Care transitions (CT) are critical junctures in the healthcare delivery process. Effective transitions reduce the need for subsequent transfers between healthcare settings, including nursing homes. Understanding social services (SS) involvement in these processes in nursing homes is important from a quality and holistic care perspective. Using logistic regression, this study examines structural and relational factors identified with higher involvement of SS in care transitions and admissions. SS directors from 924 nursing homes were evaluated in relation to SS involvement in care transitions and admissions processes. Results suggest the level of SS involvement in care transitions and admissions are associated with structural factors such as size of facility, geographical location, ratio of FTE's to beds, ownership status, and standalone SS departments, as well as relational factors, including perceptions and utilization of SS staff by facility leadership, coworkers, and family. Additionally, SS staff with higher levels of expertise and with social work degrees are less involved in admissions tasks.

This article aims to increase knowledge of cardiac assessment. Anatomy and physiology of the heart are briefly reviewed and reference is made to pathology that can cause cardiac dysfunction. The main features to look for when taking a cardiac history are discussed, with suggestions for questions to elicit this information, and the signs to look for when undertaking a cardiac examination. There is also an introduction to the main investigations to aid differential diagnosis and clinical reasoning. A follow-up article will look in more detail at some common cardiac conditions presenting to emergency care, with an emphasis on critical thinking and diagnostic reasoning. These articles are written from an emergency care perspective, and therefore do not focus in great detail on invasive investigation of cardiology conditions, but more so on picking up these possibilities in undifferentiated patients presenting to emergency care.

In 1973, analysis of the mummified remains of a young boy dated to 700AD and from the Late Nasca period (Peru) identified (i) chronic Pott’s disease, leading to loss of lower body mobility, and (ii) acute miliary tuberculosis, affecting most organs and the immediate cause of death ( Allison et al., 1973 ). This report was the first to establish, beyond dispute, the presence of tuberculosis in the Americas before the arrival of Europeans. Here, we revisit the ‘Nasca Boy’ from a bioarchaeology of care perspective. Contextualising the original study’s results within what is known of contemporary lifeways, we apply the bioarchaeology of care methodology in considering the Nasca Boy’s experience of living with tuberculosis; the type of care he required and how this may have evolved over a period of deteriorating health; and what such caregiving may suggest both about social organisation within his community and some of the more everyday aspects of Nasca existence. Up to now, the bioarchaeology of care approach has been employed almost exclusively with skeletal evidence; in this analysis of the Nasca Boy’s remains, and in the accompanying wider-ranging discussion, we illustrate the potential of preserved soft tissue evidence to contribute to research into disability and care in the past. Although this report functions as a stand-alone case study, to obtain maximum benefit it should be read in conjunction with the Introduction to the special International Journal of Paleopathology issue on ‘mummy studies and the bioarchaeology of care’ ( Nystrom and Tilley, 2018 ).

Objective The aim of this study was to develop an instrument that measures all healthcare staff's perceptions of a culture of caring. Background Healthcare organizations have increased their focus on cultures of caring within their staff to improve staff satisfaction and patient satisfaction. Nurses and physicians traditionally have been the focus in understanding caring cultures excluding non-direct care staff who do impact organizational culture. Methods An exploratory and partial confirmatory factor analysis was completed on 2 instruments that measure the perception of a caring culture in both direct care and non-direct care staff. Results Results demonstrate that both the direct and non-direct care instruments are valid and reliable measures for assessing a caring culture. Conclusions The new 14-item clinical and new 10-item nonclinical staff caring assessment instruments will provide healthcare and nursing leaders with the ability to assess their culture through a direct and non-direct care perspective.

