The aim of the study was to determine the relationship between flatfoot morphology and body mass and height in children aged 6–12 years. A total of 6471 Chinese children (mean age 9.0 ± 1.9 years, 41% female) were assessed for foot morphometry, body height, and body mass index. Foot morphology, including foot length, width, girth, arch height, hallux valgus angle, and rearfoot valgus angle, was measured using a 3D laser scanner. Flatfoot evaluations were conducted using the Sztriter–Godunov index (KY) from footprints. All measurements were analyzed by age and sex using the mean values of the left and right sides. Comparisons were performed between flatfoot groups, between body mass index (BMI) groups, and between body height groups. The study revealed a significant decrease in the incidence of bipedal flatfoot with age (p < 0.001), whereas the prevalence of obesity remained consistent (p > 0.05). Bipedal flatfoot was associated with distinct morphological changes, including lower arches, reduced instep height, diminished ankle heights and a greater rearfoot valgus angle (p < 0.05). When comparing the BMI groups, overweight children had larger and thicker feet (p < 0.05), but no differences were found in arch height and ankle height (p > 0.05). When comparing the body height groups, short-statured children had a shorter feet girth, shorter arches, and shorter ankle height (p < 0.05), but no differences were found in the rearfoot valgus angle (p > 0.05). Conclusion: The main characteristics of flat feet include lower arches and instep heights and ankle heights but higher rearfoot valgus angles. In general, overweight children's feet do not have the common features of flat feet. In contrast, short children had similar features of flatfoot except for rearfoot valgus. Assessment of posture, such as rearfoot valgus, can be critical in identifying children with flat feet. Background. The purpose of the study was to describe the most sensitive and specific radiological parameter to diagnose symptomatic adult acquired flatfeet deformity (AAFD) with weight bearing radiographs in Indian population. Methods. A retrospective study was conducted in which 50 consecutive patients with 100 flat feet were included at department of orthopedics from August 2021 to October 2022. Amongst them, 32 were bilateral (64 feet) and 18 were unilateral (18 flat feet and 18 normal feet) flat feet. Results. Out of these 50 subjects, 32 (64%) had bilateral flat feet, 18 (36%) were unilateral (11 left foot and 7 right foot). Out of 50 patients (100 feet), 24 had symptoms on left foot, whereas 26 had symptoms on right foot (total number of symptomatic patients were 50). Amongst our study population (50 subjects), 23 (46%) patients did not have any co-morbidities. 18 (36%) patients had type 2 diabetes, 5 (10%) patients had hypertension, 3 (6%) patients were associated with hypothyroidism and one individual (2%) showed psychiatric disorder. The talar 1st metatarsal angle in lateral view, talar uncoverage angle and the talar 1st metatarsal angle in anteroposterior (AP) view, were found to be statistically significant. Conclusions. Three radiologic parameters-talar 1st metatarsal angle in lateral view and talar uncoverage angle and talar-1st metatarsal angle in anteroposterior view can be the most relevant radiological parameters in the assessment of AAFD in Indian population. Abstract Background Idiopathic flexible flatfoot is a common condition in children which typically improves with age and remains asymptomatic. However, the condition can sometimes be more severe, and cause mechanical impairment or pain. The aim of the study was to perform a prospective clinical, radiological, podoscopic and pedobarographic assessment (static and dynamic) of subtalar titanium screw arthroereisis for the treatment of symptomatic, idiopathic, flexible flatfeet. Methods A prospective, consecutive, non-controlled, cohort, clinical follow-up study was performed. In total, 30 patients (41 feet), mean age 10 (6 to 16 years), were evaluated. Clinical and standing radiological assessments, static and dynamic pedobarography, as well as podoscopy, were performed before surgery and at final follow-up. Results Treatment was associated with significant improvements in heel valgus angle, radiographic parameters (lateral and dorso-planar talo-first metatarsal angle, calcaneal inclination angle, talar declination angle, longitudinal arch angle) and podoscopic parameters (Clark’s angle, Staheli’s arch index and Chippaux-Smirak index). Significant increases were noted for lateral loading, forefoot contact phase and double support / swing phase, and