Undocumented college students in the United States encounter a number of structural barriers to postsecondary education success, including disparate in-state resident tuition (ISRT) policies across the country. Texas, the first state to establish ISRT benefits for undocumented college students, has been a site of tension respective to this issue over the last 20 years. In fact, there have been eight legislative attempts to repeal the state's affirmative ISRT policy. In order to investigate this ongoing ISRT debate in Texas, we used critical discourse analysis methods to analyze the implicit and explicit messages communicated in the policy and surrounding policy discourse. Our conceptual framework, grounded in three constructs of critical whiteness studies including ontological expansiveness, color evasiveness, and individualization, allowed us to uncover whiteness as a pernicious undergirding force within this policy discourse.

The higher education community often views international students through a homogenous lens. To challenge the cultural norms set by the dominant group, researchers need to explore how these norms affect international teaching assistants (ITAs). The following questions guided the current study: (a) How do ITAs construct intersecting identities of teacher and learner that reflect the presence of dominant cultural norms within a predominantly White institution? (b) What strategies do ITAs use to navigate cultural and linguistic power dynamics within a predominantly White institution as they seek to establish authority? We conducted a case study through an intersectionality framework. Findings revealed participants' marginalization, authority, and strategies to overcome oppression. We offer recommendations about power dynamics that require increased institutional support.

The categorization of predominantly white institutions (PWIs) and minority serving institutions (MSIs) affects the distribution of resources, privileges, and inequities in higher education. As PWIs increasingly tout having "majority-minority" student bodies and take on enrollment-based MSI statuses due to demographic shifts, it remains unclear how these institutions will disrupt the dominant ideologies of privilege and whiteness that have been pervasive throughout their history in order to create positive campus environments for people of color. Using Ray's ("American Sociological Review," 84(1), 26-53, 2019) theory of racialized organizations as a sociological framework to test these dynamics, we aimed to interrogate the organizational identity of PWIs transitioning to MSI. Through a comparative content analysis of existing PWI and MSI literature, we established the dominant features of each institution type and provided implications of the power dynamics as PWIs increasingly take on MSI characteristics. Given that enrollment-based MSIs gain eligibility to apply for federal grant funding, regardless of intentions to better serve students of color, the ways that PWIs are privileged throughout the literature have implications for institutional actors, policymakers, and scholars alike.

Through a narrative inquiry study, we sought to understand how 12 student affairs professionals (SAPs) experienced whiteness and perceived the influence of whiteness on campus responses to racialized incidents. We used components of critical whiteness studies (CWS) and critical race theory (CRT) as a theoretical underpinning to disrupt the dominant narratives that center whiteness in practices, policies, and spaces at predominantly white institutions. We used a critical resistance analysis (CRA) process to demonstrate how SAPs' narratives served as counternarratives, antenarratives, and resistance to institutional actors who refuse to acknowledge and name racism. Our findings illuminate whiteness as a racial ideology shedding light on the way whiteness is normalized, advantaged by the dominant group, and perpetuated through color evasive and race-neutral responses to racialized incidents. Several implications related to this work are described as there is a need to openly name racialized incidents that occur on campus and create safe spaces for SAPs of Color and Students of Color. Additional recommendations for theory and future research are offered.

Community College Men of Color Initiatives (CCMCIs) are increasingly popular strategies utilized to promote retention and completion among Men of Color (MOC). CCMCI directors offer a unique positionality in understanding how their identities, institutional contexts, and power dynamics affect their design of programs and participating students. Using a critical phenomenological approach, the authors of this study explore the lived experiences of CCMCI directors in conceptualizing, implementing, and assessing these initiatives.

