Introduction Children who suffer from severe caries in childhood may have negative impacts on the growth, development, nutritional problems and quality-of-life problems related to the oral health of the child and their family. There are no studies that have compared rehabilitative techniques of primary anterior teeth regarding patient-centred outcomes and even longevity of the restoration. Thus, this project aims to evaluate the effectiveness of restorative treatment of anterior primary teeth with monochromatic composite resin in single insertion through polyvinyl crowns, after selective removal of carious tissue compared with the effectiveness of conventional restoration.Methods and analysis This study proposes to conduct a randomised clinical trial, composed of a sample of 194 deciduous central and lateral incisors with active cavitated lesions, simplified International Caries Detection and Assessment System (ICDAS) C+ score (active and extensive stage caries: ICDAS 5 and 6), with involvement of more than two surfaces. This sample will be divided into two experimental groups, both with selective removal of carious tissue: a group in which conventional restoration will be performed using opaque resins, and another group with monochrome resin with chameleon effect and polyvinyl crowns. The explanatory variables—gender, age, toothbrushing, use of fluoridated toothpaste and dental floss, and socioeconomic status—will be collected through a questionnaire with open questions. The progression of caries lesions after 24 months of follow-up will be considered as the primary outcome. Secondary outcomes will include tooth survival, longevity of restoration, quality of life, perception and satisfaction of the participants’ parents/guardians.Ethics and dissemination This protocol has been approved by the Human Research Ethics Committee of Universidade Metropolitana de Santos (protocol number: 6.019.297. Approved 24 April 2023). Results will be submitted to international peer-reviewed journals and presented at international conferences.Trial registration number NCT05875064.

Introduction Childhood early oral ageing syndrome (CEOAS) is a condition involving oral abnormalities resulting from systemic diseases of different origins that are related to the current lifestyle of the paediatric population. Enamel defects associated with intrinsic and extrinsic factors promote the early loss of tooth structure at an accelerated pace, with negative impacts on function, aesthetics and quality of life. The aim of the study is to identify the prevalence of early tooth wear in childhood and its severity using the CEOAS index, which is a tool for the diagnosis of the condition and for epidemiological surveys, involving the investigation of abnormalities of the oral cavity in the paediatric population and possible factors associated with the severity of the condition.Methods and analysis Children aged 3–6 years will be recruited from the paediatric clinic of Universidade Nove de Julho. Children with amelogenesis imperfecta, dentinogenesis imperfecta and genetic syndromes, as well as those undergoing orthodontic treatment will be excluded. A questionnaire will be administered addressing general health, sleep quality, dietary habits, oral hygiene habits and parafunctional habits. Clinical examinations will then be performed using the novel CEOAS index to assess tooth wear (scores 0–3) and determine the occurrence of enamel defects (scores I–III). Abstract Background Important evidence has been constantly produced and needs to be converted into practice. Professional consumption of such evidence may be a barrier to its implementation. Then, effective implementation of evidence-based interventions in clinical practice leans on the understanding of how professionals value attributes when choosing between options for dental care, permitting to guide this implementation process by maximizing strengthens and minimizing barriers related to that. Methods This is part of a broader project investigating the potential of incorporating scientific evidence into clinical practice and public policy recommendations and guidelines, identifying strengths and barriers in such an implementation process. The present research protocol comprises a Discrete Choice Experiment (DCE) from the Brazilian oral health professionals’ perspective, aiming to assess how different factors are associated with professional decision-making in dental care, including the role of scientific evidence. Different choice sets will be developed, either focusing on understanding the role of scientific evidence in the professional decision-making process or on understanding specific attributes associated with different interventions recently tested in randomized clinical trials and available as newly produced scientific evidence to be used in clinical practice. Discussion Translating research into practice usually requires time and effort. Shortening this process may be useful for faster incorporation into clinical practice and beneficial to the population. Understanding the context and professionals’ decision-making preferences is crucial to designing more effective implementation and/or educational initiatives. Ultimately, we expect to design an efficient implementation strategy that overcomes threats and potential opportunities identified during the DCEs, creating a customized structure for dental professionals. Abstract This trial-based economic evaluation aimed to estimate the incremental cost of implementing an active learning strategy (theoretical-practical workshop) to substitute the didactic lecture as the sole method for students training in caries detection. We also provided a budget impact analysis and explored the composition of costs related to the activity. Data from the coordinating centre of a multicentre randomized and controlled study (IuSTC01) was analyzed as the first part of our main economic analysis plan. The perspective of the educational provider (the institution implementing the activity) was considered, and an immediate time horizon was adopted. All used resources were valued in Brazilian Real by adopting a microcosting strategy. Costs for each strategy were estimated and converted into international dollars. The incremental Cost per student and the total cost of implementing the complete teaching strategy for 80 students were calculated. Monte Carlo simulations were used to estimate the uncertainties. The incremental Cost estimated for the workshop implementation would be $7.93 per student (interquartile range (IQR): $7.8–8.1), and the total cost of the teaching activity would be $684 (IQR:672–696). The laboratory training comprised more than 50% of the total amount spent, and a higher percentage of this value was related to human resources costs (72%). Saving 40% of the costs could be expected for the next rounds of activities in the institution, assuming no need for additional preparation of didactic materials and tutor training. A modest incremental cost per student and an acceptable organizational budget impact should be expected for the institution when including active learning training in caries detection for undergraduate students, mainly related to the human resources involved.

Objective: To assess the relationship between celiac disease and the prevalence of dental caries in children and adolescents and also evaluate if dental enamel defects in celiac children predispose to dental caries. Material and Methods: Searches were performed in the following databases: PubMed, Scopus, Cochrane Library, Latin American and Caribbean Literature on Health Sciences, and OpenGrey. The risk of bias was assessed using the Newcastle-Ottawa Scale. DMFT/dmft and DMFS/dmfs data of observational studies that compared the prevalence of caries between children and adolescents with celiac disease and healthy individuals. Meta-analysis was performed using a random effects model. Heterogeneity between studies was estimated using Cochran’s Q test, and inconsistency was measured using I2 statistics. Results: Of the 121 studies retrieved, 17 were selected, and 12 were included in the meta-analysis. The prevalence of caries in the primary dentition (dmft) did not differ between celiac patients and controls [SMD = -0.35; 95% CI (-0.83; 0.13); p = 0,15; I2 = 89%]. There was also no difference in the prevalence of caries in permanent teeth (DMFT) between groups [SMD = -0.44; 95% CI (-1.02; 0.14); p = 0.14; I2 = 95%]. Conclusion: Celiac disease is not a determinant factor in the development of dental caries in children and adolescents compared to the control group.

