Abstract Systemic lupus erythematosus (SLE) is an autoimmune and multisystem disease with a high public health impact. Lupus nephritis (LN), commonly known as renal involvement in SLE, is associated with a poorer prognosis and increased rates of morbidity and mortality in patients with SLE. Identifying new urinary biomarkers that can be used for LN prognosis or diagnosis is essential and is part of current active research. In this study, we applied an untargeted metabolomics approach involving liquid and gas chromatography coupled with mass spectrometry to urine samples collected from 17 individuals with SLE and no kidney damage, 23 individuals with LN, and 10 clinically healthy controls (HCs) to identify differential metabolic profiles for SLE and LN. The data analysis revealed a differentially abundant metabolite expression profile for each study group, and those metabolites may act as potential differential biomarkers of SLE and LN. The differential metabolic pathways found between the LN and SLE patients with no kidney involvement included primary bile acid biosynthesis, branched-chain amino acid synthesis and degradation, pantothenate and coenzyme A biosynthesis, lysine degradation, and tryptophan metabolism. Receiver operating characteristic curve analysis revealed that monopalmitin, glycolic acid, and glutamic acid allowed for the differentiation of individuals with SLE and no kidney involvement and individuals with LN considering high confidence levels. While the results offer promise, it is important to recognize the significant influence of medications and other external factors on metabolomics studies. This impact has the potential to obscure differences in metabolic profiles, presenting a considerable challenge in the identification of disease biomarkers. Therefore, experimental validation should be conducted with a larger sample size to explore the diagnostic potential of the metabolites found as well as to examine how treatment and disease activity influence the identified chemical compounds. This will be crucial for refining the accuracy and effectiveness of using urine metabolomics for diagnosing and monitoring lupus and lupus nephritis.

Contexto: el síndrome metabólico (SM) se define como un conjunto de factores de riesgo que aumentan la probabilidad de padecer enfermedad cardiovascular. El síndrome poscovid es una complicación a largo plazo de la infección por COVID-19, definida por la Organización Mundial de la Salud (OMS) como la perpetuación de los síntomas de la enfermedad aguda más allá de 12 semanas sin otra causa aparente. La asociación entre las enfermedades cardiometabólicas y las complicaciones de la infección aguda por SARS-CoV-2 ha estado clara desde el principio, no siendo así con el síndrome poscovid. Objetivo: determinar la asociación existente entre la presencia de enfermedades cardiovasculares en el momento de la infección aguda por SARS-CoV-2 y el desarrollo de síndrome poscovid. Metodología: estudio observacional de cohortes y retrospectivo, con un total de 278 pacientes, 140 en la cohorte expuesta y 138 en la cohorte no expuesta, para establecer la asociación entre la presencia de enfermedades cardiovasculares en la fase aguda de infección por COVID-19 y el síndrome poscovid. Resultados: en la cohorte expuesta, el 74,1?% presentó el síndrome poscovid, mientras que en la cohorte no expuesta el 25,9?% presentó dicho desenlace. Luego del análisis multivariado, se estableció una asociación estadísticamente significativa entre el diagnóstico de síndrome metabólico en la fase aguda de la infección por COVID-19 y el desarrollo de síndrome poscovid, con un RRa de 3,18, IC 95?% 1,42-7,14; p = 0,005. Conclusiones: se logró establecer una asociación clara entre la presencia del síndrome metabólico en la fase de infección aguda por COVID-19 y la aparición de síndrome poscovid.

Lupus nephritis represents a significant immune-mediated glomerulonephritis, constituting the most important organ involvement induced by systemic lupus erythematosus (SLE), with variable epidemiology and clinical presentation among populations. Objective: to identify clinical and immunological factors associated with the progression of lupus nephritis in a population from the Colombian Caribbean. Methods: we evaluated 401 patients diagnosed with SLE and lupus nephritis, treated at a reference center in the Colombian Caribbean, gathering data recorded in medical records. Results: A proportion of 87% were female, with a median age of 42 years. Most patients presented with proliferative classes (90%), with class IV being the most common (70%). A proportion of 52% of patients did not respond to treatment, which is described as the lack of complete or partial response, while 28% had a complete response. A significant decrease in hemoglobin, glomerular filtration rate, and proteinuria was identified by the third follow-up (p < 0.001), along with an increase in creatinine, urea, and hematuria (p < 0.001). Patients with initial proteinuria > 2 g/day were found to be 27 times more likely to be non-responders (p < 0.001). Mortality was associated with the presence of serum creatinine >1.5 mg/dL (p = 0.01) (OR: 1.61 CI 95% 0.75–3.75) and thrombocytopenia (p = 0.01) (OR: 0.36; CI 95% 0.12–0.81). Conclusions: identifying factors of progression, non-response, and mortality provides an opportunity for more targeted and personalized intervention, thereby improving care and outcomes for patients with lupus nephritis.

Objectives. Atypical hemolytic uremic syndrome (aHUS) is a rare complement-mediated kidney disease with genetic predisposition and represents up to 10% of pediatric hemolytic uremic syndrome (HUS) cases. Few studies have evaluated aHUS in Latin American population. We studied a Colombian pediatric cohort to delineate disease presentation and outcomes. Methods. A multicenter cohort of 27 Colombian children with aHUS were included. Patients were grouped by age at onset. Clinical features were compared using analysis of variance (ANOVA) and Fisher exact tests. Renal biopsy was performed on 6 patients who were suspected of having other renal diseases before aHUS diagnosis. Results. Most patients were male (70%). The onset of aHUS occurred frequently before age 4 years (60%) and followed gastroenteritis as the main triggering event (52%). Age groups showed comparable clinical presentation, disease severity, treatment, and outcomes. Pulmonary involvement (67%) was the main extrarenal manifestation, particularly in the 1 to 7 age group ( P = .01). Renal biopsies were as follows: 3 had membranoproliferative glomerulonephritis (MPGN) type I, one MPGN type III, one C3-glomerulonephritis, and one rapidly progressive GN. Genetic screening was available in 6 patients and identified 2x CFHR5 , 2xMCP , 1x ADAMTS13/THBD , and 1x DGKE mutations. A total of 15 relapses were seen, of which 8 (72%) occurred in the 1 to 7 age group. The renal outcome was not significantly different regardless of age group. Conclusion. In our cohort, we observed a relatively high frequency of extrarenal involvement at first presentation represented by pulmonary manifestations. The renal prognosis at initial presentation was worse than in previous reports.