reduced medial loading (dynamic pedobarography), as well as lateral midfoot area and loading, but decreased were observed for medial forefoot loading (static pedobarography). Four patients reported persistent pain in the sinus tarsi region (six feet), and in one case, the implant was replaced for a larger one due to undercorrection. No overcorrections or infection complications were noted in the study group. Conclusions Subtalar arthroereisis is a minimally-invasive and effective surgical method for treating symptomatic, idiopathic, flexible flatfeet; it has an acceptable complication rate with good early clinical results. This systematic review aimed to evaluate custom-made 3D printed insoles for people with flatfeet. PubMed, Embase, ISI web of knowledge, ProQuest, Scopus, and Cochrane databases, were searched from inception until January 2022. The quality assessment of included studies was performed through the Downs and Black checklist. A narrative analysis was performed since a meta-analysis could not be conducted. Ten studies including 225 subjects with flexible flatfeet were chosen for final evaluation. Although the evidence from selected literature was generally weak, using insoles with 3D printing technology may positively affect pain (comfort score) and foot function, with no significant change in vertical loading rate during walking or running. There were discrepancies among studies for plantar pressures, center of pressure trajectories, 3D ankle joint kinematics and kinetics of gait while wearing these insoles. Dose-response effects of medial posting on 3D printed insoles suggested beneficial effects on lower limb gait biomechanics in people with flatfeet. There was insufficient evidence to conclude the comparison between 3D printed insoles and other types of insoles. Severe flexible flatfeet with triceps surae complex shortening are prognostically unfavorable in early childhood and may compromise normal foot development. This retrospective, IRB-approved study included 20 children (38 feet) under 6 years with severe flexible flatfeet and triceps surae complex shortening. Treatment included minimally invasive percutaneous achilles tendon lengthening followed by a 4-week cast fixation and corrective orthotic therapy under talo-navicular reposition for at least 6-months. Preoperative weightbearing x-rays and at the last available follow-up included anteroposterior talus-first metatarsal angle and lateral talus pitch, Meary's and talocalcaneal angle and were compared to reference values. ROM, surgeon-rated clinical outcomes and complications/re-interventions were evaluated. Age at surgery was 3.7 years (1.3–5.9 y) and follow-up time was 4.3 years (1.1–8.9 y). No complications occurred. Clinical outcome was good (68 %) to very good (26 %). Ratio of normal angles increased significantly for three angles. Dorsiflexion ROM improved from −5.0 ± 6.8° at baseline to 15.7 ± 7.6°. With significant improvements in clinical and radiographic outcomes, minimal-invasive percutaneous Achilles tendon lengthening followed by orthotic therapy seems to be a valuable treatment option for selected preschool-aged patients with severe, flexible flatfeet with significantly shortened triceps surae. This study aimed to estimate annual trend hospitalizations for flatfoot in Italian paediatric population from 2001 to 2016. Methods Data of this study were collected from the National Hospital Discharge Reports (SDO) reported at the Italian Ministry of Health regarding the years of this paper (2001–2016). The yearly number of hospital admission for flatfoot, the percentage of males and females, the average age, the average days of hospitalization, primary diagnoses and primary procedures in the whole Italian population were calculated using descriptive statistical analyses. Results 109,300 hospitalizations for flatfoot of young patients were performed during this period. 59.3% of patients were male and 40.7% female of the 10–14 years-old age class. The average days of hospitalization stay were 1.73 ± 1.27 days. The data highlights that the burden of flatfoot surgery is growing and affecting the healthcare system. The mean rate of hospital admissions in Italy for flatfoot in the young population was 82.14 for 100,000 inhabitants of the same age class. Conclusions The data highlights that the cases of flatfoot surgery increased from 2001 to 2016. The most common treatment was the “Internal Fixation Of Bone Without Fracture Reduction, Tarsals And Metatarsals followed by Subtalar Fusion and Arthroereisis. Further prospective studies on this topic may be conducted to improve the evidence of the results.

