The sinus lift procedure has become the most common method for maxillary bone augmentation. The most frequently observed intraoperative complication is the perforation of the Schneiderian membrane. Various treatment options have been proposed for managing these perforations, including the use of resorbable membranes, centrifugated blood products as PRF, or PRGF, suturing, and fibrin glue application. While long-term studies exist for the use of resorbable membranes to close perforations, there is limited data on the long-term outcomes of suturing or gluing the perforated sinus membrane. The aim of this retrospective study is to evaluate the long-term outcomes of suturing and/or applying fibrin glue to repair perforated sinus mucosa during sinus floor elevation procedures. Between 2005 and 2009, a total of 692 patients underwent 923 sinus lift surgeries, and Schneiderian membrane perforation occurred in 202 sinus floor elevations (21.98%) across 168 patients. The main documented causes of perforations, which ranged from 2 to 10 mm in diameter, were the presence of septa, followed by thin and adherent membranes. Of the perforations, 100 (49.5%) were treated with microsurgical suturing combined with fibrin glue, 78 (38.6%) were treated with fibrin glue alone, and 24 (11.9%) were treated exclusively with suturing. Sinus grafting was performed using autogenous bone in combination with a biomaterial, following the layering technique. All surgeries resulted in primary healing without complications, enabling all patients to undergo restoration as planned. The long term clinical and radiological evaluations of 44 randomly selected patients who followed the recall program up to 10 years post operative confirmed the effectiveness of this treatment approach. This review aimed to evaluate CGF’s effectiveness in maxillary sinus augmentation, focusing on clinical, radiographic, and histological outcomes. A comprehensive literature search was conducted across PubMed, Scopus, Web of Science, Google Scholar, and Cochrane Library databases using specific mesh terms and keywords and adhered to PRISMA guidelines. Studies up to March 2024 involving transcrestal or lateral maxillary sinus floor elevation with CGF, either alone or in combination with grafting materials, were included. Out of 783 publications, 13 studies met the eligibility criteria. The review assessed CGF’s role in minimizing postoperative complications, enhancing new bone formation, and improving implant survival. Despite data variability among studies, the majority reported that CGF positively influenced maxillary sinus augmentation outcomes. While more robust randomized clinical trials are required to draw definitive conclusions, initial results are promising, and the findings suggest that CGF utilization in maxillary sinus augmentation seems to enhance clinical outcomes by promoting vascularization and regeneration at the surgical sites and improving both the quality and quantity of newly formed sinus bone. Furthermore, it exhibits potential for improving postoperative sequelae and achieving high implant survival rates.

Abstract Sinus lift surgeries are part of the daily practice of dentists. This study evaluates the long-term structure of the bone placed in sinus lifts through the fractal dimension. We conducted a retrospective study on a sample of 35 patients with 51 sinus lifts performed using a lateral window approach and filling material placement. We radiologically analyzed the graft bone to observe its evolution up to one and a half years after the surgical procedure. The obtained results were the average area of the sinuses analyzed was 1401.96 mm2, with a mean area occupied by the filling material of 297.75 mm2. Significant differences are observed when comparing the fractal dimension values obtained on the initial day and one year after prosthesis loading. Objectives: A mucous retention cyst is a common, asymptomatic lesion that may cause complications during or after the sinus lift procedure. The goal of this study is to assess the effectiveness of the Croco Eye Technique (CET), which allows simultaneous excision of the cyst and sinus floor elevation. Methods: The technique was thoroughly described in two versions, and the group of 33 patients was analyzed. Patients who qualified for this procedure had insufficient alveolar ridge height, and their CBCT showed radiological images typical for retention cysts. Analyzed parameters included the version of CET, demographic data, anatomical parameters, intraoperative complications, recurrence of the cyst, success rate of the sinus lift and implants, and the follow-up period. Results: Out of the 33 cases, 9 were of the primary version (27.27%) and 24 of the final version (72.73%). The average height of a retention cyst was 24.05 mm, with the average alveolar ridge height of 1.86 mm. In three cases (9.09%), implants were placed immediately. The prevalence of uncontrolled Schneiderian membrane perforation was reduced from 55.56% to 4.17% between the primary and final versions. The cyst's recurrence rate was 3.13%. The implant survival rate was 100%. The mean follow-up period was 48.625 months (max 110 months). Conclusions: The Croco Eye Technique, despite the perforation of the Schneiderian membrane, enables successful sinus lift and implantation with a success rate of 100%. Purpose: To analyze the visibility of the maxillary sinus septa (MSS) in panoramic radiography (PR) versus cone beam computed tomography (CBCT) and to investigate whether the buccal cortical bone thickness (BT) or the septa dimensions influence their visibility. Methods: Corresponding PR and CBCT images of 355 patients were selected and examined for MSS visibility. The septa dimensions (width, height, depth) and the BT were measured. Results were analysed statistically. Results: Comparing the corresponding regions on CBCT and PR, 170 MSS were identified; however, only 106 of these were also visible using PR. The MSS visibility was significantly higher on CBCT versus PR images (P1: p = 0.039, P2: p = 0.015, M1: p = 0.041, M2: p = 0.017, M3: p = 0.000), except region C (p = 0.625). Regarding the measurements of MSS dimensions, only the height in region M1 (p = 0.013) and the width in region P2 (p = 0.034) were significantly more visible on CBCT. The BT in the area of the MSS was found to have a marginal influence on its visibility on the PR images only in regions M3 and M1 (M3: p = 0.043, M1: p = 0.047). In terms of MSS visibility based on the dimensions, significance was found for all three influencing variables only in region P2 (width; p = 0.041, height; p = 0.001, depth; p = 0.007). There were only isolated cases of further significance: M3 for width (p = 0.043), M2 for height (p = 0.024), and P1 for depth (p = 0.034), no further significance was noted. Conclusion: MSS visibility appears significantly higher on CBCT versus PR images. It is concluded that the septa dimensions and BT can influence MSS visibility on PR images just in certain regions. Background: This review aims to explore the role of the alveolar antral artery and to contribute a combined data set pertaining to the artery's prevalence, diameter, and proximity of related structures. Methods: A comprehensive literature review. Five databases were searched, namely: Web of Science, Scopus, MEDLINE-PubMed, EMBASE, CINHAL-Ultimate in September 2023. Collected and analyzed data included the prevalence and diameter of the alveolar antral artery, the diameter of the artery divided into groups of less than 1 mm, 1–2 mm, and more than 2 mm, and the distance between the artery and the alveolar ridge. The AQUA-tool was utilized to assess the quality of each study. The data was extracted and organized into comparison tables. Results: 50 articles met the inclusion criteria. The studies reported differences between genders regarding the included criteria. The review identified inconsistent nomenclature used to describe the alveolar antral artery, potentially leading to confusion or misinterpretation among researchers and clinicians. The literature discusses a variety of scenarios regarding the artery in surgical operations, including techniques for managing and preserving it. Discussion: The literature highlights the importance of standardizing anatomical nomenclature for anastomosis and emphasizes the necessity for developing clinical guidelines in the management of the artery.

