In this narrative review, we investigate the essential role played by the computed tomography Aortic Valve Calcium Score (AVCS) in the cardiovascular diagnostic landscape, with a special focus on its implications for clinical practice and scientific research. Calcific aortic valve stenosis is the most prevalent type of aortic stenosis (AS) in industrialized countries, and due to the aging population, its prevalence is increasing. While transthoracic echocardiography (TTE) remains the gold standard, AVCS stands out as an essential complementary tool in evaluating patients with AS. The advantage of AVCS is its independence from flow; this allows for a more precise evaluation of patients with discordant findings in TTE. Further clinical applications of AVCS include in the assessment of patients before transcatheter aortic valve replacement (TAVR), as it helps in predicting outcomes and provides prognostic information post-TAVR. Additionally, we describe different AVCS thresholds regarding gender and the anatomical variations of the aortic valve. Finally, we discuss various scientific studies where AVCS was applied. As AVCS has some limitations, due to the pathophysiologies of AS extending beyond calcification and gender differences, scientists strive to validate contrast-enhanced AVCS. Background: At present, combined provision of direct composite restorations and dental bleaching has become increasingly popular to meet the esthetic demands of patients. This study aimed to assess the effect of internal bleaching on microtensile bond strength of composite to freshly veneered enamel. Methods: In this in vitro experimental study, 50 sound extracted human premolars were randomly divided into five groups (n=10). All teeth received composite veneers. Two groups underwent no further intervention and were subjected to measurement of microtensile bond strength of composite to enamel 24 hr and one week after the veneering. The remaining three groups underwent internal bleaching. Group 3 underwent bleaching one month after the veneering, and microtensile bond strength was measured after 24 hr. In groups 4 and 5, bleaching was performed immediately after the veneering. Microtensile bond strength was measured after 24 hr in group 4 and after one month in group 5. Data were analyzed using one-way ANOVA. Results: All the groups were not significantly different regarding the mean bond strength (p=0.110), but they were significantly different in terms of the mode of failure (p=0.01) such that the mode of failure was mainly adhesive in the three bleached groups. Introduction: Successful endodontic treatment depends on completely clearing, shaping, and filling the prepared canals. Knowledge of the common and aberrant varying pulp anatomies is essential for suitable root canal treatment. Since, this anatomy is complex and varies morphologically in different populations. This study aims to determine the number of roots, canals, and evaluation of mandibular premolars canals using cone-beam computed tomography (CBCT) images in one of Iran's northern provinces (Golestan). Materials and Methods: CBCT axial, coronal, and sagittal slices of two hundred and twelve mandibular premolars were analyzed to determine the number of roots and canals based on Vertucci type. The images were analyzed in a one-millimeter slice in mesiodistal and buccolingual dimensions based on gender. The data were analyzed using the Chi-score test to compare the components if the defaults were not established. A significance level of 0.05 was considered. Result: Of the two hundred twelve teeth evaluated, 130 (61.3%) were first premolars, and 82 (38.6%) were second premolars. Most first premolars (78.5%) and almost all second premolars (97.6%) had one root. Morphological types of root canals were identified based on Vertucci's classification types I, II, III, IV, V, or VIII, and type I was the most frequent. There were no significant associations between number of roots and sex (P>0.05). Conclusion: Mandibular premolars mostly had one root and type I Vertucci morphology. The frequency of two-canal premolars was higher in the male population. Although the abundance of two or three roots with different morphologies was also observed, the possibility of the presence of an additional root canal should be considered. [ABSTRACT FROM AUTHOR]

Methods This randomized split-mouth controlled clinical trial was conducted on 12 patients undergoing orthodontic treatment with extraction of maxillary first premolars. Following initial leveling and alignment, an alginate impression was made from the maxillary arch, and Er: YAG laser corticotomy was performed in one of the maxillary quadrants of each patient. Canine retraction was started immediately after corticotomy by placement of nickel-titanium (NiTi) closed coil springs at both sides. At the end of each month, alginate records were repeated for 4 months. Study models were scanned, and the anteroposterior movement of canine was quantified bilaterally. Pain was also measured by a visual analog scale (VAS). Probing depth (PPD) of canines and two adjacent teeth was also evaluated and pulp vitality was assessed by performing the cold test. Data were analyzed by paired and independent t-test and one-way ANOVA (alpha = 0.05). Results The rate of canine retraction was significantly greater in the laser-assisted corticotomy quadrant than the control (P 0.05). Conclusions Flapless Er: YAG laser corticotomy significantly enhanced canine retraction rate with no adverse effect on other parameters.

Purpose: The existence of valves in the azygos vein are not discussed in depth in the current body of literature. Although the clinical relevance of the valves in these veins are not well understood, these valves may have implications in diagnostic imaging where their appearance on imaging studies can impact interpretation. This study aims to use cadaveric dissections to determine the anatomic prevalence and morphology of valves in the arch of the azygos vein in comparison to current data reported from clinical imaging studies and anatomy textbooks. Methods: Approval of this research study on human cadaveric specimen was sought from the St. George's University Institutional Review Board (IRB) and was deemed exempt from full review (IRB – 16015). This research project involved 47 azygos veins dissected from human cadaveric donor specimen. Azygos veins were harvested from the level of the diaphragm to the inflow into the superior vena cava for gross inspection of valve presence and morphology. The data collected was recorded and sorted by sex, valve presence, number and morphology and valve location. Chi square analysis was performed to assess possible relationships in valve findings based on sex. A brief search of the literature using the keywords “azygos vein” and “valves” was conducted to see the current understanding of azygos valve prevalence. Results: Of the donor specimen, 53.2% (25 of 47) were male and 46.8% (22 of 47) were female. Of the 47 cadavers that were dissected, 89.3% (42 of 47) were found to have valves in the azygos vein, while 10.6% (5 of 47) were found to have none. The azygos vein contained mostly one valve along its length, mainly located in the arch. The valves were unicuspid (8.5%; 4 of 47), bicuspid (70.2%; 33 of 47) or tricuspid (10.64%; 5 of 47). Dividing the arch of the azygos into quartiles, valves were found most commonly in the third quartile of the distance from the opening of the right superior intercostal vein into the azygos vein to the superior vena cava. Four cadavers had multiple valves, each with varying valve morphology. The length of the cusp of the bicuspid valve ranged from 10 mm to 12 mm. Conclusion: This study shows that the azygos vein contains valves, predominantly within the arch at a prevalence of 89.3%. The presence and variability in morphology of valves in the azygos vein and its potential implications for contrast appearance on imaging studies is clinically important. Radiologists should have a working knowledge of the different appearances of azygos arch valves to minimize the risks of confusing normal variations with thoracic pathology. Though not commonly described in anatomy texts, the findings of this research not only add more detail to the historical cadaveric study information on azygos vein valves, but also shows that the prevalence of azygos arch valves surpasses the numbers reported in clinical studies. As such, it provides support for the inclusion of azygos arch valves in anatomical textbooks and in the teaching of image interpretation.

