Abstract Background: As fat grafting is commonly used as a filler, Adipose-derived stem/stromal cells (ASC) have been reported to be key player in retention rate. Paracrine and differentiation potential of those cells confer them strong pro-angiogenic capacities. However, a full characterization of the influence of aging on ASC has not been reported yet. Here we’ve investigated the effect of age on paracrine function, stemness and angiogenic potential of ASC. Methods: ASC were extracted from young and old adult donors. We assessed stromal vascular fraction cell populations repartition, ASC stemness potential, capability to differentiate into mesenchymal lineages as well as their secretome. Angiogenic potential was assessed using a sprouting assay, an indirect co-culture of ASC and dermal microvascular endothelial cells (EC). Total vascular sprout length was measured, and co-culture soluble factors were quantified. Pro-angiogenic factors alone or in combination as well as ASC-conditioned medium (CM) were added to EC to assess sprouting induction. Results: Decrease of endothelial cells yield and percentage is observed in cells extracted from adipose tissue of older patients, whereas ASC percentage increased with age. Clonogenic potential of ASC is stable with age. ASC can differentiate into adipocytes, chondrocytes and osteoblasts, and aging does not alter this potential. Among the 25 analytes quantified, high levels of pro-angiogenic factors were found, but none is significantly modulated with age. ASC induce a significantly longer vascular sprouts compared to fibroblasts, and no difference was found between young and old ASC donors on that parameter. Higher concentrations of FGF-2, G-CSF, HGF and IL-8, and lower concentrations of VEGF-C were quantified in EC/ASC co-cultures compared to EC/fibroblasts co-cultures. EC/ASC from young donors secrete higher levels of VEGF-A compared to old ones. Neither soluble factor nor CM without cells are able to induce organized sprouts, highlighting the requirement of cell communication for sprouting. CM produced by ASC supporting development of long vascular sprouts promote sprouting in co-cultures that establish shorter sprouts. Conclusion: Our results show cells from young and old donors exhibit no difference in all assessed parameters, suggesting all patients could be included in clinical applications. We emphasized the leading role of ASC in angiogenesis, without impairment with age, where secretome is a key but not sufficient actor.

Schiff bases are crucial intermediates in organic chemistry. They are used to introduce carbon–nitrogen double bonds in organic molecules. In this study, 4-(2-hydroxyanilino) pent-3-in-2-one (L) was synthesized from the reaction of 2,4-pentanedione and 2-aminophenol. The structure of L was confirmed through analytical and spectroscopic data. In addition, its thermal stability was examined using thermogravimetric and differential thermal analysis. In the gas phase, investigations of the highest occupied molecular orbital (HOMO) and lowest unoccupied molecular orbital (LUMO) revealed an energy gap (ΔE) of 3.9304 eV. The antioxidant capacity of L was assessed using various techniques, including hydroxyl radical scavenging, scavenging of 2,2-diphenyl-1-picrylhydrazyl (DPPH), β-carotene bleaching, and scavenging of 2,2′-azino-bis(3-ethylbenzthiazoline-6-sulphonic acid (ABTS). The synthesized Schiff base displayed significant antioxidant activity, with a half-maximal inhibitory concentration (IC50) of 19.8 µg/mL in the DPPH assay, 0.14 µg/mL in the ABTS assay, and 59.6 μg/mL in the hydroxyl radical scavenging assay. The bioactivity of the 4-(2-hydroxyanilino) pent-3-in-2-one was confirmed by the DFT study, and the MEP analysis indicates the reactivity of nitrogen and oxygen atoms. Docking studies and wave function analyses were conducted to provide further insights into the compound’s potential applications in the pharmaceutical field.

Avian pathogenic Escherichia coli (APEC) represents a major challenge for the poultry industry, causing significant economic losses. This problem is exacerbated by the misuse use of antibiotics in Veterinary Medicine, leading to the emergence of resistant strains and thus creating a significant risk to Public Health. This study, carried out on 38 poultry farms in Algeria, involved the collection of 200 samples for the isolation of E. coli strains. The resistance of these strains to frequently used antibiotics was assessed using the agar diffusion method. Multiple Correspondence Analysis (MCA) was used to determine potential risk factors. The obtained results revealed that E. coli was present in 30% of samples. Alarming levels of resistance were observed against Tetracycline (81.6%), Ampicillin (78.3%), Ciprofloxacin (68.3%) and Nalidixic acid (60%). Stressful environmental conditions in poultry houses, such as temperature variations, high humidity, poor ventilation and stocking density were identified as key factors in the development of avian colibacillosis. In conclusion, the current study highlights the urgent need to strictly monitor and regulate the use of antibiotics in Veterinary Medicine and improve animal welfare in order to minimize the risk it pose to Public Health originated in the farms. In addition, it is essential that farmers maintain optimal environmental conditions in chicken rearing.

