Nasopharyngeal carcinoma (NPC) is a common malignant tumor in China. Radiotherapy is the first-line treatment. After appropriate radiotherapy, about 5%-15% patients experience recurrence. In view of the poor efficacy and high incidence of severe late toxicities associated with re-irradiation, salvage surgery by the transnasal endoscopic approach is recommended for recurrent NPC (rNPC). Compared with re-irradiation, endoscopic surgery can better prolong survival, improve the quality of life, and reduce complications and medical expenses of patients with rNPC. However, the complexity of the nasopharyngeal skull base enhances the difficulty and risk of surgery. Expanding the boundary of surgical resection remains a clinical challenge for otolaryngologists. In this regard, to help more advanced patients with rNPC, the surgical innovative system of NPC needs to be established by multi-disciplinary cooperation, involving skull base anatomy-based investigation, appropriate administration of the internal carotid artery (ICA), repair of skull base defect, and establishment of various types of endoscopic endonasal nasopharyngectomy.

The demand for lithium resources is growing rapidly due to the continuous development of the lithium‐ion battery, which plays an important part in the renewable energy industry. Global sources of lithium are ores and brine, of which 59% are distributed in saline brine. However, the significant lithium resources in saline brine have not been fully utilized. The electrochemical deintercalation method (EDM) for lithium extraction from saline brine is a promising technique because of its environmental friendliness, high selectivity, and cost‐effectiveness. Nevertheless, the application of EDM is greatly limited by the easy dissolution of electrode materials like LiMn2O4 and the cost of mass production. Also, there are a few existing review articles on the EDM for lithium extraction. To address this gap, this review provides a comprehensive overview of the current methods for lithium extraction from saline brine, systematically summarizes the technical status of the EDM, and pays special attention to the preparation method and modification of electrode materials. The deployment of cloud services enables users to outsource their data to cloud servers and retrieve the target data efficiently. However, the application of quantum computers in clouds would be realized from the recent breakthrough results of quantum computers. Existing authentication schemes based on conventional hardness assumptions will be confronted with quantum attackers in the near future. A lattice-based authentication scheme resolves this tension. Although, existing lattice-based authentication schemes employing identity-based signcryption (IBS), the security against misbehaved private key generators (mPKG) is not well-considered, which may cause privacy issues in some application scenarios since the mPKG can generate the secret key for any given identity. This paper proposes an IBS-based authentication scheme for cloud storage dubbed LAMA that thwarts adversaries who have quantum computing power. We integrate the IBS scheme with the lattice-based cryptography, which is the variant of existing IBS-based authentication schemes and is post-quantum secure. We integrate identity certifying authority (ICA) with private key generator (PKG) to ensure security against mPKG. Our comprehensive security proof demonstrates that LAMA is indistinguishable against chosen plaintext attacks and secure against mPKG. We conduct a comprehensive performance evaluation of LAMA, which demonstrates the high efficiency of LAMA in terms of computation and communication overhead and proves that it can be easily deployed on low configured user devices. Objective: To investigate the effects of endoscopic endonasal surgery (EES) on longitudinal quality of life (QoL) in patients with malignant tumors of the anterior skull base. Methods: Eligible patients prospectively completed the Anterior Skull Base Surgery Questionnaire (ASBQ) and the 22-item Sino-Nasal Outcome Test (SNOT-22) questionnaires in referring to 3 different periods throughout their treatment and recovery. Results: Forty patients were included. The median volume coronal maximum length of the tumor was 3.6 cm (95% CI 2.7– 4.1cm). Overall QoL significantly worsened at 1 month postoperatively but returned to baseline after 1 year. Unrelieved symptoms in specific domains prompted further evaluation of individual items. Transient worsening of taste (p=0.011) and olfaction (p=0.004) lasted for 1 month but gradually relieved within the first postoperative year, but vision consistently worsened over the course of the treatment (p=0.126). Age> 50 years (p< 0.001), comorbidities (p< 0.001), tumor necrosis (p< 0.001) and recurrence (p=0.001) were associated with worse preoperative QoL. Poor long-term QoL was noted in those undergoing adjuvant therapy (p=0.032). Overall ASBQ scores (p=0.024), subdomain scores in specific symptoms (p=0.016), and vision scores (p=0.009) were worse only in patients with the greater coronal maximum diameter at 1-month postoperatively. Greater coronal maximum diameter was related to worse preoperative subdomain scores regarding specific symptoms (p=0.030) and decreased postoperative long-term decreased vision scores (p=0.014). Conclusion: Long-term site-specific and sinonasal QoL eventually stabilized after EES. Greater coronal maximum diameter was significantly associated with worsened vision function. Temporarily worse olfactory, vision, and taste function may be tied to decreased short-term QoL. [ABSTRACT FROM AUTHOR]