Maxillary lateral sinus floor elevation, or external sinus lift, is a widespread surgical intervention in the dental field. Insertion of implants in the posterior region of the maxilla often requires reconstruction of the remaining native bone that has insufficient volume. Background and aims. Much of the research published involves using artificial products, like xenografts and resorbable collagen membranes, after a prior Cone Beam Computer Tomography (CBCT) investigation. Nowadays, more accessible access, less financial costs, a biological approach, and faster healing are objectives that surround this procedure. Leucocytes and platelets rich in Fibrin (L-PRF) are a natural component with a high concentration of growth factors. Due to its regenerative properties and lack of complications, it is used in several medical fields, like orthopedics, dermatology, and oral surgery. This retrospective study aims to compare results in bone height and volume obtained through external sinus lift, either by using xenografts or autologous plasma rich in fibrin, by evaluating the quantity of new bone formation from a radiological point of view. Methods. Fifty-eight Caucasian patients were included in this retrospective study; 48 were submitted to xenograft procedure, and 10 were selected for L-PRF grafting material with simultaneous implant placement. Lack of clinical and histological studies performed on patients with L-PRF surgeries limited us in choosing a larger group for the radiological analysis. CBCT evaluation was performed before surgery and 6 months after. All patients selected for the study presented good general and oral health, acute oral and sinus infections excluded; smoking and periodontal disease were also criteria of exclusion. Two operators performed the measurements in pre-established landmarks in different time frames. The two independent groups were compared with the Wilcoxon rank-sum test for quantitative data. Qualitative characteristics were described as counts and percentages. All analyses were performed in an R environment for statistical computing and graphics. Results. Mean bone height gain in the xenograft group in the regions was as follows: 7.44 for the anterior landmark, 12.14 for the median and 8.28 for the distal. The mean group height gained for the L-PRF group was 0.1 anteriorly, -0.18 for the median measurement, and 0.23 distally. We obtained excellent overall reliability for all the height measurements between the two operators. Conclusions. Further studies must be conducted to establish new sets of surgical protocols in case L-PRF alone is found to be a reliable, stable, biological alternative to the welldocumented xenografts in external sinus lifts. Radiological results, although promising, must be further applied in long term clinical survival of the implants in the grafted sites. Also, studies combining L-PRF in conjunction with xenograft might bring improved clinical results in terms of reduced postoperative complications and accelerated healing. [ABSTRACT FROM AUTHOR]

Background and Objectives: The aim of this systematic review was to assess the efficiency of using allografts for sinus lift. Materials and Methods: This systematic review was written under the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines and recommendation of the Cochrane Handbook for Systematic Reviews of Interventions. Three electronic databases were screened until October 2023. The risk of bias was assessed according to the Strengthening the Reporting of Observational studies in Epidemiology (STROBE) guidelines. Statistical analysis was performed for median bone volume and implant survival rate. Results: From 321 articles retrieved, 7 articles were included in this review. A comparison between freeze-dried bone allograft (FDBA) and deproteinized bovine bone (DBB) for mean bone volume indicated a weighted mean difference (WMD) of −0.17 [−0.69, 0.36] (95% confidence interval (CI)), p = 0.53. For implant survival rate, a comparison was made between FDBA and autogenous bone indicating a risk ratio (RR) of 1.00 [0.96, 1.05] (95% CI), p = 1.00. Conclusions: The available evidence suggested that allograft bone can be used in sinus lift procedures. The results obtained are insufficient to compare with other types of bone graft, requiring a longer follow-up time. Abstract Purpose Sinus lift operations are a tried and tested means of providing adequate implant prosthetics to patients with compromised jawbones. Knowledge of the arterial supply of the maxillary sinus region is essential for surgical treatment in this area. The aim of the present comparative study was to determine whether alveolar antral artery (AAA) canal can be diagnosed both in corresponding panoramic radiography (PR) and cone-beam computed tomography (CBCT). Methods A total of 335 patients with 635 sites and corresponding maxillary sinus in both PR and CBCT were selected and examined for AAA canal visibility. Results The visibility of the AAA canal was significantly higher in CBCT than in PR. A total of 154 (46.0%) AAA canals could be identified in the maxillary sinus on the right. However, only four (1.2%) of these were also visible in PR. The detected values of the AAA canals in the maxillary sinus on the left in the PR and CBCT images were similar to those of the right. While 164 AAA canals (49%) were observed in CBCT images, only 1 (0.3%) was identifiable in PR. Conclusions The results show that CBCT can be recommended for visualising the AAA canal when surgically planning sinus augmentation procedures. Graphical Abstract