The goal of this systematic review was to to assess outcomes in individuals with AAFD (focusing on stage II). Three databases were searched for terms referring to the PTTD and AAFD up to and including 31 July 2018. Studies were eligible if they were published in English language and contained data on outcomes of stage II Management. Thirty-two eligible studies were included in this review. The sample size of the treatment trials ranged from 12 to 129 patients, with only two trials having more than 100 participants. The age ranges were wide. Pain, adverse effects, function or disability indices of foot, patients’ satisfaction, radiographic parameters, alignment and improvement of foot function, and quality of life were the searched outcomes. Due to clinical and methodological heterogeneity, data were not pooled into meta-analysis. The evidence from the selected studies is currently too limited about each procedure compared to its counterpart to draw definitive conclusions about the use of each intervention for AAFD. Future high quality comparative studies are warranted in this field. Only limited interventions commonly used in practice have been studied and there is much debate over the treatment of symptomatic and asymptomatic adult pes planus.

Abstract Background Achilles tendinopathy is one of the most common overuse injuries in running, and forefoot pronation, seen in flatfeet participants, has been proposed to cause additional loading across the Achilles tendon. Foot orthoses are one of the common and effective conservative treatment prescribed for Achilles tendinopathy, it works by correcting the biomechanical malalignment and reducing tendon load. Previous studies have shown reduction of Achilles Tendon load (ATL) during running by using customized arch support orthosis (CASO) or an orthotic heel lift (HL). However, there are still little biomechanical evidence and comparative studies to guide orthotic prescriptions for Achilles tendinopathy management. Therefore, this study seeks to investigate the two currently employed orthotic treatment options for Achilles tendinopathy: CASO and HL for the reduction of ATL and Achilles tendon loading rate (ATLR) in recreational runners with flatfeet. Methods Twelve participants were recruited and run along the runway in the laboratory for three conditions: (1) without orthoses, (2) with CASO (3) with HL. Kinematic and kinetic data were recorded by 3D motion capturing system and force platform. Ankle joint moments and ATL were computed and compared within the three conditions. Results Participants who ran with CASO (p = 0.001, d = 0.43) or HL (p = 0.001, d = 0.48) associated with a significant reduction in ATL when compared to without orthotics while there was no significant difference between the two types of orthoses, the mean peak ATL of CASO was slightly lower than HL. Regarding the ATLR, both orthoses, CASO (p = 0.003, d = 0.93) and HL (p = 0.004, d = 0.78), exhibited significant lower value than the control but similarly, no significant difference was noted between them in which the use of CASO yielded a slightly lower loading rate than that of HL. Conclusions Both CASO and HL were able to cause a significant reduction in peak ATL and ATLR comparing to without orthotics condition. There were subtle but no statistically significant differences in the biomechanical effects between the two types of orthoses. The findings help to quantify the effect of CASO and HL on load reduction of Achilles tendon and suggests that foot orthoses may serve to prevent the incidence of Achilles tendon pathologies. Although the foot is involved in load-bearing and shock absorption, foot pressure (FP), ground contact area (CA), and gait cycle (GC) in flatfeet (FF) have not been examined in detail. We aimed to analyze the influence of FF on FP, CA, and GC. We included 20 and 21 women with FF and normal feet (NF), respectively. A Footscan plantar pressure plate (RsScan International, Belgium) was used to analyze FP, CA, and GC. FP was applied to the unit area of 10 compartments. GC analysis was performed using phase-time measurements by dividing the GC into four phases. In the analysis, FP and CA were compared between the FF and NF groups. A comparison of GC was similarly performed between the two groups.The data provided in this article will be useful when designing studies on the effect of foot shape on FP, CA, and GC during gait.

