The article explores the impact of recent technological advancements in coloproctology, including nanotechnology, artificial intelligence, robotics, and advanced imaging. These advancements are changing the role of surgeons and increasing reliance on machines for diagnosis and treatment. The article also discusses the use of machine learning platforms in cancer management and the potential of targeted cell therapies and immunotherapies. It highlights the changing landscape of surgical interventions for conditions like rectal cancer, inflammatory bowel diseases, and anal fistulae. 18. Imaging the levator ani and the puborectalis muscle: implications in understanding regional anatomy, physiology and pathology.

Abstract

Purpose: To review the findings of recent dynamic imaging of the levator ani muscle in order to explain its function during defecation. Historical anatomical studies have suggested that the levator ani initiates defecation by lifting the anal canal, with conventional dissections and static radiologic imagery having been equated with manometry and electromyography. Materials and methods: An analysis of the literature was made concerning the chronological development of imaging modalities specifically designed to assess pelvic floor dynamics. Comparisons are made between imaging and electromyographic data at rest and during provocative manoeuvres including squeeze and strain. Results: The puborectalis muscle is shown distinctly separate from the levator ani and the deep external anal sphincter. In contrast to conventional teaching that the levator ani initiates defecation by lifting the anus, dynamic illustration defecography (DID) has confirmed that the abdominal musculature and the diaphragm instigate defecation with the transverse and vertical component portions of the levator ani resulting in descent of the anus. Current imaging has shown a tendinous peripheral structure to the termination of the conjoint longitudinal muscle, clarifying the anatomy of the perianal spaces. Planar oXy defecography has established patterns of movement of the anorectal junction that separate controls from those presenting with descending perineum syndrome or with anismus (paradoxical puborectalis spasm). Conclusions: Dynamic imaging of the pelvic floor (now mostly with MR proctography) has clarified the integral role of the levator ani during defecation. Abstract

In this research, the researcher adopts a practical and the theoretical parts. First, is the practical part, it consists of the problem, the aim, the hypothesis, the procedures and the limits of the study. These will be in the first part. In part two which is the theoretical part, the researcher indicates two main types of researches which are quantitative and qualitative research. Then the research is dedicated to the description of methodology, the results and finally the conclusion. The study aims first to clarify the quantitative and qualitative researches and second to investigate the performance of Iraqi EFL researchers in writing quantitative and qualitative researches. The main tool that is used in this research is the questionnaire which was included personal information such as (bachelor of education) and teachers (master and doctorate) and different questions about quantitative or qualitative researches. It combined (30) participants, (20) as fourth stage researchers, and (10) for lecturer, it was designed to measure which of the researchers has knowledge about the two types of researches, and how can they distinguish between each one, what are the main different points between quantitative and qualitative research. We can see the results from their answers when we analyze the questionnaire.

Abstract

Von Hippel‐Lindau (VHL) is a hereditary multisystem disorder caused by germline alterations in the VHL gene. VHL patients are at risk for benign as well as malignant lesions in multiple organs including kidney, adrenal, pancreas, the central nervous system, retina, endolymphatic sac of the ear, epididymis, and broad ligament. An estimated 30%–35% of all families with VHL inherit a germline deletion of one, two, or all three exons. In this study, we have extensively characterized germline deletions identified in patients from 71 VHL families managed at the National Cancer Institute, including 59 partial (PD) and 12 complete VHL deletions (CD). Deletions that ranged in size from 1.09 to 355 kb. Fifty‐eight deletions (55 PD and 3 CD) have been mapped to the exact breakpoints. Ninety‐five percent (55 of 58) of mapped deletions involve Alu repeats at both breakpoints. Several novel classes of deletions were identified in this cohort, including two cases that have complex rearrangements involving both deletion and inversion, two cases with inserted extra Alu‐like sequences, six cases that involve breakpoints in Alu repeats situated in opposite orientations, and a “hotspot” PD of Exon 3 observed in 12 families that involves the same pair of Alu repeats., Von Hippel‐Lindau (VHL) is a hereditary multisystem disorder caused by germline alterations in the VHL gene. This study characterized 59 partial and 12 complete VHL germline deletions in 71 VHL families ranging in size from 1.09 to 355 kb. Exact breakpoints were mapped for 55 partial and 3 complete germline deletions and 95% involved Alu repeats at both breakpoints, with a common “hotspot” pair of Alu repeats observed in 12 families.

