IntroductionOn March 11, 2011, the Great East Japan Earthquake occurred in Japan, with a nuclear accident subsequently occurring at the Fukushima Daiichi Nuclear Power Plant. The disaster forced many evacuees to change particular aspects of their lifestyles. However, the effect of evacuation on the new-onset of hyperuricemia have not been sufficiently elucidated. This study assessed the association between evacuation and new-onset hyperuricemia after the earthquake based on the Fukushima Health Management Survey from a lifestyle and socio-psychological perspective.Materials and methodsThis is a 7-year prospective longitudinal study included 18,140 residents (6,961 men and 11,179 women) with non-hyperuricemia who underwent both the Comprehensive Health Check and the Mental Health and Lifestyle Survey in fiscal year 2011. Associations between new-onset hyperuricemia and lifestyle- and disaster-related factors, including evacuation, were estimated using a Cox proportional hazards regression model analysis. Hyperuricemia was defined as uric acid levels > 7.0 mg/dL for men and > 6.0 mg/dL for women.ResultsDuring a median follow-up of 4.3 years, 2,996 participants (1,608 men, 23.1%, 1,388 women, 12.4%) newly developed hyperuricemia. Significant associations were observed between evacuation and onset of hyperuricemia in women (adjusted hazard ratio 1.18, 95% confidence interval, 1.05-1.32, p = 0.007), but not in men (adjusted hazard ratio 1.11, 95% confidence interval, 0.99-1.24, p = 0.067).DiscussionEvacuation after a natural disaster is an independent risk factor for the new-onset of hyperuricemia in women. The possibility of hyperuricemia developing in response to natural disasters should be considered.

Abstract Background Low tongue pressure contributes to oral frailty. It is a condition in which the pressure between the tongue, palate, and food decreases due to a decrease in tongue muscle function. Low tongue pressure interferes with chewing and swallowing and causes bloating, increasing the risk of malnutrition and aspiration pneumonia due to decreased intake. Its causes include aging, neuromuscular disease, trauma, sequelae, low nutrition, and sarcopenia. Although quantitative evaluation of tongue function has been reported to be useful for prevention of low tongue pressure, there is no report of the evaluation’s effectiveness in patients with high-risk renal failure. This study aimed to evaluate the prevalence of low tongue pressure and to clarify the relationship between the tongue pressure value and sarcopenia in peritoneal dialysis (PD) patients. Methods A cross-sectional study of 105 PD patients (67 ± 13.5 years old, 73.3% men, 26.7% women; 26.7% with diabetes) was conducted in our hospital. At the start of the study, clinical backgrounds were investigated from the patients’ medical records. Sarcopenia was diagnosed by muscle mass, grip strength, and walking speed. Inflammatory, nutritional, and cardiovascular markers were measured. Univariate and multivariate analyses with muscle and nutrition markers were performed to determine the low tongue pressure prevalence rate in PD patients. Results Low tongue pressure prevalence rate was 55.2% in patients under 65 and 64.1% in patients 65 and over. Significantly lower tongue pressure (p = 0.019) was observed in the patients in the sarcopenic group. This study deals with an application of blazed plate heat exchangers to a condenser used in a heat pump water heater. The heat transfer performance was experimentally evaluated by using a pump driven two-phase flow loop in the operating condition for water heating at the environmental temperature. The effects of number of refrigerant paths and setting orientation, such as a vertical orientation with downward refrigerant flow or a horizontal orientation, were evaluated. HFC134a was used as the refrigerant. Superheated vapor was supplied to the condenser, and heated water, and then subcooled liquid was exhausted. Water was supplied to form a counter flow heat exchange. Three kinds of heat exchangers with different refrigerant paths of 6, 10, and 14 were used. Refrigerant temperatures at the exit of each path were measured by inserted thermocouples to evaluate the flow distribution. As a result, it was shown that vertical orientation produced higher heat transfer rate than the horizontal orientation. For the horizontal orientation, heat transfer rate decreased with an increase of refrigerant channels from 10 to 14. The reason would be due to a maldistribution of the refrigerant. The deterioration in heat transfer performance for the horizontal orientation could be improved by the inclination of 15° of the heat exchanger.