Multidrug-resistant (MDR) Gram-negative bacteria remain a global public health issue due to the barrier imposed by their outer membrane and their propensity to form biofilms. It is becoming imperative to develop new antibacterial strategies. In this context, this study aims to evaluate the antibacterial efficacy of Origanum vulgare essential oil (OEO), alone and in combination with antibiotics, as well as its antibiofilm action against multidrug-resistant Gram-negative strains. OEO components were identified by gas chromatography-mass spectrometry (GC-MS), and antibacterial activity was assessed using the agar diffusion test and the microdilution method. Interactions between OEO and antibiotics were examined using the checkerboard method, while antibiofilm activity was analyzed using the crystal violet assay. Chemical analysis revealed that carvacrol was the major compound in OEO (61.51%). This essential oil demonstrated activity against all the tested strains, with inhibition zone diameters (IZDs) reaching 32.3 ± 1.5 mm. The combination of OEO with different antibiotics produced synergistic and additive effects, leading to a reduction of up to 98.44% in minimum inhibitory concentrations (MICs). In addition, this essential oil demonstrated an ability to inhibit and even eradicate biofilm formation. These results suggest that OEO could be exploited in the development of new molecules, combining its metabolites with antibiotics.

Background: Oxymel is a functional beverage with a rich historical background of use in multiple societies. Various simple and compound oxymels are prescribed in certain complementary and traditional medical systems, including traditional Persian Medicine. In recent years, numerous clinical and preclinical studies have been conducted in the pharmacy and food industry to investigate the efficacy of various oxymel formulations. This article aims to systematically review and summarize interventional studies on oxymel in both clinical research and animal models. Methods: Relevant articles were searched in Embase, MEDLINE, Web of Science Core Collection, Scopus, and Google Scholar from inception to July 2023 using the keyword “Oxymel” and its equivalents in other languages. Animal and human interventional studies were selected from the search results for review. Results: This review includes twenty studies, comprising twelve clinical trials, two case studies, and six animal studies. The most commonly reported actions of oxymel include positive effects on the cardiovascular system, as well as antioxidant and anti-inflammatory properties. Furthermore, compound oxymel formulations have demonstrated additional benefits depending on the inclusion of specific medicinal herbs. Conclusion: Based on our findings, oxymel appears to be a valuable functional food for healthy individuals and a potentially effective and safe treatment option for managing certain diseases such as asthma, obesity, and type 2 diabetes. However, further clinical trials with larger sample sizes and longer durations are needed to fully elucidate the potential side effects and benefits of both simple and compound oxymels in various disease states.

Close follow-up of patients with liver cirrhosis has led to increased detection of hepatocellular carcinoma (HCC) at an early stage, especially with magnetic resonance imaging (MRI) innovations. We report the case of a 70-year-old man, with a recent history of liver cirrhosis due to chronic hepatitis C virus (HCV) complicated by hepatocellular carcinoma (HCC), and for whom trans-arterial chemoembolization (TACE) was planned, as the patient was assigned Child B7 at admission. Angiography performed during the first TACE cycle shows not only the “tumor blush” corresponding to previously detected HCC but also an additional small foci of HCC uptake seen within a large dysplastic nodule giving the appearance of “nodule-within-nodule.” Early detection of hepatocellular carcinoma improves prognosis. Hence, it is essential to be aware of all early aspects of HCC, including the nodule-within-nodule appearance on cross-sectional imaging, and also in angiography, as in this case.

Acute intestinal intussusception in adults is a rare condition, most often secondary to an organic lesion (tumor or inflammation), representing 1%-5% of intestinal obstructions. Pure colic intussusception on lipoma rectal causing bowel obstruction is an exceptional situation. A 60-year-old man presented to the emergency department for acute abdominal pain with marked abdominal distention and red rectal bleeding. A contrast-enhanced abdominal CT scan was performed, which revealed a recto-sigmoid intussusception on lipoma, causing mechanical intestinal obstruction. The patient underwent a partial reduction of the intussusception with partial sigmoid resection and end colostomy. Colonic lipomas of the recto-sigmoid region represent a very rare condition and a subsequent etiology for intussusception and bowel obstruction in adults. However, it should be considered in the differential diagnosis of such situations.

