente material mobilizou mais habilidades associadas ao momento em que o estudante coloca seu conhecimento em ação para realizar a parte procedimental, e o ambiente virtual mobilizou mais a habilidade de natureza interpretativa. Quando analisamos se o fator ambiente (material/virtual) atuou de maneira diferente no momento em que foram aplicadas as atividades tradicional e investigativa, verificamos que: tanto o ambiente material como o virtual promoveu a mobilização de habilidades específicas de maneira diferenciada quando foi aplicada a atividade investigativa; quando foi aplicada a atividade tradicional apenas o ambiente material promoveu a mobilização de habilidades específicas. O terceiro artigo investigou o grau de profundidades em que as habilidades do tipo procedimental e interpretativa se apresentaram inferidas nas respostas à/ao tarefa/desafio. Construímos uma ferramenta metodológica para investigarmos aspectos do emprego de diferentes habilidades nos seus distintos níveis de complexidade em atividades de natureza experimental, mas com características distintas, tanto em termos de abordagem como em termos de ambiente de aplicação. Verificamos que, ao aplicarmos as atividades tradicionais e investigativas nos ambientes de ensino material e virtual, houve a explicitação de habilidades de planejamento, execução e avaliação para ambos os ambientes em diferentes graus de profundidade a depender de sua natureza. Quando levamos em consideração o aspecto ambiente e natureza da atividade utilizada, verificamos que a habilidade procedimental, associada ao uso da ferramenta matemática, foi a que apresentou maior nível de complexidade. O quarto artigo teve como objetivo investigar a existência de possíveis preditores de aprendizagem que podem exercer influência no momento em que os estudantes utilizam as estratégias de aprendizagem para resolver a atividade tradicional/investigativa no ambiente de ensino material/virtual. Através de uma análise exploratória, verificamos que três preditores atuaram: gênero, curso e instituição. Ao consideramos a natureza da atividade e o tipo de ambiente de ensino em que a prática experimental foi realizada, verificamos que a influência dos preditores se deu em habilidades específicas a depender da natureza da atividade resolvida e do tipo de intervenção aplicada. This thesis presents an investigation about the strategies used by students to solve an experimental practice on formal content (Simple Pendulum), when presented in a traditional (tasks) and investigative (challenges) nature in two distinct environments: material and virtual. The analysis method used was fundamentally mixed, based on exploratory analysis, statistical tests, as well as the classification of strategies from a categorical system of skills and SOLO Taxonomy. We had the participation of about 341 students from 12 integrated high school classes of a federal public school, on two campi. The thesis is in multipaper format and contains four articles. The first article aims to investigate which strategies were used by students when performing the experimental practice, when presented in a traditional and investigative nature. The results showed that skills of two natures were mobilized by the students at the time of task/challenge resolution: procedural and interpretative. For the procedural ability we identified five specific types of categories, inferred from the students' answers: ability to execute measures (EXM); ability to do average calculation (FCM); ability to solve mathematical equations (EQM); ability to apply mathematical rules (ARM) and the category of investigative activity: ability to propose a solution to the problem (PSP). For the interpretive, we identified the category ability to verify (examine) the validity of the answer found (VRE). The results indicate that the procedural ability, for the general sample of respondents, is significantly different from the interpretive ability, however, the last one is more considered. When we performed the analysis according to the nature of the activity answered, we identified that the traditional activity required procedural and interpretive skills in a balanced way. On the other hand, the investigative activity required more the mobilization of the procedural skills EQM and EXM, and the interpretive ability VRE. Statistical tests showed that there are significant differences between the skills mobilized by traditional activity and investigative activity. The second article aimed to investigate which strategies the students used when performing the experimental practice, when presented in a traditional and investigative nature in two different environments: material/virtual. Thus, we made the general comparison in relation to the environment in which the activity was carried out. Statistical tests showed that the material environment mobilized more skills associated with the moment when the student puts his knowledge into action to perform the procedural part, and the virtual environment mobilized more the skill of interpretive nature. When we analyzed whether the environmental factor (material/virtual) acted differently at the time when traditional and investigative activities were applied, we found that: both the material and virtual environments promoted the mobilization of specific skills differently when the investigative activity was applied; when the traditional activity was applied only the material environment promoted the mobilization of specific skills. The third article investigated the degree of depths in which procedural and interpretative skills were inferred in the responses to the task/challenge. We have built a methodological tool to investigate aspects of the use of different skills, in its different levels of complexity in experimental activities, but with distinct characteristics, both in terms of approach and in terms of application environment. We verified that, when applying traditional and investigative activities in material and virtual, teaching environments there was the explicitness of planning, execution and evaluation skills for both environments in different degrees of depth depending on their nature. When we took into account the environmental aspect and nature of the activity used, we verified that the procedural ability associated with the use of the mathematical tool was the one with the highest level of complexity. The fourth article aimed to investigate the existence of possible predictors of learning that can influence the moment when students use learning strategies to solve traditional/investigative activity in the material/virtual teaching environment. Through an exploratory analysis we verified that three predictors acted: gender, course and institution. When we considered the nature of the activity and the type of teaching environment in which the experimental practice was performed, we verified that the influence of the predictors occurred on specific skills depending on the nature of the activity solved and the type of intervention applied.