BACKGROUND: Tooth gemination is a single enlarged or joined tooth with a normal tooth count when the anomalous tooth is counted as one. Mandibular second premolars show an elevated variability of crown morphology. Only nine cases of isolated second premolar macrodontia have been reported in the literature. CASE DESCRIPTION: This case report presents the clinical and radiographic findings and conservative treatment of an atypical and rare case of localized bilateral molarization of mandibular second premolars. CONCLUSION: Dental professionals should acquire deeper knowledge about anomalies and plan treatment carefully to avoid unexpected complications during dental procedures caused by morphological ignorance. Objective The aim of this study was to assess the frequency of mandibular second premolar (MnP2) distoangulation among orthodontic patients and the angular changes after a longitudinal follow-up. Methods The sample was collected from the orthodontic records of 865 patients. The distoangulation group was composed of 42 patients (mean age 9.29 ± 1.24 years, 16 male, 26 female) with distoangulation of MnP2. The control group was composed of 32 patients (mean age 9.38 ± 1.10 years, 15 male, and 17 female) without distoangulation of MnP2. Panoramic radiographs taken in the mixed (T1) and the early permanent dentition (T2) were analyzed in both groups. Longitudinal angular changes (distal angle θ and premolar-molar angle γ), degree of root formation, second premolar depth, and prevalence of associated dental anomalies were analyzed. Intergroup comparison was performed with Mann–Whitney, t-tests, and chi-square tests (P  < 0.05). Results The prevalence of MnP2 distoangulation in the mixed dentition was 4.85%. The distoangulation group showed a smaller initial distal angle (59.34o ± 8.41) when compared to control group (79.88o ± 7.60). The spontaneous eruption of the MnP2 with distoangulation was observed in 76.57% of the sample. MnP2 distoangulation was significantly associated with agenesis of its antimere, small maxillary lateral incisors, and deciduous molar infraocclusion. Limitations Severe cases of MnP2 distoangulation were absent in this study. Conclusions The frequency of MnP2 distoangulation among orthodontic patients was 4.85%. Mild to moderate distoangulated Mnp2 spontaneously uprighted from the mixed to the permanent dentition. Small lateral incisors, second premolar agenesis, and infraocclusion of deciduous molar were frequently found in cases with MnP2 distoangulation. [ABSTRACT FROM AUTHOR]

Foundation: advances in imaging and its implementation in Dentistry allow for better treatments. Objective: to describe the results of the orthoradial technique vs parallel technique in the identification of upper first premolars canals and lower first molars. Methods: observational, descriptive and cross-sectional study in patients in need of root canal treatment of upper first premolars or lower first molars, who attended the Dental Care Unit of the Andes Regional Autonomous University from April to August 2019. The sample consisted of 62 patients, separated into two upper premolar groups (n=30) or lower first molars (n=32) and both into two other groups according to the technique used (parallel technique and orthoradial technique). Results: in the upper first premolars, the orthoradial technique showed better results than the parallel technique to obtain accurate images (100% vs. 20%). In the lower first molars, the orthoradial technique showed superior results to the parallel technique for the mesiolingual canal (93.75% vs 31.25%) and distolingual canal (31.25% vs 12.5%). Root elongation predominated in the orthoradial technique (n=8), as did dental overlap (n=10). Conclusions: the orthoradial technique showed superior results when locating both the buccal and palatine canal in the upper first premolars and for the mesiolingual canal and the distolingual canal. Endodontic treatment of second premolars is challenging due to their complex anatomy and the likelihood of anatomical variations. The current report presents successful treatments of mandibular and maxillary second premolars with different anatomies in a single patient. The first case involved a patient referred from a general practitioner who was unable to complete the root canal treatment of her mandibular second premolar with four root canals. The initial radiograph showed an apical radiolucency, periodontal ligament (PDL) widening, and a fast break suggesting the possibility of root canal branching. With the diagnosis of previous treatment and chronic apical periodontitis, the tooth was successfully retreated with the aid of cone-beam computed tomography (CBCT), a dental operating microscope, and ultrasonic tips. The second case was a maxillary second premolar with Vertucci's type VI root canal morphology, which was not responsive to pulpo-periapical tests. Along with a deep carious lesion and a PDL widening on the initial radiograph, a diagnosis of necrosis with chronic apical periodontitis was made. The root canals were located and treated using a combination of radiography with different angulation and CBCT. Both teeth were clinically and radiographically asymptomatic at the 4-month follow-up session. This case report highlights the importance of thorough preoperative evaluation and the use of advanced technology and techniques in achieving successful endodontic treatment outcomes. Purpose: To determine the prevalence of developmental defects of the enamel (DDE) in premolars whose infected predecessors were submitted to pulp therapy with antibiotic paste or extractions due to pulp necrosis.Materials and Methods: A cross-sectional study with a consecutive sample consisting of children and adolescents who presented with fully erupted premolars, was evaluated. Data were collected by dental examinations, in which the modified DDE index was applied. Dental records were evaluated and three groups of premolars were determined according to the clinical history of predecessors: GCTZ: with pulp necrosis and treated with CTZ (chloramphenicol, tetracycline, zinc oxide and eugenol) paste; GE: with pulp necrosis and treated by extraction; GH: healthy and physiologically exfoliated. Descriptive analysis and a logistic regression (p <0.05) were performed.Results: The study included 1017 premolars, DDE was present in 22.5%. Premolars belonging to the GE group presented higher odds of DDE (odds ratio (OR) = 3.52, 95% CI:2.29-5.40) than those of GCTZ group (OR = 2.43, 95% CI:1.51-3.91) and GH group (p <0.01). Enamel defects were more frequent in maxillary premolars (OR = 3.22, 95% CI:1.65-6.27, OR = 3.39, 95% CI:1.67-6.90, OR = 2.90, 95% CI:1.48-5.66 and OR = 3.10, 95% CI:1.54-6.23).Conclusions: The prevalence of enamel defects was higher in premolars whose predecessors were removed because of necrosis, followed by those treated with CTZ paste and those that were healthy by the time exfoliation occurred. Introduction: The efficacy of the trigeminal nerve block varies between 70% and 98%, relying heavily on the accurate identification of anatomical landmarks. The anatomical position and incidence of supernumerary foramina of the supraorbital (SOF), infraorbital (IOF) and mental (MF) foramina vary amongst population groups, sex, age, and climate conditions. Hence, the localisation of the foramina is crucial in the administration of anaesthesia to the related nerves. The objective of this study was to document evidence of the morphology and morphometry of the SOF, IOF, and MF – in the form of a narrative review. Materials and methods: This review analyses reports on the localisation of the SOF, IOF and MF, and their variations (if any). A literature search was conducted on multiple electronic databases (PubMed, MEDLINE, Google Scholar and Science Direct). A total of 149 articles were eligible for inclusion in this review. Discussion and conclusion: The results of the literature search shows that the SOF has not been studied as extensively as the IOF and MF. Therefore, it is suggested that further research be conducted on the anatomy and clinical relevance of the SOF. The SOF, however, presents frequently as a notch and is, therefore, called the supraorbital notch. The IOF is often an oval-shaped opening and may present with one accessory foramen. The MF is often in line with the 2nd mandibular premolar tooth and may also have accessory openings. This review revealed that the IOF and MF frequently lie lateral to the SOF, contradicting standard anatomical literature which states that they all lie on the same vertical plane. Medical professionals should consider the location and shape of the studied foramina, and the occurrence of accessory foramina, to successfully administer the corresponding nerve block. Landmarks used to localise the foramina highlighted in this review may be applied to evaluate their clinical relevance.