Objective: Sinus floor elevation is commonly done in the maxillary posterior region prior to dental implant placement. This study primarily aimed at assessing the location of the posterior superior alveolar artery (PSAA) canal on cone beam computed tomography (CBCT) scans and its relation to the alveolar ridge and maxillary sinus from a fixed reference point. Material and Methods: A total of 226 edentulous maxillary molar sites were included in this retrospective analysis. The distance from the PSAA to the sinus floor (SF), alveolar crest (AC) and a fixed reference point, that is, the roof of sinus (RS) were measured. The alveolar bone height (ABH), thickness of the crestal keratinized mucosa (CKM) and thickness of Schneiderian membrane (SM) were also evaluated. Results: 54 sites (23.89%) were excluded from the study. The SF, AC, RS and ABH values averaged at 11.91 mm ± 3.63 mm, 16.05 mm ± 3.96 mm, 25.32 mm ± 7.13 mm and 4.93 mm ± 4.27 mm respectively. SF and AC was higher in second molar than first molar region (p < 0.001), but RS did not show significant difference (p = 0.85). CKM and SM averaged at 2.02 mm ± 0.68 mm and 1.31 mm ± 0.81 mm respectively. Conclusion: The PSAA can be visualized in CBCT scans with a prevalence of 76.11% and may not be detected when adherent to the sinus membrane. Objective: Nursing comprises the care of sick or well people, including health promotion, maintenance, or recovery. As stated in Article 7 of the Saudi Law of Health Jobs, the stated monthly working hours are 176 hours. When they exceed 8 hours in outpatient departments, 12 hours in inpatient departments, or 176 monthly hours, they are considered overloaded. Providers seek to provide cost-free qualified care. Many countries have developed new techniques to combat the rising costs, which may lower quality. This study is concerned with the nurse shortage and the growing burden of healthcare. Materials and Methods: A self-structured survey was distributed among different departments in five hospitals. The study was conducted in November and December, 2021. Results: 21.5% of males and 78.5% of females, with an average age of 44.3 years, contributed. 77.8% have a bachelor's, 79% are trained in patient safety, and 84% had no patient safety problems. A Likert scale-based scoresheet was distributed to measure patient safety practice, and higher scores were found in expert nurses, nurses working in isolation departments, and those who had received patient safety training. It also showed lower scores as the working hours increased. Using a t-test and an ANOVA provides insignificant results. Conclusion and Recommendation: Average working hours improve quality and safety. This distinguishes decision-making from action-taking. What care to offer and how to provide and allocate time are interwoven practice decisions. Due to schedules, policies, and workflows, nurses may not be able to make timely decisions. Introduction. The presence of bone septum in the maxillary sinus is one of the most common anatomical findings. So-called Underwood septa (US) are an atypical bone formation in the maxillary sinuses. Mostly they are quite easily found in CBCT studies and have major importance in sinus lift procedures in dental surgery. Furthermore, the shape, location, and size of the bony septa are important in each maxillary sinus surgery. Material and methods. A retrospective study of 120CBCT scans from the authors' own database was conducted. Results. Approximately 37.5% of each CBCT was associated with the occurrence of US, while just 25% had a full septum, and a total of only 14 patients had a half septa. More females have US, while healthy pneumatized maxillary sinus is most commonly found (82.22%). There is no correlation between the occurrence of silent sinus syndrome (p = 0.174), mucosal thickening (p = 0.325), or retention cyst formation (p = 0.272). Most sinuses are without any opacification in CBCT evaluation (91.11%), while other syndromes are not statistically relevant. Conclusions. It seems that the occurrence of Underwood septa is not statistically related to any clinical, radiological, or pathological condition within the sinus (p > 0.05). (1) Background: The demands of patients for aesthetic and functional rehabilitation of edentulous areas led to the use of dental implants as therapeutic means on an increasingly large scale. This aspect determined the appearance of some infectious pathologies with a peri-implant starting point that can be complicated by various sinus diseases. The purpose of this review article is to synthesize the existing information in the specialized literature regarding the existing correlations between peri-implant and maxillary sinusitis. (2) Methods: The articles published in five databases were researched using different combinations of search terms. We selected 12 articles from the 250 found, by applying the inclusion and exclusion criteria and removing duplicates. (3) Results: We analyzed the included studies and we found that all of them reported a positive correlation between maxillary sinusitis and peri-implant infectious diseases. There are also reported other pathologies with a peri-implant infectious disease as a starting point such as abscesses, oro-antral communications, or foreign body reactions due to implant or bone graft materials migration. (4) Conclusions: This scoping review highlighted the existence of correlations between peri-implant and sinus pathology and the importance of preventing peri-implant diseases of an infectious nature to avoid the occurrence of these complications. Missing teeth may result in a functional and cosmetic deficit and have traditionally been replaced with dentures or bridges. Dental implants rely on the maintenance of a direct structural and functional connection between living bone and the implant surface, this is termed osseointegration. Osseointegration has undoubtedly been one of the most significant scientific breakthroughs in dentistry over the past 40 years. Insufficient bone volume is a common problem encounter in the rehabilitation of the edentulous posterior maxilla with implant-supported prostheses. The bone available for implant placement may be limited by the presence of the maxillary sinus together with loss of alveolar bone height. Bone volume may be increased by augmentation, commonly the sinus cavity is augmented with autogenous bone or biomaterials or both. Procedures are variously described in the literature as sinus lift, sinus augmentation, sinus floor elevation or augmentation of atrophic maxillary sinus. Implant placement may be combined with sinus augmentation as one-stage technique. Alternatively sinus augmentation may be carried out as some time prior to implant placement as a two-stage technique.

Introduction and aim: Dental implant treatment in the posterior maxilla is often complicated by the poor healing of the alveolar process and by the pneumatization of the maxillary sinus. The authors are explaining the Cologne Classification of the Alveolar Ridge Defects (CCARD) with special emphasis on the maxillary sinus area and listing the main fators influencing process of selection of dental implant treatment. The aim of this article is to demonstrate, using case reports, four types of treatment. Results and conclusions: There are four types of treatment implant surgeon must choose from while considering treatment in posterior maxilla. Current literature does not give a guidance as to which treatment method to use. Transalveolar sinus lift has a good clinical outcome regardless of presence or absence of grafting material. Short implants with a long clinical crown are good alternative to treatment at times where Schneiderian membrane manipulation is not advised.

The method for the production of PRF (Platelet Rich Fibrin) was developed in 2000 and belongs to the second generation of platelet concentrates. Several clinical studies have demonstrated its effectiveness in combination with various bone substitutes. The concentrate is prepared by centrifuging the patient's own blood without addition of anticoagulants, and according to the established protocol. The process yields a fibrin-rich clot of leukocytes and platelets that serves as an extracellular matrix. The growth factors from white blood cells and platelets are released slowly (2–3 weeks), and thus, have a beneficial effect on the wound healing. In this article, the authors present their review of the data available in the main literature and report on potential applications of PRF in dental, dentoalveolar, and maxillofacial surgeries (implantology, bone grafting, dentoalveolar surgery, periodontology, sinus closure, sinus elevation, osteonecrosis, endodontics).

