Background: Little information is available regarding the incidence and clinical outcome of the SARS-CoV2 infection in patients with multiple sclerosis (pwMS). Objective: To determine the incidence, clinical outcome, and impact of COVID-19 on pwMS. Methods: This observational study was prospectively performed on a cohort of pwMS (N = 11,560) followed up by 47 out of 51 Brazilian MS referral centers that registered pwMS with COVID-19 at the REDONE platform from 13 March to 4 June 2020. Results: The incidence of COVID-19 for pwMS patients was 27.7/10,000 patients and for the general population was 29.2/10,000 inhabitants. A total of 94 (77 women) pwMS patients, aged 40 ± 10.25 years, presenting 9.9 ± 8.6 years of MS disease duration, developed the COVID-19, most of them (87%) exhibited the mild form of the disease. Eighty (96%) patients maintained the use of MS disease-modifying treatment (DMT) during COVID-19 pandemic and 14 patients were not in use of DMTs. Conclusion: Incidence of COVID-19 in Brazilian pwMS was not different from those observed for the general Brazilian population. Natalizumab is a therapeutic option for treating multiple sclerosis (MS) and is particularly efficacious for patients with highly active disease. A long washout period has been recommended between withdrawal of natalizumab and start of fingolimod (another option for treating MS). This long washout period has been associated with a significant increase in MS activity. In the present study, a group of 96 patients who were switched from natalizumab to fingolimod had short washout periods between drugs, or monthly corticosteroid pulse therapy if longer washout periods were recommended. This therapeutic approach led to the lowest reported relapse rate so far, among patients with MS switching from natalizumab to fingolimod (8.3%). Multiple sclerosis (MS) is a chronic, neurological, immune-mediated disease that can worsen in the postpartum period. There is no consensus on the use of immunoglobulin for prevention of disease relapses after delivery. The article describes the case of a 51-year-old woman diagnosed with meningioma during treatment with interferon beta (INFb)-1a for multiple sclerosis (MS) in São Paulo, Brazil. The growth of meningioma in the patient was confirmed by magnetic resonance imaging (MRI). The role of INFb therapy in the enlargement of meningiomas is tackled. According to the authors, the case does not confirm a definite relationship between meningioma growth, the presence of transforming growth factor-beta (TGF-b) receptor in the tumor cells and INFb therapy.

Neuropsychological studies have consistently reported cognitive dysfunctions associated with multiple sclerosis. One-hundred fifteen subjects with relapsing-remitting multiple sclerosis (RRMS) were compared with forty health controls according to a neuropsychological test battery, which included digit span, trail making, cancellation and stroop test. Both groups were matched for age, sex and educational level. Subjects with RRMS had a worse performance the speed of response. Subjects with RRMS spent more time to complete the test in either sections A (p=0.001) or B (p=0.001), although there was no significant difference in terms of number of errors. The total time required to finish the Stroop test was higher for subjects with RRMS (p<0.001), being the time difference between groups significant at trial 4 (p<0.001). Attention impairment in subjects with RRMS is related to slowed central processing, which may be affected in all stages, including impairment of automatic and controlled processing of information and in the motor program.