Background: There are approximately 872 certified nurse practitioners (NPs) in Japan as of April 2024. However, research on the results of their specific activities is still scarce. Purpose: This study aimed to compare health care outcomes before (i.e., 2019) and after (i.e., 2021) employing NPs in cardiovascular hospitals in Japan. Methodology: We conducted a retrospective chart review and analyzed 114 patients who underwent cardiac surgery in Hospital A and 381 patients who received pacemaker implantation/replacement in Hospital B. Hospital A hired one NP for cardiac surgery service, and Hospital B hired one NP for pacemaker device service. The NPs assisted in the surgical procedures and provided postsurgical management. Results: In Hospital A, the median length of hospitalization and intubation were shorter in 2021 than in 2019 (p = .02 and .01, respectively). In Hospital B, medical fee reimbursement was lower in 2021 (p < .001) than in 2019, and the median procedure duration was shorter (p = .01), which remained statistically significant after controlling for age, comorbidities, and device types. Some outcomes improved following the employment of NPs, whereas others remained unchanged. Conclusions: Nurse practitioners managed surgical patients well and contributed to the quality care of cardiovascular medicine. Background: Middle cerebral artery (MCA) has a significantly lower incidence of anatomical variations than other intracranial arteries. We present an extremely rare case of unruptured aneurysms with the segmental duplicated MCA (d-MCA) formed a fenestrated structure at origin. Case Description: A 55-year-old female underwent direct surgery for the unruptured aneurysms at the top of the right internal cerebral artery with d-MCA. The d-MCA branches separated at the right internal cerebral artery top and had comparable with that of the main MCA trunk. Moreover, there was an anastomosis between the d-MCA branches. We diagnosed this anastomosis as segmental d-MCA. Two aneurysmal domes were identified during surgery at the origin of the d-MCA, which the main dome protruding backward was wide necked and another small one was collapsed or thrombosed protruding forward. We used a fenestrated clip for the posterior projecting dome, and the aneurysms were successfully obliterated. Conclusion: Although cerebral aneurysms associated with d-MCA are rare, there are technical difficulties in the surgical management. A fenestrated clip might be most reasonable to obtain patency of the parent arteries for the posterior projecting aneurysms if the perforators can be avoided. [ABSTRACT FROM AUTHOR]

Purpose: This study evaluated the contrast-to-noise ratio (CNR), spatial resolution, and subjective quality of dual imaging plates (DIP) intraoral radiography.Methods: The DIP and conventional single IP (CSIP) methods both used YCR DT-1 imaging plates (Yoshida Co.). The DIP, comprising a front IP (FIP) and back IP (BIP), was constructed. DIP images were synthesized from the FIP and BIP images. An aluminum step phantom was used to measure the CNR. A line pair gauge was used to measure the spatial imaging resolution. A phantom comprising a porcine mandible embedded in acrylic resin was used for subjective evaluation.Results: The CNR of the DIP image was 32% higher than that of the FIP image. The spatial resolution achieved using the FIP, DIP, and CSIP was highly comparable except above 4 line pairs/mm, where that of the CSIP was highest. In subjective evaluation, the noise in the DIP images was significantly lower than in those obtained using the FIP and CSIP.Conclusion: The CNR of the DIP was higher than that of the FIP. The risk assessment of liquefaction damage is crucial for recovery from large-scale earthquakes, which are predicted to occur in recent years. In this study, the liquefaction risk is evaluated over a wide area through effective stress analysis, considering the interaction between the soil particles and pore water. The target area of the analysis was the 23 wards of Tokyo, including the zero-meter zone, and a 3D ground model was constructed from the borehole data. A vertical columnar ground model is established at 463 locations from the 3D ground model using the material parameter determination flow. The liquefaction risk of a wide area is evaluated by performing seismic response analysis using two types of earthquake motions for each analysis point, thus showing the difference in the risk and degree of liquefaction by location. Purpose: This study was performed to determine the accuracy of measurements of bone mineral density (BMD) and cortical bone thickness (CBT) at miniscrew implantation sites in small animals and to verify the usefulness of in vivo microcomputed tomography (micro-CT). Materials and Methods: Rat femurs were scanned before and after placing miniscrews using in vivo micro-CT. The images were superimposed using a subtraction method with bone volume measurement software. At each screw site, the total BMD was calculated as the average BMD of a cylinder 1.6 mm in diameter and 2.0 mm in depth, while the cortical BMD was the average BMD of a cylinder 1.6 mm in diameter and 1.0 mm in depth. CBT was measured three times on transaxial images of the rat