Premature, non-physiological tooth wear in childhood has numerous repercussions for oral health. This is a growing problem with multifactorial causes and associated with the current lifestyle. The introduction of new concepts and indices is crucial for enhancing the understanding and management of dental conditions. In this context, we present the Childhood Early Oral Aging Syndrome (CEOAS) and the associated index, which aim to assess early enamel wear in the primary dentition. Rather than focusing solely on isolated etiologies, the index proposes a comprehensive evaluation of signs and symptoms, considering various factors that contribute to the premature and non-physiological loss of dental structure, including wear of both intact and hypomineralized enamel. Indices that evaluate the main factors of early oral aging in childhood and their interactions are fundamental for understanding the condition and developing effective control and treatment strategies, particularly given that there is currently no global index encompassing this information. The aim of the present study was the development and determination of content validity of the Childhood Early Oral Aging Syndrome (CEOAS) index for the primary dentition as a diagnostic and epidemiological survey tool. Dental caries remains the most prevalent chronic disease globally, and its management should adhere to the principles of minimal intervention dentistry. This study reports a series of successful cases involving the selective removal of carious tissue in permanent molars, with follow-up periods of up to 21 years. Six permanent molars with severe and deep carious lesions were treated with selective caries removal and restored with high-viscosity glass ionomer cement. Clinical examination revealed that the teeth were free from pain and sensitivity. Follow-up assessments were conducted at 7 and 21 years posttreatment. Treatment success was defined by the absence of clinical and radiographic signs, symptoms of pulp alterations, and lesion arrest. Successful minimally invasive approaches were observed with selective removal of carious tissue and maintenance of pulp vitality for up to 21 years.

Abstract Background Hall technique (HT) has been indicated for teeth with dentinal caries lesion; however, extensive cavities, with more than two surfaces still seem challenging for restorative treatment in pediatric dentistry, resulting in a higher failure rate and an increased need for retreatment. Objectives To compare the survival rate of the Hall technique preformed metal crown (HT) with resin composite restoration (RC) for multi-surface cavitated caries lesions in primary molars. Methodology In this multicenter two-arm randomized clinical trial, children between 4 and 9 years of age with at least one primary molar with cavitated caries lesion involving more than two surfaces, including one buccal or palatal/lingual surface, were selected from 17 Brazilian cities. A total of 364 teeth were allocated into two groups: (1) teeth treated with selective caries removal and RC and (2) treated with the HT. The survival rate was assessed at 6 and 12 months after the interventions. Survival analysis was performed with the Kaplan‒Meier method. Cox regression was used to determine the influence of explanatory variables on the survival rate (α=5%). Results After 12 months, 292 teeth were re-evaluated. A total of 358 teeth were re-evaluated at least once during the study and included in the survival analysis. The HT (87.8%) resulted in a higher survival rate than RC restoration (75.7%) (p=0.004). Conclusion HT has a higher survival rate than RC as a treatment for multi-surface cavitated caries lesions in primary teeth. ClinicalTrials.gov: NCT02782390

ABSTRACT In pediatric dentistry, restorative treatment requires a material with good mechanical properties, ease of handling, and usability. For the restoration of deciduous teeth, colored compomers are an alternative material for motivating children in dental treatment and, consequently, behavior management. This study aimed to provide information about colored compomers by presenting a case of restoration in a deciduous molar with a five-year follow-up. Additionally, the study highlights the child’s motivation toward dental treatment and oral care, thus offering an alternative to other restorative materials for clinicians to consider. The treatment plan for tooth 75 was selective removal of decayed tissue and restoration with compomer blue- colored Twinkly Star. The restoration was performed without anesthesia under relative isolation following the clinical protocol recommended by the manufacturer. As a result, it was observed that the restoration in colored Twinky Star compomer remained aesthetically and functionally satisfactory after five years. In addition, it was favorable for the child’s collaboration in dental treatment. It is concluded that colored Twinky Star compomer can be a viable alternative for restoring deciduous teeth, contributing to children’s dental treatment and oral care motivation.

Aim: Due to the particular anatomy of primary teeth, improvements in the materials and protocols used for disinfecting and modelling the root canal system are needed to develop a more objective and efficient approach. The aim of this in vitro experimental study was to compare the disinfection of the root canal system of primary teeth with manual, rotary and photodynamic therapy systems. Methods: Forty-eight canals of primary molar teeth were infected with a suspension of Enterococcus faecalis and divided into four groups (n = 12): group G1, composed of canals prepared with manual H-type files from the Angelus® system via the crown-down technique; group G2, treated with the Sequence of Baby Files™ rotary system by the pecking motion technique; group G3, using the G2 treatment + photodynamic therapy, with methylene blue as photosensitizer at a concentration of 0.005%, irradiated with a 660 nm diode laser for 100 seconds, 100 mW power, 10 J energy and 357 J/cm² fluence; and group G4 (negative control), which received no treatment. Samples of the canal contents were collected with sterile paper cones before and after instrumentation, diluted and seeded on BHI agar plates. The bacterial colonies formed were counted, and the results were log transformed and subjected to analysis of variance (ANOVA) and Dunnett’s test. Results: Showed a bacterial reduction in all studied groups compared to the control group. Conclusions: We concluded that the manual, rotary and photodynamic therapy systems all significantly reduced the E. faecalis count.

