11. Progress in the application of neurosurgical collaborative robot systems

Abstract

Currently, the common neurosurgical robot systems rely on traditional optical navigation for positioning and movement, which is large and expensive, and its clinical application is limited. In recent years, the neurosurgical robot systems have gradually developed to the direction of collaborative robot systems. Collaborative robot systems can cooperate with the surgeon to improve the efficiency of surgery and ensure more accurate and safe surgery. Compared to supervisory control system and master⁃slave system, collaborative robot systems are simpler to construct and operate, less costly, more closely integrated with human perception, and can be directly integrated into surgical scenarios. This article reviews the structure and working principle of Evolution 1, ROVOT⁃m, RoboticScope, iArmS, Craniostar prototype and a new surgical cooperative robot based on 3D machine vision, which are 6 kinds of surgical robot systems for stabilizing surgical procedures and facilitating observation of surgical field, to promote its clinical application in neurosurgery.

20. Intraosseous epidermal inclusion cyst of the right ring finger for 40 years

Abstract

Introduction. Intraosseous epidermal inclusion cysts (IEpC) are benign bone lesions lined with squamous epithelium. Finger phalanges are the second most common site of predilection after the skull. Case report. We presented a case of a typical IEpC at the distal phalanx of the right ring finger following a remote history of a crush injury to the finger (40 years earlier). The patient experienced painful enlargement and progressive swelling of that finger during the previous month. On physical examination, the finger showed typical “clubbing” with local tenderness. X-ray showed bone destruction, and magnetic resonance imaging (MRI) revealed abnormal signals in bone tissue in the distal phalanx of the right ring finger. The patient underwent distal phalanx amputation of the right ring finger. The diagnosis of IEpC was histopathologically confirmed. At follow-up 2 years later, the stump healed well and without recurrence. Conclusion. IEpC with a history of up to 40 years is very rare, and although the patient presents with a typical “clubbing” finger, the diagnosis was eventually con-firmed by surgery and pathology.

22. Preliminary exploration of intraoperative ultrasound⁃guided endoscopic evacuation of spontaneous intracerebral hematoma

Abstract

Objective To investigate the clinical value of intraoperative ultrasound and endoscopy technique on the surgery of spontaneous intracerebral hematoma. Methods A total of 17 patients who underwent ultrasound⁃guided endoscopic evacuation of spontaneous intracerebral hematoma from October 2016 to October 2019 in Department of Neurosurgery of the First Medical Center of PLA General Hospital and Hainan Hospital were selected. All hematomas were located by ultrasound during operation. At the same time, feedback the degree of hematoma clearance and calculate the hematoma removal rate. Record ultrasound and endoscopy ⁃ related complications or adverse events. Use Karnofsky Performance Status (KPS) to evaluate postoperative neurological function improvement. Results Intraoperative ultrasound and endoscopic technique were conducted in all 17 patients with a success rate of 100%, and no ultrasound and endoscopic⁃related complications or adverse events occurred. In all cases, endoscopic working sheath was once accurately introduced into the center of hematoma. Preoperative median volume of hematoma was 44.40 (31.20, 54.35) ml, and postoperative median volume was 3.00 (1.80, 5.10) ml, and the extent of hematoma removal rate was 94.01% (87.11, 96.08)%. Preoperative median KPS score was 20 (15, 30), and median KPS score was 40 (35, 50) 2 weeks after operation, which was significantly improved (Z = ⁃ 3.671, P = 0.000). Conclusions These results suggest that intraoperative ultrasound⁃guided endoscopic surgery is a safe and feasible surgical modality for spontaneous intracerebral hematoma. This combined technique may help to localize the hematoma more accurately, enhance the extent of hematoma removal, and reduce complications and facilitate neurological recovery. doi：10.3969/j.issn.1672⁃6731.2021.03.014

42. Mediating the Role of Perceived Social Support Between Frailty and Self-Perceived Burden in Elderly Patients with Diabetes.

Abstract

To explore the mediating effects of perceived social support between frailty and self-perceived burden (SPB) in elderly patients with diabetes and to provide a theoretical basis for reducing that burden. Methods: A total of 169 elderly patients with diabetes who were hospitalised in the endocrinology department of a third-class hospital in Wuxi between May 2020 and July 2022 were included in this study using the convenience sampling method. Patients were assessed by the general information questionnaire, the Chinese version of the Tilburg frailty inventory (TFI), the Self-Perceived Burden Scale (SPBS) and the Perceived Social Support Scale (PSSS). The SPSS 22.0 software was used for Pearson's correlation analysis and multiple linear regression analysis. Model four of the SPSS PROCESS was used for mediating the effect analysis. Results: The SPBS of elderly patients with diabetes was positively correlated with the TFI (P < 0.01) and negatively correlated with the PSSS (P < 0.01). The results of the Bootstrap test showed that the mediating effect of the PSSS on the relationship between the TFI and the SPBS in elderly patients with diabetes was 0.296 (95% CI: 0.007, 0.066), and the mesomeric effect accounted for 17.3% of the total effect. Conclusion: The debilitation of elderly patients with diabetes can be reduced by decreasing their SPB through perceived social support.
