MRI-detected inflammation around the extensor tendons of metacarpophalangeal (MCP-) joints is prevalent in RA and poses a markedly increased risk of RA development when present in arthralgia patients. Such inflammation is called ‘peritendinitis’ since anatomy literature reports no presence of a tenosynovial sheath at these tendons. However, the presence or absence of tenosynovium at these extensor tendons has never been studied. Therefore, an anatomical and histological study of extensor tendons at the MCP-joints of three embalmed human hands was performed. Immunohistochemical staining showed the presence of markers for synovial macrophages and fibroblast-like synoviocytes bordering a natural dorsal space next to the extensor tendon, suggesting the presence of a synovial lining. This implies that contrast-enhancement on MRI around extensor tendons at MCP-joints observed in early RA and pre-RA likely represents tenosynovitis and that inflammation of this synovial tissue is an early feature of RA.

Crystal arthropathies are a group of joint disorders due to deposition of crystals in and around joints, which lead to joint destruction and soft tissue masses. Clinical presentation is variable and diagnosis might be challenging. Therefore, first-line imaging of ultrasound in accessible joints is recommended to assess crystal arthropathies; however, DECT is a useful adjunct to differentiate gout from CPPD.

BackgroundIn 2009, ASAS published a ‘Definition of active sacroiliitis on MRI for classification of axial spondyloarthritis (axSpA)’. This definition relied on two MRI sequences to make this determination – semicoronal T1 and STIR. Since then, this approach has frequently been used for diagnosis, even though that was never the intent of the definition. In 2015, the European Society of Skeletal Radiology (ESSR) published its recommendations for an SIJ MRI image acquisition protocol (IAP) for diagnostic purposes that required 4 MRI sequences but there is still no IAP that has been widely accepted as a minimum standard worldwide. In 2020, an informal survey of 24 academic sites (12 Europe, 12 North America) confirmed that 24/24 sites performed a minimum of 3 MRI sequences for diagnosis (19 performed 4-8 sequences) because the 2-sequence protocol was considered inadequate.ObjectivesTo develop the minimum requirements for a standardized IAP for MRI of the sacroiliac joints for diagnostic ascertainment of sacroiliitis.MethodsAll radiologist members of the ASAS and SPARTAN Classification in axSpA (CLASSIC) project, along with one European and one North American rheumatologist with extensive MRI experience in SpA clinical practice and research, were invited to participate in a consensus exercise. A draft IAP was circulated to all participants along with background information and justification for the draft proposal. Feedback on all issues was received by email, tabulated and recirculated. Participants were broadly in favour of the proposal and two months later a teleconference meeting took place and remaining points of contention were resolved. Examples of the proposed IAP performed on new, 10 and 22 years’ old MRI scanners were made available for review in DICOM format. Next the revised draft of the IAP was presented at the ASAS annual meeting to the entire membership on 14 January 2022, and voted on.ResultsA 4-sequence IAP, 3-semicoronal and 1-semiaxial, is recommended for diagnostic ascertainment of sacroiliitis and its differential diagnoses (Table 1). It must meet the following requirements: Semicoronal sequences should be parallel to the dorsal cortex of the S2 vertebral body, and include: 1) a sequence sensitive for the detection of active inflammation being T2-weighted with suppression of fat signal; 2) a sequence sensitive for the detection of structural damage in bone and bone marrow with T1-weighting; 3) a sequence that is designed to optimally depict the bone-cartilage interface of the articular surface and be sensitive for detection of bone erosion; plus 4) a semiaxial sequence sensitive for inflammation detection. Background: MRI-detected subclinical joint inflammation in the hand joints of patients with undifferentiated arthritis (UA) predicts progression to rheumatoid arthritis (RA). It is unknown if adding MRI of the foot increases predictive accuracy compared to the hand alone. Methods: 1.5-T contrast-enhanced MRI of the unilateral foot (MTP-1-5) and hand (MCP-2-5 and wrist) was performed in 123 patients presenting with UA (not fulfilling the 2010 RA criteria) and scored for bone marrow edema (BME), synovitis and tenosynovitis. Symptom-free controls (n = 193) served as a reference for defining an abnormal MRI. Patients were followed for RA development ≤ 1 year, defined as fulfilling the classification criteria or initiation of disease-modifying antirheumatic drugs because of the expert opinion of RA. The added predictive value of foot MRI to hand MRI was evaluated. Results: Fifty-two percent developed RA. Foot tenosynovitis was predictive (OR 2.55, 95% CI 1.01–6.43), independent of BME and synovitis (OR 3.29, 95% CI 1.03–10.53), but not independent of CRP and number of swollen joints (OR 2.14, 95% CI 0.77–5.95). Hand tenosynovitis was also predictive independent of BME and synovitis (OR 3.99, 95% CI 1.64–9.69) and independent of CRP and swollen joints (OR 2.36, 95% CI 1.04–5.38). Adding foot tenosynovitis to hand tenosynovitis changed the sensitivity from 72 to 73%, specificity from 59 to 54% and AUC from 0.66 to 0.64; the net reclassification index was − 3.5. Conclusion: MRI-detected tenosynovitis of the foot predicts progression to RA. However, adding MRI of the foot does not improve the predictive accuracy compared to MRI of the hand alone. In view of cost reduction, the performance of foot MRI for prognostic purposes in UA can be omitted.