Abstract Background With the COVID-19 pandemic, thousands of children had their dental care interrupted or postponed, generating a pent-up demand for primary care. To minimize the impact of this outage, information and communication technologies (ICT) could be an alternative. The aim of this study is to elucidate the impact of implementing the ICTs in primary dental care for children on resolving the pent-up demand for primary dental care to children in the national health system service (SUS) due to the COVID-19 pandemic. Methods Different research strategies are being proposed to demonstrate such effect and extrapolating findings to a real-world context to guide further research, practice and policies: two clinical trials (one randomized controlled by the waiting list trial (RCT) and a before-and-after study), one simulation study to prospect trial results to a broader population and three economic evaluations using different effects. Children enrolled in a reference dental unit will be invited to participate in the before-and-after study for trials. The first 368 families will be randomized for the RCT to the intervention vs waiting list. All participants will receive the intervention, but the waiting list group will be assessed before the intervention is available for them. The intervention comprises standardized non-face-to-face primary dental care using the V4H platform. The problem-solving and the family's perception will be the primary outcomes set for the before-and-after study and RCT, respectively. They will be measured 2 weeks after randomization. Based on trial findings, we will develop theoretical models to estimate how the intervention could benefit the population included in the national health system. Three economic evaluations will be carried out considering different trial effects (cost-effectiveness analyses). A societal perspective and the pandemic time horizon will be considered. Possible social impact (inequalities) will also be explored. Discussion This ongoing trial may be an essential contribution to clarify positive and negative aspects related to the use of technologies for non-face-to-face dental care for children. Trial products may bring relevant contributions to the pandemic context and the post-pandemic period. Potential benefits may be feasible to implement and preserve in the health system even in the post-pandemic period. Aim: This study aimed to evaluate the level of knowledge and literacy in oral health of mothers of children with microcephaly. Methods: It was used a sample consisting of 74 mothers of children with microcephaly, under multiprofessional monitoring at the rehabilitation center for special patients at the Microcephaly Clinic of CEIR. A validated questionnaire was used to assess the mothers’ degree of knowledge about oral health and the BREALD-30 to assess the level of oral health literacy. Results: It was observed that most mothers (63.5%) studied between 8 and 11 years old. In most interviewees, family income was 1 minimum wage. Este estudo avaliou a retenção de conhecimento, em médio prazo, de graduandos expostos a uma atividade educativa de detecção de cárie. Esta é uma parte do estudo multicêntrico IuSTC. Alunos dos últimos anos foram convidados a participar e passaram pela atividade. Foram avaliados quanto à retenção do conhecimento após 1,5 ano. Avaliações teóricas e práticas (pontuadas de 0 a 10) foram preparadas para avaliar conhecimento, habilidades e atitudes. Os escores foram definidos como desfechos. Análises de regressão multinível testaram a associação entre a performance dos alunos e sua percepção sobre a importância da atividade dentro do seu currículo. Particularidades relacionadas à performance em tópicos específicos e diferentes tipos de avaliação também foram explorados. 302 alunos participaram das avaliações. Em média, os alunos tiveram medias (desvio-padrão (DP), de 6,68 (1,88) teórica e 5,5 (2,27) prática. A percepção dos alunos esteve associada com sua performance. Na avaliação teórica, os alunos tiveram pior performance ao relacionar a condição clínica com a histológica (21% de acertos) ou em justificar a conduta frente a lesões inativas (36% de acertos). Na avaliação prática, 70-90% dos alunos demonstraram habilidade de detectar, avaliar atividade e tomar decisões clínicas frente a lesões iniciais e severas, enquanto 50% avaliaram adequadamente as lesões moderadas e apenas 32% demonstrou atitude correta do manejo. Em conclusão, a retenção do conhecimento dos alunos está dentro do esperado em uma análise de médio prazo. Para alguns tópicos (por exemplo, relacionados a prática e justificativa de atitudes), a performance dos alunos tende a ser pior.

Objective: To investigate whether the dimensions of cavitated dentin carious lesions on the occlusoproximal surfaces of primary teeth could predict the location of cement-enamel junction (CEJ). Material and Methods: Two hundred extracted primary molars were selected and digital images were obtained. The teeth were set in arch models for clinical measurement. The cervical-occlusal (CO) and buccal-lingual/palatal (BL/P) cavities’ dimensions were obtained by digital (Image J) and clinical (periodontal millimeter probe) assessments. The cervical margin location was also determined. The thresholds (cut-off points) were determined by sensitivity, specificity and the areas under the receiver operating characteristics curves (Az) for the two methods. Pearson's correlation coefficient was used to investigate the correlation between clinical and digital measurements. Logistic regression analysis was performed to evaluate the association between the dimensions and cervical margin location. Results: There was a strong correlation between methods for all measurements (CO: r=0.90, VL/P: r=0.95). Cavities with BL/P distance higher than 4.5 mm and CO dimension higher than 3.5 mm had a lower chance of presenting the cervical limit above the CEJ, irrespective of the measurement method. Conclusion: CO and VL/P dimensions could be used to predict the CEJ location and, ultimately, as a clinical parameter for restorative decision-making.

A mínima intervenção vem ganhando cada vez mais notoriedade na Odontologia, refletindo uma crescente demanda de profissionais que buscam por técnicas que promovam tratamentos eficientes e preservem a estrutura dental. Objetivo: O presente estudo teve como objetivo promover um controle cavitário não restaurador (CCNR) em um paciente infantil, por meio de uma regularização da cavidade da lesão de cárie para permitir uma melhor higienização da área, associado a um protocolo complementar de flúor. Caso clinico: Paciente do sexo masculino, 4 anos de idade, com higiene bucal regular, entretanto com acúmulo de biofilme entre os incisivos centrais superiores e sem sintomatologia ou queixa estética. As superfícies mesiais dos incisivos centrais foram regularizadas com tiras abrasivas para remover áreas retentivas e facilitar o acesso para melhorar a higiene bucal. O verniz fluoretado foi aplicado em 4 sessões, com intervalo de uma semana entre cada aplicação. A mãe do paciente foi orientada quanto à higiene e cuidados com a dieta. Durante 36 meses, houve o acompanhamento clínico e radiográfico do paciente com retornos a cada 12 meses. O índice de placa visível e os resultados da orientação de higiene e dieta também foram reavaliados. Conclusão: Neste caso clínico, a regularização de superfícies associada a fluorterapia, mostrou-se uma alternativa de tratamento de sucesso a longo prazo, permitindo a preservação do tecido dentário e impedindo a progressão das lesões de cárie. O conceito ultraconservador pode ser uma alternativa válida devido à sua eficácia, simplicidade, baixo custo e boa aceitação pelas crianças e seus pais.

Although ectopic eruption of the maxillary canine has a low prevalence, a late diagnosis can have serious consequences. A careful clinical examination, assisted with radiographic examination, ensures early diagnosis, facilitates planning, and minimizes possible adverse consequences. This study reports a case of ectopic eruption of the permanent maxillary canine, with complete root resorption of the central permanent incisor, the consequences of which caused functional, aesthetic, and psychological harms to the patient. The procedures used included canine ectopic remodeling of the ectopic canine in the central incisor and orthodontic correction, which treated the anomaly and rebuilt the patient’s self-esteem.

Abstract Introduction: the avulsion of primary teeth is a disturbing and unexpected event. Description: this report describes the clinical case of a three-year-old child who suffered an avulsion and replantation of the primary upper central incisors at the site of the injury. The guardians sought treatment for the child at the Federal University of Minas Gerais after the replantation. Four months later, the child suffered a new trauma and the replanted teeth presented advanced mobility, root resorption and fistula. The clinical conduct was extraction and rehabilitation with a fixed esthetic maintainer. Discussion: the literature describes two treatment options for avulsion of primary incisors: replantation and non-replantation. According to a recent systematic review, the difficulty in obtaining a consensus regarding the best clinical conduct is due, in part, to the scarcity of publications that present not only follow-ups with clinical success, but also with failures. The outcomes of replantation can be influenced by several factors. The time elapsed between replantation and splinting, and the new episode of trauma, negatively influenced the prognosis in the present case, leading to failure. Replantation of primary incisors is not yet evidence-based treatment. Therefore, this option must be chosen with caution and in ideal situations. It requires constant clinical and radiographic monitoring for evaluation of outcomes.

