Introduction. Meningioma is one of the most common central nervous system tumors, accounting for 39.7 % of all primary brain tumors. The tumor originates from arachnoid meningothelial cells and is characterized by a wide range of histological types classified into 15 subtypes. The histological classification of meningiomas allows us to predict meningioma behavior and the risk of disease recurrence, as well as to define treatment strategies. However, clinical outcomes in histological subgroups of patients are often inconsistent with the histological grade of malignancy. Thus, a more reliable method is needed both to determine the histological subtype of the tumor and to predict the clinical course of the disease with the potential for targeted treatment.The purpose of the study was to summarize the available data on the effect of results of the genomic and proteomic tumor analysis on carcinogenesis with the relationship between the mutational changes and noninvasive diagnosis, treatment and the course of the disease.Material and Methods. Literature search was carried out in the PubMed, Elibrary system, publications were included mainly from 2010 to 2023. with the identification of articles by the keyword “genetic analysis of meningiomas” and synonyms. 550 articles were found, of which 55 were used to write a literature review.Conclusion. The study of the molecular genetic profile of meningiomas will improve the classification and establish a correlation with MRI data, the course of the disease and prognosis.

Aim. To identify predictors of cardiovascular events (CVEs) in patients with heart failure (HF) and P. aeruginosa pneumonia.Material and methods. The study included 92 patients with HF with mildly reduced or reduced ejection fraction and nosocomial pneumonia. Based on sputum or bronchoalveolar lavage microbiological examination, patients were divided into 2 following groups: group I — 51 patients with HF and P. aeruginosa pneumonia, group II — 41 patients with HF and pneumonia caused by other pathogens (Staphylococcus aureus, Streptococcus pneumoniae). During the hospital stay, the following morbidities were assessed: cardiovascular death, acute coronary syndrome, non-fatal cerebrovascular accident, pulmonary embolism, lower extremity venous thrombosis.Results. CVEs were significantly more often observed in patients with HF and P. aeruginosa pneumonia — 30 (58,8%) than in patients with HF and pneumonia caused by S. aureus or S. pneumoniae — 5 (12,1%). Complete blood count in group I patients revealed a decrease in hemoglobin level to 114 [95; 133] g/l, when compared with patients in group II — 139 [118; 150] g/l (p11×109/l (RR 2,412; 95% CI 1,399-4,158; p=0,048), CRP >133 mg/l (RR 3,115; 95% CI 1,611-6,025; p

Aim. To assess the contribution of Pseudomonas aeruginosa pneumonia to the development of cardiovascular events in patients with heart failure (HF).Material and methods. The study included 219 patients who were divided into a group of patients with pneumonia caused by P. aeruginosa (Group I) and a group of patients with pneumonia caused by S. aureus, S. pneumoniae (Group II). All patients underwent chest computed tomography (CT), blood tests, echocardiography, electrocardiography (ECG), 24-hour Holter ECG monitoring, pulse oximetry (SpO2), and Doppler ultrasound of the lower limb veins.Results. According to the study, cardiovascular events were more often observed in the group of patients with P. aeruginosa pneumonia as follows: death from cardiovascular events was registered in 11,4% of patients in group I and 0% of patients in group II (p=0,001); acute coronary syndrome — in 8,7 vs 0% (p=0,002), lower limb thrombosis — in 14 vs 1,9% (p=0,001). The occurrence of P. aeruginosa pneumonia in patients with HF increased the cardiovascular death risk by 8,8 times (95% confidence interval (CI): 1,2-65,7; p=0,005), acute coronary syndrome — in 8,0 times (95% CI: 1,1-60,2; p=0,014), lower limb thrombosis — in 9,6 times (95% CI: 1,3-71,2; p=0,004).Conclusion. Patients with HF and P. aeruginosa pneumonia are at greater risk of cardiovascular events than patients with pneumonia caused by S. pneumonia and S. aureus.

Аim: to analyze a series of cases of dysplasia and early gastric cancer developed in patients with autoimmune gastritis (AIG), and to identify their clinical and morphological features.Materials and methods. The study included six cases of detection of early gastric cancer on the background of AIG. Four out of six patients underwent endoscopic treatment, and in three out of six cases patients underwent endoscopic mucosectomy with dissection in the submucosal layer. One patient underwent endoscopic surgery in 2017, he continues annual dynamic follow-up without signs of tumour recurrence. A female patient with type 1 neuroendocrine tumour and mild dysplasia is awaiting treatment.Results. In five out of six patients with AIG, the localization of lesions prevailed in the stomach body, while all detected tumours were early ones, and according to the immunophenotype — of a gastric type. The presented cases of tumours were not accompanied by hyperplasia of neuroendocrine cells directly in the area of the tumour itself, but only in the surrounding gastric mucosa. Additionally, several cases of early gastrointestinal stromal tumours were found, which were characterized by low proliferative activity and had small sizes.Conclusion. The predominance of pseudopyloric metaplasia in the surrounding mucosa allows us to consider pseudopyloric metaplasia as a potentially significant change in the gastric mucosa of patients with AIG that may precede the development of gastric cancer.

