The radicular inflammatory cyst develops at the apex of odontogenic roots where the inflammation persists for a long time. Some therapies can help complement treatment, such as the use of fibrin-rich plasma (L-PRF). Sticky bone is a mixture of autologous fibrin glue and bone graft, allowing the material to be modeled. The association of these materials accelerates tissue regeneration. Photodynamic therapy (PDT) presents excellent results for the treatment of bacterial infections and promotes greater postoperative comfort. This work reports a clinical case of a 31-year-old patient, referred for treatment of a cystic lesion. The procedure was performed under local anesthesia. After gaining access, enucleation and curettage of the lesion were performed. The collected material was sent for anatomopathological examination, apicoectomy of teeth 21, 22 and 23 and retrograde filling were performed. PDT was performed and the sticky bone graft associated with L-PRF was placed. After 1 year of follow-up, satisfactory results were observed in the clinical and imaging examination. The use of complementary therapies in the reconstructive treatment of oral lesions can benefit bone healing and photodynamic therapy contributes to the prevention of local infections and a better postoperative period. Background: Temporomandibular joint disorder is a complex disease requiring multimodal treatment and often a delayed diagnosis. Purpose: The primary objective of this study is to evaluate the feasibility of simultaneously recording joint sounds while obtaining 3-tesla cine-magnetic resonance imaging (3T cine-MRI) of the temporomandibular joint (TMJ). A secondary objective pertains to optimization of a dynamic TMJ imaging protocol to maximize image quality and speed while maintaining synchronicity. Methods: Investigators enrolled four subjects (August 2018 to August 2019) eliciting audible joint sounds during continuous open-close jaw movements during clinical evaluation at the New York University College of Dentistry. A contact fiber-optic microphone (OptiRhythm 4130, Optoacoustics, Yehuda Israel) was utilized to record joint sounds elicited with functional movements during MR dynamic imaging at NYU Langone Health and all data was recorded appropriately. Results: Results of this feasibility study demonstrate the successful integration of audio and 3-Tesla MR imaging for the TMJ in all subjects. This protocol was shown to be well-tolerated by the subjects. A precise correlation of the movement of TMJ structures within cine-MRI images and with audible observations was achieved and assessed by an experienced oral and maxillofacial surgeon and an experienced radiologist. Conclusion: This protocol for simultaneous audio/cine-MRI is a viable tool for studying TMJ health and disease. In future studies, we anticipate this protocol will ultimately refine our understanding of TMJ sounds and their relation to anatomy, physiology, and pathology. Background: Condylar hyperplasia is an abnormal growth affecting the temporomandibular joint caused by overactive cells in mandibular condyle. It is relatively rare, and its exact frequency is unknown. Purpose: To describe the clinical, radiographic, and histological characteristics from pathology of two patients with active condylar hyperplasia, whose purpose is to focus on specific surgical treatment of condylar hyperplasia without the need for additional management with orthognathic surgery and its satisfactory evolution. Methods: A systematic review of the literature was carried out through of search in different databases such as LiLACS, PubMed, ScienceDirect and Scopus considering articles published between 2008 and 2023, a period of 15 years. 41 articles corresponding to literature reviews, clinical cases and systematic reviews were selected. Results: surgical management of condylectomy versus combined with orthognathic surgery, it was found that 63,64 % about the articles analyzed were treated only with high condylectomy and subsequent orthodontia, while 36,36 % of these were managed with high condylectomy plus orthognathic surgery in the same surgical act or in a second surgical procedure. Presentation of cases: The presentation of two clinical cases of condylar hyperplasia who report aesthetic and functional changes, treated with condylectomy on the affected side is made. Conclusions: Condylar hyperplasia is a pathological disorder characterized by generating asymmetries and facial deformations, deviations, malocclusions, functional, aesthetic and psychological alterations, condylectomy being the procedure of choice which can be complemented with orthognathic surgery and orthodontics. Background: Tranexamic acid (TXA) is a synthetic analogue of lysine, which inhibits fibrinolysis by reducing the conversion of plasminogen into plasmin, thereby promoting the preservation of the clot formed in the operated area and reducing the risk of bleeding. It is eventually used as a complement in oral surgery. Purpose: To analyze the evidence on the effectiveness in hemostasis, appropriate dose, safety margin, benefits, and postoperative evolution of TXA used in oral surgery. Methods: The systematic review was structured from a search tree (PubMed, Google Scholar, ProQuest, and Scopus), with the keywords and Boolean connector: Tranexamic Acid AND Oral Surgery. The search was limited to the English, Portuguese, and Spanish languages; open-access articles; systematic reviews, meta-analyses, randomized clinical trials, and case reports. We established a secondary selection based on the title and abstract of the articles according to the purpose of the study. Findings: The literature reports an efficacy of 80 % of TXA in reducing bleeding risk during and after surgical procedures, which optimizes the procedures. Conclusions: Topical or intravenous TXA in doses not exceeding 20 mg has proven to be effective to reduce the risk of bleeding in oral surgery. [ABSTRACT FROM AUTHOR]

