Topological correctness plays a critical role in many image segmentation tasks, yet most networks are trained using pixel-wise loss functions, such as Dice, neglecting topological accuracy. Existing topology-aware methods often lack robust topological guarantees, are limited to specific use cases, or impose high computational costs. In this work, we propose a novel, graph-based framework for topologically accurate image segmentation that is both computationally efficient and generally applicable. Our method constructs a component graph that fully encodes the topological information of both the prediction and ground truth, allowing us to efficiently identify topologically critical regions and aggregate a loss based on local neighborhood information. Furthermore, we introduce a strict topological metric capturing the homotopy equivalence between the union and intersection of prediction-label pairs. We formally prove the topological guarantees of our approach and empirically validate its effectiveness on binary and multi-class datasets. Our loss demonstrates state-of-the-art performance with up to fivefold faster loss computation compared to persistent homology methods.

To ensure the efficiency of robot autonomy under diverse real-world conditions, a high-quality heterogeneous dataset is essential to benchmark the operating algorithms' performance and robustness. Current benchmarks predominantly focus on urban terrains, specifically for on-road autonomous driving, leaving multi-degraded, densely vegetated, dynamic and feature-sparse environments, such as underground tunnels, natural fields, and modern indoor spaces underrepresented. To fill this gap, we introduce EnvoDat, a large-scale, multi-modal dataset collected in diverse environments and conditions, including high illumination, fog, rain, and zero visibility at different times of the day. Overall, EnvoDat contains 26 sequences from 13 scenes, 10 sensing modalities, over 1.9TB of data, and over 89K fine-grained polygon-based annotations for more than 82 object and terrain classes. We post-processed EnvoDat in different formats that support benchmarking SLAM and supervised learning algorithms, and fine-tuning multimodal vision models. With EnvoDat, we contribute to environment-resilient robotic autonomy in areas where the conditions are extremely challenging. In Magnetic Resonance Imaging (MRI), high temporal-resolved motion can be useful for image acquisition and reconstruction, MR-guided radiotherapy, dynamic contrast-enhancement, flow and perfusion imaging, and functional assessment of motion patterns in cardiovascular, abdominal, peristaltic, fetal, or musculoskeletal imaging. Conventionally, these motion estimates are derived through image-based registration, a particularly challenging task for complex motion patterns and high dynamic resolution. The accelerated scans in such applications result in imaging artifacts that compromise the motion estimation. In this work, we propose a novel self-supervised deep learning-based framework, dubbed the Local-All Pass Attention Network (LAPANet), for non-rigid motion estimation directly from the acquired accelerated Fourier space, i.e. k-space. The proposed approach models non-rigid motion as the cumulative sum of local translational displacements, following the Local All-Pass (LAP) registration technique. LAPANet was evaluated on cardiac motion estimation across various sampling trajectories and acceleration rates. Our results demonstrate superior accuracy compared to prior conventional and deep learning-based registration methods, accommodating as few as 2 lines/frame in a Cartesian trajectory and 3 spokes/frame in a non-Cartesian trajectory. The achieved high temporal resolution (less than 5 ms) for non-rigid motion opens new avenues for motion detection, tracking and correction in dynamic and real-time MRI applications.

Deep learning has made significant strides in medical imaging, leveraging the use of large datasets to improve diagnostics and prognostics. However, large datasets often come with inherent errors through subject selection and acquisition. In this paper, we investigate the use of Diffusion Autoencoder (DAE) embeddings for uncovering and understanding data characteristics and biases, including biases for protected variables like sex and data abnormalities indicative of unwanted protocol variations. We use sagittal T2-weighted magnetic resonance (MR) images of the neck, chest, and lumbar region from 11186 German National Cohort (NAKO) participants. We compare DAE embeddings with existing generative models like StyleGAN and Variational Autoencoder. Evaluations on a large-scale dataset consisting of sagittal T2-weighted MR images of three spine regions show that DAE embeddings effectively separate protected variables such as sex and age. Furthermore, we used t-SNE visualization to identify unwanted variations in imaging protocols, revealing differences in head positioning. Our embedding can identify samples where a sex predictor will have issues learning the correct sex. Our findings highlight the potential of using advanced embedding techniques like DAEs to detect data quality issues and biases in medical imaging datasets. In natural language processing and computer vision, self-supervised pre-training on large datasets unlocks foundational model capabilities across domains and tasks. However, this potential has not yet been realised in time series analysis, where existing methods disregard the heterogeneous nature of time series characteristics. Time series are prevalent in many domains, including medicine, engineering, natural sciences, and finance, but their characteristics vary significantly in terms of variate count, inter-variate relationships, temporal dynamics, and sampling frequency. This inherent heterogeneity across domains prevents effective pre-training on large time series corpora. To address this issue, we introduce OTiS, an open model for general time series analysis, that has been specifically designed to handle multi-domain heterogeneity. We propose a novel pre-training paradigm including a tokeniser with learnable domain-specific signatures, a dual masking strategy to capture temporal causality, and a normalised cross-correlation loss to model long-range dependencies. Our model is pre-trained on a large corpus of 640,187 samples and 11 billion time points spanning 8 distinct domains, enabling it to analyse time series from any (unseen) domain. In comprehensive experiments across 15 diverse applications - including classification, regression, and forecasting - OTiS showcases its ability to accurately capture domain-specific data characteristics and demonstrates its competitiveness against state-of-the-art baselines. Cardiac magnetic resonance imaging (CMR), considered the gold standard for noninvasive cardiac assessment, is a diverse and complex modality requiring a wide variety of image processing tasks for comprehensive assessment of cardiac morphology and function. Advances in deep learning have enabled the development of state-of-the-art (SoTA) models for these tasks. However, model training is challenging due to data and label scarcity, especially in the less common imaging sequences. Moreover, each model is often trained for a specific task, with no connection between related tasks. In this work, we introduce a vision foundation model trained for CMR assessment, that is trained in a self-supervised fashion on 36 million CMR images. We then finetune the model in supervised way for 9 clinical tasks typical to a CMR workflow, across classification, segmentation, landmark localization, and pathology detection. We demonstrate improved accuracy and robustness across all tasks, over a range of available labeled dataset sizes. We also demonstrate improved few-shot learning with fewer labeled samples, a common challenge in medical image analyses. We achieve an out-of-box performance comparable to SoTA for most clinical tasks. The proposed method thus presents a resource-efficient, unified framework for CMR assessment, with the potential to accelerate the development of deep learning-based solutions for image analysis tasks, even with few annotated data available., Comment: 11 pages, 3 figures, 4 tables