4. The spectral profile of cortical activation during a visuospatial mental rotation task and its correlation with working memory.

Introduction: The search for a cortical signature of intelligent behavior has been a longtime motivation in Neuroscience. One noticeable characteristic of intelligence is its association with visuospatial skills. This has led to a steady focus on the functional and structural characteristics of the frontoparietal network (FPN) of areas involved with higher cognition and spatial behavior in humans, including the question of whether intelligence is correlated with larger or smaller activity in this important cortical circuit. This question has broad significance, including speculations about the evolution of human cognition. One way to indirectly measure cortical activity with millisecond precision is to evaluate the event-related spectral perturbation (ERSP) of alpha power (alpha ERSP) during cognitive tasks. Mental rotation, or the ability to transform a mental representation of an object to accurately predict how the object would look from a different angle, is an important feature of everyday activities and has been shown in previous work by our group to be positively correlated with intelligence. In the present work, we evaluate whether alpha ERSP recorded over the parietal, frontal, temporal, and occipital regions of adolescents performing easy and difficult trials of the Shepard-Metzler's mental rotation task, correlates or are predicted by intelligence measures of the Weschler's intelligence scale. Methods: We used a database obtained from a previous study of intellectually gifted (N = 15) and average intelligence (N = 15) adolescents. Results: Our findings suggest that in challenging task conditions, there is a notable difference in the prominence of alpha event-related spectral perturbation (ERSP) activity between various cortical regions. Specifically, we found that alpha ERSP in the parietal region was less prominent relative to those in the frontal, temporal and occipital regions. Working memory scores predict alpha ERSP values in the frontal and parietal regions. In the frontal cortex, alpha ERSP of difficult trials was negatively correlated with working memory scores. Discussion: Thus, our results suggest that even though the FPN is task-relevant during mental rotation tasks, only the frontal alpha ERSP is correlated with working memory score in mental rotation tasks. 5. Equidad y distribución geográfica de recursos financieros en los sistemas de salud

