Introduction Infertility is mediated by several changes system‐wide. These changes are likely to cause other systems‐related pathologies, such as changes in systemic immune response, particularly inflammatory response can lead to cardiovascular diseases and breast cancer. Methods These morbidities can exist immediately or years after the diagnosis of infertility. Therefore, understanding the mechanism is important to move toward therapeutic interventions. Results Several extragonadal pathologies are reported due to infertility, as well as, how these might also contribute to reproductive disabilities. Detailed evidence are still not present that can give stronger result. Conclusion This review highlights some of the most frequent comorbidities that are seen in infertile women, hence requiring a need for complete clinical screening and care, as well as diagnosis and treatment in early stages., Several extragonadal pathologies are reported due to infertility, as well as how these might also contribute to reproductive disabilities. Women presenting subfertility or infertility need to be screened for abovementioned disorder. Adaptions in lifestyle might subsidize these effects.

Genital herpes is one of the sexually transmitted diseases that is reported with a greater incidence of primary and secondary recurrence. In this study, olive leaf extract was used for its antiviral properties to treat the infection. The randomized‐clinical trials using such a therapeutic approach are required in this field.

Background and Objectives: The Quercus (oak) fruit and especially the internal layer of the fruit (jaft) is rich in tannin (polyphenolic compound) and due to its astringent effects on the smooth muscles, we decided to evaluate this effect of extract of Oak Gall for vaginal tightening and rejuvenation in women with vaginal relaxation. Methods: This double blind clinical trial was performed on women with vaginal relaxation at Asali Hospital in Khorramabad from May to October 2012 and was candidate for colporrhaphy due to different types of pelvic organ prolapse. Sampling method was done on nonprobability basis according to the number of patients referred to clinic. Using blocked random allocation method, the participants were divided into 4 groups of 30 patients (three intervention group and one control group) who were age-matched. Then, an extract of Oak Gall in specified concentrations (10 - 15 and 20 grams in 1.5%, 2% and 2.5% jells) were administered to each intervention group. Finally, the obtained data were analyzed with Chi-square, Pearson, Fisher’s exact test and ANOVA using SPSS software version 19. Results: A total of 120 patients entered into the study with mean age of 36 ± 5.4 years. The evaluation of mean scores showed that sense of sexual satisfaction and orgasm, sense of vaginal tightness during intercourse, sense of vaginal lubrication and loss of vaginal dryness was significantly improved in patients in intervention group. Also other parameters including urinary incontinence, sense of vaginal mass exit, lack of orgasm, sense of air outflow during intercourse, sense of calmness after sex and loss of libido were significantly reduced in the group of patients who received jell 2.5% compared to other groups (P < 0.0001). Conclusion: Our findings show that using topical jell of Oak Gall not only is associated with contracting effects, but also improves vaginal lubrication and reduces its dryness. Regarding the lack of previous studies in this field and considering some limitations of this study, we suggest that Oak Gall to be used for management of women with vaginal relaxation.

Preeclampsia is a hypertensive disorder of pregnancy causing a high rate of both maternal and neonatal morbidity and mortality. Recent reports suggest that free radical-induced endothelial cell injury might be an etiologic factor in the pathogenesis of preeclampsia. The aim of this study was to determine the effect of antioxidants such as E and C vitamins on the prevention of preeclampsia. Materials and Methods: This clinical- trial study was conducted in the year 2009 in the clinical centers of Lorestan university of medical sciences. 160 nulliparous women with the age range of 18 to 35 without any risk factors for preeclampsia were divided into two groups. The first group received oral vitamin E 400 IU/day, and vitamin C 1000 mg/day with iron tablets during the 20th to 24th weeks of pregnancy while the control group received only iron tablets. Finally, the data was analyzed using descriptive statistics and frequency distribution tables. Results: Incidence of preeclampsia in the control group was 17.5% and in the case group it was 5% that was statistically significant. The means of systolic pressure before and after intervention in the control group were 99.43 7.8, and 12.44 19.1 while in the case group they were 99.3 8.3 and 106.12 13.25. Diastolic pressure means before and after intervention in the control group were 62.7 13.6 and 62.7 4.7. Conclusion: As the results showed, use of antioxidants can clearly reduce the risk of preeclampsia, and can reduce the mean systolic and diastolic pressure.

General practitioner, Information and drugs center of Lorestan health department Abstract Background: In this study we evaluate the outcome of pregnancy in the patient that received M. MR vaccine during M.MR vaccination program on Lorestan of fall 2003. Materials and methods: This is a descriptive epidemiologic study, we evaluated all of women that vaccinated with M.MR vaccine during pre and post conception. We collected and analyzed all of information like prognosis of pregnancy, congenital rubella syndrome, gestational age weigh of newborn. Results: In 92 pregnancy women infected with MMR vaccine, the prognosis of pregnancy clearly seen 88 pregnant women (96%) 73 (83%) had normal and healthy newborn infant and 5 (6%) had spontaneot abortion and 10 pregnant women (11%) had legal abortion. Mean gestational age at birth was 39.5 ± 2.1 and weigh of newborn was 3257 ± 535 grams. None of the live born infants had not congenital rubella syndrome. Conclusion: In this study we showed M.MR vaccination during pregnancy had not any complication pregnancy. But pregnancy yet is an contraindication for M.MR vaccibnation, vecause this patient theories have risk of congenital rubella syndrome.

