This paper discusses the application of a qualitative methodology-forming ideal types by understanding-to the psychoanalytic psychotherapy of suicidal men. The paper focuses on a particular phase of the methodology: the validation through external expert replication. Data of cases from psychoanalytic treatment, analysed by the originating group in Germany, is analysed by a group in London, UK, which formed to undertake the task of blind comparison. We describe the contribution of this method in the field of psychoanalytic research, the method developed for generating data and the processes through which data are analysed. We demonstrate the comparative findings from the first (German) and second (UK) analyses and explore the meanings of the similarities and differences. The paper concludes with an appraisal of the strengths and limitations (in terms) of moderate levels of generalisation of results from this method for research in the field of qualitative psychotherapy research. Summary In 1995, a new team was specifically created at the Tavistock Clinic, London, UK, to work with people with learning disability of all ages using psychodynamic ideas, principles and methods. This was a new organization, born out of the growing interest and work in this field, work that had been going on at the Tavistock Clinic for many years. This paper describes brief work with pregnant teenagers, adolescent mothers, their infants and their wider families, a group who would not ordinarily seek treatment in child and adolescent mental health services. The work takes place in a community context in London and includes a home visiting service for young people unable to attend appointments in an advanced stage of pregnancy or in the early weeks of new motherhood. Accounts of emotional deprivation, disrupted care and separation in early childhood are often a feature of the history of many of these adolescents. Two case examples are considered in some detail. It is argued that for some young people, embarking on parenthood in adolescence is accompanied by the displaced attempt at repairing their own early relationships, as an alternative to the painful work of mourning. The management of apparent sleep problems is often not straight-forward because of a proportion of clients attending with a 'hidden agenda', requiring more in-depth long-term treatment for psychological problems. This paper looks at a sample of 100 consecutive referrals to a community sleep clinic in the East End of London and attempts to address the question of who comes to the sleep clinic and why. A proportion of clients responded to brief intervention of a behavioural nature but 19% were referred on to other services for psycho-therapeutic help. [ABSTRACT FROM AUTHOR]

Background: The paper reports on the collection of routine outcome data from an ongoing audit at a voluntary sector psychotherapy service for young people aged 12 to 21 years in London offering once-weekly psychotherapy. Method: The study uses intake and follow-up data from an ongoing audit of the psychotherapy service that started in 1993; 1608 young people were included in the study. Measures and areas of interest include the Youth Self Report Form, a significant other (SO) version of the Teacher’s Report Form, the Young Adult Self Report Form, and the Young Adult Behaviour Check List. Results: Percentage returns at intake were 94% (self), 66% (SO) and 80% (therapist), but became 35%, 21% and 38% at 3-month follow-up, and decreased further at 6- and 12-month follow-up. At all time points, significant other report rates were lower than self or therapist report rates. Young people who did not provide data at intake were more likely to have dropped out of treatment. Over the 15-year period of the audit, intake self-report data rates remained stable (about 94%) whereas SO and especially therapist report rates increased. However, there was a reduction in self, significant other and therapist report rates at 3- and 6-month follow-up. Conclusions: Collecting routine outcome data was compromised by a variety of factors, and systematic efforts, including introducing initiatives for participation are needed to increase follow-up data rates and improve their quality. The article presents information on the 10th annual British Association for Counseling and Psychotherapy research conference in London, England in May 2006. The conference had a wonderful atmosphere and gained enormously from a wide mix of cultures and nationalities. The exhibition hall was decorated with traditional cloths and items from some of the co-host organizations, making a fantastic display.

Background: Despite growing evidence of the effectiveness of cognitive-behavioural therapy (CBT) for psychosis, typically only about 50% of patients show a positive response to treatment. This paper reports the first comprehensive investigation of factors which predict treatment outcome.Method: In a randomised controlled trial of CBT for medication-resistant psychosis (see Part I) measures were taken at baseline of demographic, clinical and cognitive variables. Changes over time were assessed on the Brief Psychiatric Rating Scale and the relationship between potential predictor variables and outcome was investigated using analysis of variance and covariance.Results: A number of baseline variables were identified as predictors of good outcome in the CBT group. Key predictors were a response indicating cognitive flexibility concerning delusions (P = 0.005) and the number of recent admissions (P = 0.002). Outcome was less predictable in the control group and was not predicted by any cognitive variable.Conclusions: Good outcome is strongly predicted in patients with persistent delusions by a cognitive measure, while this was not the case in controls. Thus we argue that positive outcome in CBT is due in part to specific effects on delusional thinking. [ABSTRACT FROM AUTHOR]