Active learning (AL) is a widely used technique for optimizing data labeling in machine learning by iteratively selecting, labeling, and training on the most informative data. However, its integration with formal privacy-preserving methods, particularly differential privacy (DP), remains largely underexplored. While some works have explored differentially private AL for specialized scenarios like online learning, the fundamental challenge of combining AL with DP in standard learning settings has remained unaddressed, severely limiting AL's applicability in privacy-sensitive domains. This work addresses this gap by introducing differentially private active learning (DP-AL) for standard learning settings. We demonstrate that naively integrating DP-SGD training into AL presents substantial challenges in privacy budget allocation and data utilization. To overcome these challenges, we propose step amplification, which leverages individual sampling probabilities in batch creation to maximize data point participation in training steps, thus optimizing data utilization. Additionally, we investigate the effectiveness of various acquisition functions for data selection under privacy constraints, revealing that many commonly used functions become impractical. Our experiments on vision and natural language processing tasks show that DP-AL can improve performance for specific datasets and model architectures. However, our findings also highlight the limitations of AL in privacy-constrained environments, emphasizing the trade-offs between privacy, model accuracy, and data selection accuracy.

Predicting future disease progression risk from medical images is challenging due to patient heterogeneity, and subtle or unknown imaging biomarkers. Moreover, deep learning (DL) methods for survival analysis are susceptible to image domain shifts across scanners. We tackle these issues in the task of predicting late dry Age-related Macular Degeneration (dAMD) onset from retinal OCT scans. We propose a novel DL method for survival prediction to jointly predict from the current scan a risk score, inversely related to time-to-conversion, and the probability of conversion within a time interval $t$. It uses a family of parallel hyperplanes generated by parameterizing the bias term as a function of $t$. In addition, we develop unsupervised losses based on intra-subject image pairs to ensure that risk scores increase over time and that future conversion predictions are consistent with AMD stage prediction using actual scans of future visits. Such losses enable data-efficient fine-tuning of the trained model on new unlabeled datasets acquired with a different scanner. Extensive evaluation on two large datasets acquired with different scanners resulted in a mean AUROCs of 0.82 for Dataset-1 and 0.83 for Dataset-2, across prediction intervals of 6,12 and 24 months., Comment: accepted in MICCAI 2024

Although existing medical image segmentation methods provide impressive pixel-wise accuracy, they often neglect topological correctness, making their segmentations unusable for many downstream tasks. One option is to retrain such models whilst including a topology-driven loss component. However, this is computationally expensive and often impractical. A better solution would be to have a versatile plug-and-play topology refinement method that is compatible with any domain-specific segmentation pipeline. Directly training a post-processing model to mitigate topological errors often fails as such models tend to be biased towards the topological errors of a target segmentation network. The diversity of these errors is confined to the information provided by a labelled training set, which is especially problematic for small datasets. Our method solves this problem by training a model-agnostic topology refinement network with synthetic segmentations that cover a wide variety of topological errors. Inspired by the Stone-Weierstrass theorem, we synthesize topology-perturbation masks with randomly sampled coefficients of orthogonal polynomial bases, which ensures a complete and unbiased representation. Practically, we verified the efficiency and effectiveness of our methods as being compatible with multiple families of polynomial bases, and show evidence that our universal plug-and-play topology refinement network outperforms both existing topology-driven learning-based and post-processing methods. The Orientation Distribution Function (ODF) characterizes key brain microstructural properties and plays an important role in understanding brain structural connectivity. Recent works introduced Implicit Neural Representation (INR) based approaches to form a spatially aware continuous estimate of the ODF field and demonstrated promising results in key tasks of interest when compared to conventional discrete approaches. However, traditional INR methods face difficulties when scaling to large-scale images, such as modern ultra-high-resolution MRI scans, posing challenges in learning fine structures as well as inefficiencies in training and inference speed. In this work, we propose HashEnc, a grid-hash-encoding-based estimation of the ODF field and demonstrate its effectiveness in retaining structural and textural features. We show that HashEnc achieves a 10% enhancement in image quality while requiring 3x less computational resources than current methods. Accurate calibration of finite element (FE) models of human intervertebral discs (IVDs) is essential for their reliability and application in diagnosing and planning treatments for spinal conditions. Traditional calibration methods are computationally intensive, requiring iterative, derivative-free optimization algorithms that often take hours or days to converge. This study addresses these challenges by introducing a novel, efficient, and effective calibration method for an L4-L5 IVD FE model using a neural network (NN) surrogate. The NN surrogate predicts simulation outcomes with high accuracy, outperforming other machine learning models, and significantly reduces the computational cost associated with traditional FE simulations. Next, a Projected Gradient Descent (PGD) approach guided by gradients of the NN surrogate is proposed to efficiently calibrate FE models. Our method explicitly enforces feasibility with a projection step, thus maintaining material bounds throughout the optimization process. The proposed method is evaluated against state-of-the-art Genetic Algorithm (GA) and inverse model baselines on synthetic and in vitro experimental datasets. Our approach demonstrates superior performance on synthetic data, achieving a Mean Absolute Error (MAE) of 0.06 compared to the baselines' MAE of 0.18 and 0.54, respectively. On experimental specimens, our method outperforms the baseline in 5 out of 6 cases. Most importantly, our approach reduces calibration time to under three seconds, compared to up to 8 days per sample required by traditional calibration. Counterfactual explanations (CEs) aim to enhance the interpretability of machine learning models by illustrating how alterations in input features would affect the resulting predictions. Common CE approaches require an additional model and are typically constrained to binary counterfactuals. In contrast, we propose a novel method that operates directly on the latent space of a generative model, specifically a Diffusion Autoencoder (DAE). This approach offers inherent interpretability by enabling the generation of CEs and the continuous visualization of the model's internal representation across decision boundaries. Our method leverages the DAE's ability to encode images into a semantically rich latent space in an unsupervised manner, eliminating the need for labeled data or separate feature extraction models. We show that these latent representations are helpful for medical condition classification and the ordinal regression of severity pathologies, such as vertebral compression fractures (VCF) and diabetic retinopathy (DR). Beyond binary CEs, our method supports the visualization of ordinal CEs using a linear model, providing deeper insights into the model's decision-making process and enhancing interpretability. Experiments across various medical imaging datasets demonstrate the method's advantages in interpretability and versatility. The linear manifold of the DAE's latent space allows for meaningful interpolation and manipulation, making it a powerful tool for exploring medical image properties. Cardiac Magnetic Resonance Imaging (CMR) is the gold standard for diagnosing cardiovascular diseases. Clinical diagnoses predominantly rely on magnitude-only Digital Imaging and Communications in Medicine (DICOM) images, omitting crucial phase information that might provide additional diagnostic benefits. In contrast, k-space is complex-valued and encompasses both magnitude and phase information, while humans cannot directly perceive. In this work, we propose KMAE, a Transformer-based model specifically designed to process k-space data directly, eliminating conventional intermediary conversion steps to the image domain. KMAE can handle critical cardiac disease classification, relevant phenotype regression, and cardiac morphology segmentation tasks. We utilize this model to investigate the potential of k-space-based diagnosis in cardiac MRI. Notably, this model achieves competitive classification and regression performance compared to image-domain methods e.g. Masked Autoencoders (MAEs) and delivers satisfactory segmentation performance with a myocardium dice score of 0.884. Last but not least, our model exhibits robust performance with consistent results even when the k-space is 8* undersampled. We encourage the MR community to explore the untapped potential of k-space and pursue end-to-end, automated diagnosis with reduced human intervention.

