Abstract JEM-EUSO is an international program for the development of space-based Ultra-High Energy Cosmic Ray observatories. The program consists of a series of missions which are either under development or in the data analysis phase. All instruments are based on a wide-field-of-view telescope, which operates in the near-UV range, designed to detect the fluorescence light emitted by extensive air showers in the atmosphere. We describe the simulation software ESAF in the framework of the JEM-EUSO program and explain the physical assumptions used. We present here the implementation of the JEM-EUSO, POEMMA, K-EUSO, TUS, Mini-EUSO, EUSO-SPB1 and EUSO-TA configurations in ESAF. For the first time ESAF simulation outputs are compared with experimental data.

The data presented in this article are related to the research paper entitled “Observation of night-time emissions of the Earth in the near UV range from the International Space Station with the Mini-EUSO detector” (Remote Sensing of Environment, Volume 284, January 2023, 113336, https://doi.org/10.1016/j.rse.2022.113336).The data have been acquired with the Mini-EUSO detector, an UV telescope operating in the range 290-430 nm and located inside the International Space Station. The detector was launched in August 2019, and it has started operations from the nadir-facing UV-transparent window in the Russian Zvezda module in October 2019. The data presented here refer to 32 sessions acquired between 2019-11-19 and 2021-05-06. The instrument consists of a Fresnel-lens optical system and a focal surface composed of 36 multi-anode photomultiplier tubes, each with 64 channels, for a total of 2304 channels with single photon counting sensitivity. The telescope, with a square field-of-view of 44°, has a spatial resolution on the Earth surface of 6.3 km and saves triggered transient phenomena with a temporal resolution of 2.5 µs and 320 µs. The telescope also operates in continuous acquisition at a 40.96 ms scale.In this article, large-area night-time UV maps obtained processing the 40.96 ms data, taking averages over regions of some specific geographical areas (e.g., Europe, North America) and over the entire globe, are presented. Data are binned into 0.1° × 0.1° or 0.05° × 0.05° cells (depending on the scale of the map) over the Earth's surface. Raw data are made available in the form of tables (latitude, longitude, counts) and .kmz files (containing the .png images). These are – to the best of our knowledge – the highest sensitivity data in this wavelength range and can be of use to various disciplines.

La vertigine parossistica posizionale benigna (VPPB) è il tipo più comune di vertigine periferica. Frequentemente dopo il primo episodio la VPPB presenta recidive, con un tasso di ricorrenza tra il 15% ed il 50%. Ad oggi non vi è chiarezza sui processi eziopatogenetici che portano al distacco degli otoconi né su quali siano i fattori che rendono la VPPB una patologia recidivante, ma recenti studi epidemiologici hanno evidenziato una possibile associazione con fattori di rischio cardiovascolari. Lo scopo del presente studio (Sesto Senso Survey) è stato quello di valutare nella popolazione italiana, attraverso unindagine osservazionale, le principali caratteristiche demografiche e cliniche dei pazienti con VPPB (primo episodio o ricorrente), con particolare attenzione ai potenziali fattori di rischio cardiovascolare. Lindagine è stata condotta in 158 centri di Vestibologia in tutta Italia su 2.682 pazienti (età media 59,3 ± 15,0 anni; 39,1 maschi e 60,9% femmine) affetti da VPPB, da gennaio 2013 a dicembre 2014. I risultati hanno mostrato in questi pazienti lalta prevalenza di fattori di rischio cardiovascolari come ipertensione arteriosa (55,8%), ipercolesterolemia (38,6%) e diabete (17,7%), oltre ad una elevata familiarità per malattie cardiovascolari (49,4%). In unelevata percentuale di pazienti si è inoltre registrata la presenza di ipoacusia (42,9%), acufeni (41,2%) o entrambi (26,8%). Significativamente correlata agli episodi di recidiva di VPPB è risultata la presenza di ipertensione arteriosa, dislipidemia e comorbidità cardiovascolare accertata (range OR tra 1,84 e 2,31). Rilevanti anche le associazioni con diabete e patologie tiroidee e autoimmuni (range OR tra 1,73 e 1,89). I risultati dellindagine confermano la significativa associazione tra comorbidità cardiovascolari e VPPB recidivanti e le identificano come importante potenziale fattore di rischio per le recidive di VPPB nella popolazione italiana, aprendo la strada alla valutazione di nuove strategie terapeutiche nel trattamento di questa patologia.