Hispanic-serving institutions (HSIs) increasingly provide educational opportunities for underrepresented groups such as Latinx students; however, their categorization as minority-serving institutions is a function of their enrollment data rather than an explicit commitment to serving specific cultural needs. The authors of this study add to the literature on HSIs by further recognizing the need for community colleges to delve further into the different experiences of Latino males based on the intersection of race and gender. This study as part of the Texas Education Consortium for Male Students of Color explored how HSI community college might express a commitment to serving Latino males through the design and implementation of programming. The authors utilized a framework including concepts of HSI organizational identity, norms of racism in higher education, and gendered racism to conceptualize a Latino male-serving organizational identity. Findings revealed that colleges should go beyond Latino male outcomes to create a culture of commitment embedded throughout the college and not just isolated to men of color programming. This research has implications for the significant role that HSI community colleges can take in becoming leaders in serving Latino males through a purposeful and intentional commitment.

Background: Sacituzumab govitecan (sacituzumab) emerged as an important agent in metastatic and locally recurrent HER2‐negative breast cancer treatment. UGT1A1 polymorphisms have also been shown to predict sacituzumab toxicity. Methods: In this retrospective study, we sought to evaluate the associations between UGT1A1 status, toxicity, and therapeutic outcomes in sacituzumab recipients with advanced breast cancer who underwent genotype testing for UGT1A1 alleles (N = 68). Results: We found 17 (25%) of our patients to be homozygous for UGT1A1*28 and 24 (35.3%) were heterozygous. Of seven African American patients with triple‐negative breast cancer, five were homozygous for UGT1A1*28 and two were heterozygous. Patients with a homozygous UGT1A1*28 genotype were significantly more likely to have treatment terminated because of adverse effects. However, the polymorphism was not associated with treatment discontinuation because of disease progression. Conclusion: This retrospective, real‐world analysis suggests potential clinical utility in UGT1A1 testing for patients receiving sacituzumab, but future trials are needed to confirm the association between genotypes and treatment outcomes. Program directors at community colleges must navigate institutional rhetoric to effectively support Men of Color. This study considers how administrators often exhibit a non-performative commitment to diversity, in that stated commitment might not equate to action. Utilizing a framework grounded in critical race theory and cognitive frames regarding diversity, deficit, and equity, the authors of the study interviewed directors of men of color programs to explore their administrators' commitments to support the work. They also explored the ways that diversity rhetoric affected the success of their programs. Findings revealed that rhetoric often took the place of tangible action, that diversity rhetoric might contradict directors' experiences, and that directors regularly acted as commodities to do the work of diversity with little support. Through this research, the authors offer several implications related to the need for more explicit institution-wide practices that center on equity-mindedness and move past generic frames about diversity. Simple Summary: There is increasing evidence that Type-2 diabetes and pre-diabetes may increase cancer risk; however, the biology that links these diseases is poorly understood. Epigenetics is the biological study of how DNA can be modified without mutations; epigenetic changes link medical, social, and environmental changes with cancer. Here, we investigated whether insulin resistance (pre-diabetes) may result in epigenetic changes that increase cancer risk. We discovered that women with insulin resistance have epigenetic changes that increase inflammation and perhaps accelerated aging. Our study is important because the inflammatory changes we see are associated with an increased risk for heart disease, kidney disease, and cancer. Background: Epigenetic changes link medical, social, and environmental factors with cardiovascular and kidney disease and, more recently, with cancer. The mechanistic link between metabolic health and epigenetic changes is only starting to be investigated. In our in vitro and in vivo studies, we performed a broad analysis of the link between hyperinsulinemia and chromatin acetylation; our top "hit" was chromatin opening at H3K9ac. Methods: Building on our published preclinical studies, here, we performed a detailed analysis of the link between insulin resistance, chromatin acetylation, and inflammation using an initial test set of 28 women and validation sets of 245, 22, and 53 women. Results: ChIP-seq identified chromatin acetylation and opening at the genes coding for TNFα and IL6 in insulin-resistant women. Pathway analysis identified inflammatory response genes, NFκB/TNFα-signaling, reactome cytokine signaling, innate immunity, and senescence. Consistent with this finding, flow cytometry identified increased senescent circulating peripheral T-cells. DNA methylation analysis identified evidence of accelerated aging in insulin-resistant vs. metabolically healthy women. Conclusions: This study shows that insulin-resistant women have increased chromatin acetylation/opening, inflammation, and, perhaps, accelerated aging. Given the role that inflammation plays in cancer initiation and progression, these studies provide a potential mechanistic link between insulin resistance and cancer. [ABSTRACT FROM AUTHOR]

