- Abstract
Objective: JDM is a rare chronic immune-mediated inflammatory disease with a predominant role for type I IFN responses. We aimed to determine the potential of Siglec-1 expression on monocytes as a novel IFN-inducible biomarker for disease activity monitoring and prediction of treatment response in patients with JDM. Methods: Siglec-1 was measured by flow cytometry on circulating monocytes of 21 newly diagnosed JDM patients before start of treatment and, for 10 of these, also during follow-up. The expression levels of five type I IFN-stimulated genes, MX1, IFI44, IFI44L, LY6E and IFIT3, were measured by RT-qPCR to determine the IFN signature and calculate an IFN score. IFN-inducible plasma proteins CXCL10 and galectin-9 were measured by multiplex immunoassay. Results: Siglec-1 and IFN score were increased in JDM patients compared with controls and correlated with clinical disease activity. Stratification of patients by Siglec-1 expression at diagnosis identified those with high Siglec-1 expression as having a higher risk of requiring treatment intensification within the first 3 months after diagnosis (55% vs 0% of patients, P = 0.01). Siglec-1 expression strongly correlated with plasma levels of previously validated biomarkers CXCL10 (rs = 0.81, P < 0.0001) and galectin-9 (rs = 0.83, P < 0.0001), and was superior to the IFN score in predicting treatment response (area under the curve 0.87 vs 0.53, P = 0.01). Conclusion: Siglec-1 on monocytes is a novel IFN-inducible biomarker in JDM that correlates with clinical disease activity and identifies patients at risk for a suboptimal treatment response. Further studies are required to validate these findings and their clinical potential.

Sinds de invoering van de Algemene Verordening Gegevensbescherming (AVG) in 2018 staat iedere onderneming die persoonsgegevens verwerkt, voor de uitdaging om aan deze wetgeving te voldoen en te blijven voldoen. Een persoon wiens gegevens worden verwerkt heeft rechten op deze gegevens, deze rechten staan beschreven in hoofdstuk 3 van de AVG “Rechten van de betrokkene”. De onderneming die persoonsgegevens verwerkt zal als verwerkingsverantwoordelijke gehoor moeten geven aan deze rechten en aan moeten kunnen tonen dat ze deze rechten conform de AVG faciliteren en wel met zo min mogelijk fouten en zonder te veel inspanning. Het verwerken van persoonsgegevens vindt plaats binnen het uitvoeren van bedrijfsprocessen en een kant en klare methode voor het operationaliseren van deze rechten binnen de dagelijkse bedrijfsprocessen van de onderneming is daarom wenselijk. In dit onderzoek is getracht hier een methode voor aan te reiken via het gebruik van privacy services. Allereerst zijn privacy services uit hoofdstuk 3 van de AVG geïdentificeerd; vastgelegd in een conceptueel model en getoetst aan modellen binnen de bestaande literatuur. Door dit conceptuele model, met de gevonden concepten en relaties, vervolgens toe te passen op een primair bedrijfsproces binnen een onderneming middels een case study, wordt getoetst of dit een bruikbare methode is bij het voldoen aan de wetgeving van de AVG en het kunnen aantonen daarvan. De bevindingen van de case study worden gegeneraliseerd als een methode voor het operationaliseren van de wettelijke eisen van hoofdstuk 3 van de AVG.