Introduction: Flexible flatfoot refers to the loss of the medial longitudinal arch of the foot on weight bearing and is associated with excessive heel eversion or forefoot abduction. Unless symptomatic, flexible flatfeet are best managed non-operatively. The calcaneo-cuboid-cuneiform osteotomy is a procedure that restores the anatomical shape of the foot without arthrodesis of the joints. Our study aims to evaluate the functional and radiological outcomes of patients treated with calcaneo-cuboid-cuneiform osteotomy in patients with planovalgus feet. Methodology: A retrospective review of records and radiographs of patients with symptomatic flexible planovalgus feet, who were operated with the calcaneo-cuboid-cuneiform osteotomy by a single senior surgeon in a time period between April 2016 and July 2017 was done. The clinical and radiological outcomes were evaluated in 12 feet in 8 children. Results: A total of 12 feet in 8 children were operated (6 males and 2 females). Average age of patients was 11 ± 1.27 years; average follow up was 14.7 months ± 2.7 months. Two patients had planovalgus feet secondary to spastic diplegia and 6 had idiopathic planovalgus feet. There was a statistically significant improvement in the pain score as well as the radiographic parameters in all the operated patients. Conclusion: The calcaneo-cuboid-cuneiform osteotomy has potential to give good results for symptomatic planovalgus feet with minimal complications. [ABSTRACT FROM AUTHOR]

Background: Overcorrection is a recognized problem following surgical treatment of congenital clubfoot. Recently this complication has also been mentioned following Ponseti treatment.Research Question: Do overcorrected clubfeet (OCCF) caused by surgery behave differently from those caused by Ponseti treatment in terms of segmental motion of the feet and show differences in the severity of deformity on X-rays?Methods: Children between 7 and 12 years with OCCF were included in this study. Depending on the aetiology causing them, the feet were divided into 2 groups (Ponseti and peritalar release surgery). 25 typically developing children served as controls. All subjects were subjected to clinical and radiological examination and 3-Dimensional gait analysis using the Oxford Foot Model.Results: Thirty-two children with OCCF, of these 18 feet in the surgical and 14 feet in the Ponseti group, were included in the study. No radiological differences were seen in the flatfoot parameters between OCCF groups except in the calcaneal inclination angle that was more pathological in the Ponseti group. The clinical ankle plantar flexion was significantly reduced in the surgical group. During walking the range motion of the hindfoot in the frontal plane was significantly reduced in surgically treated feet compared to the Ponseti group. The other parameters did not show any significant difference between groups.Significance: The overcorrected clubfeet following surgery and Ponseti showed similar appearance and showed no significant differences in 11/12 radiological parameters. The segmental motion of the feet showed no significant differences between groups except the in the range of motion of the subtalar eversion. Background: This study compares radiological and functional outcomes of patients with symptomatic Adult Acquired Flat Foot Deformity (AFFD) secondary to stage 2B Posterior Tibial Tendon Dysfunction (PTTD) treated with either a Subtalar Arthroereisis (SA) implant or a Lateral Column Lengthening (LCL) procedure. The aim of the study is to determine if the Subtalar Arthroereisis procedure is an acceptable surgical adjunct in our treatment armamentarium. Materials and Methods: 21 patients (22 feet) were evaluated. 12 consecutive patients (n = 12) underwent a Subtalar Arthroereisis procedure while 9 patients (n = 10) underwent a Lateral Column Lengthening procedure. Functional outcomes were evaluated using the American Orthopaedic Foot and Ankle Score (AOFAS) midfoot score, Short Form 36 (SF-36) and Visual Analog of Pain Scale (VAS), preoperatively and at 6 and 12 months postoperatively. 10 radiological parameters were analyzed. All complications were tabulated. Results: Significant improvements in functional outcomes were noted in both groups of patients. Radiographically, some loss of correction was noted in the SA group and no loss of correction in the LCL group, respectively. In the SA group, 6 patients complained of sinus tarsi pain in the postoperative period and 4 required removal of implants. No implants were removed in the LCL group. Conclusion: The results suggest that subtalar arthroereisis may be a reasonable procedure to utilize in the treatment of AAFD, albeit with a relatively high implant-removal rate. Introduction: Flatfoot (pes planus) is a clinical condition associated with unusual biomechanical alterations in medial longitudinal arch. The medial longitudinal arch of foot is impaired, and hence, the entire sole touches the ground making the sole of foot to flatten. The aim of this work was to determine the prevalence of flatfoot among males and females of the student community aged between 18 and 25 by calculating the Chippaux-Smirak index (CSI). Materials and Methods: The study included 50 college students. Their ages ranged between 