The first was a 24-year-old lady with a recently diagnosed DM. She developed rapidly progressive pneumonia, interstitial lung disease (ILD), pneumomediastinum, subcutaneous emphysema, and acute respiratory distress syndrome (ARDS). She died despite treatment with steroids and immunosuppressants (methotrexate and mycophenolate mofetil (MMF)). The second was a 30-year-old man diagnosed with amyopathic DM. He developed pneumomediastinum prior to ILD, which worsened over time, and subcutaneous emphysema evolved. However, he recovered completely after corticosteroid, MMF, and rituximab.Conclusion: Spontaneous pneumomediastinum and subcutaneous emphysema may complicate DM. Corticosteroids, immunosuppressants, and respiratory support are the mainstay of management for these conditions. The reported cases of COVID-19-related dermatomyositis are heterogeneous in their clinical presentation and implemented therapies.Case Study: We report a 23-year-old female patient diagnosed with a 3-year history of dermatomyositis. She has been well-controlled on maintenance therapy. However, 6 weeks after a mild COVID-19 infection, she developed a dermatomyositis flare. She improved only after aggressive treatment with pulse steroids, intravenous immunoglobulin, and rituximab.Conclusion: Exacerbation of dermatomyositis can be encountered following a COVID-19 infection, even if the infection is mild. Aggressive therapy should be considered in such cases. The prognosis, however, is generally favorable.Keywords: COVID-19 infection, dermatomyositis, Rituximab

Saudi Arabia (SA) also encounters a limited geographical distribution of rheumatologists and rheumatology fellowship training centres.Objective: Reporting the Saudi rheumatologists’ advisory meeting conducted in Makkah, SA in January 2020 with the aim to discuss the “Saudi Vision 2030” for rheumatology training programs.Materials and Methods: A meeting of Saudi rheumatology experts and consultants was conducted to address the future directions, challenges, and recommendations of rheumatology training. The 10th Rheumatology Practice Symposium was organised by Alzaidi Chair of Research in Rheumatic Diseases (ZCRD), and conducted in Makkah Commerce Chamber, Makkah, SA on January 28, 2020. More than 30 consultants and rheumatology fellows with five Saudi experts in the field of rheumatology assembled to form 10 recommendations that tackle rheumatology training challenges in SA.Results: The meeting recommendations shed light on the clinical practice of rheumatology training in SA; challenges and opportunities in rheumatology fellowship programs; efforts of the Saudi Commission for Health Specialties (SCFHS) to design and implement a competent postgraduate rheumatology training; and challenges with trainers, trainee, and within training centres.Conclusion: To address rheumatology challenges in SA, rheumatology consultants and fellows assembled to form 10 recommendations. The recommendations tackled the challenges of rheumatology fellowship programs and the efforts to implement a competent postgraduate rheumatology training. Recently, wireless sensor network (WSN) applications have seen an increase in interest. In search and rescue, battlefield reconnaissance, and some other such applications, so that a survey of the area of interest can be made collectively, a set of mobile nodes is deployed. Keeping the network nodes connected is vital for WSNs to be effective. The provision of connectivity can be made at the time of startup and can be maintained by carefully coordinating the nodes when they move. However, if a node suddenly fails, the network could be partitioned to cause communication problems. Recently, several methods that use the relocation of nodes for connectivity restoration have been proposed. However, these methods have the tendency to not consider the potential coverage loss in some locations. This paper addresses the concerns of both connectivity and coverage in an integrated way so that this gap can be filled. A novel algorithm for simultaneous-node repositioning is introduced. In this approach, each neighbour of the failed node, one by one, moves in for a certain amount of time to take the place of the failed node, after which it returns to its original location in the network. The effectiveness of this algorithm has been verified by the simulation results.

