This paper describes the application of a physically based agrohydrological model (named FLOWS), coupled with a kinematic wave approach model (named KWV) for water and solute runoff routing, for interpreting the fate of water and nutrients coming from cultivated fields to surface drainage network located in 'Piana del Sassu' in the Arborea plain, a hydraulically reclaimed area with shallow groundwater. Modelling was supported by a large complex database on soil, groundwater and surface drainage water, which was used for establishing the boundary conditions for simulations, as well as for calibrating and validating the model. The model FLOWS provided the water and nutrient fluxes to the surface water, which were passed to the KWV model for their routing along the elementary fields in the experimental area and from these to the ditches and finally to the drainage channel. The modelling approach effectively predicted the water and solute distribution along the soil profile, as well as the losses of water and nutrients to the surface water. The results showed a significant amount of water and dissolved nutrients to flow quickly from the soil uppermost layer to the surface drainage network during both the irrigation season and during rainfall events. During irrigation applications, losses were mostly due to rainfall intensity exceeding the maximum infiltration velocity of the shallow soil layer in the case of sprinkler irrigation and to subsurface lateral drainage in the case of exceeding irrigation water provided by drip irrigation. This makes the Sassu plain a significant contributor of nutrients (nitrate and phosphorus) to the surface water. Consequently, even though the agricultural activities might not be an important issue for the groundwater vulnerability, the management of water and nutrients should be significantly improved to avoid ecohydrological threats to the important coastal water bodies present in the area. This research aims at studying the intrinsic vulnerability of groundwater to diffuse environmental pollutants in the Muravera coastal agricultural area of Sardinia, Italy. The area faces contamination risks arising from agricultural practices, especially the use of fertilizers, pesticides, and various chemicals that can seep into the groundwater. The study examined the interplay among hydrological elements, including soil characteristics, groundwater depth, climate conditions, land use, and aquifer properties. To do that, the outcomes of FLOWS 1D physically-based agrohydrological model were analyzed in parallel with those of the overlay-and-index model SINTACS, in a sort of reciprocal benchmarking. By using FLOWS, water movement and solute transport in the unsaturated zone were simulated by, respectively, solving the Richard Equation (RE) and the Advection-Dispersion equation (ADE). As such, this model allowed to account for the role of soil hydraulic and hydro-dispersive properties variability in determining the travel times of a conservative solute through the soil profile to the groundwater. For FLOWS simulations, a complete dataset was used as input, including soil horizons, soil physical and hydraulic properties of 36 soil profiles, average annual depth to groundwater table at each soil profile (ranging from 1 to 50 meters), and climatic temporal series data on rainfall and evapotranspiration. Detailed analyses of travel times for the movement of 25, 50, 75, and 100% of the solute mass to reach groundwater were conducted, revealing that the depth to groundwater predominantly influences vulnerability. Background/Objectives: A compartmental pharmacokinetics (PK) analysis of new extended half-life FVIII concentrates has never been performed in a large cohort of hemophilia patients. An improved PK analysis of individual outcomes may help to tailor hemophilia replacement treatment. Methods: PK outcomes after the infusion of a standard single dose of Efmoroctocog alfa were collected from 173 patients with severe/moderately severe hemophilia A in 11 Italian hemophilia centers. Factor VIII clotting activity (FVIII:C) was measured by one-stage clotting assay (OSA) in all patients, and chromogenic substrate assay (CSA) in a subgroup (n = 52). Fifty patients underwent a comparative PK assessment with standard half-life (SHL) recombinant FVIII (rFVIII) products. Non-compartmental analysis (NCA), one compartment model (OCM), and TCM were used to analyze the decay curves of all patients, and one-way paired ANOVA to compare the PK outcomes. Results: All 173 PKs conformed to the NCA and OCM, but only 106 (61%) conformed to the TCM based on the biphasic features of their decay curves. According to the TCM, the Beta HL and MRT of rFVIIIFc were 20.42 ± 7.73 and 25.64 ± 7.61 h, respectively. ANOVA analysis of the outcomes from the three PK models showed significant differences in clearance, half-life (HL), and mean residence time (MRT) (p < 0.001 for all parameters). As anticipated, the HL and MRT of rFVIIIFc were longer than those of SHL rFVIII. Comparing OSA with CSA outcomes, Cmax resulted higher when measured by CSA (p = 0.05) and, according to TCM, Beta HL resulted longer when measured by OSA (p = 0.03). FVIII:C trough levels obtained with SHL concentrates were significantly lower than those obtained with rFVIIIFc at each post-infusion time point. Conclusions: In a large group of hemophilia A (HA) patients, three different PK models confirmed the improved pharmacokinetic (PK) characteristics of rFVIIIFc, compared with standard half-life rFVIII concentrates. The TCM only fits two-thirds of the PKs, highlighting their biphasic decay and