Background: Whilst there are a number of publications comparing the relationship between body mass index (BMI) of kidney transplant recipients and graft/patient survival, no study has assessed this for a French patient cohort.Methods: In this study, cause-specific Cox models were used to study patient and graft survival and several other time-to-event measures. Logistic regressions were performed to study surgical complications at 30 days post-transplantation as well as delayed graft function.Results: Among the 4691 included patients, 747 patients were considered obese with a BMI level greater than 30 kg/m2. We observed a higher mortality for obese recipients (HR = 1.37, p = 0.0086) and higher risks of serious bacterial infections (HR = 1.24, p = 0.0006) and cardiac complications (HR = 1.45, p < 0.0001). We observed a trend towards death censored graft survival (HR = 1.22, p = 0.0666) and no significant increased risk of early surgical complications.Conclusions: We showed that obesity increased the risk of death and serious bacterial infections and cardiac complications in obese French kidney transplant recipients. Further epidemiologic studies aiming to compare obese recipients versus obese candidates remaining on dialysis are needed to improve the guidelines for obese patient transplant allocation. Calcineurin inhibitor (CNI) toxicity contributes to chronic allograft nephropathy (CAN). In the 2-year, randomized, study, we showed that 50% cyclosporin (CsA) reduction in combination with mycophenolate mofetil (MMF) treatment improves kidney function without increasing the risk for graft rejection/loss. To investigate the long-term effect of this regimen, we conducted a follow up study in 70 kidney transplant patients until 5 years after REFERENCE initiation. The improvement of kidney function was confirmed in the MMF group but not in the control group (CsA group). Four graft losses occurred, 2 in each group (graft survival in the MMF group 95.8% and 90.9% in control group). One death occurred in the control group. There was no statistically significant difference in the occurrence of serious adverse events or acute graft rejections. A limitation is the weak proportion of patient still remaining within the control group. On the other hand, REFERENCE focuses on the CsA regimen while opinions about the tacrolimus ones are still debated. In conclusion, CsA reduction in the presence of MMF treatment seems to maintain kidney function and is well tolerated in the long term. [ABSTRACT FROM AUTHOR]

In immunosuppressed individuals, such as hematopoietic stem cell transplant recipients, adenoviruses (ADVs) are a well-known cause of morbidity and mortality, with limited treatment options. However, only a few cases were reported in patients with acquired immunodeficiency syndrome (AIDS), and little is known about the relevance of such an infection in these patients with many other concomitant opportunistic infections. We report the case of a 34-year-old man with AIDS presenting with gross hematuria, right flank pain, and acute decrease in kidney function superimposed on chronic kidney disease. His CD4 count was 0/muL despite highly active antiretroviral therapy. A computed tomographic scan showed enlargement of the right renal pelvis. Cystoscopy showed no clots or macroscopic lesions. Urine analysis showed no bacteria or abnormal epithelial cells. ADV was found in viral culture and by using real-time polymerase chain reaction in the patient's urine and later in blood. The renal biopsy specimen showed ADV-related tubulointerstitial nephritis with intranuclear inclusions in tubular cells stained by anti-ADV antibodies, in addition to chronic tubular and vascular changes. The ADV serotype belonged to subgroup B. Cidofovir therapy was contraindicated for this patient; therefore, he was administered intravenous ribavirin. The efficiency of this treatment could not be assessed because he rapidly developed neutropenia and disseminated aspergillosis and died. The XIII Banff meeting, held in conjunction the Canadian Society of Transplantation in Vancouver, Canada, reviewed the clinical impact of updates of C4d‐negative antibody‐mediated rejection (ABMR) from the 2013 meeting, reports from active Banff Working Groups, the relationships of donor‐specific antibody tests (anti‐HLA and non‐HLA) with transplant histopathology, and questions of molecular transplant diagnostics. The use of transcriptome gene sets, their resultant diagnostic classifiers, or common key genes to supplement the diagnosis and classification of rejection requires further consensus agreement and validation in biopsies. Newly introduced concepts include the i‐IFTA score, comprising inflammation within areas of fibrosis and atrophy and acceptance of transplant arteriolopathy within the descriptions of chronic active T cell–mediated rejection (TCMR) or chronic ABMR. The pattern of mixed TCMR and ABMR was increasingly recognized. This report also includes improved definitions of TCMR and ABMR in pancreas transplants with specification of vascular lesions and prospects for defining a vascularized composite allograft rejection classification. The goal of the Banff process is ongoing integration of advances in histologic, serologic, and molecular diagnostic techniques to produce a consensus‐based reporting system that offers precise composite scores, accurate routine diagnostics, and applicability to next‐generation clinical trials.