Aim: The aim of this study was to access and compare fracture resistance of tooth restored with Cention-N, Zirconomer and Filtek Z350 XT. Materials and Methods: Seventy five extracted maxillary premolar were divided into five groups; (Group I) Unprepared intact teeth, (Group II) Teeth with class II mesio-occluso-distal (MOD) cavity prepared and unrestored, (Group III) Teeth with class II MOD cavity prepared and restored with Zirconomer (Shofu Inc), (Group IV) Teeth with class II MOD cavity prepared and restored with Cention N, (Group V) Teeth with class II MOD cavity prepared and restored with Universal Composite Restorative Material (Filtek Z350 XT). Specimen was individually tested in a universal testing machine for facture resistance. Results: Filtek Z350 XT showed statistically significant fracture resistance as compared to Group II, Group III and Group IV (P<0.001). Cention-N showed better fracture resistance than Zirconomer but didn't vary statistically. Conclusion: All the test restorative material showed acceptable fracture resistance under load. The aim of this paper is to assess the association between valve morphology and vortical structures quantitatively and to highlight the influence of valve morphology/orientation on aorta's susceptibility to shear stress, both proximal and distal. Four-dimensional phase-contrast magnetic resonance imaging (4D PCMRI) data of 6 subjects, 3 with tricuspid aortic valve (TAV) and 3 with functionally bicuspid aortic values (BAV) with right-left coronary leaflet fusion, were processed and analyzed for vorticity and wall shear stress trends. Computational fluid dynamics (CFD) has been used with moving TAV and BAV valve designs in patient-specific aortae to compare with in vivo shear stress data. Vorticity from 4D PCMRI data about the aortic centerline demonstrated that TAVs had a higher number of vortical flow structures than BAVs at peak systole. Coalescing of flow structures was shown to be possible in the arch region of all subjects. Wall shear stress (WSS) distribution from CFD results at the aortic root is predominantly symmetric for TAVs but highly asymmetric for BAVs with the region opposite the raphe (fusion location of underdeveloped leaflets) being subjected to higher WSS. Asymmetry in the size and number of leaflets in BAVs and TAVs significantly influence vortical structures and WSS in the proximal aorta for all valve types and distal aorta for certain valve orientations of BAV. Aims: The study aimed to compare paravalvular leak (PVL) changes after a transcatheter aortic valve replacement (TAVR) with self-expandable prosthesis between different aortic valve morphologies and evaluate the impact of paravalvular leak regression on clinical prognosis. Methods: Patients with aortic stenosis (AS) successfully treated with a selfexpandable TAVR who were followed up for at least 1 year at our centre were consecutively enrolled from January 2016 to August 2019. Paired serial changes in paravalvular leak and other haemodynamic parameters by echocardiography were collected and compared between the bicuspid valve (BAV) and tricuspid aortic valve (TAV). A logistic regression model was used to explore the predictors of paravalvular leak regression (<1 grade) 1 year after transcatheter aortic valve replacement, while its impact on subsequent clinical outcomes (allcause mortality and rehospitalisation for heart failure (HF)) was further evaluated using Kaplan-Meier analysis. Results: A total of 153 bicuspid valve and 114 tricuspid aortic valve patients were finally enrolled; haemodynamic parameters and paravalvular leak severity were comparable before the discharge between the two groups. The peak transaortic velocity, mean transvalvular gradient, and effective orifice area all significantly improved (p < 0.05) without intergroup differences at all follow-up timepoints. Significant paravalvular leak reduction was observed only in the TAV group (1.75% vs. 4.39%, p = 0.029), while moderate paravalular leak was still more prevalent in BAV (7.19% vs. 1.75%, p = 0.041) at the 1-year follow-up. Multivariable analyses identified the bicuspid valve, asymmetric calcification, and undersizing as independent predictors of failure of the 1-year paravalvular leak reduction in patients with mild or moderate paravalvular leak after discharge. Patients without a paravalvular leak reduction within 1 year showed a relatively higher 2-year all-cause mortality and HF (HR: 5.994, 95% CI: 1.691-21.240, and p = 0.053) rates thereafter. Conclusion: In AS patients after self-expandable transcatheter aortic valve replacement, paravalvular leak regression within 1 year was less prevalent in bicuspid valve morphology. Abstract Background There is a lack of available data on specific prognostic comparisons between transcatheter aortic valve replacement (TAVR) using self-expandable valves (SEV) in patients with stenotic Type 0, Type 1 bicuspid aortic valve (BAV) and tricuspid aortic valve (TAV). Objectives To evaluate the association between aortic valve morphology and outcomes following self-expandable TAVR. Methods Consecutive patients with aortic stenosis(AS) undergoing self-expandable TAVR were enrolled and categorized into three groups (Type 0/Type 1 BAV or TAV) according to the Sievers classification. The primary endpoint was a composite of all-cause mortality and rehospitalization for heart failure (HF) within 2 years. Secondary outcomes included procedural complications and major cardiovascular events observed in clinical follow-ups. Clinical outcomes at 2 years following TAVR were compared among three groups using Kaplan-Meier curve and multivariable Cox proportional hazards regression models. Results A total of 344 AS patients (Type 0: 86; Type 1: 109; TAV: 149) were enrolled. The presence of moderate or severe paravalvular leak (PVL) was significantly higher in patients with Type 0 and Type 1 BAV versus TAV (10.47% vs. 16.51% vs. 6.71%, p = 0.043). All-cause 30-day mortality (2.33% vs. 0.92% vs. 2.68%, p = 0.626) and 2-year mortality (3.49% vs. 5.50% vs. 6.71%, p = 0.657) was comparable among the three groups. However, rehospitalization for HF within 2 years was significantly higher in Type 1 BAV (11.63% vs. 20.18% vs. 8.72%, p = 0.020). Multivariate Cox analysis showed that a higher STS score, Type 1 BAV morphology and excess leaflet calcification (≥ median calcium volume (CV) of the entire population) were independent predictors for HF rehospitalization. Additional intragroup Kaplan‒Meier analysis showed that excess leaflet calcification could predict higher long-term mortality and rehospitalization risk for HF(HR (95% CI): 3.430 (1.166–10.090), log rank p = 0.017) in Type 1 BAV patients. Conclusion Outcomes of self-expandable TAVR in BAV-AS patients might vary depending on valve subtypes. BAV patients with excess leaflet calcification and a raphe, especially calcified, had an increased risk of moderate PVL and HF readmission in mid-to-long term follow-ups.

Purpose: This study evaluated anatomical variations in the root canals of the lower premolars and molars in a Brazilian sub-population using cone-beam computed tomography (CBCT). Materials and Methods: In total, 121 CBCT images of patients were selected from a database. All images contained lower first and second premolars and molars on both sides of the arch, fully developed roots, and no treatment, resorption, or calcifications. In each image, the root canals of the lower premolars and molars were evaluated according to the Vertucci classification in On-Demand 3D software in the multiplanar reconstruction with dynamic navigation. Twenty-five percent of the images were re-assessed to analyze intraobserver confidence with the kappa test. Data were statistically evaluated with linear regression to evaluate the correlations of anatomic variations with age and sex, and the Wilcoxon test to analyze the laterality of variations, with a significance level of 5%. Results: The intraobserver agreement (0.94) was excellent. In general, the root canals of lower premolars and molars showed a higher prevalence of type I than other Vertucci classification types, followed by type V in premolars and type II in molars. When the molar roots were evaluated separately, type II was more frequent in mesial roots and type I in distal roots. Although age showed no correlations with the results, sex and laterality showed correlations with tooth 45 and the lower second premolars, respectively. Introduction: the use of custom implants is a very common treatment; we assess and compare their behavior against that of conventional implants. This study aimed to make sure that the stress zones of the custom implant are different from those presented by the conventional prefabricated implant by photoelasticity. Methods: we subjected samples of n=10 bicuspid teeth, n=10 sintered custom implants, and n=10 conventional prefabricated implants to 3 fixed and controlled forces and observed the samples through a polariscope to analyze the distributions of effort generated. The effort zones present in the different samples were analyzed under 3 different forces. Results: the amounts of effort in the two types of implants under force 1 (chi-square test, p=0.596) are different, as is also the case under force 2 (chi-square test, p=0.407). Under force 3 (Levene test, p=0.899), there is no difference in the distributions of effort between the two types of implants. Conclusions: it was determined that the conventional prefabricated implant distributes and concentrates the effort generated under different forces better than the sintered custom implant.