Background: Tooth loss is associated with suboptimal nutrient intake and greater risk of malnutrition. Objective: To develop and field‐test a stakeholder‐informed diet education tool that addresses the unique needs of older adults with tooth loss who do not wear dentures. Methods: An iterative user‐centered approach was used. Initial content was developed based on findings from previous research. Stakeholder panels of older adults with 20 or fewer teeth, and dentists, were conducted at two time points to obtain feedback on the tool, which was revised following each panel. The tool was field‐tested in a dental school clinic and evaluated using the Patient Education Materials Assessment Tool; it was further revised based on feedback. Results: A diet education tool entitled "Eating Healthier With Tooth Loss" was developed. Sections for fruits and vegetables, grains, and proteins food groups, and one addressing socioemotional aspects of eating with missing teeth were included. Panel members provided constructive, positive feedback; recommendations for editing text, images, design, and content were integrated. Field‐testing in the dental clinic with 27 pairs of student dentists and their patients resulted in scores of 95.7% for understandability and 96.6% for actionability, with over 85% agreement with each item. The tool was revised based on field‐testing feedback. Conclusion: A diet education tool for older adults with tooth loss was developed using a user‐centered approach, integrating the 'patient voice' and patient experiences with US dietary guidelines. Universities increasingly call upon employees to educate campus community members on diversity, yet the experiences of these educators are rarely addressed. Via scholarly personal narratives, a team of diversity educators at a predominantly White research university shared their experiences facilitating diversity trainings while attempting to maintain self-care, including their processes of (a) navigating resistance through intersectional identities, (b) proving legitimacy as diversity educators, (c) experiencing burnout, and (d) validating and supporting each as other cofacilitators.

The national prevalence of systemic bias, such as police brutality, has influenced resistance to institutionalized racism on college campuses. Because of the daily microaggressions Black students experience at predominantly White institutions, they often utilize various forms of engagement and activism to hold institutions accountable for equity in higher education. Through this study the authors examined how Black student leaders challenge inequitable policies through their individual as well as collective efforts. Using a social identity framework and case study methodology, Black student leaders' reflections on their strategies in invoking change on campus are explored. Through heterogeneous approaches of engagement and activism, participants called attention to the greater need of communities of color being included in institutional reform.

Islamophobia has become increasingly evident in the sociocultural landscape of the United States. The current political climate which centers on the influx of refugees and concerns of extremists has in effect othered individuals of Arab ancestry as a bounded group. Arab students represent a heterogeneous group of individuals, encompassing a variety of viewpoints that challenge the fixed and static notion of Arab identity. This study utilizes the concept of racialization to explore the ways that a group of Arab students within a predominantly white institution in the southern region were othered through the influence of a negative Arab image. Through case study methodology including interview and observational data, participants explained the multiple ways they were distanced and connected to the dominant group as well as other Arab students. Related to the ways participants navigated dominant norms to establish a sense of belonging, findings reveal that institutional leaders and practitioners need to engage in greater critical reflection to maximize support for marginalized groups.

The anti-critical race theory movement has caused educators strife and fear nationally. In these current socio-political times, faculty are navigating unprecedented experiences of racism as they teach and research critical race theory. Through our work, we examined 40 faculty experiences challenging legislation at a national, state, and institutional level. We present recommendations for practice and policy that resist the surveillance experienced during this socio-political backlash. [ABSTRACT FROM AUTHOR]