Our findings indicate that adopting "advanced" computational elements fails to significantly improve registration accuracy. Instead, well-established registration-specific designs offer fair improvements, enhancing results by a marginal 1.5\% over the baseline. Our findings emphasize the importance of rigorous, unbiased evaluation and contribution disentanglement of all low- and high-level registration components, rather than simply following the computer vision trends with "more advanced" computational blocks. We advocate for simpler yet effective solutions and novel evaluation metrics that go beyond conventional registration accuracy, warranting further research across diverse organs and modalities. General vision encoders like DINOv2 and SAM have recently transformed computer vision. Even though they are trained on natural images, such encoder models have excelled in medical imaging, e.g., in classification, segmentation, and registration. However, no in-depth comparison of different state-of-the-art general vision encoders for medical registration is available. In this work, we investigate how well general vision encoder features can be used in the dissimilarity metrics for medical image registration. We explore two encoders that were trained on natural images as well as one that was fine-tuned on medical data. We apply the features within the well-established B-spline FFD registration framework. In extensive experiments on cardiac cine MRI data, we find that using features as additional guidance for conventional metrics improves the registration quality. In this article, we propose a novel standalone hybrid Spiking-Convolutional Neural Network (SC-NN) model and test on using image inpainting tasks. Our approach uses the unique capabilities of SNNs, such as event-based computation and temporal processing, along with the strong representation learning abilities of CNNs, to generate high-quality inpainted images. The model is trained on a custom dataset specifically designed for image inpainting, where missing regions are created using masks. The hybrid model consists of SNNConv2d layers and traditional CNN layers. The SNNConv2d layers implement the leaky integrate-and-fire (LIF) neuron model, capturing spiking behavior, while the CNN layers capture spatial features. In this study, a mean squared error (MSE) loss function demonstrates the training process, where a training loss value of 0.015, indicates accurate performance on the training set and the model achieved a validation loss value as low as 0.0017 on the testing set. Clinicians spend a significant amount of time reviewing medical images and transcribing their findings regarding patient diagnosis, referral and treatment in text form. Vision-language models (VLMs), which automatically interpret images and summarize their findings as text, have enormous potential to alleviate clinical workloads and increase patient access to high-quality medical care. While foundational models have stirred considerable interest in the medical community, it is unclear whether their general capabilities translate to real-world clinical utility. In this work, we show that foundation VLMs markedly underperform compared to practicing ophthalmologists on specialist tasks crucial to the care of patients with age-related macular degeneration (AMD). To address this, we initially identified the essential capabilities required for image-based clinical decision-making, and then developed a curriculum to selectively train VLMs in these skills. The resulting model, RetinaVLM, can be instructed to write reports that significantly outperform those written by leading foundation medical VLMs in disease staging (F1 score of 0.63 vs. 0.11) and patient referral (0.67 vs. 0.39), and approaches the diagnostic performance of junior ophthalmologists (who achieve 0.77 and 0.78 on the respective tasks). Furthermore, in a reader study involving two senior ophthalmologists with up to 32 years of experience, RetinaVLM's reports were found to be similarly correct (78.6% vs. 82.1%) and complete (both 78.6%) as reports written by junior ophthalmologists with up to 10 years of experience. Machine unlearning is the process of removing the impact of a particular set of training samples from a pretrained model. It aims to fulfill the "right to be forgotten", which grants the individuals such as patients the right to reconsider their contribution in models including medical imaging models. In this study, we evaluate the effectiveness (performance) and computational efficiency of different unlearning algorithms in medical imaging domain. Our evaluations demonstrate that the considered unlearning algorithms perform well on the retain set (samples whose influence on the model is allowed to be retained) and forget set (samples whose contribution to the model should be eliminated), and show no bias against male or female samples. They, however, adversely impact the generalization of the model, especially for larger forget set sizes. Moreover, they might be biased against easy or hard samples, and need additional computational overhead for hyper-parameter tuning. In conclusion, machine unlearning seems promising for medical imaging, but the existing unlearning algorithms still needs further improvements to become more practical for medical applications.

