The mixture cure rate model (MCM) is the most widely used model for the analysis of survival data with a cured subgroup. In this context, the most common strategy to model the cure probability is to assume a generalized linear model with a known link function, such as the logit link function. However, the logit model can only capture simple effects of covariates on the cure probability. In this article, we propose a new MCM where the cure probability is modeled using a decision tree-based classifier and the survival distribution of the uncured is modeled using an accelerated failure time structure. To estimate the model parameters, we develop an expectation maximization algorithm. Our simulation study shows that the proposed model performs better in capturing nonlinear classification boundaries when compared to the logit-based MCM and the spline-based MCM. This results in more accurate and precise estimates of the cured probabilities, which in-turn results in improved predictive accuracy of cure. We further show that capturing nonlinear classification boundary also improves the estimation results corresponding to the survival distribution of the uncured subjects. Background: Artemether–lumefantrine (AL) has been the primary anti-malarial drug used to treat uncomplicated Plasmodium falciparum malaria in Ethiopia since 2004. However, there have been recent reports of AL resistance mutations in different African countries, including Ethiopia. This is concerning and requires periodic monitoring of anti-malarial drug resistance. Therefore, the current study aimed to evaluate the therapeutic efficacy of AL in treating uncomplicated P. falciparum malaria in the Arba Minch Zuria District, Gamo Zone, Southwest Ethiopia. Methods: A single-arm prospective study with a 28-day follow-up period was conducted from July to October 2022. Capillary blood samples were collected for RDT and microscopic examination. The study enrolled monoinfected P. falciparum patients aged ≥ 18 years at Ganta Sira Health Post. Sociodemographic and clinical data were recorded, and a dried blood spot (DBS) was prepared for each participant. Nested polymerase chain reaction (nPCR) genotyping of the msp-1 and msp-2 genes was only performed for recurrent cases to distinguish between recurrence and reinfection. Data entry and analysis were performed using the WHO Excel spreadsheet and SPSS version 26. Results: A total of 89 patients were enrolled, and 67 adequately completed the 28-day follow-up period. AL showed a 100% clearance rate for fever on day 2 and asexual parasites on day 3. Gametocytes were detected in 13.5% (12/89) of the participants. The gametocyte clearance rate was 58.3% (7/12) until day 7 and 100% (12/12) until day 14. Five participants developed recurrent malaria, three of whom experienced relapse and two of whom experienced reinfection. Based on the Kaplan–Meier survival analysis, the PCR-uncorrected and PCR-corrected cumulative incidence of success were 93.7% (95% CI 85.5–97.3) and 96.2% (95% CI 85.5–98.7), respectively. Conclusion: AL was efficacious in treating uncomplicated P. falciparum malaria in the study area. The use of herbal products for the treatment of malaria, has increased globally. However, inadequate scientific studies about the safety and effectiveness of such herbal products have been raised. Also, the reduced sensitivity of the malaria parasites to artemisinin-based combination therapies is of concern. There is therefore the need for new antimalarial medications including those from alternative sources such as herbal medicinal products. In this study, a prospective, comparative parallel group randomized, clinical study was done to assess the safety and effectiveness of Mist Amen Fevermix and Mist Edhec Malacure with Artemether/Lumefantrine as control at the Tafo Government Hospital, Kumasi between July and November 2019, after Committee on Human Research, Publication and Ethics approval (CHRPE/AP/424/19). The study was conducted in accordance with Good Clinical and Laboratory Practice (GCLP) and registered with the Pan African Clinical Trials Registry with trial number PACTR202109664146698. Participant completed an informed consent form. Randomization was based on a single sequence to allocate participants to a group. SPSS version 19. One-way ANOVA test and exploratory statistics was used for data analysis. Total sample size was 150 participants with 50 on each arm of the group. Male and female patients aged 15–45 years and meet inclusion criteria with clinically established malaria were treated with Mist Amen Fevermix and Mist Edhec Malacure, at the specified doses of 45 mls (0.1063 g) and 30 mls (0.0521 g) three times daily after meals for three days. Artemether/Lumefantrine was administered at a dose of 80/480 mg/kg twice daily after meals for three days. Baseline data was taken on day 0. Patients were then followed up on Day 3, 7 and 28 to establish treatment outcomes and any side effect using a checklist for signs and symptoms and Karnofsky's scale to assess the quality of life. Mist Amen Fevermix was effective with a cure rate of 95.89%. Mist Edhec Malacure was also effective with a cure rate of 91.87%. The cure rate of Artemether/Lumefantrine was 97.25%. Kidney and liver panels were within normal reference range at the end of the 28-day study. This study supports the use of Mist Amen Fevermix and Mist Edhec Malacure, two multi-component products as safe and effective for the treatment of uncomplicated malaria. This paper establishes a new fractional frSIS model utilizing a continuous time random walk method. There are two main innovations in this paper. On the one hand, the model is analyzed from a mathematical perspective. First, unlike the classic SIS infectious disease model, this model presents the infection rate and cure rate in a fractional order. Then, we proved the basic regeneration number R0 of the model and studied the influence of orders a and b on R0. Second, we found that frSIS has a disease-free equilibrium point E0 and an endemic equilibrium point E∗. Moreover, we proved frSIS global stability of the model using R0. If R0<1, the model of E0 is globally asymptotically stable. If R0>1, the model of E∗ is globally asymptotically stable. On the other hand, from the perspective of infectious diseases, we discovered that appropriately increasing a and decreasing b are beneficial for controlling the spread of diseases and ultimately leading to their disappearance. Background and Aims: Soil‐transmitted helminths are one of the most prevalent causes of both intellectual and physical disability in the world. Albendazole (ALB) is a drug recommended for mass treatment of the high burden of soil‐transmitted helminths in schoolchildren, particularly in developing countries. However, some researchers have reported that the efficacy of albedazole against soil‐transmitted helminths is inconsistent. Monitoring the programs is crucial to evaluating the effectiveness of 400 mg of ALB against soil‐transmitted helminths, as well as any changes in its therapeutic efficacy. Thus, the purpose of this study was to evaluate ALB effectiveness in treating soil‐transmitted helminthes in Salgy Primary School Children. Methods: An uncontrolled experimental study was conducted at Salgy Primary School Children, Northwest Ethiopia, from March to May 2020. A total of 439 schoolchildren were enrolled and screened for soil‐transmitted helminths by stratified proportionate systematic random sampling to get 228 positive