BackgroundThe increasing incidence of COVID-19 infection has challenged health care systems to increase capacity while conserving personal protective equipment (PPE) supplies and minimizing nosocomial spread. Telemedicine shows promise to address these challenges but lacks comprehensive evaluation in the inpatient environment. ObjectiveThe aim of this study is to evaluate an intrahospital telemedicine program (virtual care), along with its impact on exposure risk and communication. MethodsWe conducted a natural experiment of virtual care on patients admitted for COVID-19. The primary exposure variable was documented use of virtual care. Patient characteristics, PPE use rates, and their association with virtual care use were assessed. In parallel, we conducted surveys with patients and clinicians to capture satisfaction with virtual care along the domains of communication, medical treatment, and exposure risk. ResultsOf 137 total patients in our primary analysis, 43 patients used virtual care. In total, there were 82 inpatient days of use and 401 inpatient days without use. Hospital utilization and illness severity were similar in patients who opted in versus opted out. Virtual care was associated with a significant reduction in PPE use and physical exam rate. Surveys of 41 patients and clinicians showed high rates of recommendation for further use, and subjective improvements in communication. However, providers and patients expressed limitations in usability, medical assessment, and empathetic communication. ConclusionsIn this pilot natural experiment, only a subset of patients used inpatient virtual care. When used, virtual care was associated with reductions in PPE use, reductions in exposure risk, and patient and provider satisfaction.

The COVID-19 pandemic has disproportionately impacted migrant farmworkers (MFWs). MFWs have experienced higher rates of infection and death than any other occupational group in the United States due to workplace exposure risks, overcrowded housing, and limited access to personal protective equipment. Barriers to accessing COVID-19 vaccines have also contributed to these disparities, especially in rural areas where the pandemic's impact has been devastating. Mobile health clinics, in partnership with community-based organizations, are an effective method for vaccine distribution to rural communities where many MFWs live and work. Between June 2021 and October 2022, the University of Minnesota's Mobile Health Initiative (MHI) organized health fairs in southern Minnesota to distribute vaccines to MFWs and their families. The success of these events can be attributed to partnering with trusted local organizations, bridging geographic barriers, ensuring language-concordant care, and offering multiple health services in one place. MHI's health fairs serve as a model for future mobile vaccination events. As the COVID-19 pandemic has ended as of this time, future pandemics may occur, and equitable vaccine access must be a priority for MFWs. Background Musculoskeletal health problems are one of the greatest healthcare expenses in the United States but patient-driven screening procedures to detect risk factors do not exist. Hypothesis/Purpose The purpose was to establish the inter-rater reliability of the Symmio Self-Screen application in untrained individuals and to investigate its accuracy to detect MSK risk factors such as pain with movement, movement dysfunction, and decreased dynamic balance. Study Design Cross-Sectional Methods Eighty (42 male, 38 female) healthy individuals mean age 26.5 ± 9.4 participated in the study. The inter-rater reliability of Symmio application was established by comparing self-screen scores from untrained subjects with the results simultaneously determined by a trained healthcare provider. Each subject was evaluated for pain with movement, movement dysfunction, and deficits in dynamic balance by two trained evaluators who were blinded to the Symmio results. The validity of Symmio was determined by comparing self-screen performance dichotomized as pass or fail with the reference standard of pain with movement, failure on the Functional Movement Screen™, and asymmetry on the Y Balance Test-Lower Quarter™ using three separate 2x2 contingency tables. Results The mean Cohen's kappa coefficient was 0.68 (95% CI, 0.47-0.87) and the absolute agreement was 89% between self-assessment of subjects and the observation of a trained healthcare provider. There were significant associations for the presence of pain with movement (p=0.003), movement dysfunction (p=0.001), and dynamic balance deficits (p=0.003) relative to poor Symmio performance. The accuracy of Symmio to identify pain with movement, movement dysfunction, and dynamic balance deficits were 0.74 (95% CI, 0.63-0.83), 0.73 (95% CI, 0.62-0.82), and 0.69 (95% CI, 0.57-0.79), respectively. Conclusions The Symmio Self-Screen application is a reliable and feasible screening tool that can be used to identify MSK risk factors. [ABSTRACT FROM AUTHOR]