Negli ultimi anni, il crescente sviluppo di dispositivi acustici ha condotto a unanalisi critica dei metodi standard che sono stati impiegati per valutare la funzione uditiva. Gli esami audiologici tradizionali, basati sulla soglia audiometrica tonale e sulle liste di parole mono/ bisillabiche nel silenzio, si sono nel tempo dimostrati inadeguati perché troppo distanti dalla realtà. Un test audiometrico vocale nel rumore, che utilizza un metodo roving-level adattivo, adopera segnali target e segnali competitivi modificabili con lo scopo di riprodurre le condizioni di eloquio della vita quotidiana, quindi esplorare un più ampio range uditivo. A oggi, solamente pochi test roving-level adattivi sono disponibili in letteratura. Gli autori hanno condotto un test roving-level adattivo in adulti italiani sani, al fine di ottenere nuovi dati normativi in una lingua di origine latina.

La malattia MYH9-correlata è una rara sindrome genetica caratterizzata da piastrinopenia congenita associata al rischio di sviluppare, durante linfanzia o letà adulta, ipoacusia neurosensoriale, nefropatia e cataratta presenile ad andamento evolutivo. Furono inclusi in uno studio retrospettivo tutti i casi con sordità da severa a profonda arruolati consecutivamente nel Registro Italiano dei pazienti affetti da malattia MYH9-correlata. La popolazione esaminata coinvolse 147 pazienti Italiani con malattia MYH9-correlata: lipoacusia fu identificata nel 52% dei casi e solo 4 pazienti (6%) presentarono un quadro di sordità da severa a profonda alletà media di 33 anni. In tutti i 4 pazienti, la sordità fu associata ad un lieve sanguinamento spontaneo e in 3 pazienti fu accompagnata da un coinvolgimento renale. Limpianto cocleare fu eseguito in 3 casi, con beneficio, in assenza di complicanze maggiori. La diagnosi di malattia MYH9-correlata fu eseguita circa 28 anni dopo la prima manifestazione clinica della malattia che non fu mai sospettata da un otorinolaringoiatra. Saranno discussi gli aspetti clinici e diagnostici di 4 pazienti con sordità da severa a profonda affetti da malattia MYH)-correlata, focalizzando anche le implicazioni terapeutiche.

La vertigine e l’instabilità sono sintomi molto comuni nella popolazione la cui prevalenza è stimata tra il 20 e il 56%. L’obiettivo del nostro lavoro è stato quello di determinare la prevalenza di questi sintomi in una popolazione di 2672 soggetti. È stato somministrato loro loro un questionario; nella prima parte sono stati richiesti i dati demografici e se avessero mai sofferto di vertigine o instabilità nella loro vita. L’età media del campione è stata di 48,3 ± 15 anni, il 46,7% erano maschi. Sul totale della popolazione 1077 (40,3%) hanno riferito di aver sofferto di vertigine o instabilità nella loro vita, con un primo episodio occorso all’età di 39,2 ± 15,4 anni. Nella seconda parte del questionario sono state indagate le caratteristiche delle vertigini (età del primo episodio, il tipo di vertigine, presenza di più episodi, esacerbazione posizionale della vertigine, presenza di sintomi cocleari infine la presenza di cefalea da moderata o severa nel corso della vita e le sue caratteristiche cliniche (riferita a un emicrania, pulsante, associata a fono o fotofobia, peggiore con l’attività fisica). È stata osservata una correlazione della vertigine con l’età e con il sesso, essendo la prima 4,4 volte più frequente nelle donne e 1,8 volte nei soggetti con oltre 50 anni. Sul campione complessivo di 2672 soggetti, 13,7% hanno riferito vertigine rotatoria, 26,3% episodi recidivanti, 12,9% esacerbazione correlata alla posizione e il 4,8% presenza sintomi cocleari; il 34,8% ha lamentato cefalea nel corso della loro vita. I soggetti affetti da cefalea presentavano un’incidenza aumentata di vertigini recidivanti, di esacerbazione correlata alla posizione, di sintomi cocleari e un’età più giovane di comparsa del primo episodio di vertigine/instabilità. Nella discussione i nostri dati sono stati confrontati con quelli di precedenti studi. Gli autori sottolineano la correlazione tra vertigine/instabilità da un lato e cefalea con caratteristiche emicraniche dall’altro.