schoolchildren. Students in grades one through eight were grouped based on their educational attainment. Using the Kato‐Katz thick smear technique, the selected stool sample collected from school children was examined using the Kato‐Katz thick smear technique to determine the cure and egg reduction rates. The statistical package for social science software, version 20, was used to analyze the data. To determine the relationship between CR (cure rate) and ERR (egg reduction rate) by age, a chi‐square test (X2) was employed and significance was considered at A 95% confidence interval and p Value (p < 0.05). Results: A 400 mg single dosage of ALB showed a 99.35% CR and a 97.30% egg reduction rate against Ascaris lumibricoides. Additionally, a 400 mg dose of ALB showed a 95.75% CR and an 82.07% egg reduction rate, suggesting questionable effectiveness against hookworm infections. Trichuris trichiura showed a decreased efficacy, with a 43.53% CR and a 23.12% egg reduction rate. Conclusion: A single dose of 400 mg ALB is effective (satisfactory), doubtful, and unsatisfactory against Ascaris lumbricoides, hookworm, and T. trichiura infections, respectively. Further studies using different brands, doses, and routes will be needed to treat hookworm and T. trichiura infections successfully by using a larger sample size. [ABSTRACT FROM AUTHOR]

(1) Background: Among Korean research papers there have been studies on the correlation between tuberculosis-hypertension and diabetes and the correlation between dementia-hypertension and diabetes, but there were no analysis data specifically on tuberculosis and dementia. (2) Methods: A total of 2992 tuberculosis patients in the Gyeongbuk region were analyzed through a final analysis of integrated disease and health management system data collected from 2021 to 2022. In this selection, patients with tuberculosis under 50 years of age and 368 people diagnosed with tuberculosis were excluded. (3) Results: From 2021 to 2022, among the 2992 tuberculosis patients in Gyeongsangbuk-do aged 50 or older, 2722 (91.0%) belonged to the general tuberculosis patient group, while 270 (9.0%) belonged to the dementia–tuberculosis patient group. The average age in the dementia–tuberculosis group was 81.4 years, significantly higher than the general group's average of 75.7 years. Within the dementia–tuberculosis patient group, 235 patients (87.0%) had underlying medical conditions in addition to dementia and tuberculosis. The tuberculosis treatment cure rate was 56.3% (1477 patients) in the general group and 38.9% (105 patients) in the dementia–tuberculosis patient group. (4) Conclusions: The cure rate was notably higher in the general group. Similarly, the mortality rate (deaths due to tuberculosis) was significantly higher in the dementia–tuberculosis patient group (7.0%, 19 patients) compared to the normal group (3.0%, 81 patients). Abstract Background Preventive chemotherapy with a single dose of praziquantel given to an all-at-risk population through mass drug administration is the cornerstone intervention to control and eliminate schistosomiasis as a public health problem. This intervention mainly targets school age children, and pre-school age children (pre-SAC) are excluded from receiving preventive chemotherapy, partly due to scarcity of data on praziquantel treatment outcomes. Methods We conducted active efficacy and safety surveillance of praziquantel treatment among 240 Schistosoma mansoni-infected pre-SAC who received a single dose of praziquantel (40 mg/kg) in southern Ethiopia. The study outcomes were egg reduction rates (ERR) and cure rates (CRs) four weeks after treatment using the Kato–Katz technique and treatment-associated adverse events (AEs) that occurred within 8 days post-treatment. Results The overall ERR was 93.3% (WHO reference threshold ≥ 90%), while the CR was 85.2% (95% CI = 80.0–89.5%). Baseline S. mansoni infection intensity was significantly associated with CRs, 100% among light infected than moderate (83.4%) or heavy (29.4%) infected children. An increase of 100 in baseline S. mansoni egg count per gram of stool resulted in a 26% (95% CI: 17%, 34%) reduction in the odds of cure. The incidence of experiencing at least one type of AE was 23.1% (95% CI: 18.0%, 29.0%). Stomachache, diarrhea, and nausea were the most common AEs. AEs were mild-to-moderate grade and transient. Background: The present epidemic of dermatophytosis in India is marked by an increase in chronic, recurrent and disseminated cases. A combination of oral itraconazole and topical luliconazole is being increasingly utilised by dermatologists in India. The superiority of this combination is not supported by robust clinical trial data. Objective: We conducted this randomised, open-label, two arms, parallel assignment intervention trial between November 2022 and May 2023 to determine the superiority of topical 1% Luliconazole over bland emollient as adjuvant to systemic Itraconazole therapy in the management of dermatophytosis. Method: In this study, 135 patients of either sex were randomised to two study cohorts. Major exclusions being concomitant medical illness, use of concomitant medication and substance abuse. Participants were randomly assigned to receive topical bland emollient, (Cohort I, n = 67) or topical luliconazole, (Cohort II, n = 68). Both cohorts received oral itraconazole 200 mg/day (100 mg BID) and levocetirizine 5 mg twice a day as a systemic regime. Clinical and mycological cure at the end of 6 weeks and clinical relapse among cure patients during 10-week follow-up were observed. Results: The cure rates for Cohorts I and II at 6 weeks were 50 (74.62%) and 56 (82.35%), (p = .46), respectively. During the 4-week follow-up period, clinical relapses were observed in 16 (32%) of the 50 patients in Cohort I and 12 (21.43%) of the 56 patients in Cohort II (p = .18). Luliconazole cohort shows a significantly higher medical cost (p < .05). Conclusion: Our study shows a similar cure rate and relapse rate for patients receiving topical Luliconazole versus topical bland emollient as an adjuvant to the systemic itraconazole regime. The objective was to examine the pooled effects of antibiotic–probiotic combinations by examining the cure rate and recurrence rate for bacterial vaginosis (BV). A systematic literature search was conducted from electronic databases. All parallel randomized controlled trials (RCTs) that focused on the effects of antibiotics combined with intravaginal probiotics were included. Cure rate and recurrence rate were the primary and secondary outcomes to be analyzed. Meta-analysis was conducted following the Cochrane handbook for Systematic Reviews of Interventions. As a result, of 923 studies identified, 11 articles involving 1,493 BV patients met the inclusion criteria and nine were available for meta-analysis. A meta-analysis of two studies evaluated the recurrence rate 12–16 weeks after treatment. Results showed a statistically significant difference favoring the antibiotics plus probiotics group vs the antibiotics plus placebo group (relative risk 0.62, 95% confidence interval [CI]: 0.45–0.85). The narrative review in one study indicated that the cure rate was higher in the antibiotics plus probiotics group, giving a significant HR ratio of 0.73 (95% CI 0.54–0.98) (p = 0.042). In conclusion, vaginal application of Lactobacillus in combination with antibiotics for the treatment of BV could be a promising method for both reducing the recurrence rate and relieving symptoms of BV.