Background: There is a lack of available data on specific prognostic comparisons between transcatheter aortic valve replacement (TAVR) using self-expandable valves (SEV) in patients with stenotic Type 0, Type 1 bicuspid aortic valve (BAV) and tricuspid aortic valve (TAV). Objectives: To evaluate the association between aortic valve morphology and outcomes following self-expandable TAVR. Methods: Consecutive patients with aortic stenosis(AS) undergoing self-expandable TAVR were enrolled and categorized into three groups (Type 0/Type 1 BAV or TAV) according to the Sievers classification. The primary endpoint was a composite of all-cause mortality and rehospitalization for heart failure (HF) within 2 years. Secondary outcomes included procedural complications and major cardiovascular events observed in clinical follow-ups. Clinical outcomes at 2 years following TAVR were compared among three groups using Kaplan-Meier curve and multivariable Cox proportional hazards regression models. Results: A total of 344 AS patients (Type 0: 86; Type 1: 109; TAV: 149) were enrolled. The presence of moderate or severe paravalvular leak (PVL) was significantly higher in patients with Type 0 and Type 1 BAV versus TAV (10.47% vs. 16.51% vs. 6.71%, p = 0.043). All-cause 30-day mortality (2.33% vs. 0.92% vs. 2.68%, p = 0.626) and 2-year mortality (3.49% vs. 5.50% vs. 6.71%, p = 0.657) was comparable among the three groups. However, rehospitalization for HF within 2 years was significantly higher in Type 1 BAV (11.63% vs. 20.18% vs. 8.72%, p = 0.020). Multivariate Cox analysis showed that a higher STS score, Type 1 BAV morphology and excess leaflet calcification (≥ median calcium volume (CV) of the entire population) were independent predictors for HF rehospitalization. Additional intragroup Kaplan‒Meier analysis showed that excess leaflet calcification could predict higher long-term mortality and rehospitalization risk for HF(HR (95% CI): 3.430 (1.166–10.090), log rank p = 0.017) in Type 1 BAV patients. Conclusion: Outcomes of self-expandable TAVR in BAV-AS patients might vary depending on valve subtypes. BAV patients with excess leaflet calcification and a raphe, especially calcified, had an increased risk of moderate PVL and HF readmission in mid-to-long term follow-ups. [ABSTRACT FROM AUTHOR]

Background and objectives: Microleakage at the interface of tooth structure and fissure sealant plays a crucial role in failure of the treatment. This in vitro study aimed to determine the microleakage of self-adhesive flowable composite resins as pit and fissure sealants. Material and Methods: 60 healthy extracted premolar teeth were randomly divided into the 4 groups (n=15), group 1) clinpro sealants (control), group 2) flowable composite resin with bonding agent (total-etch), group 3) flowable composite resin with self-etch bonding agent, and group 4) self-adhesive composite resin. After thermocycling the specimens were immersed in 2% methylene blue for 48 hours and then sectioned in the buccolingual direction. The microleakage was assessed by dye penetration using a stereomicroscope at 15X magnification. Data analysis was performed using SPSS version 18.0 as well as Mann-Whitney and Kruskal-Wallis tests. Results: There was a statistically significant difference in microleakage score between the groups (P-value = 0.006). The lowest mean of microleakage level was observed in the flowable composite group with total-etch bonding (group 2) 1.26±0.96, followed by the Clinpro sealant group (group 1) 1.62±1.20, and then the self-adhesive composite group (group 4) 1.85±1.00. Conclusion: Based on the results of this study, the microleakage of self-adhesive composite resin revealed no significant difference with conventional fissure sealant; however, the microleakage of these composite resin was higher than flowable composite resin with total-etch bonding agent.

This aim of this study is to evaluate anatomy and morphology of such variants using cone-beam computed tomography (CBCT), and to evaluate dentin thickness of the buccal and lingual canals of maxillary single rooted premolars with two canals. Methods: In this in-vitro study fifty single rooted maxillary premolars with two canals were collected from medical centers in Mashhad, Iran. The number of canals was assessed using periapical radiography. CBCT was utilized to scan teeth and were evaluated within the axial section for the position of root canals with respect to outer root surfaces and canal variants based on Vertucci’s classification in apical, intermediate, and coronal sections. Result: The smallest dentin thickness was 0.40 and 0.83 ± 0.25 mm for the apical third in mesial and distal direction, respectively. Also, the largest result was 3.10 mm for the coronal third buccal direction. The largest relative frequency of Vertucci class types was found to be type IV, while the smallest relative frequency was derived to be type III. The results revealed that seventeen cases had isthmus. Conclusion: Because of the thin dentin related to the mesial aspect, it is required to utilize low-taper files. It is not recommended to make use of orifice shapers.

Background/Aims We evaluated changes in the ascending aorta dimension post-transcatheter aortic valve replacement (TAVR) in bicuspid aortic valve (BiAV) and tricuspid aortic valve (TAV) patients. Methods Patients with severe aortic stenosis undergoing TAVR at Seoul National University Hospital were consecutively recruited. Patients with less than 12 months’ follow-up and/or with an ascending aorta size larger than 50 mm were excluded. The ascending aorta size was measured on a parasternal long axis view using transthoracic echocardiography. Results Among the 67 patients who were included (age: 76.5 ± 6.5 years; male: 52.2%; AV area: 0.67 ± 0.15 cm2), 19 (28.4%) had BiAV; 48 (71.6%) had TAV. The median (interquartile ranges) follow-up duration was 398 days (361 to 451). BiAV patients were younger (73.2 ± 7.2 vs. 77.8 ± 5.8, p = 0.008), and had lower incidences of chronic renal disease (5.3% vs. 35.4%, p = 0.014) and history of coronary intervention (15.8% vs. 50.0%, p = 0.013), than TAV patients. On pre-procedural echocardiography, the ascending aorta dimensions in BiAV patients were larger than those in TAV patients (40.5 ± 3.8 mm vs. 35.9 ± 4.2 mm, p < 0.005). The ascending aorta dimension changed minimally during follow-up; post-TAVR, the ascending aorta’s growth rate was –0.11 ± 1.9 and 0.26 ± 1.8 mm/yr in patients with BiAV and TAV, respectively (p = 0.50). Progression of the ascending aorta’s dimension postTAVR was not clinically significant in BiAV patients. Conclusions The concern about the progression of aortopathy in BiAV patients post-TAVR may not be a clinical issue. This should be confirmed in studies with a larger population and with a longer follow-up duration.