ObjectiveTo determine associations between magnetic resonance imaging (MRI) lesions originating from either axial spondyloarthritis (SpA) or from degeneration and pain in patients with chronic back pain of 35 years (OR 5.19; P=0.001), high-intensity zone lesions in females not fulfilling ASAS criteria (OR 5.09; P=0.001), and herniation in various subgroups (OR range 2.07-4.66) were associated with pain. ConclusionSpecific degenerative lesions, but not typical axial SpA lesions, of the spine are associated with pain at the same location in some subgroups. Objectives The purpose was to determine prevalence of enchondromas and atypical cartilaginous tumour/chondrosarcoma grade 1 (ACT/CS1) of the knee on MRI in a large cohort study, namely the Netherlands Epidemiology of Obesity (NEO) study. Methods Participants aged 45 to 65 years were prospectively included, oversampling overweight and obese persons. Within a subgroup of participants, MRI of the right knee was performed and screened for incidental cartilaginous tumours, as defined by their characteristic location and appearance. Results Forty-nine cartilaginous tumours were observed in 44 out of 1285 participants (estimated population prevalence 2.8 %, 95 % CI 2.0–4.0 %). Mean largest tumour diameter was 12 mm (range 2–31 mm). Eight participants with a tumour larger than 20 mm or a tumour with aggressive features were referred to rule out low-grade chondrosarcoma. One was lost to follow-up, three had histologically proven ACT/CS1 and four had dynamic contrast MRI findings consistent with benign enchondroma. Conclusions Incidental cartilaginous tumours were relatively common on knee MRI and may be regarded as a normal concurrent finding. However, more tumours than expected were ACT/CS1. Because further examination was performed only when suspicion of chondrosarcoma was high, the actual prevalence might be even higher. Key Points • Incidental cartilaginous tumours are relatively common on knee MRI. • Most incidental cartilaginous tumours are small and lack suspicious features. • Small cartilaginous tumours without suspicious findings may be a normal concurrent finding. • Large tumours and/or those with suspicious findings should be further investigated. • Atypical cartilaginous tumour/chondrosarcoma grade 1 was found more often than expected.

