Abstract

BACKGROUND: We evaluated dolutegravir pharmacokinetics in infants with human immunodeficiency virus (HIV) receiving dolutegravir twice daily (BID) with rifampicin-based tuberculosis (TB) treatment compared with once daily (OD) without rifampicin. METHODS: Infants with HIV aged 1-12 months, weighing ≥3 kg, and receiving dolutegravir BID with rifampicin or OD without rifampicin were eligible. Six blood samples were taken over 12 (BID) or 24 hours (OD). Dolutegravir pharmacokinetic parameters, HIV viral load (VL) data, and adverse events (AEs) were reported. RESULTS: Twenty-seven of 30 enrolled infants had evaluable pharmacokinetic curves. The median (interquartile range) age was 7.1 months (6.1-9.9), weight was 6.3 kg (5.6-7.2), 21 (78%) received rifampicin, and 11 (41%) were female. Geometric mean ratios comparing dolutegravir BID with rifampicin versus OD without rifampicin were area under curve (AUC)0-24h 0.91 (95% confidence interval, .59-1.42), Ctrough 0.95 (0.57-1.59), Cmax 0.87 (0.57-1.33). Abstract

BackgroundAlthough super-boosted lopinavir/ritonavir (LPV/r; ratio 4:4 instead of 4:1) is recommended for infants living with HIV and receiving concomitant rifampicin, in clinical practice, many different LPV/r dosing strategies are applied due to poor availability of pediatric separate ritonavir formulations needed to superboost. We evaluated LPV pharmacokinetics in infants with HIV receiving LPV/r dosed according to local guidelines in various sub-Saharan African countries with or without rifampicin-based tuberculosis (TB) treatment.MethodsThis was a 2-arm pharmacokinetic substudy nested within the EMPIRICAL trial (#NCT03915366). Infants aged 1-12 months recruited into the main study were administered LPV/r according to local guidelines and drug availability either with or without rifampicin-based TB treatment; during rifampicin cotreatment, they received double-dosed (ratio 8:2) or semisuperboosted LPV/r (adding a ritonavir 100 mg crushed tablet to the evening LPV/r dose). Six blood samples were taken over 12 hours after intake of LPV/r.ResultsIn total, 14/16 included infants had evaluable pharmacokinetic curves; 9/14 had rifampicin cotreatment (5 received double-dosed and 4 semisuperboosted LPV/r). Abstract

BackgroundPneumonia is the primary cause of death among HIV-infected children in Africa, with mortality rates as high as 35-40% in infants hospitalized with severe pneumonia. Bacterial pathogens and Pneumocystis jirovecii are well known causes of pneumonia-related death, but other important causes such as cytomegalovirus (CMV) and tuberculosis (TB) remain under-recognized and undertreated. The immune response elicited by CMV may be associated with the risk of developing TB and TB disease progression, and CMV may accelerate disease caused both by HIV and TB. Minimally invasive autopsies confirm that CMV and TB are unrecognized causes of death in children with HIV. CMV and TB may also co-infect the same child. The aim of this study is to compare the impact on 15-day and 1-year mortality of empirical treatment against TB and CMV plus standard of care (SoC) versus SoC in HIV-infected infants with severe pneumonia.MethodsThis is a Phase II-III, open-label randomized factorial (2 × 2) clinical trial, conducted in six African countries. The trial has four arms. Infants from 28 to 365 days of age HIV-infected and hospitalized with severe pneumonia will be randomized (1:1:1:1) to (i) SoC, (ii) valganciclovir, (iii) TB-T, and (iv) TB-T plus valganciclovir. The primary endpoint of the study is all-cause mortality, focusing on the short-term (up to 15 days) and long-term (up to 1 year) mortality. Secondary endpoints include repeat hospitalization, duration of oxygen therapy during initial admission, severe and notable adverse events, adverse reactions, CMV and TB prevalence at enrolment, TB incidence, CMV viral load reduction, and evaluation of diagnostic tests such as GeneXpert Ultra on fecal and nasopharyngeal aspirate samples and urine TB-LAM.DiscussionGiven the challenges in diagnosing CMV and TB in children and results from previous autopsy studies that show high rates of poly-infection in HIV-infected infants with respiratory disease, this study aims to evaluate a new approach including empirical treatment of CMV and TB for this patient population. Abstract

Epilepsy is a neurological disorder (the third most common, following stroke and migraines). A key aspect of its diagnosis is the presence of seizures that occur without a known cause and the potential for new seizures to occur. Machine learning has shown potential as a cost-effective alternative for rapid diagnosis. In this study, we review the current state of machine learning in the detection and prediction of epileptic seizures. The objective of this study is to portray the existing machine learning methods for seizure prediction. Internet bibliographical searches were conducted to identify relevant literature on the topic. Through cross-referencing from key articles, additional references were obtained to provide a comprehensive overview of the techniques. As the aim of this paper aims is not a pure bibliographical review of the subject, the publications here cited have been selected among many others based on their number of citations. To implement accurate diagnostic and treatment tools, it is necessary to achieve a balance between prediction time, sensitivity, and specificity. This balance can be achieved using deep learning algorithms. The best performance and results are often achieved by combining multiple techniques and features, but this approach can also increase computational requirements.

Abstract

The clustering methods have been used in a variety of fields such as image processing, data mining, pattern recognition, and statistical analysis. Generally, the clustering algorithms consider all variables equally relevant or not correlated for the clustering task. Nevertheless, in real situations, some variables can be correlated or may be more or less relevant or even irrelevant for this task. This paper proposes partitioning fuzzy clustering algorithms based on Euclidean, City-block and Mahalanobis distances and entropy regularization. These methods are an iterative three steps algorithms which provide a fuzzy partition, a representative for each fuzzy cluster, and the relevance weight of the variables or their correlation by minimizing a suitable objective function. Several experiments on synthetic and real datasets, including its application to noisy image texture segmentation, demonstrate the usefulness of these adaptive clustering methods.