Introduction: Mesoamerican nephropathy is a tubule-interstitial nephropathy whose etiology is still unknown. However, clinical cases like Mesoamerican nephropathy have been described in other geographically distant and ethnically diverse regions. Still, they all have a common factor: the intensity of heat and rural physical labor. Objective: To study whether this entity could occur among rural workers in a non-Mesoamerican region with similar climatic and working conditions, in the Colombian Caribbean countryside, and to consider how much repetitive dehydration could weigh in its pathogenesis. Methodology: An observational study was carried out, based on field work in a farm in Sitio Nuevo (Colombia) with 28 rural worker volunteers (rice fields), who were measured for weight, blood pressure, and blood and urine samples to measure electrolytes and osmolarity, at 2 times of the day (morning and evening). Insulin is a hormone that is composed of 51 amino acids and structurally organized as a hexamer comprising three heterodimers. Insulin is the central hormone involved in the control of glucose and lipid metabolism, aiding in processes such as body homeostasis and cell growth. Insulin is synthesized as a large preprohormone and has a leader sequence or signal peptide that appears to be responsible for transport to the endoplasmic reticulum membranes. The interaction of insulin with the kidneys is a dynamic and multicenter process, as it acts in multiple sites throughout the nephron. Insulin acts on a range of tissues, from the glomerulus to the renal tubule, by modulating different functions such as glomerular filtration, gluconeogenesis, natriuresis, glucose uptake, regulation of ion transport, and the prevention of apoptosis. On the other hand, there is sufficient evidence showing the insulin receptor’s involvement in renal functions and its responsibility for the regulation of glucose homeostasis, which enables us to understand its contribution to the insulin resistance phenomenon and its association with the progression of diabetic kidney disease.

Our study aimed to describe the glomerular diseases, both primary glomerular disease (PGD) and secondary glomerular disease (SGD) in the Colombian Caribbean based on the first regional Colombian Nephropathy Registry (NEFRORED®). A descriptive and retrospective study of adult patients with glomerular diseases from the Colombian Caribbean region was made. All diagnoses by renal biopsy with light microscopy and immunofluorescence obtained between January 2008 and June 2018 were recorded. Eight hundred and seventy-one renal biopsies were obtained. The main clinical indication for biopsy was nephritic syndrome (36%). SGD was more frequent than PGD (55% vs. 45%). Within SGD group, lupus nephritis (LN) was the most frequent etiology (83%). Within PGD group, membranous nephropathy (33%) and focal segmental glomerulosclerosis (FSGS) (19%) were the most common glomerular diseases. At a 24-month follow-up, the patients with FSGS and paraproteinemia-mediated glomerular disease had the worst renal survival prognosis. This is the first Colombian Nephropathy Registry in a Caribbean population, demonstrating a high predominance of SGD due to LN.

Introducción: el síndrome respiratorio agudo severo inducido por coronavirus-2 (SARS-CoV-2) se caracteriza por la instalación de una afectación pulmonar. Sin embargo, existen reportes de afectación renal concomitante, con manifestaciones clínicas como hematuria, proteinuria y daño renal agudo. Objetivo: presentamos dos casos clínicos de síndrome nefrótico asociado a enfermedad por coronavirus (COVID-19). Presentación del caso: un paciente presentó evidencia histológica de cambios mínimos y fusión de pedicelos de podocitos en muestras histológicas evaluadas por microscopía electrónica. Discusión: la proteinuria en COVID-19 puede ser secundaria a lesiones glomerulares y tubulares agudas, con un origen multifactorial: factores hemodinámicos, tormentas de citocinas, infecciones secundarias, nefrotoxicidad inducida por fármacos e infección viral directa de las células del túbulo proximal y podocitos. Esta última podría deberse a una infección directa por el virus debido al mayor tropismo renal de este. Conclusión: el presente informe presenta dos casos de síndrome nefrótico secundario a podocitopatía en pacientes con infección aguda por COVID-19.

Objetivo: Determinar los factores clínicos, histopatológicos y sociodemográficos relacionados con el patrón morfológico de glomeruloesclerosis focal y segmentaria (GEFS). Método: Estudio observacional descriptivo de corte transversal. Se realizó una búsqueda de reportes de biopsia compatibles con el patrón de GEFS por microscopía óptica o electrónica en el periodo 2008-2022 en el archivo de patología. Los datos de antecedentes y presentación clínica, tipo de manejo, paraclínicos, variante histopatológica, patrón de depósito por inmunohistoquímica y factores sociodemográficos se tomaron de las historias médicas. Resultados: Se encontraron 69 registros de pacientes con GEFS en el periodo de estudio, que fueron previamente revisados por un nefropatólogo clínico. El 55.1% fueron de sexo femenino, con una mediana de edad de 37 años. El antecedente de hipertensión arterial fue el más frecuente (53%) y la principal forma de presentación clínica fue proteinuria en rango nefrótico (63%). Con respecto al manejo, el tratamiento con glucocorticoides fue ampliamente utilizado (78%); sin embargo, el 27.8% de los pacientes presentaron corticorresistencia. La variante histopatológica clásica fue reportada en un 87%. Se encontró una asociación estadísticamente significativa entre la presentación clínica de la GEFS y su respuesta al tratamiento, donde los casos de presentación como síndrome nefrótico franco tuvieron una mayor proporción de corticorresistencia, en contraste con presentaciones menos habituales como proteinuria en rango no nefrótico o hematuria, donde la remisión parcial o total fue más frecuente (p < 0.05). Conclusiones: La distribución global de las características clínicas y sociodemográficas, como la edad y el sexo, fue muy similar a la reportada en la literatura. Sin embargo, no se encontró asociación entre la variante histopatológica de presentación y la respuesta al tratamiento con corticoides y el manejo antiproteinúrico, posiblemente sujeto a la representatividad de cada variante en el estudio, la baja proporción de casos de GEFS secundaria y la alta proporción de población mestiza (no caucásico, no afrodescendiente). No obstante, es el estudio descriptivo sobre GEFS más grande reportado en la literatura en Colombia, que constituye el punto de partida de una línea de investigación, propia de la región Caribe colombiana, en podocitopatías.

This paper describes the main characteristics of coronavirus diseases 2019 (COVID-19) patients suffering from acute kidney injury (AKI) assisted at a high complexity clinic in Barranquilla, Colombia. The patients included in this study (n = 48) were those with a positive diagnosis of COVID-19 confirmed by polymerase chain reaction detection of severe acute respiratory syndrome coronavirus 2, who had developed AKI during their hospital stay. Serum and urine parameters, as well as patient’s viral load and clinical frailty scale (CFS) were recorded. A statistical analysis of the recorded parameters, such as comparisons, and correlations between variables of interest, were explored. The prevalence of COVID-19 induced AKI was 41%, being the majority of them classified as AKI network classification 3, with a renal replacement therapy requirement of 29%, and an associated mortality of 73%. AKI patients’ mortality showed a significant positive correlation (33%) with patients’ CFS score but not with their viral load. COVID-19 induced AKI significantly correlated with patients’ frailty status but not to their viral load.

