1. Prevalence of prolonged QTcinterval in electrocardiograms of 1 -12 year-old seizure patients

Abstract

Background: Children with long QT intervals are prone to life-threatening ventricular arrhythmias which may lead to seizure and syncope and may be misdiagnosed as seizure. Objective: This study aimed to assess the frequency of long QT intervals in children with and without convulsion. Method: This study is case-control. ECG tracings were requested for all children aged between one to twelve years who were hospitalized due to convulsion with no underlying etiology and simultaneously for children of the same age and gender who were admitted due to other thanseizure as case group. Consequently, QT intervals were measured and compared in the two groups. Results: If long QT interval wasdefined to be longer than 0.47 second, no significant difference was noted between two groups. On the other hand, if this interval was defined to be equal to or longer than 0.46 second, long QT intervals are more frequent in convulsive children. Conclusions: In this study, long QT interval, defined as QT interval ³ 0.46 second, is found more frequently in children with seizure than non-convulsing ones. It is recommended that children with history of seizure without any identifiable causes and that is unresponsive to anticonvulsive drugs should be investigated with ECG.

2. Personalized Prediction Models for Changes in Knee Pain among Patients with Osteoarthritis Participating in Supervised Exercise and Education

Abstract

Knee osteoarthritis (OA) is a widespread chronic condition that impairs mobility and diminishes quality of life. Despite the proven benefits of exercise therapy and patient education in managing the OA symptoms pain and functional limitations, these strategies are often underutilized. Personalized outcome prediction models can help motivate and engage patients, but the accuracy of existing models in predicting changes in knee pain remains insufficiently examined. To validate existing models and introduce a concise personalized model predicting changes in knee pain before to after participating in a supervised education and exercise therapy program (GLA:D) for knee OA patients. Our models use self-reported patient information and functional measures. To refine the number of variables, we evaluated the variable importance and applied clinical reasoning. We trained random forest regression models and compared the rate of true predictions of our models with those utilizing average values. We evaluated the performance of a full, continuous, and concise model including all 34, all 11 continuous, and the six most predictive variables respectively. All three models performed similarly and were comparable to the existing model, with R-squares of 0.31-0.32 and RMSEs of 18.65-18.85 - despite our increased sample size. Allowing a deviation of 15 VAS points from the true change in pain, our concise model and utilizing the average values estimated the change in pain at 58% and 51% correctly, respectively. Our supplementary analysis led to similar outcomes. Our concise personalized prediction model more accurately predicts changes in knee pain following the GLA:D program compared to average pain improvement values. Neither the increase in sample size nor the inclusion of additional variables improved previous models. To improve predictions, new variables beyond those in the GLA:D are required.

The present review was carried out to determine the relationship between the concentrations of these elements and autism by meta-analysis.Methods: In this study, searching Scopus, PubMed, and Science Direct databases, 18 articles conducted in different countries from 1982 to 2019 were collected. Studies’ heterogeneity was investigated using the I2 index. The data were analyzed using R and STATA software.Results: In these 18 studies, 1797 patients (981 cases and 816 controls) aged 2 to 16 years were examined. Concentration of the samples (blood, hair, and nails) for both case and control groups was evaluated. There was no significant relationship between copper concentration and autism (SMD (95% CI): 0.02 (− 1.16,1.20); I2=97.7%; P=0.972); there was a significant relationship between mercury concentration and autism (SMD (95% CI): 1.96 (0.56,3.35); I2=98.6%; P=0.006); there was also a significant relationship between lead concentration and autism (SMD (95% CI): 2.81 (1.64,3.98); I2=97.8%; P=0.000).Conclusion: There is, nevertheless, a significant relationship between mercury concentration and autism. Thus, the concentration of mercury can be listed as a pathogenic cause (disease-causing) for autism.Keywords: autism, copper, lead, mercury, meta-analysis

In thalassemia patients, bone diseases are one of the causes of mortality. Our goal was to investigate the association between vitamin D deficiency and increased iron uptake by cardiac myocytes and hepatocytes.Materials and Methods: Forty patients with thalassemia major were studied in Amir Kabir Hospital, Arak, Iran. The information obtained through clinical examination. Serum ferritin level was determined by ELISA and T2*MRI performed for measuring iron content in the heart and the liver.Results: The average age of the patients was 23.8 ± 10.7 years. The mean T2*MRI values were 23.7 ± 7. The vitamin D3 level in 33 patients (82.5% cases) was less than 20 ng/dl, 2 patients (5%) in the range of 20– 30 ng/dl, and the others had above 30 ng/dl. Correlation between vitamin D and age was 0.611. Correlation coefficient between heart and liver T2*MRI with ferritin level in patients was 0.437 and 0.335, respectively.Conclusion: Due to significant associations, the periodic measurement of vitamin D, as well as PTH, is recommended for patients with thalassemia major.Keywords: thalassemia major, T2*MRI, cardiac and liver iron level, vitamin D3