Abstract

Pneumonia is a lung infection that causes 15% of childhood mortality, over 800,000 children under five every year, all over the world. This pathology is mainly caused by viruses or bacteria. X-rays imaging analysis is one of the most used methods for pneumonia diagnosis. These clinical images can be analyzed using machine learning methods such as convolutional neural networks (CNN), which learn to extract critical features for the classification. However, the usability of these systems is limited in medicine due to the lack of interpretability, because of these models cannot be used to generate an understandable explanation (from a human-based perspective), about how they have reached those results. Another problem that difficults the impact of this technology is the limited amount of labeled data in many medicine domains. The main contributions of this work are two fold: the first one is the design of a new explainable artificial intelligence (XAI) technique based on combining the individual heatmaps obtained from each model in the ensemble. This allows to overcome the explainability and interpretability problems of the CNN "black boxes", highlighting those areas of the image which are more relevant to generate the classification. The second one is the development of new ensemble deep learning models to classify chest X-rays that allow highly competitive results using small datasets for training. We tested our ensemble model using a small dataset of pediatric X-rays (950 samples) with low quality and anatomical variability (which represents one of the biggest challenges). We also tested other strategies such as single CNNs trained from scratch and transfer learning using CheXNet. Our results show that our ensemble model outperforms these strategies obtaining highly competitive results. Finally, we confirmed the robustness of our approach using another pneumonia diagnosis dataset [1].

22. Interaction of particles with a cavitation bubble near a solid wall

Abstract

Hard particle erosion and cavitation damage are two main wear problems that can affect the internal components of hydraulic machinery such as hydraulic turbines or pumps. If both problems synergistically act together, the damage can be more severe and result in high maintenance costs. In this work, a study of the interaction of hard particles and cavitation bubbles is developed to understand their interactive behavior. Experimental tests and numerical simulations using computational fluid dynamics (CFD) were performed. Experimentally, a cavitation bubble was generated with an electric spark near a solid surface, and its interaction with hard particles of different sizes and materials was observed using a high-speed camera. A simplified analytical approach was developed to model the behavior of the particles near the bubble interface during its collapse. Computationally, we simulated an air bubble that grew and collapsed near a solid wall while interacting with one particle near the bubble interface. Several simulations with different conditions were made and validated with the experimental data. The experimental data obtained from particles above the bubble were consistent with the numerical results and analytical study. The particle size, density and position of the particle with respect to the bubble interface strongly affected the maximum velocity of the particles.

Simple Summary: Heartworm disease is a zoonotic illness primarily affecting dogs and cats, which poses a significant risk to public health. Gran Canaria (Canary Island, Spain) serves as a hyperendemic region for this disease, offering a model for its study. This research aimed to track Dirofilariosis prevalence and distribution among dogs, cats, and residents from 1994 to 2020. Data from 5841 dogs, 1203 cats, and 1604 humans were analyzed, considering geographical and climatic factors. Results revealed a decline in Dirofilaria immitis prevalence over the years: from 67.02% in dogs (1994) to 15.81% (2020), and from 33.03% (2010) to 17.20% (2020) in cats. Human incidence dropped from 18.66% (2008) to 8.27% (2020). Prevalence was highest in temperate cold zone (TC) and temperate mild zone (TM) climates. Despite a decrease in prevalence, Gran Canaria remains hyperendemic. The study underscores the significance of a "One Health" approach and highlights ongoing risks of disease transmission. The incidence of D. immitis in humans in 2008 was 18.66%, while in 2020 it was 8.27%. For all study groups, temperate cold zone (TC) and temperate mild zone (TM) climates had the highest prevalence. Throughout these 20 years, the prevalence of heartworm disease has decreased. Despite this, it continues to be a hyperendemic island. Abstract

Profilicollis rancoensis n. sp. is the tenth species of Profilicollis Meyer, 1931 which includes 9 other species mostly known from marine decapod crabs and shore birds. Cystacanths of P. rancoensis are described from the dominant freshwater crab Aegla abtao in Ranco Lake, Chile and are morphologically distinguished from cystacanths of the 9 other species based on a combination of 4 characters. These are body size, number of proboscis hook rows, number of hooks per row, and length of the largest anterior 2–4 hooks. Male and female cystacanths of P. rancoensis are 2.10–3.33 mm long having an ovoid proboscis with 14 rows of 6–7 hooks per row, with the largest anterior 2–4 hooks being 105–110 micrometers long; the anterior trunk has many small spines in 70–80 concentric rings, each with 50–60 spines around them; hook roots are simple, directed posteriorly, about as long as the blades anteriorly with unremarkable anterior manubria; the cephalic ganglion are in mid-receptacle just anterior to the level of the anterior trunk; the lemnisci are long and slender; the testes are in the anterior trunk, posterior trunk, or one in each; the primordia of 2 tubular cement glands are evident; strong bundles of fibers link the anterior and posterior trunk; and the posterior trunk has a corrugated surface cuticula. Molecular analysis (COI and 18S) sequences coincided with the morphology and support its taxonomy. The phylogenetic profile revealed that P. rancoensis n. sp. fell into the Profilicollis clade. Both sequences showed low genetic variation, and three different haplotypes were found. The new species was more closely related to P. botulus (Van Cleave, 1916) Witenberg, 1932 than to other Profilicollis species.