Introducción: La enfermedad renal crónica (ERC) es un importante problema de salud pública en todo el mundo. Según el Atlas Mundial de Salud Renal, la prevalencia estimada de ERC en Colombia para el año 2021 fue del 10.7%. Objetivo: Describir la situación de la ERC en Colombia. Metodología: Estudio descriptivo, retrospectivo, que analiza los datos disponibles sobre la ERC en Colombia. Resultados: Para junio de 2021 se reportaron 5,347,829 personas con ERC y sus precursoras, diabetes mellitus (DM) e hipertensión arterial (HTA). La prevalencia cruda de ERC fue del 1.7%; la incidencia de ERC fue de 3.05 casos nuevos por 1,000 habitantes. Hay un importante subregistro de ERC. Se informaron 37,751 casos fallecidos con HTA o DM con ERC concomitante. Se reportaron 42,712 pacientes en terapia de reemplazo renal, el 58.3% en hemodiálisis. Además, se reportaron 7,816 pacientes con trasplante renal. Del total de recursos dispuesto para enfermedades de alto costo, la cohorte de ERC y precursoras consume cerca del 48%. Conclusiones: La ERC en Colombia constituye una de las principales causas de morbilidad y mortalidad, sobre todo en grupos con comorbilidades como DM e HTA.

Abstract Introduction: This article describes the main differences between COVID-19-induced acute kidney injury (AKI-COVID19) in patients with previous normal renal function (AKI-NRF) and those with chronic kidney disease (AKI-CKD) treated in a high complexity clinic in Barranquilla (Colombia). Material and Methods: The patients included in this study (n: 572) were those with a positive diagnosis of COVID-19 confirmed by detection of a positive PCR for SARS-CoV-2. Of these patients, 188 developed AKI during their hospital stay. Patients’ epidemiological data, serum parameters, and clinical frailty status were recorded. Statistical analysis and comparison among AKI-NRF, AKI-CKD, and non-AKI patients were performed. Results: The incidence of COVID-19-induced AKI was 33%, with the majority classified as AKIN 1, 16% requiring renal replacement therapy, and AKI-COVID19 mortality of 68%. A significantly higher prevalence of hypertension, cardiac disease, and serum reactive C-protein and lower albumin values in AKI-CKD patients was recorded. Mortality rate, invasive ventilation requirement, and D-dimer levels were significantly higher in AKI-NRF patients: Conclusion: Different clinical patterns between AKI-NRF and AKI-CKD were documented.

Diabetic kidney disease (DKD) is a highly prevalent condition worldwide. It represents one of the most common complications arising from diabetes mellitus (DM) and is the leading cause of end-stage kidney disease (ESKD). Its development involves three fundamental components: the hemodynamic, metabolic, and inflammatory axes. Clinically, persistent albuminuria in association with a progressive decline in glomerular filtration rate (GFR) defines this disease. However, as these alterations are not specific to DKD, there is a need to discuss novel biomarkers arising from its pathogenesis which may aid in the diagnosis, follow-up, therapeutic response, and prognosis of the disease.

Background: Renal biopsy is the gold standard for the diagnosis and classification of lupus nephritis (LN). However, a consecutive biopsy can predict the clinical course and optimize the therapeutic strategies. Objectives: To compare the histopathological findings with clinical responses. Patients and Methods: Thirty patients with active LN were included. Renal biopsies were performed at the time of diagnosis and subsequently under clinical criteria according to consensus of Spanish Society of Nephrology. The response to treatment was defined as complete response, partial responder or non-responder. The histological change in second biopsy towards LN classes I, II or III/IV-C was defined as histological response (HR). Results: In initial renal biopsy, 28 (93%) patients showed proliferative LN; III-A or A/C (n; 7), IV-A or A/C (n: 19) and mixed; III+IV/V (n; 2). LN class V was presented in two cases. The clinical response was; complete response (10%), partial response (20%), and non-response (70%). HR was manifested in 37% and non-histologic response in 63% of patients. Around 33% of patients with complete response/partial response showed active lesions in the consecutive renal biopsy. Conclusions: In Colombian Caribbean, LN is aggressive and refractory to treatment. The consecutive renal biopsy allowed to demonstrate the persistence of the activity of the lesion in almost half of the patients, which may provide additional information to create better response criteria. The consecutive renal biopsy is a tool that allows improving the evaluation of the response to treatment in the LN.

Introducción. La glomerulonefritis membranoproliferativa (GnMP) es un patrón de lesión glomerular hipercelular mesangial con adelgazamiento de la membrana basal glomerular y proliferación endocapilar que está mediado por las inmunoglobulinas o el sistema del complemento en el mesangio y endotelio capilar. Objetivo. Evaluar la respuesta a la farmacoterapia en pacientes diagnosticados con GnMP en una clínica de Barranquilla entre los años 2007 y 2014. Materiales y métodos. Estudio de cohorte retrospectivo en el que se evaluaron 58 pacientes con diagnóstico de GnMP por biopsia renal, quienes se clasificaron como respondedores y no respondedores. Se realizó una evaluación de tratamiento estándar según tipo de GnMP: mediado por complemento y mediado por inmunocomplejos e inmunofluorescencia negativa a los 6 y 12 meses de tratamiento. Resultados. La edad promedio de los participantes fue de 35±13 años. De 58 pacientes, 52% eran mujeres, 63% desarrolló enfermedad renal crónica (ERC) al año de evaluación, 25.8% logró remisión (22.4% completa y 3.4% parcial) y 74.2% no logró entrar en remisión. Conclusión. La GnMP es una causa importante de ERC entre la población estudiada. La respuesta al tratamiento inmunosupresor no demostró beneficios estadísticamente significativos, independiente del tipo de GnMP.

The review is based on the contribution that each discipline should provide the patient for a holistic care, which include medical assessment, monitoring and counselling as emotional support, assessment and nutritional monitoring as a key element in core requirements, physical activity that optimize the quality of life, social activities that can enter the individual in active groups, follow-up by nurses to the fulfillment of the ordered drug treatment, car care and orientation education to the family. The novelty of this proposal is to basically carry out care of the interdisciplinary team for treatment adherence. This review concluded that patients with lupus nephritis (NL) treated after assessment and follow-up holistic, such as system monitoring and adherence to the treatment of comprehensive care, provides better quality of life, and minimizes the risks of complication of the patient, avoiding recurrent hospitalizations.

Objectives: To determine the behavior of lupus nephropathy (LN) in a population group of a reference center in the Colombian Caribbean during 2012 and 2013, analyzing the most prevalent clinical and laboratory findings and its relation to the immunohistotopathological findings, define the class of Lupus nephritis with greater prevalence and the clinical or paraclinic data more correlated with this one. Methods: Descriptive, retrospective, cross-sectional study conducted from January 1st of 2012 to December 31st 2013, which included 53 patients with systemic lupus erythematosus according to diagnostic critearia of the Colegio Americano de Reumatologia, with manifestations of renal impairment. Results: In our study population in the Colombian Caribbean, LN predominates more in women and class IV represents 66.03% of the total cases analyzed, in which there was more hematuria present, hypertension and proteinuria with nephrotic pattern, as well as higher levels of serum creatinine. Despite the lack of data, none of the clinical or paraclinical variables was specific for a certain class of LN and in relation to the immunostaining, lambda chains were positive in 100% of the cases and only C4 and fibrinogen were specific to LN class IV. Conclusions: Class IV LN is the most prevalent and with a higher percentage of predominance of the variables hematuria, proteinuria, hypertension, elevated serum creatinine and bun levels. At the same time they are not specific for some class of LN in our population.