5. Compare of the effect of doxepin and citerizine in treatment of allergic rhinitis

Abstract

Abstract Background: Allergic rhinitis is a common disease which is present in 20% of general population. Major symptoms include sneezing, rhinorrhea, nasal congestion, and nasal pruritus. It seems that tricyclic antidepressants blocking histamine receptors may be used as an effective treatment in allergic rhinitis. Materials and Methods: In this clinical trial study, 84 patients with allergic rhinitis entered the study and randomly divided into two groups. Patients of 1st group received cetirizine and 2nd group received doxepin for 2 weeks. After 2 weeks patients were evaluated about sneezing, rhinorrhea, nasal congestion, and nasal pruritus. Results: The results of our study showed that clinical score of patients after 2 weeks was not different in both groups (p=0.261). The only symptom that was affected by the kind of treatment and was significantly different between groups was sneezing (p=0.005). Conclusion: It seems that there is not much difference in using cetirizine and doxepin in treatment of seasonal allergic rhinitis sympotoms. It is recommended that future studies use TCAs with more potent blocking anti-histamine receptors feature and on more samples.

Abstract

Background: To evaluate the ability of Ankle/Brachial Index (ABI), that is a sensitive and specific test for detection of PAD, to foretell the possibility of ischemic heart diseases in diabetic patients. Materials and Methods: All of diabetic patients who visited in our diabetes research center between May 2000 and May 2001 and were confirmed diabetics since 2 or more years ago were enrolled in the study. The ABI was calculated for all of the patients and their demographic specifications and ischemic heart disease risk factors were recorded. All of the patients refer to perform an exercise test, but 279 patients were conferred to performing stress test. They were containing: 127 patients with ABI ==1.4(group III) as case groups and, 105 patients with 1.1 60 years (P = 0.02). 238 patients were in group I (25.1 %), 625 patients in group II (65.9 %) and 85 patients in group III (9 %). 52.8% of 125 patients in Group I, 19.6% of 107 patients in group II, and 44.7% of 47 patients in Group III had positive exercise test. I, II (P  0.00)- II , III (P = 0.05) A total of 108 patients from among the 279 patients had positive exercise test (38%), and most of them were from groups I &III. 72.2% of group I , and 52.5% of group II had high blood pressure (P  0.00). Both hypertensive and normotensive patients with ABI ==1.4 had positive exercise test significantly more than patients with 1.1=1.4 is an independent predictor of coronary artery diseases in diabetic patients especially in those who are hypertensive.

Abstract

Background: All the patients with Angina pectoris have not stenotic coronary artery. Syndrome x consists of patients with Angina pectoris and positive Exercise test along with normal coronary angiography. Syndrome x includes 25-30 percent of those who undergo diagnostic coronary arteriography at the different centers. In our study it was attempted to know the prevalence and characteristics of this special group in Afshar cath-lab. Materials and Methods: In a prospective cross sectional descriptive study 200 patients who were admitted for coronary angiography evaluated in terms of Risk factors: HTN, Family history Hyperlipidemia, DM, Cigarette smoking , clinical symptoms , response to TNG, different symptoms, ECG and Exercise test. data in two subgroups with normal and abnormal Coronary angioghraphy were compared. Six patients were excluded because of ambiguous Coronary angiography. Results: Among 194 patients 51 patients (26.3% )had normal coronary artery. 38.9% of female and 21.4% of males showed normal coronary angioghraphy (P = 0.013). The prevalence of risk factors among those with syndrome x and abnormal coronary Artery were as follow. HTN 37% vs 42% , Hyperlipidemia 50% vs 51% , smoking 25% vs 37% , DM 12% vs 28% , obesity 45% vs 47% Positive family history for CAD = 21.5% vs 47% respectively. Angina relieved suitably with TNG in 24% of those with syndrome x compared to 76% suitable response in those with abnormal coronary arteries.(P Value = 0.49 ). Regarding to functional class , 87% of patients with normal coronary artery belonged to class I and II. also 26% had atypical pain 50% had normal rest ECG and 51% achieved stage III or IV of Bruce protocol during Exercise test. Conclusion: Syndrome x was more prevalent among women ,also angina was less typical and less responsive to TNG. Most patients had not positive familial history for CAD. normal ECG was more prevalent among those with syndrome x .