Contains fulltext : 137063.pdf (Publisher’s version ) (Open Access) OBJECTIVE: To investigate if the amount of fat mass (FM) or skeletal muscle mass (SMM) is more strongly associated with knee osteoarthritis (OA), in both men and women. METHODS: The Netherlands Epidemiology of Obesity (NEO) study is a population-based cohort aged 45-65 years, including 5313 participants (53% female, median body mass index (BMI) 29.9 kg/m(2)). FM (kg), fat percentage, SMM (kg) and skeletal muscle (SM) percentage were estimated using bioelectrical impedance analysis (BIA). Clinical OA was defined following the ACR criteria. Structural OA was defined based on magnetic resonance imaging (MRI) in 1142 participants. Logistic regression analyses were used to examine the associations of all body composition measures with clinical and structural knee OA per standard deviation (SD), stratified by sex and adjusted for age and height. RESULTS: Clinical or structural OA was present in 25% and 14% of women and 12% and 13% of men, respectively. FM and fat percentage were positively associated with clinical knee OA in men and women. SMM was positively associated, while the SM percentage was negatively associated with clinical OA in both men and women. The FM/SMM ratio was positively associated with clinical OA. All determinants showed even stronger ORs for structural knee OA. In men, SMM was more strongly associated with knee OA as compared to FM whereas in women, FM was most strongly associated. CONCLUSION: Especially a high FM/SMM ratio seems to be unfavorable in knee OA. In men, SMM is most strongly associated with knee OA whereas in women FM seems to be of most importance.

Several genes involved in the familial appearance of thoracic aortic aneurysms and dissections (FTAAD) have been characterized recently, one of which is SMAD3. Mutations of SMAD3 cause a new syndromic form of aortic aneurysms and dissections associated with skeletal abnormalities. We discovered a small interstitial deletion of chromosome 15, leading to disruption of SMAD3, in a boy with mild mental retardation, behavioral problems and revealed features of the aneurysms-osteoarthritis syndrome (AOS). Several family members carried the same deletion and showed features including aortic aneurysms and a dissection. This finding demonstrates that haploinsufficiency of SMAD3 leads to development of both thoracic aortic aneurysms and dissections, and the skeletal abnormalities that form part of the aneurysms-osteoarthritis syndrome. Interestingly, the identification of this familial deletion is an example of an unanticipated result of a genomic microarray and led to the discovery of important but unrelated serious aortic disease in the proband and family members.

This thesis describes a multifocal approach on wrist related injuries following trauma, addressing its complexity in diagnosis and treatment strategies.