Background: Recent incidents of racism at predominantly White institutions (PWIs) have gained increased national attention. The backlash to individuals speaking out against racialized practices is often masked through discourse that dismisses the adverse effects of racism. Because university administrators often center their responses to incidents of racism on upholding free speech, scholars should analyze the ways that administrators' responses might reinforce the existence of such racist behaviors and affect marginalized students. Purpose and Research Questions: Rather than placing the burden on students to disrupt institutionalized racism, the author critically analyzed the discourse administrators utilized in their responses to understand the role of power in language. The following research questions informed the study: (a) what are the various characteristics of the discourse of university administrators as they respond to incidents of racism? and (b) how do university administrators' responses to racism support or disrupt larger patterns of social power and privilege? Research Design: The author utilized critical discourse analysis (CDA) to deconstruct relationships between the speech patterns of university presidents and larger Discourses about social power. Through a process of description, interpretation, and explanation, the author sought to reveal the underlying ideologies that go beyond surface-level discourse about free speech. Data Collection and Analysis: Based on the context of increased police brutality and student protests on college campuses, the author reviewed data on recent incidents of racism at PWIs. The three final cases chosen for analysis represented varying approaches utilized by administrators to respond to racist incidents. Through multiple phases of coding, the author interpreted relationships between textual patterns to reveal a larger narrative about administrative accountability. Findings: Each university president utilized a different approach to campus racism. The major discourse patterns represented were (a) a direct reproach of individuals as accountable for racism; (b) an indirect and theoretical approach to the reality of racism; and (c) a denial of or diversion from racism through authority. Conclusions and Recommendations: Across the cases, administrators utilized speech that either downplayed the existence of racism or defined it through privilege or colorblind ideology. With the last incident resulting in the firing of the president, the analysis revealed ways that presidents can utilize emotional speech without substantial action. In order to be more responsive to marginalized communities, administrators need to intentionally engage with marginalized groups who are often silenced because their beliefs do not fit the standard of the dominant norm.

This study explored the dual identity of Hispanic-serving research institutions (HSRIs) by highlighting how employees at four HSRIs perceived how both Hispanic-Serving Institutions and research-intensive statuses are reflected in the organization. Using the theory of racialized organizations (Ray, 2019), findings revealed how racialization occurs when research prestige becomes the dominant status, and certain individuals take on additional labor. Introduction: Cancer inequity is one of the most critical public health issues faced by ethnic minorities and people of lower socioeconomic status. The disparate burden of cancer is caused by poor access to care and inadequate delivery of cancer treatment, as well as comorbid and co-occurring conditions. Diabetes is a common and serious comorbid condition of cancer. Methods: To better understand diabetes prevalence among diverse cancer patients, this study analyzed and described characteristics of cancer patients with diabetes from local-level Service Planning Area (SPA) data using City of Hope Comprehensive Cancer Center data, and United States national-level data from The National Health Interview Survey. Results: Findings from national level data showed that patients in racial/ethnic minority groups had a higher occurrence of being diagnosed with diabetes, especially for non-Hispanic Blacks (OR=1.76, 95% CI=1.51, 2.03) and Hispanic/ Latino individuals (OR=1.34, 95% CI=1.18, 1.52). Cancer patients who are older, ethnic minority, overweight/obese and with lower educational levels were more likely to have co-occurring diabetes. SPA-level patient data found similar results. Discussion: In response to our findings and other reports, clinicians and health system including health coverage organizations should routinely assess cancer patients for cooccurring chronic illnesses, in particular diabetes. Interventions improving coordinated care that integrates oncology, endocrinology and primary care, targeting cancer patients -especially racial/ethnic minorities, overweight/obese, and older patients who are at increased risk for diabetes - ought to be considered as best practice Whole Person care. With coordinated care management, ethnic disparities in cancer may be better addressed and reduced. Additionally, policymakers can contribute by enacting policies improving access to and coverage of integrated oncology, chronic disease prevention, and associated specialty care i.e., endocrinology to equalize quality care for ethnic minority, lower educated, overweight/obese and older cancer patients who are more likely to suffer greater comorbidity, and inadequate oncology and coordinated care to reduce disparities. The article examines the role of evidence-based interventions to enhance the medical care of bacterial sexually transmitted infection among low-income people with or at risk of HIV. Topics mentioned include the importance of virtual training sessions for health care professionals, the implementation of welcoming clinical space indicators for LGBTQ+ community, and the creation of sexual history data and patient satisfaction data.