Long-term data on disease control rates after surgery are limited. Our aim was to estimate short- and long-term rates of biochemical control after pituitary surgery in acromegaly and identify predictive factors. Methods: Patients operated for GH-secreting pituitary adenomas between 2005–2020 were included from the local pituitary registry (n = 178). Disease activity and treatment data were recorded at one-year (short-term) and five-year (long-term) postoperative follow-up. Biochemical control was defined as insulin-like growth factor 1 (IGF-1) ≤ 1.2 × upper limit of normal value. Multivariate regression models were used to identify factors potentially predicting biochemical control. Results: A total of 178 patients with acromegaly (median age at diagnosis 49 (IQR: 38–59) years, 46% women) were operated for a pituitary adenoma. Biochemical control was achieved by surgery in 53% at short-term and 41% at long-term follow-up, without additional treatment for acromegaly. Biochemical control rates by surgery were of same magnitude in paired samples (45% vs. 41%, p = 0.213) for short- and long-term follow-up, respectively. At short-term, 62% of patients with microadenomas and 51% with macroadenomas, achieved biochemical control. At long-term, the biochemical control rate was 58% for microadenomas and 37% for macroadenomas (p = 0.058). With adjunctive treatment, 82% achieved biochemical control at long-term. Baseline IGF-1 levels significantly predicted biochemical control by surgery at short-term (OR: 0.98 (95% CI: 0.96–0.99), p = 0.011), but not at long-term (OR: 0.76 (95% CI: 0.57–1.00), p = 0.053). Conclusion: In unselected patients with acromegaly, the long-term biochemical control rate remains modest. In classical survival analysis, it is assumed that all the individuals will experience the event of interest. However, if there is a proportion of subjects who will never experience the event, then a standard survival approach is not appropriate, and cure models should be considered instead. This paper deals with the problem of adapting a machine learning approach for classical survival analysis to a situation when cure (i.e., not suffering the event) is a possibility. Specifically, a brief review of cure models and recent machine learning methodologies is presented, and an adaptation of machine learning approaches to account for cured individuals is introduced. In order to validate the proposed methods, we present an extensive simulation study in which we compare the performance of the adapted machine learning algorithms with existing cure models. The results show the good behavior of the semiparametric or the nonparametric approaches, depending on the simulated scenario. The practical utility of the methodology is showcased through two real-world dataset illustrations. In the first one, the results show the gain of using the nonparametric mixture cure model approach. Purpose: This study aimed to describe the distribution of TB AFB+ incidence in Surabaya by mapping and to analyze the relationship between population density, TB AFB+ cure rates, and death rates during TB AFB+ treatment with AFB + TB incidence in 31 sub-districts of Surabaya City from 2018 to 2020. Methods: This was a descriptive study with secondary data processing obtained from the Surabaya Health Profile Book 2018-2020 using the Pearson Correlation Product Moment statistical test. Results: There was a correlation between population density and the incidence of TB AFB+ in 2018 (0.61), 2019 (0.65), and 2020 (0.62). Then there was a correlation between TB AFB+ cure rate and TB AFB+ incidence in 2018 (0.98), 2019 (0.96), and 2020 (0.91). There was a correlation between the mortality rate during TB AFB+ treatment and the incidence of TB AFB+ in 2018 (0.31), 2019 (0.71), and 2020 (0.88). An advantage of the standard mixture cure model over an usual survival model is how it accounts for the population heterogeneity. It allows a joint estimation for the distribution related to the susceptible and non‐susceptible subjects. The estimation algorithm may provide ±∞$$ \pm \infty $$ coefficients when the likelihood cannot be maximized. This phenomenon is known as Monotone Likelihood (ML), common in survival and logistic regressions. The ML tends to appear in situations with small sample size, many censored times, many binary or unbalanced covariates. Particularly, it occurs when all uncensored cases correspond to one level of a binary covariate. The existing frequentist solution is an adaptation of the Firth correction, originally proposed to reduce bias of maximum likelihood estimates. It prevents ±∞$$ \pm \infty $$ estimates by penalizing the likelihood, with the penalty interpreted as the Bayesian Jeffreys prior. In this paper, the penalized likelihood of the standard mixture cure model is considered with different penalties (Bayesian priors). A Monte Carlo simulation study indicates good inference results, especially for balanced data sets. Finally, a real application involving a melanoma data illustrates the approach. Introduction: Vonoprazan, a novel potassium-competitive acid blocker, has a strong acid suppression effect and potent efficacy in acid-associated diseases, including Helicobacter pylori eradication. We performed a systematic review and meta-analysis to investigate the efficacy and safety of vonoprazan/amoxicillin dual therapy for H. pylori eradication. Methods: We conducted a systematic literature search through PubMed, Web of Science, EMBASE, and the Cochrane Library up to June 2022, to identify randomized controlled trials and cohort studies comparing vonoprazan/amoxicillin dual therapy and triple therapies for H. pylori eradication. Primary outcomes were cure rates and relative efficacy. Secondary outcomes included adverse events, dropout rate, and subgroup analysis. Results: Five studies with 1,852 patients were included in the analysis. The cure rates of vonoprazan/amoxicillin dual therapy were 85.6% with 95% confidence interval (CI) of 79.7–91.5% and 88.5% (95% CI: 83.2–93.8%) in the intention-to-treat and per-protocol analyses. The efficacy of vonoprazan/amoxicillin dual therapy was not inferior to that of triple therapy with pooled risk ratio (RR) of 1.03 (95% CI: 0.97–1.10) and 1.02 (95% CI: 0.98–1.08) in intention-to-treat and per-protocol analyses; while it was