Quantifying the impact of biochar on carbon persistence across soil textures is complex, owing to the variability in soil conditions. Using artificial soils with precise textural and mineral compositions, we can disentangle the effects of biochar from the effects of soil particle size. We can show that biochar application significantly reduces the early-stage carbon mineralization rates of plant residues in various soil textures (from 5 % to 41 % clay) but more significantly in sandy soils. Clay and silt particles alone also reduce C mineralization, but the magnitude of the changes is negligible compared to the impact of biochar. This finding suggests that biochar can compensate for the lack of clay in promoting C persistence in soil systems. This short report contributes substantially to understanding soil texture and biochar application interactions.

Improper treatment of antibiotics in wastewater has led to the spread of antimicrobial resistance. One of the solutions to this global problem is the Bio-Fenton reaction, which is effective in removing antibiotics. To stimulate Bio-Fenton reaction, magnetite fine particles are employed in the advanced activated sludge process. While combining the activated sludge process with membrane filtration can be more efficient, membrane bioreactors (MBRs) require frequent cleaning to prevent membrane fouling, inducing chemical and energy costs. To address this issue, a magnetic filter to capture activated sludge flocs with the magnetic particles was equipped near the MF (microfiltration) membrane surface to mitigate membrane fouling. The objective of this study is to assess the effectiveness of the magnetic filter in a lab-scale advanced Bio-Fenton MBR system. Transmembrane pressure (TMP) was measured as an indicator of the fouling extent, and the effect of the magnetic filter was evaluated by examining TMP change, supported by SEM (Scanning electron microscopy) analysis of foulant composition. Results showed lower TMPs and less Fe composition in the foulants with magnetic filter across various conditions. From the results, we concluded that the magnetic filter was effective for alleviating membrane fouling in the advanced Bio-Fenton MBR system.

Objectives: Effective treatments for diarrhea-type irritable bowel syndrome (IBS-D) are limited. Hangeshashinto (HST), an anti-inflammatory Kampo medicine, may offer benefits but its efficacy for IBS-D requires further investigation. This study evaluated IBS-D symptom improvement and gut microbiota changes following HST administration. Methods: This was a multicenter retrospective study with a prospective analysis of microbiota conducted at five affiliated institutions. Patients diagnosed with IBS-D based on the ROME IV criteria between April 2019 and December 2023, who received HST 7.5 g/day for 2-3 weeks were included. The outcome measures were improvement rates in overall symptoms, stool frequency, stool consistency, and abdominal pain. Intestinal microbiota was analyzed using 16S rRNA gene sequencing from fecal samples of 20 patients before and after HST treatment. Results: One hundred patients (42 males/58 females, mean age: 69.5±11.8 years) were analyzed. The overall improvement rate of HST was 82.0%. Those of males and females were 81.0% and 82.8% (p=0.816). Background: Glenoid bone deficiency can lead to early component loosening and implant failure during reverse total shoulder arthroplasty (rTSA). Recently, the glenoid Vault Reconstruction System (Zimmer-Biomet, Warsaw, IN, USA), a computer-aided design ot computer-assisted manufacturing system, was developed, with good clinical outcomes, including no radiographic loosening. This study examined the postoperative accuracy of glenoid component placement using this system at three different facilities. Methods: Nine patients undergoing rTSA with vault reconstruction system performed by three board-certified, fellowship-trained shoulder surgeons at three different institutions between August 2020 and January 2023 were included. Preoperative and postoperative computed tomography was performed, and glenoid inclination and version were measured using a postoperative three-dimensional evaluation system. Surgical time and intraoperative blood loss were also measured. Results: The range of errors of glenoid inclination and version were 3.5 ± 2.5° (0.4–8.3) and 3.2 ± 2.2° (0.4–6.7), respectively. In primary cases, the error ranges of both glenoid inclination and version were within 5° in six of seven cases (85.7%). In revision cases, both glenoid inclination and version were within 10°. The mean operative time was 131.4 ± 48.9 (80–206) min and the mean intraoperative blood loss was 161.1 ± 94.2 (30–300) ml; there were no intraoperative complications. Conclusion: In the present study, the placement position was good in primary and revision cases, making the placement of the glenoid component of the rTSA using vault reconstruction system in cases of glenoid bone deficiency highly reproducible.

Abstract UBA1 is an E1 ubiquitin-activating enzyme that initiates the ubiquitylation of target proteins and is thus a key component of the ubiquitin signaling pathway. Three disorders are associated with pathogenic variants of the UBA1 gene: vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic (VEXAS) syndrome, lung cancer in never smokers (LCINS), and X-linked spinal muscular atrophy (XL-SMA, SMAX2). We here report a case of infantile respiratory distress syndrome followed by continuing neuromuscular symptoms. We identified a de novo hemizygous mutation, c.1660 C > T (p.Pro554Ser), in exon 15 of the UBA1 gene in this baby. This missense mutation was located with the AAD (active adenylation domain) of the protein, a known hotspot of SMAX2 mutations. This case lends support to the genotype-phenotype correlation regarding the UBA1 mutation and its related diseases.