Abstract

This study was carried out on 2200 men and women aged 20-70 years in isfahan who participatd in isfahan Cardiovacular Risk Facor survery, in order to find prevalence of cigarette smoking in this population and estimate the relation of smoking with other CAD risk factors. All information was obtained by standard questionnaires mainly concerning smoking habits of subjects and their personal characteristics, etc. Serum lipids and fasting blood glucose levels were measured from 12-14 hr fasting specimen by ELAN 2000 autoanalyzer. Blood pressure was measured according to WHO standardized method. The prevalence of cigarette smoking among Isfanhan population is 11.1% (0.92 in women, 23.3%) in men. 11. Asymptomatic Giardiasis in school children in Rey city

Abstract

This cross-sectional descriptive study was undertaken to know the rate of intestinal parasites infestations in our school children population. A sum of 1155 fecal samples were analyzed from an equal number of children whose age were between 6-11 years old. The percentage of infestations were: Giardia lamblia (14.11%), hymenolepis nana (1.21%), ascaris lumbricoides (0.08%) and enterobius vermicularis (0.08%). We found no differences with regard to sex, age, father and mother levels of education, family size, body mass index (BMI) and the erythrocyte sedimentation rate (ER) between the stools positive and the stools negative cases. Nearly all cases of giardiasis (99.4%) were symptomatic. Eosinophilia was seen in (5.5%) of the infested children with giardiasis and in (0.5%) of the non-infested children, the difference was statistically non-significant (P=0.056). All cases of giardiasis (N=163) were treated with metronidazole 15 mg/kg/day in 3 divided doses for 7 days. Metronidazole was effective in (92.2%) of the treated children.

Abstract

Since an initial case in 2006, we noted multiple patients undergoing heart transplantation (HTx) for Chagas cardiomyopathy (CC) at our transplant program. The clinical characteristics, laboratory results and outcomes of patients with CC undergoing HTx in the United States have not been reported previously. In 2010, we implemented a systematic screening and management program for patients undergoing HTx for CC. Before HTx, all patients with idiopathic dilated cardiomyopathy who were born in a Chagas disease endemic country were screened for Trypanosoma cruzi (TC) infection with serology. After HTx, monitoring for TC reactivation was performed using clinical visits, echocardiography, endomyocardial biopsy and serial whole blood polymerase chain reaction (PCR) testing. Between June 2006 and January 2012, 11 patients underwent HTx for CC. One patient was empirically treated due to the presence of TC amastigotes in explanted cardiac tissue. Two patients experienced allograft dysfunction due to TC reactivation and three patients experienced subclinical reactivation (positive PCR results), which were treated. Chagas disease is a common cause of dilated cardiomyopathy in patients from endemic countries undergoing HTx at a transplant program in the United States. Reactivation is common after transplantation and can cause adverse outcomes.

38. Frequency and Drug Resistance of Group B Streptococcus in Pregnant Women in Markazi Province, Iran

Abstract

Background and Objective: Group B streptococcus is one of the agents leading to infantile blood infection and meningitis as well as aggressive infections in pregnant women. The vast use of antibiotics in order to prevent from the diseases caused by this bacterium has led to some concerns about the emergence of drug resistance in Group B streptococcus. Thus, we aimed to investigate Group B streptococcus drug resistance patterns to aid appropriate drug prescriptions. Material and Methods: The study was conducted on 268 pregnant women. Having been transferred to Todd Hewitt broth, the samples were cultured on blood agar and the identity of Group B streptococcus was confirmed through biochemical tests (gram stain tests, the catalase test, CAMP, and sodium hippurate hydrolysis). Results: Of 268 samples, 14 (5.2 percent) showed positive Group B streptococcus culture. All cases (100 percent) were resistant to penicillin, eight (57.15 percent) to erythromycin, 13 (92.2 percent) to clindamycin, three (7.1 percent) to Cefazolin, seven (50 percent) Ceftizoxime and four (28.57 percent) were resistant to Cefotaxime. Three (21.4 percent) were semi-sensitive toward Cefazolin. No resistance was observed toward ampicillin or Vancomycin. Abstract