Abstract There are many glass ionomer cements available on the Brazilian market for Atraumatic Restorative Treatment (ART), however, there is still a gap in the literature regarding their cost-effectiveness. Objectives To evaluate the influence of restorative materials (Ketac Molar, 3M ESPE; and Vitro Molar, Nova DFL) in the two-year survival rate and cost-effectiveness of occluso-proximal ART restorations in primary molars. Methodology A total of 117 children (aged four to eight years) with at least one occluso-proximal carious lesion in primary molars were selected and randomly divided in treatment groups (KM or VM) in this parallel randomized controlled trial. Treatments followed ART premises and were conducted in public schools by trained operators in Barueri, Brazil. A trained, calibrated, and blinded examiner performed the evaluations after two, six, 12, and 24 months (k=0.92). Kaplan-Meier survival analysis was used to estimate restoration survival and Cox regression was used to test the association with clinical factors (α=5%). For cost analysis, material and professional costs were considered. Monte Carlo analysis was used to generate a cost-effectiveness plane and bootstrapping was used to compare material costs over the years. Results The overall survival rate was 36.9% after two years (48.6% for KM and 25.4% for VM). Restorations with VM failed more than those with KM (HR=1.70; 95% CI=1.06–2.73; p=0.027). VM presented lower initial cost, but no difference was observed between groups considering the two-year incremental cost. Conclusion After a two-year evaluation, KM proved to be a better option than VM for occluso-proximal ART restorations in primary molars. ClinicalTrials.gov: NCT02267720

ABSTRACT The treatment of cavity carious lesions in anterior deciduous teeth is a frequent demand in the pediatric dentistry clinic, since the control of the disease in early childhood still presents itself as an inter-factor challenge in child health care. While the scientific evidence on the minimally invasive principles of approach in Dentistry is presented at an increasing level, the alternatives for restorative treatment in anterior deciduous teeth still face the low availability of reports that associate minimal intervention with the aesthetic needs inherent to the treatment. Thus, this article presents a series of cases in which direct restorations in composite resin, without removing carious tissue using polyvinyl matrices, were treatment alternatives for moderate and severe caries lesions in anterior deciduous teeth. It was observed, therefore, that the use of this artifact helped the rehabilitation of compromised dental elements with speed and satisfactory 50 result, suggesting that this is an appropriate approach for application in Pediatric Dentistry, 51 since it offers resistance, durability, besides providing aesthetics and functionality, it presents lower cost and agility as it eliminates the laboratory phase.

ABSTRACT The diagnosis and treatment of supernumerary teeth pose a challenge for dental clinicians and pediatric dentists. These teeth are asymptomatic and may cause the delayed eruption of the permanent dentition, altered bone growth and ectopic positioning. This paper reports a case of the surgical removal of supernumerary teeth in the anterior region of the maxilla of a child under general anesthesia in a hospital setting. A six-year-old female patient visited a private dental office in the city of Garanhuns, Brazil, accompanied by her mother, who reported a tooth with a “strange shape” in the anterior region as the main complaint and also reported that her daughter’s first dental experience was negative. The clinical examination and imaging tests were performed under general anesthesia to confirm the diagnosis. In view of the patient’s dental and behavioral history, surgical treatment was performed in a hospital setting and assisted by a multidisciplinary team. Care in the hospital setting was found to be feasible, providing greater comfort for uncooperative patients.

Aim: To verify the relationship between temporal-mandibular dysfunction (TMD) with depression, sleep, sleepiness and quality of life in adolescents aged 13 to 18 years old. Methods: Thirty-eight adolescents being seen at the UFAL Dental Clinic (Federal University of Alagoas), for TMD, and qualifying according to the Research Diagnostic Criteria for Temporomandibular Disorders (RDC / TMD), participated in the study. Two instruments were used to investigate sleep quality: the Pittsburgh Sleep Quality Index (PSQI) and the Epworth Sleepiness Scale (ESS); the Oral Health Impact Profile (OHIP-14) for quality-of-life assessment; and the Beck Depression Inventory (BDI-II) for depression assessment. Pearson’s correlation coefficient was used for the relationship of numerical variables. For the means tests, the Student t test was applied (using, when necessary, the Welch correction). For the analyses, the Bonferroni correction was considered. Results: After calculation, αBonferroni correction was applied equal to 0,0005. Of the total number of participants (56% female and 44% male), with a mean age of 14.7). In all comparisons between groups (with and without TMD), there were statistically significant indices for adolescents with TMD in relation to: depression (p=5.6∙10-11), quality of life (p=4.3∙10-12), sleep quality (p=5.0∙10-10), and somnolence (p=0.0002). From the correlation matrix, it was observed that all correlations were significantly positive and moderate. Conclusions: Adolescents with a diagnosis of TMD presented an increase of depression and somnolence, as well as impairment of sleep quality and quality of life, and these same variables can influence on the onset of TMD.

Abstract Objectives: a quantitative analytical cross-sectional study aimed to identify demographic characteristics to access health services and housing conditions in relation to inadequate feeding practices among Brazilian children from 12 to 23 months of age. Methods: the analysis of the feeding practices (consumption of recommended food groups) of 2541 Brazilian children (aged 12-23 months old) was performed using data from the Pesquisa Nacional de Saúde 2013 (National Health Survey). Descriptive and multivariate statistics used the Decision Tree Analysis based on CHAID (Chi-squared Automatic Interaction Detector) algorithm, as well as hierarchically adjusted Poisson regression analyses were performed. The variables were entered in a hierarchical model at distal (demographic), intermediate (access to health) and proximal (housing conditions) levels. ABSTRACT Objective: to determine the prevalence of ankyloglossia in neonates and verify factors that interfere with exclusive breastfeeding and cause difficulties in breastfeeding. Methods: an observational longitudinal study conducted at a public maternity hospital in Teresina, Piauí, Brazil. The sample comprised 397 mother-child pairs. Ankyloglossia was diagnosed with the part I of the Baby Lingual Frenulum Assessment Protocol with Scores. Six months after the children were born, the mothers were interviewed and answered questions on the indication and performance of frenotomy and time of breastfeeding. Descriptive analysis of the data, chi-square test and bivariate and multivariate Poisson regression were conducted (p < 0.05). Results: the prevalence of ankyloglossia was 4.3% (n = 17). The attendance rate for the second moment of the research was 70.8% (n = 281). Of the examined children, 14 were indicated for lingual frenotomy, but only one was submitted to the procedure. The time of exclusive breastfeeding of children with ankyloglossia was equal to that of children with a normal lingual frenulum (p-value = 0.876). Ankyloglossia was not associated with breastfeeding difficulties (PR = 1.0 95%CI 0.8-1.1, p-value = 0.441), neither was it a factor for not breastfeeding exclusively for 6 months (PR = 1.1 95%CI 0.9-1.2, p-value = 0.390). Conclusions: the prevalence of ankyloglossia in the studied population was 4.3%. Baby bottle use in the first month of life and breastfeeding difficulties were hindering factors for exclusive breastfeeding.