significantly superior to the omeprazole or lansoprazole-based triple therapy (RR = 1.15, 95% CI: 1.05–1.25, p = 0.001). For clarithromycin-resistant strains, vonoprazan/amoxicillin dual therapy showed superiority to vonoprazan-based triple therapy (86.7% vs. 71.4%, RR = 1.20, 95% CI: 1.03–1.39, p = 0.02); however, vonoprazan/amoxicillin dual therapy was significant inferior to vonoprazan-based triple therapy for clarithromycin-sensitive strains (83.0% vs. 92.8%, RR = 0.90, 95% CI: 0.85–0.95, p = 0.0002). The adverse effects of vonoprazan/amoxicillin dual therapy were lower than those of triple therapy (21.2% vs. 26.5%, RR = 0.86, 95% CI: 0.73–1.01, p = 0.06), especially the incidence of diarrhea (p = 0.01). Conclusions: The efficacy of vonoprazan/amoxicillin dual therapy is noninferior to vonoprazan-based triple therapy but superior to the omeprazole or lansoprazole-based triple therapy and has less side effects. In this article, we propose a failure rate based step‐stress accelerated life testing (SSALT) model assuming that the time‐to‐event distribution belongs to a fairly general family of distributions and the underlying population consists of long term survivors. With increase in stress levels, it is expected that the mean time to the event of interest gets shortened leading to an order restriction among the mean times‐to‐event. We propose here a method of obtaining order restricted maximum likelihood estimators (MLEs) based on expectation maximization (EM) algorithm coupled with the method of generalized isotonic regression technique. Additionally, we address the testing of hypothesis problem for the presence of long term survivors in the underlying population based on both asymptotic and non‐asymptotic approaches. Cognitive decision has the basic characteristics of risk avoidance and benefit seeking. To explore the neural response process of cognitive decision making, we asked 32 undergraduates to make a decision on whether to accept a specific treatment option with a certain cure rate and a certain risk rate while recording their electrical brain responses. The results showed that more participants chose the treatment option with a high cure rate and moderate or low risk. Compared with low and high risk, medium risk produced greater N1 and smaller P300. Low risk produced larger LPP than the moderate risk in the left hemisphere. The right prefrontal region appeared to have a smaller LPP for low risk than for high risk. The results suggest that individuals prioritize risk when making cognitive decisions. In addition, in medium-risk conditions, solution integration is more difficult. The effect of benefit size appears at the late stage of cognitive decision making and adjusts the effect of risk. These results support the satisfaction principle of decision making. Background. Since the first outbreak of COVID-19, most hospitals restricted patients' family support accompaniment during medical treatment of infectious transmission. On the other hand, accompaniment has also been recognized as an essential part of the treatment. Therefore, this study aims to determine the effect of family presence accompanying COVID-19 patients during hospitalization on the recovery rate and survival time. Objective: this study was conducted in a private hospital designated as a referral hospital for COVID-19 cases in Surabaya, East Java province, Indonesia. Materials and Methods. There were 541 COVID-19 patients included in the study, consisting of 251 women and 290 men. The requirements set as a sample are patients treated between January 1st 2021 and March 31st 2021. This study used a survival analysis study design. The data used is secondary data and uses total sampling. Results. The result of this study is that patients who get support from their families can survive longer than patients who do not get family support. Among the 251 female samples, only 34 were accompanied by their families, with 29.411% fatalities recorded. Among 290 male patients, 25.71% fatalities were recorded in 35 accompanying family presence. Furthermore, female patients have a probability of better outcomes than males (P<0.001). Conclusions. Based on these results, family presence has the benefit of improving outcomes and recovery. So, the authorities are expected to reconsider the restrictions on family presence by maintaining proper safety protocols of isolation and quarantine. [ABSTRACT FROM AUTHOR]

Introduction: A devastating complication after total hip arthroplasty (THA) is chronic periprosthetic joint infection (PJI). Most frequently spacers (Sp) with or without antibiotics are implanted in a two-stage procedure even though not always indicated due to unknown pathogen, femoral and acetabular defects or muscular insufficiency. Materials and methods: A retrospective analysis of a prospectively collected database was conducted, analyzing the treatment of 44 consecutive cases with chronic PJI undergoing two-stage revision using a Girdlestone situation (GS) in the interim period between 01/2015 and 12/2018. Diagnostics included intraoperative microbiological cultures, histological analysis, sonication of the initial implant, analysis of hip aspiration, as well as laboratory diagnostics and blood cultures. We analyzed the general and age-group-specific success rate of treatment using GS. Furthermore, we compared our data with the current literature on spacer implantation regarding common complications. Results: In total, 21 female and 23 male patients at a mean age of 59.3 ± 9.6 years were included. Age groups were divided into young, mid-age, and elderly. In most patients, microbiology revealed Staphylococcus epidermidis in 39.1% of cases, following Staphylococcus lugdunensis and Staphylococcus aureus in 10.9% after THA explantation. For histology, Krenn and Morawietz type 2 (infectious type) was diagnosed in 40.9%, type 3 (infectious and abrade-induced type) in 25.0%. With GS, the total cure rate was 84.1% compared to 90.1% (range 61–100%) using Sp as described in the literature. Among age-groups, cure rate varied between 77.8 and 100%. Other complications, which only occurred in the mid-age and elderly group, included the necessity of transfusion in 31.1%, and in total, one periprosthetic fracture was identified (2.3%). Conclusion: GS shows an acceptable cure rate at a minimum of 2 