Maxillary sinus lift is a common procedure to achieve adequate alveolar bone height in patients with sinus hyper-pneumatization. Simultaneous implant placement with sinus floor augmentations is possible when appropriate primary stability could be achieved. In severe atrophic posterior maxillary ridge, vertical bone height is sometimes less than 4mm, which may hinder simultaneous implant placements and necessitate two stage procedure. In current study, a novel technique is described to perform a single stage implant placement with bicortical stability in severe atrophic maxilla that can shorten the treatment time of an edentulous patient. During the procedure, to achieve an adequate access a conventional lateral window osteotomy is performed. After elevating the Schneiderian membrane using conventional instruments, an autologous ramus block is harvested from the mandible and then the block will be fixed in correct location in the sinus using screws withb appropriate length to build sufficient stability as superior cortex for simultaneous implant insertion. With increasing experience and in an attempt to shorten overall treatment times, implant placement in combination with tooth extractions and sinus lift procedures has become popular. In both cases, primary stability has to be achieved by either engaging apical and oral regions of trabecular bone or by engaging residual host bone beneath the sinus cavity. Extraction sites were formed by pressing a root analog into homogeneous low density polyurethane foam which was used as bone surrogate while a 3 mm thick sheet of medium density foam was used for mimicking a sinus lift situation. Two types (n = 10) of bone level implants with a conventional tapered design and a cervical back taper (NobelActive; control) and a novel design characterized by a shift in core diameter and thread geometry (AlfaGate; test) were placed in these models following conventional osteotomy preparation. Insertion torque was measured using a surgical motor and primary stability was determined by resonance frequency analysis. Statistical analysis was based on Welch two sample t tests with the level of significance set at α = 0.05. In sinuslifting, NobelActive implants required significantly higher insertion torques as compared to AlfaGate (p = 0.000) but did not achieve greater implant stability (p = 0.076). In extraction sites, AlfaGate implants showed both, significantly higher insertion torques (p = 0.004) and significantly greater implant stability (p = 0.000). The novel implant design allowed for greater primary stability when being placed in simulated extraction sockets and sinuslift situations. While in extraction sockets the position of condensing threads in combination with an increase in core diameter is beneficial, the deep cervical threads of the novel implant lead to superior performance in sinuslift situations. Oral maxillofacial rehabilitation of the atrophic maxilla with or without pneumatization of the maxillary sinuses routinely presents limited bone availability. This indicates the need for vertical and horizontal bone augmentation. The standard and most used technique is maxillary sinus augmentation using distinct techniques. These techniques may or may not rupture the sinus membrane. Rupture of the sinus membrane increases the risk of acute or chronic contamination of the graft, implant, and maxillary sinus. The surgical procedure for maxillary sinus autograft involves two stages: removal of the autograft and preparation of the bone site for the graft. A third stage is often added to place the osseointegrated implants. This is because it was not possible to do this at the same time as the graft surgery. A new bioactive kinetic screw (BKS) bone implant model is presented that simplifies and effectively performs autogenous grafting, sinus augmentation, and implant fixation in a single step. In the absence of a minimum vertical bone height of 4 mm in the region to be implanted, an additional surgical procedure is performed to harvest bone from the retro-molar trigone region of the mandible to provide additional bone. The feasibility and simplicity of the proposed technique were demonstrated in experimental studies in synthetic maxillary bone and sinus. A digital torque meter was used to measure MIT and MRT during implant insertion and removal. The amount of bone graft was determined by weighing the bone material collected by the new BKS implant. The technique proposed here demonstrated the benefits and limitations of the new BKS implant for maxillary sinus augmentation and installation of dental implants simultaneously. Introduction: Sinus lift surgery allows sufficient volume of bone to be created in the posterior part of the maxilla. The aim of this study was to investigate the changes in maxillary sinus volume after a sinus lift and the rate of increase in ridge height at the site of the graft. Methods: Eleven patients were chosen for sinus lift from among those who were referred to the radiology department for implant placement in the posterior region of the maxilla and whose bone height at the posterior of the maxilla was less than 4 mm on the cone-beam computed tomography (CBCT) image. The sinus volume was measured after importing the CBCT file in DICOM format into Mimics software. After determining the sinus volume, the patients underwent sinus lift surgery, and the amount of material used during the surgery was measured. After the time required to repair the area, the CBCT image was taken again. Then, the changes in the volume of the maxillary sinus and the increase in the height of the maxillary ridge at the surgical site were calculated. Then, the second stage of the surgery was performed to place the implant at the implant site. Results: For an average of 1.40 cm3 of material, the rate of increase in ridge height was 10.52 mm, and the average change in sinus volume was 1.19 cm3. Background and objectives: The aim of this systematic review with meta-analysis was to assess the performance of short implants in comparison with standard implants and sinus floor elevation in atrophic posterior maxilla. Materials and methods: The protocol of the study was registered in the PROSPERO database (CRD42022375320). An electronic search on three databases (PubMed, Scopus, Web of Science) was performed to find randomized clinical trials (RCTs) with ≥5 years' follow-up, published until December 2022. Risk of bias (ROB) was calculated using Cochrane ROB. A meta-analysis was performed for primary (implant survival rate, ISR) and secondary outcomes (marginal bone loss, MBL; biological and prosthetic complications). Results: Of 1619 articles, 5 RCTs met the inclusion criteria. The ISR showed a risk ratio (RR) of 0.97 [0.94, 1.00] (CI 95%), p = 0.07. The MBL indicated a WMD of −0.29 [−0.49, −0.09] (CI 95%), p = 0.005. Biological complications showed a RR of 0.46 [0.23, 0.91] (CI 95%), p = 0.03. Prosthetic complications showed a RR of 1.51 [0.64, 3.55] (CI 95%), p = 0.34. Conclusions: The available evidence suggests that short implants might be used as an alternative to standard implants and sinus floor elevation. After 5 years, in terms of ISR, standard implants and sinus floor elevation showed a higher survival rate comparted to short implants, although statistical significance was not achieved. Background and Objectives: The aim of this systematic review was to assess the efficiency of using allografts for sinus lift. Materials and Methods: This systematic review was written under the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines and recommendation of the Cochrane Handbook for Systematic Reviews of Interventions. Three electronic databases were screened until October 2023. The risk of bias was assessed according to the Strengthening the Reporting of Observational studies in Epidemiology (STROBE) guidelines. Statistical analysis was performed for median bone volume and implant survival rate. Results: From 321 articles retrieved, 7 articles were included in this review. A comparison between freeze-dried bone allograft (FDBA) and deproteinized bovine bone (DBB) for mean bone volume indicated a weighted mean difference (WMD) of −0.17 [−0.69, 0.36] (95% confidence interval (CI)), p = 0.53. For implant survival rate, a comparison was made between FDBA and autogenous bone indicating a risk ratio (RR) of 1.00 [0.96, 1.05] (95% CI), p = 1.00. Conclusions: The available evidence suggested that allograft bone can be used in sinus lift procedures. The results obtained are insufficient to compare with other types of bone graft, requiring a longer follow-up time. Future clinical trials are needed in order to evaluate the advantages of using allograft bone.

