OBJECTIVE: to report and analyse time trends in cancer incidence among children (0-14 years of age), adolescents (15-19 years) and young adults (20-24 years) living in the Italian province of Trieste (2003 population, 242,000), between 1972 and 2003. DESIGN: population-based study of descriptive epidemiology. Setting and participants: the new cases of cancer diagnosed to the residents of the province of Trieste below 25 years of age were extracted from the database of the Trieste Cancer Registry (period 1972-1994) and from the database of the Friuli-Venezia Giulia Cancer Registry (period 1995-2003), according to the International Classification of Childhood Cancer (3rd edition). MAIN OUTCOME MEASURES: age-specific and age-standardized (Italian 1981 census population as standard) incidence rates, by diagnostic group, sex and period of diagnosis. Time trend in incidence was analysed by using a Poisson regression model adjusted for calendar year, sex and 5 year age-group, and was expressed as annual percent change (APC) in rates. RESULTS: in the period 1972-2003, the new cases of cancer were 168 in the age-group 0-14 years, 79 in the age-group 15-19 years and 111 in the age-group 20-24 years, while the person-years at risk were respectively: 1,050,027; 431,673; 496,450. The APC in the incidence of all cancers combined was 2.3% (IC 95% 0.6%-3.9%) in children, 4.4% (IC 95% 1.8%-7.1%) in adolescents and 5.1% (IC 95% 2.8%-7.5%) in young adults. Hodgkin lymphomas (APC =12.7%; IC 95% 2.6%-23.7%; 7 cases) in the age-group 0-14 years, skin melanomas and carcinomas (APC =8.2%; IC 95% 4.5%-12.0%; 49 cases) and central nervous system tumours (APC = 6.4%; IC 95% 1.5%-11.5%; 25 cases) in the age-group 15-24 years were the malignancies characterised by the highest increase in incidence. CONCLUSION: the increase in incidence rates observed in this study can be only partly explained by the small number of ascertained cases, by an improvement in diagnostic techniques and by more efficient registration. However, few environmental and hereditary factors are consistently associated with cancers affecting young people. Therefore, it is imperative to continue to carry out descriptive and analytical studies with primary prevention as the ultimate aim.

Skeletal remodelling is a continuous process during life and is still active also in extreme senescence. In the elderly, bone resorption often prevails over bone formation, causing bone loss and fragility. Elderly subjects are exposed to the risk of fractures, and loss of self-sufficiency, if considering that the proximal femur is the most frequently involved site. Bone remodelling can maintain circulating calcium within physiological ranges, at the expense of a substantial loss of this ion from the skeleton, particularly during senescence. Calcium metabolism is regulated at cellular/molecular level by a network of cytokines, growth factors, systemic hormones that act on bone in paracrine/autocrine/systemic fashion. Among the molecules involved in bone metabolism, parathyroid hormone (PTH) and vitamin D present some peculiar aspects during senescence. The osteometabolic features in a consistent group of centenarians have been evaluated. It results that a severe hypovitaminosis D was present in 99 out of 104 centenarians (25-OH vitamin D below 5 nmol/L), and that it plays an important role as a factor inducing a vicious circle involving hypocalcemia, secondary hyperparathyroidism, together with biochemical features indicating a consistent bone loss. Serum C-terminal cross-linking telopeptide, a specific marker of bone resorption was elevated in 92% of these subjects. Moreover, it has been found that several femoral fractures had occurred after 90 years of age. These data offer a rational for the possible prevention of elevated bone turnover, bone loss and consequently the reduction of osteoporotic fractures and fractures-induced disability, in the oldest olds, through the simple supplementation with calcium and vitamin D.