years when compared to the cure rate reported in the literature for Sp without major complications. For patients with increased risks for treatment failure using spacer, GS seems to be an alternative for chronic PJI when looking at the success rate of treatment. Objective: To analyse the safety and effectiveness of platelet-rich plasma (PRP) in anal fistula patients. Methods: Online databases including PubMed, Embase, Cochrane Library, and Web of Science were searched from inception to December 5, 2022, for eligible studies about evaluating the efficacy of platelet-rich plasma (PRP) in treating anal fistula. Literature search, screening, data extraction, and quality assessment were carried out by two independent investigators. The overall cure rate, the complete cure rate, the recurrence rate, and the adverse event rate with their 95% confidence intervals (95% CI) were the primary calculation indexes. Subgroup analyses were conducted primarily according to whether PRP was combined with other treatments. Softwares of MedCalc 18.2 and Review Manager 5.3 were used for meta-analysis. Results: A total of 14 studies with 514 patients were included in the meta-analysis. The overall cure rate of 14 studies was 72.11% (95% CI 0.64–0.79). The cure rate of PRP alone was 62.39% (95% CI 0.55–0.69). The combined cure rate of PRP with other treatments was 83.12% (95% CI 0.77–0.88). The cure rate of interventions involving PRP were superior to the cure rate of surgery methods without using PRP significantly in the 4 randomized controlled studies (RR = 1.30, 95% CI 1.10–1.54, p = 0.002). The complete cure rate of the 8 studies was 66.37% (95% CI 0.52–0.79). The recurrence rate of the 12 studies was 14.84% (95% CI 0.08–0.24). The adverse event rate of the 12 studies was 6.31% (95% CI 0.02–0.12). Background: The World Health Organization recommends efficacy and safety surveillance of anti-helminths used in mass drug administration campaigns. We evaluated the effectiveness of single-dose praziquantel against Schistosoma mansoni infection, and the safety of praziquantel plus albendazole preventive chemotherapy (PC) in Schistosoma mansoni infected school children (n = 512) in Southern Ethiopia. Method: Stool examinations were done using thick smear Kato-Katz at baseline, week-4, and week-8 of post-Mass drug administration (MDA) to assess praziquantel efficacy. Participants were followed for MDA-associated adverse events up to day 7 of post-MDA. The primary and secondary study outcomes were praziquantel efficacy (parasitological cure and egg reduction rates) and MDA-associated adverse events (AEs), respectively. Result: The overall cure rates at week-4 and week-8 were 89.1% (95%CI = 86.1-91.7) and 87.5% (95%CI = 83.6-90.8), respectively. Cure rates among moderate-to-heavily infected children were significantly lower (p = 0.001) compared to those with light infection at week-4 (84.4% vs. 91.1%, p = 0.03) and week-8 (78.6% vs. 91.9%, respectively). Older children had a higher cure rate than younger ones at week-8 (90.1% vs. 79.5%, p = 0.01). Among those who were Schistosoma egg-free (cured) at week 4, 7.8% became egg-positive at week 8. The overall egg reduction rate (ERR) at week-4 and week-8 were 93.5% and 91.3%, respectively, being lower among the 5-9 years old age groups (p = 0.01) at week-8. The proportion of children who remained schistosoma egg-positive throughout the study follow-up period was 4.6%, and their ERR at week-4 and week-8 was 50% and 51%, respectively, which is below the 90% World Health Organization threshold for efficacy. The incidence of experiencing at least one type of MDA-associated AEs were 17.0% (95%CI = 13.8%-20.5%); abdominal pain, headache, and vomiting were the most common. The proportion of mild, moderate, and severe AEs was 63.2%, 26.3%, and 10.5%, respectively. Females experienced more AEs than males (p = 0.03). Conclusion: Single-dose praziquantel is still effective for the treatment of intestinal schistosomiasis. Praziquantel and albendazole preventive chemotherapy is safe and tolerable, and associated AEs are mostly mild-to-moderate and transient. However, the reduced PZQ effectiveness in moderate-to-heavy infection and observed AEs in about one-fifth of infected children underscores the need for better treatment strategies and surveillance for early detection of parasite resistance and management of AEs. Study objective: To explore the effect of pretreatment with ureteroscopic triage for iatrogenic ureterovaginal fistula (UVF) resulting from radical hysterectomy. Design: A retrospective cohort study. Setting: Department of gynecology at a tertiary medical center. Patients: Women diagnosed with UVF secondary to radical hysterectomy at our center between April 2008 to June 2018. Interventions: The patients were divided into two groups according to whether pretreatment with ureteroscopic triage was performed. Those in the non-triage group underwent retrograde placement of a double-J stent immediately following diagnosis as the first-line therapy. Patients in the triage group were first evaluated under ureteroscopy, their ureteral injuries were then classified into different grades and then underwent different treatments as the first-line therapy, including stent placement or reconstruction surgeries. The cure rate of the first-line therapy and the timeliness of the implementation of adjuvant radiotherapy were subsequently analyzed. Measurements and main results: Ninety-eight UVF patients were included. The demographics, ECOG status, stage of cervical cancer (FIGO 2009), types and onset time of symptoms were not different between the two groups. There were 54 patients in the nontriage group, with an overall first-line cure rate of 70.4% and a timely implementation rate of adjuvant radiotherapy of 38.5%. Abstract Background Severe acute malnutrition (SAM) is one of the leading causes of morbidity and mortality among children below five years with sub-Saharan Africa being the most affected. In Kabale district, SAM affects 2.9% children under the age of five. Uganda government introduced Outpatient therapeutic care (OTC) programme in all health centre level III and IV of Kabale. However, there was limited information about the cure rate and its associated factors among children under the programme hence the cause for the study. Methods A