Aim to develop a new method of endonasal augmentation of the alveolar process of the upper jaw to reduce postoperative risks. Material and methods. The study involved 10 patients during the period of December, 2019, and June, 2021. The examination methods were: computed tomography of the nose and paranasal sinuses, upper and lower jaw, and video-endoscopy of the nasal cavity. The patients received the indonasal sinus inlay with the use of allogenic bone chips. Results. The surgical access through the lower nasal passage, under the lower nasal concha, allows to expose the maxillary sinus floor with preservation of mucous membrane integrity and to fill in the space between them with the allogenic bone material. Formation of a bone window in the lateral nasal wall using a piezotome prevents the rupture of the sinus mucous membrane. A bony fragment of the lateral nasal cavity wall transferred into the cavity of the maxillary sinus as one block with the mucosa creates additional protection for the Schneiderian membrane. Allogenic material introduced by endonasal method is substituted by the own organotypic tissue, restoring the lost volume of the maxillary sinus alveolar process. Conclusion. The endonasal method of alveolar augmentation may be used in clinical practice as an alternative to the classic methods.

In this article, the authors want to present the possibilities of solving some difficult cases, treated in a conventional surgical and implant manner and which were a failure, as well as the possibilities of avoiding these failures using Strategic Implant® technology. In the first case presented, the patient goes through bone augmentation procedures, without reaching the time of implant insertion and prosthesis, with the loss of bone addition on one side of the mandible, the case being later completed with corticobasal implants in immediate prosthetic loading. The second case in which the patient successively loses the inserted two stage implants, without reaching their functional load, resorts to the treatment with corticobasal implants in immediate loading and the fixed prosthetic restoration of the mandible in 3 days postimplantation. The last case in which the patient comes for the removal of a maxillary prosthetic work fixed on 4 two stage implants with bone addition and unilateral sinuslift, with mobility, and in which the treatment with corticobasal implants and fixed prosthetic work in 3 days was the optimal way of treatment. These cases as well as the feedback of the treatment after a number of years show us that the treatment with corticobasal implants can be made much easier using only the native bone of the patient using implants that adapt to the morphology of the existing bone and a immediate loading protocol to allow the optimal functioning of prosthetic works; knowledge of surgical and prosthetic protocols of Strategic Implant® technology lead to predictable prosthetic results with minimally invasive surgery, without sinus lift and bone addition, in a short time of 3 days from the surgical step until the final oral rehabilitation.

The purpose of this study was to evaluate, using cone-beam computerized tomography (CBCT), the rate of sinus membrane perforation in osteotome sinus floor elevation (OSFE) performed with and without a graft material. Thirty patients with 52 OSFE sites were included in the study. Patients were divided into the control group (OSFE performed without graft material) and test groups (OSFE performed with autograft or xenograft). The autograft was harvested from the maxillary tuberosity using bone forceps. The xenograft was a commercial product originating from bovine bone. Graft volume was measured using the water displacement method. CBCT was performed at the initial examination and immediately after surgery to measure the residual bone height and to evaluate the endo-sinus bone gain and membrane perforation. The rate of sinus membrane perforation was 15.4%. Of the 52 OSFE procedures, 26.9% were performed without grafting and 34.6% and 38.5% were performed with autografts and xenografts, respectively. Membrane perforation was significantly higher in the autograft group (P =.033). The median volume of graft materials was 0.3 mL. The difference in graft volume between the autograft and xenograft was not statistically significant (P =.768). The mean endo-sinus bone gain was 6.55 mm in patients without membrane perforation and 8.71 mm in patients with membrane perforation; this difference was statistically significant (P =.035). The volume and physical properties of graft materials are important factors in membrane perforation. Further clinical studies with larger and standardized samples are needed to confirm the effect of graft materials on sinus membrane perforation in OSFE. [ABSTRACT FROM AUTHOR]

Soft-tissue defect and alveolar bundle bone resorption (horizontally and vertically) are inevitable events following tooth extraction predominately on the buccal aspect which will impact the implant surgical planning starting from implant size selection, position, emergence profile, etc. To overcome the same, complex soft and hard tissue regeneration procedures have been recommended in the literature but they are highly expensive and technique sensitive too. To prevent buccal bone resorption and gain good emergence profile socket-shield technique (SST) is widely recommended. Additionally, in order to improve implant biomechanical primary stability, bone mineral density, and bone to implant contact novel osseodensification approach is recommended for osteotomy preparation and sinus membrane lift whereas platelet rich fibrin (PRF) autograft is a suitable alternative for the protection of Schneiderian membrane during immediate implant placement in sinus sub cavity. The present case report describes the successful single-stage management of symptomatic endodontically maltreated posterior root stumps site utilizing SST in conjunction with Densah Bur and PRF autograft-assisted minimally invasive osteotomy preparation and sinus lift with immediate implant placement.