Summary: As blood flow to the reverse sural artery flap (RSAF) occurs via the peroneal artery, the health of the peroneal artery is generally considered important. In severe limb injuries, the main trunk of the peroneal artery is often disrupted at the fracture site, which brings uncertainty in the use of RSAF. We hypothesized that RSAF could be used even in cases where the main trunk of the peroneal artery is interrupted if retrograde blood flow from the plantar side forms a communication network with the superficial sural artery. Therefore, we performed RSAF in cases where the blood flow of the communicating branch could be confirmed. This study included patients who underwent RSAF when the main trunk of the peroneal artery was interrupted by trauma. Patient demographics, characteristics of injury, and treatment course were obtained from medical records. The outcome of this study included flap survival and complications, such as partial necrosis or flap infection. Five limbs that underwent RSAF met the inclusion criteria. In all cases, the skin flaps survived and soft tissue reconstruction was completed. Infection and partial necrosis of the skin flaps were observed in 2 cases each. This study showed that the RSAF can be used, even in cases where the main trunk of the peroneal artery had been interrupted, if retrograde blood flow from the plantar side forms a communication network with the superficial sural artery.

ABSTRACT The principal causes of death among 68,555 patients with diabetes and 164,621 patients without diabetes who died in 208 hospitals throughout Japan between 2011 and 2020 were determined based on a survey of hospital records. The most frequent cause of death in patients with diabetes was malignant neoplasms (38.9%) (lung 7.8%, pancreas 6.5%, liver 4.1%), followed, in order of descending frequency, by infectious diseases (17.0%) and then vascular diseases (10.9%) (cerebrovascular diseases 5.2%, ischemic heart diseases 3.5%, renal failure 2.3%). The proportion of deaths from malignant neoplasms and vascular diseases has trended upward and downward, respectively. Almost all deaths from ischemic heart diseases were due to myocardial infarction, and the proportion of deaths from heart diseases other than ischemic heart diseases was relatively high (9.0%), with most cases due to heart failure. Diabetic coma associated with hyperglycemia accounted for only 0.3% of deaths. The proportion of deaths from malignant neoplasms, infectious diseases, renal failure, ischemic heart diseases, and heart failure was significantly higher in patients with diabetes than in those without diabetes, and the proportion of deaths from cerebrovascular diseases was significantly lower in patients with diabetes. With regard to the relationship between the age and cause of death in patients with diabetes, malignant neoplasms were the most frequent cause of death in all age groups, and the incidence was around 50% for those in their 50s and 60s. The incidence of death due to infectious diseases was highest in patients older than their 70s. The incidence of death due to vascular diseases for patients in their 40s and 50s was higher than that due to infectious diseases. The highest incidence of death due to ischemic heart diseases was observed for patients in their 40s, and that due to renal failure and heart failure in patients older than their 70s. Compared with patients without diabetes, patients with diabetes demonstrated a higher incidence of death due to pancreatic cancer, infectious diseases, renal failure, ischemic heart diseases, and heart failure, and a lower incidence of death due to cerebrovascular diseases in all age groups. The average age at death of patients with diabetes was 74.4 years old in men and 77.4 years old in women, which were lower than the average lifespan of the Japanese general population in 2020 by 7.2 and 10.3 years, respectively. However, these differences were smaller than in previous surveys. The average age at death due to all causes, especially due to ischemic heart diseases, cerebrovascular diseases, heart failure, infectious diseases, and diabetic coma, was lower in patients with ‘poorer’ glycemic control than in those with ‘better’ glycemic control. In the total survey population, the average age at death of patients with diabetes was significantly higher than that of patients without diabetes. The average age at death due to malignant neoplasms and cerebrovascular diseases was higher in patients with diabetes than in those without diabetes and that due to renal failure, ischemic heart diseases, and infectious diseases was lower in patients with diabetes than in those without diabetes.

When hexapod robots walk on uneven terrain, they need to avoid the legs from collisions with obstacles, their own legs, and the ground, while also considering joint limits and reaching their feet to a destination. To address these issues, several local controls are defined in this paper for the swing legs in the Follow-the-Contact-Point (FCP) gait, which are integrated with Riemannian Motion Policies (RMPs). RMPs allow for multiple local control policies to be considered simultaneously for different points on the leg frames. For the local controls, a range sensor is installed on the foot to prevent dragging and contacting the ground firmly, even if the destination is in the air. Additionally, linear position sensors are equipped around the tibia to detect and avoid obstacles. The effectiveness of the proposed method is confirmed in walking experiments on flat and uneven terrain.