Xenotransplantation between discordant species leads to a hyperacute rejection mediated by natural antibodies. both of the IgG and IgM isotypes, activation of complement and endothelial cell activation. The combination of these mechanisms leads to a transplant survival of minutes to a few hours. Polyclonal human irnmunoglobulins for intravenous use ([V]g) from normal donors have proved effective in a number of antibody-mediated disorders, as well as in inflammatory disorders. We demonstrate that administration of [V]g in a guinea pig to rat model of cardiac xenografting can effectively delay hyperacute rejection. This effect is mediated by the F(ab')2 fragments of [V]g. and is correlated to an anti-complementary activity. [ABSTRACT FROM AUTHOR]

Sixty HIV-infected patients presenting renal symptoms who underwent percutaneous renal biopsies were analysed. According to the CDC classification, 44 patients were staged in group IV, five in group III, and 11 in group II. Patients were divided in two groups according to their ethnic origin (29 black patients and 31 white patients). Risk factors such as homosexuality, multiple transfusions or intravenous drug abuse (IVDA) were identified in all white patients except two, but in only nine (31%) of the black patients. Three main patterns of renal disease were observed: focal and segmental glomerulosclerosis (FSGS) was found predominantly in black patients (23 black patients versus 3 Caucasians, < 0.001) and was associated with the nephrotic syndrome; immune-complex-type glomerulonephritis (ICGN) was frequent in black and white patients (21% and 52% respectively) including four cases of IgA nephritis all seen in white patients; and 10 cases of lupus-like nephritis (4 black and 6 white patients). The frequent hypergammaglobulinaemia in those patients suggests a pathogenic role of polyclonal B cell activation in ICGN. Interstitial nephritis was present in 48 and 52% of the black and white patients respectively and did not seem related to drug toxicity or superimposed infectious disease. In addition to interstitial nephritis, the coexistence of multivisceral lymphocytic infiltration involving accessory salivary glands, liver and/or lung, found in six patients possibly suggests a virus-induced immune disorder. Introduction Le trafic d’organe, le tourisme de transplantation et le commerce de greffons ont fait l’objet d’un sommet à Istanbul au printemps 2008 aboutissant à la Déclaration d’Istanbul dont le but était de condamner et de faire cesser ces pratiques [1]. En collaboration avec l’Agence de la biomédecine, une enquête nationale auprès des médecins des centres de transplantation rénale avait été réalisée entre 2008 et 2012. Patients/Matériels et méthodes Nous avons réactualisé, en 2017, les données médicales de cette cohorte, notamment les complications et la survie des greffons et des patients et évalué le nombre de nouveaux patients transplantés en France dans le cadre du tourisme de transplantation depuis 2012. Observation/Résultats Nous avons identifié 59 patients dont 41 hommes et 18 femmes, qui ont été transplantés à l’étranger à partir de rein de donneurs rémunérés, entre 1985 et 2017. Onze patients ont été transplantés avant 2000, 38 entre 2000 et 2008 et 10 après 2008. La durée médiane de suivi était de 5,6 années (3,7 ; 12,1). La créatinine moyenne était à 111 ± 49,2 μmol/L à la date du dernier contrôle. Quatre patients ont présenté des complications chirurgicales (deux sténoses de l’artère du greffon, une infection de cicatrice et un hématome). Les infections représentaient la complication la plus fréquente puisqu’elles ont concerné 34 patients. Parmi eux, il y eu une séroconversion VHC, sept réactivations VHC, deux réactivations VHB, une coréactivation VHC et VHB, deux tuberculoses dont une disséminée, trois pneumocystose, une aspergillose, deux infections fongiques. Seize patients ont présenté un rejet dont 7 rejets humoraux et un rejet mixte. Quatorze patients (23,7 %) ont perdu leur greffon. Onze patients (18,6 %) sont décédés. Discussion/Conclusion Le tourisme de transplantation en France reste un phénomène marginal. Le nombre de patients transplantés avec un greffon provenant d’un donneur rémunéré est en nette régression depuis la conférence d’Istanbul en 2008. Ces pratiques sont associées à une importante morbi-mortalité [2]. Néanmoins, chez les patients n’ayant pas présenté de complications, la fonction du greffon demeure satisfaisante à long terme. Introduction Les études comparant l’efficacité des machines de perfusion