Traditional Variational Autoencoders (VAEs) are constrained by the limitations of the Evidence Lower Bound (ELBO) formulation, particularly when utilizing simplistic, non-analytic, or unknown prior distributions. These limitations inhibit the VAE's ability to generate high-quality samples and provide clear, interpretable latent representations. This work introduces the Entropy Decomposed Variational Autoencoder (ED-VAE), a novel re-formulation of the ELBO that explicitly includes entropy and cross-entropy components. This reformulation significantly enhances model flexibility, allowing for the integration of complex and non-standard priors. By providing more detailed control over the encoding and regularization of latent spaces, ED-VAE not only improves interpretability but also effectively captures the complex interactions between latent variables and observed data, thus leading to better generative performance.

With the increasing incidence of neurodegenerative diseases such as Alzheimer's Disease (AD), there is a need for further research that enhances detection and monitoring of the diseases. We present MORPHADE (Morphological Autoencoders for Alzheimer's Disease Detection), a novel unsupervised learning approach which uses deformations to allow the analysis of 3D T1-weighted brain images. To the best of our knowledge, this is the first use of deformations with deep unsupervised learning to not only detect, but also localize and assess the severity of structural changes in the brain due to AD. We obtain markedly higher anomaly scores in clinically important areas of the brain in subjects with AD compared to healthy controls, showcasing that our method is able to effectively locate AD-related atrophy. We additionally observe a visual correlation between the severity of atrophy highlighted in our anomaly maps and medial temporal lobe atrophy scores evaluated by a clinical expert. Finally, our method achieves an AUROC of 0.80 in detecting AD, out-performing several supervised and unsupervised baselines. We believe our framework shows promise as a tool towards improved understanding, monitoring and detection of AD. Cardiac Cine Magnetic Resonance Imaging (MRI) provides an accurate assessment of heart morphology and function in clinical practice. However, MRI requires long acquisition times, with recent deep learning-based methods showing great promise to accelerate imaging and enhance reconstruction quality. Existing networks exhibit some common limitations that constrain further acceleration possibilities, including single-domain learning, reliance on a single regularization term, and equal feature contribution. To address these limitations, we propose to embed information from multiple domains, including low-rank, image, and k-space, in a novel deep learning network for MRI reconstruction, which we denote as A-LIKNet. A-LIKNet adopts a parallel-branch structure, enabling independent learning in the k-space and image domain. Coupled information sharing layers realize the information exchange between domains. Furthermore, we introduce attention mechanisms into the network to assign greater weights to more critical coils or important temporal frames. Training and testing were conducted on an in-house dataset, including 91 cardiovascular patients and 38 healthy subjects scanned with 2D cardiac Cine using retrospective undersampling. Additionally, we evaluated A-LIKNet on the real-time 8x prospectively undersampled data from the OCMR dataset. The results demonstrate that our proposed A-LIKNet outperforms existing methods and provides high-quality reconstructions. The network can effectively reconstruct highly retrospectively undersampled dynamic MR images up to 24x accelerations, indicating its potential for single breath-hold imaging.

Segmentation is often the first step in many medical image analyses workflows. Deep learning approaches, while giving state-of-the-art accuracies, are data intensive and do not scale well to low data regimes. We introduce Deep Conditional Shape Models 2.0, which uses an edge detector, along with an implicit shape function conditioned on edge maps, to leverage cross-modality shape information. The shape function is trained exclusively on a source domain (contrasted CT) and applied to the target domain of interest (3D echocardiography). We demonstrate data efficiency in the target domain by varying the amounts of training data used in the edge detection stage. We observe that DCSM 2.0 outperforms the baseline at all data levels in terms of Hausdorff distances, and while using 50% or less of the training data in terms of average mesh distance, and at 10% or less of the data with the dice coefficient. The method scales well to low data regimes, with gains of up to 5% in dice coefficient, 2.58 mm in average surface distance and 21.02 mm in Hausdorff distance when using just 2% (22 volumes) of the training data., Comment: Best oral paper award at ISBI 2024

