3. The Role of Nonclassroom Spaces in Living-Learning Communities

Abstract

A body of research suggests that learning communities provide a range of academic benefits by increasing social connectedness. Researchers have also hypothesized that informal learning spaces--nonclassroom spaces (NCSs)--can facilitate learning by supporting social connectedness. This study uses qualitative methods to explore the way nonclassroom spaces facilitate learning-related activities within a recently established learning community at Michigan State University. Our findings suggest that NCSs function as social hubs, help make intellectual and creative work visible to the larger learning community, and provide access to important resources. More generally, NCSs help to facilitate community formation and identity.

4. Urinary Catheterization in Infants: When It’s Knot so Simple

Abstract

Pediatric fever is one of the most common presenting complaints to emergency departments (ED). While often due to a viral illness, in young children without a source the most common bacterial infection is pyelonephritis. For this reason, when no focal source can be identified a urinary specimen is recommended. In young children who are unable to urinate on demand, a straight catheter is required to obtain a sterile specimen. This is generally a benign procedure and is performed frequently in EDs. We report a case of a young girl who underwent straight bladder catheterization and was subsequently found to have a retained catheter that had become knotted in the bladder. This case report highlights a rare complication of this common procedure and describes the technique required to remove the catheter. An understanding of these issues may avoid the need for transfer to a pediatric facility or for subspecialty consultation.

10. Investigation of hepatitis C virus (HCV) interactions with host lipid metabolism : a translational research study

Abstract

Research into chronic hepatitis C virus (HCV) infection (CHC) is needed because only ~50% of patients with CHC are cured with existing treatments. HCV interacts with metabolism of cholesterol and very low density lipoproteins (VLDL) for replication, assembly, secretion and entry. Patients with CHC exhibit a dyslipidemia characterised by low LDL cholesterol (LDL-C) and insulin resistance. This translational study characterised the dyslipidemia apparent in CHC in retrospective and prospective HCV cohorts. Distinct metabolic phenotypes were defined between HCV genotypes 1 and 3. LDL-C was markedly reduced in HCV-G3, which exerted a greater effect than apoE genotype on LDL-C levels. Prospective analysis of non-cholesterol sterol intermediates established that disordered cholesterol synthesis in HCV was mediated predominantly via the lathosterol pathway. In HCV-G3, levels of Proprotein Convertase Subtilisin Kexin type 9 (PCSK9) were low compared to healthy controls, suggesting increased LDL clearance. Low LDL-C and high triglyceride/HDL ratio were found to be predictive of poor response to anti-viral therapy. Collaboration in a genome wide association study revealed that SNPs in IL28B rather than in lipid regulating genes are the major host genetic determinants of treatment response. Analysis of HCV lipoviral particles (LVP) by iodixanol density gradient ultracentrifugation revealed correlations between insulin resistance and triglycerides (TG) with LVP in HCV-G1. Determination of apoB in VLDL1, VLDL2, IDL and LDL fractions confirmed fasting TG are predominately in the VLDL1 fraction, implying that HCV-G1 preferentially associates with the VLDL1 pathway. Interferon γ inducible protein 10 (IP10), a marker of hepatic interferon stimulated gene expression correlated with LDL-C and HCV LVP ratio in HCV-G1, explaining the association between low LDL-C and poor treatment response. A randomised pilot trial in 60 CHC non-responders indicated that 12 weeks of treatment with Fluvastatin can lower total viral load in HCV-G1 & G4 and that low dose n3 PUFAs improved IP10 levels.