The success of dental implants lies in their strong and lasting integration into the patient's receiving bone. The first biological interactions at the implant surface determine the subsequent evolution of the integration process. In this study we set our objective to analyze the mechanistic interaction of the early regenerative matrix at implant surfaces modified with calcium ions (Ca) as compared to standard implant surfaces (NoCa). We put the surfaces in a Quartz Crystal Microbalance with Dissipation (QCM-D) to monitor the frequency shift (f) and the viscoelastic properties of the adsorbed biofilms and used Scanning Electron Microscopy (SEM) to visualize the resulting interfaces. Upon the addition of human blood plasma, Ca surfaces formed an adsorbed three-dimensional film attached to the surface (∆f = −40 Hz), while with NoCa, the biofilm formed but was not attached to the surface (∆f = 0 Hz). After 20 min in blood, two representative commercial implants with Ca and NoCa surfaces showed also distinct interfaces: Ca implants formed a visible clot attached to the implant which was composed mainly of platelets (Surface Coverage: 40 ± 20%) and some red blood cells (SC: 9 ± 3%) entrapped within a fibrin network (SC: 93 ± 5%). The NoCa implants were largely populated by red blood cells (SC: 67 ± 12%) with scarce fibrin remnants (SC: 3 ± 2%), and the implants showed no clot on their surfaces macroscopically. The pre-clinical and clinical results discussed in this work encourage the modification of titanium implant surfaces with calcium ions to improve the bone regenerative process. Taken together, these results add more information about the roles of Ca ions in bridging the formation of the provisional matrix at implant surfaces and their effects on implant osseointegration. The goal of this systematic study was to compare the survival rate (SR), marginal bone loss (MBL) and clinical complications between extra‐short implants (≤6 mm) and 6‐mm‐longer implants in randomized clinical trials. A systematic electronic and manual search was performed using the PubMed, Web of Science, Scopus and DOAJ databases. A meta‐analysis was conducted to compare the SR and MBL between both groups. We have selected 17 studies out of 1016 articles for qualitative and quantitative analysis. The data from 956 patients and 1779 implants were used with an overall mean clinical follow‐up of 3.88 years ranging from 1 to 8 years. Overall, the SR of extra‐short implants (93.12%) was lower than the observed in 6‐mm‐longer implants (95.98%); however, there was no statistical significance on these findings (P > 0.10). MBL analysis showed that extra‐short implants and the 6‐mm‐longer group presented an average of −0.71 and −0.92 mm after 1‐year respectively. Three years follow‐up showed MBL of −0.42 mm (≤6 mm) and −0.43 mm (>6 mm); 5 years follow‐up showed an MBL of −0.69 mm (≤6 mm) and −0.46 mm (>6 mm); and after 8 years of follow‐up, it was found an MBL of −1.58 mm (≤6 mm) and −2.46 mm (>6 mm). Within the limitation of this study, the results indicated that SR of extra‐short implants was similar to 6‐mm‐longer implants. Calcium sulfate is used as a synthetic graft material in orthopedics, plastic surgery, oncological surgery, and dentistry, and it has been used in a variety of clinical applications, such as the repair of periodontal defects, the treatment of osteomyelitis, maxillary sinus augmentation, and as a complement to the placement of dental implants. To carry out this systematic review, a bibliographic search was carried out. The PICO (Patient, Intervention, Comparison, Outcome) question was: Does the use of calcium sulfate as a material in guided bone regeneration in dentistry have better results compared to other bone graft materials? Finally, a case series is presented using the calcium sulfate for different procedures. Currently, the available literature on the use of calcium sulfate as a graft material in implant surgery is scarce, and what is available provides low-quality evidence. That is why more research studies on the subject are necessary to allow more comparisons and meaningful conclusions. After using Bond Apatite® in our case series, we can conclude that it is a useful and easy-to-handle material in implantology practice, but more controlled studies should be carried out in this regard to assess its long-term efficacy, especially in horizontal and/or vertical regeneration. With the spread of oral implant therapy, serious medical complications related to implant surgery are becoming a social problem. Although the number of complications after implant surgery in the edentulous jaw is decreasing in Japan, maxillary-sinus-related complications (MSRCs) have reached the highest number since 2012. It is essential to identify and eliminate possible predisposing risk factors for MSRCs at an early stage to prevent MSRCs. In this review article, we highlight the causal factors of postoperative complications with or without sinus augmentation for the maxillary molar region to achieve optimal treatment outcomes and reduce complications. In particular, we focus on anatomical variations that can cause the impairment of maxillary sinus drainage. Furthermore, we emphasize that the paradigm for personalized medicine for patients with a maxillary edentulous jaw by ENT specialist and dentist cooperation is shifting from the traditional assessment of maxillary sinus pathologies alone to the new assessment of anatomic variations that can cause the impairment of maxillary sinus drainage in addition to maxillary sinus pathologies. Straumann BLX is a novel implant system that has been proclaimed to provide an ideal primary stability in all types of bone. In the current study, the primary stability of the Straumann BLX implant systems with Straumann tapered effect (TE) implants have been comparatively assessed in bovine ribs by using a simultaneous sinus elevation and implant insertion model. In the study group, BLX (4.0 × 12 mm), TE (4.1 × 12 mm), BLX (4.5 × 12 mm), and TE (4.8 × 12 mm) were placed in each bony window, which resembles the sinus maxillaris. As a control, BLX and TE implants with the same sizes were inserted into the proximal diaphysis of the bovine ribs. A total of 40 implant insertions were performed. Stability was measured with resonance frequency analysis. In the study group, 4.8-mm TE implants showed significantly higher values compared with 4.5-mm BLX implants (P =.116). However, 4.0-mm BLX implants in the control group showed higher stability compared with 4.0-mm-diameter TE (P =.014). The primary stability of the BLX implants in the control group was significantly higher compared with the experimental group in both widths (P=.018 for BLX 4.0 mm and P =.002 for BLX 4.5 mm, respectively). The use of the TE design with a wide diameter in simultaneous implant placement with sinus lift could present higher implant stability quotient values and might be a more appropriate option for implant recipient sites with poor bone volume and quality. Introduction: One of the most challenging cases in implantology is severely atrophic alveolar ridges which do not have adequate bone for implant placement. One common treatment option for dealing with this challenge is LeFort I osteotomy with inlay bone graft which allows implant insertion and correction of the maxillomandibular relationship simultaneously. Although this method has a high success rate, it is somewhat invasive and has potential complications in some cases. In recent decades, implant surgeons have focused on less invasive and more predictable treatment options for bone augmentation and implant rehabilitation. Now a question comes into mind when there are not much discrepancies in maxillomandibular skeletal relationship and inter arch distance, do we really need LeFort I down-grafting and take its risks? Methods: In this study, we present a case who did not need any change in the skeletal relationship of the jaws and inter arch space based on examination and prosthetic consultation, so we implemented bilateral simultaneous sinus and nasal floor augmentation instead of LeFort 1 interpositional inlay bone graft. Results: Six months after the surgery, new radiography showed adequate height and width of augmented bones gained in and under sinuses and nose floor. So there is no limitation on choosing suitable implant lengths and diameters. Conclusion: This study indicated successful augmentation of severely resorbed maxillary alveolar ridge with bilateral sinus and nose floor grafting technique instead of LeFort I interpositional bone graft in a case who didn’t have more discrepancies in skeletal and inter arch space.

