Objective: Monochorionic diamniotic twin pregnancies complicated by Type-III selective fetal growth restriction (sFGR) are at high risk of fetal death. The aim of this study was to identify predictors of fetal death in these pregnancies. Methods: This was an international multicenter retrospective cohort study. Type-III sFGR was defined as fetal estimated fetal weight (EFW) of one twin below the 10th percentile and intertwin EFW discordance of ≥ 25% in combination with intermittent absent or reversed end-diastolic flow in the umbilical artery of the smaller fetus. Predictors of fetal death were recorded longitudinally throughout gestation and assessed in univariable and multivariable logistic regression models. The classification and regression trees (CART) method was used to construct a prediction model of fetal death using significant predictors derived from the univariable analysis.Results: A total of 308 twin pregnancies (616 fetuses) were included in the analysis. In 273 (88.6%) pregnancies, both twins were liveborn, whereas 35 pregnancies had single (n = 19 (6.2%)) or double (n = 16 (5.2%)) fetal death. On univariable analysis, earlier gestational age at diagnosis of Type-III sFGR, oligohydramnios in the smaller twin and deterioration in umbilical artery Doppler flow were associated with an increased risk of fetal death, as was larger fetal EFW discordance, particularly between 24 and 32 weeks' gestation. None of the parameters identified on univariable analysis maintained statistical significance on multivariable analysis. The CART model allowed us to identify three risk groups: a low-risk group (6.8% risk of fetal death), in which umbilical artery Doppler did not deteriorate; an intermediate-risk group (16.3% risk of fetal death), in which umbilical artery Doppler deteriorated but the diagnosis of sFGR was made at or after 16 + 5 weeks' gestation; and a high-risk group (58.3% risk of fetal death), in which umbilical artery Doppler deteriorated and gestational age at diagnosis was < 16 + 5 weeks' gestation. Conclusions: Type-III sFGR is associated with a high risk of fetal death. A prediction algorithm can help to identify the highest-risk group, which is characterized by Doppler deterioration and early referral. Further studies should investigate the potential benefit of fetal surveillance and intervention in this cohort.

Objective To assess the performance of fetal magnetic resonance imaging (MRI) in detecting associated anomalies in fetuses diagnosed with isolated corpus callosal (CC) anomaly on multiplanar ultrasound evaluation of the fetal brain (neurosonography). Methods This was a multicenter, retrospective cohort study involving 14 fetal medicine centers in Italy, UK, Portugal, Canada, Austria and Spain. Inclusion criteria were fetuses with an apparently isolated CC anomaly, defined as an anomaly of the CC and no other additional central nervous system (CNS) or extra-CNS abnormality detected on expert ultrasound, including multiplanar neurosonography; normal karyotype; maternal age >= 18 years; and gestational age at diagnosis >= 18 weeks. The primary outcome was the rate of additional CNS abnormalities detected exclusively on fetal MRI within 2 weeks following neurosonography. The secondary outcomes were the rate of additional abnormalities according to the type of CC abnormality (complete (cACC) or partial (pACC) agenesis of the CC) and the rate of additional anomalies detected only on postnatal imaging or at postmortem examination. Results A total of 269 fetuses with a sonographic prenatal diagnosis of apparently isolated CC anomalies (207 with cACC and 62 with pACC) were included in the analysis. Additional structural anomalies of the CNS were detected exclusively on prenatal MRI in 11.2% (30/269) of cases, with malformations of cortical development representing the most common type of anomaly. When stratifying the analysis according to the type of CC anomaly, the rate of associated anomalies detected exclusively on MRI was 11.6% (24/207) in cACC cases and 9.7% (6/62) in pACC cases. On multivariate logistic regression analysis, only maternal body mass index was associated independently with the likelihood of detecting associated anomalies on MRI (odds ratio, 1.07 (95% CI, 1.01-1.14); P = 0.03). Associated anomalies were detected exclusively after delivery and were missed on both types of prenatal imaging in 3.9% (8/205) of fetuses with prenatal diagnosis of isolated anomaly of the CC. Conclusion In fetuses with isolated anomaly of the CC diagnosed on antenatal neurosonography, MRI can identify a small proportion of additional anomalies, mainly malformations of cortical development, which are not detected on ultrasound. OBJECTIVE Little is known regarding fetal growth patterns of monochorionic twins complicated by type 3 selective fetal growth restriction (sFGR). We aimed to assess fetal growth and Doppler patterns in type 3 sFGR across gestation and evaluate the effect of changing Doppler flow patterns on growth and intertwin weight discordance. METHODS We retrospectively reviewed all type 3 sFGR pregnancies managed at nine fetal centers over a 12-year time period. Higher-order multiples, major fetal anomalies, or other monochorionicity-related complications at initial presentation were excluded. Estimated fetal weights (EFW) for each twin pair in each of five gestational age blocks (16-20, 21-24, 25-28, 29-32 and above 32���weeks' gestation) were reviewed and compared to singleton growth as well as uncomplicated monochorionic twin growth. EFW and intertwin growth discordance were compared between pregnancies with normalization of umbilical artery Dopplers of the smaller twin and those with persistently abnormal Dopplers . RESULTS 328 pregnancies (656 fetuses) met the study criteria. In type 3 sFGR, the smaller twin was smaller than the average singleton fetus (z-score ranging from -1.52 at 16���weeks to -2.7 at 36���weeks) and the average monochorionic twin (z-score ranging from -1.73 at 16���weeks to -1.49 at 36���weeks), throughout the entire pregnancy, while the larger twin was larger than an average singleton fetus until 22���weeks' gestation and similar in weight to an average monochorionic twin throughout gestation. As pregnancy advanced, both twin's growth velocities decreased with the larger twin remaining appropriately grown and the smaller twin becoming more growth restricted. Intertwin growth discordance remained stable throughout gestation. In multivariable longitudinal modelling, normalization of fetal Dopplers was associated with better growth of the smaller twin (p=0.002) but not the larger twin (p=0.1), without affecting the intertwin growth discordance (p=0.08). CONCLUSION Abnormal fetal growth of the smaller twin in type 3 sFGR was evident from early in pregnancy, while the larger twin's growth remained normal throughout gestation. Normalization of Dopplers resulted in improved fetal growth of the smaller twin. This article is protected by copyright. All rights reserved.