Abstract Background Very little is known about factors influencing adolescent childbearing despite an upward trend in adolescent childbearing prevalence in Burundi, and its perceived implications on the rapid population growth and ill-health of young mothers and their babies. To adress this gap, this study aimed to examine the prevalence, trends and determinants of adolescent childbearing in Burundi. Methods Secondary analyses of the 1987, 2010 and 2016–17 Burundi Demographic and Health Surveys (BDHS) data were conducted using STATA. Weighted samples of 731 (1987 BDHS), 2359 (2010 BDHS) and 3859 (2016-17BDHS) adolescent girls aged 15–19 years old were used for descriptive and trend analyses. Both bivariable and multivariable two-level logistic regression analyses were performed to identify the main factors associated with adolescent childbearing using only the 2016–17 BDHS data. Background: A major problem with the University of Pennsylvania Smell Identification Test (UPSIT) is its poor sensitivity for malingering detection in a group of people familiar with the test mechanism. This study aimed to evaluate the modification of UPSIT to detect anosmia malingering. Methods: This was a pilot experimental study conducted in 2019 in Tehran. The participants were 60 healthy subjects classified into two groups of 30 people. The first group was requested to deliberately feign a negative result on the Iranian version of UPSIT, Iran Smell Identification Test (ISIT) (malingering group). The second group consisted of participants, who did not scratch the odorant part of ISIT during the tests (anosmia group). ISIT was modified in two steps. At each step, one incorrect option was deleted from the available choices. The number of each group’s answers, altered away from the correct choice, was then calculated and compared. Results: The coached malingering group participants were able to feign anosmia in the original ISIT exam. Heavy metals pollution associated with oil spills has become a major concern worldwide. It is essential to break down these contaminants in the environment. In the environment, microbes have been used to detoxify and transform hazardous components. The process can function naturally or can be enhanced by adding nutrients, electron acceptors, or other factors. This study investigates some factors affecting hydrocarbon remediation technologies/approaches. Combinations of biological, chemical, and eco-toxicological techniques are used for this process while monitoring the efficacy of bacterial products and nutrient amendments to stimulate the biotransformation of contaminated soil. Different hydrocarbon removal levels were observed with bacterial augmentation (Beta proteobacterium and Rhodococcus ruber), exhibiting a total petroleum hydrocarbon (TPH) reduction of 61%, which was further improved to a 73% reduction using bacterial augmentation combined with nutrient amendment (nitrogen, potassium, and phosphorus). A heavy metal analysis of the polluted soils showed that the combination of nutrient and bacterial augmentation resulted in a significant reduction (p-value < 0.05) in lead, zinc, and barium. Toxicity testing also showed that a reduction of up to 50% was achieved using these remediation approaches.

Background Adolescents are a vulnerable group to body dissatisfaction that causes unhealthy attitudes and behaviours. It has been shown that many factors affect body image dissatisfaction. Aim To describe body image size perception, degree of body dissatisfaction, avoidance behaviours, and their relationship with gender, age, and weight status, and to assess the relationship between body dissatisfaction and avoidance behaviours among a group of Moroccan adolescents. Subjects and methods This cross-sectional survey was comprised of a sample of 487 adolescents. Stunkard’s Figure Rating Scale was used to assess perceptions of current and ideal body image and body dissatisfaction. Body Image Avoidance Questionnaire was employed to assess avoidance behaviours. Results Body dissatisfaction was found to be strongly associated with weight status in both male and female adolescents. Avoidance behaviours were positively associated with gender, age, weight status, and body dissatisfaction. Indeed, avoidance behaviours were frequently reported among females, young adolescents, those with high BMI z-score, and those seeking to lose weight. Adolescents tended to behave under the influence of two factors: clothing and weighing-grooming. Conclusion Weight status is an important factor that deeply impacts adolescent body dissatisfaction. Adolescents resort to behaviour avoidance and physical appearance control to cope with body dissatisfaction.

The dielectric constant (DC) is one of the key properties for detection of threat materials such as Improvised Explosive Devices (IEDs). In the present paper, the density functional theory (DFT) as well as ab-initio approaches are used to explore effective methods to predict dielectric constants of a series of 12 energetic materials (EMs) for which experimental data needed to experimentally determine the dielectric constant (refractive indices) are available. These include military grades energetic materials, nitro and peroxide compounds, and the widely used nitroglycerin. Ab-initio and DFT calculations are conducted. In order to calculate dielectric constant values of materials, potential DFT functional combined with basis sets are considered for testing. Accuracy of the calculations are compared to experimental data listed in the scientific literature, and time required for calculations are both evaluated and discussed. The best functional/basis set combinations among those tested are CAM-B3LYP and AUG-cc-pVDZm, which provide great results, with accuracy deviations below 5% when calculated results are compared to experimental data.