femurs, and the average value was used. All miniscrews were placed using the maximum torque, verified with a digital torque tester. To verify the usefulness and accuracy of in vivo micro-CT, CBT on micro-CT images was compared with that measured on histologic sections. Results: Significant correlations were observed between placement torque and cortical BMD (R = 0.572), total BMD (R = 0.732), and CBT (R = 0.788). There was a significant correlation between CBT measured via field-emission scanning electron microscopy images and CBT measured with in vivo micro-CT (R = 0.974). Conclusions: The BMD over a narrow range can be measured accurately in small animals with high reproducibility of the trabecular structure using a combination of high-resolution in vivo micro-CT and image superimposition using a three-dimensionalsubtraction method. Recently, the occurrence of large earthquakes has become a concern in Japan, prompting the need for a numerical method that can accurately predict real phenomena, such as the elastoplastic soil-water coupled finite element method. Because the elastoplastic constitutive model requires many material parameters, various soil tests and specialized knowledge are needed to determine these material parameters. In this study, a new method was proposed to determine the material parameters from the limited ground information obtained by borehole inspections and the method was validated by comparing the numerical results with monitored seismic behavior. One-dimensional seismic analyses were conducted using the material parameters estimated from borehole data near the target sites. It was confirmed that the amplitude and predominant period obtained by numerical simulation were consistent with the monitored data. As a result, it was found that the dynamic numerical result was more consistent with the monitored data when the compression index was set to be larger than that in the static analysis. Recently, wearing a mouthguard (MG) has become mandatory in contact sports. A properly fitted MG protects against sports injuries to the stomatognathic system, and stabilized the occlusal position, which might leads to improved sports performance. The present study uses a questionnaire to determine the amount of dental knowledge and motivation to wear a MG among dental college students. The current situation regarding MG use, discomfort, and experience of oral trauma and concussion was evaluated in 175 rugby football players, 63 American football players, 62 skiers, 55 karate, 27 Shorinji kempo and 32 Nippon kempo athletes who participate d in the Annual Athletic Meet for All Japanese Dental Students (414 participants). The results of the questionnaire about MG use indicated that almost all players experienced discomfort. Dental trauma was experienced in 90% and 43% of rugby football payers and karate athletes, respectively. The rates were lowest among American football players, Shorinji kempo athletes and skiers, being 5.8%, 7.4%, 12.9%, respectively. Concussion was experienced by > 36% of rugby football players and 25% of Nippon kempo athletes. Nippon kempo athletes may use a MG to protect against concussion. In conclusion, rules requiring players to wear a MG should be enforced or rules should be changed to make wearing a MG compulsory, awareness of dental students should be improved, injury should be prevented by wearing a MG and not only organizations but individual players should be educated about the necessity of wearing a MG. Purpose: The spatial resolution of cone-beam computed tomography (CBCT) in small fields of view (FOVs) is important for clinical applications. However, it is difficult to measure spatial resolution reliably due to error factors such as noise. The aim of this study was to obtain a modulation transfer function (MTF) more accurately.Methods: A CBCT apparatus was used with small FOV. An aluminum pipe slightly tilted at an inclination ratio of 77/3 (25.7) was used as the measurement phantom. The MTF was calculated from the edge image of the phantom. The actual oversampling ratio was determined by regression analysis. The experiment was repeated 16 times and the edge-spread function (ESF) was approximated by the least-square method. Furthermore, a low-pass filter (LPF) was applied to eliminate the component at frequencies above the Nyquist frequency. Finally, the MTF was calculated from the pre-processed ESF.Results: Results showed that pre-processing reduced the noise of the ESF. The MTFs at frequencies of 1.0 and 2.0 LP/mm were 0.59 and 0.18, respectively, in air and 0.52 and 0.16, respectively, in water.Conclusion: The repeating-edge oversampling method combined with ESF pre-processing improved the accuracy of the MTF under clinically relevant conditions with a phantom. Purpose: Circadian rhythm is associated with the pathogenesis of systemic disease and bone mineral metabolism. This study aimed to radiographically evaluate morphological characteristics of the interalveolar septum in circadian rhythm deficient animals.Methods: Heads of 10 brain and muscle arnt-like protein-1 (BMAL1)-knockout (KO) mice and 10 wild-type mice sacrificed at 36 weeks were imaged using micro-computed tomography. The mean depth from the cementoenamel junction to