A 29-year-old man with diabetic nephropathy presented with fever and chills 4 days postdischarge following hospitalization for hyperglycemia. Abdominal computed tomography revealed a splenic abscess. Percutaneous drainage was performed, and intravenous meropenem was administered. Subsequent culture of the drained abscess identified Lancefieldella rimae. Based on the antimicrobial susceptibility results, the patient was switched to oral levofloxacin. This combined treatment led to the resolution of the abscess, with no recurrence after 6 months. This is the first case of a splenic abscess caused by L. rimae successfully managed by prompt percutaneous drainage and appropriate antibiotics.

SPring-8-II is a major upgrade project of SPring-8 that was inaugurated in October 1997 as a third-generation synchrotron radiation light source. This upgrade project aims to achieve three goals simultaneously: achievement of excellent light source performance, refurbishment of aged systems, and significant reduction in power consumption for the entire facility. A small emittance of 50 pm rad will be achieved by (1) replacing the existing double-bend lattice structure with a five-bend achromat one, (2) lowering the stored beam energy from 8 to 6 GeV, (3) increasing the horizontal damping partition number from 1 to 1.3, and (4) enhancing horizontal radiation damping by installing damping wigglers in long straight sections. The use of short-period in-vacuum undulators allows ultrabrilliant X-rays to be provided while keeping a high-energy spectral range even at the reduced electron-beam energy of 6 GeV. To reduce power consumption, the dedicated, aged injector system has been shut down and the high-performance linear accelerator of SACLA, a compact X-ray free-electron laser (XFEL) facility, is used as the injector of the ring in a time-shared manner. This allows the simultaneous operation of XFEL experiments at SACLA and full/top-up injection of the electron beam into the ring. This paper overviews the concept of the SPring-8-II project, the system design of the light source and the details of the accelerator component design.

Abstract Background The relationship between the combination of platelet count and neutrophil‐lymphocyte ratio (COP‐NLR) and prognosis in patients with advanced non‐small cell lung cancer (NSCLC) treated with immune checkpoint inhibitor (ICI) combination therapy with chemotherapy remains unclear. Thus, we investigated prognostic factors, including the COP‐NLR, to identify patients who could benefit from the therapeutic efficacy of ICI combination therapy for advanced NSCLC. Furthermore, we evaluated the relationship between the COP‐NLR score during ICI combination therapy and treatment response. Methods We conducted a retrospective cohort study of 88 patients with NSCLC who initially received ICI combination therapy. The primary outcome was overall survival (OS). The prognostic factors were extracted using the Cox proportional hazards model. The relationship between COP‐NLR score at 3 weeks after starting ICI combination therapy and a good response (complete response [CR] and partial response [PR]) to treatment was analyzed using the chi‐square test. Results The median OS was 15.7 months. In the multivariable analysis, Eastern Cooperative Oncology Group Performance Status (ECOG PS) 2, distant metastatic sites ≥2, and baseline COP‐NLR scores of 1, 2 were extracted as significant poor prognostic factors. The proportion of patients with CR and PR in the 3‐week COP‐NLR score of 0 group was significantly higher than that in scores of 1, 2 group. Conclusions Baseline COP‐NLR, ECOG PS, and number of distant metastatic sites were prognostic factors in patients with NSCLC with ICI combination therapy. A lower 3‐week COP‐NLR was associated with a good response to treatment.

Objectives: To date, there have been no reports on the long-term effects of a method that combines external hemorrhoidectomy with aluminum potassium sulfate and tannic acid (ALTA) sclerotherapy. This study aimed to investigate the efficacy and safety of external hemorrhoidectomy combined with ALTA sclerotherapy (EA) in reducing postoperative complications associated with conventional hemorrhoidectomy. Methods: EA was performed under sacral epidural anesthesia, and ALTA sclerotherapy was applied to the remaining internal hemorrhoids after resection of the external hemorrhoids. Mixed internal and external hemorrhoids were treated with EA, whereas internal hemorrhoids without external hemorrhoids were treated with ALTA sclerotherapy. The three EA patterns were defined as EA1, EA2, and EA3, depending on the number of EAs performed. Results: This study included 3,403 patients who underwent EA for grade II-IV mixed hemorrhoids. EA1 was the most common, with 1,789 (52.6%) cases, followed by EA2 (36.2%) and EA3 (11.2%). Postoperative complications occurred in 120 (3.5%) patients and increased with the number of EAs, with fever and bleeding being common in the early postoperative period, and perianal abscess and/or fistula being common after 1 month. The mean postoperative follow-up period was 26.4 months, and 136 (4.0%) patients underwent reoperation for recurrent internal or mixed hemorrhoids. The reoperation rate for EA1 was significantly higher than that for EA2 and EA3. The 5- and 10-year cumulative recurrence-free rates were 92.5% and 67.2%, respectively. Conclusions: EA is as curative as conventional hemorrhoidectomy and reduces postoperative complications. Therefore, EA is the preferred procedure for patients with mixed hemorrhoids.