The tumor lesions of the reproductive system organs carry the leading position in the structure of gynecological diseases. One of the rare tumors that can affect the organs of the reproductive system is pecoma, which is a tumor of mesenchymal origin. Because of it’s rare occurrence the gynecologists often misinterpret available clinical data and the results of patient`s examination, diagnosing and treating the tumor as a myoma before having received the results of the histological examination. Our article describes a clinical case of a patient who had a preliminary diagnosis of uterine fibroids, and because of this fact the planned surgical treatment was carried out. But a retroperitoneal tumor originating from the uterine vein was detected during the surgery. The diagnosis of sclerosing pecoma was established by histological and immunohistochemical studies. The article analyzes the clinical features of the patient`s disease, as well as the available results of instrumental methods of examination which can lead to final diagnosis of pecoma, also difficult issues of diagnosis and tactics of diagnostic search are noted.The literature data on the frequency of detection of pecomas, the features of their structure are presented. It is marked, that there is also a possibility of pecoma`s localization in various organs, for example, lungs, liver, kidneys, as well as in soft tissues. The possibility of multiple lesions – pecomatosis is emphasized. It is noted that among the pelvic organs the uterus is affected most often, but furthermore, the pelvic lymph nodes, the broad ligament, the omentum, the peritoneum of the pelvis and the rectum wall can be also involved in the pathological process. Risk factor of pecomas malignancy is discussed separately, the presence of three types of pecomas is determined. This fact significantly effects the probability of relapse of the tumor after surgical removal and, respectively, the tactics of postoperative follow‑up of such patients.

Aim of the study. To investigate the esophageal dysmotility, changes in the esophageal mucosa and the immune response depending on the type of refluxate in gastroesophageal reflux disease (GERD) patients.Material and methods. 68 patients with GERD were recruited: 28 (14 men; mean age, 45.74 ± 2.23 years) nonerosive reflux disease (NERD), 22 (15 men; mean age, 45.0 ± 3.24 years) erosive reflux disease (EE), 18 (13 men; mean age, 47.22 ± 2.95) Barrett’s Esophagus (BE). GERD patients underwent esophageal high-resolution manometry (HRM) with a 22-channel water-perfused catheter and Solar GI system (Medical Measurements Systems, Enschede, the Netherlands), 24-hour impedance and pH monitoring using the Ohmega Ambulatory Impedance pH Recorder (Medical Measurements Systems). We analyzed receptor characteristics of monocyte-derived macrophages in all groups of patients.Results. On HRM examination, we showed that DCI (distal contractile integral) in NERD patients was higher than in EE (p = 0.088) and BE (p = 0.076), also LES RP (lower esophageal sphincter resting pressure) in NERD patients was higher than in EE (p = 0.039) and BE (p = 0.012). The analysis of reflux characteristics showed that the total reflux time with pH < 4 for BE patients was longer than that for NERD and EE patients. An analysis of receptor characteristics of monocyte-derived macrophages showed the prevalence of CD25 and CD80 expression in all groups of patients.Conclusion. An analysis of the phenotype of macrophages derived from blood monocytes of GERD patients revealed a prevalence of М1 macrophages that was typical for the Th1 type of immune response. The degree of esophageal dysmotility was correlated with GERD severity and type.

This paper is aimed at investigating the microbiota of the esophagus and stomach in patients with gastroesophageal reflux disease (GERD) and healthy volunteers.Materials and methods. The study included 15 patients suffering from GERD and 6 healthy volunteers. All subjects underwent sampling of esophageal and gastric contents. The study of the microbiota in the obtained samples was performed by sequencing the 16S gene of ribosomal RNA (rRNA).Results. The most common types of bacteria in the esophagus and stomach in patients with GERD and healthy volunteers are found to be Bacteroidetes, Firmicutes, Actinobacteria, Proteobacteria, Fusobacterium. By comparing the relative contents of the main types of bacteria in the esophageal mucus and gastric contents, a significant decrease in the proportion of Proteobacteria was observed in patients with GERD as compared to healthy volunteers. The decrease in the relative number of bacteria belonging to the Acetobacteraceae, Bacillaceae, Bdellovibrionaceae, Clostridiales Insertae Sedis XI, Fusobacteriaceae, Moraxellaceae, Pasteurellaceae and Rhodocyclaceae families was observed in the esophagus in patients with GERD as compared to healthy volunteers. A higher bacterial content of the Leptotrichiaceae and Veillonellaceae families was detected in the stomach of patients with GERD.Conclusions. The obtained results indicate differences in the intraluminal microbiota of the esophagus and stomach in patients with GERD and healthy volunteers. Further study should be carried out to study the effect of changes in bacterial composition on those in the esophagus and stomach.