a long Beta half-life. In these patients, the TCM would be preferable to properly evaluate individual PK features. Background: Treatment options for people with haemophilia are evolving at a rapid pace and a range of prophylactic treatment options using various technologies are currently available, each with their own distinct safety and efficacy profile. Treatment goals: The access to replacement therapy and prophylaxis has driven a dramatic reduction in mortality and resultant increase in life expectancy. Beyond this, the abolition of bleeds and preservation of joint health represent the expected, but rarely attained, goals of haemophilia treatment and care. These outcomes also do not address the complexity of health‐related quality of life impacted by haemophilia and its treatment. Conclusion: Capitalizing on the major potential of therapeutic innovations, 'Normalization' of haemostasis, as a concept, should include the aspiration of enabling individuals to live as normal a life as possible, free from haemophilia‐imposed limitations. To achieve this—being supported by the data reviewed in this manuscript—the concept of haemostatic and life Normalization needs to be explored and debated within the wider multidisciplinary teams and haemophilia community. Background: Human papillomavirus (HPV) infection is linked to several cancers, including anal and oral cancers. The incidence of anal cancer is particularly high among HIV-positive men who have sex with men (MSM). DNA methylation markers have shown promise as biomarkers for identifying precancerous lesions and cancer in HPV-infected individuals. The aim of this study was to investigate the correlation of DNA methylation with HPV infection in oral samples and the correlation of DNA methylation with lesion degree in the anal samples of HIV-positive MSM. Methods: This study investigated DNA methylation in oral and anal samples from HIV-positive MSM at the National Institute for Infectious Diseases (INMI) in Rome, Italy. Exfoliated oral epithelial cells and anal samples were collected and analyzed for 28 HPV genotypes using the Allplex 28 HPV assay. DNA methylation was assessed with the PrecursorM+ kit for oral samples and the AnoGyn kit for anal samples, focusing on the promoter regions of specific genes. Results: The study included 63 participants, with a median age of 49 and a median CD4+ count of 705 cells/µL. The oral samples showed HPV16 as the most common type, with 22% testing positive for DNA methylation. The anal samples exhibited HPV-related methylation changes linked to cytological lesions, with a 30% increase in the observed ddCt ratio. Significant differences were found in both ASCL1 and ZNF582 genes, particularly for HSILvsNILM and HSILvsLSIL lesions. Of the samples with an increased ddCt ratio, 80% were from patients over 35 years old, and multiple HPV infections were common. Conclusions: DNA methylation markers could be valuable in identifying high-risk HPV infections in oral samples and detecting potential precancerous lesions in anal samples. These markers may enhance the early detection and prevention strategies for HPV-related cancers in high-risk populations, with follow-up data indicating potential for monitoring lesion progression. The article discusses a study conducted in Italy that evaluated the use of recombinant factor IX-albumin fusion protein (rIX-FP) as a treatment for adult patients with hemophilia B. The study found that rIX-FP prophylaxis was effective in reducing bleeding episodes and required fewer infusions compared to previous treatments. The most common dosing regimen was every 14-15 days, with one patient using a 21-day regimen. Objectives: To evaluate pattern of use and clinical outcomes in pediatric/adolescent patients enrolled in the IDEAL study. Methods: This post‐hoc analysis of IDEAL retrospective‐prospective observational study focused on patients <18 years, 100% on prophylaxis during the entire observation period. Results: Thirteen subjects (median age 10.0 years; 61.5% ≤ 11 years) were analyzed. The infusion frequency changed from 2/week in 84.6% (N = 11) of patients with previous rFIX, to less than 1/weekly in 76.9% (N = 9) with rIX‐FP and the annualized number of infusions reduced of 57% (p =.002), from a mean ± SD of 95.1 ± 22.77 to 40.4 ± 6.79, respectively. Annualized mean consumption decreased of about 56% (p =.001), from 3748.4 ± 1155.40 IU/kg with previous rFIX, to 1656.8 ± 456.63 IU/kg of rIX‐FP. Mean FIX trough level changed from 3.0% ± 1.98% to 10.92% ± 3.6%. Low mean Annualized Bleeding Rate was maintained across all prophylaxis regimens (0.8 ± 1.69 vs. 0.3 ± 0.89) and zero bleeding patients moved from 69.2% (N = 9) with previous rFIX to 84.6% (N = 11) with rIX‐FP (p =.63). Two adverse events, none related to rIX‐FP, occurred in two patients. No inhibitors development was reported. Conclusions: The results in this pediatric/adolescent subgroup support rIX‐FP prophylaxis may reduce infusion frequency, while providing high FIX trough levels, stable annualized bleeding rate and a good safety profile. Soil water content (θ), matric potential (h) and hydraulic conductivity (K) are key parameters for hydrological and environmental processes. Several sensors have been developed for measuring soil θ–h–K relationships. The cost of such commercially available sensors may vary over several orders of magnitude. In recent years, some sensors have been designed in the framework of Internet of Things (i.e., IoT) systems to make remote real-time soil data acquisition more straightforward, enabling low-cost field-scale monitoring at high