Introduction. The EORTC 22922/10925 trial investigated the potential survival benefit and toxicity of elective irradiation of the internal mammary and medial supraclavicular (IM-MS) nodes Accrual completed in January 2004 and first results are expected in 2012. We present the toxicity reported until year 3 after treatment. Patients and methods. At each visit, toxicity was reported but severity was not graded routinely. Toxicity rates and performance status (PS) changes at three years were compared by x2 tests and logistic regression models in all the 3 866 of 4 004 patients eligible to the trial who received the allocated treatment. Results. Only lung (fibrosis; dyspnoea; pneumonitis; any lung toxicities) (4.3% vs. 1.3%; p < 0.0001) but not cardiac toxicity (0.3% vs. 0.4%; p = 0.55) significantly increased with IM-MS treatment. No significant worsening of the PS was observed (p = 0.79), suggesting that treatment-related toxicity does not impair patient's daily activities. Conclusions. IM-MS irradiation seems well tolerated and does not significantly impair WHO PS at three years. A follow-up period of at least 10 years is needed to determine whether cardiac toxicity is increased after radiotherapy. [ABSTRACT FROM AUTHOR]

Raising plasma free fatty acid (FFA) levels reduces muscle glucose uptake, but the effect of FFAs on splanchnic glucose uptake, total glucose output, and glucose cycling may also be critical to producing lipid-induced glucose intolerance. In eight normal volunteers, we measured glucose turnover and cycling rates ([2H7]glucose infusion) during a moderately hyperglycemic (7.7 mmol/l) hyperinsulinemic clamp, before and after ingestion of a labeled (dideuterated) oral glucose load (700 mg/kg). Each test was performed twice, with either a lipid or a saline infusion; four subjects also had a third test with a glycerol infusion. As shown by similar rates of exogenous glucose appearance, the lipid infusion did not reduce first-pass splanchnic glucose uptake (saline 1.48+/-0.18, lipid 1.69+/-0.17, and glycerol 1.88+/-0.17 mmol/kg per 180 min; NS), but it reduced peripheral glucose uptake by 40% (P < 0.01 vs. both saline and glycerol infusions). Before oral ingestion of glucose, total glucose output was similarly increased by the lipid and glycerol infusions. Total glucose output was significantly increased by FFAs after oral ingestion of glucose (saline 3.68+/-1.15, glycerol 3.68+/-1.70, and lipid 7.92+/-0.88 micromol x kg(-1) x min(-1); P < 0.01 vs. saline and P < 0.05 vs. glycerol). The glucose cycling rate was approximately 2.7 micromol x kg(-1) x min(-1) with the three infusions and tended to decrease all along the lipid infusion, which argues against a stimulation of glucose-6-phosphatase by FFAs. It is concluded that in situations of moderate hyperinsulinemia-hyperglycemia, FFAs reduce peripheral but not splanchnic glucose uptake. Total glucose output is increased by FFAs, by a mechanism that does not seem to involve stimulation of glucose-6-phosphatase. [ABSTRACT FROM AUTHOR]

La radiothérapie externe avec chimiothérapie concomitante (5-fluoro-uracile–mitomycine C) est le traitement de référence du cancer de canal anal. Une analyse des données de sept essais a été faite pour quantifier l'effet des paramètres relié avec la radiothérapie externe sur les résultats thérapeutiques. Après fusion des jeux de données l'impact des paramètres de radiothérapie (étalement, durée de l'intervalle de temps, dose totale) sur les résultats thérapeutiques (récidive locorégionale, taux de survie sans progression à 5 ans et toxicité) ont été évalués. Les données ont été reçues de dix essais publiés sur 13, réalisés entre 1987 et 2008 (n = 3031). Une régression de Cox a été utilisée. Seuls les dossiers de 1343 patients des sept essais plus récents et homogènes ont été analysés (durée médiane de suivi :4,1 ans). Un taux supérieur de récidive locorégionale à 5 ans avait une corrélation significative avec un étalement plus long (22,8 %, intervalle de confiance à 95 % [IC 95 %] : 22,3–27,3 %, p = 0,030), une taille tumorale supérieure (p < 0,001) et avec les sexe masculin (p = 0,045). Les analyses par sous-groupe de la récidive locorégionale(intervalle de dose : 50,4–59 Gy) semblaient en faveur des doses plus basses (p = 0,412). La comparaison avec un intervalle de temps de deux semaines (dose : 59,4 Gy) suggérait que trois semaines pouvaient être désavantageux (p = 0,245), et ne montrait pas de différence (p = 0,89) avec aucun intervalle de temps (intervalle de dose : 55–59,4 Gy). La probabilité de survie sans progression à 5 ans était 65,7 % (IC 95 % : 62,8–68,5 %). Des taux de survie sans progression plus élevés ont été observés chez les femmes (p < 0,001), les tailles tumorales plus petites (p < 0,001) et les étalements plus courts (p = 0,025). La survie globale à 5 ans était en relation avec les femmes (76,7 % ; IC 95 % : 73,9 % > 79,3 % ; p < 0,001), tailles tumorales plus petites (p = 0,027) et étalements plus courts (p = 0,026). Les données descriptives de la toxicité seront présentées. Pour des patients pris en charge par chimioradiothérapie concomitante par deux agents et une radiothérapie externe, un étalement plus long semble désavantageux. De futurs essais avec des techniques modernes pourraient mieux définir l'étalement optimal et le rôle de l'escalade de dose. [ABSTRACT FROM AUTHOR]

In this retrospective study we evaluated the long-term results of 35 early-stage favourable T1-2 N0 M0 anal cancer patients treated with intensity-modulated radiotherapy techniques combining low dose prophylactic inguinal-pelvic irradiation with dose-escalated boost. Optimal locoregional control and good tolerance makes this treatment a valuable alternative to brachytherapy boost and involved-field radiotherapy plans. [ABSTRACT FROM AUTHOR]