The Italian multicentric study on centenarians (IMSC) was aimed at assessing the level of preservation of the sense of taste, and at estimating to what extent the recognition of various gustative stimuli can give satisfaction and information regarding the surrounding environment for the centenarians. Taste sensitivity has been qualitatively established in a group of 126 Italian centenarians (mean age 101.9+/-1.4 years) and compared to that of a group of 100 elderly subjects (mean age 70.5+/-5.0 years). All the individuals included in the study groups had a mini mental state examination (MMSE) score larger than 19. The results revealed that taste sensitivity is significantly reduced in the centenarians; as a matter of fact, the elderly control subjects are able to perceive taste stimuli in 84.25+/-3.40% of the testing, while the centenarians were successful only in 74.23+/-6.19% of the experiments (P

Life expectancy has dramatically expanded, the global population has aged unprecedentedly and the number of centenarians has significantly increased. The analysis of autopsy series, cancer registries data, vital statistics and surveys specific to this age group allows unique observations with respect to incidence and prevalence, cause of death by cancer, frequency of primary tumours, metastatic patterns, occurrence of incidental cancers and of multiple primary tumours.Data analysis demonstrates how cancer incidence and cause of death present a threefold decrease after age 90 and reach 0-4% above age 100. In addition, the number of metastatic sites are remarkably less and incidental malignant tumours or multiple primary cancers are more frequent, indicating that cancer in centenarians carries a more indolent behaviour.The unique features of malignant tumours in this population is hereby presented and discussed following a systematic review of the available literature.Cancer in the very elderly is relatively uncommon as a disease and as a cause of death. Rationale. The prevalence of Borrelia burgdorferi in Ixodes ricinus ticks is one indicator for identifying the risk of infection in a given territory. Methods. Two PCR procedures were developed to detect B. burgdorferi infection in Ixodes ricinus nymphs. For the first method we used primers to amplify a fragment of the flagellin gene (fla) specific for B. burgdorferi sensu lato. In the second, we employed species-specific oligonucleotides directed against 16S rRNA which were able to specifically identify the genospecies of B. burgdorferi sensu stricto, B. garinii and B. afzelii, and another set for B. burgdoroferi sensu lato. Results. No amplification product was detected in uninfected control ticks whereas both techniques gave positive amplification for B. burgdorferi sensu lato in 73.8-79.0% of the field-collected ticks. The two PCR procedures were in agreement in identifying B. burgdorferi sensu lato infection. The use of flagellin primers seemed more sensitive than the oligonucleotide-based approach but it was also more complex for processing a large number of tick samples. The most prevalent species were B. burgdorferi sensu stricto and B. garinii and co-infected the same tick in 25% of PCR-positive samples. Only a few ticks proved to be infected by B. afzelii. We believe that use of PCR is accurate for prevalence studies. Moreover, it provides information about the geno-species of B. burgdorferi circulating in the tick vectors.

To investigate the relationship between air pollution and histologic type of lung cancer we conducted a case-control study among deceased men in Trieste, Italy. 755 cases of lung cancer and 755 controls were identified through the local autopsy registry. Information on smoking habits, occupation and place of residence was obtained from the subject's next-of-kin. Air pollution at the residence of each subject was estimated from the average value of total particulate at the nearest monitoring station. Spatial models and logistic regression were used to evaluate the effect of residence and air pollution on LC after adjustment for age, smoking habits, likelihood of exposure to occupational carcinogens and social group. The risk of lung cancer increased with increasing level of air pollution for all types of lung cancer combined (P = 0.022), for small cell carcinoma (P = 0.016) and for large cell carcinoma (P = 0.049). Compared with inhabitants of the residential area, residents of the rural area had a relative risk (RR) of 0.6 (95% confidence interval (CI): 0.4-1.0). The RR was 1.5 (1.0-2.2) for residents of the center of the city and 1.4 (1.0-2.1) for residents of the industrial area. In the center of the city the excess risk was almost completely restricted to small cell carcinoma (RR = 2.0) and to large cell carcinoma (RR = 2.6). In the industrial area the risk was increased especially for adenocarcinoma (RR = 2.1). These results provide evidence that air pollution is a moderate risk factor for certain histologic types of lung cancer.