Lupus nephritis is one of the most severe manifestations of systemic lupus erythematous. Renal involvement in patients with systemic lupus erythematous is an important cause of morbidity and mortality. The pathogenesis of lupus nephritis involve multiple factors, wich include genetic predisposition, epigenetic regulation and environmental interaction. Conventional clinical parameters such as creatinine clearance, proteinuria, urinary sediments, antibodies anti-double-strand DNA and complement proteins they are not enough sensitive or specific to detect disease activity. In the last decades, “Omics” technologies (Proteomic, genomic, transcriptomic, metobolomic) have been used in an extensive way looking for biomarkers, which allowed to discovery variants associated with systemic lupus erythematous and lupus nephritis. Such findings have expanded our knowledge about molecular basis of disease and they have been very important to identification of potential therapeutic targets to prediction of disease and early treatment. In this review, we resume some of recent studies focused in identification of biomarkers associated with lupus nephritis in diverse biological fluids.

Rationale & Objective: The risk implications of the Kidney Disease: Improving Global Outcomes (KDIGO) chronic kidney disease classification in older adults are controversial. We evaluated the risk of adverse outcomes in this population across categories of estimated glomerular filtration rate (eGFR) and urinary albumin-creatinine ratio (UACR). Study Design: Prospective cohort. Settings & Participants: In total, 2,509 participants aged ≥75 years in the Systolic Blood Pressure Intervention Trial (SPRINT). Exposure: KDIGO eGFR and UACR categories. We combined KDIGO categories G1 and G2, G3b and G4, as well as A2 and A3. Outcomes: Primary SPRINT outcome (composite of myocardial infarction, other acute coronary syndromes, stroke, heart failure, or death from cardiovascular causes), and all-cause death. Analytical Approach: Multivariable Cox proportional hazard models. Results: Mean age was 79.8 years, and 37.4% were female. The mean eGFR was 64.0 mL/min/1.73 m2, and the median UACR was 13.1 mg/g. In multivariable Cox proportional hazard analysis, compared with participants with eGFR ≥ 60 mL/min/1.73 m2 and UACR 1 g/day were excluded from SPRINT. Conclusion: Among older adults SPRINT participants, low eGFR without albuminuria was associated with higher mortality but not with increased risk of cardiovascular events. Additional studies are needed to evaluate an adapted chronic kidney disease stage-based risk stratification for older adults. Plain-Language Summary: Using data from participants in the SPRINT trial, we evaluated the association of chronic kidney disease stage with adverse clinical outcomes among adults older than 75 years without diabetes. We found that low level of kidney function determined by a low estimated glomerular filtration rate with moderately or severely increased urine albumin excretion was associated with increased risk for cardiovascular events and all-cause mortality. However, low estimated glomerular filtration rate with normal or mildly increased urinary albumin excretion was not consistently associated with these adverse outcomes. This finding supports the need for additional studies to evaluate an age-adapted classification of chronic kidney disease to improve risk stratification among older adults.

Abstract Background Adiponectin and leptin are pivotal in the regulation of metabolism. Pediatric lupus nephritis (pLN), a manifestation of childhood systemic lupus erythematosus (SLE) affecting the kidneys, is associated with impaired adipokine levels, suggesting a role in pLN pathogenesis. The aim of this study was to explore the potential relationship between specific single-nucleotide polymorphisms (SNPs)—methylenetetrahydrofolate reductase (MTHFR) rs1801131 and fibrinogen gamma chain (FGG) rs2066865—and the serum levels of leptin and adiponectin in patients with pLN. Methods Ninety-eight pLN patients and one hundred controls were enrolled in the study. Serum leptin and adiponectin levels were measured using ELISA. DNA extraction and real-time PCR genotyping were performed for MTHFR rs1801131 and FGG rs2066865 SNPs. Results Compared to healthy controls, pLN patients exhibited significantly greater serum leptin (11.3 vs. 18.2 ng/mL, p 0.05). However, the AG genotype of FGG gene rs2066865 SNP was significantly associated with high leptin levels (> 15 ng/mL) (p = 0.01). Conclusion Serum adiponectin and leptin levels are associated with pathological manifestations of pLN. High leptin levels are associated with the AG genotype of FGG rs2066865 SNP in pLN patients, suggesting direct involvement in disease progression and potential utility as a disease biomarker.

Abstract Arterial hypertension (AH) is a multifactorial and asymptomatic disease that affects vital organs such as the kidneys and heart. Considering its prevalence and the associated severe health repercussions, hypertension has become a disease of great relevance for public health across the globe. Conventionally, the classification of an individual as hypertensive or non-hypertensive is conducted through ambulatory blood pressure monitoring over a 24-h period. Although this method provides a reliable diagnosis, it has notable limitations, such as additional costs, intolerance experienced by some patients, and interferences derived from physical activities. Moreover, some patients with significant renal impairment may not present proteinuria. Accordingly, alternative methodologies are applied for the classification of individuals as hypertensive or non-hypertensive, such as the detection of metabolites in urine samples through liquid chromatography or mass spectrometry. However, the high cost of these techniques limits their applicability for clinical use. Consequently, an alternative methodology was developed for the detection of molecular patterns in urine collected from hypertension patients. This study generated a direct discrimination model for hypertensive and non-hypertensive individuals through the amplification of Raman signals in urine samples based on gold nanoparticles and supported by chemometric techniques such as partial least squares-discriminant analysis (PLS-DA). Specifically, 162 patient urine samples were used to create a PLS-DA model. These samples included 87 urine samples from patients diagnosed with hypertension and 75 samples from non-hypertensive volunteers. In the AH group, 35 patients were diagnosed with kidney damage and were further classified into a subgroup termed (RAH). The PLS-DA model with 4 latent variables (LV) was used to classify the hypertensive patients with external validation prediction (P) sensitivity of 86.4%, P specificity of 77.8%, and P accuracy of 82.5%. This study demonstrates the ability of surface-enhanced Raman spectroscopy to differentiate between hypertensive and non-hypertensive patients through urine samples, representing a significant advance in the detection and management of AH. Additionally, the same model was then used to discriminate only patients diagnosed with renal damage and controls with a P sensitivity of 100%, P specificity of 77.8%, and P accuracy of 82.5%.