Background In Neonatal Intensive Care Units (NICUs), parent satisfaction and their experiences are fundamental to assess clinical practice and improve the quality of care delivered to infants and parents. Recently, a specific instrument, the EMpowerment of PArents in THe Intensive Care-Neonatology (EMPATHIC-N), has been developed in the Netherlands. This instrument investigated different domains of care in NICUs from a family-centered care perspective. In Italy, no rigorous instruments are available to evaluate parent satisfaction and experiences in NICU with family-centered care. The aim of this study was to translate and validate the EMPATHIC-N instrument into Italian language measuring parent satisfaction. Methods A psychometric study was conducted in nine Italian NICUs. The hospitals were allocated across Italy: four in the North, four in Central region, one in the South. Parents whose infants were discharged from the Units were enrolled. Parents whose infants died were excluded. Results Back-forward translation was conducted. Twelve parents reviewed the instrument to assess the cultural adaptation; none of the items fell below the cut-off of 80% agreement. A total of 186 parents of infants who were discharged from nine NICUs were invited to participate and 162 parents responded and returned the questionnaire (87%). The mean scores of the individual items varied between 4.3 and 5.9. Confirmatory factor analysis was performed and all factor loadings were statistically significant with the exception of item ‘Our cultural background was taken into account’. The items related to overall satisfaction showed a higher trend with mean values of 5.8 and 5.9. The Cronbach’s alpha’s (at domain level 0.73-0.92) and corrected item-total scale correlations revealed high reliability estimates. Conclusions The Italian EMPATHIC-N showed to be a valid and reliable instrument measuring parent satisfaction in NICUs from a family-centered care perspective. Indeed, it had good psychometric properties, validity, and reliability. Furthermore, this instrument is fundamental for further research and internationally benchmarking. Over the past decade, there has been substantial growth in empirical evidence supporting that proper assessment and treatment of trauma using evidence-based practices can effectively reduce a wide range of symptoms in both children and adults. Given the complex nature of trauma treatment, trauma-based educational programming in social work is most commonly found at the graduate level. Yet, to date, there has been little discussion calling for the inclusion of trauma content in BSW education. In this paper, we examine the current state of trauma-content inclusion in social work education, and offer a guiding framework for integrating core trauma content into the BSW curriculum that is based on the core principles of trauma-informed care.

An updated version of a chapter originally appearing in A Century of Transformation: Studies in Honor of the 100th Anniversary of the Eastern Communication Association, this piece examines the history and continued development of interpersonal communication. We begin by examining the progression of interpersonal communication from the 1920s through the 1980s. We then explore contemporary trends in interpersonal communication since the 1990s. Finally, we consider the challenges and future trajectories of interpersonal communication as it continues to develop and transform. We identify major aspects of research and how they are informed by identity and raise critical challenges to studying interpersonal communication. We end by exploring how transformations in technology are inexorably connected to our understanding and valuing interaction with others, and identify emerging areas of inquiry.

Study Objective To determine the sexual health and well-being needs of current generations of youth with intersex/Differences of Sex Development (DSD) during transition from pediatric to adult health care. Design Qualitative narrative analyses, quantitative descriptives, and questionnaires. Setting Peer support networks and outpatient clinics. Participants Eighteen adolescents aged 16-21 years with intersex/DSD. Interventions Semi-structured interviews and/or survey. Main Outcome Measures Youths learning about bodily differences, their sexual experiences and motives (eg, agency, pleasure), body image, sexual communication inside and outside of health care, and perceived gaps between current and ideal transitional care. Quantitative and qualitative content of the surveys and interviews were analyzed to identify key topics. Results We found that (1) there is a need for open-minded perceptions of health care providers about what it means to have a sex variation: (2) there is a need for continued support and information about lived realities relating to the diagnosis and treatments as well as experiential aspects of sexuality: and (3) there are communication obstacles with providers in a multidisciplinary team setting as well as with parents. Conclusion A user-centered care perspective for adolescents with sex variations includes their stories and feedback toward service improvement. This pilot study shows that adolescents want to be more involved in their sexual health care in ways that connect to their specific questions. Their desire for information increases as they grow older, and underlines the most essential lesson that health care providers can bring their patients about their bodies: how to care for, respect, and enjoy them.