The lingual nerve is one of the most important nerves in the oral cavity and is often injured during certain oral surgery procedures. Due to its location and possible anatomical variations, some surgical procedures that encompass this region can become complex and may result in intra- and post-operative complications, such as nerve damage. Among the various causes of these injuries are incorrect anaesthetic techniques, trauma to hard and soft tissue, especially of the lingual flap, and implant-supported fixed rehabilitations. The main objective of this study is to help dentists to clarify how to avoid and/or resolve most of these complications. Based on exhaustive bibliographical research, this work focuses on aspects such as the frequency of occurrence and degree of severity of lingual nerve injuries, as well as the most common ways to treat them. The sensorineural changes resulting from complications related to dental treatments are a growing concern of dentists and, therefore, in this study, some examples will be focussed on, namely the extraction of third molars, as one of the procedures most frequently responsible for the high number of lesions of the lingual nerve. Given the above, good pre-surgical planning is essential, and the dentist should collect as much information as possible, given the inherent complications, including sensory changes in the orofacial region, which, in the last case, can irreversibly interfere with speech, chewing and consequently in the patient's social interactions. Sialolithiasis is the most common salivary gland disease. It is characterized by obstruction of the salivary flow rate by calculus in the duct or, in some cases, in the parenchyma. The submandibular gland is the most affected by this disease, about 85% of cases, due to its position, its secretion and size of duct, which is long and contains an anti-gravity path when compared to others. Its etiology is still much discussed in studies and literature, containing many theories around the subject. Treatment will depend on the case, leading to either a conservative or surgical treatment. The object of this study is aimed to make an updated review on the subject and describe a clinical case reporting an atypical case of double sialolithiasis affecting both excretory ducts of the submandibular glands. Therefore, it is possible to conclude that: its etiology is still much discussed in literature; the treatment should be decided by the size, location, quantity and quality of the calculus and the general conditions of the patient. Introduction: The objective of this clinical case report is to describe the presence of bilateral fourth molars. The occurrence of supernumerary teeth (ST) is a relatively unusual dental anomaly. It is even more unusual to find patients with distomolar teeth also denominated fourth molar teeth. Presentation of the case: This article describes a clinical case of a 24-year-old patient presenting with a maxillary fourth molar, who was diagnosed by dental x-ray, and surgical resolution of the case. Introduction: Calcifying Odontogenic Cyst (COC) is an uncommon and painless odontogenic cyst, that causes bone marrow destruction and cortical bone expansion. It was first reported by Gorlin, in 1962. Objective: in this article, we report a case of COC in the rarest region of its appearance, which is the posterior region of the maxilla in a young man who is only 15 years old. Case Report: the patient was admitted with an extraoral swelling that grew slowly during the previous 8 months; in the oral cavity, edema was also observed in the region of the teeth 26, 27 and 28, and also marked mobility; radiographically, there was unilocular radiolucent area with some radiopaque areas (like an orange skin). The histological findings observed include proliferation of phantom cells, and columnar basal cells, similar to Ameloblasts and cells with loose superficial layers. Conclusion: the patient underwent enucleation with cyst curettage as a treatment; due to high rates of recurrence the patient had a follow up every three months for a period of 2 years without signs of relapse. Bisphosphonates are increasingly present in prescribing physicians for patients with bone disorders or even metastases of breast, lung, prostate cancer. This has been happening because they are a class of drugs that interact in the bone remodeling, but the accumulation of these medicines that have a possible high half-life and can stay in the bone for more than