Abstract Background There are few reports of trigger wrist in the literature, as it is a rare pathology. Furthermore, various authors report that it is also hard to diagnose. It manifests with neurological symptoms at the affected wrist, which are usually induced by wrist movement, and can lead to partial or full loss of wrist function and sensitivity. The reason for reporting this specific case is that it was hard to differentiate between trigger finger and trigger wrist by clinical symptoms; no pathology was palpable or clearly seen on magnetic resonance imaging scan of the wrist. We propose a new diagnostic statement relative to this pathology. Case presentation A case of a 45-year-old white slavic man with trigger wrist associated with carpal tunnel syndrome, caused by a fibroma of the flexor tendon sheath, is reported. Despite careful clinical examination, it was not possible to differentiate between trigger finger and trigger wrist. Magnetic resonance imaging was performed to arrive at the right diagnosis but did not reveal any pathology in the wrist area. Carpal tunnel release was performed with a fibroma identified and excised. Wrist function was maintained well; no signs of carpal tunnel syndrome were seen at last follow-up. Conclusions Trigger wrist can be misdiagnosed as trigger finger even if adequate clinical evaluation is performed, and this can lead to inadequate treatment. We state that, when clinical symptoms of both trigger wrist and trigger finger are present, except painful palpation of the A-1 pulley region, the case should be referred to as trigger wrist.

Four species of Crambidae are reported from the territory of Omsk Region of Russia. Titanio ledereri (Staudinger, 1870) is new to Asian part of Russia, Loxostege peltalis (Eversmann, 1842), Catoptria verellus (Zincken, 1817) and Ostrinia peregrinalis (Eversmann, 1852) are new to West Siberia.

Anaplastic pancreatic carcinoma is a very rare histological subtype of pancreatic cancer, which is characterized by a more aggressive course than for conventional ductal adenocarcinoma. In this article, we consider the features of the clinical course, the difficulties of diagnosis in connection with the absence of pathognomonic signs of pancreatic cancer, and the morphological picture of this disease in a patient 60 years of age. This clinical case clearly demonstrates the complexities of the pancreatic carcinoma diagnosis, fast disease progression, and extremely unfavorable prognosis. It is important for clinicians to remember that this pathology often has a subclinical course, and the oncomarker levels are not always true.

The article analyzes the diagnosis and treatment of anti-GBM antibody disease (Goodpasture's syndrome) - a rare, severe progressive disease, associated with anti-glomerular basement membrane antibody-induced pulmonary hemorrhage and glomerulonephritis. The main problem of this pathology is late diagnosis, resulted in ineffective treatment. The article provides current information on the epidemiology, etiology and pathogenesis, diagnosis, and treatment of Goodpasture’s syndrome, as well as clinical case of a patient with this rare disease.

Clinical features of overlap autoimmune hepatitis/primary biliary cholangitis and morphological-proved sarcoid lesions (lungs, lymph nodes, skin) were performed. Data of long-term clinical observation presented in comparison with the results of laboratory datas, instrumental and morphological studies of liver tissue, lungs, skin. The modern aspects of pathogenesis of association autoimmune and granulomatous diseases arediscussed on the example of clinical cases of combination of cholestatic variants of autoimmune hepatitis and generalized sarcoidosis.

The results of a clinical study showing the importance of surfactant proteins A and D in assessing the activity and progression of idiopathic pulmonary fibrosis and chronic lung sarcoidosis are presented. The article presents a clinical observation of two patients with generalized sarcoidosis. The woman typical granulomatous changes in the lungs and lymph nodes combined with atrial fibrillation, kidney failure and hereditary thrombophilia, men with atherosclerotic coronary arteries, re-myocardial infarction, cholestasis, tubulointerstitial nephritis. The accession of systemic manifestations was accompanied by increase of level of angiotensin-converting enzyme in the blood serum, morphological examination of lung tissue in both cases there were high histological activity of vasculitis and granulomatous inflammation. Extrapulmonary symptoms regressed when conducting immunosuppressive therapy. Discusses modern aspects of diagnosis of sarcoidosis in clinical practice.