Onder de radar. Alledaagse omgang als kenbron voor zorgorganisaties in transitie Zorgorganisaties staan voor de taak om in de transitie van de zorg en een steeds complexer wordende samenleving hun maatschappelijke opdracht vorm te geven. Dan is de vraag aan de orde welke bronnen zorgorganisaties hebben om te weten waarheen zij wensen te navigeren. Voor zorgorganisaties die ondersteuning bieden aan jongeren met een licht verstandelijke beperking (lvb-jongeren) heeft deze opdracht een dubbel karakter. Enerzijds bestaat ze uit het beschermen van de lvb-jongeren en de samenleving, anderzijds uit het realiseren van een leven in vrijheid. Op basis van zorgethisch georienteerd, conceptueel en kwalitatief empirisch onderzoek wijzen wij op de morele betekenis van alledaagse omgang tussen lvb-jongeren en hun professionele ondersteuners. Aan de hand van fragmenten uit ons institutioneel etnografisch onderzoek die gerelateerd worden aan de theorie van Rahel Jaeggi over levensvormen, laten we zien dat er in de alledaagse omgang een bron van moreel kennen aanwezig is die oplossingskracht in zich draagt. Het antwoord op de vraag “wat behelst goed samenleven?” blijkt verborgen te liggen in de alledaagse omgang tussen lvb-jongere en zorgprofessional zelf. Deze bron wordt veelal over het hoofd gezien. In plaats van enkel op orientatie van buitenaf te koersen hebben zorgorganisaties “onder de radar” een interne morele kenbron ter beschikking. Under the radar: everyday contact as a source of knowledge for healthcare organizations in transition The Dutch healthcare and welfare are systems in transition (RVS, 2016) and healthcare organizations are facing the task of reinventing their mission in society. Which compass should they use in this respect? The mission of organizations that support MID youngsters is dual in nature. On the one hand, it consists of protecting the MID youngsters and society; on the other, it should assist them in realizing a life of freedom. The ultimate issue is: What constitutes the good life for MID youngsters? On the basis of ethical and care-oriented, qualitative empirical and conceptual research we provide some answers, suggesting that if healthcare organizations were more willing to inquire into the everyday dealings of MID youngsters and their care professionals, they would detect moral orientations that already support a good life. Care organizations can find their moral compass in the good life that MID youngsters and professionals invent on an everyday basis. The ethical care perspective of our research reveals the moral relevance of everyday interactions (Tronto, 1993; Sevenhuijsen, 1998; Pols, 2014; Barnes, 2012; Barnes, Branelly, Ward & Ward, 2015). In addition, an ethics of care can capture an ideal of the good life in everyday life in an institutional context (Laugier, 2014a). As Michel de Certeau pointed out in his ethnographic theory (1984), where everyday life is institutionally constricted, people themselves: 1) keep appropriating freedom and 2) generating knowledge. Thus, we are encouraged to pay closer attention to the epistemological dimensions of “power” – who provides the knowledge that counts and in what, possibly unexpected, ways is this effective? In the qualitative empirical part of the study, Institutional Ethnography (Devault, 2013; Prodinger & Turner, 2013; Smith, 2006) is used as a research methodology. This approach enabled the identification of the role of institutions in everyday dealings. We recruited nineteen respondents from three Dutch healthcare organizations, all aged between 18 and 27. Each youngster was shadowed (Johnson, 2014) during the course of one day. The observation reports were analysed using the constant comparative method (Patton, 2002; Boeije, 2005). Here we focus on results that are of relevance to care organizations attempting to find an appropriate position in society. The first finding is that while the institutional context projects contradictory normative ideas about what is “good” in everyday dealings, it does allow for these dealings by means of the physical infrastructure (building), the professionals involved, etc. Framed praxeologically (Nicolini, 2012), the institutional context itself is regarded as an actor in everyday dealings. The second finding is that in every meeting MID youngsters constantly highlight their concerns: what matters to them (Sayer, 2011). Finally, care professionals appear to respond in two ways to the dynamics implicit in the everyday dealings, either by: 1) responding to specific requirements arising from the institutional context; or 2) responding by engaging with the concerns of the youngster. Below, we expound on the latter, calling it “attending to the relationship”. This characterization is based on Hannah Arendt’s concept of power (Vosman, 1999). According to Arendt, power is not the same as dominance but is the ability to express what matters in an interaction. If the everyday interaction is fostered: 1) the professional’s involvement will be visible, 2) the MID youngster’s concerns will be voiced and heard, and 3) it will become apparent that care relationships are located at the intersection of the private and the public. To obtain a clearer picture of the way in which a healthcare organization shapes society, we refer to Rahel Jaeggi’s (2014) theory and critique of life forms. We consider these everyday dealings between MID youngsters and their care professionals as a “life form” in Jaeggi’s sense: as a bundle of practices. Jaeggi also offers a well-argued case for how these can be critically evaluated. First and foremost, have these life forms proved to be morally decent? According to Jaeggi, every life form addresses a problem. The particular life form we are concerned with here exists because MID youngsters cannot independently participate in society because of their learning disabilities, whereas paradoxically this participation is demanded by society. Every life form is characterized by an ensemble of conflicting, normatively charged practices. At the same time, life forms are “instances of problem solving” (Jaeggi, 2014), in which problems are transformed. In this way, while problems do not disappear, ways of dealing with them are found. The criteria for determining whether such a “solution” can be regarded as morally satisfactory are immanent to the practices. This is because the criteria are located in the dynamic of the “solution” itself. A “solution”: a) is provided for the actual problem, b) provides a critical reflection on how the problem has arisen, c) allows room for the experiences of all of the people concerned and d) does not result in stagnation but leads to transformation. These “instances of problem-solving” are key to a morally well-organized society (Jaeggi, 2014). Finally, the life form, as problem solver, enables all parties involved to construct or adjust life forms. In this case, this can only be achieved with the knowledge of the MID youngsters themselves, which allows them to manoeuvre through the adversities associated with their vulnerability. If healthcare organizations in transition focus their attention on the everyday interactions between healthcare professionals and MID youngsters they will discover that: 1) they have an implicit source of moral wisdom that grounds their critical knowledge about living well together and 2) they can create a space in society from which the voice of MID youngsters can be heard (De Certeau, 1984) and from where, following Arendt, their personality can appear in a significant way – in this relationship. This touches on the political heart of the organizations themselves (Heijst & Vosman, 2010). Healthcare organizations are in the position to create a “social arrangement” in which everyday life can be lived and simultaneously examined in a critical way. For healthcare organizations in transition it is important to not only focus on external ideological concepts or on developing professional expertise, but also to search “under the radar” and draw insight from the source of the good life in everyday dealings.