O objetivo deste estudo foi relatar um caso clínico de selamento de lesões de cárie ativas, escore 5 do International Caries Detection and Assessment System- ICDAS, em primeiros molares permanentes de um menino de 9 anos. Ao exame clínico, foram identificadas lesões ativas, escore 5 e 3 nos elementos 16 e 26, respectivamente. Entretanto, no exame radiográfico observou-se que a lesão do dente 26 também estava em metade externa de dentina. Dessa forma, ambas as lesões eram escore 5. O tratamento de mínima intervenção com o selamento das lesões de cárie foi realizado, usando selante resinoso com camada intermediária de adesivo. Em 6 meses de acompanhamento, observou-se perda de integridade do selante no dente 16, sendo realizado o reparo; após um ano, as lesões estavam paralisadas. Portanto, o selamento de lesão de cárie parece ser adequado para lesões em esmalte atingindo metade externa de dentina.

Abstract: This study compared the cost of endodontic treatment in primary teeth involving a technique that does not require root canal instrumentation using antibiotic paste (CTZ) with that of the instrumented technique using iodoform paste (GP). This study is part of a randomized, controlled, parallel arm, noninferiority, 1:1 allocation, blinded (patient) multioperator study of 52 primary incisors of children aged 3 to 6 years with caries lesion and pulp involvement. Each technique was performed according to the creators’ descriptions. The cost was assessed by analyzing the costs of capital, dental supplies, and professional labor according to the time taken to perform the procedure and the CHEERS guidelines were used to report the cost assessment. Endodontic treatment with CTZ had a 58.33% lower execution cost than GP (US$6.73 and US$16.15, respectively). The t-test showed significant differences between groups regarding treatment time and total cost (p < 0.0001). The CTZ technique seems to be more economically viable than GP for endodontic treatment of primary teeth, requiring a shorter treatment time and lower costs.

Objective: To compare the capacity of silver diamine fluoride (30%) and the bioactive giomer light-curing varnish for arresting ICDAS score 5 non-retentive caries lesions. Material and Methods: An in vitro analytical study was conducted using 36 healthy primary teeth, in which a cavity was created, divided into 3 groups: control (distilled and deionized water), giomer-based light-curing varnish - Barrier Coat, and Silver Diamine Fluoride 30% – Cariestop (SDF). The samples were analyzed for mineral loss as measured by surface microhardness (SMH). The Knoop microhardness test was performed using the HVS-1000 microhardness tester. The data were compared by two-way analysis of variance – treatment and time (before and after treatment) followed by Tukey's post-test. The significance level adopted in all analyzes was 5%. Results: The specimens treated with giomer showed higher SMH compared to the untreated group but were similar to those of the SDF group. The SDF group, on the other hand, showed no statistically significant difference from the control group (p=0.010). Conclusion: The giomer varnish had a remineralizing effect on artificial dentinal caries.

Los defectos del esmalte dental em La dentición primarias es comum esen niños con antecedentes de prematuridad y bajo peso. Una de las causas de La prematuridad es El embarazo gemelar, que hace que La mayoría de los fetos sean prematuros debido al crecimiento intrauterino restringido. El objetivo de este artículo es informar La importancia de un diagnóstico correcto y temprano de HMD engemelos prematuros, y lãs posibilidades de tratamiento. Materiales y métodos: Dos niñas gemelas idénticas de 5 años que tenían El mismo patrón HMD em los dientes 55.65,75,85 ya com gran destrucción e historia de sensibilidad. Ya fueron abordados por outro profesional, pero solo uno recibió tratamiento intervencionista y sin éxito, La otra no aceptó El condicionamiento, no recibió ninguna intervención; ambas con diagnóstico erróneo. El examen clínico y radiográfico revelo La posibilidad de realizar la técnica de Hall em los dientes 55 y 65 del gemelo 1 y 85 del gemelo 2. Resultados: Los dientes 75 y 85 del gemelo 1 recibieron restauraciones de resina compuesta, ya que han sido intervenidos. Los dientes 55 y 65 del gemelo 2 recibieron restauraciones ionoméricas y la pulpectomía deldiente 75 y la corona de acero. Conclusión: La técnica de Hall es muy bien venida cuando se trata de rehabilitación en hipoplasia, especialmente com respecto al manejo de La edad temprana, todavia hay errores de diagnóstico al examinar una HMD (hipomineralización molar primário)

Introducción: Labio y paladar fisurado es una malformación congénita en la región del labio y/o paladar, pudiendo ser uni o bilateral, causada por la falta de cierre de dichas estructuras, su gravedad puede variar y afectar los hábitos y la calidad de vida en relación con la salud oral. Objetivo: El objetivo de este estudio es evaluar la calidad de vida relacionada con la salud oral y las características socio-conductuales de pacientes fisurados menores de 7 años. Materiales y Métodos: Después de la aprobación del comité de ética, el estudio se llevó a cabo en un centro de atención clínica de los pacientes con dicha alteracion, y la muestra de conveniencia consistió en 19 pacientes cuyos cuidadores aceptaron participar y responder a los cuestionarios solicitados. Un cuestionario para medir la calidad de vida en relación con la salud oral se aplicó a los responsables. Las variables socioeconómicas se recolectaron a través de un cuestionario específico utilizado previamente, así como las preguntas sobre el miedo y el cuidado de la higiene bucal de los niños. Los datos se analizaron de manera descriptiva. Resultados: La mayoría de los pacientes pertenecían al sexo masculino y la edad media fue de 3,9 años. Gran parte de los cuidadores reportó dificultades y el miedo para la realización de la higiene bucal y alimentación de los niños. En cuanto a la calidad de vida se percibió un mayor impacto en la sesión del niño que en de la familia. Conclusión: Se puede concluir que los pacientes menores de 7 años con labio y paladar fisurados pueden tener dificultades en la limpieza y la alimentación, así como los cambios en la calidad de vida relacionada con la salud oral.

Objetivos: Evaluar in vitro la acción antimicrobiana de las pastas GuedesPinto modificadas con Diprogenta® y con Otosporin®, comparándolas con la pasta Guedes-Pinto convencional. Materiales y métodos: Se evaluó la acción antimicrobiana de las pastas Guedes Pinto contra cepas aisladas de los siguientes microbios: Streptococcus mutans ATCC 25175 Staphylococcus aureus ATCC 25923, Enterococcus faecalis - ATCC 29212, Pseudomonas aeruginosa - ATCC 4116, Candida albicans ATCC 10231. Se empleó la técnica de difusión en agar por el método de pocillo y en triplicado, utilizando el digluconato de clorexidina al 0,12% como control positivo y solución salina como control negativo. Se transfirió 2 mL del inóculo producido de cada microbio a 100mL de agar fundido a 45ºC, dispensando la mezcla en 30 placas de petri. Las placas fueron mantenidas a temperatura ambiente por dos horas y luego incubadas a 35ºC por 24 y 48 horas. La lectura de los resultados fue sometida al Análisis de varianza de un factor y Test de Tukey para comparaciones múltiples. Resultados: La pasta Guedes-Pinto Convencional formó los mayores halos de inhibición para la mayoría de los microbios, con excepción de Candida albicans, donde ninguna de las sustancias produjo efecto y de la Pseudomonas aeruginosa, para la cual la pasta modificada con Diprogenta® obtuvo mejores resultados, sin diferencia estadística entre ellas. La pasta Diprogenta® presentó mejores resultados que la pasta Otosporin® para todos los demás microorganismos y esto ultimo no formó halo de inhibición para Enterococcus faecalis. Conclusión: La utilización de sustitutos de Rifocort® parece tener un potencial antimicrobiano efectivo contra los principales microbios encontrados en los canales radiculares.