Background: Experimental and clinical data suggest that reducing inflammation without affecting lipid levels may reduce the risk of cardiovascular disease. Yet, the inflammatory hypothesis of atherothrombosis has remained unproved. Methods: We conducted a randomized, double-blind trial of canakinumab, a therapeutic monoclonal antibody targeting interleukin-1β, involving 10,061 patients with previous myocardial infarction and a high-sensitivity C-reactive protein level of 2 mg or more per liter. The trial compared three doses of canakinumab (50 mg, 150 mg, and 300 mg, administered subcutaneously every 3 months) with placebo. The primary efficacy end point was nonfatal myocardial infarction, nonfatal stroke, or cardiovascular death. RESULTS: At 48 months, the median reduction from baseline in the high-sensitivity C-reactive protein level was 26 percentage points greater in the group that received the 50-mg dose of canakinumab, 37 percentage points greater in the 150-mg group, and 41 percentage points greater in the 300-mg group than in the placebo group. Canakinumab did not reduce lipid levels from baseline. At a median follow-up of 3.7 years, the incidence rate for the primary end point was 4.50 events per 100 person-years in the placebo group, 4.11 events per 100 person-years in the 50-mg group, 3.86 events per 100 person-years in the 150-mg group, and 3.90 events per 100 person-years in the 300-mg group. The hazard ratios as compared with placebo were as follows: in the 50-mg group, 0.93 (95% confidence interval [CI], 0.80 to 1.07; P = 0.30); in the 150-mg group, 0.85 (95% CI, 0.74 to 0.98; P = 0.021); and in the 300-mg group, 0.86 (95% CI, 0.75 to 0.99; P = 0.031). The 150-mg dose, but not the other doses, met the prespecified multiplicity-adjusted threshold for statistical significance for the primary end point and the secondary end point that additionally included hospitalization for unstable angina that led to urgent revascularization (hazard ratio vs. placebo, 0.83; 95% CI, 0.73 to 0.95; P = 0.005). Canakinumab was associated with a higher incidence of fatal infection than was placebo. There was no significant difference in all-cause mortality (hazard ratio for all canakinumab doses vs. placebo, 0.94; 95% CI, 0.83 to 1.06; P = 0.31). Conclusions: Antiinflammatory therapy targeting the interleukin-1β innate immunity pathway with canakinumab at a dose of 150 mg every 3 months led to a significantly lower rate of recurrent cardiovascular events than placebo, independent of lipid-level lowering. Background: The aim of this study was to optimize a Question Prompt List which is designed to improve communication about the heart failure trajectory among patients, family members, and health care professionals. Methods: Data were collected in a two-round Delphi survey and a cross-sectional survey, including patients with heart failure, their family members, and health care professionals working in heart failure care in Sweden and the Netherlands. Acceptability for and demand of the Question Prompt List were assessed. Results: A total of 96 patients, 63 family members and 26 health care professionals participated in the study. Regarding acceptability, most of the original questions were found to be relevant by the participants for inclusion in the Question Prompt List but some cultural differences exist, which resulted in two versions of the list: a Swedish version including 33 questions and a Dutch version including 38 questions. Concerning demand, participants reported that they were interested in discussing the questions in the revised Question Prompt List with a physician or a nurse. Few patients and family members reported that they were worried by the questions in the Question Prompt List and hence did not want to discuss the questions. Conclusions: This Question Prompt List has successfully been adapted into a Swedish version and a Dutch version and includes questions about the HF trajectory which patients, their families, and health care professionals perceived to be relevant for discussion in clinical practice. Overall, patients and family members were not worried about the content in the Question Prompt List and if used in accordance with patients’ and family members’ preferences, the Question Prompt List can help to improve communication about the heart failure trajectory.

- Abstract
The role of social learning as a governance mechanism in natural resource management has been frequently highlighted, but progress in finding evidence for this role and gaining insight into the conditions that promote it are hampered by the lack of operational definitions of social learning and practical methods to measure it. In this article, we present a simple and flexible method to measure social learning, whether it has occurred and to what extent, among stakeholders in natural resource management. The method yields measurements of social learning that are visual, quantitative and qualitative. First, we elaborate our definition of social learning as a convergence of perspectives and outline how stakeholder perspectives in natural resource management can be described with Cultural Theory. Next, we provide a generic description of the method, followed by two examples illustrating its application to the domains of water and land management. Finally, we discuss relative strengths and weaknesses of the method and how it could be applied to improve our understanding of factors that contribute to social learning. - Abstract