Este trabajo aborda la temática de la equidad en la salud, pero específicamente, en la distribución geográfica de los recursos financieros. Hace un repaso de las principales teorías de justicia social en el pensamiento contemporáneo y discute el concepto de equidad en general y en el campo de la salud. Tomando como base la discusión de algunas experiencias internacionales (Inglaterra, España e Italia), se identifica como metodología distributiva más adecuada la fórmula RAWP llevada a cabo en Inglaterra, que proporciona dimensiones a las necesidades relativas a partir de los perfiles tanto demográficos como epidemiológicos de la población. Finalmente se presentan los resultados obtenidos en simulación efectuados para el caso brasileño. Los resultados presentados para el caso brasileño permitieron verificar que, para una distribución geográfica de recursos financieros más equitativa, sería necesaria una redistribución en favor de los estados de las regiones Norte y Nordeste. Finalmente se destaca que, la comparación entre la utilización de recursos efectuada en 1994 por el Ministerio de Salud y los resultados obtenidos en la simulación de la metodología RAWP para el caso brasileño muestran que los principios enunciados en la legislación no estuvieron presentes en la distribución geográfica de recursos financieros.

The article analyzes the World Health Organization Report for 2000, with emphasis placed on the methodology used to analyze the indicators utilized to compare and classify the performance of the health systems of the 191 member countries. The Report's contribution was the compromise of monitoring the performance of the health systems of member countries, but because of the inconsistent way it was elaborated, and the utilization of questionable scientific evaluation methodologies, the Report fails to give a clear picture. A criterion-based methodology revision is imposed. The main problems in evidence are the choice of individual indicators of disparity in health that discount the population profile, the inadequate control of the impact of social disparities over the performance of the systems, the evaluation of the responsibility of systems that are only partially articulated to the right of the citizens, the lack of data for a great number of countries, consequently having inconsistent estimations, and the lack of transparency in the methodological procedures in the calculation of some indicators. The article suggests a wide methodological revision of the Report.

A inteligência pode ser definida como um conjunto de habilidades cognitivas decorrentes de diferentes processos intelectivos, estruturados em trajetórias exclusivas de desenvolvimento, resultantes da interação de fatores biológicos e culturais. O quoeficiente de inteligência (QI), compreendido como a expressão atual do nível de habilidade de um indivíduo em relação a seu dado normativo, delimita grupos da curva de desenvolvimento de inteligência, caracterizando os grupos de deficiência intelectual e altas habilidades/superdotação. Porém, defende-se aqui que tais grupos não representam configurações antagônicas em sua totalidade, ou seja, a deficiência intelectual, assim como as altas habilidades/superdotação são configuradas por pontos de força e fragilidade. Por outro lado, as relações entre inteligência, funções executivas e criatividade têm gerado frutíferos debates. Nesse sentido, o objetivo do presente estudo foi investigar as relações entre estas dimensões nos dois extremos da curva de inteligência, problematizando os perfis neuropsicológicos para além de uma abordagem quantitativista. Participaram desse estudo 16 indivíduos entre 12 e 20 anos, com classificações de QI nos dois extremos da curva de inteligência. Foram avaliadas as dimensões da inteligência, criatividade e o funcionamento executivo. Os dados foram analisados através de estatística descritiva e de análise de cluster. Os principais resultados sugerem que a dimensão da compreensão verbal da inteligência é o domínio que melhor explica a diferença entre os grupos, sendo o domínio da velocidade de processamento aquele no qual a diferença entre os grupos é menor. Em termos de funcionamento executivo, a memória operacional e a flexibilidade cognitiva despontaram como as habilidades mais díspares entre os dois grupos. Por fim, no domínio da criatividade, o fator de enriquecimento de ideias foi o que melhor explicou a diferença entre sujeitos com DI e sujeitos com AH/SD. Espera-se que os resultados encontrados possam subsidiar intervenções junto aos dois grupos, ambos considerados com necessidades educacionais especiais, a partir dos perfis que os caracterizam, considerando pontos de fragilidade, mas em especial os pontos de força, avançando para um entendimento da educação especial para além dos limites impostos pelo quociente de inteligência, pois este não é aqui considerado como destino. Intelligence can be defined as a set of cognitive abilities emerging from different intellectual processes, builded in exclusive paths of development, as a result of the interaction biological and cultural factors. The intelligence quotient (IQ), known as the expression of an individual's level of ability in relation to their normative data, lines up groups at intelligence curve of development, describind groups of intellectual disability and giftedness. However, it is argued in this study, that such groups do not opose each other in completude, wich means that intelectual disabilities and giftedness are configured by points of strength and fragility. On the other hand, the relationship between intelligence, executive functions and creativity have provoked fruitful debates. In this sense, the current study aims to investigate how these dimensions can relate with the extremity groups of intelligence curve, investigate the relationships between these dimensions in the two extremes of the intelligence curve, triyng to understand these neuropsychological profiles beyond a quantitative approach. Sixteen individuals between 12 and 20 years old, with IQ classifications at the two extremes of the intelligence curve, participated in this study. The dimensions of intelligence, creativity and executive functioning were evaluated. Data were analyzed through descriptive statistics and cluster analysis. The main results suggests that verbal comprehension dimension of intelligence is the domain wich better explains the difference between groups, and processing speed is the one with smaller diferences between them. In terms of executive functioning, operational memory and cognitive flexibility emerged as the most distinct abilities between groups. At last, enrichment factor of ideas explained the main diferences between individuals of both groups, related to criativity production. It is hoped that the results found may subsidize interventions in both groups, either considered with special educational needs, from the profiles that characterize them, considering fragility points, but especially the points of strength, moving towards an understanding of special education beyond the limits imposed by the quotient of intelligence, since it is not here considered as destiny.