Maxillary sinus floor augmentation (MSFA) is among the more studied bone-guided regeneration procedure in the literature. The MSFA could be considered the gold standard procedure for bone-guided regeneration as it provides suitable functional and aesthetic solutions to alveolar ridge atrophy due to tooth loss.Purpose: This study aimed to conduct a detailed histomorphometric evaluation of collagen production in SFAs bone-guided regeneration, using nano-hydroxyapatite/ß-tricalcium phosphate (nano-HA/ß-TCP) composite.Patients and Methods: A 52-year-old female had the left upper second premolar condemned due to periodontal disease, then a tooth implant replacement was planned. Due to maxillary sinus pneumatization, the MSFA had to be done before implant placement. Nano-HA/ß-TCP composite (2g) was used in the MSFA procedure. After nine months of the healing process, during the Cone Morse implant installation process, bone samples were collected for histologic analysis (sirius red, hematoxylin/eosin, polarized microscopy). Six months after implant installation, a ceramic crown was installed according to the patient’s request.Results: Proper masticatory function and aesthetics were re-established. The histomorphometric evaluation indicated that nano-HA/ß-TCP composite did not show any area devoid of cellular activity in sirius red or hematoxylin/eosin staining and the percentage (%) of new bone collagen fibers was achieved using polarization technique evaluation.Conclusion: According to these results, nano-HA/ß-TCP composite presented clinical and histomorphometric properties suit to be used as bone-guided regeneration biomaterial in MSFA. With increasing experience and in an attempt to shorten overall treatment times, implant placement in combination with tooth extractions and sinus lift procedures has become popular. In both cases, primary stability has to be achieved by either engaging apical and oral regions of trabecular bone or by engaging residual host bone beneath the sinus cavity. Extraction sites were formed by pressing a root analog into homogeneous low density polyurethane foam which was used as bone surrogate while a 3 mm thick sheet of medium density foam was used for mimicking a sinus lift situation. Two types (n = 10) of bone level implants with a conventional tapered design and a cervical back taper (NobelActive; control) and a novel design characterized by a shift in core diameter and thread geometry (AlfaGate; test) were placed in these models following conventional osteotomy preparation. Insertion torque was measured using a surgical motor and primary stability was determined by resonance frequency analysis. Statistical analysis was based on Welch two sample t tests with the level of significance set at α = 0.05. In sinuslifting, NobelActive implants required significantly higher insertion torques as compared to AlfaGate (p = 0.000) but did not achieve greater implant stability (p = 0.076). In extraction sites, AlfaGate implants showed both, significantly higher insertion torques (p = 0.004) and significantly greater implant stability (p = 0.000). The novel implant design allowed for greater primary stability when being placed in simulated extraction sockets and sinuslift situations. While in extraction sockets the position of condensing threads in combination with an increase in core diameter is beneficial, the deep cervical threads of the novel implant lead to superior performance in sinuslift situations.

Objective: The aim of this study was to evaluate the clinical and radiographic outcomes of the "vertically expander screw" (VES) technique as a novel approach for maxillary sinus floor elevation and simultaneous implant placement. Materials and Methods: Forty-four patients (26 females, 18 males) received 71 implants with simultaneous transalveolar sinus floor elevation via the VES technique. The threaded bone expanders were used to elevate the sinus floor as well as bone expansion. The patients were followed-up on a regular basis, and final clinical and radiographic examinations were performed at least 24 months following functional loading. Statistical analysis was conducted using the Chi-square test, the correlation coefficient, and the independent t test (p < 0.05, 95% confidence interval). Results: All implants showed successful osseointegration. The mean marginal bone loss (MBL; 0.38 ± 0.75 mm) at the site of premolars was not significantly different from the value at the site of molars (0.17 ± 0.50 mm). The mean intrasinus bone gain (IBG) was not significantly different (3.47 ± 1.22 mm and 4.11 ± 1.67 mm, respectively; p = 0.92). Evaluation of implant success index (ISI) score revealed no difference between the premolars and molars (p = 0.12). Conclusion: Despite the limitations of the present study, it seems that the VES technique and simultaneous implant placement in carefully selected cases may result in promising outcomes. Clinical Relevance: Insufficient alveolar bone height and poor bone quality often complicate implant placement in the edentulous posterior maxilla. Abstract Background In locations where the alveolar bone height is low, such as at the maxillary molars, implant placement can be difficult, or even impossible, without procedures aimed at generating new bone, such as sinus lifts. Various types of bone graft materials are used after a sinus lift. In our study, a three-dimensional image analysis using a volume analyzer was performed to measure and compare the volume of demineralized bovine bone mineral (Bio-Oss®) and carbonate apatite (Cytrans®) after a sinus lift, as well as the amount of bone graft material resorption. Patient data were collected from cone-beam computed tomography images taken before, immediately following, and 6 months after the sinus lift. Using these images, both the volume and amount of resorption of each bone graft material were measured using a three-dimensional image analysis system. Results The amount of bone resorption in the Bio-Oss®-treated group was 25.2%, whereas that of the Cytrans®-treated group was 14.2%. A significant difference was found between the two groups (P < 0.001). Conclusions Our findings indicate that the volume of bone resorption was smaller in the Cytrans®-treated group than in the Bio-Oss®-treated group, suggesting that Cytrans® is more promising for successful implant treatments requiring a sinus lift.