retrospective cross-sectional study was carried out on records of children 6–59 months (n = 637), presenting with SAM on OTC programme in the health centres of Kabale between 2013 and 2015. Data on cure rate (outcome) and other independent factors were collected, cleaned in excel and then exported into STATA 12 for analysis. Univariate, bivariate and logistic regression analysis was run to generate frequencies and factors associated. Results The cure rate was 36.3% (n = 231 cases) with a median recovery time of 21 days. The default rate was 58.6% (n = 373 cases) while the non-response and death rate were 0.6% (n = 4) and 1.1% (n = 7) respectively. Source at admission (Adjusted Odds Ratio [AOR] = 0.1, 95% CI 0.0, 0.7, p = 0.012), Weight at admission (AOR = 0.5, 95% CI 0.0, 0.9, p = 0.014) and Number of visits to the program (AOR = 14.9, 95% CI 9.3, 24.2, p = 0.040) were positively associated with cure rate of SAM children on OTC programme in Kabale. Conclusion Overall the cure and default rate for children on OTC programme in Kabale were significantly higher than national and international standards making the findings quite alarming. However, the weight of the child at admission, the number of visits to the programme to receive services and the source where the child was coming from were very important determinants of cure rate. To improve the cure rates of SAM children in Kabale, there is need for policy makers and programme implementers to think about a community based management of severe acute malnutrition program approach.

Plain Language Summary Our meta-analysis found that probiotics may play an active role in adjuvant treatment of bacterial vaginosis by conventional antibiotic therapy. It was emphasized that oral administration of L. rhamnose was more effective than vaginal application of L. rhamnose in the treatment of bacterial vaginosis. The therapeutic effect of probiotics varies with the administration route and dosage of probiotics.

The objective was to examine the pooled effects of antibiotic–probiotic combinations by examining the cure rate and recurrence rate for bacterial vaginosis (BV). A systematic literature search was conducted from electronic databases. All parallel randomized controlled trials (RCTs) that focused on the effects of antibiotics combined with intravaginal probiotics were included. Cure rate and recurrence rate were the primary and secondary outcomes to be analyzed. Meta-analysis was conducted following the Cochrane handbook for Systematic Reviews of Interventions. As a result, of 923 studies identified, 11 articles involving 1,493 BV patients met the inclusion criteria and nine were available for meta-analysis. A meta-analysis of two studies evaluated the recurrence rate 12–16 weeks after treatment. Results showed a statistically significant difference favoring the antibiotics plus probiotics group vs the antibiotics plus placebo group (relative risk 0.62, 95% confidence interval [CI]: 0.45–0.85). The narrative review in one study indicated that the cure rate was higher in the antibiotics plus probiotics group, giving a significant HR ratio of 0.73 (95% CI 0.54–0.98) (p = 0.042). In conclusion, vaginal application of Lactobacillus in combination with antibiotics for the treatment of BV could be a promising method for both reducing the recurrence rate and relieving symptoms of BV. Objectives: Ultrasound (US)-guided thermal ablation for primary hyperparathyroidism (PHPT) is a relatively novel minimally invasive treatment. The recurrence rate after ablation is between 10 and 15%. The characteristics of patients who can benefit from thermal ablation therapy are not clear yet. The aim of this research was to investigate the validity of a parathyroid hormone (PTH)–based classifier for stratifying patients with PHPT. Methods: A total of 171 patients were screened, 148 (86.5%) of whom were eligible and were divided into development (n = 104) and external validation (n = 44) cohorts. The potential relationship between the PTH-based classifier and the cure rate of patients was initially assessed in the primary cohort and then validated in the external validation cohort. The nomogram was computed from the logistic regression model. Results: A cut-off of PTH < 269.1 pg/mL or ≥ 269.1 pg/mL as the optimal prognostic threshold in the training cohort was generated to stratify the patients into low-risk and high-risk groups. Patients with PTH levels < 269.1 pg/mL in the training cohort had a higher cure rate than patients with PTH levels ≥ 269.1 pg/mL (p < 0.001). The PTH level remained the strongest predictor of the cure rate in all cohorts. Furthermore, a nomogram based on the PTH level was developed to predict the cure rate in the training cohort and it performed well in the external validation cohort (AUC: 0.816, 95%CI 0.703 to 0.930; AUC: 0.816, 95%CI 0.677 to 0.956). Conclusions: The PTH-based classifier may help with individualised treatment planning for selecting patients who may benefit from thermal ablation. Hepatitis B is a disease that damages the liver, and its control has become a public health problem that needs to be solved urgently. In this paper, we investigate analytically and numerically the dynamics of a new stochastic HBV infection model with antiviral therapies and immune response represented by CTL cells. Through using the theory of stochastic differential equations, constructing appropriate Lyapunov functions and applying Itô's formula, we prove that the disease-free equilibrium of the stochastic HBV model is stochastically asymptotically stable in the large, which reveals that the HBV infection will be eradicated with probability one. Moreover, the asymptotic behavior of globally positive solution of the stochastic model near the endemic equilibrium of the corresponding deterministic HBV model is studied. By using the Milstein's method, we provide the numerical simulations to support the analysis results, which shows that sufficiently small noise will not change the dynamic behavior, while large noise can induce the disappearance of the infection. In addition, the effect of inhibiting virus production is more significant than that of blocking new infection to some extent, and the combination of two treatment methods may be the better way to reduce HBV infection and the concentration of free virus.