18 and 21 years. Footprints of all the 50 subjects were obtained using simple ink print method. The presence of flatfeet was diagnosed by calculating the CSI. If the CSI is greater than 45%, then the individual is considered flatfooted. Results: Of 50 subjects, 25, i.e. 50% had flatfeet with P < 0.05. Ten of 25 females (40%) were found to be flatfooted, whereas for males, it was 13 of 25 (52%). Conclusion: The study was successful as we concluded with a significant result (P < 0.05) about men having quite more significant flatfeet. The foot screening procedure presented in this study provides a strategy for recruiting participants with normal and flatfoot impression. Background: The paediatric flatfoot is a common presentation but it is unclear whether the condition will resolve on its own as the child gets older or whether treatment is required. Therefore, the study objective was to evaluate paediatric foot posture, and anthropometry, in children at two time points, three years apart.Material and Methods: A sample of 1032 healthy children (505 boys, 527 girls; aged 5-11 years) was recruited for foot posture index (FPI) and anthropometry assessment (weight, height and body mass index, BMI). Assessment was repeated when the children were aged 8 years to 14 years. Paired t-tests, Anova, frequency tables and a multiple regressions were conducted.Results: Initially, approximately 70% had a neutral FPI range, 20% pronated, 3% highly pronated, and 4% supinated. Initial mean FPI was 3.6 ± 2.8, being higher in boys 3.7 ± 2.8 than in girls 3.4 ± 2.7 (p = 0.034). All FPI categories changed over time, with supinated and neutral FPI increased by 19.5% and 4.7% respectively. In contrast, pronated and highly pronated FPI reduced by 10.6% and 55.6% respectively. Regression showed only 1% FPI change was explained by increased height. FPI scores were significantly reduced after three years (3.57 to 3.33; p < 0.001).Conclusion: Children's foot posture shifts toward neutral as age increases. There is minimal relationship with weight, height or BMI. Appreciation of developing foot posture could reduce over diagnosis and unnecessary treatment of paediatric flatfeet. [ABSTRACT FROM AUTHOR]

Backgrounds The effect of foot orthoses on plantar pressure distribution has been proven by researchers but there are some controversies about advantages of custom-made foot orthoses to less expensive prefabricated foot orthoses. Methods Nineteen flatfeet adults between 18 and 45 participated in this study. CAD–CAM foot orthoses were made for these patients according to their foot scan. Prefabricated foot orthoses were prepared according to their foot size. Plantar pressure, force and contact area were measured using pedar ® -x in-shoe system wearing shoe alone, wearing CAD–CAM foot orthoses and wearing prefabricated foot orthoses. Repeated measures ANOVA model with post-hoc, Bonferroni comparison were used to test differences. Results CAD–CAM and prefabricated foot orthoses both decreased pressure and force under 2nd, 3–5 metatarsal and heel regions comparing to shoe alone condition. CAD–CAM foot orthosis increased pressure under lateral toe region in comparison to shoe alone and prefabricated foot orthosis. Both foot orthoses increased pressure and contact area in medial midfoot region comparing to shoe alone condition. Increased forces were seen at hallux and lateral toes by prefabricated foot orthoses in comparison with CAD–CAM foot orthoses and control condition, respectively. Conclusion According to the results, both foot orthoses could decrease the pressure under heel and metatarsal area. It seems that the special design of CAD–CAM foot orthoses could not make great differences in plantar pressure distribution in this sample. Aim The aim of this study was to investigate the intratester reliability of digital photographic method for quantifying static lower extremity alignment in individuals with flatfeet and normal feet types. Methods Thirteen females with flexible flatfeet and nine females with normal feet types were recruited from university communities. Reflective markers were attached over the participant's body landmarks. Frontal and sagittal plane photographs were taken while the participants were in a standardized standing position. The markers were removed and after 30 min the same procedure was repeated. Pelvic angle, quadriceps angle, tibiofemoral angle, genu recurvatum, femur length and tibia length were measured from photographs using the Image j software. Results All measured variables demonstrated good to excellent intratester reliability using digital photography in both flatfeet (ICC: 0.79–0.93) and normal feet type (ICC: 0.84–0.97) groups. Conclusion The findings of the current study indicate that digital photography is a highly reliable method of measurement for assessing lower extremity alignment in both flatfeet and normal feet type groups. Purpose: The purpose of this study was to clarify the difference in foot kinematics, including detailed analysis of the mid- and forefoot, between normal and flat feet. Methods: Forty-six feet of 33 young normal volunteers were participated in this study. All subjects were categorized two groups which were normal foot group and flat foot group. The fifteen color markers