Crosswalk is an essential element of the transport system of any city. A priority assignment in any such crossing is to ensure the safety of pedestrians. Underground pedestrian crossings are much safer than crossings at another level, but they lose in attraction when it comes to cost. The cost of the construction of an underground pedestrian crossing often leads to project abandonment. The cost of construction can be reduced through the use of flexible corrugated metal structures instead of ordinary concrete structures. The stress-strain state of the structural shells need to be known to facilitate their rational design. The stress-strain state of flexible corrugated metal shells largely depends on the strength and deformation characteristics of the surrounding soil. Therefore, improving the characteristics of backfill soil is an urgent task in reducing the cost of construction for such tunnels. One way to improve the strength and deformation characteristics of soils is the use of reinforcement. Currently, there are a large number of reinforcement schemes and also associated reinforcing materials. One of the most prospective methods of soil reinforcement is the use of fibre filaments. Fibre reinforced soils have significantly higher strength and deformation characteristics in comparison with unreinforced soils. Numerical modelling of a tunnel shell made out of a corrugated metal structure was undertaken to evaluate the effectiveness of using fibre reinforced sand. Ordinary sand and sand reinforced with polypropylene fibres have been used as soil backfill. The calculation results for a pedestrian tunnel structure involving different strength and deformation characteristics of the backfill soil are presented in this article.

AIM. To assess the role of angiogenic factors (vascular endothelial growth factor (VEGF) and endothelin-1 (ET-1) in interstitial lung diseases (ILD), such as fibrosing alveolitis, sarcoidosis. MATERIALS AND METHODS. The blood levels of endothelial dysfunction and neoangiogenesis markers (ET-1 and VEGF) were investigated in 96 patients with different clinical forms of ILD at it different stages; the found changes were compared with the clinical and morphological manifestations of the disease. RESULTS. It has been ascertained that regardless of the clinical type of ILD, there is a correlation between the blood levels of VEGF and ET-1 and the intensity of lung neoangiogenesis, the expression of VEGF by the endothelium of newly formed blood vessels, the production of angiogenic factors, the degree of endothelial dysfunction, the extent of pulmonary fibrosis, the degree of pulmonary vascular remodeling, and the severity of pulmonary hypertension. The findings suggest that the markers of neoangiogenesis play an important role in the mechanisms of ILD progression. CONCLUSION. The study of these parameters in the blood may be used to clarify the activity and prognosis of ILD.

Aim. To examine clinical features of Sjogren's syndrome (SS) and morphological picture of the lesser salivary glands (LSG) in chronic hepatitis С (СНС). Material and methods. The examination of 42 patients with SS and chronic HCV infection (mean age 44.3 ± 13.7 years) has detected signs of chronic hepatitis and hepatic cirrhosis, respectively, in 31 (71.4%) and 11 (26.2%) patients. "Dry syndrome" was diagnosed by criteria of European SS Study Group. LSG biopsy of the lower lip was conducted in 23 (54.7%) of 42 patients. Results. The "dry" syndrome in CHC ran subclinically in 73.8% patients. Apparent symptoms of SS were seen primarily in middle-aged and aged women with CHC history over 10 years. The first signs of SS occurred in 25 (59.5%) patients 2.9+3.1 years prior to diagnosis of hepatic disease. All the patients had xerostomy. Xerophthalmia was recorded 1.5 times less frequently. In 16 (47.1%) patients with CHC "dry eye" and in 6 (17.6%) patients dry keratoconjunctivitis were detected. Pathohistological changes of LSG were diagnosed in 21 (91.3%) of 23 patients with CHC. In the majority of cases (86.9%) the glands exhibited insignificant inflammatory infiltration and advanced fibrosis. LSG in CHC is characterized by fibrosis prevalence over cell infiltration. 83.3% CHC patients had SS and other extrahepatic lesions. SS was most evident in 28.6% CHC patients with cryoglobulinemia. Conclusion. Registration of SS symptoms in CHC patients depends on targeted examination of patients with chronic HCV infection. The severity of the symptoms correlates directly with the infection duration and age of the patient. LSG lesions in CHC patients with SS are characterized by fibrosis predomination over cell infiltration.