the alveolar ridge (virtual bone sounding: VBS) of the interalveolar septum between the first and second molars, and the bone mineral density (BMD) of the interalveolar septum and the mandibular inferior cortex region were calculated. Tooth diameter was also measured.Results: The VBS of the interalveolar septum in the BMAL1-KO mice was significantly deeper than that in wild-type mice. The BMD in the BMAL1-KO mice was significantly lower than in the wild-type mice in both regions. No significant difference was observed in tooth diameter between BMAL1-KO and wild-type mice.Conclusion: These results suggest that low BMD in the interalveolar septum accelerates bone resorption in the interalveolar septum in BMAL1-KO mice. The present study used an aluminum phantom to calculate accurate vertical magnification values around the mental foramen on panoramic radiography (PR) and compared corrected PR (Cor-PR) thickness measurements of the aluminum phantom and mandibular cortical width (MCW) of the human head with cone-beam computed tomography (CBCT) measurements at two fields of view (FOVs). The calculated vertical magnification value for PR around the mental foramen was 1.37. Measurements of the aluminum phantom significantly differed between Cor-PR and CBCT with an FOV of 100 mm and between CBCT with FOVs of 40 and 100 mm; however, MCW measurements did not significantly differ among the three methods. There was a very strong correlation between Cor-PR and CBCT with an FOV of 40 mm and between CBCT with FOVs of 40 mm and 100 mm, and intraobserver and interobserver agreement was good-to-excellent for all methods. This study was performed to develop a new rat model of reduced masticatory activity in order to assess the effect of this reduction on the morphology of the temporomandibular joint (TMJ) over time. Female rats were used, and ovariectomy was performed to simulate aged/postmenopausal status. Twenty-four SD rats aged 6 weeks were divided into four groups: ovariectomy/sham procedure (Ov/S); ovariectomy/reduced masticatory activity (Ov/RMA); non-Ov/S (NO/S); and non-Ov/RMA (NO/RMA). The RMA procedure involved grinding down the edges of the upper and mandibular incisors by about 3 mm and supplying the rats with a powdered diet. The bilateral TMJ was examined by microcomputed tomography at 0, 1, 2, 4, 6, and 8 weeks after the start of RMA. Condylar width was greater in the NO/S group than in the Ov/S group after the 2nd week, showing that ovariectomy reduced the width of the condyle. After the 2nd week, significant differences in condylar width were apparent between the NO/S and NO/RMA groups, and between the Ov/S and Ov/RMA groups. This RMA procedure appeared to provide a good model of reduced masticatory activity. The present findings in female rats suggest that reduction of appropriate mastication activity in the growth period results in poor growth of the mandibular condyle and immediately induces atrophy of the mandibular condyle under conditions simulating aged/postmenopausal status. We compared the diagnostic reliability of 3.0-T magnetic resonance imaging (MRI) for detection of osseous abnormalities of the temporomandibular joint (TMJ) with that of the gold standard, cone-beam computed tomography (CBCT). Fifty-six TMJs were imaged with CBCT and MRI, and images of condyles and fossae were independently assessed for the presence of osseous abnormalities. The accuracy, sensitivity, and specificity of 3.0-T MRI were 0.88, 1.0, and 0.73, respectively, in condyle evaluation and 0.91, 0.75, and 0.95 in fossa evaluation. The McNemar test showed no significant difference (P > 0.05) between MRI and CBCT in the evaluation of osseous abnormalities in condyles and fossae. The occurrence of ameloblastic fibro-odontoma (AFO) in the oral region is unusual and accounts for 1-3% of all odontogenic tumors. AFO presents mixed radiopaque patterns within the lesion with diverse findings; therefore, it is important to compare this tumor with other odontogenic tumors that radiographically present with calcified bodies. Herein, we observed the calcification patterns within the lesion in seven AFO cases (five males, two females; mean age, 8.3 years; age range, 4-13 years). Periapical and panoramic radiographs were obtained from all seven cases. Five cases underwent conventional computed tomography (CT) scanning, and one underwent cone beam CT. Classification of the calcifications primarily involved the following two characteristics on the X-rays: appearance and location of the lesions. All seven cases were located in the molar regions of the mandible in association with impacted teeth. The calcification patterns of these AFOs were mixed or inhomogeneous within the lesion with various findings, including complex odontoma-like calcifications. However, the patterns differed between panoramic radiography and CT in some cases. As part of our ongoing investigation of risk and predictive factors associated with temporomandibular disorders, we used magnetic resonance imaging (MRI) to identify risk factors for sideways disk displacement of the temporomandibular joint in 26 patients with MRI-confirmed unilateral pure sideways disk displacement (medial or lateral disk displacement) and normal positioning of