Since the Transformer architecture emerged, language model development has grown, driven by their promising potential. Releasing these models into production requires properly understanding their behavior, particularly in sensitive domains like medicine. Despite this need, the medical literature still lacks practical assessment of pre-trained language models, which are especially valuable in settings where only consumer-grade computational resources are available. To address this gap, we have conducted a comprehensive survey of language models in the medical field and evaluated a subset of these for medical text classification and conditional text generation. The subset includes 53 models with 110 million to 13 billion parameters, spanning the Transformer-based model families and knowledge domains. Different approaches are employed for text classification, including zero-shot learning, enabling tuning without the need to train the model. These approaches are helpful in our target settings, where many users of language models find themselves. The results reveal remarkable performance across the tasks and datasets evaluated, underscoring the potential of certain models to contain medical knowledge, even without domain specialization. This study thus advocates for further exploration of model applications in medical contexts, particularly in computational resource-constrained settings, to benefit a wide range of users. Existing learning-based cortical surface reconstruction approaches heavily rely on the supervision of pseudo ground truth (pGT) cortical surfaces for training. Such pGT surfaces are generated by traditional neuroimage processing pipelines, which are time consuming and difficult to generalize well to low-resolution brain MRI, e.g., from fetuses and neonates. In this work, we present CoSeg, a learning-based cortical surface reconstruction framework weakly supervised by brain segmentations without the need for pGT surfaces. CoSeg introduces temporal attention networks to learn time-varying velocity fields from brain MRI for diffeomorphic surface deformations, which fit an initial surface to target cortical surfaces within only 0.11 seconds for each brain hemisphere. A weakly supervised loss is designed to reconstruct pial surfaces by inflating the white surface along the normal direction towards the boundary of the cortical gray matter segmentation. This alleviates partial volume effects and encourages the pial surface to deform into deep and challenging cortical sulci. We evaluate CoSeg on 1,113 adult brain MRI at 1mm and 2mm resolution. CoSeg achieves superior geometric and morphological accuracy compared to existing learning-based approaches. In differentially private (DP) machine learning, the privacy guarantees of DP mechanisms are often reported and compared on the basis of a single $(\varepsilon, \delta)$-pair. This practice overlooks that DP guarantees can vary substantially even between mechanisms sharing a given $(\varepsilon, \delta)$, and potentially introduces privacy vulnerabilities which can remain undetected. This motivates the need for robust, rigorous methods for comparing DP guarantees in such cases. Here, we introduce the $\Delta$-divergence between mechanisms which quantifies the worst-case excess privacy vulnerability of choosing one mechanism over another in terms of $(\varepsilon, \delta)$, $f$-DP and in terms of a newly presented Bayesian interpretation. Moreover, as a generalisation of the Blackwell theorem, it is endowed with strong decision-theoretic foundations. Through application examples, we show that our techniques can facilitate informed decision-making and reveal gaps in the current understanding of privacy risks, as current practices in DP-SGD often result in choosing mechanisms with high excess privacy vulnerabilities., Comment: ICML 2024

Field-of-view (FOV) recovery of truncated chest CT scans is crucial for accurate body composition analysis, which involves quantifying skeletal muscle and subcutaneous adipose tissue (SAT) on CT slices. This, in turn, enables disease prognostication. Here, we present a method for recovering truncated CT slices using generative image outpainting. We train a diffusion model and apply it to truncated CT slices generated by simulating a small FOV. Visual prompting infuses visual information into the input image to adapt models toward specific predictions and tasks. Recently, manually crafted markers such as red circles are shown to guide the model to attend to a target region on the image. However, these markers only work on models trained with data containing those markers. Moreover, finding these prompts requires guesswork or prior knowledge of the domain on which the model is trained. This work circumvents manual design constraints by proposing to learn the visual prompts for guiding the attention of vision transformers. The learned visual prompt, added to any input image would redirect the attention of the pre-trained vision transformer to its spatial location on the image. Specifically, the prompt is learned in a self-supervised manner without requiring annotations and without fine-tuning the vision transformer. Cardiac Magnetic Resonance (CMR) imaging serves as the gold-standard for evaluating cardiac morphology and function. Typically, a multi-view CMR stack, covering short-axis (SA) and 2/3/4-chamber long-axis (LA) views, is acquired for a thorough cardiac assessment. However, efficiently streamlining the complex, high-dimensional 3D+T CMR data and distilling compact, coherent representation remains a challenge. In this work, we introduce a whole-heart self-supervised learning framework that utilizes masked imaging modeling to automatically uncover the correlations between spatial and temporal patches throughout the cardiac stacks. This process facilitates the generation of meaningful and well-clustered heart representations without relying on the traditionally required, and often costly, labeled data. The learned heart representation can be directly used for various downstream tasks. Furthermore, our method demonstrates remarkable robustness, ensuring consistent representations even when certain CMR planes are missing/flawed. We train our model on 14,000 unlabeled CMR data from UK BioBank and evaluate it on 1,000 annotated data. The proposed method demonstrates superior performance to baselines in tasks that demand comprehensive 3D+T cardiac information, e.g. cardiac phenotype (ejection fraction and ventricle volume) prediction and multi-plane/multi-frame CMR segmentation, highlighting its effectiveness in extracting comprehensive cardiac features that are both anatomically and pathologically relevant.

Real-time computational speed and a high degree of precision are requirements for computer-assisted interventions. Applying a segmentation network to a medical video processing task can introduce significant inter-frame prediction noise. Existing approaches can reduce inconsistencies by including temporal information but often impose requirements on the architecture or dataset. This paper proposes a method to include temporal information in any segmentation model and, thus, a technique to improve video segmentation performance without alterations during training or additional labeling. With Motion-Corrected Moving Average, we refine the exponential moving average between the current and previous predictions. Using optical flow to estimate the movement between consecutive frames, we can shift the prior term in the moving-average calculation to align with the geometry of the current frame. The optical flow calculation does not require the output of the model and can therefore be performed in parallel, leading to no significant runtime penalty for our approach. We evaluate our approach on two publicly available segmentation datasets and two proprietary endoscopic datasets and show improvements over a baseline approach.

The prevailing deep learning-based methods of predicting cardiac segmentation involve reconstructed magnetic resonance (MR) images. The heavy dependency of segmentation approaches on image quality significantly limits the acceleration rate in fast MR reconstruction. Moreover, the practice of treating reconstruction and segmentation as separate sequential processes leads to artifact generation and information loss in the intermediate stage. These issues pose a great risk to achieving high-quality outcomes. To leverage the redundant k-space information overlooked in this dual-step pipeline, we introduce a novel approach to directly deriving segmentations from sparse k-space samples using a transformer (DiSK). DiSK operates by globally extracting latent features from 2D+time k-space data with attention blocks and subsequently predicting the segmentation label of query points. We evaluate our model under various acceleration factors (ranging from 4 to 64) and compare against two image-based segmentation baselines. Our model consistently outperforms the baselines in Dice and Hausdorff distances across foreground classes for all presented sampling rates.