Achtergrond: Het is van belang om een mogelijke Licht Verstandelijke Beperking (LVB) vroegtijdig te signaleren bij kinderen, zodat tijdig de juist hulp geboden kan worden en ernstige gedrags- en emotionele problemen voorkomen kunnen worden. De school kan een belangrijke rol spelen in de vroegtijdige signalering, maar er is nog geen theoretisch onderbouwd en gevalideerd instrumentarium beschikbaar om de scholen hierbij te helpen. In de DSM-5 wordt bij de definiëring van een verstandelijke beperking meer nadruk gelegd op de term ‘adaptief functioneren’. Een verstandelijke beperking verwijst naar gebrekkige algemene mentale vermogens die invloed hebben op het adaptief functioneren binnen drie domeinen (conceptueel, sociaal en praktisch). Deze drie domeinen bepalen hoe goed een individu omgaat met dagelijkse taken. Onderhavig onderzoek is gericht op de validering van de nieuw ontwikkelde Schaal Adaptief Functioneren (SAF; totaalschaal 15 items; drie subschalen: conceptueel, sociaal en praktisch), een screeningsinstrument dat ingezet kan worden om een risico op LVB mee te signaleren. Indien de score op de SAF laag is, kan vervolgdiagnostiek ingezet worden om uit te wijzen of er bij het kind daadwerkelijk sprake is van een LVB. Methode: De dataverzameling heeft plaatsgevonden in twee fases: de ontwikkelfase en de toepassingfase. Tijdens de ontwikkelfase (fase 1) hebben leerkrachten van basisscholen in Amsterdam en Almere via een online vragenlijst de SAF, SDQ, en Vineland Adaptive Behavior Scales (VABS) ingevuld voor leerlingen in hun klas. Tijdens deze fase zijn SAF-gegevens ingevuld over 1047 leerlingen (gemiddelde leeftijd = 8.9 jaar). De SAF is tevens na twee maanden nogmaals ingevuld over 756 leerlingen. Tijdens de toepassingsfase (fase 2) hebben leerkrachten via het Hart & Ziel leerlingvolgsysteem SDQ en cito gegevens ingevuld. De SAF werd door de leerkrachten ingevuld wanneer de leerling laag scoorde op de ‘pro-sociaal gedrag’ subschaal van de SDQ en op tenminste één van de cito-onderdelen en de leerling geen relatief hoge score (A of ii) had op de cito-onderdelen. Tijdens deze fase zijn SDQ en cito-gegevens ingevuld over 685 leerlingen en SAF-gegevens over 317 leerlingen. Tot slot zijn van deze groep 45 leerlingen verder onderzocht met de zogenaamde ‘Gouden standaard’, oftewel, LVB-experts hebben bij deze kinderen beoordeeld of er sprake was van een LVB op basis van de resultaten op een uitgebreide intelligentietest (VALT) en na een gesprek met de ouders en/of leerkracht van de leerling. Resultaten: De resultaten van huidig onderzoek bevestigen de betrouwbaarheid (interne consistentie, test-hertest betrouwbaarheid, intra-beoordelaars betrouwbaarheid), validiteit (inhoudsvaliditeit, meet invariantie voor kinderen met een autochtoon Nederlandse en allochtoon etnische achtergrond, constructvaliditeit), en er zijn aanwijzingen voor het discriminerend vermogen van de SAF, als screeningsinstrument waarmee gesignaleerd kan worden of er sprake is van een risico op LVB. Daarnaast zijn de sekse- en etnische verschillen in de SAF verkend. Vervolgonderzoek is in ontwikkeling en zal zich richten op het verzamelen van gegevens over meer leerlingen, zodat huidige gegevens gerepliceerd kunnen worden.

Objective: Self-perceived health status may be helpful in identifying patients at high risk for adverse outcomes. The Euro Heart Survey on Coronary Revascularization (EHS-CR) provided an opportunity to explore whether impaired health status was a predictor of 1-year mortality in patients with coronary artery disease (CAD) undergoing angiographic procedures. Methods: Data from the EHS-CR that included 5619 patients from 31 member countries of the European Society of Cardiology were used. Inclusion criteria for the current study were completion of a self-report measure of health status, the EuroQol Questionnaire (EQ-5D) at discharge and information on 1-year follow-up, resulting in a study population of 3786 patients. Results: The 1-year mortality was 3.2% (n = 120). Survivors reported fewer problems on the five dimensions of the EQ-5D as compared with non-survivors. Aims: Revascularization in patients with coronary artery disease changed over the last two decades, favouring the number of patients treated by means of percutaneous coronary interventions (PCI) when compared with coronary artery bypass grafting (CABG). Many randomized controlled trials (RCTs) have been performed to compare these two competing revascularization techniques. Because of the strict enrolment criteria of RCTs in which highly selected patients are recruited, the applicability of the results may be limited in clinical practice. The current study evaluates to what extent patients in clinical practice were similar to those who participated in RCTs comparing PCI with CABG. Methods and results: Clinical characteristics and 1-year outcome of 4713 patients enrolled in the Euro Heart Survey on Coronary Revascularization were compared with 8647 patients who participated in 14 major RCTs, comparing PCI with CABG. In addition, we analysed which proportion of survey patients would have disqualified for trial participation (n = 3033, 64%), aiming at identifying differences between trial-eligible and trial-ineligible survey patients. In general, important differences were observed between trial participants and survey patients. Patients in clinical practice were