El síndrome de Rubinstein-Taybi (SRT) es una rara anomalía congénita, de etiología todavía incierta. Su incidencia es de 1:100 000 a 1:300 000 nacimientos. El objetivo de este reporte de caso fue describir los hallazgos orofaciales y el acompañamiento dental por 7 años de un paciente del sexo masculino diagnosticado como portador del SRT. Las principales características orofaciales observadas fueron: mirada antimongoloide, “nariz en pico”, orejas grandes y de baja inserción, estatura corta, gran cantidad de cabello, pequeña abertura de boca, queilitis angular, paladar estrecho y profundo, apiñamiento dental, mordida cruzada posterior bilateral, hipomineralización de molares primarios (HMP), hipomineralización de molares e incisivos (HMI), mala higiene oral, presencia de cálculos supragingival, gingivitis y gran número de lesiones de caries activas. El SRT muestra muchas manifestaciones orofaciales y su conocimiento puede ayudar en el diagnóstico y tratamiento temprano. Las consultas odontológicas a intervalos más cortos ayudan a acondicionar al paciente y reducen el riesgo de lesiones de caries y problemas gingivales.

Objetivo: Evaluar la resistencia de unión al desalojo de sellantes resinosos en cavidades en dentina de diámetros diferentes. Material y métodos: Se realizó un estudio S. mutans aleatorio, doble ciego, con 60 incisivos bovinos (n = 10 cuerpos de prueba por grupo) que recibieron un corte en sentido longitudinal, utilizando disco de corte diamantado de alta concentración doble cara, para obtención de una lámina de esmalte / dentina de 1 mm de espesor. A continuación, fueron confeccionadas restauraciones con dos diferentes sellantes resinosos - Fluroshield® (padrón oro) e Ionoseal® - en cavidades con diámetros de 3, 4 y 5 mm. Después de 2 meses de almacenamiento en Originalagua, los especímenes fueron sometidos a la prueba de resistencia de desalojo Resultados: Los sellantes evaluados presentaron valores de resistencia de unión similares (p = 0,357). De la misma forma, el diámetro de las cavidades no interfirió en los valores de resistencia de unión de los sellantes resinosos evaluados (p = 0,068). Conclusión: Podemos sugerir que es posible sellar lesiones cavitadas en la mitad externa de dentina con diámetros de 3, 4 y 5 mm con diferentes sellantes resinosos, ya que estos materiales presentaron resistencias similares cuando se aplican a la prueba desalojo.

Objective: To evaluate the sealing of cavities of dentinal occlusal caries lesions, reproduced in vitro, with flow resin compared to cavity restorations presenting healthy dentin using microhybrid composite resin. Methods: The sample consisted of 27 healthy deciduous molars where cavities of approximately 2 mm in the fossa region were performed and occlusal cleft of each tooth were sealed, impermeabilization was performed and the sample was randomly divided into 2 groups: group I underwent cariogenic challenge and occlusal sealing with resin flow. The teeth of group II were restored with microhybrid composite resin. The teeth were immersed in 5% methylene blue for 8 hours at 37° C and washed until all the dye was removed from the surface. The teeth were sectioned in the mesio-distal direction. The penetration of the dye was evaluated: 0- no penetration; 1- dye penetration up to 1/3 of the restoration; 2- dye penetration up to 2/3 of the restoration depth; 3 - penetration of dye into the pulp wall. The results were analyzed by the Biostat 4.0 program. Descriptive analysis and the mode among the examiners submitted to the Mann-Whitney test. Results: There was no significant difference in microleakage between restoration performed in healthy dentin with microhybrid composite resin or maintenance of infected dentin in primary teeth sealed with resin flow (p = 0.6035). Conclusion: It was concluded that the marginal infiltration of primary molars sealed with microhybrid composite resin and resin flow was not influenced by the removal -or not -of the carious tissue or the material used.

Purpose: To evaluate the bonding strength to push out of resinous sealants in dentin cavities of different diameters. Materials and methods: A double-blind, randomized, in vitro study with 60 bovine incisors (n = 10 test specimens per group) was performed in a longitudinal direction using a high concentration, double-sided diamond cutting disc for obtaining a slice of enamel / dentin 1 mm thick. Restorations were then made with two different resin sealants - Fluroshield® (gold standard) and Ionoseal® - in wells with diameters of 3, 4 and 5 mm. After 2 months of storage in water, the specimens were submitted to the push-out resistance test. Results: The evaluated sealants showed similar bond strength values (p = 0.357). Likewise, the diameter of the wells did not interfere in the bond strength values of the resin sealants evaluated (p = 0.068). Conclusion: It is possible to seal cavitated lesions in Latinoamericanathe external half of dentin with diameters of 3, 4 and 5 mm with different resinous sealants, since the materials presented similar resistance when applied to the push out test.

This paper aims to describe an inflammatory fibrous hyperplasia case caused by chronic irritation due to poor dental positioning after trauma. An 11-year-old female patient was sent to a dental specialty center with the complaint of a soft tissue growth in the place of an unerupted maxillary central incisor, causing her behavioral changes becoming more timid. After anamnesis and clinical examination it was noted that tooth 11 was in a vestibular position, covered with a hyperplastic lesion. Surgical treatment was performed and the histopathological exam revealed inflammatory fibrous hyperplasia. Over the course of the follow-up appointments, it was possible to observe improvement on the mentioned tooth and the patient’s satisfaction. In face of the low prevalence of inflammatory fibrous hyperplasia, it is worth mentioning that the tooth positioning, outside its correct alignment in the arch, can cause repeated trauma to the mucosa, and then turning into etiological factor to inflammatory fibrous hyperplasia, affecting even children and adolescents. Thus, the dentistry must be alert to establish a diagnosis and clinical treatment, in addition to monitoring these cases.