In response to rising rates of bacterial sexually transmitted infections (STIs) in the United States, this evaluative study of the implementation of four evidence-based interventions was developed and implemented. In three STI and HIV high-incidence jurisdictions of the United States, nine federally funded Health Resources and Services Administration Ryan White HIV/AIDS Program clinical demonstration sites implemented (1) audio computer-assisted self-interview sexual history taking, (2) patient self-collection of urogenital and extragenital site chlamydia/gonorrhea nucleic acid amplification test specimens, (3) sexual and gender minority welcoming indicators, and (4) provider training, to make STI screening, testing, and treatment routine in their HIV primary care clinics. The priority populations of young adults, men who have sex with men, and sexual and gender minority patients were found to have risk behaviors identified in the self-interview sexual history, to prefer to self-collect urogenital and extragenital site specimens for STI testing, and to notice and like the sexual and gender minority welcoming indicators. Testing positive for a bacterial STI was significantly associated with using alcohol or recreational drugs before sex, being younger than 50 years, and having two or more sexual partners with other concurrent sexual partners. Of 255 cases of chlamydia, gonorrhea, and syphilis infections, only 13.73% of patients reported related symptoms when screened and tested. [ABSTRACT FROM AUTHOR]

Invasive alien species (IAS) are a key driver of global biodiversity loss. Reducing their spread and impact is a target of the Sustainable Development Goals (SDG target 15.8) and of the EU IAS Regulation 1143/2014. The use of citizen science offers various benefits to alien species' decision-making and to society, since public participation in research and management boosts awareness, engagement and scientific literacy and can reduce conflict in IAS management. We report the results of a survey on alien species citizen science initiatives within the framework of the European Cooperation in Science and Technology (COST) Action Alien-CSI. We gathered metadata on 103 initiatives across 41 countries, excluding general biodiversity reporting portals, spanning from 2005 to 2020, offering the most comprehensive account of alien species citizen science initiatives on the continent to date. We retrieved information on project scope, policy relevance, engagement methods, data capture, data quality and data management, methods and technologies applied and performance indicators such as the number of records coming from projects, the numbers of participants and publications. The 103 initiatives were unevenly distributed geographically, with countries with a tradition of citizen science showing more active projects. The majority of projects were contributory and were run at a national scale, targeting the general public, alien plants and insects, and terrestrial ecosystems. These factors of project scope were consistent between geographic regions. Most projects focused on collecting species presence or abundance data, aiming to map presence and spread. As 75% of the initiatives specifically collected data on IAS of Union Concern, citizen science in Europe is of policy relevance. Despite this, only half of the projects indicated sustainable funding. Nearly all projects had validation in place to verify species identifications. Strikingly, only about one third of the projects shared their data with open data repositories such as the Global Biodiversity Information Facility or the European Alien Species Information Network. Moreover, many did not adhere to the principles of FAIR data management. Finally, certain factors of engagement, feedback and support, had significant impacts on project performance, with the provision of a map with sightings being especially beneficial. Based on this dataset, we offer suggestions to strengthen the network of IAS citizen science projects and to foster knowledge exchange among citizens, scientists, managers, policy-makers, local authorities, and other stakeholders. Simple Summary: The HER2 status of breast cancers is accurately determined by measuring HER2 protein overexpression and gene amplification. However, these clinical diagnostic tests cannot predict the response to therapy. Single molecule imaging approaches can quantify molecular features of HER2, such as receptor nano-organization, with exquisite spatial resolution and sensitivity. The aim of our study was to assess how the molecular features of HER2 varied with the therapy response. According to our results in cultured cell lines and six patient specimens, the therapy response was associated with high detected HER2 densities and clustering. This advanced imaging approach can thus provide key data to complement the current diagnostic standards. Trastuzumab, the prototype HER2-directed therapy, has markedly improved survival for women with HER2-positive breast cancers. However, only 40–60% of women with HER2-positive breast cancers achieve a complete pathological response to chemotherapy combined with HER2-directed therapy. The current diagnostic assays have poor positive-predictive accuracy in identifying therapy-responsive breast cancers. Here, we deployed quantitative single molecule localization microscopy to assess the molecular features of HER2 in a therapy-responsive setting. Using fluorescently labeled trastuzumab as a probe, we first compared the molecular features of HER2 in trastuzumab-sensitive (BT-474 and SK-BR-3) and trastuzumab-resistant (BT-474R and JIMT-1) cultured cell lines. Trastuzumab-sensitive cells had significantly higher detected HER2 densities and clustering. We then evaluated HER2 in pre-treatment core biopsies from women with breast cancer undergoing neoadjuvant therapy. A complete pathological response was associated with a high detected HER2 density and significant HER2 clustering. Background: Radial scars (RS) and complex sclerosing lesions (CSL) are breast radiologic findings described as small, stellate lesions causing architectural distortion. This can mimic malignancy. Core needle biopsy (CNB) is often performed. Advances in breast imaging have led to increased detection of RS/CSL. The upstage rate of RS/CSL to in situ or invasive disease is 0-40%. We sought to determine the upstaging rate of RS/CSL to in situ, invasive disease, or high-risk lesion at our institution to create excision guidelines.Methods: The pathology database of a single center was searched for RS/CSL, from January 2013 to September 2020. We included CNB without malignancy or high-risk lesion (eg, atypical ductal hyperplasia). Patient demographics, indications for biopsy, imaging findings, biopsy procedure, and final pathology were collected.Results: Forty-four patients were included. 52.3% had CNB for architectural distortion on mammography, 18.2% for mass, 11.4% for calcifications, 2.3% for abnormal MRI, and 15.9% for multiple reasons (eg, calcifications and mass). Most had an ultrasound: 43.2% had no abnormality and 34.1% had a mass. All CNB were vacuum assisted, 65.9% with 9-gauge needle, and averaged 10.0 cores. 77.3% were stereotactic biopsies, 13.6% ultrasound, and 6.8% MRI. 59.1% had excision after CNB. 82.1% of patients did not upstage. One patient upstaged to invasive ductal carcinoma (3.6%) and two patients to high-risk lesion (7.1%).Discussion: There was low upstage rate of RS/CSL on excisional biopsy. Centers could consider close surveillance for RS/CSL on CNB. Longer follow-up in cases of deferred excision is needed to ensure oncologic safety. [ABSTRACT FROM AUTHOR]