The article discusses a new methodology, where in polystyrene-bound chiral derivatizing agents (CDA-resins) are employed for assigning the configuration of chiral mono- and polyfunctional compounds by using NMR spectroscopy. The technique is shown to be highly versatile and effective.

Este artigo apresenta uma revisão da Matriz de Dimensões da Avaliação do Sistema de Saúde no Brasil desenvolvida em 2003, e uma atualização conceitual de parte das subdimensões de avaliação do desempenho dos serviços de saúde: efetividade, acesso, eficiência e adequação. Descreve o processo de seleção dos indicadores utilizados e uma síntese dos resultados para cada subdimensão do desempenho. O comportamento dos indicadores utilizados para avaliar o desempenho dos serviços de saúde no Brasil, no que se refere às quatro subdimensões selecionadas, não é uniforme e as melhorias mais acentuadas são observadas naquelas influenciadas pela atuação dos serviços no campo da atenção primária, as melhorias mais significativas foram observadas nas Efetividade e Acesso. Em relação à Eficiência dos serviços de saúde coexistem situações de alta eficiência com outras de baixo desempenho. A atuação dos serviços de saúde na subdimensão Adequação foi pior do que nas demais apresentadas. This paper presents a review of the Dimension Matrix for Evaluation of the Brazilian Health System that was initially developed in 2003, as well as a conceptual update of some of the sub-dimensions for the evaluation of health service performance, namely effectiveness, access, efficiency and appropriateness of health care. It also describes the indicator selection process as well as the results obtained in each performance dimension. The behavior of the indicators used to assess the performance of health services in Brazil, with respect to each sub-dimension, was not uniform. Areas of marked improvement were found in indicators that are influenced by activities in the field of primary care. The most significant improvements were seen in the sub-dimensions of Effectiveness and Access. With respect to the Efficiency of health services, situations of high efficiency coexist with others with substandard performance. The performance of health services in the sub-dimension of Appropriateness of Health Care was the lowest of all indicators.

