The discharge of untreated or partially treated wastewater can have detrimental impacts on the quality of water bodies, posing a significant threat to public health and the environment. In Ecuador, previous research indicates a high prevalence of antimicrobial resistant (AMR) bacteria in surface waters affected by human activities, including irrigation channels. In this study, we analyzed sediment samples collected from an irrigation channel utilized for agricultural purposes in northern Ecuador, using microbiological techniques and whole-genome sequencing (WGS). Our investigation revealed the first documented occurrence of E. kobei in Ecuador and the initial report of environmental E. kobei ST2070. Furthermore, we identified the coexistence of OXA-10-type class D β-lactamase and KPC-2-type class A β-lactamase in the E. kobei isolate (UTA41), representing the first report of such a phenomenon in this species. Additionally, we detected various antibiotic resistance genes in the E. kobei UTA41 isolate, including blaCTX-M-12, fosA, aac(6′)-lb, sul2, msr(E), and mph(A), as well as virulence genes such as bacterial efflux pump and siderophore biosynthesis genes. We also identified two intact prophage regions (Entero_186 and Klebsi_phiKO2) in the isolate. Our study presents the first evidence of E. kobei isolate containing two carbapenemase-encoding genes in environmental samples from Latin America. This finding indicates the potential spread of critical-priority bacteria in water samples originating from anthropogenic sources, such as urban wastewater discharges and livestock facilities.

IntroductionSystemic lupus erythematosus (SLE) is a clinically heterogeneous disease that presents a challenge for clinicians. To identify potential biomarkers for diagnosis and disease activity in SLE, we investigated a selected yet broad panel of cytokines and autoantibodies in patients with SLE, healthy controls (HC), and patients with other autoimmune diseases (AIDs).MethodsSerum samples from 422 SLE patients, 546 HC, and 1223 other AIDs were analysed within the frame of the European PRECISESADS project (NTC02890121). Cytokine levels were determined using Luminex panels, and autoantibodies using different immunoassays.ResultsOf the 83 cytokines analysed, 29 differed significantly between patients with SLE and HC. Specifically, CCL8, CXCL13, and IL-1RA levels were elevated in patients with active, but not inactive, SLE versus HC, as well as in patients with SLE versus other AIDs. The levels of these cytokines also correlated with SLE Disease Activity Index 2000 (SLEDAI-2K) scores, among five other cytokines. Overall, the occurrence of autoantibodies was similar across SLEDAI-2K organ domains, and the correlations between autoantibodies and activity in different organ domains were weak.DiscussionOur findings suggest that, upon validation, CCL8, CXCL13, and IL-1RA could serve as promising serum biomarkers of activity in SLE.

El virus de chikunguña es un arbovirus de la familia Togaviridae y fue aislado por primera vez en Tanzania y Uganda, en 1953. Esta infección se ha desarrollado de manera endémica y es transmitida por el mosquito del género Aedes. El mecanismo exacto de la transmisión de madre a hijo aún no se tiene claro, pero se han identificado potenciales complicaciones de la transmisión vertical, como malformaciones congénitas, mortinatos, restricción del crecimiento intrauterino y partos pretérmino. En este artículo se presentan dos casos clínicos de neonatos que presentaron arritmias cardiacas en sus primeros días de vida, hijos de madres con síndrome febril en curso por chikunguña. Se diagnosticó miocarditis por chikunguña y transmisión vertical, confirmado por reacción de cadena de polimerasa. Ambos pacientes recibieron manejo médico, incluidas inmunoglobulinas, con mejoría del cuadro clínico, sin desenlaces fatales.

Introducción: comparar la eficacia de verapamilo y losartán para controlar cifras tensionales y la función renal. Materiales y métodos: se incluyeron 109 pacientes entre hombres y mujeres, y se comparó el periodo en que fueron manejados con verapamilo (160 mg/día para el grupo A) y cuando se realizó la prescripción de losartán (100 mg/ día en el grupo B) para mejorar el control de cifras tensionales. La edad media fue de 67 ± 11 años. Resultados: las variables fueron las presiones arteriales sistólicas (PAS), diastólica (PAD) y la función renal. La PAS inicial fue de 135,8 ± 19,2 en el grupo A, y de 102,7 ± 12,7 en el grupo B. La PAD fue de 85 ± 9,3 en el grupo A y de 71 ± 8,57 en el grupo B. La PAD fue de 67 mm Hg (grupo A) y de 60 mm Hg (grupo B). Las disminuciones de PAS, PAD y PAM fueron mayores en el grupo B que en el grupo A. Resultados similares se obtuvieron en la función renal con un clearance de creatinina, de 59,04 mL/min (grupo B) y de 54,75 mL/min (grupo A). Conclusión: el uso de verapamilo y losartán son considerablemente diferentes en cuanto a la eficacia para reducir PAD, PAS y PAM. El losartán tiene mayor eficacia sobre la presión diastólica y la función renal.

La información es representada como patrones de actividad neuronal o pulsos, lo que crea una diferencia significante entre las redes neuronales pulsantes y las redes neuronales clásicas. Una característica diferente de las redes neuronales pulsantes es que la información es codificada en patrones de actividad neuronal y estas se comunican usando trenes de pulsos en lugar de valores individuales. Además, este tipo de redes neuronales pulsantes trabajan con una gran cantidad de neuronas donde se requiere grandes recursos computacionales para ser simuladas. En el presente trabajo se realiza un análisis teórico de las redes neuronales pulsantes, para el caso de dos neuronas acopladas. Se logra obtener teóricamente las condiciones para acoplamiento excitatorio e inhibitorio en las neuronas.

Calcinosis cutis (CC) is defined as the deposition of calcium salts on the skin and subcutaneous tissue. It is associated with different conditions, including some autoimmune diseases, and it can generate significant inflammation, pain, and functional impairment. Different therapies have been tried with limited results. Intralesional sodium thiosulfate seems a promising therapeutic option. We report a patient with diffuse systemic sclerosis who presented with two symmetrical plaques on both axillae, which caused pain and skin retraction. The clinical diagnosis was consistent with CC, which was confirmed by skin biopsy and ultrasound. The patient was treated with a 250 mg/ml solution of sodium thiosulfate injected into the plaques. Complete resolution was achieved after three monthly sessions. The only reported adverse effect was a transient burning sensation during the injections. Given its effectiveness and safety, we believe that intralesional sodium thiosulfate could become a valid first-line option for the treatment of CC. [ABSTRACT FROM AUTHOR]