This service evaluation study aimed to demonstrate the impact of implementing ward-based multidisciplinary therapy input on an acute psychiatric ward in a London hospital for a six-month period. The results indicated a high level of patient engagement with the project and referrals facilitated for patients following discharge. A reduction of incidents on the ward was reported over the project period compared to the previous year and there was less use of containment measures by staff by the end of the project. The article presents the author's response to the lecture delivered by Gerhard Wilke entitled "Second Generation Perpetrator Symptoms in Groups" at the 31st S. H. Foulkes Annual Lecture on May 18-20, 2007 in London, England. The author argues that Wilke's talk focuses on understanding how social trauma has been transmitted to the children and families of Second World War perpetrators and victims in Germany. He stresses that guilt and shame are felt by Second Generation perpetrator children concerning the involvement of their parents in the war. The author points out that Wilke's group analysis which focuses on the transpersonal approach affecting individuals and societies based on the transmission of trauma plays a significant role in the development of an effective psychotherapy.

Providers and service users associated with supported housing are increasingly using tools to measure outcomes against targets with indicators that equate change with progress. This article reports and reflects on a small research project undertaken by London South Bank University for Carr Gomm, using the World Health Organisation's Quality of Life Application Model to assess outcomes of support in relation to person-centred planning, the chosen principled support approach adopted by Carr Gomm. The evaluation is based on a small number of case studies which serve to prompt providers and commissioners of supported housing to ask what constitutes quality of life from the client's perspective, and how in turn this challenges the priorities inherent in the supported housing service. I describe how the Centre for Attachment-based Psychoanalytic Psychotherapy (CAPP) in London has gone about developing an attachment-based/relational curriculum for training psychotherapists. A key problem the training is meant to address is how to maintain a balance between enabling students to absorb research and clinical knowledge of psychological processes at the same time as they learn to maintain the flexibility necessary to address the uniqueness of each individual encounter in the consulting room. I describe the task of the training in terms of a linguistic metaphor: the training needs to ensure that students are theoretically multi-lingual while developing higher levels of fluency in the language of attachment-based/relational psychotherapy. In the pursuit of this approach, my colleagues and I developed a set of ten key propositions - the spine of the relational world - which we felt encapsulated the main theoretical assumptions of attachment-based practice that students need to understand and explore. This has been used to evolve a spiral curriculum, designed to help students engage constructively with the often confusing emotional aspects of clinical work, and with the multiplicity of clinical viewpoints that characterises contemporary psychoanalytic psychotherapy. This article presents an author's personal account on the Median Group, which conducts group analysis in London, England run by Patrick de Maré. Sessions conducted by the group are described, as well as the author's reaction towards the other members of the group. It was during these sessions that the author learned from de Maré that since the larger setting is less predictable than the smaller, more structure is necessary. Further, the group is the object of treatment and the members are the agents of this treatment. And finally, that the work of the members of the group is to humanize the group rather than socialize the individual.

In October 1997, 60 people gathered in the East End of London, England for the third Community Psychology Conference. The previous conference a year earlier in Glasgow had stimulated such exciting debate and a sense of shared purpose, that psychologists in Newham, Tower Hamlets, Haringey and Hackney were keen to take on the organization of the 1997 conference. Being an area with a history of dynamic and growing ethnic diversity combined with a well-defined voluntary sector, East End of London has a wealth of exciting opportunities for the practice of community psychology. During the first day, the emphasis was on project work that had been set up by clinical psychologists in East London. Three commentators from the fields of feminist psychology, social constructionist psychotherapy, and ethnography shared their observations and thoughts after hearing each presentation on how their theoretical perspectives might be used to extend and develop these projects. For many of the conference participants, one of the most important aspects of the conference had been networking and re-establishing contact with like-minded people.
