The gaps in academic standards between students receiving free school meals and their peers is significant. By the end of secondary school, the most disadvantaged students are on average two years of learning behind their better-off peers. Among girls, the difference is even more stark. These students often demonstrate weaker character/essential life skills such as a general lack of motivation, resilience and determination and have low attendance. The research centres on closing the attainment gaps for disadvantaged girls. The purpose is to investigate whether an intervention programme based upon research in the field of resilience could affect positive change in female students in order to impact upon their levels of achievement as well as their possible future aspirations, their resilience factors and retention at Sixth Form or College post Year 11. The thesis looks at the intrinsic factors that impact upon female students, such as values, motivations, aspirations and resilience, as well as extrinsic factors such as exam results, school, community and family. The research uses the Child and Youth Resilience Measure - 28 (CYRM28) and qualitative interviews to measure students' resilience levels and attitudes to school, achieving and aspiration before and after an intervention programme. The intervention programme focuses on developing qualities and the protective factors that contribute to resilience such as developing problem solving skills, having at least one supportive adult they can turn to within school, helping students to formulate plans for the future and supporting others through volunteering and developing selfsoothing techniques. The findings from this research suggest that academic resilience does impact both positively and negatively upon academic achievement, attitudes to learning and as a result future life chances and aspirations. They also show that resilience is complex and can be developed given the right conditions but is also something that can decrease depending on situations, experiences and interactions which happen both in and out of school. It opens up possibilities for future research such as how schools could consider developing strategies to build resilience and develop students' sense of identity and feelings of self-worth for a particular demographic who do not necessarily identify with a culture or belief system.

The author's published works are integrated throughout and critical discussion of methodological and theoretical aspects are expanded. The principal aims include optimisation of self-management of long-term conditions and the improvement of access to healthcare for marginalised groups. It also specifically examines improved quality of care through the delivery of appropriate communication skills, and the identification of reliable and valid outcomes for people affected with illness or disability. Broader aims include improved patient satisfaction with health care services and an evaluation of systemic aspects of care and patient, public involvement in research.

Gastrostomy feeding is a route of enteral nutrition for children with feeding difficulties. This study investigated caregiver experiences of the transition to gastrostomy feeding. A survey was administered to caregivers of children <18 years in a major pediatric center in Ireland. Experiences of decision-making, support, and adjusting to tube feeding were examined. Seventy-six caregivers participated. Median satisfaction with the information provided by the hospital was high. Almost half (48%) spoke to another caregiver of a child with a gastrostomy prior to their own child's gastrostomy insertion and most (88%) felt reassured by this. Concerns following insertion included managing the tube and their child's oral intake and feelings about the tube. The oral intake of 83% of children who had some intake prior to gastrostomy insertion did not change or increased following insertion. Most (89%) would make the same decision to insert the tube. Feelings associated with the transition included relief and stress. Gastrostomy tube insertion presents logistical and psychosocial challenges for caregivers. Empowerment is a key aspect of relational, partnership-based care, and has been highlighted as a key outcome of group antenatal care (gANC). In order to explore how gANC facilitates empowerment, a systematic search and framework analysis of evaluations of gANC was undertaken. Findings from retrieved papers were mapped onto Zimmerman’s theory of empowerment. This suggested that gANC facilitates empowerment through helping others, knowledge about resources, self-efficacy and perceived control. However, situational awareness, decision making and problem solving were less apparent, indicating that birthing people might be further empowered if these mechanisms were incorporated into their care.

The Irish Defense Forces (DF) responded to the COVID-19 pandemic and national public health crisis by deploying personnel to aid domestic civil authorities in medical and care settings, contact tracing, logistics, and operations. Current research on COVID-19 reveals increased psychological distress among frontline workers and the general public. Resilience has previously been associated with lower levels of psychological distress. This study sets out to test these associations, and to examine mental health differences between DF personnel deployed in Ireland on pandemic-related duties (DIPD) and non-DIPD. Participants were 231 DF members who completed the: Connor-Davidson Resilience Scale-10, Patient Health Questionnaire-9, Generalized Anxiety Disorder-7, Perceived Coronavirus Threat Questionnaire, Brief Trauma Questionnaire, Post-traumatic Stress Disorder Checklist-5, and Alcohol Use Disorder Identification Test. Independent t-tests revealed no differences between DIPD and non-DIPD on measures of psychological distress or on self-rated mental health prior to COVID-19 (PC19) and during COVID-19 (DC19). Results of multiple hierarchical regression analyses revealed that depression predicted lower levels of resilience, while multiple traumatic events predicted higher levels of resilience. The total adjusted variance explained by the model was 25%. Clinical and policy implications for improving access to psychological support within the DF and military populations are discussed. In undertaking a doctoral research project looking at the impact of Scottish secure care experience on the identity constructions of autistic young people, I began by carrying out an extensive literature review. This was to inform the research, by understanding what was already known about this topic, and where the gaps were. The literature review covered a number of areas, one of these being the history of secure care. To align with the theme of this special issue considering historical perspectives, the findings of this section of the literature review will be presented here. The paper begins by defining secure care, and the numbers of children this is relevant for. It provides a very brief summary of the history of residential childcare, before moving on to consider in some detail the main policy drivers which have influenced the direction of travel in secure care over the years. 25. Beliefs About Worry and Pain Amongst Adolescents With and Without Chronic Pain.