spatio-temporal scales. In this paper, we introduce a new multi-parameter sensor designed for the simultaneous estimation of θ and h at different soil depths and, due to the sensor's specific layout, the soil hydraulic conductivity function via the instantaneous profile method (IPM). Our findings indicate that a second-order polynomial function is the most suitable model (R2 = 0.99) for capturing the behavior of the capacitive-based sensor in estimating θ in the examined soil, which has a silty-loam texture. The effectiveness of low-cost capacitive sensors, coupled with the IPM method, was confirmed as a viable alternative to time domain reflectometry (TDR) probes. Notably, the layout of the sensor makes the IPM method less labor-intensive to implement. The proposed monitoring system consistently demonstrated robust performance throughout extended periods of data acquisition and is highly suitable for ongoing monitoring of soil water status. Background: Extended half-life (EHL) factor IX (FIX) concentrates allow for prophylaxis with prolonged dosing intervals and high bleeding protection in persons with hemophilia B. Long-term real-world studies are lacking. Methods: In a retrospective–prospective study, the six-year use of prophylaxis with the EHL recombinant FIX–albumin fusion protein (rIX-FP) was analyzed, comparing outcomes with previous standard half-life (SHL) FIX in patients already on prophylaxis. Results: Prophylaxis with rIX-FP was prescribed in 15 patients (10 severe, 5 moderate; follow-up: 57 ± 17 months). Based on a pharmacokinetic assessment and clinical needs, the first regimen was 47 ± 7 IU/Kg every 9 ± 2 days. All but one patient remained on rIX-FP prophylaxis, adjusting infusion frequency and/or dose; the last prescribed frequency was ≥10 days in 10/13 patients, being reduced in seven and increased in four vs. the first regimen. The weekly FIX dose was unchanged; FIX trough levels were >5% in all patients. The annual infusion number and FIX IU/Kg significantly decreased (~60%) in eight patients previously on SHL FIX prophylaxis, with similar concentrate costs. Very low bleeding rates (most traumatic bleeds and the last quartile of the infusion interval), improved orthopedic and pain scores, unchanged HEAD-US scores and problem joints, and high treatment adherence (>90%) and satisfaction were registered. Conclusions: Personalized, carefully adjusted rIX-FP regimens contribute to the diffusion and optimization of prophylaxis in persons with severe and moderate hemophilia B, with long-term favorable bleeding, joint, and patient-reported outcomes. Biomass gasification is a thermochemical process that produces a gaseous end product known as syngas, the main components of which are CO, CO2, and H2. Starting from syngas it is possible to upgrade this mixture into Synthetic Natural Gas (SNG) through a catalytic methanation process. In this work, a thermodynamic study was conducted to see how operative parameters such as the temperature of the methanation reactor, the quality of the syngas (related to different gasification technologies), and the presence of a conditioning stage based on water gas shift reaction, can influence the output variables as the conversion degree of the reactants, the mass yield and the low heating value of the final products. Introduction: Considering the advances in haemophilia management and treatment observed in the last decades, a new set of value‐based outcome indicators is needed to assess the quality of care and the impact of these medical innovations. Aim: The Value‐Based Healthcare in Haemophilia project aimed to define a set of clinical outcome indicators (COIs) and patient‐reported outcome indicators (PROIs) to assess quality of care in haemophilia in high‐income countries with a value‐based approach to inform and guide the decision‐making process. Methods: A Value‐based healthcare approach based on the available literature, current guidelines and the involvement of a multidisciplinary group of experts was applied to generate a set of indicators to assess the quality of care of haemophilia. Results: A final list of three COIs and five PROIs was created and validated. The identified COIs focus on two domains: musculoskeletal health and function, and safety. The identified PROIs cover five domains: bleeding frequency, pain, mobility and physical activities, Health‐Related Quality of Life and satisfaction. Finally, two composite outcomes, one based on COIs, and one based on PROIs, were proposed as synthetic outcome indicators of quality of care. Conclusion: The presented standard set of health outcome indicators provides the basis for harmonised longitudinal and cross‐sectional monitoring and comparison. The implementation of this value‐based approach would enable a more robust assessment of quality of care in haemophilia, within a framework of continuous treatment improvements with potential added value for patients. Backgroud: A huge amount of data about psychosocial issues of people with haemophilia (PwH) are available; however, these materials are fragmentary and largely outdated, failing to reflect the impact of current treatment strategies. Aim: Describing the influence of illness on psychosocial aspects of adult PwH (≥18 years) and caregivers of children with haemophilia (CPwH) without inhibitors, in Italy. Methods: Surveys (for adult PwH, CPwH and haemophilia specialists) were developed by a multidisciplinary working group and conducted from November 2019 to June 2020. Results: A total of 120 PwH without inhibitors and 79 CPwH completed the survey. Adult patients reported a significant impairment