Dairy farms that had participated in previous and ongoing projects with the National Farm Medicine Center (NFMC), Migrant Clinicians Network (MCN), and Upper Midwest Agricultural Safety and Health Center (UMASH) were asked to participate in a 17-question survey by phone or email to investigate biosecurity principles on Minnesota and Wisconsin dairy farms in response to COVID-19 and the effects of the pandemic on the dairy industry. Three additional farms were recruited via a press release published in agricultural newsletters. Of 76 farms contacted, 37 chose to participate in this study from June to July 2020. In response to the COVID-19 pandemic, dairies have implemented or increased biosecurity measures and COVID-19 precautions. Dairies reported adequate personal protective equipment for their workers, though face masks were not required on most dairies (n = 32, 86%). Producers were concerned about the safety of their families, maintaining a healthy workforce, and keeping their farms profitable. Access to healthcare was not perceived to be an issue for their workers. One-quarter of dairies reported COVID-19 infections on their farms. Even though the majority had an isolation protocol in place if someone on the farm were to become ill, less than half of respondents felt their farm was protected against COVID-19. Two-thirds of producers have not had to decrease production, and a majority of operations have not furloughed or terminated employees due to COVID-19. Our data suggest that dairy farms in Minnesota and Wisconsin have implemented biosecurity and safety measures in response to COVID-19. These measures can be improved. Farms would benefit from additional guidance and education on implementation of personal protective measures and disease prevention strategies to keep workers employed and safe. Bovine tuberculosis is caused by Mycobacterium bovis (M. bovis), which infects both humans and cattle. In 2018, a dairy farm in Wisconsin was affected by M. bovis, including a farmworker with exposure to the affected herd. Largely eradicated by effective public health strategies in the United States, most cases are now associated with risk factors including occupational hazards, food consumption, and iatrogenic infections. M. bovis continues to cause disease worldwide affecting certain at-risk populations in the United States. Infections more often result in extrapulmonary sequelae and resistance to pyrazinamide is universal. Thus, successful treatment depends on early and correct identification of the mycobacterium species. A One Health approach to control this re-emerging disease is crucial. [ABSTRACT FROM AUTHOR]

Here we present the first reported case of a noninvasive Rhizopus fungal ball confined to the bladder of a patient with poorly controlled diabetes and right flank pain. The patient developed bilateral hydronephrosis after several hospital admissions for urinary tract infections with multiple failed courses of antibiotics. During cystoscopy to replace a ureteral stent, he was found to harbor a fungal ball in the bladder that was removed and grew Rhizopus in culture. Patient received treatment with amphotericin B and transitioned to long-term posaconazole therapy. This case highlights the importance of considering fungal agents in urinary tract infections, especially in persistent or refractory cases, and the role of the clinical microbiology laboratory in correct identification of the infectious source. This paper presents the first architecture for a cloud-based, secure SCADA-as-a-Service solution. Our system, SKYDA, leverages the unique level of scalability and reliability offered by a cloud-based infrastructure while mitigating the domain-specific challenges that arise when moving SCADA from a standard utility control center to the cloud. SKYDA applies high-performance messaging and fault tolerance protocols to both the network and the SCADA Master application itself, resulting in a SCADA system that is easier to deploy and offers a lower total cost of ownership, significantly higher availability, better security and better performance than is possible today. A copper(II)-mediated diamination of 1,3-butadienes is reported. The reaction employs an inorganic base, copper dibromide as a reagent, and saccharin as the nitrogen source. Providers of critical infrastructure services strive to maintain the high availability of their SCADA systems. This paper reports on our experience designing, architecting, and evaluating the first survivable SCADA system—one that is able to ensure correct behavior with minimal performance degradation even during cyber attacks that compromise part of the system. We describe the challenges we faced when integrating modern intrusion-tolerant protocols with a conventional SCADA architecture and present the techniques we developed to overcome these challenges. The results illustrate that our survivable SCADA system not only functions correctly in the face of a cyber attack, but that it also processes in excess of 20 000 messages per second with a latency of less than 30 ms, making it suitable for even large-scale deployments managing thousands of remote terminal units. An intramolecular diamination of acrylates is reported using sulfamates as nitro sources. This reaction proceeds under palladium(II) catalysis with copper as oxidant gives rise to anti-configured 2,3-diamino carboxylates as bicyclic sulfamate derivatives. An aminobrominated intermediate within the diamination reaction was isolated that allowed to clarify the reaction mechanism and to rationalize the observed preferential product stereochemistry. Background: Displaced olecranon fractures constitute a challenging problem for elbow surgeons. The purpose of this study is to evaluate the role of suture anchor fixation for treating patients with displaced olecranon fractures. Methods: A retrospective review was performed for all consecutive patients with displaced olecranon fractures treated with suture anchor fixation with at least 2 years of