Pregnancy-associated atypical hemolytic uremic syndrome (P-aHUS) is a rare condition characterized by microangiopathic hemolytic anemia and kidney injury from thrombotic microangiopathy. P-aHUS occurs in approximately 1 in 25,000 pregnancies and is strongly related to complement dysregulation and pregnancy-related disorders, such as preeclampsia, eclampsia, and hemolysis, elevated liver enzymes, low platelet (HELLP) syndrome, resulting in adverse perinatal and fetal outcomes. Complement dysregulation in P-aHUS is commonly attributed to genetic mutations or autoantibodies affecting complement factors, including CFH, CFI, and MCP. We present a case of a 25-year-old primigravida who experienced severe preeclampsia and HELLP syndrome followed by the development of complicated P-aHUS during the early postpartum period. The patient exhibited severe clinical manifestations, including hypertensive emergency, central nervous system involvement, renal impairment, and microangiopathic hemolytic anemia. Timely initiation of eculizumab therapy resulted in successful disease remission. Further genetic analysis revealed a likely rare pathogenic MCP gene variant.

Abstract Isometric handgrip or (wall) squat exercise performed three times per week produces reductions in systolic blood pressure (SBP) in adults with hypertension. We aimed to compare these interventions and the potential to retain benefits with one exercise session per week. We compared blood pressure changes following handgrip and squat isometric training interventions with controls in a randomized controlled multicentre trial in 77 unmedicated hypertensive (SBP ≥ 130 mmHg) adults. Exercise sessions were performed in the workplace and consisted of four repetitions—three sessions per week for the first 12 weeks (phase 1), and one session per week for the subsequent 12 weeks (phase 2). Office blood pressure (BP) was measured at baseline, post‐phase 1 and post‐phase 2. Post‐phase 1, mean reductions in SBP were significantly greater in handgrip (–11.2 mmHg, n = 28) and squat (–12.9 mmHg, n = 27) groups than in controls (–.4 mmHg; n = 22) but changes in DBP were not. There were no significant within‐group changes during phase 2 but SBP was 3.8 mmHg lower in the wall squat than the handgrip group—a small magnitude but clinically important difference. While both interventions produced significant SBP reductions, the wall squat appears to be more effective in maintaining benefits with a minimal training dose. The low time investment to achieve and retain clinically significant SBP reductions—42 and 12 min, respectively—and minimal cost, particularly of the wall squat, make it a promising intervention for delivery in public health settings.

Background: Metabolic syndrome (MetS) is a disorder associated with an increased risk for the development of diabetes mellitus and its complications. Lower isometric handgrip strength (HGS) is associated with an increased risk of cardiometabolic diseases. However, the association between HGS and arterial stiffness parameters, which are considered the predictors of morbidity and mortality in individuals with MetS, is not well defined. Objective: To determine the association between HGS and HGS asymmetry on components of vascular function in adults with MetS. Methods: We measured handgrip strength normalized to bodyweight (HGS/kg), HGS asymmetry, body composition, blood glucose, lipid profile, blood pressure, pulse wave velocity (PWV), reflection coefficient (RC), augmentation index @75 bpm (AIx@75) and peripheral vascular resistance (PVR) in 55 adults with a diagnosis of MetS between 25 and 54 years old. Results: Mean age was 43.1 ± 7.0 years, 56.3% were females. HGS/kg was negatively correlated with AIx@75 (r = −0.440), p < 0.05, but these associations were not significant after adjusting for age and sex. However, when interaction effects between sex, HGS/kg and age were examined, we observed an inverse relationship between HGS/kg and AIx@75 in the older adults in the sample, whereas in the younger adults, a weak direct association was found. Presentar una serie de casos de COVID-19 con requerimiento de ingreso a Unidad de CuidadosIntensivos. La información fue tomada de las historias clínicas, y su evaluación y diagnóstico fue realizado mediante estudios paraclínicos en sangre, orina, PCR e imágenes diagnósticas en 4 pacientes con diferentes comorbilidades y nexo epidemiológico presente para desarrollo de la enfermedad. Los cuatro casosfueron manejados con cloroquina 300 mg vía oral, cada 12 horas, y azitromicina 1 gr vía oral, cada 24 horas, durante 5 días,sin complicaciones ni toxicidad asociada. El caso 1 desarrolló falla orgánica múltiple, incluyendo injuria renal aguda con una estancia en UCI de 4 días antes de su fallecimiento, mientraslos casos 2, 3 y 4 tuvieron una evolución favorable y fueron dados de alta de UCI. Se requieren estudios multicéntricos rápidos que orienten científicamente hacia un mejor abordaje diagnóstico y manejo, en el contexto de una enfermedad con un comportamiento clínico-epidemiológico que debe estudiarse en profundidad y que probablemente cobrará muchas vidas; además, debido a la ausencia de pruebas diagnósticas rápidas, la utilización de una clasificación basada en la severidad de lesiones radiológicas llamada CO-RADS (Covid-19 Imaging Reporting and Data System) podría ser de gran importancia para instalar de manera temprana los tratamientos farmacológicos disponibles y la asistencia respiratoria mecánica precoz.

Renal involvement in Systemic Lupus Erythematous (SLE) patients is one of the leading causes of morbidity and a significant contributor to mortality. It's estimated that nearly 50% of SLE individuals develop kidney disease in the first year of the diagnosis. Class IV lupus nephritis (LN-IV) is the class of lupus nephritis most common in Colombian patients with SLE. Altered miRNAs expression levels have been reported in human autoimmune diseases including lupus. Variations in the expression pattern of peripheral blood circulating miRNAs specific for this class of lupus nephritis could be correlated with the pathophysiological status of this group of individuals. The aim of this study was to evaluate the relative abundance of circulating microRNAs in peripheral blood from Colombian patients with LN-IV. Circulating miRNAs in plasma of patients with diagnosis of LN-IV were compared with individuals without renal involvement (LNN group) and healthy individuals (CTL group). Total RNA was extracted from 10 ml of venous blood and subsequently sequenced using Illumina. The sequences were processed and these were analyzed using miRBase and Ensembl databases. Differential gene expression analysis was carried out with edgeR and functional analysis were done with DIANA-miRPath. Analysis was carried out using as variables of selection fold change (≥2 o ≤-2) and false discovery rate (0.05). We identified 24 circulating microRNAs with differential abundance between LN-IV and CTL groups, fourteen of these microRNAs are described for the first time to lupus nephritis (hsa-miR-589-3p, hsa-miR-1260b, hsa-miR-4511, hsa-miR-485-5p, hsa-miR-584-5p, hsa-miR-543, hsa-miR-153-3p, hsa-miR-6087, hsa-miR-3942-5p, hsa-miR-7977, hsa-miR-323b-3p, hsa-miR-4732-3p and hsa-miR-6741-3p). These changes in the abundance of miRNAs could be interpreted as alterations in the miRNAs-mRNA regulatory network in the pathogenesis of LN, preceding the clinical onset of the disease. The findings thus contribute to understanding the disease process and are likely to pave the way towards identifying disease biomarkers for early diagnosis of LN.