Relevance. Despite significant improvement, the epidemiological situation regarding tuberculosis in Russian prisons remains not entirely favorable. It is advisable to assess the epidemiological situation regarding penitentiary tuberculosis in Russia in the post-pandemic period and against the backdrop of a special military operation. Aim: to assess the dynamics of the development of the tuberculosis epidemic situation and the state of provision of TB care in prisons of the Russian Federation in 2023. Scientific novelty: for the first time, a study of the epidemic situation of penitentiary tuberculosis was carried out in the context of a special military operation and in the period following the COVID-19 pandemic. Materials and Methods. Data from official (federal and departmental) statistical observation for 2000-2023 was studied. Results and Discussion. In 2023, there is an acceleration in the rate of decline in the incidence of tuberculosis in pre-trial detention centers to 15.8% (p0.001), while maintaining a stable rate of decline in correctional institutions. In general, in penitentiary institutions, the incidence of tuberculosis decreased from 580,4 in 2022 to 540,0 per 100,000 in 2023. The share of new cases of tuberculosis detected in penitentiary institutions among all new cases of tuberculosis decreased from 6.49% in 2022 to 5.63% in 2023. Mortality from tuberculosis changed statistically insignificantly (from 5.8 in 2022 to 4.5 per 100,000 in 2023; p=0.5). The prevalence of tuberculosis fell to 1982.3 per 100,000. The proportion of patients with tuberculosis combined with HIV in 2023 was 36.4%. Conclusion. The special military operation and the COVID-19 pandemic did not have a negative impact on the epidemiological situation of tuberculosis in Russian prisons. There may be some positive impact of the special military operation on the epidemic situation regarding tuberculosis, associated with the involvement of socially vulnerable sections of the population in economic and defense activities, which resulted in an increase in their standard of living and a decrease in the incidence of tuberculosis, which manifested itself in a decrease in the incidence of tuberculosis in pre-trial detention centers to a historical minimum.

This work is devoted to the problem of infrared monitoring of the metal temperature of the surface of superheater pipes. Monitoring is implemented on the basis of a thermal imager cameras that performs contactless monitoring of the surface temperature of the pipes of the wall superheater. The results of modeling the temperature distribution along the pipe of a wall-mounted superheater in the presence of contamination and without are presented. The calculation of the number of thermal imager cameras required for infrared monitoring is performed. The developed design solutions for the installation site of thermal imager cameras on a TP-82 type boiler have been confirmed experimentally.

Cell death is an important feature of the development of multicellular organisms, a critical factor in the occurrence of cardiovascular diseases. Understanding the mechanisms that control cell death is crucial to determine its role in the development of the pathological process. However, the most well-known types of cell death cannot fully explain the pathophysiology of heart disease. Understanding how cardiomyocytes die and why their regeneration is limited is an important area of research. Ferroptosis is an iron-dependent cell death that differs from apoptosis, necrosis, autophagy, and other forms of cell death in terms of morphology, metabolism, and protein expression. Ferroptotic cell death is characterized by the accumulation of reactive oxygen species resulting from lipid peroxidation and subsequent oxidative stress, which can be prevented by iron chelates (eg, deferoxamine) and small lipophilic antioxidants (eg, ferrostatin, liproхstatin). In recent years, many studies have been carried out on ferroptosis in the context of the development of atherosclerosis, myocardial infarction, heart failure, and other diseases. In addition to cardiovascular diseases, the review also presents data on the role of ferroptosis in the development of other socially significant diseases, such as COVID-19, chronic obstructive pulmonary disease. With the study of ferroptosis, it turned out that ferroptosis participates in the development of bacterial infection associated with the persistence in the host body of Pseudomonas aeruginosa. The review summarizes the recent advances in the study of ferroptosis, characterizing this type of cell death as a novel therapeutic target.

Reporting of the «transferred out» outcome leads to significantly worse treatment results of tuberculosis patients treated in the institutions of the Federal Penitentiary Service of Russia.The objective: to investigate the patients’ routes that led to reporting of the «transferred out» outcome, and propose measures to reduce the risk of this outcome.Subjects and Methods. We studied the places of registration and routes of 1,496 pulmonary tuberculosis patients registered for treatment in 2019 (new cases and relapses) with the outcome of «transfered out».Results. In the structure of transferred out patients, in 631/1,496 (42.2%) patients, the «transferred out» outcome was associated with their transfer between institutions within the penitentiary system. Additionally 462/1,496 (30.9%) patients were released in the region where they continued their treatment, 85/1,496 (5.7%) were released and moved to a different region, 301/1,496 (20.6%) were released from pre-trial detention , and 10/1,496 (0.6%) left Russia and moved abroad.Conclusion: the procedure should be developed to transfer the information about patients moving within the penitentiary system and beyond it. To motivate the patient immediately after the detection of tuberculosis, it is necessary to conduct individual conversations explaining the nature of the disease, importance and necessity of treatment. Tuberculosis patients in pre-trial detention centers should be provided with leaflets containing information about medical organization where patients can continue treatment, patients released from penitentiary units should be informed about the system of social adaptation and services available for tuberculosis patients.