Abstract

The experimental work of this study deals with the performance of pipe in sand with various responses subjected to incremental static loading where the backfill is reinforced with two types of reinforcement (geogrid and geocell) to investigate the effects of backfill cover. The pipes considered are installed in sand with different densities; i.e., loose, medium and dense sand, and a constant burial depth (2D, where D is the pipe diameter). It was concluded that the maximum deformation occurs at the crown of the pipe. The deformation increases when the sand density decreases. For a burial depth equal to 1.5 D, the maximum strain on the pipe crown increases by about 22.5% when the sand changes from dense to medium and 36.8% when it changes from dense to loose. The vertical stress above a pipe buried in loose sand is reduced by 8.3% when the geogrid width is 2B where B is the footing width, while it is equal to 22% when the geogrid width is B. When a geocell is used, the percent reduction is equal to 62%. The percent reduction in the vertical stress above a pipe buried in medium sand is about 28% when the geogrid width is 2B and 40% when the geogrid width is B and equal to 68% when the geocell is used. Abstract

This paper deals with the problem of a flexible pipe with internal diameter of 110 mm buried in laboratory soil box facilities for this study. The box is filled with sand reinforced by geogrid layer of different widths to investigate the effect of geogrid width layer on the performance of buried pipe subjected to static load. Two type of reinforcement used (geogrid and geocell). The geogrid location is kept constant at a depth of 0.5d (do is o the pipe o diameter) below the loading plate. The results show that the reduction in crown strain in loose sand, when the geogrid width equals 1B iso13% in comparison with results of tests excluding geogrid, this percent reduces to 11% when the geogrid width equals 2B, while the reduction percent in medium sand becomes 14% and 12% when the geogrid width was B and 2B, respectively and becomes in dense sand 15 % and 11% when the width of geogrid change from B to 2B.

31. Socio-Ecologic Perspective: Barriers Complicating Post-Intensive Care Syndrome Mitigation

Abstract

Objective:Post-intensive care syndrome (PICS) is a phenomenon whereby survivors of an intensive care unit (ICU) admission subsequently experience issues with physical, cognitive, or mental health status persisting beyond the acute hospitalization. Risk factors for developing PICS include prolonged mechanical ventilation with sedation and immobility. PICS is a devastating illness that negatively alters the life path of many individuals with tremendous economic impact. Methods: This qualitative study employed a grounded theory approach to understand the systemic barriers blocking mitigation and treatment of PICS in all seven ICUs across Essex County, New Jersey (NJ) through semi-scripted interviews conducted with 11 members of the healthcare teams with at least one from each site. Thematic analysis was performed with open, axial, and selective coding. Results: Applying socio-ecologic viewpoint to data illustrate significant barriers on both an interpersonal and organizational level that decrease the operationalization of PICS mitigation measures as identified by healthcare providers. Of those interviewed, eight (73%) were physicians and the remaining were nurses. Significant thematic issues included understanding the risk factors of PICS but feeling powerless to institute mitigation efforts; experiencing lack of enthusiasm due to the absence of institutionalized mitigation protocols; noting frustration about closing the gap between academic recommendations and the ability to operationalize these appropriately; and feeling unable to effectuate meaningful change. Conclusion: Providing education to the target population and healthcare provider stakeholders regarding the barriers against PICS mitigation can alter the status quo.

Abstract

The study sought to identify the extent of the impact of educational technology in the quality of education and to enhance the level of world ranking of the University of Babylon (study population) on a sample of its teachers, which consisted of (90) person from the university to express their opinions through a questionnaire developed for this purpose, descriptive statistics were used and simple linear regression to enterprise the relationship between the variables. The study reached that there is an effect relationship. In the use and employment of educational technology (independent variable) in the dimensions of the quality of higher education (dependent variable) (quality of curricula, quality of scientific research, quality of libraries, and quality of university teachers). Which has an impact on enhancing the level of world ranking of the University. The study recommends that the University of Babylon should pay more attention to the use and employment of educational technology and its innovations, as it has not achieved a high level in this field.