Abstract Cigarette smoking has a negative effect on respiratory and skeletal muscle function and is a risk factor for various chronic diseases. To assess the effects of 14 days of smoking cessation on respiratory and skeletal muscle function, markers of inflammation and oxidative stress in humans. Spirometry, skeletal muscle function, circulating carboxyhaemoglobin levels, advanced glycation end products (AGEs), markers of oxidative stress and serum cytokines were measured in 38 non-smokers, and in 48 cigarette smokers at baseline and after 14 days of smoking cessation. Peak expiratory flow (p = 0.004) and forced expiratory volume in 1 s/forced vital capacity (p = 0.037) were lower in smokers compared to non-smokers but did not change significantly after smoking cessation. Objectives. The aim of this study is to assess physical appearance behavior avoidance among a group of Moroccan adolescents and to identify the associated factors. Material and methods. The data were gathered from a cross-sectional study conducted on 487 adolescents (223 boys and 264 girls), with the average age of 14.6 years. Gender, age, body mass index (BMI), and satisfaction of their body weight and height were considered in this study. Body satisfaction was evaluated by two questions: "Are you satisfied with your body weight?" and "are you satisfied with your body height?". The Body Image Avoidance Questionnaire (BIAQ) test was used to assess the adopted behaviors. Results. The average score on the BIAQ test was 24.4. According to the different factors of the BIAQ test, the factors "clothing", "grooming and weighing", and "eating restraint", were more noticed among adolescents. The results showed that behavioral avoidance was associated with gender, age, BMI, and body dissatisfaction. It was females, the youngest group of adolescents, overweight, and those dissatisfied with their body weight or height who had the highest scores. This reflects the fact that adolescents resort to physical appearance behavior avoidance or control including practices such as dressing, grooming, and weighing. Conclusions. It is recommended by health professionals to enhance awareness among adolescents about the risks of adopting such behaviors on their physical and mental health, to correct the narrow and unrealistic standards of physical appearance, and to encourage healthy behaviors.

Objective. The aim of this study was to assess the health related quality of life of elderly Moroccans living at home as well as determining the factors that influence it. Material and methods. The data was gathered through a questionnaire survey conducted between 2017 and 2018. Age, gender, level of education, socio-professional activity, morbidity and perception of old age were chosen as variables, and the LEIPAD scale was used to evaluate the respondent’s quality of life, French version. Results. This study included 520 subjects aged 65 and above (277 men and 243 women). The average age was 71.2 years, and 60.8% of them were illiterate, 22.3% employed, 25.0% retired, and 52.7% have never worked. In addition, 57.9% of participants had at least one chronic illness and 42.9% had a pessimistic attitude toward old age. Overall, the surveyed subjects’ quality of life was adequate and satisfactory. The main ratings for the areas involving "taking care of oneself" and "depression-anxiety" were the lowest (23.7 and 28.6). On the other hand, the "sexual functioning" area was the most affected (score=73.7), thus impacting the participants’ quality of life. Furthermore, socio-demographic characteristics proved to have a negative impact on the participants’ quality of life. The highest average test scores were reported by women, the oldest subjects (>-75 years), subjects with low levels of education, without professions and retirees, suffering from chronic diseases and those who negatively perceived the old age, which showed a relatively impaired quality of life. Conclusions. The majority of the surveyed people had a good quality of life. Nevertheless, providing adequate treatment for the most disadvantaged elderly people could improve their quality of life and help them reach the old age in good health.

Context.-The Nottingham Grading System (NGS) developed by Elston and Ellis is used to grade invasive breast cancer (IBC). Glandular (acinar)/tubule formation is a component of NGS. Objective.-To investigate the ability of pathologists to identify individual structures that should be classified as glandular (acinar)/tubule formation. Design.-A total of 58 hematoxylin-eosin photographic images of IBC with 1 structure circled were classified as tubules (41 cases) or nontubules (17 cases) by Professor Ellis. Images were sent as a PowerPoint (Microsoft) file to breast pathologists, who were provided with the World Health Organization definition of a tubule and asked to determine if a circled structure represented a tubule. Results.-Among 35 pathologists, the [kappa] statistic for assessing agreement in evaluating the 58 images was 0.324 (95% CI, 0.314-0.335). The median concordance rate between a participating pathologist and Professor Ellis was 94.1% for evaluating 17 nontubule cases and 53.7% for 41 tubule cases. A total of 41% of the tubule cases were classified correctly by less than 50% of pathologists. Structures classified as tubules by Professor Ellis but often not recognized as tubules by pathologists included glands with complex architecture, mucinous carcinoma, and the "inverted tubule" pattern of micropapillary carcinoma. A total of 80% of participants reported that they did not have clarity on what represented a tubule. Conclusions.-We identified structures that should be included as tubules but that were not readily identified by pathologists. Greater concordance for identification of tubules might be obtained by providing more detailed images and descriptions of the types of structures included as tubules. INTRODUCTION: Although fertility control remains a major priority for the Burundian government and most of its partners, few studies on Burundi´s fertility determinants are available to guide interventions. To address this gap, our study aims to examine the most factors influencing fertility differentials in Burundi by using the latest Burundi Demographic and Health Survey data. METHODS: using data from the 2016-17 Burundi Demographic and Health Survey, one-way analysis of variance was performed to describe variations in mean number of children ever born across categories of correlate variables. Then univariable and multivariable poisson regression analyses were carried out to identify the most factors influencing fertility differentials in Burundi. RESULTS: in our sample, the total number of children ever born ranged from 0 to 15 children by women with a mean number of 2.7 children (±2.8 SD). Factors such as urban residence (aIRR 0.769, 95% CI: 0.739 - 0.782, p = 0.008), increase in the level of education of both women and husbands (aIRRs of 0.718, 95% CI: 0.643 - 0.802, P =0.001 and 0.729, 95% CI: 0.711 - 0.763, p=0.001 respectively), no history of infant mortality experience (aIRR 0.722, 95% IC: 0.710 - 0.734, p=0.001) and increase in age at first marriage or first birth (aIRRs of 0.864, 95% CI: 0.837 - 0.891, P =0.001 and 0.812, 95% CI: 0.781 - 0.845 , p=0.001 respectively) are associated with a low fertility rate while factors such as residence especially in Southern region (aIRR 1.129, 95% IC: 1.077 - 1.184, p=0.001), women and husband´s agricultural profession (aIRRs of 1.521, 95% CI: 1.429 - 1.568, P =0.001 and 1.294, 95% CI: 1.211 - 1.316, p=0.001 respectively), household poverty (aIRR 1.117, 95% IC: 1.080 - 1.155, p0.001), lack of knowledge of any contraceptive methods (aIRR 1.502, 95% IC: 1.494 - 1.564, p=0.001) and non-use of modern contraceptive methods (aIRR 1.583, 95% IC: 1.562 - 1.607, p=0.001) are associated with a high fertility rate. CONCLUSION: the results of this study suggest that actions aimed at promoting education in general especially female education, improving child survival, women´s socioeconomic status, agriculture mechanization and increasing number and scope of family planning services, could help reduce Burundi fertility rate.