Objective: Many authors have tried to face the anatomical limitations resulting from maxillary bone atrophy. Up to five millimeters bone height, the lateral sinus floor elevation is the most commonly used and validated strategy to achieve the prosthetic rehabilitation. However, the disadvantages of this technique are its invasiveness and delayed rehabilitation. The aim of this paper was to assess 5 years clinical outcome of implants placed with a technique that allows the percrestal sinus floor elevation and the immediate implant placement. Materials and Methods: 30 transcrestal sinus floor elevations with immediate implant placement were performed in severely atrophic maxillae. Implant survival, marginal bone level variation, harvested bone height variation and periodontal indices were assessed. Results: After a five year follow up none of the thirty implants were lost. The mean value of vertical harvested bone loss was 5%. The mean crestal bone loss was -0.33 mm (Standard Deviation (SD) 0.11 mm). The mean value of periodontal indices was respectively: PD 1.22 mm (SD 0.72 mm), PI 17.47% (SD 15.01 mm), BOP 9, 87 %.,( SD 19.17 mm). Conclusion: The results obtained are comparable with success criteria in implant rehabilitation. The reported technique proved to be successful in the population observed, with minimal trauma and reduced invasiveness.

The purpose of this systematic review was to analyze sinus lifting procedures and to compare the efficiency of this treatment associated with the second generation of platelet-rich fibrin related to its effects on bone gain and to clarify the regenerative efficacy in sinus lift procedure, whether alone or as a coadjutant to other bone graft materials. The PICOT question was, "In clinical studies with patients needing a maxillary sinus lift (P), does the use of PRF either alone (I) or in conjunction with other biomaterials (C) improve the clinical outcome associated with bone gain and density (O), with at least three months of follow-up (T)?" An electronic search was conducted in the MEDLINE (PubMed), Science Direct, and Scopus databases through a search strategy. A total of 443 articles were obtained from the electronic database search. Sixteen articles met all criteria and were included in this review. Within the limitation of this study and interpreting the results carefully, it was suggested that a higher risk for implant failure after a sinus elevation might be seen in patients with residual bone ≤4 mm, and PRF application was effective, suggesting reducing the time needed for new bone formation. Introduction: The purpose of this monocentric retrospective observational investigation is to evaluate the implant failure rate observed in an oral surgery department and analyze the risk factors associated with them. Preventative measures will be suggested to reduce the incidence of implant failure. Material and method: All implants removed between 2014 and 2020 were analyzed. The main criterion assessed was the overall failure rate over 6 years of activity;the secondary criteria were the risk factors associated with implant failure. Results: 12 out of 376 implants placed between 2014 and 2019 in 11 patients (mean age: 55.5 ±11.5 years);sex ratio M/F=5/6) were removed, for an overall failure rate of 3.11%. The majority, 83% (10/12) of the lost implants, were in the maxilla, while only 17% (2/12) were placed in the mandible. The main risk factors identified were: a III−IV bone type density (75%, 9/12), pre-implant sinus lift surgery (42%, 5/12) smoking (8.3%, 1/12), surgical site infection (8.3%, 1/12) and rheumatoid arthritis (8.3%, 1/12). Conclusion: The failure rate observed in this oral surgery unit is consistent with the other international studies, confirming the compliance with good clinical practices of the healthcare team. The main objective of this study was to compare the clinical and histological results of synthetic bone substitutes used in maxillary sinus floor augmentation. A systematic review and meta-analysis was performed by searching the PubMed/MEDLINE, Scopus, and Cochrane databases for studies of the success of endosseous dental implant, implant failure, peri-implant marginal bone loss, newly formed bone, and residual bone graft material. After the search, 10 randomized clinical trials were included, with 4 studies ranging from low to unclear risk of bias were used for meta-analysis, comparing biphasic calcium phosphate (BCP) and deproteinized bovine bone (DBB). A greater amount of residual graft material (mean difference [MD]: −4.80 mm; 95% CI: −9.35 to −0.26; P =.040) was found in the DBB group. No other statistically significant differences were found between BCP and DBB for the remaining outcomes. Our results suggested that BCP can be considered a suitable alternative to DBB in maxillary sinus floor augmentation due to its clinical and histological results. [ABSTRACT FROM AUTHOR]

Objective: Dental implant placement in the posterior maxilla is challenging due to poor bone quality and volume. Displacement of implants into the sinus is a common complication. This study was conducted to evaluate the risk of displacement in relation to various factors. Materials and Methods: A retrospective observational study was conducted on 21 patients with 22 implants accidentally displaced into the maxillary sinus during a period of three years (November 2017-October 2002) The following data are collected: patient information, medical condition and smoking habit, geometry and size of the implant and anatomical region of displacement, time of displacement and removal after placement, available bone height and the associated sinus lift procedure. The surgical technique and type of anaesthesia for implant removal were also recorded. Results: The mean age of patients was 43±9 years with male to female ratio of 2:1. About 67% of patients were smokers and score II ASA (American Society of Anaesthesia). More than half of patients had oroantral communication and 1/4th had sinusitis. Most implants were in the first molar region (68%) followed by the second molar (18%). The displaced implants accompanied by closed sinus lift were 82%, in addition, 82% of cases dislodged intraoperatively. The height and diameter of implants were 8-10 mm (59%) and 4-5 mm (73%) respectively, 82% were tapered and 64% were associated with 2-4 mm of residual bone height. Conclusion and Recommendation: Dental implant displacement into the maxillary sinus should be expected when placing implant in the posterior maxilla with deficient bone volume and poor quality. Abstract Background The insertion of dental implants in the atrophic posterior maxilla can be a challenge. One option is to modify the residual native bone in preparation for proper, prosthetically-driven implant placement. The procedure presented in this study is called Navigated Antral Bone Expansion (N.A.B.E). This procedure employs the use of a navigation system to plan and guide the initial pilot drilling, bone expansion, final site preparation, and implant insertion. The aim of this study was to compare the distance between the alveolar ridge and the sinus floor measured before and after the surgery performed using the N.A.B.E. technique. Methods Thirty-seven partially edentulous patients who were candidates for implant supported restoration in the posterior maxilla, with a bone height ranging from 4 to 7 mm were enrolled. The N.A.B.E procedure was used to increase the bone height. Paired-samples t-test evaluated the distance between the alveolar ridge and the sinus floor measured before and after surgery. The occurrence of post-surgical complications, and the angular deviation between the planned osteotomy and the actual placed implant trajectories were evaluated. Results Out of the 37 consecutive patients enrolled in the study, 35 were considered in the data analyses.
