Abstract

International audience; Dear Editor,A recent report questioned whether one vaccine was better than another and how to evaluate this difference, but only homologous vaccination schedules were considered. Concern about the safety of the ChAdOx1-S vaccine have prompted some European countries to recommend using a heterologous booster, as proposed for mRNA vaccines for people under 55 years old . Immunogenicity and reactogenicity of a BNT162b2 booster in ChAdOx1-S-primed subjects was assessed in a multicenter, open-label, randomised, controled phase 2 trial, but included no homologous vaccination comparator. Few studies have compared the immune responses to homologous and heterologous vaccinationschedules in humans.

International audience; A recent study in this journal reported that prior SARS-CoV-2 infection is protective even in the absence of a detectable humoral immune response.1 Our prospective longitudinal study was designed to measure the changes in the binding and neutralizing antibody titers in seropositive healthcare workers (HCWs) over a year and the changes in individuals who had and had not been vaccinated. We also assessed the incidence of positive SARS-CoV-2 RNA tests among HCWs who were seropositive for SARS-CoV-2 antibodies and in those who were seronegative.

38. Occupational asbestos exposure and incidence of colon and rectal cancers in French men: the asbestos-related diseases cohort (ARDCo-Nut)

Abstract

BACKGROUND: The relationships between asbestos exposure and colorectal cancer remain controversial. OBJECTIVES: We examined the association between asbestos exposure and colorectal cancer incidence. METHODS: Volunteer retired workers previously exposed to asbestos were invited to participate in the French ARDCo screening program between 2003 and 2005. Additional data on risk factors for colorectal cancer were collected from the ARDCo-Nut subsample of 3,769 participants in 2011. Cases of colon and rectal cancer were ascertained each year through 2014 based on eligibility for free medical care following a cancer diagnosis. Survival regression based on the Cox model was used to estimate the relative risk of colon and rectal cancer separately, in relation to the time since first exposure (TSFE) and cumulative exposure index (CEI) to asbestos, and with adjustment for smoking in the overall cohort and for smoking, and certain risk factors for these cancers in the ARDCo-Nut subsample. RESULTS: Mean follow-up was 10.2 years among 14,515 men, including 181 colon cancer and 62 rectal cancer cases (41 and 17, respectively, in the ARDCo-Nut subsample). In the overall cohort, after adjusting for smoking, colon cancer was significantly associated with cumulative exposure (HR = 1.14; 95% CI: 1.04, 1.26 for a 1-unit increase in In-CEI) and [greater than or equal to] 20-40 years since first exposure (HR = 4.67; 95% CI: 1.92, 11.46 vs. 0-20 years TSFE), and inversely associated with 60 years TSFE (HR = 0.26; 95% CI: 0.10, 0.70). Although rectal cancer was also associated with TSFE 20-40 years (HR = 4.57; 95% CI: 1.14, 18.27), it was not associated with ln-CEI, but these findings must be interpreted cautiously due to the small number of cases. 50. Une initiative originale de prise en charge spécifique de l’épuisement des professionnels des secteurs santé et médicosociaux : le centre PEPS porté par le CHU de Toulouse

Abstract

Le burn-out ou épuisement professionnel est un état d’épuisement physique, émotionnel et mental lié à une dégradation du rapport d’une personne à son travail. Il touche tous les métiers mais a initialement été observé chez les soignants. Il concerne particulièrement les professionnels très engagés dans le travail. Ce mal-être, amplifié par la pandémie COVID et les difficultés d’effectif de soignants et en l’absence de structures de prise en charge spécifique, le CHU de Toulouse a ouvert, en mai 2023, une nouvelle offre de soin aux professionnels des structures sanitaires, de soin et médicosocial à risque ou confrontés à l’épuisement professionnel. Le centre de prévention de l’épuisement professionnels des soignants (PEPS), issus du Centre régional de pathologies professionnelles et environnementales (CRPPE Occitanie) du CHU de Toulouse a vocation de proposer une prise en charge spécialisée et pluridisciplinaire (individuelle et/ou collective) afin de prévenir aux trois temps l’épuisement professionnel. Le bilan d’activité à 6 mois compte 557 consultations, médicales et psychologiques (psychologie clinique et spécialisée du travail), avec 106 soignants pris en charge, principalement des IDE (42), des AS (35), majoritairement des femmes (80 %) et pour syndrome anxiodépressif (47 %) et troubles anxieux (18 %). Cinquante ateliers collectifs ont été réalisés, en lien avec les différents temps de la prévention, en primaire et secondaire sur l’information et la sensibilisation à l’épuisement professionnelle et en tertiaire afin de développer ses ressources pour faire face à l’épuisement professionnelle et accompagner les soignants vers le retour dans l’emploi. Le centre PEPS est un dispositif novateur dans l’offre de soin par son approche clinique, notamment cognitivo-comportementale, et dans sa prise en charge individuelle et confidentielle mais aussi collective, avec un objectif principal de prévention primaire.
