Introduction: Binswanger's disease (B D) i s characterized by microvascular disease in the core penetrating arterioles. Case report: Patient 77 year old woman presents for three months holocraneal intense headache associated with retrograde amnesia, dysarthria, gait slowing and drowsiness. Its history II Diabetes Mellitus and Hypertension diagnosed six years ago at irregular treatment with Glibenclamide and Captopril. Interpretation: Because of the age of the patient, multiple medical comorbidities, complementary diagnostics and the progressive evolution of clinical diagnosis of subcortical encephalopathy of Binswanger was considered..