Progress in cancer care has improved the survival rates of treated cancer patients. With patients living longer, radiation oncologists are more likely to manage patients who have had prior courses of adjacent or overlapping radiation. To facilitate and improve the safety of re-irradiation, the special physics consult (SPC) was adopted in certain clinics. SPCs are ordered by physicians and performed by physicists to address complex planning and physics considerations (including physics evaluation of patients with implantable devices, anatomical changes during treatment, and monitoring surface dose). Results are used to inform clinical decision-making in patients who may be at an increased risk of complications. This study aims to evaluate the clinical impact of using SPCs by evaluating the extent to which results can alter management. We evaluated 100 consecutive SPC orders entered over a 5-month period. The purpose for ordering the SPC and its impact on management were recorded for all cases. For re-irradiation SPCs, the dose from prior treatments was applied onto the current CT dataset using rigid or deformable registration, then all dose was scaled to equivalent dose in 2-Gy fractions (EQD 2) and summed. Patients with implantable devices were evaluated following the recommendations of AAPM TG 203. When in-vivo dosimetry was ordered, it was performed on patients using an optically stimulated luminescence dosimeter system. In the case of anatomical changes to a patient throughout the course of treatment, an adaptive plan was created using the daily imaging CBCT and the dose was compared to the originally planned dose. The results of all SPCs were reviewed with the ordering physician prior to final clinical decision-making. The majority of SPCs were ordered for re-irradiation cases (88.0%). SPCs were otherwise ordered to evaluate implantable devices (8.0%), adaptive replanning (3.0%) and in-vivo dosimetry (1.0%) as shown in table 1. Across all SPCs, 37.0% yielded one or more adjustments to the treatment plan. For re-irradiation cases, the SPC results produced one or more planning changes in 38.6% of cases. These changes include re-optimization (29.5%), target contour revisions (9.1%), replanning (5.7%), modality change (4.5%), OAR contour revisions (2.3%), and fractionation changes (1.1%). Our data shows that SPC results can impact management in more than a third of evaluated cases. Re-irradiation is the most common reason for ordering an SPC, highlighting it as a valuable tool for ensuring patient safety and quality of care. À ce jour, les informations sur l'impact de la pandémie de la COVID-19 sur la pratique de la rhumatologie et sur les rhumatologues sont limitées. L'objectif principal de l'étude était d'évaluer l'impact de la pandémie de la COVID-19 sur la pratique de la rhumatologie dans les pays arabes. Les objectifs secondaires étaient d'évaluer l'attitude des rhumatologues envers la télémédecine et de recueillir des propositions pour améliorer la pratique de la rhumatologie dans la région arabe. Il s'agit d'une étude observationnelle transversale électronique. Un questionnaire en ligne a été conçu par un groupe de travail de la Ligue arabe des associations de rhumatologie (ArLAR) et validé par le comité scientifique de l'ArLAR en anglais et en français. Le questionnaire a été diffusé par e-mail et sur les réseaux sociaux aux rhumatologues des 15 pays de l'ArLAR. Il comprenait 19 questions portant sur les caractéristiques démographiques, l'impact de la pandémie sur les activités (en pourcentage, où 100 % correspond à une suspension complète des activités), l'attitude envers la télémédecine, et deux questions ouvertes sur les besoins non satisfaits dans la pratique de la rhumatologie. L'analyse descriptive de l'ensemble des caractéristiques a été effectuée en effectifs et pourcentage. Une analyse univariée puis multivariée concernant l'étude de l'impact de la COVID19 sur la pratique rhumatologique ont été réalisés. L'analyse statistique a été réalisée à l'aide du logiciel SPSS. Un p < 0,05 était considéré comme statistiquement significatif. Entre le 9 et le 24 mai 2020, 858 rhumatologues ont été inclus dans l'analyse (27,3 % des rhumatologues inscrits dans les pays de l'ArLAR), 37 % étaient âgés entre 35 et 44 ans, 60 % étaient des femmes et 48 % travaillaient dans le secteur privé. L'impact de la pandémie COVID-19 sur la pratique de la rhumatologie était significatif, avec une diminution de 69 % de l'hospitalisation conventionnelle, de 65 % de l'activité de consultation externe, de 56 % des hospitalisations de jour dans les centres de perfusion et de 43 % des revenus. Un impact plus élevé sur l'activité clinique était