BACKGROUND AND OBJECTIVE: Mandibular second premolars are the third most frequent impacted. By evaluating the three-dimensional position of the impacted tooth and timely treatment, possible problems and damage to adjacent structures can be prevented. This study was designed to assess the position of Impacted Mandibular Second Premolar (MnP2) as well as the effect on adjacent structures using CBCT imaging due to the lack of adequate studies in this field. METHODS: This cross-sectional study was performed on 25 impacted mandibular second premolars. Two radiologists assessed and compared CBCT images to determine the following variables: unilateral/bilateral, position of the crown, root resorption of the adjacent permanent teeth, root dilaceration, depth of impaction, type of impaction, pathology incidence, residual primary tooth, position and distance to inferior alveolar nerve canal and the mental foramen. FINDINGS: In most cases, unilateral impaction was seen (19(76%), p=0.063). Crown position of most of MnP2 was displaced lingually (17(68%), p=0.324). Vertical and distoangular position of tooth were more common (12(48%) and 5(20%), p=0.922). Root resorption of the adjacent permanent teeth and dental pathologies did not occur in any of cases. Most of Mnp2 were categorized under the classification as mild (14(56%), p=0.270). CONCLUSION: According to the results of the present study, it is unlikely that the MnP2 of young patients analyzed in this study would adversely affect adjacent structures.

The study compared 1-year outcomes between transcatheter aortic valve replacement (TAVR) patients with bicuspid aortic valve (BAV) morphology and clinically similar patients having tricuspid aortic valve (TAV) morphology. There are limited prospective data on TAVR using the SAPIEN 3 device in low-surgical-risk patients with severe, symptomatic aortic stenosis and bicuspid anatomy. Low-risk, severe aortic stenosis patients with BAV were candidates for the PARTNER 3 (Placement of Aortic Transcatheter Valves 3) (P3) bicuspid registry or the P3 bicuspid continued access protocol. Patients treated in these registries were pooled and propensity score matched to TAV patients from the P3 randomized TAVR trial. Outcomes were compared between groups. The primary endpoint was the 1-year composite rate of death, stroke, and cardiovascular rehospitalization. Of 320 total submitted BAV patients, 169 (53%) were treated, and most were Sievers type 1. The remaining 151 patients were excluded caused by anatomic or clinical criteria. Propensity score matching with the P3 TAVR cohort (496 patients) yielded 148 pairs. There were no differences in baseline clinical characteristics; however, BAV patients had larger annuli and they experienced longer procedure duration. There was no difference in the primary endpoint between BAV and TAV (10.9% vs 10.2%; P = 0.80) or in the rates of the individual components (death: 0.7% vs 1.4%; P = 0.58; stroke: 2.1% vs 2.0%; P = 0.99; cardiovascular rehospitalization: 9.6% vs 9.5%; P = 0.96). Among highly select bicuspid aortic stenosis low-surgical-risk patients without extensive raphe or subannular calcification, TAVR with the SAPIEN 3 valve demonstrated similar outcomes to a matched cohort of patients with tricuspid aortic stenosis. ObjectiveWe have previously reported that human calcified aortic cusps have abundant expression of smooth muscle (SM) markers and co-activators. We hypothesised that cells in bicuspid aortic valve (BAV) cusps and those affected by rheumatic heart valve (RHV) disease may follow a similar phenotypic transition into smooth muscle cells, a process that could be regulated by transforming growth factors (TGFs).AimsCusps from eight patients with BAV and seven patients with RHV were analysed for early and late SM markers and regulators of SM gene expression by immunocytochemistry and compared to healthy aortic valves from 12 unused heart valve donors. The ability of TGFs to induce these markers in valve endothelial cells (VECs) on two substrates was assessed.ResultsIn total, 7 out of 8 BAVs and all the RHVs showed an increased and atypical expression of early and late SM markers α-SMA, calponin, SM22 and SM-myosin. The SM marker co-activators were aberrantly expressed in six of the BAV and six of the RHV, in a similar regional pattern to the expression of SM markers. Additionally, regions of VECs, and endothelial cells lining the vessels within the cusps were found to be positive for SM markers and co-activators in three BAV and six RHV. Both BAVs and RHVs were significantly thickened and HIF1α expression was prominent in four BAVs and one RHV. The ability of TGFβs to induce the expression of SM markers and myocardin was greater in VECs cultured on fibronectin than on gelatin. Fibronectin was shown to be upregulated in BAVs and RHVs, within the cusps as well as in the basement membrane.ConclusionBicuspid aortic valves and RHVs expressed increased numbers of SM marker-positive VICs and VECs. Concomittantly, these cells expressed MRTF-A and myocardin, key regulators of SM gene expression. TGFβ1 was able to preferentially upregulate SM markers and myocardin in VECs on fibronectin, and fibronectin was found to be upregulated in BAVs and RHVs. These findings suggest a role of VEC as a source of cells that express SM cell markers in BAVs and RHVs. The similarity between SM marker expression in BAVs and RHVs with our previous study with cusps from patients with aortic stenosis suggests the existance of a common pathological pathway between these different pathologies.

Objective: To determine the prevalence of cross-sectional variations in the apical thirds of the root canals in maxillary and mandibular teeth. Material and Methods: Eighty tooth samples (maxillary second premolar, maxillary first molar, and mandibular first molar) were scanned using micro-computed tomography. The apical third area of each root canal was sectioned and the maximum and minimum diameters were calculated from the mesiodistal and buccolingual diameters. The shapes were categorized as a round, oval, long oval, and flat based on the ratios obtained. Results: The most common shape of the apical third of the root canals in the maxillary second premolars was oval (66.7%), followed by long oval (24.6%), flat (7%), and round (1.7%). The corresponding values in the mesiobuccal root of the maxillary first molars were oval (68.2%), long oval (22.7%), flat (9.1%), 94.1% of the distobuccal roots were oval, while the remaining were long oval (5.9%). All the palatal root canals were oval. In the mesiobuccal root of the mandibular first molars, 47.4% were long oval in shape, followed by 36.8% oval and 15.8% flat canals. All the mesiolingual root canals were oval, whereas, in the distal root, 68.4% were oval, 21.1% long oval, and 10.5% were flat. Conclusion: The oval shape was most commonly observed in the majority of the root canals. Knowledge about the apical anatomy of the root can help the operator improve the root canal treatment's success.