Therapeutic interpersonal relationships are the primary component of all health care interactions that facilitate the development of positive clinician-patient experiences. Therapeutic interpersonal relationships have the capacity to transform and enrich the patients' experiences. Consequently, with an increasing necessity to focus on patient-centered care, it is imperative for health care professionals to therapeutically engage with patients to improve health-related outcomes. Studies were identified through an electronic search, using the PubMed, Cumulative Index to Nursing and Allied Health Literature, and PsycINFO databases of peer-reviewed research, limited to the English language with search terms developed to reflect therapeutic interpersonal relationships between health care professionals and patients in the acute care setting. This study found that therapeutic listening, responding to patient emotions and unmet needs, and patient centeredness were key characteristics of strategies for improving therapeutic interpersonal relationships.

This article explores motivation in a social context: how people pursue goals with others, with information on others, and for the self and others. As people incorporate close others into their extended selves (Aron et al., 1991), they begin to treat others' actions and outcomes as partially their own. This tendency, in turn, has implications for coordinating goal pursuits with others and for the preference for actions that maximize the total benefits for the self and others. To demonstrate these principles – coordination and joint-benefits maximization – we first explore coordination in pursuing goals with others (i.e., working in teams), showing that people respond to others' actions and lack of action similarly to how they respond to their own actions and lack of action. We next explore coordination in pursuing goals with information on others, showing that people conform to others' preferences and attitudes yet choose actions that complement others' actions. Finally, we review research on pursuing goals for the self and others, showing that people wish to maximize the total benefits for the group.