Este artigo analisa, a partir de microdados de 1998, 2003 e 2008 da PNAD/IBGE, a utilização de serviços de saúde sob a perspectiva de seu financiamento Entre os principais resultados da análise, destacam-se os seguintes: 1) o SUS continua financiando a maioria dos atendimentos e das internações realizados no País, participação que aumentou significativamente entre 1998 e 2003, mantendo-se praticamente estável entre 2003 e 2008; 2) a participação do SUS no financiamento da utilização dos serviços de saúde foi preponderante em todas as regiões, sendo maior nas regiões Norte e Nordeste com situações sanitárias e sócio-econômicas mais precárias; 3) o SUS é o principal financiador dos dois níveis extremos de complexidade da atenção à saúde: o de atenção básica e o da alta complexidade; 4) apesar do aumento verificado nas taxas de utilização do SUS, para atendimentos e internações, ainda persistem grandes inequidades no uso de serviços de saúde entre a população atendida pelo SUS e a população beneficiaria de planos e seguros de saúde privados; 5) foi observado um aumento na utilização de serviços do SUS por parte dessa população com asseguramento privado. This article analyzes the use of health services from the perspective of financing based on PNAD/IBGE micro-data related to 1998, 2003 and 2008. Among the main results, the following can be highlighted: 1) The Unified Health System (SUS) continues to be the major financing agent of most consultations and hospitalizations in Brazil; its participation increased significantly between 1998 and 2003 and remained almost stable between 2003 and 2008; 2) SUS participation in financing the use of the health services has been predominant in all Brazilian regions, especially in the North and North-East, which feature the most precarious socio-economic and health conditions; 3) SUS is the major financing agent of the two extreme levels of complexity of health care: primary care and high complexity services. 4) In spite of a significant rise in utilization rates of SUS services for consultations and hospitalizations, great inequities can still be observed between the population that exclusively uses SUS and that which has private health insurance; 5) There has been an increase in the use of SUS health services by part of the population with private health insurance plans.

Indicators of sanitary needs are useful tools in any process of elaboration and evaluation of public policies aimed at health promotion. This article aimed to identify the inequalities among the municipalities of the state of Rio de Janeiro, using the needs indexes calculated from the construction of a compound indicator, which synthesizes the set of epidemiologic and socioeconomic variables associated with the most common sanitary conditions of the population. Based on the modeling adopted, it was possible to detect significant sanitary inequalities among the municipalities under study, even those belonging to the same region. The modeling results also show that the epidemiologic and socioeconomic variables considered in the study are not independent. Such evidence suggests that a municipal policy aimed at simultaneous actions towards the areas of basic health, sanitation and teaching would reduce the local sanitary needs in a more expressive and immediate form, compared to an action exclusively directed towards primary health care.Key-Words: Primary Health Care; Health Status Indicators; Health Inequalities; Public Health. Indicadores de necessidades sanitárias são instrumentos úteis em qualquer processo de elaboração e avaliação de políticas públicas voltadas à promoção da saúde. O presente artigo objetivou identificar as desigualdades nas necessidades sanitárias entre os municípios do estado do Rio de janeiro, utilizando-se de índices de necessidades, calculados a partir da construção de um indicador composto, que sintetiza o conjunto de variáveis epidemiológicas e socioeconômicas ligadas às condições sanitárias mais comuns da população. A partir dos índices obtidos da modelagem adotada, foi possível detectar significativas desigualdades sanitárias entre os municípios fluminenses, mesmo entre aqueles pertencentes à mesma região. Os resultados da modelagem também mostram que as variáveis epidemiológicas e socioeconômicas consideradas no estudo não são independentes. Tal evidência sugere que uma política municipal contemplando ações simultâneas nas áreas da atenção básica à saúde, do saneamento e do ensino fundamental reduziria as necessidades sanitárias locais de forma muito mais expressiva e imediata, quando em comparação com uma ação exclusivamente na área da atenção básica.