were mounted over the anatomical landmarks. Foot motion in a stance with the measurement foot forward, the body weight loaded on the forefoot as much as possible with the lower leg inclined forward and the entire plantar surface in contact with the floor was recorded using four hi-definition digital video cameras. All markers were manually digitized using the Frame-DIAS4 software program (DKH Co. Ltd, Tokyo, Japan). The three directional coordinates of each marker were calculated, and the three directional movements of each marker were compared between normal feet and flat feet using the Man-Whitney U-test. Moreover, discriminant functional analyses were performed on all combinations if significant differences were found between normal feet and flat feet. Results: Normal foot showed medial inclination with maintaining the arch structure, however flat foot showed that forward splaying with collapsed arch structure. Moreover, the forward movements of the cuboid and medial movement of the third metatarsal base may be key movements. Abstract Background Symptoms associated with pes planovalgus or flatfeet occur frequently, even though some people with a flatfoot deformity remain asymptomatic. Pes planovalgus is proposed to be associated with foot/ankle pain and poor function. Concurrently, the multifactorial weakness of the tibialis posterior muscle and its tendon can lead to a flattening of the longitudinal arch of the foot. Those affected can experience functional impairment and pain. Less severe cases at an early stage are eligible for non‐surgical treatment and foot orthoses are considered to be the first line approach. Furthermore, strengthening of arch and ankle stabilising muscles are thought to contribute to active compensation of the deformity leading to stress relief of soft tissue structures. There is only limited evidence concerning the numerous therapy approaches, and so far, no data are available showing functional benefits that accompany these interventions. Methods After clinical diagnosis and clarification of inclusion criteria (e.g., age 40–70, current complaint of foot and ankle pain more than three months, posterior tibial tendon dysfunction stage I & II, longitudinal arch flattening verified by radiography), sixty participants with posterior tibial tendon dysfunction associated complaints will be included in the study and will be randomly assigned to one of three different intervention groups: (i) foot orthoses only (FOO), (ii) foot orthoses and eccentric exercise (FOE), or (iii) sham foot orthoses only (FOS). Participants in the FOO and FOE groups will be allocated individualised foot orthoses, the latter combined with eccentric exercise for ankle stabilisation and strengthening of the tibialis posterior muscle. Participants in the FOS group will be allocated sham foot orthoses only. During the intervention period of 12 weeks, all participants will be encouraged to follow an educational program for dosed foot load management (e.g., to stop activity if they experience increasing pain). Functional impairment will be evaluated pre‐ and post‐intervention by the Foot Function Index. Further outcome measures include the Pain Disability Index, Visual Analogue Scale for pain, SF‐12, kinematic data from 3D‐movement analysis and neuromuscular activity during level and downstairs walking. Measuring outcomes pre‐ and post‐intervention will allow the calculation of intervention effects by 3×3 Analysis of Variance (ANOVA) with repeated measures. Discussion The purpose of this randomised trial is to evaluate the therapeutic benefit of three different non‐surgical treatment regimens in participants with posterior tibial tendon dysfunction and accompanying pes planovalgus. Furthermore, the analysis of changes in gait mechanics and neuromuscular control will contribute to an enhanced understanding of functional changes and eventually optimise conservative management strategies for these patients. Three FPI‐6 scores levels defined the range of flatfeet detected: FPI‐6 ≥ +6; FPI‐6 ≥ +8; FPI‐6 ≥ +10. BMI cut‐points were used to define overweight for each age group. Results In the study population of 728 children, flatfeet (FPI ≥ +6) were found in 290 (40 %) cases and non‐flatfeet in 438 (60 %) cases. FPI ≥ +8 yielded flatfeet in 142 (20 %) cases and FPI ≥ +10 yielded flatfeet in 41 (5 %) cases. Whilst 272 (37 %) children were overweight, only 74 (10.1 %) of the overweight children had flatfeet (FPI ≥ +6), which diminished to 36 (4.9 %) at FPI ≥ +8, and 9 (1.2 %) at FPI ≥ +10. Conclusions: This study found no association between increased body mass and flatfeet in children, a finding in contrast to that repeatedly concluded by many previous studies. Whilst properties of the FPI and BMI are limiting, these findings question the concern about children's increased body mass as a specific influence on (flatter) foot posture, and also the validity of footprint versus anatomically based foot posture measures. Different postural stability may be a contributor to secondary injuries in individuals with flexible flatfeet (FF) compared to those with neutral feet (NF). However, the differences between static and dynamic stability of FF and NF have not been examined. This study compared the static and dynamic stability of