Students are often placed in groups to facilitate learning. Understanding the cognitive processes involved when students learn from others is important for creating group situations that facilitate learning. Research from cognitive psychology predicts who will learn most from whom and in what situations. It also provides a set of methods that education researchers can use to further explore “Learning from Others.” However, because the goals of cognitive psychologists and educators often differ, it is important to apply cognitive psychology research cautiously within an educational context.

Much of the attention concerning youth in care focuses on the ways they are being helped and supported. This study focuses on the motivations and experiences which lead youth in care to assume a helper role, the meaning they ascribe to such a role and the benefits consequently gained. The study sample consisted of 28 Israeli and German care leavers, aged 18–26, who had begun, were about to begin or had already finished higher education. The results show various motivations for assuming a helper role e.g. socialization through early parental roles within biological families, modeling by significant others, and exposure to pro-social values and opportunities for volunteering within the care systems. The ways that these young people support others vary. Some volunteer within their communities, some support members of their family of origin, and others integrated the idea of supporting others into their career choice. According to these young people, assuming a helper role provided a strong sense of purpose in life and contributed to their self-efficacy, social connectedness and ability to cope with their adverse past. In particular, supporting others seems to reflect care leavers' wish to lead a normal life.

This paper uses Tronto's political ethics of care as a normative framework to evaluate a model of teaching and learning professional development. This framework identifies five integrated moral elements of care – attentiveness, responsibility, competence, responsiveness and trust. This paper explicates on each of these elements to evaluate the piloting and implementation of a teaching and learning professional development model at a South African higher education institution. The political ethics of care was found to be a useful normative framework for a group of higher educators to reflect on the process of engaging in teaching and learning professional development in that it revealed the importance of differential power relations, the importance of working collaboratively and being attentive to the needs of both caregivers and care receivers.

A variety of factors impacts the transition from childhood to adulthood for persons with special health care needs or disabilities. Among these are the barriers in care coordination associated with our bifurcated health care delivery system in which pediatric medicine does not smoothly link up with the medical care received by adults. After reviewing the literature, we discuss the strong case to be made for reconceptualizing the medical home model, introducing a life span perspective to resolve the postpediatrician transition. Rather than a “hand off” from pediatrician to adult provider, this approach provides a pathway to fully integrating this patient population into our evolving health care system.

Significant others are often crucial for suicidal persons or suicide attempters’ access to care, yet little is known about their efforts to seek help. This article presents the findings of a qualitative pilot study carried out in Switzerland on the help-seeking process of 18 significant others, their perception of the care received by their loved one, and the interactions and collaboration they experienced with professionals. Most significant others repeatedly sought out support for their loved one and themselves. The help-seeking process seemed mostly difficult, was seldom successful on the first attempt, and was filled with multiple difficulties, such as availability and continuity of care and cooperation issues with professionals. Two-thirds of participants were not satisfied with the care provided to their loved ones and half of them faced challenges in their cooperation with professionals, i.e., poor sharing of information or not being acknowledged as partners or supported by professionals. Based on their experience, providing education about suicidal crises and care programs to significant others might lighten their burden and improve their cooperation with professionals, who in turn may benefit from training in communication issues and specific methods of cooperation with significant others in suicidal situations.

This study explores whether or not technology-mediated interpersonal communication activities with co-ethnics weakens the vital role that direct social engagements with members of the host society play in the cross-cultural adaptation process. Grounded in Kim's integrative theory of cross-cultural adaptation, this study addresses this question through a detailed examination of the extent to which non-natives participate in face-to-face and mediated “host interpersonal communication” and “ethnic interpersonal communication” through dyadic relationships and organizational affiliations. The analysis utilizes portions of the quantitative and qualitative data obtained from in-depth interviews with 51 relatively well-educated foreign-born residents in the United States. The results indicate that: (1) technology-mediated forms of communication such as email and the Internet serve as the primary means for maintaining contacts with family and friends who remain in the country of origin; (2) non-natives are engaged predominantly in activities of host interpersonal communication rather than ethnic interpersonal communication; and (3) non-natives’ involvements in direct host interpersonal communication, but not their ethnic interpersonal communication, are significantly correlated with their functional and psychological well-being. These and related findings suggest the continuing primacy of non-natives’ direct social engagement with members of the host society in the cross-cultural adaptation process.