years after the interruption of the use of these. The relationship with dentistry is due to the osteonecrosis of the jaw associated with such drugs, which, although much cited in the literature, it is not yet known how this is triggered, but a possible relation with the inhibition of the action of osteoclasts can be mentioned. Decrease of bone resorption and consequent inhibition of natural bone remodeling. In this way, the present work aims at a literature review relating the class of drugs cited to dentistry and its practice, enabling the professional dentist surgeon a greater knowledge in the area, which has been shown to be increasingly necessary, thus showing signs and symptoms present in these cases, risk factors and the importance of information on the subject. O reparo de fístulas oronasais representa um desafio para o cirurgião, devido à freqüente ocorrência de recidivas. Com o intuito de avaliar o emprego de osso bovino esponjoso inorgânico liofilizado em bloco (OBEILB) no tratamento dessa afecção, foi realizado um experimento utilizando dez cães sem raça definida, fêmeas com peso variando entre 8-17kg, submetidos à indução de fístulas oronasais após exodontia dos caninos. O procedimento cirúrgico foi realizado em duas etapas: na fístula do lado esquerdo, foi feito reparo imediato após realização do defeito, e, no lado direito, o reparo foi efetuado após sete dias, ambos preenchidos com OBEILB e recobertos com retalho de mucosa de origem gengival. Os animais foram avaliados clínica e radiograficamente, semanalmente, entre sete e 120 dias. O material usado é eficaz no reparo de fístulas oronasais do alvéolo do dente canino maxilar, não apresentando evidências de rejeição e recidivas.The oronasal fistula repair still represents a challenge to the surgeons, due to the high incidence of recidivation. An experiment using 10 adult mongrel dogs weighting between 8 and 17 kilograms was performed in order to evaluate the in block lyophilized inorganic bovine bone cancellous (BLIBBC) in the treatment of this disease. Each dog had an exodontic procedure of both canine teeth, in order to produce an oronasal fistula. The surgical procedure realized consisted in two different surgical steps: the left side defect was repaired right after the defect have been produced, while the right side defect was repaired seven days after the fistula induction; both sides were repaired using BLIBBC and covered with gengival flap. The animals were radiological and clinicaly evaluated every week for 120 days. The material used was effective in the repair of both fistulas, without rejection or recurrence.

Mesiodens is a disorder of odontogenesis represented by an anomalous supernumerary central incisor, which exhibits a classic higher prevalence in men. The morphology is presented conoid to tuberculated, with unique or multiple distribution in deciduous or permanent dentition and more frequent in upper arch. The etiopathogenesis is variable and influenced by genetics, association with dentigerous cyst and other dental abnormalities. The clinical and radiographic diagnosis displays accuracy and standard treatment adds the surgical and orthodontic steps. The aim of this study was to present a case report of upper mesiodens in pediatric patient with favorable surgical treatment served in Ceara, Brazil and discuss the clinical and epidemiological relevance of this dental abnormality according to a recent literature. O presente trabalho tem como objetivo avaliar, através de uma revisão sistemática da literatura, a necessidade de suspensão dos novos anticoagulantes orais (NOACs) para os pacientes com indicação de exodontias. O estudo realizado baseado nos critérios PRISMA-2009, sendo a estratégia de busca estruturada com o uso dos operadores booleanos, combinando os descritores "Tooth Extraction", "Oral Surgery Procedures" e "Anticoagulants", aplicadas individualmente em bases de dados eletrônicas. A busca foi realizada nas bases de dados Cochrane, Embase, Lilacs, Livivo, PubMed, Scopus e Web of Science, sendo encontrados inicialmente 2.281 estudos. Após a leitura dos títulos e dos resumos, foram excluídos 1.996 trabalhos. Dos 285 restantes, foram excluídos 116 por indisponibilidade, e descartados 69 por estarem duplicados. Foram selecionados 100 artigos para serem lidos na íntegra, dos quais 99 foram descartados pela incompatibilidade com os critérios de inclusão. Dessa forma podemos concluir que a literatura neste campo de estudo seja ampla. Por isso, sugere-se a necessidade de mais estudos clínicos controlados sobre o assunto para avaliar de fato se existe ou não a necessidade de suspender o fármaco em questão. E, por não haver evidências científicas suficientes até o momento para a suspensão ou não, os NOACs devem ser suspensos em situações em que se espera maior sangramento. [ABSTRACT FROM AUTHOR]

The cleft palate is an abnormal communication between the oral and nasal cavities which might affect different regions of the palate, what make its severity and clinical signs variable. The main signs are difficulties with breast-feeding, rhinitis, recurrent sneezing and in severe cases of pneumonia by aspiration. There are a lot of surgical procedures to repair the cleft palate.