older, more often had comorbid conditions, single-vessel disease, and left main stem stenosis when compared with trial participants. Almost identical differences were observed between trial-eligible and trial-ineligible survey patients. In clinical practice, PCI was the treatment of choice, even in patients who were trial-ineligible (46% PCI, 26% CABG, 28% medical). PCI remained the preferred treatment option in patients with multi-vessel disease (57% in trial-eligible and 40% in trial-ineligible patients, respectively, P < 0.001); yet, the risk profile of patients treated by PCI was better than that for patients treated either by CABG or by medical therapy. In the RCTs, there was no mortality difference between PCI and CABG. In clinical practice, however, we observed 1-year unadjusted survival benefit for PCI vs. CABG (2.9 vs. 5.4%, P < 0.001). Survival benefit was only observed in trial-ineligible patients (3.3 vs. 6.2%, P < 0.001). Conclusion: Many patients in clinical practice were not represented in RCTs. Moreover, only 36% of these patients were considered eligible for participating in a trial comparing PCI with CABG. We demonstrated that RCTs included younger patients with a better cardiovascular risk profile when compared with patients in everyday clinical practice. This study highlights the disparity between patients in clinical practice and patients in whom the studies that provide the evidence for treatment guidelines are performed. © The European Society of Cardiology 2006. - Abstract
This article presents the results of an extensive coring programme and a test excavation at Wetsingermaar (province of Groningen, the Netherlands). The corings made clear that the archaeological site spans over 1.5 ha and islocated on the waterfront of a submerged Pleistocene ridge. The test excavation yielded ceramics, flints and other stone material, and archaeozoological remains. On the basis of its cultural remains it is concluded that Wetsingermaar constitutes an early site of the Funnel Beaker Culture (Trichterbecherkultur, TRB), predating the Horizon 1 of Drouwen TRB as defined by Brindley (1988b). This early phase is termed pre-Drouwen TRB (cf. Bakker, 1979: 115). The flint industry is similar to younger TRB assemblages, while the ceramic and archaeozoological evidence is difficult to interpret as a result of the fragmentary condition of the finds and the near absence of contemporaneous sites.

- Abstract
Het project WINN staat voor "Waterinnovatie Rijkswaterstaat". Binnen het WINN project worden in 2009 door Deltares, als uitvoerend partner binnen WINN, enkele innovatie-ideeën binnen het thema Water & Energie geëvalueerd op praktische haalbaarheid. Een van die ideeën is de mogelijkheid om golfdemping langs kades/oevers van binnenvaartkanalen te combineren met het opweken van energie en de constructie een functioneel onderdeel van een oeververdediging of dijkbekleding te laten vormen. Als in het Amsterdam-Rijnkanaal voor een grootschalige toepassing 200 Euro/m wordt geïnvesteerd, dan zal de terugverdientijd korter zijn dan 15 jaar waarbij de verkregen golfdemping gratis is. Als aan de golfdemping ook een waarde wordt toegekend, dan wordt de terugverdientijd veel korter dan 15 jaar. Veel belovende constructies zijn drijvers, golfaftopping en het direct toepassen van piëzo-elektrische materialen. Aanbevolen wordt de golven in het Amsterdam-Rijnkanaal in een monitoring programma te meten, andere locaties te onderzoeken en een pilot opstelling langs het Amsterdam-Rijnkanaal voor te bereiden.

- Abstract
OBJECTIVE: To determine the efficacy of 2 nurse-directed programmes of different intensity for the counselling and follow-up of patients hospitalised for heart failure, compared with standard care by a cardiologist. DESIGN: Multicentre randomised clinical trial (www.trialregister.nl: NCT 98675639). METHOD: A total of 1023 patients were randomized after hospitalisation for heart failure to 1 of 3 treatment strategies: standard care provided by a cardiologist, follow-up care from a cardiologist with basic counselling and support by a nurse specialising in heart failure, or follow-up care from a cardiologist with intensive counselling and support by a nurse specialising in heart failure. Primary end points were the time to rehospitalisation due to heart failure or death and the number of days lost to rehospitalisation or death during the 18-month study period. Data were analysed on an intent-to-treat basis. RESULTS: Mean patient age was 71 years, 38% were women, 50% had mild heart failure and 50% had severe heart failure. During the study, 411 patients (40%) were rehospitalised due to heart failure or died from any cause: 42% in the control group, and 41% and 38% in the basic and intensive support groups, respectively (differences not significant). The time to rehospitalisation or death was similar in the 3 groups: hazard ratios for the basic and intensive support groups versus the control group were 0.96 (95% CI: 0.76-1.21; p = 0.73) and 0.93 (95% CI: 0.73-1.17; p = 0.53), respectively. The number of days lost to rehospitalisation or death was 39,960 in the control group; this number was 15% less in the intervention groups, but the difference was not significant. However, there was a trend toward lower mortality in the intervention groups. In all 3 groups, more visits occurred than planned, which may have had a considerable effect on care, notably in the control group.