El artículo discute los resultados de la operacionalización del Planeamiento Estratégico Situacional (PES) adaptado al nivel local en salud, considerando el enfoque comunicativo y la dimensión de la equidad en una parroquia en Venezuela. Se utilizaron dos criterios innovadores: la estimativa de las necesidades de salud y el análisis del potencial participativo de los actores. Los problemas identificados fueron relacionados con los artículos referentes a derechos de la Constitución venezolana. Se midieron inequidades con indicadores de salud, asociados al problema seleccionado; se incorporaron criterios de equidad en las propuestas de acción y elementos comunicativos. Se dio prioridad al problema "baja capacidad resolutiva de la red de servicios de salud" y se seleccionaron cinco nudos críticos para el plan de acción, que quedó constituido por 6 operaciones y 21 acciones. Se concluyó que la articulación de la epidemiología y planeamiento amplía la explicación situacional. La incorporación del enfoque comunicativo y de la dimensión de equidad al PES permite potencializar la gestión en salud y contribuir a la disminución de las brechas por inequidad. The article discusses the results of operationalizing Situational Strategic Planning adapted to the local level in health, considering the communicative approach and equity in a parish in Venezuela. Two innovative criteria were used: estimated health needs and analysis of the actors' potential for participation. The problems identified were compared to the corresponding article on rights in the Venezuelan Constitution. The study measured inequalities using health indicators associated with the selected problems; equity criteria were incorporated into the action proposals and communicative elements. Priority was assigned to the problem of "low case-resolving capacity in the health services network", and five critical points were selected for the action plan, which finally consisted of 6 operations and 21 actions. The article concludes that the combination of epidemiology and planning expands the situational explanation. Incorporation of the communicative approach and the equity dimension into Situational Strategic Planning allows empowering health management and helps decrease the gaps from inequality.

People constantly seek to improve in their personal and professional ambit and in net relationships with the aim of achieving their targets. The competition and the competitiveness motivate an individual to become more qualified and overcome their limitations, offering to the organizations an efficient, flexible and dynamic professional. The courses of professional improvement arise as an answer to this social and professional demand, allowing a continuous, accessible and qualified learning. Brazil has qualified institutions that can provide this kind of training and improvement. Some of them are inserted in the Sistema S, which belongs to the Third Sector. Although these courses completely attend the actual demand, they do not have a profile of ideal professional that is already mapped and constructed in order to facilitates the management and dealing with customers. The aim of this paper is to create a tool that can allow managers of the institutions which offer courses of professional training and improvement to map and construct a profile of ideal teaching. For the creation of this profile will be interviewed teachers, coordinators and technicians from one of the institutions that is part of the Sistema S, SENAC-SP, two branches were chosen Osasco and Santo André. Through this study is expected that this tool can be created and become a facilitator for the manager (who works with the offer of courses of professional improvement) whose major role, in such organizations, is to select and hire professionals able to teach these courses, as well as permanently overseeing for their professional improvement.(AU) O final do século XX e o início século XXI foram conceituados como a era do conhecimento e da informação . As pessoas buscam constantemente aperfeiçoar-se no âmbito pessoal, profissional e em sua rede de relacionamentos com o propósito de atingirem suas metas. A concorrência e a competitividade fazem o indivíduo motivar-se para se tornar mais habilitado e superar suas limitações, oferecendo às organizações um profissional eficiente, flexível e dinâmico. Os cursos de aperfeiçoamento profissional surgem como uma resposta a esta demanda social e profissional, possibilitando o aprendizado contínuo, acessível e qualificado. O Brasil possui instituições habilitadas a fornecer este tipo de treinamento e aperfeiçoamento. Algumas delas estão inseridas no Sistema S, pertencentes ao Terceiro Setor. Esses cursos, apesar de atenderem plenamente a demanda existente, não possuem um perfil ideal de profissional já mapeado e construído para facilitar a gestão e o atendimento da sua clientela. Objetiva-se através deste estudo criar uma ferramenta que possibilite aos gestores das instituições oferecedoras de cursos de treinamento e aperfeiçoamento profissional, mapear e construir um perfil de docência ideal. Para criação de tal perfil serão entrevistados docentes, coordenadores e técnicos de uma das instituições que fazem parte do Sistema S, o SENAC-SP, em duas unidades Osasco e Santo André. Espera-se com este estudo, que tal ferramenta seja criada e torne-se uma facilitadora para o gestor (que trabalhe com oferta dos cursos de aperfeiçoamento profissional), cuja principal atividade, em tais organizações, é selecionar e contratar profissionais aptos a ministrarem estes cursos, além de zelar permanentemente por seu aperfeiçoamento profissional.(AU)