Objectives: This study aims to investigate the compliance of randomized controlled trials (RCTs) in posterior restorations with the Consolidated Standards of Reporting Trials Statement (CONSORT) statement and to analyze the risk of bias (RoB) of these studies. Methods: A systematic search was performed in PubMed, Scopus, Web of Science, LILACS/BBO, and Cochrane Library. Only RCTs published in peer-reviewed journals were included. The compliance with the CONSORT was evaluated in a 0–2 scale where 0 = no description, 1 = poor description and 2 = adequate description. Descriptive analyses of the CONSORT mean score by journal, country, and RoB were performed. The RoB in RCTs was evaluated by using the Cochrane Collaboration's tool version 1.0. Results: A total of 15,476 studies were identified after duplicates removal. O only 202 meet the eligibility criteria, among which 31 were follow-up studies. Concerning the overall RoB, only 29 out of 171 were classified as low risk of bias. The overall mean CONSORT score was 19 ± 5.4 points, which means compliance of approximately 59%. Significant differences among countries, publication period, and RoB were observed (p < 0.001). The journal's impact factor was not correlated with the overall CONSORT score (p = 0.36). Conclusions: The adherence of RCTs conducted in posterior restorations to the CONSORT Statement is still low. In addition, most studies were classified as at unclear risk of bias. These results call up an urgent need for improvement. Clinical relevance: Most RCTs conducted in posterior teeth have poor reporting and are mainly classified as having an unclear risk of bias. Introduction: Considering that the existence of anatomical variations causes endodontic treatment failures, therefore it is important to diagnose them. This study aimed to determine the reproducibility and criterion validity of phosphor plate radiographs and sensor-based radiovisiography to identify anatomical variations detected by cone-beam computed tomography (CBCT) in lower premolars. Materials and Methods: 140 premolars images were obtained by CBCT, radiographs and radiovisiography. Independent interpretation was performed by two endodontists to evaluate the Vertucci classification and ramifications. Intraobserver and interobserver reproducibility were determined. Sensitivity, specificity and areas under the receiver-operator curve (AUC) were calculated using CBCT as the gold standard. Results: Intraobserver and interobserver reproducibility was higher for radiography. For Vertucci type I, radiography showed higher sensitivity (94.7%), specificity (64.9%) and AUC (0.795) than radiovisiography (89.3%, 62.2% and 0.757, respectively), similarly for type V (radiography at 69.2%, 93% and 0.8111; radiovisiography at 50%, 84.2% and 0.671, respectively). None of the techniques contributed to the diagnosis of type III (AUC < 0.5). Ramifications were infrequent (2.9%) with a low level of detection (sensitivity at 25% for radiography and 0% for radiovisiography). Discussion: This is the first study to evaluate the reproducibility and validity of these two radiographic techniques compared with CBCT for the detection of anatomical variations in teeth. Conclusions: Phosphor plate radiography showed higher reproducibility and validity for the diagnosis of Vertucci types I and V, which were the most frequent premolar variations found. Introduction: In this study, we aimed to retrospectively evaluate the results of type A intramural hematoma (TA-IMH) cases that underwent ascending aortic surgery. Methods: One hundred ninety-four patients who underwent aortic surgery between 2010 and 2018 were included in this study. TA-IMH was differentiated according to tomography angiographic images. Demographic data, operation type, hypothermic circulatory arrest times, echocardiographic findings, wall thickness of IMH, complications, and prognosis were retrospectively analyzed. Results: TA-IMH (n=14) or type A aortic dissection (AD) (n=35) data were collected from patients' files and 49 cases were enrolled into the study. Bentall operation was performed in eight patients (type A AD = six [17.1%], TA-IMH = two [14.3%]); 41 patients underwent tubular graft interposition of ascending aorta (AD = 29 [82.9%], TA-IMH = 12 [85.7%]). There was no significant difference in terms of age, gender distribution, aortic dimensions, cardiopulmonary bypass times, hypothermic circulatory arrest times, hospital ward stay, and intensive care unit stay between the two groups. The mortality rate of AD group was 34.4% and of TA-IMH group was 14.3%. There was no significant difference in terms of mortality between the groups. In our study, 45.7% of patients had hypertension and that rate was lower than the one found in the literature. In addition, bicuspid aorta was not observed in both groups. Connective tissue disease was not detected in any group. Conclusion: Surgical treatment of aorta is beneficial for TA-IMH. Our aortic surgical indications comply with the European aortic surgical guidelines. Hypertension control should be provided aggressively. [ABSTRACT FROM AUTHOR]

Background: Bicuspid aortic valve may be associated with increased complications during transcatheter aortic valve implantation (TAVI). Aims: Compare balloon-expandable transcatheter heart valve (THV) safety and efficacy in severe tricuspid (TAV) and bicuspid (BAV) aortic stenosis. Methods: Transfemoral TAVI was performed in 743 patients (Jan 2014–June 2019) using the SAPIEN 3 THV. Aortic valve morphology was determined using computed tomography. Valve Academic Research Consortium-2 (VARC-2) derived safety and efficacy endpoints at 1 year were evaluated. Results: BAV patients (n = 78), were younger (77 [72, 81] vs. 81 [78, 85] years, p < 0.001) with lower surgical risk (EuroSCORE II 2.96% vs. 4.51% p < 0.001). Bicuspid valves were more calcified (BAV 1308mm3, TAV 848mm3p < 0.001) with more asymmetric calcification (BAV 63/78 (81%), TAV 239/665 (36%), p < 0.001). Device success (BAV 94%, TAV 90%, p = 0.45) and major vascular complications (BAV 6%, TAV 9%, p = 0.66) were comparable. At 1 year, there was a trend toward lower combined all-cause mortality and rehospitalization for congestive heart failure in BAV patients (BAV 7%, TAV 13%, p = 0.08) with significantly lower all-cause mortality in this cohort (BAV 1%, TAV 9%, p = 0.020). VARC-2 time-related valve safety (BAV 22%, TAV 20%, p = 0.60) was comparable; however, bioprosthetic valve thrombosis remained more common in BAV patients (BAV 7%, TAV 2%, p = 0.010, Hazard ratio 3.57 [95% confidence interval 1.26, 10.10]). After propensity score matching, only bioprosthetic valve thrombosis remained significantly different. Conclusions: Safety and efficacy of the SAPIEN 3 balloon-expandable THV in BAV is comparable with TAV. Higher rates of bioprosthetic valve thrombosis require further investigation. Abstract Background Based on low-dose radiation Cone-bean computed tomography (CBCT) images, This study aims to establish a space coordinate system, which offers more precise and comparable evaluation on changes of maxillary third molars influenced by orthodontic treatment with premolar extraction in adults. The system suggests promising application prospect in future studies related to CBCT superimposition and evaluation for its feasibility and efficiency. Methods Forty-nine maxillary third molars from 27 patients (mean age, 20.78 years) were included. Abstract Introduction: Reconstruction of right ventricular outflow tract during primary repair of tetralogy of Fallot often requires the placement of a transannular patch which results in pulmonary regurgitation (PR). We compared the short-term outcomes of bicuspid polytetrafluoroethylene membrane valve versus transannular pericardial patch reconstruction of the right ventricular outflow tract. Methods: Thirty consecutive patients undergoing primary repair of tetralogy of Fallot were randomly allocated to two groups - polytetrafluoroethylene valve (PTFEV) group (n=15) and transannular pericardial patch (TAP) group (n=15). The two groups had similar preoperative demographic characteristics. We compared the short-term clinical and echocardiographic outcomes between these groups. The transthoracic echocardiographic follow-up was performed at one week, one month and six months after surgery. Results: The PTFEV group had significantly lower central venous pressure in the immediate postoperative period compared to the TAP group (7.60±2.06 vs. 10.13±1.73, P=0.002). Extubation time was significantly shorter in the PTFEV group compared to the TAP group (12.93±7.55 hrs vs. 22.23±15.11 hrs, P=0.04). PR in the PTFEV group was absent in five patients at 24 hours post-surgery. At the study endpoint, PR was absent in six, trivial in one and mild in eight patients in the PTFEV group compared to TAP group, where all 15 patients had severe PR. Conclusion: The bicuspid polytetrafluoroethylene membrane valves significantly decrease the central venous pressure in the immediate postoperative period, facilitate early extubation and, thus, prevent ventilator-related comorbidities. They achieve a high degree of pulmonary competence and do not increase the right ventricular outflow tract gradient in short-term follow-up.

