Abstract

The placement of a stent in coronary arteries that have become blocked again after coronary artery bypass seems to be better than balloon angioplasty in some ways. A stent is a small cylinder placed in the artery to keep it open. In a group of 220 patients who had received a coronary bypass, those who received a stent were more likely to benefit than those who received balloon angioplasty. This is another technique to open blocked arteries. However, the stent was more likely to cause hemorrhage and it did not reduce the rate of re-blockage in the arteries six months later compared to angioplasty.

44. Trends in cardiovascular mortality of cancer patients in the US over two decades 1999‐2019.

Abstract

Background: Cancer is the second most common cause of death globally after cardiovascular disease, and cancer patients are at an increased risk of CV death. This recognition has led to publication of cardio‐oncological guidelines and to the widespread adoption of dedicated cardio‐oncology services in many institutes. However, it is unclear whether there has been a change in the incidence of CV death in cancer patients. Methods and Results: Using Centers for Disease Control and Prevention Wide‐Ranging, Online Data for Epidemiologic Research (CDC WONDER) Multiple Cause of Death dataset, we determined national trends in age‐standardised mortality rates attributed to cardiovascular diseases in patients with and without cancer, from 1999 to 2019, stratified by cancer type, age, gender, race, and place of residence (state and urbanisation status). Among more than 17.8 million cardiovascular deaths in the United States, 13.6% were patients with a concomitant cancer diagnosis. During the study period, among patients with cancer, the age‐adjusted mortality rate dropped by 52% (vs 38% in patients with no cancer). In cancer patients, age‐adjusted mortality rate dropped more significantly among patients with gastrointestinal, breast, and prostate malignancy than among patients with haematological malignancy (59%‐63% vs. 41%). Similar reduction was observed in both genders (53%‐54%), but more prominent reduction was observed in older patients and in those living in metro areas. Conclusions: Our findings emphasise the role of multidisciplinary management of cancer patients. Widespread adoption of cardio oncology services have the potential to impact the inherent risk of increased CV mortality in both cancer patients and survivors. [ABSTRACT FROM AUTHOR]

Abstract

Balloon-expandable coronary-stent (tube) placement may offer advantages and disadvantages in the treatment of narrowed coronary artery when compared with standard balloon angioplasty. A population of 410 patients were randomly assigned to one procedure or the other and followed for six months. The stent procedure was a success in more patients (96% versus 90%), resulted in a larger opening in the blood vessel both immediately and six months later, and there was a lower rate of recurring constriction (32% versus 42%). Heart-related complications (death, heart attack, coronary-artery bypass surgery, vessel closure, or repeat angioplasty) occurred in 19% of the stent recipients versus 24% of those undergoing standard angioplasty. Fewer patients needed procedures to reopen the vessel (10% versus 15%). However, clots formed in 3% of stent patients, the need for anticoagulants increased bleeding complications (7% versus 4%), and hospital stay was longer (6 days versus 3 days).

Abstract

Background: The role of percutaneous coronary intervention (PCI) vs coronary artery bypass grafting (CABG) in patients with coronary artery disease (CAD) and concomitant end-stage renal disease (ESRD) remains unknown.Research Design& Methods: The National Inpatient Sample (NIS) (2002-2017) was queried to identify all cases of CAD and ESRD. The relative merits of PCI vs CABG were determined using a propensity-matched multivariate logistic regression model. Adjusted odds ratios (aOR) for mortality and other in-hospital complications were calculated.Results: A total of 350,623 [CABG = 112,099 (32%) and PCI = 238,524 (68%)] hospitalizations were included in the analysis. The overall adjusted odds for major bleeding (aOR 1.28, 95% CI 1.25-1.31, P < 0.0001), post-procedure bleeding (aOR 5.19, 95% CI 4.93-5.47, P < 0.0001), sepsis (aOR 1.29, 95% CI 1.26-1.33, P < 0.0001), cardiogenic shock (aOR 1.23, 95% CI 1.20-1.26, P < 0.0001), and in-hospital mortality (aOR 1.65, 95% CI 1.61-1.69, P < 0.0001) were significantly higher for patients undergoing CABG compared with PCI. The need for intra-aortic balloon pump (IABP) placement (aOR 2.52, 95% CI 2.45-2.59, P < 0.001) was higher in the CABG group, while the adjusted odds of vascular complications were similar between the two groups (aOR 0.99, 95% CI 0.94-1.06, P = 0.82). As expected, patients undergoing CABG had a higher mean length of stay and mean cost of hospitalization.Conclusion: CABG in ESRD may be associated with higher in-hospital complications, increased length of stay, and higher resource utilization.