Liver damage during COVID-19 disease has been described in numerous studies. Its mechanism is poorly understood. It is mainly reserved for severe forms and is manifested by abnormalities of the hepatic assessment and more particularly cytolysis. Particular attention must be paid to patients with chronic liver disease, both in terms of follow-up and treatment. We wanted to know the evolution of COVID-19 and its treatment, on the liver function of a 27-year-old patient followed for chronic non-cirrhotic hepatitis B at the Hassan II University Hospital in Fez. Our patient had stopped the antiviral B treatment and presented COVID-19 infection with minimal to moderate impairment. The initial evaluation showed cytolysis at 4 times upper limit of normal (ULN). Management consisted in the immediate resumption of Tenofovir in combination with hydroxychloroquine (HCQ) and azythromycin with good clinical and biological evolution.

Plants act as a rich source of novel natural pesticides. In the backdrop of the recent revival of interest in developing plant-based insecticides, this study was carried out to investigate the pesticidal activity of Sundarban mangrove plants. A total of nine different plant parts from five plants, namely, Aegiceras corniculatum, Excoecaria agallocha, Heritiera fomes, Xylocarpus moluccensis, and Xylocarpus granatum, were extracted with methanol and tested for insecticidal activity against two common stored product pests Sitophilus oryzae and Sitophilus zeamais using direct contact feeding deterrent wafer disc method. Three bark extracts from A. corniculatum, E. agallocha, and H. fomes showed potent and statistically significant insecticidal activity against both S. oryzae and S. zeamais pests (80–100% mortality). All the active bark extracts were further fractionated using C-18 solid-phase extraction (SPE) columns and tested for their insecticidal activity against S. oryzae pest to identify the active fraction. Only the SPE4 fraction (100% MeOH) from all the three active plants showed the activity against S. oryzae pest with a lethal concentration 50% (LC50) value of 0.5, 1.0, and 1.5 mg/disc for A. corniculatum, E. agallocha, and H. fomes, respectively. The active fraction of A. corniculatum was further profiled for identification of active compounds using LC-ESI-MS and identified (along with some unknown peaks) two previously reported compounds at m/z 625.17630 (isorhamnetin 3-O-rutinoside) and 422.25346 (paspaline) as major constituents. Insecticidal activities of these plants are reported in this study for the first time and would be useful in promoting research aiming for the development of new biopesticides from mangrove plants.

Bacillus Calmette and Guérin (BCG), widely used as a vaccination to prevent tuberculosis, is also used as immunotherapy, by intrabladder instillation, to treat superficial bladder cancers and prevent recurrence. Complications following instillation of BCG are most often localized reactions, such as cystitis or hematuria. They can more rarely be generalized and potentially severe such as hepatitis, pneumopathies, aortitis, and localization to hematopoietic tissue. We have reported the observation of a 47-year-old patient followed up for a bladder tumor operated for transurethral resection of the bladder, then having benefited from an instillation of BCG therapy complicated by occurrence a week later of an acute hepatitis. The diagnostic time was 2 days, and the outcome was favorable with corticosteroid therapy.