Background— It is commonly assumed that cardiovascular disease risk factors are associated with affluence and Westernization. We investigated the associations of body mass index (BMI), fasting plasma glucose, systolic blood pressure, and serum total cholesterol with national income, Western diet, and, for BMI, urbanization in 1980 and 2008. Methods and Results— Country-level risk factor estimates for 199 countries between 1980 and 2008 were from a previous systematic analysis of population-based data. We analyzed the associations between risk factors and per capita national income, a measure of Western diet, and, for BMI, the percentage of the population living in urban areas. In 1980, there was a positive association between national income and population mean BMI, systolic blood pressure, and total cholesterol. By 2008, the slope of the association between national income and systolic blood pressure became negative for women and zero for men. Total cholesterol was associated with national income and Western diet in both 1980 and 2008. In 1980, BMI rose with national income and then flattened at ≈Int$7000; by 2008, the relationship resembled an inverted U for women, peaking at middle-income levels. BMI had a positive relationship with the percentage of urban population in both 1980 and 2008. Fasting plasma glucose had weaker associations with these country macro characteristics, but it was positively associated with BMI. Conclusions— The changing associations of metabolic risk factors with macroeconomic variables indicate that there will be a global pandemic of hyperglycemia and diabetes mellitus, together with high blood pressure in low-income countries, unless effective lifestyle and pharmacological interventions are implemented.

Abstract

The AISI 430 stainless steel with ferritic structure is a low cost material for replacing austenitic stainless steel because of its higher yield strength, higher ductility and also better polarisation resistance in harsh environments. The applications of AISI 430 stainless steel are limited due to insignificant ductility and some undesirable changes of magnetic properties of its weld area with different microstructures. In this research, a study has been done to explore the effects of parameters of laser welding process, namely, welding speed, laser lamping current, and pulse duration, on the coercivity of laser welded AISI 430 stainless steel. Vibrating sample magnetometery has been used used to measure the values of magnetic properties. Observation of microstructural changes and also texture analysis were implemented in order to elucidate the change mechanism of magnetic properties in the welded sections. The results indicated that the laser welded samples undergo a considerable change in magnetic properties. These changes were attributed to the significant grain growth which these grains are ideally oriented in the easiest direction of magnetization and also formation of some non-magnetic phases. The main effects of the above-mentioned factors and the interaction effects with other factors were evaluated quantitatively. The analysis considered the effect of lamping current (175-200 A), pulse duration (10-20 ms) and travel speed (2-10 mm/min) on the coercivity of laser welded samples. [ABSTRACT FROM AUTHOR]

Abstract

The aim of this study is to investigate the problems caused by discharge of polluted air from tunnels into the environment with a specific focus on residential areas. In city tunnels, portal or stacks, pollutant management is a big challenge. Nowadays, air quality management, particularly in urban tunnels, is considered as a part of the ventilation system design. The goal is to see the environmental impacts beforehand. From environmental aspects, preventive measures are required either inside or outside the tunnel in some cases. Niayesh tunnel in Tehran is taken as a case for proving the objectives presented in this study. Concentration of carbon monoxide at the vicinity of the portals is calculated using the proper dispersion simulation. The results of dispersion modeling for the assumed worst case of ventilation can help to understand the environmental impact of ventilation. The worst traffic emissions for a congested traffic scenario, are selected as an emission source for dispersion modeling. According to the traffic condition and fleet composition, the crucial emission extracted from the tunnel is carbon monoxide. Therefore, the performed simulation only focuses on carbon monoxide dispersion modeling. From the other side, carbon monoxide is taken as a demonstration pollutant, because it is inert and chemical reactions can be neglected in short-term considerations. Abstract

Background: Increased intra-abdominal pressure (IAP) results in dysfunction of vital organs. The aim of the present study was to evaluate the effect of mechanical ventilation mode on IAP. Methods: In a cohort study, a total of 60 patients aged 20-70 years who were admitted to the ICU and underwent mechanical ventilation were recruited. Mechanical ventilation included one of the three modes: Biphasic positive airway pressure (BIPAP) group, synchronize intermittent mandatory ventilation (SIMV) group, or continuous positive airway pressure (CPAP) group. For each patient, mechanical ventilation mode and its parameters, blood pressure, SpO2, and status of tube feeding and IAP were recorded. Results: Our findings indicate that the study groups were not significantly different in terms of anthropometric characteristics including age (64.5 ± 4, P = 0.1), gender (male/female 31/29, P = 0.63), and body mass index (24 ± 1.2, P = 0.11). Increase IAP was related to the type of respiratory mode with the more increased IAP observed in SIMV mode, followed by BIPAP and CPAP modes (P = 0.01). There were significant correlations between increased IAP and respiratory variables including respiratory rate, pressure support ventilation, and inspiratory pressure (P < 0.05). Tube feeding tolerance through NG-tube was lower in SIMV group, followed by BIPAP and CPAP groups (P < 0.05). Conclusions: There is a significant relationship between respiratory modes and IAP; therefore, it is better to utilize those types of mechanical ventilation like CPAP and BIPAP mode in patients who are prone to Intra-abdominal hypertension.