he article is devoted to the problem of the phenomenon of prison loneliness. The relevance of this study is due to the fact that the phenomenon of prison loneliness underlies penitentiary stress, as a complex psychophysiological reaction of the body to being in a penitentiary space. One of the functions of the execution of punishment as a response of society to the unlawful actions of a criminal is punishment for a committed crime, which acts in the form of induction of mental suffering of a person as a result of his artificial separation from the usual social environment, the creation of prison loneliness. At the same time, the effect of transferring suffering from the body of the criminal to his soul, noted by M. Foucault, is realized. The purpose of the work involves the analysis of the phenomenon of loneliness in the conditions of places of deprivation of liberty. The main characteristic features of prison loneliness are described. It is shown that the punishment of a criminal by imprisonment, his forcible isolation from the mainstream society is based on the mechanism of creating artificial prison deprivation. It is noted that one of the attributes of punishing a criminal is the intentional creation of a special type of loneliness — prison, causing a high level of penitentiary stress. The article shows that two types of prison loneliness can be considered. The first one is proposed to be called “physical loneliness”, which is created by the physical isolation of the convict. At the heart of the generation of suffering of the personality of this type of loneliness is the effect of a violation of the human need inherent at the gene level to communicate with other members of society. This type of prison loneliness leads to a change in the psyche of the prisoner, the loss of communication skills, his slow emotional and mental degradation. The second type of prison loneliness (loneliness in the crowd) is characterized by the fact that in conditions of prison isolation, the prisoner is almost never left alone and is constantly under the influence of carriers of the penitentiary subculture, which has a powerful negative impact on the personality of the person serving the sentence. In an environment where interests are reduced to the satisfaction of vital needs, and interpersonal aggression is the norm of behavior, negative emotions prevail in the prisoner’s sense of self, the dominant of which will be a feeling of loneliness.

The objective: to identify promising directions for prevention of HIV incidence mortality on the basis of analysis of changes in HIV situation assessing the effectiveness of interventions in penitentiary units.HIV infection has traditionally been a critical issue for the penitentiary system worldwide and in Russia particularly. Recently, Russia has managed to achieve positive changes in the epidemic situation: by 2020, HIV incidence reduced to 852.8 per 100,000 (including 2,433.6 in remand prisons and 122.8 in correctional institutions); HIV prevalence was 10,512.8 per 100,000; HIV mortality was 98.3 per 100,000; HIV lethality was 0.9 per 100,000 people living with HIV.The proportion of HIV infection as a cause of death decreased from 32.0 in 2016 to 20.5 in 2020. The positive changes developed faster compared to civilian health care and occurred during the growing coverage with antiretroviral therapy which increased from 21.7% in 2014 to 89.6% in 2020. The following problems persist: high prevalence of HIV infection among female inmates (19,275.0 versus 9,769.2 among male inmates per 100,000), the high proportion of HIV/hepatitis C co-infection (49.0%; 95% CI 48.6-49.5), and the low coverage with hepatitis C treatment (0.5% of the total number of persons with co-infection). Promising directions for the prevention of HIV incidence and mortality are related to stable supply of antiretroviral drugs, improved patient adherence to treatment, and expanding treatment for HIV/hepatitis C co-infection.

The article is devoted to the analysis of the prisons totems and taboos phenomenon. It is shown that the modern penitentiary subculture, as a negative socioregulatory system is characterized by the presence of a complex system of taboos and totems. Researchers of the penitentiary subculture have repeatedly noted the similarities between the primitive and prison communities. This phenomenon is largely due to the fact that under conditions of imprisonment, a person’s outlook narrows, intelligence decreases, and their value orientation changes. The relevance of the study is due to the fact that, despite the presence of a complex system of prison taboos and totems among convicts, as the main manifestations of the penitentiary subculture, a very small number of scientific works are devoted to the genesis and understanding of this phenomenon. The aim of the article is to analyze the phenomenon of prison taboos and totems in the penitentiary subculture. The work shows that for the formation and preservation of a developed system of totems and taboos in the prison environment, a certain critical weight of such indicators as the size of the prison population and the severity of conditions of detention of prisoners is required. An important role in this is also played by such phenomena inherent in being in the penitentiary space, such as chronic stress, a sense of fear and uncertainty. The article reveals the general principles of the formation of prison totems and taboos. At the same time, it is shown that the leading mechanisms for the formation of prison taboos are the techniques of sympathetic and imitation magic. The author’s proposed classification of prison totems and taboos are presented. It is noted that the convict’s adherence to a certain image-totem contributes to his self-identification, determination of his place in the prison social hierarchy, and the formation of a criminal model of behavior. Practical examples of prison taboos and totems are given. It is shown that prison taboos are largely associated with the existence in the social hierarchy of the prison population of castes of «untouchables». The attention of readers is focused on the harshness towards those who violated prison taboos, which can manifest itself in physical punishment, transfer to the lower prison castes, in some cases — the death sentence. The practical significance of the work is the understanding the phenomenon of prison taboos and totems, that can affect the development of measures aimed at combating the manifestations of the penitentiary subculture in places of detention.