La maladie à IgG4 encore appelée polyexocrinopathie auto-immune à IgG4 est une nouvelle entité où s´inscrit la PAI de type 1. Elle peut toucher différents organes (système nerveux central, les glandes salivaires, la thyroïde, les poumons, le pancréas, les voies biliaires, le foie, le tube digestif, les reins, la prostate…) avec des symptômes relatifs à l´organe atteint. Elle est plus fréquente chez l´homme de plus de 50 ans. Son incidence et sa prévalence sont mal connues car c´est une maladie peu fréquente. Elle est a priori plus fréquente en Asie et ne représente que 20 à 30% des PAI en occident. Son diagnostic est histologique caractérisé par la présence, dans un contexte d´élévation sérique des IgG4 dans plus de 80% des cas, d´un infiltrat lymphoplasmocytaire dense de l´organe atteint avec positivité des IgG4 en immuno- histochimie, d´une fibrose d´organe et des veinulites oblitérantes. Elle est sensible à la corticothérapie avec un risque de rechute à l´arrêt de la corticothérapie qui n´est pas négligeable et qui conduit alors à l´utilisation d´immunomodulateurs, principalement: les thiopurines (Azathioprine ou le 6-mercaptopurine), le méthotrexate et de façon plus récente le rituximab qui peut être utilisé également comme traitement d´induction. Grâce aux avancées récentes, des critères histologiques et cliniques précis sont maintenant connus permettant de limiter les prises en charge inadaptées telle que la chirurgie. Cependant, de nombreuses lacunes persistent dans nos connaissances: sur la physiopathologie, l´identification de biomarqueurs spécifiques autres que les IgG4, l´histoire naturelle de la maladie et l´évaluation du risque de cancer à long terme, les performances des outils diagnostiques comme la biopsie pancréatique sous échoendoscopie. De même, une prise en charge consensuelle internationale reste à définir dans les phases initiales de la maladie et en cas de rechute. L´objectif de cette étude est de rapporter 3 cas de ML-IgG4 en se basant sur les critères cliniques et radiologiques, la réponse thérapeutique.

Background and Objective: Previous studies had examined the effect of learning in young adults with generally worse results in cases with the delayed sleep-wake phase disorder. In this study, the relationship between circadian rhythm preferences and objective structured clinical examination (OSCE) results was examined in medical students. Materials and Methods: This study was conducted during the OSCE for clinical license in medical students who were graduation candidates. The reduced form of the Horne and Ostberg Morningness-Eveningness Questionnaire was used to estimate their circadian chronotype. The OSCE consisted of 12 stations, which evaluated different clinical skills, as well as participant’s medical knowledge. The effect of morningness-eveningness preference of each participant on score was evaluated within each station of OSCE. Results: In total, 78 students participated out of a total of 114 clinically competent candidates (aged 25-34 years) were evaluated. In terms of sex, 56% of the students were girl, who obtained significantly higher grades in three stations (P < 0.05). The grades of students with delayed sleep-wake chronotypes were higher in most of the stations; however, the difference was statistically significant in only one station (P = 0.045), which was the most difficult station of the exam. Nevertheless, the delayed sleep-wake chronotype was associated with excessive daytime sleepiness (P = 0.002). Conclusion: Contrary to other similar studies in medical students, normal circadian chronotype was associated with poorer outcome in the OSCE; which may be explained by unusual student training hours during shifts, and the resultant available time for undisturbed study .

The high viscosity and low flow properties of some crude oil make them difficult to extract from oil reservoirs. This study investigated the mechanisms responsible for the enhancement of oil recovery using fractured dolomite core models. Bacterial strains, Nocardia cyriacigeorgica, Bacillus species, and Pseudomonasputida, isolated from Libyan oil fields, had the ability to biotransform heavy crude oil by reducing its viscosity and converting heavier components into lighter ones. The efficiencies of the three bacterial strains were assessed using sand-packed column experiments through the injection of bacteria to mimic in-situ oil recovery. The optimum biotransformation values of Libyan Bouri crude oil were determined as 77.1, 61.2, and 61.1% using the Bacillus sp., P. putida, and Nocardia cyriacigeorgica, respectively, at 55 °C. Viscosity analyses showed that these strains resulted in the reduction of the viscosity of the crude oil at two different temperatures of 37 and 55 °C. The highest recovery of residual oil was about 11.3% using Bacillus sp. The study confirmed that the selected bacterial species were capable of displacing additional oil under simulated oil field conditions.