clinical follow-up. Surgical repair was performed acutely in all cases with nonmetallic suture anchors in a double-row configuration utilizing suture augmentation via the triceps tendon. Osseous union and perioperative complications were uniformly assessed. Results: Suture anchor fixation was performed on 17 patients with displaced olecranon fractures. Functional outcome scores were collected from 12 patients (70.6%). The mean age at the time of surgery was 65.6 years, and the mean follow-up was 5.6 years. Sixteen of 17 patients (94%) achieved osseous union in an acceptable position. No hardware-related complications or fixation failure occurred. Mean postoperative shortened disabilities of the arm, shoulder, and hand (QuickDASH) score was 3.8±6.9, and mean Oxford Elbow Score was 47.5±1.0, with nine patients (75%) achieving a perfect score. Conclusions: Suture anchor fixation of displaced olecranon fractures resulted in excellent midterm functional outcomes. Existing Byzantine-resilient replication protocols satisfy two standard correctness criteria, safety and liveness, even in the presence of Byzantine faults. The runtime performance of these protocols is most commonly assessed in the absence of processor faults and is usually good in that case. However, faulty processors can significantly degrade the performance of some protocols, limiting their practical utility in adversarial environments. This paper demonstrates the extent of performance degradation possible in some existing protocols that do satisfy liveness and that do perform well absent Byzantine faults. We propose a new performance-oriented correctness criterion that requires a consistent level of performance, even with Byzantine faults. We present a new Byzantine fault-tolerant replication protocol that meets the new correctness criterion and evaluate its performance in fault-free executions and when under attack. Historians have long demarcated between the public, political, and economic writings of revolutionary socialist Rosa Luxemburg and her personal affinity for plants and animals. This article, however, discovers and articulates a Luxemburgian ecosocialism across her work. This paper argues that Luxemburg interpreted nature as possessing its own order that resists the imposition of capitalist and imperialist (dis)orders. Luxemburgian ecosocialism, therefore, lies between the appreciation of beauty one finds in the Marxism of William Morris and the active participation of nature in the historic revolt against imperialism one finds in the Book of Exodus. According to Luxemburgian ecosocialism, that which unites Luxemburg's letters and Herbarium to her Die Akkumulation des Kapitels is a confidence that empires overextend themselves in their exploitation of human beings and nature, and that the natural order accordingly fights back. Beyond Luxemburg, this paper emphasizes the importance of fiction and illusion for ecosocialist storytelling, and argues for an ecosocialist vision that takes as its starting point not the elitism of aesthetic preference but radical forms of solidarity with suffering creation. Sabbaticals are an important feature of academia for faculty and their institutions. Whereas sabbaticals are common in institutions of higher learning, little is known about their role and utilization in US medical schools. This perspective piece examining sabbaticals in medical school faculty was undertaken at a time that well-being of health professionals was increasingly being recognized as a workforce health priority. We surveyed associate deans at US medical schools in 2021 about faculty who had taken sabbaticals within the past 3 years, the parameters of the sabbaticals, and institutional policies and respondents' predictions of future sabbatical use. A total of 53% of respondents reported any faculty had taken sabbaticals in the past 3 years (M = 6.27; Median = 3; range = 1-60). Institutions rated enhancing research as the most important objective, while recognizing other benefits. Sabbaticals were more commonly taken by male, white, senior faculty PhDs. Details about sabbaticals, including eligibility, expectations, length, financial support, and benefits were reviewed. Most (54.8%) respondents expected no change in the number of faculty seeking sabbaticals. Nearly all anticipated the COVID-19 pandemic would not affect sabbatical policies. In contrast to other institutions of higher learning, sabbatical-taking by medical school faculty is rare. We explore factors that may contribute to this phenomenon (eg, the tripartite mission, faculty clinical responsibilities, culture of medicine, and student debt). Despite financial and other barriers, a closer look at the benefits of sabbaticals is warranted as a mechanism that may support faculty well-being, retention, and mental health. To understand COVID-19 vaccine perspectives among healthcare workers serving diverse communities. A dual English/Spanish survey was distributed to healthcare workers in the United States from 3/12/2021–4/22/2021 by the Migrant Clinicians Network, Society of Refugee Healthcare Providers, a Federally Qualified Healthcare Center, and social media advertisement to general primary care workers. 