No início da década de 1990, foi desenvolvida na Inglaterra uma nova metodologia para alocação de recursos financeiros aos serviços de saúde baseada nas necessidades de saúde da população. Essa metodologia caracteriza-se por adotar o modelo teórico de demanda aos serviços de saúde e por empregar dados de utilização dos serviços para estimar o uso com base na necessidade de saúde. Este artigo objetivou avaliar a aplicabilidade dessa metodologia para alocação de recursos federais no Brasil. Empregaram-se dados do Sistema de Informações Hospitalares do Sistema Único de Saúde sobre as internações de curta permanência ocorridas no Brasil em 1999. Foram analisadas 134 áreas geográficas, que cobrem o país como um todo. Os modelos estatísticos para estimar o uso testaram as seguintes variáveis de necessidades: mortalidade infantil; mortalidade geral padronizada; percentual de analfabetos; percentual de domicílios permanentes chefiados por mulheres e média de pessoas por domicílio. Todos os modelos de estimativa do uso de testados apresentaram coeficientes de regressão com sinal negativo, o que indica que a metodologia estudada apresenta restrições em contextos com grandes desigualdades sociais no uso de serviços de saúde, como o do Brasil. In the early 1990s, a group of British researchers developed a new methodology for healthcare resource allocation based on need. The methodology's main characteristics are to draw on the theoretical model for healthcare services demand and apply data on health services utilization to estimate needs-based use. The objective of the current study was to assess the applicability of this methodology for allocating Federal resources at the local level in Brazil. Data from all acute hospital admissions in 1999 came from the Inpatient Information System of the Unified National Health System (SUS). The country was divided into 134 geographic areas. The statistical models tested to estimate needs-based use applied the following need variables: infant mortality rate; standardized mortality rate; illiteracy rate; proportion of households headed by women; and mean number of household members. All tested models showed negative regression coefficients, indicating that the methodology is inadequate for resource allocation based on need in places like Brazil with large social inequalities in healthcare utilization.

A metodologia desenvolvida nutre-se de elementos utilizados nas propostas de avaliação de desempenho dos sistemas de saúde canadense, australiano, inglês e a da OPS e tem o formato de um painel de controle (dashboard), onde podem ser visualizadas simultaneamente diferentes dimensões da avaliação. In this paper the year 2000 report from the World Health Organization is discussed and the methodology for comparing and classifying the performance of the health systems of the 191 member countries is analyzed. The main conclusions refer to the use of methodologies which are scientifically questionable, to the implementation of indicators which do not evaluate the performance of health systems but rather reveal the existing socio-economic inequalities, to the absence of necessary data for calculating the indicators in the majority of countries, and to the lack of transparency in the procedures adopted for calculating some indicators. En este texto se discute el Informe de la Organización Mundial de la Salud para 2000 y se analiza la metodología utilizada para comparar y clasificar el desempeño de los sistemas de salud de los 191 países miembros. Las principales conclusiones están referidas a la utilización de metodologías de evaluación científicamente cuestionables; a la implementación de indicadores que no evalúan el desempeño de los sistemas de salud sino que expresan las desigualdades socioeconómicas existentes; a la ausencia de datos en la mayoría de los países para el cálculo de los indicadores y a la falta de transparencia en los procedimientos adoptados para el cálculo de algunos indicadores.