Objectives: To present a publicly available torso segmentation network for large epidemiology datasets on volumetric interpolated breath-hold examination (VIBE) images. Materials & Methods: We extracted preliminary segmentations from TotalSegmentator, spine, and body composition networks for VIBE images, then improved them iteratively and retrained a nnUNet network. Using subsets of NAKO (85 subjects) and UK Biobank (16 subjects), we evaluated with Dice-score on a holdout set (12 subjects) and existing organ segmentation approach (1000 subjects), generating 71 semantic segmentation types for VIBE images. We provide an additional network for the vertebra segments 22 individual vertebra types. Results: We achieved an average Dice score of 0.89 +- 0.07 overall 71 segmentation labels. We scored > 0.90 Dice-score on the abdominal organs except for the pancreas with a Dice of 0.70. Automated medical image analysis systems often require large amounts of training data with high quality labels, which are difficult and time consuming to generate. This paper introduces Radiology Object in COntext version 2 (ROCOv2), a multimodal dataset consisting of radiological images and associated medical concepts and captions extracted from the PMC Open Access subset. It is an updated version of the ROCO dataset published in 2018, and adds 35,705 new images added to PMC since 2018. It further provides manually curated concepts for imaging modalities with additional anatomical and directional concepts for X-rays. The dataset consists of 79,789 images and has been used, with minor modifications, in the concept detection and caption prediction tasks of ImageCLEFmedical Caption 2023. The dataset is suitable for training image annotation models based on image-caption pairs, or for multi-label image classification using Unified Medical Language System (UMLS) concepts provided with each image. The Developing Human Connectome Project (dHCP) aims to explore developmental patterns of the human brain during the perinatal period. An automated processing pipeline has been developed to extract high-quality cortical surfaces from structural brain magnetic resonance (MR) images for the dHCP neonatal dataset. However, the current implementation of the pipeline requires more than 6.5 hours to process a single MRI scan, making it expensive for large-scale neuroimaging studies. In this paper, we propose a fast deep learning (DL) based pipeline for dHCP neonatal cortical surface reconstruction, incorporating DL-based brain extraction, cortical surface reconstruction and spherical projection, as well as GPU-accelerated cortical surface inflation and cortical feature estimation. We introduce a multiscale deformation network to learn diffeomorphic cortical surface reconstruction end-to-end from T2-weighted brain MRI. A fast unsupervised spherical mapping approach is integrated to minimize metric distortions between cortical surfaces and projected spheres. The entire workflow of our DL-based dHCP pipeline completes within only 24 seconds on a modern GPU, which is nearly 1000 times faster than the original dHCP pipeline. Manual quality control demonstrates that for 82.5% of the test samples, our DL-based pipeline produces superior (54.2%) or equal quality (28.3%) cortical surfaces compared to the original dHCP pipeline.

High-resolution manometry (HRM) is the gold standard in diagnosing esophageal motility disorders. As HRM is typically conducted under short-term laboratory settings, intermittently occurring disorders are likely to be missed. Therefore, long-term (up to 24h) HRM (LTHRM) is used to gain detailed insights into the swallowing behavior. However, analyzing the extensive data from LTHRM is challenging and time consuming as medical experts have to analyze the data manually, which is slow and prone to errors. To address this challenge, we propose a Deep Learning based swallowing detection method to accurately identify swallowing events and secondary non-deglutitive-induced esophageal motility disorders in LTHRM data. We then proceed with clustering the identified swallows into distinct classes, which are analyzed by highly experienced clinicians to validate the different swallowing patterns. We evaluate our computational pipeline on a total of 25 LTHRMs, which were meticulously annotated by medical experts. By detecting more than 94% of all relevant swallow events and providing all relevant clusters for a more reliable diagnostic process among experienced clinicians, we are able to demonstrate the effectiveness as well as positive clinical impact of our approach to make LTHRM feasible in clinical care.