Objectives: It has been reported that bulk-fill composites simplify tooth restoration with no adverse effect on the success rate. This study sought to assess the cuspal deflection of premolars with mesio-occluso-distal (MOD) cavities restored with bulk-fill and conventional posterior composite resins. Materials and Methods: This in-vitro experimental study was conducted on 64 human maxillary premolars. MOD cavities were prepared on teeth and restored with Filtek P60 conventional composite and Filtek Bulk Fill flowable, X-tra fill, and X-tra base bulk-fill composites in four groups (n=16). Distance between the cusp tips was measured before, five minutes, 24 hours, 48 hours, and one week after restoration. The data were analyzed using repeated-measures analysis of variance (ANOVA) and Tukey’s test (α=0.05). Results: The mean±standard deviation (SD) of cuspal deflection at five minutes after the restoration was 13.5±5.3, 12.2±3.5, 11.3±4.4, and 10.4±3.7 µm for Filtek P60, Filtek Bulk Fill, X-tra fill, and X-tra base, respectively. Objective To analyze the orthodontic efficiency of buccal and lingual attachments with different sizes by finite element method in order to determine the optimal position and size of the attachments. Methods Experiment 1: a normal occlusion sample was selected to establish the model. Attachments were divided into five groups: the control group, the buccal horizontal rectangular attachment group, the buccal vertical rectangular attachment group, the lingual horizontal rectangular attachment group and the lingual vertical rectangular attachment group. Except for the control group, the other 4 groups contained 3 subgroups (grouped by rectangular attachment length 3/4/5 mm). The long axis of the maxillary first premolar was set as the rotation central axis, and a 2° clockwise rotation (occlusion view) displacement was applied to carry out simulation operation to observe the stress distribution and displacement changes. In experiment 2, the attachment was modified and divided into 4 groups: the control group, the lingual b (2/3) rectangular attachment group, the lingual b (1/2) rectangular attachment group and the lingual b (1/3) rectangular attachment group. The method was the same as experiment 1. Results In experiment 1, the periodontal membrane stress, the maximum displacement of the crown and the torsion efficiency of orthodontic force showed the maximum stress with the 3 mm attachment size in the lingual vertical group, and it decreased with the increase of the attachment size units in each group. The vertical group was larger than the horizontal group, and the expression of the torsion degree efficiency was the smallest in the group without the attachment. In the experiment 2, the periodontal membrane stress and the maximum displacement of the crown of the b (2/3) lingual vertical group were the closest to that of the 3 mm lingual vertical group, while the highest expression of torsion efficiency was the 3 mm lingual vertical group, and decreased with the decrease of the size. Conclusion The 3 mm lingual vertical group shows more beneficial to correct torsional premolars, and the b (2/3) lingual vertical attachment could be used as the improved specification. We present a rare case of Brucella endocarditis in a case of the bicuspid aortic valve in a 51-year-old male presenting with pyrexia of unknown origin for the last 2 months. Infective endocarditis caused by Brucella melitensis is rare to encounter in routine clinical practice. Although we routinely think of Mycoplasma, Legionella, and Coxiella in the cases of blood culture-negative endocarditis, one should think of Brucella melitensis and Scrub typhus as the last arrow to reach the final etiology of infective endocarditis in the subset of diseased aortic or mitral valve. We successfully treated the patient with a combined regimen of rifampicin, gentamicin, and doxycycline therapy; during follow-up, the vegetation of the aortic valve was shrunken and calcified and the patient was asymptomatic.

Open in new tab Download slide Open in new tab Download slide OBJECTIVES To compare long-term outcomes after bicuspid aortic valve (BAV) repair utilizing the Cabrol annuloplasty versus valve sparing Reimplantation technique. METHODS From 1996 to 2018, 340 consecutive patients underwent BAV repair. Eighty underwent Cabrol annuloplasty and 189 underwent Reimplantation. Exclusion criteria were re-repairs (n  = 6), active endocarditis (n  = 4), no annuloplasty (n  = 41) and ring or suture annuloplasty (n  = 20). We compared both groups for survival, reoperations, valve related events and recurrent severe aortic regurgitation (AR > 2+). Inverse probability weighting (IPW) was used to balance the 2 groups. Cox regression analysis was used to identify outcome predictors. RESULTS After weighting, pre- and intraoperative characteristics were similar between groups, except for aorta replacement techniques and operative time, which was longer in the Reimplantation group (P  < 0.001). At 12 years, overall survival was similar between groups (IPW: Cabrol 97 ± 2% vs Reimplantation 94 ± 3%, P  = 0.52). Freedom from reoperation and freedom from AR > 2+ were significantly lower in the Cabrol group (reoperation IPW: 69 ± 9% vs 91 ± 4%, P  = 0.004 and AR > 2+ IPW: 71 ± 8% vs 97 ± 2%, P  < 0.001). The Reimplantation technique was the only independent predictor of reoperation (hazard ratio 0.31; confidence interval 0.19–0.7; P  = 0.005). CONCLUSIONS In this study, comparing 2 annuloplasty strategies for BAV repair, we found statistically significant differences in long-term durability favouring the Reimplantation technique, and no differences in overall survival. The results support our current strategy of Reimplantation technique and repair of AR in patients with BAV. Background: Appropriate cleaning and shaping and three-dimensional obturation of the root canal system lead to a successful endodontic treatment. To achieve this, complete knowledge of the internal anatomy of root canals is necessary. Therefore, this study evaluated the number and shape of mandibular premolar roots and canals and classified their Vertucci classification using cone-beam computed tomography (CBCT) images of an Iranian sample population. Materials and Methods: This in vivo study was performed on CBCT images of patients aged 20-70 years who had attended a dental radiology center in Isfahan. CBCT images were examined in coronal, sagittal, and especially axial dimensions. Descriptive statistics were calculated. Groups were compared using the Chi-square of the Statistical Package for the Social Sciences. The level of significance was predetermined as 0.05. Results: In both first and second premolars, the most common classes were Type I followed by Type V. The Chi-square did not show significant differences between males and females in terms of Vertucci classes in the first (P = 0.305) or second premolar (P = 0.315). Conclusion: Since a thorough knowledge of root canal anatomy is necessary for successful root canal treatment and almost one out of ten mandibular premolars has additional canal in their root canal system, accurate evaluating of preoperative radiographs, taking CBCT images if necessary, and probing the root canal system with fine and precurved files should be done to negotiate the entire canal system. To evaluate the results after selective sinus replacement (SSR) for aortic root remodeling in bicuspid aortopathy. Among 662 patients who underwent root repair using SSR between 2005 and 2020, there were 114 with bicuspid aortopathy. SSR was performed either as an isolated procedure (31) or as an adjunct to aortic valve repair (83) and was adjusted to the existing aortic annulus diameter rather than a downsized diameter. In valves with asymmetrical commissural orientation, the repair aimed for the achievement of a 180°-commissural orientation. Abolishment of aortic insufficiency (AI) ≥2+ using root repair alone was only possible in 2 patients with acute-dissection-related AI, yet isolated root repair was also performed in 29 further patients with no/mild AI. All remaining patients with AI ≥2+ presented cusp-related regurgitation and necessitated an additional valve repair. During the mean follow-up of 91 months (range, 13-196), a relevant valve defect (AI ≥3+ in 8, stenosis in 2) occurred in 10 patients (all after combined repair) resulting in an estimated freedom from a relevant aortic valve defect and/or reoperation of 96 ± 2%, 89 ± 4%, and 82 ± 6% at 5, 10, and 12 years, respectively. Echocardiographically, no patient revealed a considerable change of form or size of the repaired root nor was any root reintervention necessary. Patient-tailored root repair using SSR is a very effective and durable valve-sparing approach for bicuspid aortopathy. Objective. The restorative treatment of a decayed, non-vital upper premolar often requires an interdisciplinary approach. Esthetics and the entity of the masticatory loads are usually affecting the choice of procedures and materials. A conservative treatment in a case with esthetic needs and functional requirements is investigated in this case report. Materials and methods. A case of a 45- yearold woman with a severely decayed upper premolar is presented. The tooth has been previously treated in an emergency department with a provisional endodontic medication, but a further treatment has been then performed. The tooth was painful at the moment of the visit and the carious lesion appeared to invade the subgingival level. A surgical-conservative approach has been selected involving crown lengthening, endodontic treatment and a prosthetic crown with a monolithic Lithium- Silicate. Results. The rehabilitative process required 3 months, including maturation times after surgery and the placement of the provisional crown. The patient claimed to be both esthetically and functionally satisfied with the restoration. Conclusion. A conservative treatment of a severely involved tooth requires a critical evaluation of the remaining structures and a precise selection of the restorative materials. A monolithic crown could represent an ideal solution for restoring an upper premolar requiring esthetics and solid function, but the choice of a high translucent, easily polishable and possibly even chair-side prosthetic material might be an appreciable added value for clinicians.