The Unique System Health (SUS) was shaped to be an instrument of social justice, using the strategy of decentralization Politico-Administrative, according to the equity, universalization and integrity of the health actions. In the me anti me. after eight years of approval and constitutional legitimation, the implantation proceeding has resigned deflexions during the tragectory idealized by the Sanitarian Reíorm. In this dissertation will be íocalized the possible causes 01' the SUS misguiding obstacles, pointing out two conceiving questions: The equity and the health services decentralization. The great interrogation that permeates alI the work is through the questioning about the SUS possibility, while a theoretical model, realized in Utopian casts, passible to be transformed in more one myth of the brazilian social imaginary, than an effective Political-social, turned to the acquisition of a participating and democratical society. O Sistema Único de Saúde (SUS) foi configurado para ser um instrumento de justiça social, utilizando a estratégia de descentralização político-administrativa, com vistas à equidade, universalização e integridade das ações de saúde. No entanto, após oito anos de aprovação e legitimação constitucional, o seu processo de implantação tem sofrido desvios na sua trajetória idealizada pela Reforma Sanitária. Nesta dissertação serão enfocadas as possíveis causas dos obstáculos desencaminhadores do SUS. Pontualizando duas questões conceituais: a equidade e a descentralização dos serviços de saúde. A grande interrogação que permeia todo o trabalho está no questionamento sobre a factibilidade do SUS, enquanto um modelo teórico, realizado em moldes utópicos, passível de transformar-se em mais um mito do imaginário social brasileiro, do que uma efetiva política social, voltada para a obtenção de uma sociedade democrática e participativa.

This paper analyses the financiai restraints on the decentralization of health services. In part One we go through different types of decentralization and their economic features. And in Part Two the special case of the brazilian experience is evaluated. La eficácia de los procesos de descentralización sanitária está fuertemente condicionada por factores económico-financieros. En la primer parte de este trabajo se analiza una c1asificación de los procesos de descentralización de servicios de salud que distingue 4 tipos ideales. En cada uno de ellos son identificados los intrumentos para transferência de recursos mas adecuados, así como los condicionantes financieros de su eficácia. La segunda parte dei estudio está focalizada en la experiencia brasilena de la década dei 80. Son analizadas las políticas de descentralización sanitária identificándose el tipo ideal ai que se aproximan los instrumentos financieros que fueron adoptados en su formulación. Posteriormente , se examina la implementación de las políticas desentralizadoras evaluándose la medida en que cada una cumplio com los condicionantes correspondentes. A eficácia dos processos de descentralização sanitária está fortemente condicionada por fatores econômico-financeiros. Na primeira parte deste trabalho se analisa uma classificação dos processos de descentralização de serviços de saúde que distingue 4 tipos ideais. Para cada um deles são identificados os intrumentos de repasse de recursos mais adequados, assím como os condicionantes financeiros da sua eficácia. Na segunda parte o estudo é focalizado na experiência brasileira da década de 80. São analisadas as políticas de descentralização sanitária identificándo-se o tipo ideal ao que se aproximam os instrumentos financeiros que foram adotados na sua formulação. Posteriormente, é examinada a implementação das políticas desentralizadoras avaliándo-se a medida em que cada uma cumpríu com os condicionantes correspondentes.

- Abstract
This study focuses on equity in health and specifically the geographic distribution of financial resources. The author reviews the main contemporary theories of social justice and discusses the concept of equity in general and specifically in the health field. Based on the discussion of selected international experiences (United Kingdom, Spain, and Italy), the Resource Allocation Working Party (RAWP) formula used in the United Kingdom is identified as the most adequate distributive methodology, sizing the relative needs based on the population's demographic and epidemiological profiles. Finally, the results are presented from a simulation performed for the Brazilian case, showing that a more equitable geographic distribution of financial resources would require a redistribution favoring the States of the North and Northeast. The article concludes by highlighting that a comparison of actual fund outlays by the Ministry of Health in 1994 and the results of the simulation with the RAWP methodology for the Brazilian case show that the principles written into Brazilian legislation were absent from the geographic distribution of financial resources.