Accurate motion estimation at high acceleration factors enables rapid motion-compensated reconstruction in Magnetic Resonance Imaging (MRI) without compromising the diagnostic image quality. In this work, we introduce an attention-aware deep learning-based framework that can perform non-rigid pairwise registration for fully sampled and accelerated MRI. We extract local visual representations to build similarity maps between the registered image pairs at multiple resolution levels and additionally leverage long-range contextual information using a transformer-based module to alleviate ambiguities in the presence of artifacts caused by undersampling. We combine local and global dependencies to perform simultaneous coarse and fine motion estimation. The proposed method was evaluated on in-house acquired fully sampled and accelerated data of 101 patients and 62 healthy subjects undergoing cardiac and thoracic MRI. The impact of motion estimation accuracy on the downstream task of motion-compensated reconstruction was analyzed. We demonstrate that our model derives reliable and consistent motion fields across different sampling trajectories (Cartesian and radial) and acceleration factors of up to 16x for cardiac motion and 30x for respiratory motion and achieves superior image quality in motion-compensated reconstruction qualitatively and quantitatively compared to conventional and recent deep learning-based approaches. Report generation models offer fine-grained textual interpretations of medical images like chest X-rays, yet they often lack interactivity (i.e. the ability to steer the generation process through user queries) and localized interpretability (i.e. visually grounding their predictions), which we deem essential for future adoption in clinical practice. While there have been efforts to tackle these issues, they are either limited in their interactivity by not supporting textual queries or fail to also offer localized interpretability. Therefore, we propose a novel multitask architecture and training paradigm integrating textual prompts and bounding boxes for diverse aspects like anatomical regions and pathologies. We call this approach the Chest X-Ray Explainer (ChEX). Evaluations across a heterogeneous set of 9 chest X-ray tasks, including localized image interpretation and report generation, showcase its competitiveness with SOTA models while additional analysis demonstrates ChEX's interactive capabilities. Generative AI, in general, and synthetic visual data generation, in specific, hold much promise for benefiting surgical training by providing photorealism to simulation environments. Current training methods primarily rely on reading materials and observing live surgeries, which can be time-consuming and impractical. In this work, we take a significant step towards improving the training process. Specifically, we use diffusion models in combination with a zero-shot video diffusion method to interactively generate realistic laparoscopic images and videos by specifying a surgical action through text and guiding the generation with tool positions through segmentation masks. We demonstrate the performance of our approach using the publicly available Cholec dataset family and evaluate the fidelity and factual correctness of our generated images using a surgical action recognition model as well as the pixel-wise F1-score for the spatial control of tool generation. Neural implicit k-space representations have shown promising results for dynamic MRI at high temporal resolutions. Yet, their exclusive training in k-space limits the application of common image regularization methods to improve the final reconstruction. In this work, we introduce the concept of parallel imaging-inspired self-consistency (PISCO), which we incorporate as novel self-supervised k-space regularization enforcing a consistent neighborhood relationship. At no additional data cost, the proposed regularization significantly improves neural implicit k-space reconstructions on simulated data. Abdominal in-vivo reconstructions using PISCO result in enhanced spatio-temporal image quality compared to state-of-the-art methods. This research explores the integration of language models and unsupervised anomaly detection in medical imaging, addressing two key questions: (1) Can language models enhance the interpretability of anomaly detection maps? and (2) Can anomaly maps improve the generalizability of language models in open-set anomaly detection tasks? To investigate these questions, we introduce a new dataset for multi-image visual question-answering on brain magnetic resonance images encompassing multiple conditions. We propose KQ-Former (Knowledge Querying Transformer), which is designed to optimally align visual and textual information in limited-sample contexts. Our model achieves a 60.81% accuracy on closed questions, covering disease classification and severity across 15 different classes. For open questions, KQ-Former demonstrates a 70% improvement over the baseline with a BLEU-4 score of 0.41, and achieves the highest entailment ratios (up to 71.9%) and lowest contradiction ratios (down to 10.0%) among various natural language inference models. Furthermore, integrating anomaly maps results in an 18% accuracy increase in detecting open-set anomalies, thereby enhancing the language model's generalizability to previously unseen medical conditions. Cardiac magnetic resonance (CMR) image acquisition requires subjects to hold their breath while 2D cine images are acquired. This process assumes that the heart remains in the same position across all slices. However, differences in breathhold positions or patient motion introduce 3D slice misalignments. In this work, we propose an algorithm that simultaneously aligns all SA and LA slices by maximizing the pair-wise intensity agreement between their intersections. Unlike previous works, our approach is formulated as a subject-specific optimization problem and requires no prior knowledge of the underlying anatomy. We quantitatively demonstrate that the proposed method is robust against a large range of rotations and translations by synthetically misaligning 10 motion-free datasets and aligning them back using the proposed method.

Anatomical atlases are widely used for population analysis. Conditional atlases target a particular sub-population defined via certain conditions (e.g. demographics or pathologies) and allow for the investigation of fine-grained anatomical differences - such as morphological changes correlated with age. Existing approaches use either registration-based methods that are unable to handle large anatomical variations or generative models, which can suffer from training instabilities and hallucinations. To overcome these limitations, we use latent diffusion models to generate deformation fields, which transform a general population atlas into one representing a specific sub-population. By generating a deformation field and registering the conditional atlas to a neighbourhood of images, we ensure structural plausibility and avoid hallucinations, which can occur during direct image synthesis. We compare our method to several state-of-the-art atlas generation methods in experiments using 5000 brain as well as whole-body MR images from UK Biobank. Our method generates highly realistic atlases with smooth transformations and high anatomical fidelity, outperforming the baselines.

In this paper, we extended the method proposed in [17] to enable humans to interact naturally with autonomous agents through vocal and textual conversations. Our extended method exploits the inherent capabilities of pre-trained large language models (LLMs), multimodal visual language models (VLMs), and speech recognition (SR) models to decode the high-level natural language conversations and semantic understanding of the robot's task environment, and abstract them to the robot's actionable commands or queries. We performed a quantitative evaluation of our framework's natural vocal conversation understanding with participants from different racial backgrounds and English language accents. The participants interacted with the robot using both spoken and textual instructional commands. Based on the logged interaction data, our framework achieved 87.55% vocal commands decoding accuracy, 86.27% commands execution success, and an average latency of 0.89 seconds from receiving the participants' vocal chat commands to initiating the robot's actual physical action. Topological accuracy in medical image segmentation is a highly important property for downstream applications such as network analysis and flow modeling in vessels or cell counting. Recently, significant methodological advancements have brought well-founded concepts from algebraic topology to binary segmentation. However, these approaches have been underexplored in multi-class segmentation scenarios, where topological errors are common. We propose a general loss function for topologically faithful multi-class segmentation extending the recent Betti matching concept, which is based on induced matchings of persistence barcodes. We project the N-class segmentation problem to N single-class segmentation tasks, which allows us to use 1-parameter persistent homology, making training of neural networks computationally feasible. We validate our method on a comprehensive set of four medical datasets with highly variant topological characteristics. Our loss formulation significantly enhances topological correctness in cardiac, cell, artery-vein, and Circle of Willis segmentation.

