Summary: COVID‐19 is an infectious disease caused by severe acute respiratory coronavirus 2 (SARS‐CoV‐2). Vesicular skin rashes have been reported as associated with COVID‐19, but there is little information about this cutaneous manifestation. We designed a prospective observational study of patients diagnosed with COVID‐19 who had vesicular lesions. Clinical characterization of skin findings was conducted by dermatologists. When possible, histological analysis and detection of SARS‐CoV‐2 in the content of the vesicles was performed. In total, 24 patients were included. A disseminated pattern was found in 18 patients (75%), and a localized pattern was found in 6 (25%). Median duration of the skin rash was 10 days. Of the 24 patients, 19 (79.2%) developed the skin rash after the onset of COVID‐19 symptoms. Histological examination in two patients was consistent with viral infection, SARS‐CoV‐2 was not detected in four patients. Disseminated herpes simplex virus (HSV) is mainly seen in immunocompromised individuals. Atypical lesions can be present in both primary infection and reactivation disease. Compared with the general population, inmunocompromised hosts are at greater risk of increased persistency and severity of clinical manifestations, including severe systemic involvement such as esophagitis, meningitis, and hepatitis. Herein, we report the case of a liver transplant recipient with atypical disseminated herpes simplex virus-1 complicated by HSV-related hepatitis. The coronavirus disease 2019 (COVID-19) has been associated to a wide clinical spectrum of skin manifestations, including chilblain-like, urticarial, vesicular, maculopapular, livedoid and vasculitic lesions, among others.1,2 However, the exact pathophysiology for the appearance of skin lesions is still unknown. This delayed injection-site reaction shows similar features to COVID-19 exanthems.3 Whether it corresponds to a hypersensitivity reaction to the spike protein or to different components of the vaccine is still unknown. Secondary syphilis is a polymorphic condition resulting from the hematogenous and lymphatic dissemination of Treponema pallidum. Human immunodeficiency virus-positive patients are in greater risk of atypical and severe forms of the disease. The most common manifestation is a generalized papulosquamous eruption with variable mucosal involvement. However, annular plaques, split commissural papules, crusted necrotic or ulcerated lesions are also frequent. Granulomatous lesions, both clinically and histologically, are less common and present a diagnostic challenge. We describe a case of secondary syphilis presenting as a granulomatous rash with both clinical and histological features of tuberculoid leprosy. Chilblain-like lesions (CLL) were described early on the coronavirus disease 2019 (COVID-19) pandemic as red-to-violet macules, plaques or nodules typically appearing at the distal aspects of toes.1 Although increasing evidence suggests they are COVID-19-related,2 it is not supported by analytic controlled studies. Are SARS-CoV-2 IgA antibodies in paediatric patients with chilblain-like lesions indicative of COVID-19 asymptomatic or paucisymptomatic infection? We have read with great interest the article: Cutaneous manifestations in COVID-19: a first perspective by Recalcati S.[1] This article is the first report of the cutaneous manifestations in Coronavirus Disease 2019 (COVID-19) patients during the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic in Lombardy, Italy. Every medical and surgical specialty was integrated in this unit to provide assistance in the medical wards, overcrowded with COVID-19 patients. From a 1099 cohort of confirmed COVID-19 patients, only 2 presented "skin rash" without any further description.[5] Dermatologists have a unique opportunity to make our contribution during this pandemic and adequately describe skin manifestations of COVID-19. We read with interest the article by W. Chen,[1] where they hypothesize that juvenile gangrenous vasculitis of the scrotum (JGVS) may be the male counterpart of Lipschütz ulcers (LU). JGVS has been used to depict the abrupt appearance of scrotal ulcerations in young males, preceded by fever and pharyngeal symptoms, with no abnormalities in laboratory and microbiologic studies and non-specific signs of vasculitis in histological examination. Although difficult to estimate the rate, recurrence of LU is described.[[1], [3]] One of our three patients presented a recurrent scrotal ulcer after 4 years, with no association with EBV or streptococcal pharyngitis. The objective of this study was to describe FAV findings in demodex-related skin diseases and correlate them with RCM. RCM is considered superior to SSSB in demodex mite detection.4 Fluorescence-advanced videodermatoscopy (FAV) is a novel non-invasive method, for in vivo skin examination at very high magnification (500×).5 The working principle of FAV is based on the ability of endogenous molecules to absorb the 405 nm (±5 nm) monochromatic light emitted by the probe, and afterwards emit fluorescence.