Abstract Background Trauma is the main cause of morbidity and mortality in the pediatric population. Blunt trauma to the abdomen accounts for the majority of abdominal injuries in children. Pancreatic injury, although uncommon (2 to 9%), is the fourth most common solid organ injury. Unlike other solid organ injuries, pancreatic trauma may be subtle and difficult to diagnose. Computed tomography currently is the imaging modality of choice. As the incidence of pancreatic injury in children sustaining blunt abdominal trauma is low, management remains a challenge. Case presentation We present a 7-year-old Bahraini boy who sustained blunt trauma to his abdomen. He presented with abdominal pain and vomiting. His examination revealed abdominal distension and an epigastric bruise. Contrast-enhanced computed tomography reported grade III liver injury, grade I bilateral renal injury, a suspicion of splenic injury, and a grade III to IV pancreatic injury. He was admitted to Pediatric Intensive Care Unit and was treated conservatively. Because he was stable, he was discharged to the surgical ward at day 3. At day 18 he developed a pancreatic pseudocyst that was aspirated and recurred at day 25 when a pigtail catheter was inserted. He was kept on total parenteral nutrition through a peripherally inserted central catheter. The pigtail catheter was removed on day 36 and a low fat diet was started by day 44. He was discharged home at day 55 in good health. Out-patient follow-up and serial abdominal ultrasound showed resolution of the cyst and normalization of blood tests. Conclusion Non-operative management of pancreatic injury is effective and safe in hemodynamically stable patients with no other indication for surgery.

Hydatid disease HD or echinococcosis is a parasitic disease caused by Echinococcus granulosus. The prevalence of this parasitic disease in Morocco approximates 2 to 3% in highly endemic regions such Atlas Mountains. Liver is the main affected organ, and cholangitis is the most frequent complication of Hydatic liver cyst, however acute pancreatitis remain a very rare complication of the disease as showed by the very few cases reported in the literature. Its treatment combines the management of acute pancreatitis, endoscopic retrograde cholangiopancreatography (ERCP) and surgical treatment of the cyst. We report a series of 16 patients diagnosed for hydatid cyst of liver with pancreatitis at the University Hospital Hassan II to show the interest and outcomes of the management of such complication . We retrospectively review the clinical presentation and outcomes of management of 16 patients admitted for hydatid cyst of liver with acute pancreatitis at the department of hepato gastroenterology at the university Hospital Hassan II between January 2013 and July 2018. The average age of our patients was 44.33 years [20;66] with a female predominance. Only 12.5% (n = 2) of the patients had past history of surgical cure of liver hydatid cyst. All patients had acute pancreatitis associated with cholangitis. Abdominal CT scan showed oedematous pancreatitis in 87.5% (n = 14) and the necrotizing pancreatitis in 12.5% (n = 2). The common bile duct (CBD) was dilated in all patients and the kystobiliary fistula was identified in 68.75% (n=11).

A referential metaphor is a cohesive tie between lexical items that are metaphorically related, (e.g. "'The seagull' took the bread from the coffee table. No one heard the 'thief'"). The reference from "the thief" back to "the seagull" is metaphorical because thieves are human. The present article presents arguments and some empirical evidence that resolution of referential metaphors shares several of the core processes of general text comprehension. Two studies are reported. The first study found that resolution of referential metaphors is closely correlated to text comprehension and that ability to resolve referential metaphors contributes unique variance to reading comprehension over and above resolution of literal references. The goal of the second study was to explore in depth the inferential processes involved in the resolution of referential metaphors. This second study assessed the relative contributions of text-based ("bridging") and knowledge-based inferences to the resolution of referential metaphors. The results support the assumption that resolution of referential metaphors requires a combination of these two types of inferential processes in addition to other components that are shared with text comprehension. Together, the two studies indicate the processing of referential metaphors shares inferential processes with general reading comprehension and thus open new theoretical and practical perspectives.

ABSTRACT Glucose transporters are important for viability and infectivity of the disease-causing amastigote stages of Leishmania mexicana. The Δgt1-3 null mutant, in which the 3 clustered glucose transporter genes, GT1, GT2, and GT3, have been deleted, is strongly impaired in growth inside macrophages in vitro. We have now demonstrated that this null mutant is also impaired in virulence in the BALB/c murine model of infection and forms lesions considerably more slowly than wild-type parasites. Previously, we established that amplification of the PIFTC3 gene, which encodes an intraflagellar transport protein, both facilitated and accompanied the isolation of the original Δgt1-3 null mutant generated in extracellular insect-stage promastigotes. We have now isolated Δgt1-3 null mutants without coamplification of PIFTC3. These amplicon-negative null mutants are further impaired in growth as promastigotes, compared to the previously described null mutants containing the PIFTC3 amplification. In contrast, the GT3 glucose transporter plays an especially important role in promoting amastigote viability. A line that expresses only the single glucose transporter GT3 grows as well inside macrophages and induces lesions in animals as robustly as do wild-type amastigotes, but lines expressing only the GT1 or GT2 transporters replicate poorly in macrophages. Strikingly, GT3 is restricted largely to the endoplasmic reticulum in intracellular amastigotes. This observation raises the possibility that GT3 may play an important role as an intracellular glucose transporter in the infectious stage of the parasite life cycle. IMPORTANCE Glucose transport plays important roles for in vitro growth of insect-stage promastigotes and especially for viability of intramacrophage mammalian host-stage amastigotes of Leishmania mexicana. However, the roles of the three distinct glucose transporters, GT1, GT2, and GT3, in parasite viability inside macrophages and virulence in mice have not been fully explored. Parasite lines expressing GT1 or GT2 alone were strongly impaired in growth inside macrophages, but lines expressing GT3 alone infected macrophages and caused lesions in mice as robustly as wild-type parasites. Notably, GT3 localizes to the endoplasmic reticulum of intracellular amastigotes, suggesting a potential role for salvage of glucose from that organelle for viability of infectious amastigotes. This study establishes the unique role of GT3 for parasite survival inside host macrophages and for robust virulence in infected animals.