Abstract Introduction: In this study, we aimed to retrospectively evaluate the results of type A intramural hematoma (TA-IMH) cases that underwent ascending aortic surgery. Methods: One hundred ninety-four patients who underwent aortic surgery between 2010 and 2018 were included in this study. TA-IMH was differentiated according to tomography angiographic images. Demographic data, operation type, hypothermic circulatory arrest times, echocardiographic findings, wall thickness of IMH, complications, and prognosis were retrospectively analyzed. Results: TA-IMH (n=14) or type A aortic dissection (AD) (n=35) data were collected from patients’ files and 49 cases were enrolled into the study. Bentall operation was performed in eight patients (type A AD = six [17.1%], TA-IMH = two [14.3%]); 41 patients underwent tubular graft interposition of ascending aorta (AD = 29 [82.9%], TA-IMH = 12 [85.7%]). There was no significant difference in terms of age, gender distribution, aortic dimensions, cardiopulmonary bypass times, hypothermic circulatory arrest times, hospital ward stay, and intensive care unit stay between the two groups. The mortality rate of AD group was 34.4% and of TA-IMH group was 14.3%. There was no significant difference in terms of mortality between the groups. In our study, 45.7% of patients had hypertension and that rate was lower than the one found in the literature. In addition, bicuspid aorta was not observed in both groups. Connective tissue disease was not detected in any group. Conclusion: Surgical treatment of aorta is beneficial for TA-IMH. Our aortic surgical indications comply with the European aortic surgical guidelines. Hypertension control should be provided aggressively.

Bicuspid aortic valve (BAV) disease presents a unique management challenge both pre- and post-operatively. 4D flow MRI offers multiple tools for the assessment of the thoracic aorta in aortic valve disease. In particular, its assessment of flow patterns and wall shear stress have led to new understandings around the mechanisms of aneurysm development in BAV disease. Novel parameters have now been developed that have the potential to predict pathological aortic dilatation and may help to risk stratify BAV patients in future. This systematic review analyses the current 4D flow MRI literature after aortic valve and/or ascending aortic replacement in bicuspid aortic valve disease. 4D flow MRI has also identified distinct challenges posed by this cohort at the time of valve replacement compared to standard management of tri-leaflet disorders, and may help tailor the type and timing of replacement. Eccentric pathological flow patterns seen after bioprosthetic valve implantation, but not with mechanical prostheses, might be an important future consideration in intervention planning. 4D flow MRI also has promising potential in supporting the development of artificial valve prostheses and aortic conduits with more physiological flow patterns.

Objectives: To define the incidence of high residual gradient (HRG) after transcatheter aortic valve replacement (TAVR) in BAVs and their impact on short term outcome and 1-year mortality. Background: Transcatheter heart valves (THVs) offer good performance in tricuspid aortic valves with low rate of HRG. However, data regarding their performance in bicuspid aortic valves (BAV) are still lacking. Methods: The BEAT (Balloon vs Self-Expandable valve for the treatment of bicuspid Aortic valve sTenosis) registry included 353 consecutive patients who underwent TAVR (Evolut R/PRO or Sapien 3 valves) in BAV between June 2013 and October 2018. The primary endpoint was device unsuccess with post-procedural HRG (mean gradient ≥ 20 mmHg). The secondary endpoint was to identify the predictors of HRG following the procedure. Results: Twenty patients (5.6%) showed HRG after TAVR. Patients with HRG presented higher body mass index (BMI) (30.7 ± 9.3 vs. 25.9 ± 4.8; p < 0.0001) and higher baseline aortic mean gradients (57.6 ± 13.4 mmHg vs. 47.7 ± 16.6, p = 0.013) and more often presented with BAV of Sievers type 0 than patients without HRG. At multivariate analysis, BMI [odds ratio (OR) 1.12; 95% confidence interval (CI) 1.05–1.20, p = 0.001] and BAV type 0 (OR 11.31, 95% CI 3.45–37.06, p < 0.0001) were confirmed as independent predictors of high gradient. Conclusion: HRG following TAVR in BAVs is not negligible and is higher among patients with high BMI and with BAV 0 anatomy. [ABSTRACT FROM AUTHOR]

Objective: This retrospective study aimed to compare the occlusal and dentoskeletal initial features of patients treated with four first premolar extractions in the 1970s and after 2000. Materials and methods: Group 70′ was composed by 30 subjects with Class I malocclusion (mean age of 12.8 years, 10 male, 20 female) treated in the 1970s with four first premolar extractions and comprehensive orthodontic treatment. Group NM comprised 30 subjects with Class I malocclusion (mean age of 13.4 years, 13 male, 17 female) treated in the new millennium, similarly to Group 70′. Initial dental models and lateral cephalograms were digitized and measured using OrthoAnalyzerTM 3D software and Dolphin Imaging 11.0 software, respectively. Initial occlusal and dentoskeletal features were analyzed and compared. Intergroup comparison was performed using t tests (p < 0.05). Holm-Bonferroni correction for multiple comparison was applied. Results: Group NM showed significantly greater maxillary and mandibular effective lengths and greater maxillary and mandibular incisor protrusion in comparison with Group 70′. Group NM presented a significantly greater lower anterior facial height. Group NM also showed significantly smaller nasolabial angle and protruded inferior lip. Conclusion: Patients with Class I malocclusion treated with four first premolar extractions in the new millennium present a greater degree of dental and labial protrusion, increased lower anterior facial height, and more acute nasolabial angle compared with patients treated similarly in the 1970s. Greater dental and labial protrusion determines first premolar extractions in the new millennium. Clinical relevance: Despite the decrease of tooth extraction frequency, four first premolar extractions may be justified in cases with severe dental and skeletal protrusions. [ABSTRACT FROM AUTHOR]

BACKGROUND AND OBJECTIVE: Mandibular second premolars are the third most frequent impacted. By evaluating the three-dimensional position of the impacted tooth and timely treatment, possible problems and damage to adjacent structures can be prevented. This study was designed to assess the position of Impacted Mandibular Second Premolar (MnP2) as well as the effect on adjacent structures using CBCT imaging due to the lack of adequate studies in this field. METHODS: This cross-sectional study was performed on 25 impacted mandibular second premolars. Two radiologists assessed and compared CBCT images to determine the following variables: unilateral/bilateral, position of the crown, root resorption of the adjacent permanent teeth, root dilaceration, depth of impaction, type of impaction, pathology incidence, residual primary tooth, position and distance to inferior alveolar nerve canal and the mental foramen. FINDINGS: In most cases, unilateral impaction was seen (19(76%), p=0.063). Crown position of most of MnP2 was displaced lingually (17(68%), p=0.324). Vertical and distoangular position of tooth were more common (12(48%) and 5(20%), p=0.922). Root resorption of the adjacent permanent teeth and dental pathologies did not occur in any of cases. Most of Mnp2 were categorized under the classification as mild (14(56%), p=0.270). CONCLUSION: According to the results of the present study, it is unlikely that the MnP2 of young patients analyzed in this study would adversely affect adjacent structures. [ABSTRACT FROM AUTHOR]
