Subcapsular hematoma due to endoscopic retrograde cholangiopancreatography is a rare complication. There are only a few case reports in this subject in literature. This reports aim is to present subcapsular hematoma due to endoscopic retrograde cholangiopancreatography case and compare it with the literature. A sixty-nine-year-old female patient was admitted to the clinic with abdominal pain and jaundice. In physical examination epigastric tenderness was present. Patient went under ERCP procedure due to choledocholithiasis. 1.5 cm of filling defect was seen in hilar region in ERCP. The gallstone was extracted with endoscopic sphincterotomy, balloon and basket. 12 hours after being discharged, patient admitted to the emergency service presenting low blood pressure, fatigue and abdominal pain. Arterial blood pressure and heart rate was respectively 90/60 mmHg and 100 bpm. Abdominal examination presented tenderness, guarding and rebound in all quadrants. Ultrasonography and general abdominal computed tomography showed subcapsular liver hematoma and subdiaphragmatic free air. The patient was operated due to acute abdomen. During exploration, laceration and subcapsular hematoma was present on the anterior face of the right lobe. Lacerated area was sutured simply. Postoperative day 9 patient was discharged with no complications. Patient was admitted presenting fever and abdominal pain postoperative month 1. USG and CT revealed that an approximately 10x3 cm heterogenic collection adjacent to the left lobe of the liver was reaching out to epigastrium. Percutaneous drainage was performed. Drainage was continued for 1 week. Control USG revealed regression and drainage was terminated. No complications were observed in patients 1 year of follow-up. Subcapsular hematoma of the liver is a rare complication. It should be considered in case of sudden hypotension and abdominal pain after ERCP. Conservative treatment should be considered in the foreground, however, surgery should be scheduled in case of acute abdomen and/or hemodynamic instability. [Med-Science 2016; 5(4.000): 1046-8]

Amaç: Abdominal duvar hernilerinin tedavisinde temel prensip gerilimsiz onarımdır. Polipropilen mesh nedeniyle oluşan adezyon riskini azaltmak için antiadeziv bariyerli meshler kullanıma girdi. Bu çalışmanın amacı, omega-3 yağ asidi kaplı polipropilen mesh kullanılarak ameliyat edilen hastaların postoperatif takip sonuçlarının analiz edilmesi idi.Gereç ve Yöntem: Bu çalışma, kasım 2010 – temmuz 2015 yılları arasında omega-3 yağ asidi kaplı polipropilen mesh kullanılarak opere edilen hastaların, digital veri bankasında taranması ile prospektif olarak yapıldı. Uygulanan ameliyat tekniğine göre hastalar iki gruba ayrıldı (grub 1: laparoskopi, grub 2: açık cerrahi). Mesh olarak omega-3 yağ asidi kaplı polipropilen mesh kullanıldı (C-QURTM Mesh, Atrium Medical Corporation, Hudson, NH, USA). Postoperatif komplikasyonlar irdelendi.Bulgular: Çalışmaya 30 hasta dahil edildi. Erkek- kadın oranı eşit olup yaş ortalaması 56.2±14.5 (aralık: 17-83) idi. Yirmi iki hastada (%73) primer sütur ile onarılamayan abdominal duvar hernisi (grub 2), 8 inde (%27) laparoskopik onarılan abdominal duvar hernisi (grub 1) vardı. Dokuz hastada (%30) komplikasyon gelişti. Yaş, cinsiyet ve uygulanan cerrahi tekniğin komplikasyon gelişimine etkisi istatistiksel olarak anlamlı bulunmadı. Komplikasyon gelişen hastalarda defekt boyutu istatistiksel olarak anlamlı derecede yüksekti.Sonuç: Antiadeziv bariyerli meshlerin uzun dönem sonuçları bilinmemektedir. Bu tip meshlerin de komplikasyon, sağlık problemi ve hatta ölüme bile neden olma potansiyeli olduğu unutulmamalıdır.

Amaç: Akut süperior mezenterik ven trombozu, tüm mezenterik iskemik olayların yaklaşık % 5 -15’ini oluşturan nadir görülen bir intestinal iskemi nedenidir. Bu yazıda süperior mezenterik ven trombozu gelişen kalıtsal trombofilili 2 hastanın sunulması amaçlanmıştır. Olgu sunumu: Otuz bir ve 51 yaşında iki hastaya akut batın, mezenter iskemi nedeni ile laparotomi ve ince barsak rezeksiyonu yapıldı. Genetik incelemede her iki hastada homozigot MTHFR C677-T mutasyonu saptandı. Ameliyat sonrası dönem sorunsuz geçen hastalar 10. ve 12. günde taburcu edildi. Sonuç: Akut süperior mezenterik ven trombozu mortalitesi yüksek olan ciddi bir sağlık problemidir. Doğru tanı koyabilmek için hastalıktan şüphelenmek oldukça önemlidir. Nekroz varlığında acil eksplorasyon ve nekrotik dokuların rezeksiyonu gerekmektedir. Trombofilili hastalar tanınmalı ve uzun dönem antikoagülan kullanımı planı yapılmalıdır

Retained surgical sponge in the abdomen following abdominal and pelvic surgery is an uncommon condition. Here we present a case of retained surgical sponge with unusual presenting symptoms. A 27-year old female patient presented to our department with a foreign body localized in the anal region. She had a past history of a myomectomy 1 year earlier. Clinical examination and radiographic workout revealaed a sponge migrating towards the rectum. The sponge was removed under visual guidance of rectoscopy without laparotomy. The patient was discharged without complications.