in many psychosocial aspects, including working activities, relations with family members and social relations. Caregivers generally reported better scores in all aspects of the survey. Mobility, Pain and Mental health domains of EQ‐5D were the most frequently impaired in both patients and caregivers, reducing the perceived quality of life. Genetic counselling was an important issue, 53% of CPwH declaring unawareness of their carrier status, as well as the psychological support offered by the reference center, 67.0% of respondents reporting that no psychological support was provided at the time of diagnosis communication. Conclusion: This study provides information about PwH's and CPwH's point of view in the current scenario of continuous innovations in haemophilia treatment and management furthermore, updated insights on psychosocial problems faced by patients and caregivers are reported. Fermentation biomasses can be defined as a complex mixture of different natural components and microbes, having biodegradable and organic waste as the primary source. Its correct characterization is crucial to have proper processing in fermentative units. Firstly, proximate analysis is done to retrieve the content of specific compounds in the mixture, such as fat, proteins, and carbohydrates. However, this is often not enough to achieve the sufficient precision, since some low-concentration species are not easily found through this methodology (i.e., sulfate compounds, ethanol, caproic acid). Consequently, ultimate analysis is performed to evaluate the exact amount of every element in the mixture. For biomass-based compounds, atoms content can be synthesized in carbon, hydrogen, oxygen, nitrogen, and sulfur. The total content of these elements is also known as CHONS. From this, it is possible to derive the exact amount of the relative species in the biomass. However, the experimental procedure for its determination is rather time and budget-consuming. On the other hand, the amount of data collected in the literature, from both experimental and industrial analysis, can be exploited to build a numerical model, based on the multivariate statistical analysis and machine learning principles that predict the CHONS content for every type of biomass. In this work, a data-driven model has been developed to achieve this aim, having as input a set of relevant variables. Consequently, a dataset has been built to gather all these data. The multivariate statistical technique of Canonical Correlation Analysis (CCA) is used to find 'hidden' correlations and predict CHON content for 27 different biomass types. The use of zeolites in agriculture as a soil conditioner is becoming an important field of research in crop growth. To study the effect of synthetic zeolites and deficit irrigation on sweet pepper (Capsicum annuum L.) cultivation, an experiment was conducted in a controlled environment. In particular, sweet peppers were cultivated in a glasshouse using polypropylene pots filled with sandy loam soil, to which 2% zeolite was added. The zeolite employed in the experiments was obtained using coal fly ash as a raw material. The experiment consisted of two main treatments: (a) soil with a zeolite at 2% (Z) and (b) soil without a zeolite as a control (C). Three subplot treatments consisted of (1) full irrigation at 100% of the available water content (AWC) (100); (2) deficit irrigation at 70% of the AWC (70); and (3) deficit irrigation at 50% of the AWC (50). Sweet pepper cultivation started on 24 April 2023 and lasted until 23 June 2023; during the trial, the environmental data, such as the soil humidity, air temperature, and relative humidity, and some crop parameters, such as the plant height, leaf number, and the SPAD index, were monitored. At the end of the trial, the fresh and dry plant weights, the dry matter content, and the leaf water potential were measured. The results showed that, for the plant fresh weight and dry matter content, no significant differences were observed in the treatments and their interactions, whereas, for the other parameters, the statistical analysis showed significant differences. The study suggests that the soil's structural benefits, resulting from zeolite application, are not followed by an equal positive effect in terms of sweet pepper growth under deficit irrigation conditions. In this work, ASPENplus was used to simulate biogas upgrading by sorption-enhanced methanation in a dual interconnected bubbling fluidized bed configuration using inexpensive, abundant, and eco-friendly CaO to remove H2O from the reaction environment. The chemical looping scheme consisted of two reactors: a methanator/hydrator, where the catalytic reactions occurred on a catalyst with 20% Ni supported on alumina as well as the steam removal by CaO, and a regenerator, where the Ca(OH)2 was dehydrated back to CaO. The simulations were carried out to identify possible reactant compositions (H2 and biogas), CaO amount, and the methanation temperature able to produce an outlet gas matching the specifications for direct grid injection. When considering a stoichiometric gas feed ratio at the methanator inlet, the unwanted CaO carbonation worsened the process performance, subtracting CO2 from the desired methanation reaction. However, optimal conditions were found with hydrogen-lean gas feedings, balancing the limited H2 amount with the capture of CO2 due to the sorbent carbonation. Thermodynamic considerations pointed out the possibility of solid carbon formation induced by sorption-enhanced methanation conditions, especially for H2 sub-stoichiometric feedings.