the contralateral temporomandibular joint. Coronal morphologic harmonization between the condyle and fossa, angle between the axis of the ramus and condyle, and angle between the lateral pterygoid muscle (LPM) and condyle were evaluated. Only angle of the LPM related to the condyle was significantly correlated with mediolateral disk position; the angles of joints with medial, normal, and lateral disk positions were 70.2°, 66.7°, and 60.1°, respectively. These results suggest that a greater angle of the inferior head of the LPM to the axis of the condyle on axial MRI images may cause medial disk displacement, while a smaller angle may result in lateral disk displacement. The reactivities and torquoselectivities of electrocyclic ring opening reactions of fluoromethyl-substituted cyclobutenes and oxetenes were studied with M06-2X density functional theory. The torquoselectivities of a series of mono-, di-, and trifluoromethylcyclobutenes and oxetenes result from the interplay of favorable orbital interactions and closed-shell repulsions. When the substituent rotates inward, there can be a favorable interaction between the breaking ςCO bond and the ςCF* orbital (ςCO ςCF*) of the fluoromethyl group in fluoromethyloxetenes. The preference for rotation of a fluoromethyl group is decreased in trifluoromethyloxetenes because closed-shell repulsions between the breaking ςCO bond and trifluoromethyl substituent orbitals compete with the ςCO ςCF* interaction. We describe a rare case of osteochondroma of the coronoid process of mandible accompanied by severe trismus in a 14-year-old Japanese boy. The patient had initially been diagnosed as having internal derangement of the temporomandibular joint (TMJ), and conservative treatment had failed to improve the symptoms. Despite extremely limited TMJ movement, panoramic radiography revealed no abnormality, but magnetic resonance imaging suggested internal derangement of the left TMJ. Five months later, the trismus worsened and computed tomography revealed a bone-like mass confluent with the coronoid process, which was finally diagnosed as osteochondroma. We measured the gonial angle (GA) on panoramic radiography (PR) and analyzed the correlation between the GA on PR and lateral cephalometric radiography (LCR). In total, 49 PR films and LCR films from dentate young adults were evaluated. Orthodontists plotted four points (articulare, menton, posterior gonion, and lower gonion) on the PR and carefully traced them. Using a protractor, two radiologists measured the GA on LCR images. A simultaneous experimental study of two dry skulls was performed to compare the GA on LCR and PR. The GA was slightly smaller on the PR of the dry mandible than on the LCR and tended to decrease continuously with magnitude toward the Frankfort horizontal plane. The mean GA was 115.1 ± 5.2° on PR and 122.2 ± 6.4° on the LCR. The values were highly correlated (Pearson product-moment correlation coefficient, 0.801). The GA on PR was nonsignificantly smaller than that measured on LCR. The outbreak of novel coronavirus disease 2019 (COVID-19) caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has spread worldwide. Nasopharyngeal swabs are widely used in polymerase chain reaction (PCR) test to detect SARS-CoV-2. However, the collection of nasopharyngeal swabs has a series of drawbacks concerning exposure of healthcare staff, difficulty in collection, and discomfort of patients. Therefore, an alternative noninvasive sample for diagnostic of emerging viral diseases is required. The usefulness of saliva screening tests is compared to conventional swab tests in this report. Toll-like receptors (TLRs) play a key role in linking pathogen recognition with the induction of innate immunity. They have been implicated in the pathogenesis of chronic inflammatory diseases, representing potential targets for prevention/treatment. Vegetable-rich diets are associated with the reduced risk of several inflammatory disorders. In the present study, based on an extensive screening of vegetable extracts for TLR-inhibiting activity in HEK293 cells co-expressing TLR with the NF-κB reporter gene, we found cabbage and onion extracts to be the richest sources of a TLR signaling inhibitor. To identify the active substances, we performed activity-guiding separation of the principal inhibitors and identified 3-methylsulfinylpropyl isothiocyanate (iberin) from the cabbage and quercetin and quercetin 4'-O-β-glucoside from the onion, among which iberin showed the most potent inhibitory effect. It was revealed that iberin specifically acted on the dimerization step of TLRs in the TLR signaling pathway. To gain insight into the inhibitory mechanism of TLR dimerization, we developed a novel probe combining an isothiocyanate-reactive group and an alkyne functionality for click chemistry and detected the probe bound to the TLRs in living cells, suggesting that iberin disrupts dimerization of the TLRs via covalent binding. Furthermore, we designed a variety of iberin analogues and found that the inhibition potency was influenced by the oxidation state of the sulfur. Modeling studies of the iberin analogues showed that the oxidation state of sulfur might influence the global shape of the isothiocyanates.