Abstract Systemic lupus erythematosus (SLE) is a multifactorial disease characterized by the convergence of genetic, immunological, and viral elements resulting in a complex interaction of both internal and external factors. The role of the Epstein-Barr virus (EBV) and human endogenous retroviruses (HERV-E) as triggers and maintenance elements in the pathogenesis of SLE has been widely recognized. Previous studies have independently evaluated the effects of EBV and HERV-E in this disease. In this work, for the first time, these viral factors are jointly investigated in SLE patients. This study aimed at assessing the differential expression of immune regulatory genes and the incidence of specific viral pathogens (EBV and HERV-E), alongside the detailed characterization of surface markers in T- and B-lymphocytes in patients with SLE and control participants. A comparative analysis between patients with SLE and control participants was performed, evaluating the expression of phenotypic markers and genes involved in the immune response (TNF-α, IL-2, IL-6, IL-10, IFNG, TLR3), as well as HERV-E gag and EBV viral genes (LMP1 and BZLF1).A significant association between SLE and EBV was found in this study. A notable increase in EBV LMP1 gene expression was observed in patients with SLE . Also, a significant overexpression of HERV-E was observed, in addition to a considerable increase in the distribution of the cell surface marker CD27 + on T- and B-lymphocytes, observed in individuals with SLE compared to the control group. This study provides evidence regarding the role that EBV virus plays in lymphocytes in the context of SLE, highlighting how both the virus and the host gene expression may influence disease pathogenesis by altering immune regulatory pathways mediated by TNF-α, IFN-γ, and IL-10, as well as parallel overexpression of HERV-E gag. The decrease in TLR3 could indicate a compromised antiviral response, which could facilitate viral reactivation and contribute to disease activity.

Introducción: el hiperparatiroidismo primario se caracteriza por hipercalcemia y niveles elevados de hormona paratiroidea y se puede manifestar como tres fenotipos clínicos: clásico, asintomático y normocalcémico. Objetivo: presentar un caso de hiperparatiroidismo primario de fenotipo clásico y discutir su fisiopatología, abordaje y tratamiento. Presentación del caso: mujer de 36 años quien consultó por tres fracturas patológicas simultáneas y disminución en la densidad mineral ósea (DMO), asociado al hallazgo intrahospitalario de nefrocalcinosis, insuficiencia renal, hipercalcemia y niveles aumentados de hormona paratiroidea. Según estos hallazgos, se dio diagnóstico de hiperparatiroidismo primario de fenotipo clásico, secundario a adenoma paratiroideo izquierdo y que recibió tratamiento quirúrgico. Discusión y conclusión: el hiperparatiroidismo primario de fenotipo clásico es una entidad patológica poco frecuente y se relaciona con graves complicaciones esqueléticas y renales. Hay pocos casos descritos en la literatura, por lo que se requiere realizar más estudios en nuestro medio para definir las particularidades epidemiológicas de los pacientes, logrando de esta manera evitar los casos con manifestaciones clínicas en etapas avanzadas de la enfermedad.

Las vías clínicas son instrumentos desarrollados para planificar y coordinar la secuencia de procedimientos clínicos y administrativos necesarios para conseguir la máxima calidad asistencial. Su rápida difusión en EE. UU. explica que más del 60% de los hospitales usen vías clínicas para sus procesos más frecuentes. Esto contrasta con su escasa implementación en Colombia y los países de la Región de las Américas. Las Vías Clínicas se ajustan con el cambio que representa el enfoque en procesos, orientado a optimizar una mejor atención al paciente.

La gastrostomía endoscópica percutánea (GEP), descrita por primera vez en la década de 1980, es un procedimiento universalmente utilizado para asegurar la alimentación enteral en pacientes que conservan la función del tracto digestivo, pero que por alguna condición médica o quirúrgica no pueden ingerir alimentos sólidos o líquidos. La realización de una GEP en pacientes con diálisis peritoneal (DP) sigue siendo controvertida, ya que muchos autores la consideran una contraindicación absoluta debido al alto riesgo de morbimortalidad asociada a infección intraperitoneal. Se presenta el caso de un paciente de 70 años, con antecedente de enfermedad renal crónica en estadio terminal y portador de catéter de DP, quien requirió GEP. Se socializa la estrategia utilizada en este caso particular.

Introducción: el COVID-19 es una enfermedad causada por un nuevo beta coronavirus (SARS-CoV-2), cuyo espectro incluye casos severos con neumonía y complicaciones sistémicas que se dan como consecuencia de una liberación exagerada de mediadores inflamatorios conocida como “tormenta de citoquinas”. En este tipo de pacientes las terapias de purificación sanguínea, incluyendo la hemoadsorción y la terapia plasmática podrían tener un beneficio clínico importante y evitar las complicaciones como disfunción multiorgánica y muerte. Objetivo: revisar toda la literatura disponible acerca de las terapias de purificación sanguínea y sus beneficios en los pacientes con COVID-19. Materiales y métodos: se realizó una búsqueda de la literatura en las bases de datos ClinicalKey, Embase, PubMed y Ovid con los términos “COVID-19”, “sepsis”, “SARS-CoV-2”, “citosinas”, “insuficienciencia renal”, “neumonía”. Resultados: se encontraron 246 referencias y luego de aplicar los filtros, se seleccionaron 61 publicaciones con contenido relevante para la presente revisión. Conclusión: teniendo en cuenta su gran evidencia científica en el manejo de la tormenta de citoquinas en otros escenarios, las terapias de purificación sanguínea, que incluyen técnicas difusivas, convectivas, de hemoadsorción y terapia plasmática, pueden ser herra- mientas de tratamiento muy prometedoras en pacientes con COVID-19.

La infección por el síndrome respiratorio agudo severo (SARS-CoV-2) ha causado una de las emergencias epidemiológicas más grandes de los últimos 10 años y sus efectos patológicos son aún estudiados. Por lo anterio, resulta importante describir los mecanismos asociados al compromiso renal y digestivo en la infección por SARS-CoV-2. Los mecanismos patológicos en tejido renal y en intestino causados por la infección por SARS-CoV-2 son propios del tropismo viral por células de estos sistemas y de los mecanismos citopáticos de etapa lítica de la infección, con una liberación continua de viriones que favorece la generación de un entorno inflamatorio con la consecuente secreción descontrolada de citoquinas proinflamatorias que conducen a la infección entérica del intestino y a las alteraciones en el riñón.