L'iléus biliaire est une complication rare de la lithiase vésiculaire secondaire à une fistule cholécysto-duodénale avec passage d'un macrocalcul dans l'intestin grêle. L'occlusion est provoquée par le blocage du calcul au niveau de l'iléon. Nous rapportons le cas d'une patiente de 75 ans, présentant des vomissements avec des douleurs abdominales diffuses. L'IRM abdominal a objectivé une aérobilie importante, une vésicule biliaire atrophique associée à une occlusion grêlique terminale sur obstacle intraluminal. La laparotomie a objectivé la présence d'une fistule cholécystoduodénale, une vésicule biliaire scléro-atrophique avec la présence d'un calcul obstruant la lumière au niveau du grêle distal (à 1 mètre de la valvule de Bauhin); une enterolithotomie a été réalisé avec une cholécystectomie et fermeture du trajet fistuleux en un seul temps opératoire; les suites postopératoires ont été simples.

Background: Despite the importance of the postnatal consultation, in Morocco, only 22% of women attended these consultations. The aim of this study was to identify associated factors with these consultations and offer suggestions to improve their use. Methods: This study was conducted in 2014 in Marrakech. A sample of women in reproductive age (15-49 yr) giving birth during 2013 year was enrolled. They were examined in the public health centers for postnatal consultation or for the BCG. A descriptive and analytic cross-sectional survey was conducted. All participants (n=1029) provided consent before participating in the survey. A questionnaire makes it possible to collect information about socio-demographic, knowledge and perception of women regarding these consultations. Results: The proportion of women who attended a postnatal consultation was 30.1%. Lack of information (87%), lack of complications (68.6%); health professional poor reception (42%) and financial difficulties (3.3%) were the main reasons that hinder these consultations. In addition, women of rural origin, low education level, and low socioeconomic status are important determinants associated with non-use of postnatal consultation. Conclusion: This study confirmed the low rate of these consultations. Various determinants explain this fact. The fight against illiteracy, improving household living standards, sensitization of women on the importance of postpartum care, awareness and capacity building of health professionals in the postnatal consultation and communication, and the development of a system of home visits for non-users of postnatal care allow improving the postnatal consultation rate.

Abstract Pleuropulmonary blastoma (PPB) is a rare aggressive malignant tumor of infancy and early childhood. The tumor arises in the lung and pleura and is regarded as a pulmonary dysontogenetic or embryonic neoplasm. Four types are defined in literature. Type I PPB is a rare, cystic lung neoplasm in infants characterized by subtle malignant changes and a good prognosis. Recurrences after type I PPB are usually advanced with a poor prognosis. We report this case to increase awareness about this entity so that the pediatricians, pediatric surgeons, radiologist, and pathologist recognize it early.

We have studied the extracellular polysaccharide (EPS) produced by the type strain, M8T, of the halophilic bacterium Halomonas almeriensis, to ascertain whether it might have any biotechnological applications. All the cultural parameters tested influenced both bacterial growth and polysaccharide production. EPS production was mainly growth-associated and under optimum environmental and nutritional conditions M8T excreted about 1.7 g of EPS per litre of culture medium (about 0.4 g of EPS per gram of dry cell weight). Analysis by anion-exchange chromatography and high-performance size-exclusion chromatography indicated that the exopolysaccharide was composed of two fractions, one of 6.3 × 106 and another of 1.5 × 104 Daltons. The monosaccharide composition of the high-molecular-weight fraction was mannose (72% w/w), glucose (27.5% w/w) and rhamnose (0.5% w/w). The low-molecular-weight fraction contained mannose (70% w/w) and glucose (30% w/w). The EPS has a substantial protein fraction (1.1% w/w) and was capable of emulsifying several hydrophobic substrates, a capacity presumably related to its protein content. The EPS produced solutions of low viscosity with pseudoplastic behaviour. It also had a high capacity for binding some cations. It contained considerable quantities of sulphates (1.4% w/w), an unusual feature in bacterial polysaccharides. All these characteristics render it potentially useful as a biological agent, bio-detoxifier and emulsifier.
