Abstract

A depressão maior e a subsindrômica são altamente prevalentes na esclerose múltipla (EM). A avaliação do funcionamento psicossocial requer instrumentos psicométricos validados. As medidas de auto-relato oferecem várias vantagens, mas não existe um questionário preciso e válido para diagnosticar os sintomas depressivos na EM. O Inventário de Depressão de Beck (IDB) e o Questionário de Saúde Geral (QSG) foram validados e são amplamente usados no contexto neuropsicológico brasileiro. No IDB sintomas de EM podem ser interpretados como sintomas de depressão. Nós examinamos as propriedades psicométricas do IDB, QSG e dois questionários de auto-relato para fadiga (CPF-MS) e auto-eficácia (MSSE) na EM. Todas as escalas apresentaram exceletentes coeficientes de consistência interna.O IDB e o QSG discriminaram a amostra de EM de um grupo controle. Intercorrelações significativas foram observadas entre os escores nas diversas escalas de auto-relato, mas não entre estas e indicadores tradicionais de déficit neurológico (IA e EDSS). Os sintomas depressivos e a fadiga podem representar uma dimensão de comprometimento neurológica distinta dos déficits físicos e sensoriais. Questionários de auto-relato podem ser úteis no diagnóstico de depressão e sofrimento psíquico, mesmo quando estes não atingem níveis sindrômicos compatíveis com um diagnóstico categorial.

Em razão dos avanços no conhecimento da esclerose múltipla (EM), dos novos critérios diagnósticos e ensaios terapêuticos publicados, a atualização e expansão das diretrizes para orientação do neurologista brasileiro em relação ao tratamento da EM se tornam necessárias. O Consenso Expandido do BCTRIMS enfatiza a individualização do tratamento a critério do neurologista, e a necessidade de informação do paciente quanto aos potenciais benefícios e riscos das drogas, assim como do Consentimento Informado e compromisso de realização de controles periódicos. Na ausência de evidências científicas favorecendo uma determinada droga o médico deve considerar o custo diferencial dos imunomoduladores ao fazer sua opção terapêutica. As várias situações clínicas e os diferentes agentes modificadores da doença, assim como as outras intervenções terapêuticas de eficácia no tratamento são consideradas à luz das classes de evidências científicas e dos tipos de recomendação, aceitos pela comunidade científica internacional. O Consenso Expandido do BCTRIMS pode servir de modelo para outros países em desenvolvimento.

Abstract

Background: Patients with anti-aquaporin-4 antibody seropositive (AQP4-IgG+) neuromyelitis optica spectrum disorders (NMOSDs) frequently suffer from optic neuritis (ON) leading to severe retinal neuroaxonal damage. Further, the relationship of this retinal damage to a primary astrocytopathy in NMOSD is uncertain. Primary astrocytopathy has been suggested to cause ON-independent retinal damage and contribute to changes particularly in the outer plexiform layer (OPL) and outer nuclear layer (ONL), as reported in some earlier studies. However, these were limited in their sample size and contradictory as to the localisation. This study assesses outer retinal layer changes using optical coherence tomography (OCT) in a multicentre cross-sectional cohort.Method: 197 patients who were AQP4-IgG+ and 32 myelin-oligodendrocyte-glycoprotein antibody seropositive (MOG-IgG+) patients were enrolled in this study along with 75 healthy controls. Participants underwent neurological examination and OCT with central postprocessing conducted at a single site.Results: No significant thinning of OPL (25.02±2.03 µm) or ONL (61.63±7.04 µm) were observed in patients who were AQP4-IgG+ compared with patients who were MOG-IgG+ with comparable neuroaxonal damage (OPL: 25.10±2.00 µm; ONL: 64.71±7.87 µm) or healthy controls (OPL: 24.58±1.64 µm; ONL: 63.59±5.78 µm). Eyes of patients who were AQP4-IgG+ (19.84±5.09 µm, p=0.027) and MOG-IgG+ (19.82±4.78 µm, p=0.004) with a history of ON showed parafoveal OPL thinning compared with healthy controls (20.99±5.14 µm); this was not observed elsewhere.Conclusion: The results suggest that outer retinal layer loss is not a consistent component of retinal astrocytic damage in AQP4-IgG+ NMOSD. Longitudinal studies are necessary to determine if OPL and ONL are damaged in late disease due to retrograde trans-synaptic axonal degeneration and whether outer retinal dysfunction occurs despite any measurable structural correlates. Abstract