11. Study of the development of crystalline Proteus mirabilis biofilms on urinary catheters

Abstract

Infection by Proteus mirabilis can seriously complicate the care of patients undergoing long-term indwelling bladder catheterisation. The urease-producing bacteria colonize the catheter surfaces forming extensive biofilm communities and are capable of generating ammonia from urea and elevating the pH of the urine and biofilm. Under these conditions crystals of calcium and magnesium phosphates form in the urine and within the bacterial biofilm on the indwelling device leading to its encrustation and blockage. Urine can leak around the outside of the blocked catheter and patients become incontinent. Alternatively, urine is retained within the bladder, causing painful distension of the bladder. Reflux of infected urine to the kidneys can lead to serious symptomatic episodes such as pyelonephritis, septicaemia and endotoxic shock. All available types of indwelling catheter are vulnerable to this problem and currently there are no effective procedures available for its control. While the basic mechanism has been established for catheter encrustation we still need to know more about some of the fundamental aspects of the process. Little is known about the early events and the precise mechanisms which P. mirabilis uses to colonize catheter surfaces. The factors that control the rate at which crystalline biofilm forms on the catheters are also unknown. The aims of this study were to establish the sequence of events in the early stages of crystalline P. mirabilis biofilm formation on the range of currently available catheters for use with patients to determine the role of Mannose-Resistant Proteus-hkc fimbriae (MR/P fimbriae) in P. mirabilis crystalline biofilm formation on catheters to investigate how the pH at which calcium and magnesium phosphates precipitate from urine, the nucleation pH (pHn) can be manipulated and to determine the effect of this parameter on the rate of catheter encrustation. Using a laboratory model of the catheterised bladder, scanning electron microscopy and X-ray microanalysis, the initial stages of P. mirabilis crystalline biofilm development was observed on catheter surfaces. All-silicone, silicone-coated latex, hydrogel-coated latex and hydrogel/silver-coated latex catheters rapidly acquired a microcrystalline 'foundation layer' comprised predominantly of calcium phosphate, upon which, P. mirabilis crystalline biofilm subsequently developed. A similar 'foundation layer' was observed on the encrusted surfaces of hydrogel/silver-coated catheters removed from long-term catheterised patients. The catheters impregnated with nitrofurazone briefly delayed the onset of crystalline biofilm formation, while all-silicone and hydrogel-coated latex catheters inflated with triclosan (3 mg/ml in Na2C03) were able to maintain acidic urine pH and prevent crystalline biofilm development for the 7 day experimental period. There is evidence that MR/P fimbriae are involved in initiating infection in non-catheterised urinary tracts. The role of these adhesins in crystalline biofilm formation on indwelling catheters however, has not been investigated. Using bladder models infected with a wild type P. mirabilis strain able to express MR/P fimbriae and its derived MR/P-negative mutant, time to catheter blockage experiments and scanning electron microscopy revealed that MR/P fimbriae were not essential for P. mirabilis colonization of catheter surfaces or the development of crystalline P. mirabilis biofilm. Although the wild type and mutant strain initiated biofilm formation in different ways both rapidly blocked all-silicone catheters with crystalline material. The overriding factor in catheter blockage was the generation of alkaline urine, raising the pH above that at which crystalline formations develop. Previously it has been demonstrated that the pHn of human urine can be elevated by dilution and by increasing its citrate content. In the present study the effect of dilution and adding citrate on the pHn of artificial was assessed. Furthermore, the effect on the rate of encrustation on all-silicone catheters was examined in laboratory models supplied with these urines and infected with urease-positive P. mirabilis, Providencia rettgeri and Proteus vulgaris. The pHn of urine could be elevated from pH 6.7 in neat urine to pH 8.4 in urine diluted to 1:6. When neat, 1:1, 1:2 and 1:3 diluted urines were supplied to bladder models significant increases in catheter lifespan were recorded at each ascending dilution. Increasing the citrate content of the 1:1 diluted urine from 0 to 3.0 g/L citrate elevated the pHn from pH 7 to pH 9.1. Scanning electron microscopy of catheter sections revealed crystalline material in the biofilms could be virtually eliminated for at least 7 days in models supplied with urine with pHns of >pH 8.5. Time to catheter blockage experiments showed the rate of catheter encrustation became significantly reduced as the pHn of urine increased. Catheters in models supplied with urine containing citrate concentrations of 1.5 mg/ml (pHn >8.4) or more drained freely for the whole 7-day experimental period.

13. Open‐label, clinical trial extension: Two‐year safety and efficacy results of seladelpar in patients with primary biliary cholangitis.

Abstract

Summary: Background: Seladelpar is a potent and selective peroxisome proliferator‐activated receptor‐δ agonist that targets multiple cell types involved in primary biliary cholangitis (PBC), leading to anti‐cholestatic, anti‐inflammatory and anti‐pruritic effects. Aims: To evaluate the long‐term safety and efficacy of seladelpar in patients with PBC. Methods: In an open‐label, international, long‐term extension study, patients with PBC completing seladelpar lead‐in studies continued treatment. Seladelpar was taken orally once daily at doses of 5 or 10 mg with dose adjustment permitted for safety or tolerability. The primary analysis was for safety and the secondary efficacy analysis examined biochemical markers of cholestasis and liver injury. The study was terminated early due to the unexpected histological findings in a concurrent study for non‐alcoholic steatohepatitis, which were subsequently found to predate treatment. Safety and efficacy data were analysed through 2 years. Results: There were no serious treatment‐related adverse events observed among 106 patients treated with seladelpar for up to 2 years. There were four discontinuations for safety, one possibly related to seladelpar. Among 53 patients who completed 2 years of seladelpar, response rates increased from years 1 to 2 for the composite endpoint (alkaline phosphatase [ALP] <1.67 × ULN, ≥15% decrease in ALP, and total bilirubin ≤ULN) and ALP normalisation from 66% to 79% and from 26% to 42%, respectively. In those with elevated bilirubin at baseline, 43% achieved normalisation at year 2. Conclusions: Seladelpar was safe, and markedly improved biochemical markers of cholestasis and liver injury in patients with PBC. These effects were maintained or improved throughout the second year. Abstract