The article contains the experience of socio-philosophical analysis of the prison subculture phenomenon. The relevance of this study is due to the transformation of the basic culture of society traditional values under the influence of the criminal subculture, the development of its new socio-cultural spaces, observed in recent decades. The aim of the work is to analyze the evolution of the prison subculture from the perspective of its interaction with the culture of society in the context of the dialectical triad «thesis-antithesis-synthesis». The definition and the main characteristic features of the prison subculture are given. The main stages of its evolution are considered in the context of interaction with the basic culture of Russian society. It is shown that the main factors influencing the metamorphosis of the prison subculture are: the high number of convicts, the presence of passionarity in the Soviet penitentiary system, the detachment system of containing convicts, the unsuccessful results of the experiment carried out in the 60s of the twentieth century to reform the penal system, negative consequences economic and social reforms of the 90s. It is noted that as a result of long-term cultural genesis, the prison subculture has the following characteristics : 1) a rigid hierarchy of the social structure of the prison community with the existence of strata of «outcasts» 2) the presence of a developed system of prison mythology, customs, symbols and taboos 3) the presence of a «control center» as the group «Thieves in law», as well as channels of information transmission 4) high adaptability of the penitentiary subculture to external conditions 5) the presence of a catchy and attractive «thieves ‘ro¬mance» and «thieves’ ideology» 6) the ability of the prison subculture to reproduce itself 7) a high degree of cohesion of prison carriers subcultures 8) the spread of the norms of the prison subculture to the majority of the population of correctional institutions. In the development of the prison subculture, the authors have identified two cycles («big» and «small»). The «big cycle» describes the global relationship between the prison subculture and the basic culture of Russian society, «small» – the interaction of collectives of convicts and staff of correctional institutions influenced by the prison subculture. In both cycles, three stages of development are distinguished, characteristic of the dialec¬tical triad. The practical significance of the work lies in comprehending the ongoing changes with the subsequent adoption of managerial decisions aimed at leveling the spread of the norms of the prison subculture in society.

Complex of medicine that increases the body's resistance to the damaging effects of viruses during a pandemic in the long term will be an important tool not only to reduce the overall incidence of diseases, but also to improve the prognosis of those who have been ill due to the reduction of residual fibrotic changes in the lungs.

The article presents the analysis of uncertainties associated with reliable accounting difficulties for the radiation coefficient in non-contact temperature measurement in industrial environment. Practical recommendations of methodological and instrumental nature to reduce such measurements errors are offered. On the Planck function and the aluminum reflection coefficient mathematical model basis the analytical aluminum radiation coefficient dependence on temperature and wavelength is obtained. Methodological errors estimates of low-temperature thermometers with a spectral sensitivity ranges equal to 7–14 and 3–5 µm are produced. Experimental studies are carried out using Optris CT LT15 pyrometer and Flir E60 infrared imager.

Aim. To create a model for non-alcoholic fatty liver disease (NAFLD) development at the steatosis stage in visceral obesity in patients with coronary artery disease (CAD) and hypertension (HTN).Material and methods. The study included male patients with coronary artery disease, stable angina and hypertension: experimental group — 75 patients, control group — 38 patients. All patients underwent an anthropometric assessment, visualization and measurement of the intraabdominal fat thickness (AIFT) and epicardial fat thickness (EFT) by ultrasound, and echocardiography. The liver state was assessed using clinical and functional biochemical tests and ultrasound. Lipid and carbohydrate metabolism parameters was also evaluated. The influence of each of the studied indicators on NAFLD development was determined using the factor and correlation analysis.Results. We created a mathematical model for predicting the NAFLD at the steatosis stage. Using significant variables (body mass index (BMI), EFT, AIFT, left ventricular posterior wall thickness (LVPWT), Homeostasis Model Assessment of Insulin Resistance (HOMA-IR) value), a regression model was created. There was following resulting regression equation: prognostic index K= -24,9+0,81 хBMI+1,06хEFT+0,02хAIGT+0,6хLVPWT-1,07хHOMA-IR. To test the developed model, a double-blind randomized study was performed. At K15,38 — as high. Values from 9,37 to 15,38 are considered intermediate.Conclusion. During the study, an original model was developed to calculate the likelihood of NAFLD development. The resulting model with the assessment of risk factors in patients with CAN and HTN with visceral obesity can be used to plan a prevention strategy. Patented invention №2718325RU dated 01.04.2020.