Background: As a cancer stem cells (CSCs) surface marker, Lgr5 plays an important role in the signal transduction of cancer cells and is a potential biomarker for cancer diagnosis and prognosis. However, the expression and prognostic value of Lgr5 in recurrent nasopharyngeal carcinoma (rNPC) remains ambiguous. Materials: We used RNA sequencing to screen differentially expressed mRNAs in eleven specimens of rNPC tissues and five fresh adjacent normal tissue samples and the CSC marker, Lgr5, was identified. The expression level of Lgr5 in rNPC samples was also detected by immunohistochemistry and Western blot assay. The chi-square test was used to analyze the relationship between the clinicopathological variables and the immunostaining of Lgr5. The Log-rank method was used for prognosis analysis. The Cox regression model was used for univariate and multivariate analysis. Results: Significantly elevated expression of Lgr5 in the rNPC tissues was observed compared to the normal tissues using RNA sequencing, Western blot and immunohistochemistry. The expression of Lgr5 was significantly correlated with the T stage (P=0.014). High Lgr5 expression (P=0.007), tumor necrosis (P=0.013) and WHO type II (P=0.043) in rNPC patients exhibited worse overall survival (OS). Lgr5 expression was proved to be an independent risk factor for OS (P=0.035) in multivariate analyses, and had promising predictive value for survival and recurrence in rNPC patients (area under the ROC curve: 0.711 and 0.665, P=0.017 and 0.028, respectively). Conclusion: Lgr5 as a CSC marker is a promising therapeutic target and could be employed to predict the survival prognosis of rNPC patients. [ABSTRACT FROM AUTHOR]

The new DLL injection method and its variants can prevent the injected process from calling the common system API to load the injected DLL module so that the malicious module is invisible to the LDR linked list of the process. Traditional injection detection methods have low accuracy in forensic detection of new injection attacks. To solve this problem, this paper proposes a code injection covert memory page detection and forensic detection forensic algorithm based on a memory structure reverse analysis named MRCIF. First, the physical memory pages containing DLL features from the memory image are located, and a sub-algorithm is designed for mapping physical memory space and virtual memory space, thus realizing the reverse reconstruction of the physical page subset corresponding to the DLL code module. Then, in the virtual memory space, the LDR linked list structure of the process is reversely reconstructed, and a reverse reconstruction algorithm of the DLL virtual page subset is developed to reconstruct its virtual space. Finally, a DLL injection covert page detection sub-algorithm is designed based on the physical memory page subset and virtual space page subset. The experimental results indicate that MRCIF achieves an accuracy of 88.89%, which is much higher than that of the traditional DLL module injection detection method, and only MRCIF can accurately detect the Virtual Address Descriptor (VAD) remapping attack. Electrochemical nitrate reduction to ammonia (eNO3RR) is a green and appealing method for ammonia synthesis, but is hindered by the multistep chemical reaction and competitive hydrogen generation. Herein, the synthesis of 2D SnS nanosheets with tailored interlayer spacing is reported, including both expansion and compression, through the active diatomic Pt‐Ce pairs. Taking together the experimental results, in situ Raman spectra, and DFT calculations, it is found that the compressed interlayer spacing can tune the electron density of localized p‐orbital in Sn into its delocalized states, thus enhancing the chemical affinity towards NO3− and NO2− but inhibiting hydrogen generation simultaneously. This phenomenon significantly facilitates the rate‐determining step (*NO3→*NO2) in eNO3RR, and realizes an excellent Faradaic efficiency (94.12%) and yield rate (0.3056 mmol cm−2 h−1) for NH3 at −0.5 V versus RHE. This work provides a powerful strategy for tailoring flexible interlayer spacing of 2D materials and opens a new avenue for constructing high‐performance catalysts for ammonia synthesis. Background: The choice between endoscopic surgery and re‐radiotherapy as the main treatment modality in patients with advanced recurrent nasopharyngeal carcinoma (rNPC) remains controversial. Therefore, in this study, we compared the efficacies of endoscopic surgery and intensity‐modulated radiotherapy (IMRT) in patients with rNPC. Methods: All patients with advanced rNPC (T3 and T4) who underwent salvage treatment were enrolled