SDI emerged in the mid-90’s through peer-to-peer networking of organised slum dweller movements. Today this international network spans 32 countries where grass roots community organisations are supported by professional support NGOs in the building of social movements, transforming urban environments, and securing a more inclusive and resilient future for the world’s urban poor. SDI’s mission is to build the voice and agency of slum dweller communities, with a special focus on the role of women, in order to achieve inclusive cities in which the urban poor are to be at the centre of strategies and decision-making for equitable urban development.SDI has a commitment to project typologies that produce learning at scale around clean energy access as part of its informal settlement upgrading agenda. Since 2014 SDI has been involved in the field of access to energy particularly in Africa, India, and the Philippines where the SDI Energy Justice Programme leverages community-led collection of disaggregated energy access data (using the Know Your City tools), community empowerment programs, and pro-poor access models in contribution to energy access goals. In the face of growing needs of access to many essential services in slums, SDIs model provides bottom-up, innovative, and adaptable methodological options for catalysing pro-poor change at settlement, city, national, and global levels.

Highlights • About two thirds of caregivers intend to vaccinate their children against COVID-19. • Most common reason for acceptance was to protect the child. • Most common reason for refusal was the vaccine’s novelty. • Child age, chronic illness, vaccination history affects willingness. • Caregiver gender, vaccination history, concern about infection affect willingness., Background More than 100 COVID-19 vaccine candidates are in development since the SARS-CoV-2 genetic sequence was published in January 2020. The uptake of a COVID-19 vaccine among children will be instrumental in limiting the spread of the disease as herd immunity may require vaccine coverage of up to 80% of the population. Prior history of pandemic vaccine coverage was as low as 40% among children in the United States during the 2009 H1N1 influenza pandemic. Purpose To investigate predictors associated with global caregivers’ intent to vaccinate their children against COVID-19, when the vaccine becomes available. Method An international cross sectional survey of 1541 caregivers arriving with their children to 16 pediatric Emergency Departments (ED) across six countries from March 26 to May 31, 2020. Results 65% (n = 1005) of caregivers reported that they intend to vaccinate their child against COVID-19, once a vaccine is available. A univariate and subsequent multivariate analysis found that increased intended uptake was associated with children that were older, children with no chronic illness, when fathers completed the survey, children up-to-date on their vaccination schedule, recent history of vaccination against influenza, and caregivers concerned their child had COVID-19 at the time of survey completion in the ED. The most common reason reported by caregivers intending to vaccinate was to protect their child (62%), and the most common reason reported by caregivers refusing vaccination was the vaccine’s novelty (52%). Conclusions The majority of caregivers intend to vaccinate their children against COVID-19, though uptake will likely be associated with specific factors such as child and caregiver demographics and vaccination history. Public health strategies need to address barriers to uptake by providing evidence about an upcoming COVID-19 vaccine’s safety and efficacy, highlighting the risks and consequences of infection in children, and educating caregivers on the role of vaccination.

Abstract

The modeling of correlated RF noise in the intrinsic base and collector currents of SiGe hetrojunction bipolar transistors using quasi-static equivalent circuits in which noise are first extracted from measured noise parameters using standard noise circuit analysis is presented. The model is shown to work at frequencies up to atleast half of the peak f(sub T) and at biasing currents below high injection f(sub T) rolloff.

Abstract

The International Liver Disease Genetics Consortium (ILDGC)., Genetic variation in the IFNL3–IFNL4 (interferon-λ3–interferon-λ4) region is associated with hepatic inflammation and fibrosis1,2,3,4. Whether IFN-λ3 or IFN-λ4 protein drives this association is not known. We demonstrate that hepatic inflammation, fibrosis stage, fibrosis progression rate, hepatic infiltration of immune cells, IFN-λ3 expression, and serum sCD163 levels (a marker of activated macrophages) are greater in individuals with the IFNL3–IFNL4 risk haplotype that does not produce IFN-λ4, but produces IFN-λ3. No difference in these features was observed according to genotype at rs117648444, which encodes a substitution at position 70 of the IFN-λ4 protein and reduces IFN-λ4 activity, or between patients encoding functionally defective IFN-λ4 (IFN-λ4–Ser70) and those encoding fully active IFN-λ4–Pro70. The two proposed functional variants (rs368234815 and rs4803217)5,6 were not superior to the discovery SNP rs12979860 with respect to liver inflammation or fibrosis phenotype. IFN-λ3 rather than IFN-λ4 likely mediates IFNL3–IFNL4 haplotype–dependent hepatic inflammation and fibrosis., M.E., M.D., and J.G. are supported by the Robert W.