Abstract

Background: To assess the epidemiological characteristics of hypertensive patients in urban population of Yazd, A cen­tral city in Iran. Methods: This cross sectional study was conducted from 2005-2006 and carried out on population aging 20-74 years. It is a part of the phase I of Yazd healthy heart program that it is a community interventional study for prevention of car­diovascular disease. Data obtained from questionnaires were analyzed by SPSS version 13. P value less than 0.05 were consid­ered significant level. Results: This study comprised of 2000 participants that 847 (42.5%) were diagnosed as being hypertensive. After age ad­justment, prevalence of hypertension was 25.6% (23.3% for women and 27.5% for men (P< 0.001). Age, Total cho­lesterol, LDL-cholesterol, triglyceride, fasting blood glucose, impaired glucose tolerance test, body mass index and waist were significantly higher in the hypertensive groups. 53.7% of hypertensive cases were aware of own condition, 45% were treated, and 33.9% of treated were controlled (30.7% and 35.4% in men and women re­spectively). In other word, 24% of all hypertensives (aware or unaware about own blood pressure condition) were treated and only 8% of them were controlled. Men significantly had less awareness (P< 0.001), lower tendency to take medi­cation (P< 0.001), and less were controlled (P= 0.046). Conclusion: We understand high prevalence, low awareness, treatment, and control of hypertension and higher preva­lence of other traditional metabolic risk factors in these cases. It seems that urgent preventional studies should be con­ducted in this population.

Abstract

With 40 million people living with HIV worldwide the HIV/ AIDS epidemic threatens every aspect of global economic development. In 2005 close to 5 million people were newly infected with HIV and 3 million died of an AIDS-related illness. HIV/AIDS is among the top 10 causes of death in developing countries and the leading cause of death in Sub-Saharan Africa. Although Sub-Saharan Africa is the hardest-hit region prevalence of HIV is spreading into parts of Asia and Eastern Europe with alarming speed. HIV is transmitted primarily through three mechanisms: sexual intercourse (accounting for about 80 percent of infections worldwide; exposure to infected blood or blood products; and transmission by HIV-infected mothers to their newborns. While the international community recognizes the urgency and importance of stopping the AIDS epidemic resources have fallen short of needs. Although known interventions could prevent nearly two-thirds of new infections projected to occur between 2002 and 2010 fewer than one in five people at high risk of infection have access to the most basic prevention services. Controlling the epidemic requires political commitment and leadership at the highest levels not only to prevent the transmission of HIV but also to treat and care for people who develop AIDS. Item does not contain fulltext, BACKGROUND: Multiple sclerosis (MS) is a chronic inflammatory demyelinating disorder of the central nervous system. Evidences linking apolipoprotein E (APOE) to myelin repair, neuronal plasticity, and cerebral inflammatory processes suggest that it may be relevant in MS. The main goal of this study was to determine whether the APOE genotypes and alleles are associated with MS patients. MATERIALS AND METHODS: In total, 147 MS cases and 168 control subjects from Iranian population were genotyped for APOE gene using PCR-RFLP method. RESULTS: The frequency of APOE-epsilon2epsilon3 genotype was significantly higher in controls than cases (14.3% vs. 6.1%, P=0.009, OR=0.39) whereas APOE-epsilon3epsilon4 genotype frequency was significantly higher in cases compared with controls (8.2% vs. 3.6%, P=0.03, OR=2.4). APOE-epsilon2 allele frequency in cases was significantly lower than that of controls (4.4% vs. 8.0%, P=0.03, OR=0.52). Also male controls were significantly more likely to have APOE-epsilon2 allele (7.8% vs. 1%, P=0.01, OR=0.11). APOE-epsilon4 allele frequency in cases was significantly higher than control group (4.8% versus 2.1%, P=0.03, OR=2.35). CONCLUSION: It seems that individuals carrying APOE-epsilon4 allele and/or APOE-epsilon3epsilon4 genotype develop MS two times more than non-carriers. Also APOE-epsilon2epsilon3 genotype or APOE-epsilon2 allele may have a protective role against MS development in Iranian population. Further investigation would be warranted to understand the role of APOE alleles and genotypes and risk of MS.