Bacterial sexually transmitted infections (STIs) continue to be a worsening public health concern in the United States (US). Though the national incidence of HIV infection has decreased over recent years, that of chlamydia, gonorrhea, and syphilis have not. Despite national recommendations on prevention, screening, and treatment of these STIs, these practices have not been standardized. Nine Health Resources and Services Administration Ryan White HIV/AIDS Program funded clinics across 3 US jurisdictions (Florida, Louisiana, and Washington, DC), were selected as clinical demonstration sites to be evaluated in this mixed method needs assessment to inform a multi-site, multi-level intervention to evaluate evidence-based interventions to improve STI screening and testing of bacterial STIs among people with or at risk for HIV. These 3 US jurisdictions were selected due to having higher than national average incidence rates of HIV and bacterial STIs. Descriptive statistics and deductive analysis were used to assess quantitative and qualitative needs assessment data. Results indicate the following needs across participating sites: inconsistent and irregular comprehensive sexual behavior history taking within and among sites, limited routine bacterial STI testing (once/year and if symptomatic) not in accordance with CDC recommendations, limited extragenital site gonorrhea/chlamydia testing, limited annual training on STI-related topics including LGBTQ health and adolescent/young adult sexual health, and limited efforts for making high-STI incidence individuals feel welcome in the clinic (primarily LGBTQ individuals and adolescents/young adults). Kim et al completed a pooled analysis of 8 National Surgical Adjuvant Breast and Bowel Project trials and highlight that, when compared with White patients, Black patients with estrogen receptor–positive (ER+) breast cancer have worse distant recurrence‐free survival, especially in the setting of neoadjuvant chemotherapy when pathologic complete response is not achieved.