Anastomotic leak after bariatric surgery is a rare complication with a recent prevalence ranging from 0.8% to 1.5%. The complication nevertheless can result in morbidity and even mortality. The purpose of this paper is to present a patient who suffered from an anastomotic leak presenting 2 days after laparoscopic sleeve gastrectomy in our intensive care unit. Review of the current literature regarding this complication from critical care perspective is also attempted.

This survey explored the perceptions of South African professional accountants regarding the phenomenon interpersonal communication within their day-to-day experiences within their organisational settings and the perceived role of interpersonal communication in organisations. The literature on the concepts interpersonal communication, organisational communication and perception as elements of communication theory and the findings of the survey revealed that (1) interpersonal communication, as a relational process, assumes the middle ground between its transactional and relational nature. The social relational model describes interpersonal communication from a social point of view, whereas accountants refer to its relational nature only on a professional basis or when it concerns business; (2) accountants do not necessarily define themselves in terms of human communication but rather on the work that they do, and as such, whenever accountants are dealt with or addressed on their relations in the work place, their work should be the point of departure; and (3) interpersonal communication is perceived to have an important role not only in enhancing the human element in the profession but also as a primary basis of individuals' professional and personal identity. People often adopt self-image goals to increase others' regard for them and perhaps their own self-esteem. But do these impression management goals achieve their intended result in close relationships? And do they endure over time? We suggest that self-image goals predict decreased self-esteem and close others' regard for the self through decreased responsiveness to others. In contrast, compassionate goals, which reflect a genuine concern for others' well-being, predict increased self-esteem and others' regard through increased responsiveness. We tested these hypotheses in a longitudinal study of college roommates followed across a semester. Path analyses supported both predictions, suggesting a paradox for interpersonal goals in close relationships: explicit attempts to increase close others' regard for the self backfire and damage self-esteem, but having goals to meet others' needs result in others' positive regard and promote self-esteem. The adoption of personalisation as the principle on which policy and practices for social care in England should be developed has been hailed as marking a fundamental transformation in the nature of social care and the experiences of service users. This article examines both the discourse of personalisation and the practices that are being adopted to implement this from an ethic of care perspective. It adopts an approach based on Sevenhuijsen's ‘Trace’ analysis to trace the normative frameworks in key policy documents (in particular Putting People First), noting that critics of care have largely succeeded in relegating care to a marginal position within policy discourse and that a relational sensibility is largely absent within this. The article considers the practices associated with personalisation in relation to the moral principles of an ethic of care and conceptions of the ‘individual’ within these. Objectives: To have students identify and explain concepts of communication, self-disclosure and empowerment, and promote engagement of self and others through performing narratives of real-life experiences. The concept of communication competence has developed in the area of nonmediated interpersonal communication. With the advent of digital media, interpersonal communication has been heavily dependent on interpersonal media. The sharp distinction between theories for nonmediated communication and mediated communication is disappearing. The interconnectedness of face-to-face communication and mediated interpersonal communication is a salient current phenomenon. To explore the relation between two communication types this study investigated the relevance of the concept of communication competence with the interpersonal digital media; mobile phone, SMS, and instant messenger. Three facets were examined in particular: namely, the direct effects of communication competence and the indirect effects through individuals' cognitive process for interpersonal media and individuals' characteristics in interpersonal communication. The hypothesized model of mediated interpersonal communication competence was analyzed using structural equation model. The results revealed that communication competence was positively associated with mediated interpersonal communication competence. The indirect effects of communication competence through media efficacy and social presence were empirically supported. Relationship maintenance was found to mediate the effect of communication competence on mediated interpersonal communication competence. The links of two different communication fields and the theoretical implications of the findings were further discussed.