Ankle and distal radius fractures are two of the most common musculoskeletal injuries. Over the last decades their incidences have risen due to increasing participation in athletic activities and ageing of the population. Current national and international protocols recommend frequent outpatient clinic visits in which radiographs of the fractured extremity are obtained. The general aim of this thesis was to evaluate the added value of routine radiography in the follow-up of ankle and distal radius fractures. Specifically, we were interested in investigating whether a follow-up protocol which focuses on reducing the number of routine follow-up radiographs was able to deliver care that was comparable to the current standard of care, but without sacrificing quality nor safety, whilst increasing cost-effectiveness. Chapter 1 provides background information on the epidemiology of ankle and distal radius fractures. It also outlines the current standard of care during treatment and follow-up of the affected patients. In chapter 2 an overview of the current literature on routine radiography in extremity fractures is given. Despite the common occurrence of extremity fractures, limited data were available on the added value of routine radiography at the time that our systematic review was performed in 2018. We identified eleven studies; however, due to theirretrospective design and thus incumbent biases, the resultant quality or certainty of the evidence was considered low. Despite this, the treatment plan was modified in a very small percentage of the cases (ranging from 0 to 2.6%); therefore, the added value of routine radiography seems limited.Following our review in 2018, we conducted a retrospective analysis in four level 1 trauma centres in the Netherlands in order to determine the impact of routine radiographs on treatment strategy for patients with ankle fractures. Chapter 3 illustrates that, in accordance with studies outlined in chapter 2, the use of routine radiographs in the follow-up of ankle fracture patients in the Netherlands was common. In total, 80% of radiographs obtained after more than three weeks of follow-up were considered routine,and only 1.2% of these radiographs resulted in a change of treatment strategy. However, due to its retrospective design, the strength of the evidence was also considered low given its limitations.Chapters 4 and 5 report on the results of a multi-centre randomized controlled trial (RCT) in which participants with an ankle fracture were randomized between the current standard of care consisting of routine follow-up radiography (routine care) and a reduced imaging follow-up regimen. The clinical and functional outcomes outlined in chapter 4 suggest that routine radiographs at week 6 and 12 can be omitted without compromising treatment outcomes. Specifically, functional outcome measured with the Olerud and Molander Ankle Score was non-inferior in the reduced imaging group, while secondary outcomes, such as American Association of Orthopedic Surgeons (AAOS) foot and ankle scores, Health-Related Quality of Life (HRQoL), pain, health perception and self-perceived recovery did not differ between groups. The median number of radiographs obtained was 4 in the reduced imaging group and 5 in the routine care group (a reduction of 20%). Similar numbers and types of complications were observed in both groups; therefore, modifying the current standard of care can be implemented without sacrificing quality nor safety. In chapter 5 the results of the economic evaluation of the reduced-imaging follow-up strategy for ankle fracture patients are presented. Patients randomized to reduced imaging had a similar HRQoL in comparison with patients randomized to routine care. Costs for radiographic imaging were significantly lower in the reduced imaging group (a difference of €48 (95% CI: €-72 to €-28)). Other costs, including overall costs did not statistically differ between the groups. The probability of cost-effectiveness was 0.45 at a willingness-to-pay of €20,000 per QALY. Chapters 6 and 7 report on the outcomes from the same RCT, but which focused on patients with a fracture of the distal radius. Similar to the results of the RCT on anklefractures, functional outcome (measured with the Disabilities of Arm, Shoulder and Hand questionnaire [DASH]) was no worse than the reduced imaging group (chapter 6). Secondary outcomes such as HRQoL, pain and complications demonstrated similar outcomes between the groups. The number of radiographs obtained per patient decreased 25% to median 3 in the reduced imaging group from median 4 in the routine care group. The results of the economic evaluation described in chapter 7 demonstrated similarities to the results of the study on ankle fractures. HRQoL was similar, and costs for radiographic imaging were significantly lower in the reduced imaging group (this reduction was €48 per patient (95% CI: -68 to -27)). The probability of cost-effectiveness was 0.8 to 0.9 at willingness-to-pay of €20,000 to €80,000 per QALY.Following these analyses, we investigated which factors could encourage or discourage physicians to modify their practice behaviours, namely reduce their reliance on routine follow-up radiographs of extremities. These so-called “barriers and facilitators” were queried among orthopaedic trauma surgeons in the Netherlands (chapter 8). In total, 130 respondents (57%) completed the questionnaire, 71% indicated that they would stop ordering routine radiographs if they demonstrated no added value. In short, we identified three facilitators which were found to be independent predictors for the intention to de-implement routine radiographs: 1) ‘the reduced imaging follow-up protocol will lead to lower healthcare costs’; 2) ‘incorporation of the reduced imaging follow-up in regional protocols’; and 3) ‘reduced imaging will result in time-savings for the patient’. There was no barrier that was found to be an independent predictor for the intention to reduce the reliance on routine radiographs. With the three facilitators in mind, a proper de-implementation strategy can be drafted for the Netherlands, and other populations similar as ours. In chapter 9, I present general conclusions and discuss the clinical implications and future perspectives regarding the effectiveness and cost-effectiveness of routine radiography in ankle and distal radius fractures. This large multi-center study demonstrates that the number of routine radiographs in those with ankle and distal radius fractures can be reduced without sacrificing quality nor safety, while resulting in more cost-effective care

Rheumatoid arthritis (RA) is a disease of the synovium that involves the small joints of hands and feet. The focus in RA-research, however, has primarily been on involvement of the hands. In this thesis we studied involvement of the forefoot. Throughout this thesis we found that synovitis in the rheumatoid foot affects more than just the joints. In an anatomic study we revealed that the tendon-sheaths in the forefoot also possess a synovial layer. Inflammation of this, detected by MRI that is referred to as tenosynovitis, was an important aspect of RA involvement in the forefoot. In addition, intermetatarsal bursae also detected by MRI are also often inflamed due to disease-involvement. These extra-articular involvements of the synovium that are found in RA importantly contribute to joint swelling and walking disability, and are therefore clinically relevant. Therefore we believe that it is essential for the clinician to pay attention to the forefeet of patients with RA to have a complete idea of disease activity