In classical survival analysis, it is assumed that all the individuals will experience the event of interest. However, if there is a proportion of subjects who will never experience the event, then a standard survival approach is not appropriate, and cure models should be considered instead. This paper deals with the problem of adapting a machine learning approach for classical survival analysis to a situation when cure (i.e., not suffering the event) is a possibility. Specifically, a brief review of cure models and recent machine learning methodologies is presented, and an adaptation of machine learning approaches to account for cured individuals is introduced. In order to validate the proposed methods, we present an extensive simulation study in which we compare the performance of the adapted machine learning algorithms with existing cure models. The results show the good behavior of the semiparametric or the nonparametric approaches, depending on the simulated scenario. The practical utility of the methodology is showcased through two real-world dataset illustrations. In the first one, the results show the gain of using the nonparametric mixture cure model approach. In the second example, the results show the poor performance of some machine learning methods for small sample sizes.

The first hitting time of a boundary or threshold by the sample path of a stochastic process is the central concept of threshold regression models for survival data analysis. Regression functions for the process and threshold parameters in these models are multivariate combinations of explanatory variates. The stochastic process under investigation may be a univariate stochastic process or a multivariate stochastic process. The stochastic processes of interest to us in this report are those that possess stationary independent increments (i.e., Lévy processes) as well as the Esscher property. The Esscher transform is a transformation of probability density functions that has applications in actuarial science, financial engineering, and other fields. Lévy processes with this property are often encountered in practical applications. Frequently, these applications also involve a ‘cure rate’ fraction because some individuals are susceptible to failure and others not. Cure rates may arise endogenously from the model alone or exogenously from mixing of distinct statistical populations in the data set. We show, using both theoretical analysis and case demonstrations, that model estimates derived from typical survival data may not be able to distinguish between individuals in the cure rate fraction who are not susceptible to failure and those who may be susceptible to failure but escape the fate by chance. The ambiguity is aggravated by right censoring of survival times and by minor misspecifications of the model. Slightly incorrect specifications for regression functions or for the stochastic process can lead to problems with model identification and estimation. In this situation, additional guidance for estimating the fraction of non-susceptibles must come from subject matter expertise or from data types other than survival times, censored or otherwise. The identifiability issue is confronted directly in threshold regression but is also present when applying other kinds of models commonly used for survival data analysis. Other methods, however, usually do not provide a framework for recognizing or dealing with the issue and so the issue is often unintentionally ignored. The theoretical foundations of this work are set out, which presents new and somewhat surprising results for the first hitting time distributions of Lévy processes that have the Esscher property.

Introduction/Objective. The aim of our paper was to investigate the clinical efficacy of using a phentolamine alcohol wet dressing to treat the extravasation of an intravenously administered infusion of milk fat. This study was designed as a randomized trial, and was done at the Hengshui people’s Hospital, Hebei Province, China, from June 2019 to June 2020. Methods. In total, 300 patients were randomly divided into two groups. In the experimental group, the patients were treated using a phentolamine alcohol wet dressing, whereas in the control group, the patients were treated using a hydropathic compress with a 50% magnesium sulphate solution. The cure rate, healing time, and patient satisfaction of the two groups were compared and analyzed. Results. The cure rate of intravenous infusion extravasation was 92.67% (139/150) in the experimental group and 70.67% (106/150) in the control group (p < 0.05). In the experimental group, there were 66 patients whose cure time was less than 24 hours, 62 patients whose cure time was between 24 and 48 hours, and 22 patients whose cure time was over 48 hours. The cure time of the patients was significantly shorter in the experimental group than the control group. After treatment, in the experimental group, 67 patients were very satisfied, 52 patients were satisfied, 21 patients were generally satisfied, and 10 patients were dissatisfied; in the control group, 32 patients were very satisfied, 40 patients were satisfied, 56 patients were generally satisfied, and 22 patients were dissatisfied. The satisfaction of patients was significantly higher in the experimental group than in the control group. Conclusion. The effect of using a phentolamine alcohol wet dressing to treat the extravasation of an intravenous infusion of milk fat is significantly better than the effect of using a magnesium sulphate solution, and this type of dressing is worthy of clinical application.

Abstract In this paper, we propose an optimal control problem for an HIV infection model with cellular and humoral immune responses, logistic growth of uninfected cells, cell-to-cell spread, saturated infection, and cure rate. The model describes the interaction between uninfected cells, infected cells, free viruses, and cellular and humoral immune responses. We use two control functions in our model to show the effectiveness of drug therapy on inhibiting virus production and preventing new infections. We apply Pontryagin maximum principle to study these two control functions. Next, we simulate the role of optimal therapy in the control of the infection by numerical simulations and AMPL software.