A Odontodisplasia Regional é uma anomalia dental rara, que envolve os tecidos dentários de origem ectodérmica (esmalte) e mesodérmica (dentina, polpa e cemento). O objetivo deste trabalho foi relatar um caso clínico de odontodisplasia regional em uma criança. Paciente de nove anos, sexo masculino, tendo como queixa principal a ausência de alguns dentes decíduos e a não irrupção dos dentes permanentes em determinada região da maxila. O dente 16, que havia irrompido, apresentava-se clinicamente, com um esmalte hipoplásico e hipomineralizado e uma imagem radiográfica bastante tênue, por isso sendo conhecido como “dente fantasma”. Foram apresentados nesse relato, aspectos clínicos e radiográficos e destacadas as questões envolvidas na elaboração de um plano de tratamento. Há poucos estudos disponíveis sobre a odontodisplasia regional. A anamnese detalhada e o conhecimento das características clínicas e radiográficas são imprescindíveis para um correto diagnóstico.

This in vitro study evaluated the immediate and 6 months microshear bond strength (µSBS) of different adhesive systems to sound and eroded dentin. Sixty bovine incisors were embedded in acrylic resin and ground to obtain flat buccal dentin surfaces. Specimens were randomly allocated into two groups: sound dentin (immersion in artificial saliva) and eroded dentin (erosive challenge following a pH cycling model comprising 4 ×/day Sprite Light® drink for 10 days). Then, specimens were reassigned according to the adhesive system: etch-and-rinse adhesive (Adper Single Bond), two-step self-etch system (Clearfil SE Bond), or one-step self-etch adhesive (Adper Easy One). Polyethylene tubes with an internal diameter of 0.76 mm were placed over pre-treated dentin and filled with resin composite (Z250). Half of the specimens were evaluated by the µSBS test after 24 h, and the other half 6 months later, after water storage at 37°C. Failure mode was evaluated using a stereomicroscope (400×). Data were analyzed by three-way repeated measures analysis of variance and Tukey’s post hoc tests (α = 0.05). After 6 months of water aging, marked reductions in µSBS values were observed, irrespective of the substrate. The µSBS values for eroded dentin were lower than those obtained for sound dentin. No difference in bonding effectiveness was observed among adhesive systems. For all groups, adhesive/mixed failure was observed. In conclusion, eroded dentin compromises the bonding quality of adhesive systems over time.

The aim of this study was to evaluate the anticaries potential of 500 or 1100 ppm F dentifrices combined with fluoride varnish using a pH-cycling regimen. Seventy primary canines were covered with nail polish, leaving a 4×4 mm window on their buccal surface, and randomly assigned into 7 groups (n = 10): S: sound enamel not submitted to the pH-cycling regimen or treatment; N: negative control, submitted to the pH-cycling regimen without any treatment; D1 and D2: subjected to the pH-cycling regimen and treated twice daily with 1100 or 500 ppm F dentifrice, respectively; VF: fluoride varnish (subjected to F-varnish before and in the middle of the pH-cycling regimen); and VF+D1 and VF+D2. After 10 days, the teeth were sectioned, and enamel demineralization was assessed by cross-sectional hardness at different distances from the dental surface. Data were analyzed using a two-way ANOVA followed by Tukey's test. Dentifrice with 1100 ppm F and the combination of F-varnish with the dentifrices significantly reduced enamel demineralization compared with the negative control (p < 0.05), but the isolated effects of F-varnish and dentifrice with low concentration were not significant (p > 0.05). The effect of combining F-varnish with the dentifrices was not greater than the effect of the dentifrices alone (p < 0.05). The data suggest that the combination of F-varnish with dentifrices containing 500 and 1100 ppm F is not more effective in reducing demineralization in primary teeth than the isolated effect of dentifrice containing 1100 ppm F.

OBJECTIVE: This in situ study evaluated the discriminatory power and reliability of methods of dental plaque quantification and the relationship between visual indices (VI) and fluorescence camera (FC) to detect plaque. MATERIAL AND METHODS: Six volunteers used palatal appliances with six bovine enamel blocks presenting different stages of plaque accumulation. The presence of plaque with and without disclosing was assessed using VI. Images were obtained with FC and digital camera in both conditions. The area covered by plaque was assessed. Examinations were done by two independent examiners. Data were analyzed by Kruskal-Wallis and Kappa tests to compare different conditions of samples and to assess the inter-examiner reproducibility. RESULTS: Some methods presented adequate reproducibility. The Turesky index and the assessment of area covered by disclosed plaque in the FC images presented the highest discriminatory powers. CONCLUSION: The Turesky index and images with FC with disclosing present good reliability and discriminatory power in quantifying dental plaque.

Secondary caries has been reported as the main reason for restoration replacement. The aim of this in vitro study was to evaluate the performance of different methods - visual inspection, laser fluorescence (DIAGNOdent), radiography and tactile examination - for secondary caries detection in primary molars restored with amalgam. Fifty-four primary molars were photographed and 73 suspect sites adjacent to amalgam restorations were selected. Two examiners evaluated independently these sites using all methods. Agreement between examiners was assessed by the Kappa test. To validate the methods, a caries-detector dye was used after restoration removal. The best cut-off points for the sample were found by a Receiver Operator Characteristic (ROC) analysis, and the area under the ROC curve (Az), and the sensitivity, specificity and accuracy of the methods were calculated for enamel (D2) and dentine (D3) thresholds. These parameters were found for each method and then compared by the McNemar test. The tactile examination and visual inspection presented the highest inter-examiner agreement for the D2 and D3 thresholds, respectively. The visual inspection also showed better performance than the other methods for both thresholds (Az = 0.861 and Az = 0.841, respectively). In conclusion, the visual inspection presented the best performance for detecting enamel and dentin secondary caries in primary teeth restored with amalgam.

The objective of this study was to compare the completion rates and performance of the Brazilian version of the Oral Health Impact Profile (OHIP14) when applied as an interview or in its original self-reported form. A convenience sample of 74 adult patients was selected in a Dental Clinic (University of Araras, Brazil). One examiner administered the instrument in both formats to participants with an interval of 2 weeks between each administration. Data about dental health condition and socioeconomic status were collected and associated with total OHIP14 scores in both formats using linear regression analyses. No differences were found in the total scores and in each subscale of the OHIP14 according to the form of administration. Higher values of completion were found in the interview format. More severe impacts were recorded in the interview format than in the questionnaire format. Higher values of total OHIP-14 scores in both formats were related to the presence of dental caries. Total OHIP14 scores were not influenced by the method of administration. However, the use of the OHIP14 in the questionnaire format may result in lower completion rates and loss of data.