associé à la région (l'impact le plus faible étant au Levant [53 %]), à une faible utilisation de la télémédecine et à l'impact sur le revenu. L'impact sur l'activité clinique était aussi négativement associé au travail dans le secteur privé (en comparaison avec le secteur public et les hôpitaux universitaires, OR : 0,48 [IC95 % : 0,29–0,78]). La télémédecine a été utilisée dans 70 % des cas, mais principalement sous forme de contacts téléphoniques et d'e-mails, et n'a été remboursée que dans 12 % des cas. L'impact mental lié au stress causé par la pandémie COVID-19 a été rapporté chez 77 % des répondants (mineur dans 60,4 %, majeur dans 16,7 % des cas).Quinze rhumatologues (1,8 %) ont été personnellement infectés par le virus. Les participants ont signalé 156 cas de COVID-19 parmi leurs patients, dont 22 % sont décédés. Les besoins non satisfaits les plus cités étaient : l'accès aux médicaments et à une plateforme de télémédecine. La pandémie COVID-19 a eu un impact négatif significatif sur la pratique de la rhumatologie dans les pays arabes, entravant l'accès des patients aux soins et aux traitements, ce qui pourrait avoir un retentissement négatif significatif sur le contrôle de l'activité des rhumatismes chroniques. En conséquence, un meilleur accès aux traitements et aux plateformes de télémédecine serait recommandé pour améliorer la pratique de la rhumatologie dans la région. [ABSTRACT FROM AUTHOR]

Les premiers vaccins contre la COVID-19 ont été autorisés pour utilisation en décembre 2020. Cependant, leur acceptabilité reste controversée. Les objectifs de cette étude étaient d'estimer l'acceptabilité de la vaccination contre la COVID-19 chez les patients atteints de maladies rhumatismales chroniques et les professionnels de santé, et d'évaluer les facteurs associés à cette acceptabilité. L'étude ARCOVAX (Etude de la Ligue Arabe des Associations pour la Rhumatologie [ArLAR] sur la vaccination contre la COVID-19) a utilisé un questionnaire anonyme en ligne adapté de l'étude VAXICOV [1] , traduit en arabe par les auteurs et validé par le comité scientifique de l'ArLAR. L'enquête a été diffusée en ligne en trois langues : arabe, anglais et français entre le 13 avril et le 11 mai 2021, à travers les réseaux sociaux (Facebook, Twitter, Instagram, Snapchat, Messenger), des associations de patients, des courriels envoyés de la part de l'ArLAR et du groupe AAAA (Arab Adult Arthritis Awareness), des messages WhatsApp aux patients et aux médecins, et des invitations aux patients en utilisant un code QR affiché dans la salle d'attente. L'acceptabilité a été définie par : les participants déjà vaccinés ou désireux de se faire vacciner ; la non-acceptabilité a été définie par : les participants indécis ou refusant de se faire vacciner. Les données démographiques et de la maladie, les perceptions concernant les vaccins contre la COVID-19 ont été recueillies. Les facteurs associés à l'acceptabilité ont été analysés par deux modèles de régression logistique distincts pour les patients et les professionnels de santé. L'analyse a inclus 3176 participants de 19 pays Arabes (1594 patients et 1517 professionnels de santé). L'âge moyen des patients était de 39 ans, 73 % étaient des femmes et 18 % avaient une éducation universitaire. Vingt-neuf pour cent des patients étaient déjà vaccinés, contre 59 % des professionnels de santé. L'acceptabilité était significativement plus faible chez les patients (63 %) par rapport aux professionnels de santé (81 %), p < 0,001, et restait significativement plus faible en incluant uniquement les participants non vaccinés (48 % versus 55 %, p= 0,006). Parmi les patients non vaccinés, 57 % des indécis et 40 % de ceux qui refusaient le vaccin seraient prêts à se faire vacciner si cela était recommandé par leur médecin. Chez les patients et les professionnels de santé, l'acceptabilité était associée à un produit intérieur brut du pays plus élevé, à la perception qu'il est important d'être personnellement vacciné, à une vaccination antigrippale antérieure, à la peur de la COVID-19 et à des niveaux d'inquiétude plus faibles concernant les effets secondaires des vaccins. L'acceptabilité de la vaccination contre la COVID-19 reste sous-optimale chez les patients (63 %) et les professionnels de santé (81 %). Toutefois, cette acceptabilité pourrait être considérablement améliorée si le vaccin était recommandé par le médecin traitant. Tenir compte des principaux déterminants de l'acceptabilité, à savoir les perceptions vis-à-vis de la vaccination en général et les inquiétudes concernant les effets secondaires des vaccins, pourrait permettre d'optimiser la couverture vaccinale chez les patients et les professionnels de santé.