To evaluate reasons for using and not using alternative medicines.The cross-sectional study was conducted in 2009 on women over 18 years of age in Khorramabad, Iran. The subjects were selected by using cluster and simple random sampling method. The data were recorded in a questionnaire that involved questions about the subjects' age, marital status, their opinions on their general health, and advantages and disadvantages of conventional and alternative medicine.Of the 1600 women initially selected, 1551(97%) represented the final sample. The mean age of the participants was 35.04±10.71 years. Overall, 435(28%) spoke of disadvantages of alternative medicine; 277(18%) about the advantages of alternative medicine; 523(34%) about the advantages of conventional treatments; and 316(20%) about the disadvantages of conventional treatments. The most prevalent reason for not using the conventional treatments was the cost factor in 159(50.3%). Trust in physicians 328(62.7%) and distrust in alternative medicine therapists 317(73%) were the most prevalent reasons for using conventional treatments and not using alternative medicine.Similar studies should be done on the reasons for using and not using each medication of alternative medicines separately.

Background: Post partum psychosis is a mood disorder which starts 2 or 3 weeks after delivery. In acute form, the symptoms of this disorder are so obvious that the sick mother doesn’t want to take care of her child in some cases, she wants to hurt either the child or herself or both (the child & the mother). It seems that there are some psychosocial factors that cause this problem to occur. Material and methods: The present study aims to survey the effect of social, cultural, economical and related factors on the post partum psychosis in 2003-2005. The study was cross sectional. The study included all the women who immediately return to the one of the clinical and educational centers for psychosis symptoms. All the women came to the clinics with one of the symptoms of mood disorder 3 to 12 weeks after their delivery. They were examined by psychiatrists. They were also interviewed by clinical psychologists. Those who recognized as having acute depression and psychotic symptoms were included in the study. The data were analyzed using “SPSS” software. Results: Most of the subjects were in the range of 20-30 years old. They were housewives. In most case, they gave birth to a female newborn and the delivery was done in a normal way. Most of the patients declared that they had unplanned pregnancy. 29.6% of the patients had mental disease background. 9.1% had major depression, 5.6% had dysthymic disorder. 18.5% of the subjects had physical disease like UTI, hyper thyroid and seizure. Conclusion: There was a significant relationship between post partum psychosis and variables such as age, education, job, and disease background, number of delivery and lack of child acceptance. It is suggested that for prevention of post partum psychosis, there should be a very suitable situation for pregnant mothers. Moreover by planning and designing educational programs, we could provide the proper background for child birth in the family.

Introduction: Considering the high incidence of preterm labor and its known complications, and that human myometrium contains receptors for human chronic gonadotropin (hCG) and that hCG can inhibit myometrial contractions on vitro, made me study treating effects of hCG on preterm labor. This clinical trial study was carried out to compare the effect of hCG, and magnesium sulfate (mgso4) on prevention of preterm labor Materials and methods: The study group included 101 women at weeks 22-35 of the gestation with intact membranes and cervical dilatation less than 4cm in preterm labor in referrals of obstetric – clinics in Khorramabad city during the years 2001-2003. The case and control groups were treated with hCG protocol of the dosage consisted of one single dose of hCG 5000 1v and 10000 units of hCG in 5oo dextrose as a drip of 20 drops per minute. Treatment continued until preterm labor arrested and then complications of mother and fetus were recorded. On condition of a successful cease of labor, the patients were discharged and followed uptill deliven time. Findings: Both groups were similar to each other regarding maternal age, gravity, and gestational age . Mean delay of labor was 22/68 days after administration of hCG and 24/27 days for (mgso4) that was not significant. The number of newborns that admitted in NICU were 9 (18%) for hCG and were 9(17/8%) for magso4. Mean weight at labor was 2334gr and 2287gr for hCG and mgso4. Background: Preterm birth is the leading cause of neonatal mortality and a major public health concern. This study aimed to determine factors influencing preterm labor in women referring to hospitals in Khorramabad in 2009-10. Materials and Methods: This case-control study was conducted on 524 mothers (262 subjects in each group) referring to hospitals in Khorramabad (west of Iran) in 2009-10, selected through consecutive sampling. A questionnaire was completed through interviews for each newborn after being examined. Collected data, were analyzed by using the logistic regression and odds ratios and a significant level of 5%. The logistic regression model sensitivity in detecting cases of preterm labor was 86%. Results: The frequency of preterm labor in mothers under 20 was 5.83 times higher than that in mothers in the age range of 20-35 (CI=2.99, 11.37, and P < 0.001). The odds ratios for preterm birth were highest for multiple pregnancies and preterm delivery in mothers with a history of obstetric complications as preterm delivery, low birth weight, stillbirth and abortion is seen more than that in other mothers. Conclusion: These findings will be useful for medical staff and public health workers in attempting to identification and management of risk factors and unfavorable social environment and provide early intervention to reduce the risk of preterm delivery.