The atraumatic restorative treatment has been widely divulged among professionals in the area of Pediatric Dentistry. This restorative technique is included in the philosophy of Minimal Intervention and is considered one of the most conservative treatments, because only the layer of infected dentin caries is removed. Moreover, the atraumatic restorative treatment has been shown to be less painful than conventional approaches, and local anesthesia is rarely required. After the removal of the infected dentin, the cavities are filled with glass ionomer cement, a material that has antimicrobial capacity, good marginal sealing and constant fluorine release and recharge. In spite of the increasing number of studies about atraumatic restorative treatment, only studies related to restorations in occlusal cavities have shown scientific evidences about the technique. The aim of this study was to evaluate the feasibility of atraumatic restorative treatment in cavities with 3 or more surfaces involved, by means of a clinical case report of a patient with extensive dstruction in primary teeth, who was submitted to atraumatic restorative treatment, and observe the result of the treatment after one year of clinical and radiographic control.

The aim of this in vitro study was to evaluate four different approaches to the decision of changing or not defective amalgam restorations in first primary molar teeth concerning the loss of dental structure. Ditched amalgam restorations (n = 11) were submitted to four different treatments, as follows: Control group - polishing and finishing of the restorations were carried out; Amalgam group - the ditched amalgam restorations were replaced by new amalgam restorations; Composite resin group - the initial amalgam restorations were replaced by composite resin restorations; Flowable resin group - the ditching around the amalgam restorations was filled with flowable resin. Images of the sectioned teeth were made and the area of the cavities before and after the procedures was determined by image analysis software to assess structural loss. The data were submitted to ANOVA complemented by the Student Newman Keuls test (p < 0.05). The cavities in all the groups presented significantly greater areas after the procedures. However, the amalgam group showed more substantial dental loss. The other three groups presented no statistically significant difference in dental structure loss after the re-treatments. Thus, replacing ditched amalgam restorations by other similar restorations resulted in a significant dental structure loss while maintaining them or replacing them by resin restorations did not result in significant loss.

There has been growing concern and search for esthetic beauty and harmony over the last few years. This concern does not form part ofadults’ lives only, but also of children’s. Among the substances used for bleaching dental structures, the most outstanding are those whoseactive principle is hydrogen peroxide-based . The present study reports a clinical case of a 4-year-old girl that suffered trauma of tooth 61 with consequent color alteration, but with no alteration in pulp vitality. The main complaint by the patient and her guardians concerned esthetics, therefore external dental bleaching was performed, using Opalescence Xtra® (Ultradent), in two sessions with an interval of one month between them. External in office bleaching was the treatment of choice, due to the tooth vitality, patient’s age and presence of only one darkened tooth. After the bleaching treatments an improvement in the darkening was observed, and both the child and her guardians were satisfied with the esthetic result.

Odontopediatria é a especialidade que tem como objetivo o diagnóstico, a prevenção, o tratamento e o controle dos problemas de saúde bucal do bebê, da criança e do adolescente; a educação para a saúde bucal e a integração desses procedimentos com os dos outros profissionais da área da saúde. Na atualidade a Odontopediatria tem seu foco voltado para prevenção, tendendo a atender as crianças cada vez mais precocemente, além disso, procura-se durante o atendimento, promover tratamento compatível com as necessidades apresentadas. O objetivo deste artigo foi analisar a assistência odontológica prestada as crianças nos CEO, bem como a efetividade nesses atendimentos. A assistência odontológica infantil deve ocorrer na atenção básica e deve estar apta para atendimentos também nos CEO, dessa forma pode ocorrer assistência especializada através da atuação do odontopediatra, suprindo a demanda e cessando a dificuldade nos atendimentos. Concluiu-se que existe a necessidade do odontopediatra nos CEO, baseado na necessidade de mudar o perfil de cáries na faixa etária, principalmente de 5 anos, onde não houve melhorias nos últimos levantamentos, Projetos SB Brasil 2003 e 2010 e devido também não se ter atingido o perfil esperado pela OMS para assistência a saúde bucal infantil.

Methods: Ten cylindrical specimens of three commercial glass ionomers cements (Vidrion R - S.S. The aim of the present study was to evaluate the association between clinical parameters and the presence of active caries lesions on the occlusal surface of first permanent molars. Forty eight children (5.8-13.8 years-old) with at least one first permanent molar present were selected. The clinical parameters evaluated were gender, age, DMF-T and dmf-t, presence of active white spots in other teeth, general plaque index, tooth's dental arch (upper or lower), tooth's side (right or left), presence of visible plaque and eruption degree of the first permanent molars. The first permanent molars were evaluated through visual inspection by two examiners in order to assess the presence of active or inactive caries lesions on the occlusal surface. Univariate and multivariate analyses for determination of the association between clinical parameters and the presence of active caries lesions in these teeth were performed. The presence of active white spots in other teeth was associated with the presence of active caries lesions in the first permanent molars, in both univariate and multivariate analyses (Odds ratio = 8.8 and 1.9, respectively). The presence of abundant visible plaque on the occlusal surface of the first permanent molars (Odds ratio = 3.5 in the univariate analysis, and 3.9 in the multivariate one) also presented a significant association. In conclusion, the presence of active white spots in other teeth and the presence of considerable visible plaque were associated with the presence of active caries lesions on the occlusal surfaces of first permanent molars.O objetivo do presente trabalho foi avaliar a associação entre parâmetros clínicos e presença de lesões ativas de cárie sobre a superfície oclusal de primeiros molares permanentes. Quarenta e oito crianças (5,8-13,8 anos) com pelo menos um primeiro molar permanente foram selecionadas. Os parâmetros clínicos avaliados foram sexo, idade, CPO-D e ceo-d, presença de manchas brancas ativas em outros dentes, índice de placa, arco dentário do dente (superior ou inferior), lado do dente (direito ou esquerdo), presença de placa visível e grau de erupção dos primeiros molares permanentes. Os primeiros molares permanentes foram avaliados usando inspeção visual por dois examinadores para avaliar a presença de lesões de cárie ativas ou inativas sobre a superfície oclusal. Análises univariada e multivariada para avaliação da associação dos parâmetros clínicos e presença de lesões ativas nesses dentes foram realizadas. A presença de manchas brancas ativas em outros dentes apresentou associação significante com a presença de lesões ativas de cárie nos primeiros molares permanentes, em ambas as análises univariada e multivariada ("Odds ratio" = 8,8 e 1,9, respectivamente). A presença de placa visível abundante sobre a superfície oclusal dos primeiros molares permanentes ("Odds ratio" = 3,5 na análise univariada, e 3,9 na análise multivariada) também apresentou associação significante. Em conclusão, a presença de manchas brancas ativas em outros dentes e a presença de placa visível abundante apresentaram associação com a presença de lesões ativas de cárie sobre a superfície oclusal dos primeiros molares permanentes.

O objetivo deste trabalho é descrever um caso clínico de Histiocitose das Células de Langerhans em criança, cujas manifestações bucais foram o motivo da primeira hipótese diagnóstica da forma conhecida como granuloma eosinofílico multifocal, atualmente referida como forma crônica localizada da doença das Células de Langerhans. O diagnóstico foi estabelecido através de exames clínico, radiológico, histopatológico e imunohistoquímico.
