Multicellular organisms have an immune system, which is essential for the survival of living beings. Interest in the immune system has been expanded since common characteristics of innate immunity betweenDrosophila melanogaster(Meigen, 1830) and mammals were discovered in the 1980. Since then, immunology has mainly focused on the adaptive immune system that seems to be restricted to vertebrates. Unlike the innate immunity, the adaptive one is acquired after exposure to a specific antigen (Ag) and includes: antigen-presenting cells such as macrophages, proliferation of B and T lymphocytes, Ag-specific antibody/cytokine production and immunological memory. Innate immunity is instead a process of cellular defense at low specificity, which is designed to prevent and combat infectious agents that penetrate at the tissue level, and may be the only form of immunity present in invertebrates such as sea urchins. The immune system of invertebrates acts through (i) cellular components (cell-mediated immunity) in which the effectors of defense reactions are represented by immune cells; (ii) soluble factors (humoral immunity), secreted by the immune cells, such as lectins, agglutinins, lysins, antimicrobial peptides and the prophenoloxidase (proPO) activating system, which act in parallel with the immune cells to fight pathogens and other foreign substances. Here we aim to deepen the study on humoral immunity of invertebrates, especially referring to the phylum Echinodermata because of its features shared with protostomes and other deuterostomes, and suggesting a key step during evolution. The article presents a study on the screening programme for cervical cancer and Human Papillomavirus (HPV) in Valcamonica, Italy. It mentions that cytological smear was stained and interpreted for HPV positive women, colposcopy for positive cytologies, and the HPV negative were referred to repeat the test after one year. Results showed that 1633 are HPV positive, 843 are positive to cytology triage, and 356 are persistently positive.

Background: Primary sclerosing cholangitis (PSC) is an uncommon disorder, rarely diagnosed in children, moreover, data on its natural history and survival are still lacking. Aim: The study was undertaken to compare clinical, laboratory and survival rates in two series of PSC: one in a pediatric group (group A) and the other in an adult population (group B). Methods: Group A included 9 patients (5 males, 4 females, mean age 10 yrs, range 7-15); group B included 28 patients (19 males, 9 females, mean age 32 years, range 19-60). The mean follow-up was 5.2 years in group A and 6.9 years in group B (range 1-14 years). ERCP and colonos-copy were performed in each case. Survival was analyzed using the Kaplan-Meier method. Results: At presentation children showed significantly higher levels of IgG and AST compared to adults ( p<0.05); moreover, interface hepatitis occurred in 50% of children and in 14.2% in adults ( p= ns). During follow-up the following major events occurred: oesophageal bleeding ( n=2) in group A; progressive liver failure ( n=6), cholangiocarcinoma ( n=3), colonic cancer ( n=1) in group B. Liver transplantation (OLTx) was performed in 4 adults (one died after a retransplantation). No deaths were observed in children. The Kaplan-Meier curve in adults shows a 65% rate of survival at 10 years. Conclusions: The present findings on PSC suggest a more severe activity of the disease in children than in adults at presentation; nonetheless, the prognosis seems to be better in children than in adults. The Mayo score prognostic index does not predict the development of liver/colonic cancer. A poor outcome (defined as death or being listed for OLTx) only occurred in adults. [ABSTRACT FROM AUTHOR]

Results of our studies on the reactivity of chagasic and leishmaniasic sera with the purified T. cruzi-specific antigen 163B6, as assessed by ELISA, and with complex antigenic mixtures from T. cruzi and Leishmania mexicana, by immunoblotting, are presented here. Our objective was to identify the antigens responsible for the exhibited cross-reactivity between trypanosomiasis and leishmaniasis, and to find a specific reactivity pattern corresponding to each parasitosis. In spite of the high cross-reactivity observed with the immunoblotting, the use of 7.5% A-B gels made it possible to identify a characteristic pattern for each parasitosis that could be distinguished by the naked eye. The characteristic pattern corresponding to chagasic patients was ascribed to reactivity with T. cruzi hands of mol. wts 131, 125, 116, 111, 51-45 and 43 kD, that were not recognized by leishmaniasic sera. Trypanosoma cruzi antigens of mol. wts 85, 81, 70, 65 60, 37 and 32 kD were considered as crossing antigens, since they were recognized by leishmaniasis sera. With L. mexicana, most of the chagasic patients presented reaction with antigen of mol. wts 124, 107, 92, 59 and 32 kD, while bands of mol. wts 155, 140, 73, 56 and 48 kD were recognized only by leishmaniasic sera. In this study we found 12 out of 45 sera of patients with leishmaniasis from a region endemic for both parasitoses, which exhibited a pattern of bands very similar to those corresponding to chagasic individuals strongly suggesting a mixed infection. This hypothesis was verified by using a purified specific antigen of T. cruzi, Ag163B6, which would be the major cysteine proteinase of this specie (cruzipain). By ELISA, these 12 sera showed a positive reaction with this purified antigen, as those of chagasic patients, thus leading to the confirmation of the presence of a mixed infection. Twenty patients with active chronic hepatitis, classified on the basis of the treatment into three groups, have been studied: (1) patients treated with cyclophosphamide (10 patients); (2) patients treated with cyclophosphamide after cycles of corticosteroid therapy (four patients); (3) patients treated with corticosteroid (six patients). In all patients we observed clinical, biochemical and immunological changes during and after treatment. Hepatic needle-biopsy was performed in all patients before treatment. The histological picture was rechecked after the first cycle of therapy in twelve patients. The results obtained in the three groups of patients have been statistically compared. Cyclophosphamide treatment produces an improvement of general conditions and a persistent normalization of several biochemical and immunological data, especially in those patients in which active chronic hepatitis was not in an advanced stage (type A aggressive form). The sensitivities of three technqiues used to detect serum hepatitis B surface antigen (HBsAg) were compared in 411 patients with various types of chronic liver disease. Counterimmunoelectrophoresis proved an unreliable test. Two haemagglutination technqiues were slightly less sensitive than radioimmunoassay but were more rapidly performed. Less sensitive techniques were particularly unreliable in active liver disease where HBsAg titres were low. HBsAg was detected in patients with chronic persistent hepatitis, alcoholic liver disease, chronic active liver disease with or without cirrhosis, and primary liver cell carcinoma. Forty-six of the 68 (68%) HBsAg positive subjects were males coming from outside the United Kingdom. The HBsAg titres in 13 subjects with chronic persistent hepatitis were significantly higher (P less than 0-001) than those in 43 subjects with chronic active liver disease. Corticosteroid therapy did not alter the HBsAg titre significantly. None of the 28 HBsAg positive subjects studied serially for up to two years cleared HBsAg from the serum. Anti-HBs was examined by passive haemagglutination and found in 35 subjects, 26 of whom had no evidence of liver disease, 80% came from abroad. By affinity chromatography with a monoclonal antibody (163B6), obtained in our laboratory, we have isolated a T. cruzi antigen which could be useful for differential diagnosis of Chagas' disease from leishmaniasis. This antigen, a 52-k D protein, reacted with all sera from Chagas' disease patients tested but not with sera from patients with leishmania, in ELISA. The 52-kD antigen is widely distributed in the Trypanosoma genus since the 163B6 monoclonal antibody reacts with T. rangeli and 8 strains and a clone of T. cruzi epimastigotes.