subjects with FF and NF and investigated the relationship between static and dynamic stability. Twenty-eight subjects (14 each in the FF and NF groups) performed three tasks (single leg standing with eyes open, with eyes closed, and the Y balance test). We quantified the center of pressure (COP) speed and Y balance test score (Y score) within the tasks. COP speed was significantly greater in the FF group than in the NF group under both conditions (eyes open and closed) and directions (anteroposterior and mediolateral). Y scores did not differ significantly between the two groups. No significant relationship was observed between the COP speed and Y score in either group. These results show that individuals with FF have different static stabilities, but not dynamic stabilities, compared with those with NF. Purpose: The effect of paediatric flexible flatfeet (PFF) on health-related quality of life (HRQOL) has not been investigated. In this prospective cross-sectional study, the HRQOL of children with PFF was compared to those with typically developing feet (TDF) using two validated measures. We hypothesised that reduced HRQOL would be observed in children with PFF. The reliability of parents' perceptions of their child's symptoms was also investigated. Methods: 48 children with PFF and 47 with TDF between the ages of 8 and 15 completed The Oxford Ankle Foot Questionnaire for Children (OxAFQ-C) and Pediatric Quality of Life Inventory (PedsQL™ 4.0). Proxy questionnaires were also completed. Reliability of parent and child questionnaire scores was assessed using the intraclass correlation coefficient (ICC) and Student's t test. Differences between HRQOL between PFF and TDF were assessed using the Student's t test. Results: ICCs overall demonstrated good reliability between parent and child questionnaire domain scores. There was a tendency for parents to overestimate the impairment of the child in the PFF group. PFF children demonstrated clinically significant decreased HRQOL than TDF children. This was most marked in the physical domain scores. Conclusion: Although parents may overestimate their child's impairment, children with PFF still have significantly impaired HRQOL when compared to TDF children. The impairment can be as severe, or worse, than published HRQOL for acutely and chronically unwell children. As such, PFF cannot be regarded as just a benign normal variant. Background: Increased navicular drop (NDro) and navicular drift (NDri) are associated with musculoskeletal pathology in adults. The aim of this study was to investigate navicular motion in children, with respect to foot posture, and identify altered patterns of motion that demonstrate midfoot dysfunction. Navicular motion in different activities was evaluated as well as the role of flexibility and body mass index (BMI). Methods: Twenty-five children with flatfeet and 26 with neutral feet (age range, 8-15) underwent gait analysis using a 12-camera Vicon MX system (Vicon, UK). Navicular motion indices were calculated from marker coordinates. Student t tests and Pearson’s correlation coefficient (R) were used to investigate navicular motion differences between groups. The relationship between NDRo, NDRi, and their dynamic counterparts was also assessed. Results: Normalized NDri (NNDri) and normalized NDro (NNDro) correlated strongly in neutral feet (R = 0.56, P = .003) but not in flatfeet (R = 0.18, P > .05). Flatfeet demonstrated reduced NNDri compared to neutral footed children (0.7 vs 1.6, P = .007). No difference was observed in NNDro between groups. Standard and dynamic measures of NDri and NDRo were highly correlated. Navicular motion correlated poorly with BMI and flexibility. Conclusion: Motion of the navicular in the transverse and the sagittal plane is important when investigating foot function. Uncoupling of this motion in flatfeet may indicate impaired midfoot function. Reduced navicular medial translation in flatfeet may indicate altered alignment of the talonavicular joint. Podiatry screening of children is a common practice, which occurs largely without adequate data to support the need for such activity. Such programs may be either formalised, or more ad hoc in nature, depending upon the use of guidelines or existing models. Although often not used, the well-established criteria for assessing the merits of screening programs can greatly increase the understanding as to whether such practices are actually worthwhile. This review examines the purpose of community health screening in the Australian context, as occurs for tuberculosis, breast, cervical and prostate cancers, and then examines podiatry screening practices for children with reference to the criteria of the World Health Organisation (WHO). Topically, the issue of paediatric foot posture forms the focus of this review, as it presents with great frequency to a range of clinicians. Comparison is made with developmental dysplasia of the hip, in which instance the WHO criteria are well met. Considering that the burden of the condition being screened for must be demonstrable, and that early identification must be found to be beneficial, in order to justify a screening program, there is no sound support for either continuing or establishing podiatry screenings for children.