We introduce a conditional implicit neural atlas (CINA) for spatio-temporal atlas generation from Magnetic Resonance Images (MRI) of the neurotypical and pathological fetal brain, that is fully independent of affine or non-rigid registration. During training, CINA learns a general representation of the fetal brain and encodes subject specific information into latent code. After training, CINA can construct a faithful atlas with tissue probability maps of the fetal brain for any gestational age (GA) and anatomical variation covered within the training domain. Thus, CINA is competent to represent both, neurotypical and pathological brains. Furthermore, a trained CINA model can be fit to brain MRI of unseen subjects via test-time optimization of the latent code. CINA can then produce probabilistic tissue maps tailored to a particular subject. We evaluate our method on a total of 198 T2 weighted MRI of normal and abnormal fetal brains from the dHCP and FeTA datasets. We demonstrate CINA's capability to represent a fetal brain atlas that can be flexibly conditioned on GA and on anatomical variations like ventricular volume or degree of cortical folding, making it a suitable tool for modeling both neurotypical and pathological brains. We quantify the fidelity of our atlas by means of tissue segmentation and age prediction and compare it to an established baseline. CINA demonstrates superior accuracy for neurotypical brains and pathological brains with ventriculomegaly. Diffusion models have advanced unsupervised anomaly detection by improving the transformation of pathological images into pseudo-healthy equivalents. Nonetheless, standard approaches may compromise critical information during pathology removal, leading to restorations that do not align with unaffected regions in the original scans. Such discrepancies can inadvertently increase false positive rates and reduce specificity, complicating radiological evaluations. This paper introduces Temporal Harmonization for Optimal Restoration (THOR), which refines the de-noising process by integrating implicit guidance through temporal anomaly maps. THOR aims to preserve the integrity of healthy tissue in areas unaffected by pathology. Diseases are currently managed by grading systems, where patients are stratified by grading systems into stages that indicate patient risk and guide clinical management. However, these broad categories typically lack prognostic value, and proposals for new biomarkers are currently limited to anecdotal observations. In this work, we introduce a deep-learning-based biomarker proposal system for the purpose of accelerating biomarker discovery in age-related macular degeneration (AMD). It works by first training a neural network using self-supervised contrastive learning to discover, without any clinical annotations, features relating to both known and unknown AMD biomarkers present in 46,496 retinal optical coherence tomography (OCT) images. To interpret the discovered biomarkers, we partition the images into 30 subsets, termed clusters, that contain similar features. We then conduct two parallel 1.5-hour semi-structured interviews with two independent teams of retinal specialists that describe each cluster in clinical language. Overall, both teams independently identified clearly distinct characteristics in 27 of 30 clusters, of which 23 were related to AMD. Seven were recognised as known biomarkers already used in established grading systems and 16 depicted biomarker combinations or subtypes that are either not yet used in grading systems, were only recently proposed, or were unknown. Clusters separated incomplete from complete retinal atrophy, intraretinal from subretinal fluid and thick from thin choroids, and in simulation outperformed clinically-used grading systems in prognostic value. Overall, contrastive learning enabled the automatic proposal of AMD biomarkers that go beyond the set used by clinically established grading systems. Ultimately, we envision that equipping clinicians with discovery-oriented deep-learning tools can accelerate discovery of novel prognostic biomarkers.

Image reconstruction attacks on machine learning models pose a significant risk to privacy by potentially leaking sensitive information. Although defending against such attacks using differential privacy (DP) has proven effective, determining appropriate DP parameters remains challenging. Current formal guarantees on data reconstruction success suffer from overly theoretical assumptions regarding adversary knowledge about the target data, particularly in the image domain. In this work, we empirically investigate this discrepancy and find that the practicality of these assumptions strongly depends on the domain shift between the data prior and the reconstruction target. We propose a reconstruction attack based on diffusion models (DMs) that assumes adversary access to real-world image priors and assess its implications on privacy leakage under DP-SGD. We show that (1) real-world data priors significantly influence reconstruction success, (2) current reconstruction bounds do not model the risk posed by data priors well, and (3) DMs can serve as effective auditing tools for visualizing privacy leakage.

Analyzing temporal developments is crucial for the accurate prognosis of many medical conditions. Temporal changes that occur over short time scales are key to assessing the health of physiological functions, such as the cardiac cycle. Moreover, tracking longer term developments that occur over months or years in evolving processes, such as age-related macular degeneration (AMD), is essential for accurate prognosis. Despite the importance of both short and long term analysis to clinical decision making, they remain understudied in medical deep learning. State of the art methods for spatiotemporal representation learning, developed for short natural videos, prioritize the detection of temporal constants rather than temporal developments. Moreover, they do not account for varying time intervals between acquisitions, which are essential for contextualizing observed changes. To address these issues, we propose two approaches. First, we combine clip-level contrastive learning with a novel temporal embedding to adapt to irregular time series. Second, we propose masking and predicting latent frame representations of the temporal sequence. Our two approaches outperform all prior methods on temporally-dependent tasks including cardiac output estimation and three prognostic AMD tasks. Overall, this enables the automated analysis of temporal patterns which are typically overlooked in applications of deep learning to medicine.

Direct image-to-graph transformation is a challenging task that solves object detection and relationship prediction in a single model. Due to the complexity of this task, large training datasets are rare in many domains, which makes the training of large networks challenging. This data sparsity necessitates the establishment of pre-training strategies akin to the state-of-the-art in computer vision. In this work, we introduce a set of methods enabling cross-domain and cross-dimension transfer learning for image-to-graph transformers. We propose (1) a regularized edge sampling loss for sampling the optimal number of object relationships (edges) across domains, (2) a domain adaptation framework for image-to-graph transformers that aligns features from different domains, and (3) a simple projection function that allows us to pretrain 3D transformers on 2D input data. We demonstrate our method's utility in cross-domain and cross-dimension experiments, where we pretrain our models on 2D satellite images before applying them to vastly different target domains in 2D and 3D. Our method consistently outperforms a series of baselines on challenging benchmarks, such as retinal or whole-brain vessel graph extraction.

Biophysical modeling, particularly involving partial differential equations (PDEs), offers significant potential for tailoring disease treatment protocols to individual patients. However, the inverse problem-solving aspect of these models presents a substantial challenge, either due to the high computational requirements of model-based approaches or the limited robustness of deep learning (DL) methods. We propose a novel framework that leverages the unique strengths of both approaches in a synergistic manner. Our method incorporates a DL ensemble for initial parameter estimation, facilitating efficient downstream evolutionary sampling initialized with this DL-based prior. We showcase the effectiveness of integrating a rapid deep-learning algorithm with a high-precision evolution strategy in estimating brain tumor cell concentrations from magnetic resonance images. The DL-Prior plays a pivotal role, significantly constraining the effective sampling-parameter space. This reduction results in a fivefold convergence acceleration and a Dice-score of 95%.