The article presents the data on the prevalence of tuberculosis in the penitentiary units in the countries of the WHO European Region compared to theRussian Federation.It lists factors providing an impact on the risk of the disease and notification rate of tuberculosis cases in prisons.Monitoring of tuberculosis in the penitentiary units is described including the formation of notification rates and treatment outcomes. It presents information on the comparatively high quality of tuberculosis monitoring in the penitentiary units of theRussian Federation.Potential causes of differences between the incidence of tuberculosis in different countries of the WHO European Region are analyzed.

Aim. To reveal the specifics of arterial hypertension (AH) development under the circumstances of penitentiary stress.Material and methods. Totally, 107 patients included, among the persons first time imprisoned, comparable by age and gender — almost healthy young males 25-36 y. o. The relation studied, of psychological test and 24-hour blood pressure (BP) profile and Holter ECG, as heart rate variablity, in three groups of participants formed depending on the duration of imprisonment.Results. It was found that psychoemotional state during the first month of imprisonment is characterized by a high level of situational, personality anxiety with predominant depressive symptomatic. This is followed by heart conduction and rhythm pathology. With the time of imprisonment, level of anxiety and depression declines. Also, heart rhythm variability analysis shows significant decrease of value of LF (%) and predomination of VLF (%) in the wave spectrum. The parameters of 24-hour BP show a significant increase of BP at later periods of imprisonment. Daytime BP level shows direct relation with the parameter “duration of imprisonment”, and nocurnal BP — with the level of situational anxiety. Conclusion. Under the circumstances of penitentiary stress, one of the revealed mechanism of AH development is weakening of sympathetic modulation of vegetative innervation of the heart and decline of baroreflectory sensitivity. Increase of the duration of imprisonment and of the level of situational anxiety are the specific factors of penitentiary circumstances that influence AH development in young clinically healthy men.

Significant mortality due to oncological diseases as a whole, and oncohematological diseases in particular, motivates scientific and medical community to develop new treatment methods. One of the newest methods is adoptive cell therapy using patient’s own T-cells modified to express chimeric antigen receptors (CAR) to tumor-specific antigens. Despite high cost and side effects of treatment, promising clinical trials even in patients with advanced disease allow to anticipate successful use of this method in clinical practice.The article includes a review of the main principles of this technique, published results of clinical studies of CAR T-cells with a focus on CD19 gene targeting, complications of this therapy, mechanisms of tumor resistance to CAR T-cells, and potential ways to overcome it.

Aim. To investigate dynamics of QT interval variability during physical stress (PS) test in coronary heart disease (CHD) patients with various stages of arterial hypertension (AH). Material and methods. The study included 196 working-age males with earlier diagnosed CHD, divided into 4 groups: Group I (no AH; n=65), Group II, III, and IV (with Stage I, II, and III AH, respectively; n=41, n=43, and n=47). All subjects underwent echocardiography, veloergometry. Corrected QT interval dispersion (dQTс) at rest and during maximal PS; recovery time of baseline dQTc, and original stress tolerance coefficient (STC) were calculated. Results. CHD patients, regardless of AH presence and stage, had greater myocardial electric heterogeneity at baseline. Its increase in PS, as well as recovery time increase, were determined by AH presence and stage, being maximal in severe AH (1.76-fold increase). Myocardial electric heterogeneity, especially in PS, correlated with left ventricular (LV) hypertrophy, LV diastolic function, and the overall functional status of cardiovascular system. Conclusion. AH raise causes increase in myocardial hemodynamic workload. Compensatory increase in LV myocardial mass and myocardial O2 demand result in defective myocardial blood flow, myocardial ischemia, and increased QT interval variability, especially during PS. Taking into account dynamics of myocardial electric heterogeneity during PS may raise the informative value of ECG-controlled stress tests.