PURPOSE: Invasive mechanical ventilation is associated with complications, and its abbreviation is desirable. The imbalance between increased workload, decreased inspiratory muscle strength and endurance is an important determinant of ventilator dependence. Low endurance may be present due to respiratory muscle atrophy, critical illness, or steroid use. Specific inspiratory muscle training may increase or preserve endurance. The objective of the study was to test the hypothesis that inspiratory muscle training from the beginning of mechanical ventilation would abbreviate the weaning duration and decrease reintubation rate. As a secondary objective, we described the evolution of inspiratory muscle strength with and without inspiratory muscle training. METHODS: Prospective, randomized clinical trial in an adult clinical-surgical intensive care unit. Twelve patients trained the inspiratory muscles twice a day, and 13 patients did not (control). Training was performed adjusting the sensitivity of the ventilator based on the maximal inspiratory pressure. Patients underwent daily surveillance of the maximal inspiratory pressure. RESULTS: The weaning duration (31 ± 22 hr, control and 23 ± 11 hr, training group; P = .24) and reintubation rate (5 control and 3 training group; P = .39) were not statistically different. The maximal inspiratory pressure of the control group showed a trend toward a modest increase. In contrast, the training group showed a small decrease (P = .34). CONCLUSIONS: In acute critically ill patients, inspiratory muscle training from the beginning of mechanical ventilation neither abbreviated the weaning duration, nor decreased the reintubation rate. Inspiratory muscle strength tended to stay constant, along the mechanical ventilation, with or without this specific inspiratory muscle training.

The results of laboratory wall turbulence experiments on a shear-driven rotating boundary layer are presented. The experiments were carried out in the Turin University Laboratory rotating water tank. The flow was generated by changing the rotation speed of the platform and measured by means of particle image velocimetry. In order to analyse the influence of the rotation and of surface roughness, different cases were examined. Several rotation periods were considered. The measurements were performed both over a smooth surface and over a rough-to-smooth transition. Mean flows and the higher-order moments of the velocity probability density function are shown and discussed together with a comparison of the different experimental cases, theory and large-eddy simulations. A spin wave majority fork-like structure with feature size of 40 nm, is presented and investigated, through micromagnetic simulations. The structure consists of three merging out-of-plane magnetization spin wave buses and four magneto-electric cells serving as three inputs and an output. The information of the logic signals is encoded in the phase of the transmitted spin waves and subsequently stored as direction of magnetization of the magneto-electric cells upon detection. The minimum dimensions of the structure that produce an operational majority gate are identified. For all input combinations, the detection scheme employed manages to capture the majority phase result of the spin wave interference and ignore all reflection effects induced by the geometry of the structure.

In this study we wanted to verify the diagnostic value of tilt table test (TTT) to predict the efficacy of cardiac pacing (PM) for preventing recurrences of neurally mediated reflex syncope (NMS) in patients with cardioinhibitory activity (CI) documented by implantable loop recorder (ILR). Among patients selected by ILR in the context of severe clinical presentation of NMS, we analysed those who underwent PM implantation. In this observational and retrospective study we wanted to verify the results of TTT in the groups of treated patients with and without recurrences. We analysed 24 patients treated using a PM (10 male and 14 female, mean age 70 years). During an average follow-up period of 35 months the recurrence of syncope occurred in 7 patients (29%). 17 patients (71%) had not recurrences. TTT was negative in 15 patients out of 17 without recurrences (88%). Among the 7 patients with recurrences TTT was positive in 4 patients (57%). In conclusion, in this selected group of patients, a positive response to TTT is more likely correlated with a higher frequency of recurrences of syncope, while a negative response seems to predict the success of the pacing therapy.