Objective: To explore beliefs about worries, beliefs about pain, and worries about pain held by adolescents with and without chronic pain.Methods: Adolescents with and without chronic pain aged 14-19 completed an online survey with free text questions about pain and worry. We collected demographics and used the Penn State Worry Questionnaire and Pain Catastrophizing Scale for Children to contextualize the qualitative data, which was analyzed with reflexive thematic analysis.Results: Eighty-one participants completed the survey, 36 with chronic pain and 45 without (mean age: 16.73). Compared to adolescents without chronic pain, adolescents living with chronic pain reported significantly higher general worry and pain catastrophizing. Thematic analysis generated two themes, "Worry changes perceptions of selfhood" and "Pain changes perceptions of selfhood." Each theme comprised two sub-themes showing how current and future identity trajectories were distorted by worry and pain. The theme "Pain changes perceptions of selfhood" also included a third sub-theme: "Pain impedes future working choices." Worry content as well as process was problematic in all adolescents. Adolescents experiencing chronic pain had specific, additional worries that pain reduces future career progression. These worries appeared highly salient and challenging.Conclusions: Adolescents may need greater support in recognizing worry as part of normative development. Adolescents in pain may benefit from specific support identifying and reducing how pain-related worries interact with their futures and careers, and from school-based and vocational interventions to reduce the realistic risks they face negotiating modern labor markets. Objective: The aim of this paper was to conduct a systematic review and meta-ethnography of qualitative studies examining the experiences of parents adjusting to life after the solid organ transplant (SOT) of their child.Methods: A systematic review of the literature was conducted to identify qualitative studies that examined this topic. The search retrieved 4,964 studies to review against inclusion criteria. A total of 21 studies were included in the meta-ethnography. A reciprocal translation was conducted to compare themes identified in each study. A line of argument synthesis was then conducted in order to integrate the similarities and differences between all of the studies into a new interpretative context.Results: The synthesis identified four themes: (a) adjusting to life after transplant, (b) factors that facilitate adjustment, (c) factors that disrupt adjustment, and (d) changes caused by transplant. As a result of the synthesis, a new interpretation of parents' experiences of adjusting to life after the SOT of their child was constructed. A summary figure is presented.Conclusion: This review found that while parents may struggle to adapt to life post-transplant and the demands of caring for their child, the experience can also result in the development of new skills, a new appreciation for life, and viewing the self in a more positive manner. A better understanding of parental experiences will promote the development of more effective interventions for the adjustment of parents and families to post-transplant life. Background: cancellation of planned surgery impacts substantially on patients and health systems. This study describes the incidence and reasons for cancellation of inpatient surgery in the UK NHS. Methods: we conducted a prospective observational cohort study over 7 consecutive days in March 2017 in 245 NHS hospitals. Occurrences and reasons for previous surgical cancellations were recorded. Using multilevel logistic regression, we identified patient- and hospital-level factors associated with cancellation due to inadequate bed capacity. Results: we analysed data from 14 936 patients undergoing planned surgery. A total of 1499 patients (10.0%) reported previous cancellation for the same procedure; contemporaneous hospital census data indicated that 13.9% patients attending inpatient operations were cancelled on the day of surgery. Non-clinical reasons, predominantly inadequate bed capacity, accounted for a large proportion of previous cancellations. Independent risk factors for cancellation due to inadequate bed capacity included requirement for postoperative critical care [odds ratio (OR)=2.92; 95% confidence interval (CI), 2.12–4.02; PConclusions: a significant proportion of patients presenting for surgery have experienced a previous cancellation for the same procedure. Cancer surgery is relatively protected, but bed capacity, including postoperative critical care requirements, are significant risk factors for previous cancellations.

Introduction The role of cognition is central to the fear avoidance (FA) model of chronic pain (CP), which emphasizes the importance of catastrophic pain interpretations and has been shown to be applicable to pediatric CP populations. However, while we know that pain catastrophizing plays a distinct role in influencing outcomes for children with CP, we know little about the specifics of how young people with CP experience catastrophizing and worry, as well as their general pain beliefs. Objective To qualitatively explore beliefs about and experiences of worry and pain among a purposeful sample of adolescents with CP. Methods Individual semistructured interviews with 12 adolescents (aged 12–17) with varying forms of CP attending an outpatient pain clinic in a general children's hospital. Relevant psychometric measures were administered orally to further inform the data. Data were analyzed using critical realist thematic analysis. Results Three themes and one subtheme were identified. Themes were 1) the worry ripple: mind, body, and behavior (subtheme: worry content: personal competence and health); 2) the pain mystery: living in a "scribble of black" ; and 3) the resist or avoid conundrum. Conclusions The findings highlight the need for psychological interventions targeting acceptance of uncertainty and also informing education on mind/body connections in adolescents with CP. Comparisons of in vivo (mouse stomach) and in vitro (laboratory culture) transcriptomes of Lactobacillus reuteri strain 100-23 were made by microarray analysis. These comparisons revealed the upregulation of genes associated with acid tolerance, including urease production, in the mouse stomach. Inactivation of the ureC gene reduced the acid tolerance of strain 100-23 in vitro , and the mutant was outcompeted by the wild type in the gut of ex- Lactobacillus -free mice. Urine analysis showed that stable isotope-labeled urea, administered by gavage, was metabolized to a greater extent in Lactobacillus -free mice than animals colonized by strain 100-23. This surprising observation was associated with higher levels of urease activity and fecal-type bacteria in the stomach digesta of Lactobacillus -free mice. Despite the modulation of urea hydrolysis in the stomach, recycling of urea nitrogen in the murine host was not affected since the essential amino acid isoleucine, labeled with a stable isotope, was detected in the livers of both Lactobacillus -free and 100-23-colonized animals. Therefore, our experiments reveal a new and unexpected impact of Lactobacillus colonization on urea hydrolysis in the murine gut.