The aim of this study was to identity in silico the relationships among microRNAs (miRNAs) and genes encoding transcription factors, ubiquitylation, DNA methylation, and histone modifications in systemic lupus erythematosus (SLE). To identify miRNA dysregulation in SLE, we used miR2Disease and PhenomiR for information about miRNAs exhibiting differential regulation in disease and other biological processes, and HMDD for information about experimentally supported human miRNA-disease association data from genetics, epigenetics, circulating miRNAs, and miRNA-target interactions. This information was incorporated into the miRNA analysis. High-throughput sequencing revealed circulating miRNAs associated with kidney damage in patients with SLE. As the main finding of our in silico analysis of miRNAs differentially expressed in SLE and their interactions with disease-susceptibility genes, post-translational modifications, and transcription factors; we highlight 226 miRNAs associated with genes and processes. Moreover, we highlight that alterations of miRNAs such as hsa-miR-30a-5p, hsa-miR-16-5p, hsa-miR-142-5p, and hsa-miR-324-3p are most commonly associated with post-translational modifications. In addition, altered miRNAs that are most frequently associated with susceptibility-related genes are hsa-miR-16-5p, hsa-miR-374a-5p, hsa-miR-34a-5p, hsa-miR-31-5p, and hsa-miR-1-3p.

Antecedentes. La calidad de vida relacionada con la salud se puede afectar en personas con enfermedad renal crónica. Objetivo. Determinar la calidad de vida relacionada con la salud de los pacientes con enfermedad renal crónica (ERC) y su relación con variables sociodemográficas y datos clínicos. Materiales y métodos. Estudio analítico de corte transversal realizado en 80 personas con diagnóstico de enfermedad renal crónica. Participaron los pacientes que asistieron a la Unidad Renal y al servicio de hospitalización durante los meses de estudio. Se aplicó el cuestionario Kidney Disease Quality of Life (KDQOL-SF), valorando ocho dimensiones genéricas de la calidad de vida relacionada con la salud y tres específicas relacionadas con la enfermedad renal. Se compararon las variables socio demográficas y las relacionadas con la enfermedad con las dimensiones del KDQOOL-36. Resultados. El 23.5% de la población estudiada tenía entre 61 y 70 años y 51.3% eran hombres. Se obtuvieron promedios bajos en las dimensiones carga de la enfermedad renal (40.3), salud física (33.4) y salud mental (43.5) del componente genérico. Se encontró una correlación significativa de la edad con las dimensiones efectos de la enfermedad renal en la vida diaria (0.238, p=0.03) y el componente genérico salud física (-0.242, p=0.03). Un trabajo remunerado se correlacionó positivamente con los componentes genéricos salud física (0.499, p=0.001) y salud mental (0.375, p=0.001) y la carga de la enfermedad renal se correlacionó negativamente con el tiempo de diagnóstico de enfermedad renal (-0.241, p=0.02). Conclusión. La enfermedad renal crónica tiene una carga sobre la calidad de vida de los pacientes en términos de salud física, salud mental y carga de la enfermedad.

Introducción: La obesidad es un factor de riesgo de desarrollo y progresión de enfermedad renal crónica (ERC). En Latinoamérica existen pocos estudios donde se relacionen los estadios de ERC y grados de obesidad. Objetivo: El objetivo de estudio es evaluar la asociación de obesidad con la enfermedad renal crónica de pacientes atendidos en la consulta externa del departamento de Nefrología en la Clínica de la Costa en Barranquilla, Colombia. Materiales y métodos: Se realizó un estudio observacional descriptivo tipo serie de casos. La muestra estuvo compuesta por 300 pacientes acopiados en NefroRed©. Se realizaron medidas de tendencia central y un ?² para establecer la asociación entre los grados de obesidad con los estadios de la ERC. Los análisis estadísticos se realizaron en R-CRAN. Al realizar la prueba, se encontró una asociación entre los grados de obesidad y los diferentes estadios de la ERC [?²: 48,62; p-valor < 0,01]. Conclusión: No se encontró evidencia estadística de asociación entre el Perímetro Abdominal y los estadios de ERC [?²: 8,82; p-valor ? 0,05]. Existe asociación entre los grados de obesidad y los diferentes estadios de la ERC. No se encontró la relación entre perímetro abdominal y los estadios de ERC.

Introducción: Las glomerulonefritis (GN) primarias son enfermedades que afectan a la estructura y función del glomérulo. Dentro de las glomerulopatías proliferativas se ha observado que la GN mesangial por IgA e IgM, son las que evolucionan con mayor frecuencia a la insuficiencia renal permanente; seguida de la GN membrano-proliferativa o mesangiocapilar. La presentación clínica de la GN incluye hematuria, proteinuria e insuficiencia renal aguda o crónica. Material y métodos: Se revisaron las historias clínicas y se acopiaron en una base de datos todas las biopsias realizadas entre los años 2008 a 2014. Se realizó un ?², para establecer las asociaciones entre variables (? = 95%) y para las diferencias entre proporciones se utilizó t de Studentó U- Mann-Whitney. Resultados: Se seleccionaron 146 pacientes (88% del total analizado) que tenían los datos completos. La edad promedio fue de 8±4 años para ambos sexos. No se encontraron diferencias significativasni asociaciones entre el sexo y la edad (p? 0,05). Las GN predominantes fueron las proliferativa mesangial y la nefropatía por IgA. Las GN por membrana basal delgada, nefropatía por C3 y C4 q no se presentaron en varones. La presentación clínica más frecuente fue el Síndrome Nefrótico (58%). Conclusiones: Los hallazgos de éste registro de biopsias renales podrían ser la base para la creación de un Registro de Glomerulopatías en niños en Colombia, instrumento necesario para establecer programas de tratamiento y prevención de las enfermedades glomerulares en nuestro país a fin de disminuir su progresión.

Esta investigación intenta confirmar la hipótesis que lacalidad de vida relacionada con la salud (CVRS) se distribuye de manera distinta en los pacientes con enfermedad crónica hemodializados y no hemodializados. La CVRS es un componente importante en los estudios de salud, porque permite identificar el impacto que la enfermedad renal crónica genera en la percepción del paciente en relación con los aspectos que pueden verse afectados por dicha enfermedad como pueden ser el funcionamiento físico y el funcionamiento social. Diseño: Estudio descriptivo transversal en pacientes con diagnóstico de ERC que asisten a la Clínica de la Costa en Barranquilla.Se seleccionaron pacientes que reciben y no reciben tratamiento sustitutivo de hemodiálisis y diálisis peritoneal. Materiales y métodos: Se escogieron a todos los pacientes (80 sujetos) con diagnóstico de ERC que asistieron durante los meses de abril y mayo de 2013 a la Unidad Renal y al servicio de hospitalización de la institución para recibir tratamiento farmacológico y/o sustitutivo de hemodiálisis y diálisis peritoneal. El deterioro cognitivo se tuvo en cuenta como criterio de exclusión. Previo firma del consentimiento informado y antes de realizarse el tratamiento de hemodiálisis, se aplicó una encuesta que midió las variables socio-demográficas, sexo, edad, estado civil, escolaridad, estrato socioeconómico, aseguramiento; igualmente, el KidneyDiseaseQuality of Life (KDQOL-SF), cuestionario que valora la calidad de vida relacionada con la salud específico para pacientes con enfermedades renales, permitiendo identificar la percepción de los sujetos sobre su salud y su vida.