Within the framework of constructivism, and more specifically personal construct psychology, the current research aims at exploring self-construction and interpersonal relationships of juveniles living in residential communities. It proposes a method of doing so that is coherent with the framework adopted. Repertory grids were used with 59 youths, ages 9 to 20 years, and 176 significant others (father, mother, houseparent, and a teacher or employer); interpersonal distances were measured. Identification with others and an authority, perceived change, self and social acceptance, social negativity, self-fulfillment, commonality, and sociality led to the discrimination of different profiles that indicate different (inter)personal configurations.

In health care teams, lack of collaboration or inability to collaborate undermines the goal of providing holistic and comprehensive geriatric care. This study examines relational communication control used by hospice interdisciplinary team members in their attempt to share information and contribute to decision making in team meetings. Eighty-one hospice team discussions were coded for message control types. Analysis of the data found that the nurse’s role as primary reporter implicitly created a struggle for relational control among other team members within the meeting. This study concluded that a majority of interpersonal communication in the team meetings was aimed at gaining control of the information exchange. Future attention should be given to organizational policy aimed at shaping the structure of team meetings.

This article gives an overview of the nursing ethics arguments on euthanasia in general, and on nurses' involvement in euthanasia in particular, through an argument-based literature review. An in-depth study of these arguments in this literature will enable nurses to engage in the euthanasia debate. We critically appraised 41 publications published between January 1987 and June 2007. Nursing ethics arguments on (nurses' involvement in) euthanasia are guided primarily by the principles of respect for autonomy, nonmaleficence, beneficence and justice. Ethical arguments related to the nursing profession are described. From a care perspective, we discuss arguments that evaluate to what degree euthanasia can be considered positively or negatively as a form of good nursing care. Most arguments in the principle-, profession- and care-orientated approaches to nursing ethics are used both pro and contra euthanasia in general, and nurses' involvement in euthanasia in particular.

All critically ill patients are at risk for hematological complications during their hospitalization. It is essential that critical care nurses understand the hematological system and common complications. The purpose of this article is to briefly review some basic hematologic concepts involving each of the 3 cell lines: the white blood cell, the red blood cell, and platelets. The content focuses on how to assess these cell lines when there is dysfunction. Examples of disease processes involving the increased and decreased production of each cell line as well as destruction processes are discussed from a critical care perspective. The critical care nurse needs to continually incorporate this information into practice as research continues to formulate critical care practice.

In the Journal of Moral Education, Sherblom (2008) examined several empirical and conceptual claims related to gender and morality and re‐envisioned the legacy of Gilligan's ‘care challenge’. He concluded that the moral and scientific legitimacy of the ethic of care has been established. However, his apologetic is flawed in major ways and scholarly integrity demands a rebuttal. This article exposes how Sherblom's analysis misconstrues some of the empirical claims, fails to present relevant data, entails an incomplete reading of Kohlberg's theory, imputes an impact on moral/character education that is unwarranted, disregards some significant problems in the conceptualisation of the ethic of care and draws conclusions that are indefensible. The primary claims of the care perspective have generally been discredited and Sherblom's attempt to advance its legacy fails. The time has come to move beyond these notions of a gendered moral psychology.

Interpersonal communication is sometimes defined quantitatively (i.e., discourse between 2 and 4 interactants), though it may be defined qualitatively: Unlike mass-mediated communication, which is characterized by interactant anonymity and interchangeability, interpersonal communication hinges on the once-occurrence and noninterchangeability—the existential particularity—of interactants and interaction. Accordingly, this article advances a communicological orientation toward interpersonal communication by focusing on those aspects and dimensions of communication that critically depend on the once-occurrence and noninterchangeability of interactants and their interaction. Next, I examine how agency and efficacy in interpersonal communication amount to reckoning with once-occurrence and noninterchangeability. The predominant utilitarian conceptualization of stakeholder management within construction is reviewed. Utilitarianism will be contrasted with an ethics of nurture and care perspective. A credibility gap is identified in the utilitarian approach whereby individual parties in positions of power are unable to maximize net well‐being. The relationship management concept will be outlined and located within the ethics of care, showing how aggregated behaviours can be linked to this approach. Behaviours that management can facilitate will be outlined and links made with the ethics of care concerning stakeholder management. The analysis will conceptually establish how the credibility gap in stakeholder management can be bridged. Research into relationships within PPP/PFI projects will be explored. The relationships between the SPV and the public sector will provide a particular focus. Recommendations for practice and future research will draw the analysis to a conclusion.