hypothermiques (MP) par rapport à la conservation statique (CS) se fondent sur des critères à court terme, en particulier le retard au démarrage [1]. Notre étude a pour but d’identifier un profil de donneur qui maximiserait le bénéfice de la MP en terme de survie du patient et son greffon à moyen terme. Patients/Matériels et méthodes Il s’agit d’une étude de cohorte prospective multicentrique de patients transplantés rénaux pour la première ou la seconde fois entre 2010 et 2017 dans 8 centres hospitaliers français. Notre critère de jugement principal est la survie patient-greffon. Notre cohorte (n = 3154) a été divisée aléatoirement en deux échantillons, l’un d’apprentissage (n = 2057) et l’autre de validation (n = 1097). Les interactions entre l’utilisation des MP et les caractéristiques des donneurs ont été testées sur la cohorte d’apprentissage par un modèle de Cox puis validées la cohorte indépendante. Observation/Résultats Dans l’échantillon d’apprentissage, la médiane de suivi était de 708 jours (0 à 7,25 ans) avec 232 pertes de greffon ou de décès. Dans le sous-groupe MP (n = 508), l’âge moyen des receveurs et des donneurs était respectivement de 61 et 65 ans contre 52 et 53 ans dans le groupe CS (p < 0,001). Les greffons du groupe MP étaient issus de patients plus hypertendus, dyslipidémiques et diabétiques (p < 0,001). Globalement, nous n’avons pas pu montrer un effet significatif de la MP sur la survie patient-greffon (HR = 0,98 IC 95 % 0,69–1,39, p = 0,91), mais les greffons de donneurs âgés de plus de 70 ans présentaient une survie significativement supérieure lorsqu’ils sont conservés par MP (HR = 0,56 IC 95 % 0,33–0,95, p = 0,03). Néanmoins, cette interaction n’était pas confirmée sur l’échantillon de validation (HR = 0,93, IC 95 % 0,47–1,83, p = 0,83). Discussion La MP a montré un effet bénéfique dans la prévention du retard de fonctionnement du greffon avec une augmentation significative de la survie du greffon dans une grande étude randomisée [1]. Nos résultats suggèrent que, dans une utilisation de routine, la MP ne modifie pas le pronostic à moyen terme sur la survie greffon-patient. Nous n’avons pas pu montrer un effet plus prononcé de la MP pour un profil de donneur particulier. Conclusion Nous n’avons pas pu mettre en évidence de différence de survie par l’emploi des MP ni identifier de profil de donneur pouvant en bénéficier. [ABSTRACT FROM AUTHOR]

Introduction La greffe rénale à partir d’un donneur vivant est aujourd’hui le meilleur traitement de l’IRC terminale en termes d’espérance et de qualité de vie. Elle peut si elle est programmée avant le stade terminal de l’IRC être réalisée de façon préemptive. L’objectif principal de cette étude était de comparer dans une population de patients ayant reçu une greffe rénale à partir d’un donneur vivant (DV), la qualité de vie et la réinsertion professionnelle des receveurs greffés de façon préemptive (GP) à ceux greffés après une période de dialyse (GD). Patients et méthodes Il s’agit d’une étude transversale par questionnaire auto-administré (SF-36 et RE-TRANS-QoL) envoyé en mars 2014 au domicile des patients greffés avec DV entre le 01/04/2004 et le 31/12/2011 dans 7 centres français et vivant avec un greffon fonctionnel au 31/03/12. Les renseignements recueillis avant l’IRCT, pendant la période de dialyse (groupe GD), au cours de la 1 re année de greffe et au moment du remplissage, ont permis de reconstituer le parcours socioprofessionnel des patients au cours du temps. Résultats Trois cent cinquante-cinq des 534 patients inclus ont renvoyé le questionnaire. Les principales caractéristiques (sexe, âge à la greffe, niveau d’études) étaient similaires dans les 2 groupes GP et GD. Les évolutions familiales et sociales ne différaient pas entre les groupes contrairement à l’évolution professionnelle. Les patients GP ont gardé plus souvent une situation professionnelle stable (81,6 % vs 10,7 %), ont eu moins souvent une perte d’activité professionnelle (12,6 % vs 42,7 %) et un arrêt maladie de longue durée (19,2 % vs 29,8 %). Les facteurs associés à la QV dans les 2 dimensions physique (PCS) et mentale (MCS), après ajustement, étaient l’âge à la greffe, le niveau d’études, l’indice de comorbidité de Groll et certains effets secondaires. Le temps passé en dialyse a un effet délétère sur les dimensions PCS ( p < 0,0001) et MCS ( p = 0,0008). Discussion Chez les patients greffés avec DV, la dialyse avant greffe altère significativement le parcours professionnel et le temps passé en dialyse impacte la QV dans ses 2 dimensions.