Los sistemas de información geográficos constituyen una importante herramienta en el estudio y la vigilancia epidemiológica, ya que integran la información médica, con datos geográficos, variables ambientales, socioeconómicas, entre otras; lo que permite la detección temprana de brotes de epidemias y la toma de decisiones en un contexto geográfico. Objetivo: Desarrollar e Implementar una API de Google, que georeferenciar a pacientes con enfermedad glomerular, a través del enlace de Google Maps con la información contenida en NEFRORED. Materiales y Métodos: Se utilizó el registro de pacientes con enfermedad glomerular concentrados en la herramienta informática NEFRORED. Se generó un código en lenguaje PHP, que calcula y almacena la longitud y la latitud de la ubicación (dirección residencia) de los pacientes. Se incorporó a la línea de código de la API de Google Maps una sentencia que calcula el tamaño de la pantalla que se emplea para la visualización. Se modificó la línea de comandos para que genere un archivo. XML, con la información a georeferenciar (Nombre, sexo, dirección del paciente y las coordenadas geográficas). Resultados: Se crearon unas líneas de comando en lenguaje PHP, que permiten generara en tiempo real un mapa con la ubicación espacial de todos los pacientes con enfermedad glomerular concentrados en la herramienta informática NEFRORED, a través de Google Map y empleando para ello la dirección de domicilio reportada por cada uno de los pacientes. Igualmente se logró identificar a los pacientes empleando para ello la característica sexo, con la posibilidad de ampliarlo a otras variables de interés. Conclusión: Google maps es apropiado para georeferenciar con alto grado de exactitud a pacientes con enfermedad glomerular, empleando imágenes de alta calidad. La herramienta Google maps permite generar de forma automática, mapas actualizados de la distribución y ubicación de pacientes con enfermedad glomerular.

En el mundo existen 33,4 millones de personas infectadas por el virus de la inmunodeficiencia humana (VIH) con un total de 2 millones de defunciones en el año 2008. Afectación renal es una complicacion relativamente frecuente por diversos mecanismos ya sea por acción directa o indirecta del virus o por complicaciones sistémicas de la enfermedad o del tratamiento. La glomerulonefritis lupus like es una entidad poco frecuente, mediada por complejos inmunes, descrita en la población de pacientes infectados por el VIH, en la cual los hallazgos histopatológicos e inmunopatológicos son similares a la nefritis lúpica, pero a diferencia de ella no presenta evidencia de síntomas sistémicos ni hallazgos serológicos. Presentamos el caso de un paciente con diagnóstico de nefropatia lupus like. Caso clínico: Paciente masculino de 37 años, raza mestiza, quien consulta por cuadro clínico de 6 días de evolución consistente en edema ascendente en miembros inferiores, astenia, adinamia, como antecedente personal de importancia infección por HIV diagnosticado hace 15 días manejado con Efavirenz 600mg al día, Lamivudina/Zidivudina 150/300mg cada 12hs. Al examen físico se encontro palidez mucocutánea ligera asociado a edema en miembros inferiores, se realizaron estudios paraclínicos reportándose Hemoglobina: 9.1gr, Hematocrito :25.3%, leucocitos: 4260mm3, neutrofilos: 45.5%, linfocitos: 35.2%, plaquetas:260000mm3, BUN:28.9mg/dl, creatinina:1.94mg/dl, colesterol total:321mg/dl, triglicéridos:340mg/dl, proteínas totales:6.0gr, albumina:1.65gr, proteinuria en 24hs: 3,34gr, depuración de creatinina:59.2ml/min, parcial de orina: hematíes 18-20 por campo, estudios ultrasonografia reportaron cambios parenquimatosos. La impresión clinica diagnostica inicial fue de nefropatía asociada a VIH (NAVIH) vs glomerulopatia primaria, motivo por el cual se indico la realizacion de la biopsia renal. El estudio histopatológico reporto una proliferación mesangioendotelial global difusa asociada a depositos subendoteliales con formación de aparentes asas de alambre, presencia de trombos hialinos imagen I y proliferacion extracapilar con formación de semilunas epiteliales. Imagen II A nivel tubular se reporto ligera atrofia con pérdida moderada del borde en cepillo, con un diagnóstico final de glomerulonefritis tipo lupus like. Discusión. El primer informe de enfermedad renal en pacientes con síndrome de inmunodeficinecia humana (SIDA) fue en el año de 1984 por Rao y colaboradores, al describir un grupo de pacientes de raza negra consumidores de heroína intravenosa infectados por el virus de la inmunodeficiencia humana (VIH) con cuadros renales de glomeruloesclerosis focal y segmentaria. Las lesiones renales en el paciente VIH + pueden ser el resultado de diversos mecanismos, ya sea como resultado de la respuesta inmunológica contra el virus, asi como infecciones oportunistas, neoplasias, alteraciones hemodinámicas o terapias entre otras. La nefropatia más conocida es la nefropatia asociada al VIH ( NAVIH o HIVAN en la literatura inglesa), caracterizada por una alta proteinuria con progresion rapida a una insuficiencia renal terminal, histologicamente manifiesta por una glomeruloesclerosis focal y segmentaria con daño tubular severo. Sin embargo esta no constituye la unica nefropatia multiples reportes han demostrado una gran gama de lesiones en el pacciente VIH que van desde una enfermedad de cambios minimos a glomerulonefritis rapidamente progresiva. La glomerulonefritis lupus like es una forma de glomerulonefritis mediada por complejos inmunes, que se ha descrito en la poblacion de pacientes infectados por el VIH, caracterizada por depositos glomerulares por lo general a nivel subendotelial (asas de alambre) de inmunoglobulinas tipo IgA, IgG e IgM asociado a depositos de complemento C3 y C1q denominadose patron full house. En los pacientes con glomerulonefritis lupus like a diferencia de los pacientes con diagnostico de VIH y lupus eritematoso sistemico (LES) carecen de evidencia clínica y serológica de LES.

La amiloidosis es un grupo de enfermedades poco frecuente que se caracterizan por el depósito extracelular de un material proteico fibrilar denominado amiloide, la afectación renal en la amiloidosis sistemica es frecuente, confiriéndolem un pronóstico desfavorable siendo la falla renal la segunda causa de muerte. Caso Clínico: Hombre de 55 años hipertenso de novo, con cuadro clinico de 7 meses de evolucion de edema progresivo asociándo a disnea oliguria y hematuria documentandose síndrome nefrótico el reporte histopatologico revela acumulo de material eosinofilico rojo congo positiva a nivel glomerular, e inmunofluoresencia positiva para cadenas livianas en patron mesangial diagnosticandose amiloidosis renal. Conclusiones: La amiloidosis sigue siendo un desafío para los clínicos, en cuanto a su diagnóstico y tratamiento. El pronóstico es cada vez peor a medida que se comprometen más órganos, se debe considerar que la principal causa de muerte de los pacientes por amiloidosis obedece a trastornos cardiacos. La falla renal es la segunda causa de muerte