517 responses were at least 50% complete and included in the analysis. Among these, 88% (457/517) indicated vaccine acceptance. Factors associated with acceptance included not reporting any vaccine concerns, identifying as male, ≥65 years of age, being a physician or advanced practice provider, and interacting directly with patients from refugee, immigrant, and migrant (RIM) communities. Participants identified educational information as most helpful for themselves when making a vaccine decision, but a healthcare provider's recommendation as most helpful for their patients. Healthcare workers, especially those serving RIM communities, are vaccine accepting. This essay deconstructs the godhead that Tolkien constructs in his mythopoeia. Tolkien's polychronicon, The Silmarillion , splits the godhead between a creator God and a pantheon of gods and goddesses. Tolkien claims that Ilúvatar is a Yahweh-like God and the primary deity; on the other hand, the Valar, the fourteen gods and goddesses created by this primary God, who assist in creation, shape the world, have power over elements, and reign as 'mistaken gods' among the Elves, Dwarves, and Men, are not deities. This split of godhead is ignored, and the mythopoeic deity acclaimed as the biblical God and his angels is upheld as a Christian allegory. The essay negates the Christian parallels associated with Ilúvatar and Valar and establishes that Tolkien packages polytheism as monotheism. Monotheism does not permit secondary god/gods. Polytheism on the other hand often features an abstract creator God who creates a polytheistic pantheon. Tolkien's model, which features a Creator deity and a pantheon of created deities, falls under the second category. The essay infers that Tolkien's two-tier godhead firstly invalidates the norm of monotheism, and secondly conforms to creator deity and created deity structure of polytheism. [ABSTRACT FROM AUTHOR]

The article is an examination of the persons and trials of Soghomon Tehlirian and Sholem Schwartzbard, their political assassinations as acts of vengeance for genocide and pogroms, their trials and subsequent acquittals. It is also an examination of the influences of these two events on the evolved thinking of Raphael Lemkin on his conceptualization of the needs for an international law contra genocide. Finally, it also elaborates on what information is now available on both men and their associations, and what was known and unknown to Lemkin and whether or not these two cases remained centrally important to his understandings. The story of Lot is one of many shared by the Qur'an and the Torah, and Lot's offer of his two daughters to his people is presented in a similar way in the two books. This article compares the status of Lot in the Qur'an and Torah, and explores the moral dimensions of his character, and what scholars of the two religions make of this story. The significance of the episodes in which Lot offers his daughters to his people lies in the similarities and differences of the accounts given in the two books and the fact that, in both the past and the present, this story has presented moral problems and criticism has been leveled at Lot. Context is crucial in understanding this story, and exploration of the ways in which Lot and his people are presented is also useful in terms of comparative studies of the two scriptures. This article is divided into three sections: the first explores the depiction of Lot in the two texts, the second explores his moral limitations, and the third discusses the interpretations of various exegetes and scholars of the two books. Although there are similarities between the Qur'anic and Talmudic accounts of this episode, it is read differently by scholars from the two religions because of the different contexts of the respective accounts. A literary criticism of the novel "Don Quijote" by Miguel de Cervantes is presented. Topics discussed include its female character Zoraida; matriarchs of Israel: Sarah, Rebecca, Leah, and Rachel; conversion of religion; Zoraida's comparison with Tamar from "The Book of Genesis" or the Christian Old Testament. Other topics include Cervantes' argument in favor of tolerance; Jesus of Nazareth; and Zoraida's letter.

The article discusses whether it is necessary to use technology like excavative methods to uncover the original meanings of biblical texts and its effect on the general understanding of the text. To support this discussion the author cites the example of existence of Kings David and Solomon as in the books and the scientific proofs for the same. Also mentioned is the author's research on deuteronomistic historiography where she addresses the topic of the process of writing history.

Although The Last of the Just was awarded the Prix Goncourt in 1959, the novel and its author have been forgotten. The accusations of plagiarism were such a hard offence to the Polish-Francophone author that he nearly stopped writing as a Jew and a (although oblique) witness of the Shoah. He turned to another Diaspora in his subsequent novel, La Mulâtresse Solitude (1972) and published novels with his Guadeloupean wife Simone Schwarz-Bart in which the Shoah and slavery are intertwined. In this article, I revisit The Last of the Just, which is a masterpiece because, as a hybrid form, it combines lamentation and encyclopaedic narrative, Talmudic legend and Yiddish folktales, marvellous realism and Borgesian 'journalism'. I illustrate how Schwarz-Bart's chronicle of centuries of anti-Semitism in eight European countries offers a vast chronicle 'preparing' for Auschwitz and how his dynasty of the Lévy family, elected as being the Lamed-Vov, sheds light on the unbearable tragedy and the urgent necessity to reclaim and to remember the events of the 'Last of the Just Man', killed six million times.