AIM: To establish the provision of fetal surgery for myelomeningocele (MMC) worldwide. METHODS: Through the International Society for Prenatal Diagnosis (ISPD) Fetal Therapy Special Interest Group and the North American Fetal Therapy Network (NAFTNet), fetal therapy centres were surveyed (September 2017-June 2018) regarding availability of fetal MMC surgical repair, patient inclusion criteria, repair techniques, number of cases, and outcome reporting. Responses were summarised on an interactive map on the ISPD website. RESULTS: Forty-four of 59 centres responded (74.6%) of which 34 centres (77.1%) currently offered fetal surgery for MMC and seven centres (15.9%) were awaiting a first case after service set up. Patient inclusion criteria were similar and based on the Management of Myelomeningocele (MOMS) trial. Five centres (14.7%) operated beyond 26 weeks' gestational age, outside the MOMS criteria. Open fetal surgery was provided in 23 centres (67.6%), fetoscopic surgery only in five (14.7%), and six centres offered both types (17.6%). Neurosurgical closure was similar for open surgery but highly variable in fetoscopy surgery. The median number of cases per centre was 21 (range 1-253). CONCLUSIONS: Fetal surgery for MMC is now offered globally. Background/Aims: “Gentle” or “natural” cesarean sections are implemented to improve parental satisfaction. The objective of this study was to investigate whether there is a need for adopting such a gentle approach. Methods: We recruited those with low-risk pregnancies scheduled for elective cesarean delivery at term. Three questionnaires were administered to both partners: prior to delivery, 2-5 days after delivery, and 6 weeks postpartum. Questionnaires assessed the couple's expectations, fears, and satisfaction prior to and after cesarean section. Additionally, participants attended 2 structured interviews: one prior to delivery and one 6 weeks after delivery. Results: Fifteen couples underwent a “standard” cesarean section (group 1) and 6 underwent a “gentle” section (group 2). Overall, parental satisfaction with cesarean section was high. However, women in the standard group felt less involved in childbirth and both groups still preferred vaginal delivery in light of eventual future pregnancies. Participants in group 2 showed less fear of childbirth after delivery than prior to delivery. There were no differences in total questionnaire scores between groups or between mothers and partners. Conclusion: This study shows that there is room for improvement in satisfaction levels around cesarean section, even though satisfaction is generally high. A “gentle” cesarean section may help to achieve this.

The pregnant patient is a vulnerable subject, and even more so when a serious fetal condition is diagnosed. (Invasive) fetal therapy should only be offered when there is a good chance that the life of the fetus will be saved, or irreversible damage by the disease or disability is prevented. Following diagnosis of a potentially treatable condition, the patient needs to be referred to a center with sufficient expertise in diagnosis and all therapeutic options. Preferences of the physician towards one or another antenatal intervention is not at stake prior to that moment. When fetal therapy is justified--, it should be offered with full respect for maternal choice and individual assessment and perception of potential-- risks, and should be at the location where there is sufficient expertise. For therapies of unproven benefit, the absence of evidence must be disclosed, and therapy should only be undertaken with full voluntary consent of the mother. These ought to be undertaken within well designed and approved trials and only by experts in the treatment modality. Potential risks and eventual morbidities in case of therapeutic failure should be part of the counselling, neither-- should fetal therapy be presented as an alternative to termination of pregnancy.

The fetal and the maternal cardiovascular compartment undergo dramatic functional changes during pregnancy. In this thesis we examined the heart of fetuses with twin-to-twin transfusion syndrome (TTTS) and congenital diaphragmatic hernia (CDH) using two new ultrasound parameters of ventricular function : the myocardial performance index and speckle tracking derived myocardial strain. Fetal cardiac function was grossly abnormal in recipient fetus of TTTS, yet normalized within 6 weeks after therapy. Ultrasound based cardiac function assessment could not predict short term fetal survival after therapy, nor could it predict eventual further progression to full-blown TTTS in a pre-disease stage. Fetuses with CDH on the other hand, have normal myocardial function, yet smaller left ventricles leading to decreased left ventricular output. We showed that the lower output leads to decreased cerebral perfusion, yet without apparent impact on brain and cranial growth. On the maternal side, plasma volume strongly increases in pregnancy, in parallel with an increase in insulin-like-growth factor(IGF) II which is secreted at the level of the placenta. Experimental administration of IGF-II by continuous infusion leads to increases in plasma volume whereas decreasing IGF-II by reduction of the feto-placental mass leads to decreased plasma volumes. In contrast to IGF-II, the highly vasoactive peptide apelin decreases near term due to a faster elimination as a consequence of an increase in placental angiotensin-converting-enzyme 2. Our experiments with IGF-II and apelin substantiate an important role for the feto-placental unit in regulating maternal plasma volume expansion and (auto)regulating uterine perfusion and fetal growth.