Dengue virus infection is a disease with high incidence in some tropical and subtropical countries. A variety of neurological complications of dengue fever (DF) has been described including two cases with the phenotype of neuromyelitis optica spectrum disorder (NMOSD). However, aquaporin-4 serostatus was unknown or negative in these patients. We report two patients with NMOSD occurring in association with DF. The first patient presented with brainstem symptoms and the second one with isolated unilateral optic neuritis. Both patients tested positive for serum AQP4-antibody. Abstract

Dengue fever (DF) is a common arbovirosis in tropical and subtropical countries and may be associated with a wide range of neurological complications. We describe a 41-year-old man who developed weakness in the right arm and lower limbs, paresthesia in the upper and lower limbs, and sphincter disturbance four weeks following DF. Examination disclosed a wheel-chair bound patient with urinary catheter, areflexia in the lower limbs, and a sensation level at T10. Spinal magnetic resonance imaging showed diffuse lesions with contrast-enhanced areas extending from the medullary-cervical junction to the conus medullaris and cauda equina. A review of the literature reveals that this is the first report of clinical and imaging signs of myeloradiculitis with cauda equina involvement following DF infection. [ABSTRACT FROM AUTHOR]

Abstract

Two patients with longitudinally extensive myelopathy were initially biopsied for suspected spinal cord tumors. Both patients were later diagnosed with neuromyelitis optica spectrum disorders (NMOSD) supported by their AQP4-seropositivity. Pathological review of both biopsies revealed demyelinated lesions with thickened vessel walls and tissue rarefaction. Immunohistochemical staining demonstrated findings compatible with acute NMOSD lesions in one case while the other case exhibited findings consistent with chronic NMOSD lesions. A pre-biopsy differential diagnosis of longitudinally extensive spinal cord tumors should include NMOSD. Specific biopsy features, such as cystic changes with vascular wall thickening and astrocyte injury, should raise suspicion for NMOSD. [ABSTRACT FROM PUBLISHER]

Abstract

Background: Neuromyelitis optica (NMO) is considered relatively more common in non-Whites, whereas multiple sclerosis (MS) presents a high prevalence rate, particularly in Whites from Western countries populations. However, no study has used ancestry informative markers (AIMs) to estimate the genetic ancestry contribution to NMO patients. Methods: Twelve AIMs were selected based on the large allele frequency differences among European, African, and Amerindian populations, in order to investigate the genetic contribution of each ancestral group in 236 patients with MS and NMO, diagnosed using the McDonald and Wingerchuck criteria, respectively. All 128 MS patients were recruited at the Faculty of Medicine of Ribeirão Preto (MS-RP), Southeastern Brazil, as well as 108 healthy bone marrow donors considered as healthy controls. A total of 108 NMO patients were recruited from five Neurology centers from different Brazilian regions, including Ribeirão Preto (NMO-RP). Principal Findings: European ancestry contribution was higher in MS-RP than in NMO-RP (78.5% vs. 68.7%) patients. In contrast, African ancestry estimates were higher in NMO-RP than in MS-RP (20.5% vs. 12.5%) patients. Moreover, principal component analyses showed that groups of NMO patients from different Brazilian regions were clustered close to the European ancestral population. Conclusions: Our findings demonstrate that European genetic contribution predominates in NMO and MS patients from Brazil. [ABSTRACT FROM AUTHOR]

Abstract

Damage to the hemispheric white matter is observed both in multiple sclerosis (MS) and normal aging, causing neuropsychological deficits in tasks of executive function, working memory (WM), and speed of information processing (SPEED). In this study two main hypotheses were investigated: (a) that SPEED may constitute a determining factor of WM impairment in MS patients and in aging individuals; (b) that self-reported depressive symptoms may aggravate cognitive deficits in MS patients. The study investigated 31 elderly MS patients without depression, 36 depressed ones, 108 demographically comparable controls and 100 community recruited senior participants. Performance of the groups was compared on a self-report measure of depression and on a WM test battery. Results showed that WM performance depends on SPEED, depressive symptoms, age, and schooling - variables which interact in complex ways. [ABSTRACT FROM AUTHOR]