This article examines the opportunities and obstacles for vulnerable and victimized children's participation in family law proceedings. With the help of a set of interviews with children, a framework for the analysis of vulnerable and victimized children's participation is outlined with reference to, on the one hand, the childhood studies debates concerning children's participation and, on the other hand, the contemporary debates about children as victims of crime when exposed to men's violence to women in their family. The authors argue that participation can be viewed as central not simply to a rights perspective on children, but also to a care perspective.

Starting with a 1890 essay by Freud, the author goes in search of an interpersonal psychology native to Freud's psychoanalytic method and to in psychoanalysis and the interpersonal method in psychiatry. This derives from the basic interpersonal nature of the human situation in the lives of individuals and social groups. Psychiatry, the healing of the soul, and psychotherapy, therapy of the soul, are examined from the perspective of the communication model, based on the essential interpersonal function of language and the spoken word: persons addressing speeches to themselves and to others in relations, between family members, others in society, and the professionals who serve them. The communicational model is also applied in examining psychiatric disorders and psychiatric diagnoses, as well as psychodynamic formulas, which leads to a reformulation of the psychoanalytic therapy as a process. A plea is entered to define psychoanalysis as an interpersonal discipline, in analogy to Sullivan's interpersonal psychiatry.

This exploratory, qualitative research was conducted to obtain the perspectives of people with significant physical disabilities regarding factors that have facilitated and hindered the development of a positive self-concept, participation in the broader society, and the formation of interpersonal relationships. The sample was high achieving in terms of education and/or career and attributed positive self-perceptions and their success in the broader society and interpersonal relationships to the attitudes and perceptions regarding their abilities, talents, and potential modeled in supportive family relationships. Females reported the influence of significant others most often, and males the effectiveness of their personality characteristics. Societal barriers, both practical and attitudinal, were reported along with the process for developing a positive selfperception despite these.

This study seeks to gain knowledge of how significant others experience the nursing care of women with breast cancer and what their own caring needs might be. The overall research design is a clinical application research within the hermeneutic tradition. Thirty-seven significant others have contributed their own narratives about the care on the basis of open, structured questions. The narratives have been submitted to an analysis of contents. The result shows that significant others have no given place in nursing care. Their place in nursing care is to look on, be present or be ignored. No matter what their place is, the significant others feel a need for caring conversations and information in an ethical manner. The faith and trust of significant others, the sharing, sense of communion and the information about woman's illness strengthen the significant others' own vitality. Their inmost desire concerns the sensitiveness of caregivers with regard to the woman's as well as the significant others' personal needs. If space is allowed and this desire meets with response, significant others can constitute a source of strength in the care of the woman suffering from breast cancer.

and Introduction Abstract Despite the fact that palliative sedation can be considered a gradually accepted form of therapy within palliative care, it still remains a much debated issue in palliative nursing. Moreover, research shows that palliative sedation often involves an emotional burden for care providers and, more specifically, for nurses. This article focuses on clarifying how palliative sedation as a therapy fits into the care perspective of palliative nursing. It describes the way in which decisions can be made about sedation and how the practical procedure may be carried out, and it illustrates the role the nurse can play. It becomes clear that nurses, alongside other care providers, play a crucial role in the process of palliative sedation. By using this step-by-step plan for palliative sedation, however, the decision-making process becomes multidisciplinary and diminishes the emotional burden for nurses. Moreover, it brings about clarity among care providers on when palliative sedation is and is not an issue.