from January 2009 to December 2020. Overall survival (OS) was analyzed using a log‐rank analysis. Univariate and multivariate analyses of OS were performed using a Cox regression model. Common treatment‐related complications of endoscopic surgery were compared with those of IMRT. Results: The numbers of patients with T3 and T4 tumors were 163 (64.2%) and 91 (35.8%), respectively; 192 patients underwent endoscopic surgery, 51 received IMRT, and 11 received three‐dimensional conformal radiotherapy (3D‐CRT). The 3‐year OS of patients treated with endoscopic surgery was 59.3%, which was significantly higher than that of patients treated with IMRT (34.7%, p < 0.001) or 3D‐CRT (43.6%, p = 0.012). Multivariate analyses showed that IMRT was an independent risk factor for OS compared with endoscopic surgery (hazard ratio, 2.068; 95% confidence interval, 1.395–3.069, p < 0.001). Complications of aural fullness (p = 0.001), nasopharyngeal necrosis (p = 0.004), nasopharyngeal hemorrhage (p = 0.004), dysphagia (p < 0.001), and cerebral infarction (p = 0.030) were significantly lower in the endoscopic surgery group than in the IMRT group. Conclusion: Endoscopic surgery may be a more promising salvage treatment than IMRT to maximize survival and minimize treatment‐related complications in advanced rNPC. Background: American Joint Committee on Cancer/Union for International Cancer Control (AJCC/UICC) rT staging have great clinical impracticality. The aim of the present study was to establish a new rT staging to guide endoscopic surgery for the treatment of recurrent nasopharyngeal carcinoma (rNPC). Methods: This surgical rT staging (named Fudan rT staging) was constructed using two significant risk factors: the distance from the tumor margin to the internal carotid artery, and dural invasion. Log‐rank and receiver operating characteristic (ROC) curve analyses were used to evaluate its effectiveness. Results: Fudan rT staging can effectively separate the overall survival (OS) and progression‐free survival (PFS) of patients with rNPC according to the different rT stages (p < 0.05). In addition, ROC analysis showed that the Fudan rT staging exhibited enhanced prognostic value for OS and PFS compared with the AJCC/UICC rT staging. Conclusions: The innovative Fudan rT staging has a better predictive value for the survival of patients with rNPC than AJCC/UICC rT staging. [ABSTRACT FROM AUTHOR]

The elastoplastic behavior of the contact interface, powerfully influencing the contact stress distribution, is investigated to reinforce the uniformity of the contact stress distribution in this paper. Since the contact interface elastoplastic behavior has predominant and direct effects on contact deformation, whereas different contact deformation distributions can result in completely different contact stress distributions, it is theoretically feasible to enhance the uniformity of the contact stress distribution by optimizing the contact interface elastoplastic behavior. To realize this aim, a heuristic-based elastoplastic behavior optimization algorithm is developed, in which the elastoplastic behavior of the contact interface is optimized by changing the yield strength of the materials around contact interface. The uniformity of the contact stress is represented by its variance, and is defined as the objective function. Bilinear material model and von Mises yield criterion are adopted. Two contact cases are presented to evaluate the effectiveness of the proposed elastoplastic behavior optimization algorithm in terms of improving the uniformity of the contact stress. Background: Sinonasal inverted papilloma (IP) is a rare and benign epithelial tumor in the sinonasal tract. Recent study suggested the potential role of chronic inflammation in the pathogenesis of IP. This study aims to compare the inflammatory pattern, the capacity of epithelial cell proliferation and EGFR mutation status of unilateral IP with contralateral polyp tissue. Methods: Sixteen patients with unilateral IP and contralateral nasal polyps (NP) were identified through a retrospective chart review. The neutrophil and eosinophil infiltration in IP and NP were assessed by immunostaining for neutrophil elastase and major basic protein (MBP). Immunohistochemistry was also used to assess the expression of FoxM1, Ki67 and cyclin D1 in IP tissue and contralateral NP. Sanger sequencing was used to evaluate the EGFR mutations. Results: The neutrophil count in IP was significantly higher than contralateral NP and 68.8% patients presented with neutrophilic inflammation, whereas only 37.5% contralateral NP tissue showed neutrophilic inflammation. The percentage of positive FoxM1-staining cells was significantly increased in IP, and positively correlated with the percentage of cells with positive staining for cyclin D1 and ki67 as well as neutrophil counts. EGFR exon 20 insertions were detected in 14 (87.5%) IP samples and no EGFR mutations were found in contralateral NP sample. Conclusion: Our study demonstrated distinct inflammatory pattern between IP and contralateral NP and implied the oncogenic role of neutrophils in the pathogenesis of IP.