Abstract Background Severe acute malnutrition (SAM) is a major underlying cause of mortality among children. Around one third of the world’s acutely malnourished children live in India. The WHO recommends community-based management of acute malnutrition (CMAM) for managing children with SAM. In India, different states are implementing community-based SAM treatment programme, hereinafter called CSAM, using varieties of locally produced nutrient dense food items with different nutrient compositions. The study will assess the effectiveness of these state specific CSAM interventions. Methods The longitudinal quasi-experimental study will be undertaken in two purposively selected blocks of one district each in the four intervention states and one comparison state. Objectives: To prospectively investigate the efficacy and safety of ultrasound (US)-guided microwave ablation (MWA) and radiofrequency ablation (RFA) for primary hyperparathyroidism (PHPT). Methods: We performed a prospective multicenter study of MWA and RFA for PHPT between August 2017 and October 2020 at five centers. Laboratory testing was performed pre- and post-ablation and followed for at least 6 months. The primary outcome was the cure rate. Secondary outcomes were complications and dynamic changes in serum levels of PTH, calcium, phosphorus, and ALP after ablation. Results: A total of 132 participants (mean age, 57.33 ± 13.90 years), with 141 parathyroid nodules (median maximal diameter, 1.55 cm) undergoing either MWA or RFA, were enrolled in the study. The technique success rate was 99.29% (140/141). The follow-up period was 6–36 months (median, 12 months). The cure rate was 80.30% (106/132). Pre-ablation PTH level was the independent factor associated with cure rate (Odds ratio (OR), 0.22; 95% CI, 0.07–0.69; p = 0.0090). There was no difference in cure rate between the MWA group and the RFA group (80.22% vs. 80.49%, p = 0.971). The only main complication was hoarseness (5.30%). Conclusions: US-guided MWA and RFA for PHPT is an effective and safe procedure in the treatment of PHPT. Pre-ablation PTH level is the key factor affecting the cure rate after MWA and RFA. With the aggravation of many social psychological and psychological stress factors, the high incidence rate of depression and depression has become a major problem which puzzles people's health and even endangers life. Non drug therapy has become an effective alternative to drug therapy, which is in line with the new trend of natural medicine in the world. This paper will use the real world research method (RWS) to conduct a clinical trial of pop light music in the treatment of depression. Based on the fuzzy algorithm, a comprehensive evaluation system for the treatment of depression was established. By comparing and analyzing the main efficacy indexes between music group and traditional medicine group, we found that the cure rate, clinical control rate, significant efficiency, effective rate and ineffective rate of music therapy group were significantly better than those of drug group. Through the analysis of seven factors of HAMD (Hamilton Depression Scale) scale, we found that pop light music can improve the sleep status and physical symptoms of patients, and the improvement degree of music therapy is significantly better than that of drug therapy. The experiment was to explore the effect of prenatal feeding of compound Chinese herbal medicine preparation on the abortion rate and lambing rate of Hu sheep. A total of 2 100 ewes in late pregnancy were selected and randomly divided into two groups. Test group consisted of 1 100 ewes with 11 replicates, 100 sheep in each replicate. Control group was consisted of 1 000 ewes with 10 replicates and 100 sheep in each replicate. The sheep in control group were fed with basic feed, and sheep in test group were fed with test feed supplemented with 5 mL/head compound Chinese herbal medicine preparation for 10 d. The results showed that compared the control group, the abortion rate in test group was significantly reduced ( P<0.05). Compared with control group, the weak lambing rate and paralyzed sheep rate in test group were significantly reduced ( P<0.05). Compared with control group, the lamb mortality of test group was significantly reduced ( P<0.05), and the cure rate was significantly increased ( P<0.05). Compared with control group, the single tire weight, twin weight and triple tire weight were significantly increased ( P<0.05). The study indicates that the addition of 5 mL/head compound Chinese herbal medicine preparation to feed can improve the number of abortions, weak sheep and paralyzed sheep of pregnant ewes, and increase the lambing rate and the cure rate of lambs. In this paper we propose and study an epidemic model with nonlinear incidence rate, describing some factors effect (protection, exposure, immigration, social distancing, vaccination) on the spread of certain diseases on the community like the novel coronavirus COVID-19. The dynamical behavior of the proposed model is examined. We investigate the existence and stability of the disease-free equilibrium and the endemic equilibrium. Many antibiotics can be used for uUTI treatment. Currently, no data concerning uUTIs and cost-effectiveness have been reported in Saudi Arabia. This study aimed to investigate antibiotic and cost-effectiveness of beta-lactams, fluoroquinolones, and nitrofurantoin as first-line uUTI treatment.Patients and Methods: This study was a retrospective cohort based on a five-arm comparative outcome analysis. A cost-effectiveness analysis and comparative group of uUTI treatments in the ED at King Abdulaziz Medical City (KAMC) in Saudi Arabia over a three-month follow-up period was done. The patient group consisted of those presenting to the ED with uUTIs who were treated initially with one of five antibiotics: (1) amoxicillin/clavulanic acid, (2) cefuroxime, (3) ciprofloxacin, (4) nitrofurantoin, or (5) norfloxacin. The main outcomes were effectiveness in terms of cure rates, symptom-free days (SFDs), and estimations of cost-effectiveness among this group.Results: A total of 865 adult patients who presented with uUTIs were enrolled. Most patients (89.5%) completely recovered, whereas 10.5% of patients were readmitted to the ED with recurrent infections. Effectiveness in terms of the highest cure rate was observed with nitrofurantoin and amoxicillin/clavulanic acid (93.2% and 92.2%, respectively) followed by norfloxacin, cefuroxime, and ciprofloxacin; no significant differences in cure rates were found among these antibiotics. Antibiotic effectiveness in terms of SFDs showed that nitrofurantoin produced the longest SFD period (76 days) followed by amoxicillin/clavulanic acid (69 days). A cost-effectiveness analysis in terms of uUTI cure rates and number of SFDs indicated that nitrofurantoin presented the highest cost-effectiveness followed by amoxicillin/clavulanic acid, norfloxacin, ciprofloxacin, and cefuroxime.Conclusion: A comparison of five antibiotics for uUTI treatment did not yield clinically significant differences in cure rates.
