14. Prognostic Value of Admission Lactate Level in Patients With Myocardial Infarction With ST Segment Elevation.

Abstract

Lactate is a product of anaerobic metabolism and increases in states of tissue hypoxia. Furthermore, it is demonstrated that lactate level is correlated with the extent of infarct area and short term (30-day) mortality rates in patients with acute coronary syndrome with ST segment elevation (STE-ACS). Therefore, we aimed to test the prognostic value of this easily obtainable parameter on outcomes in patients presenting with STE-ACS. All adult patients who admitted with STE-ACS between 2019 and 2020 were screened. The cases with admission lactate levels were included. Patients were separated into two groups as low (<2mmol/l) and high (>2mmol/l) lactate groups. A total of 70 patients were enrolled. The mean age of the study population was 62.3 ± 15.0 years and 53 (75.7%) of them were male. The most common infarct related artery was the left anterior descending (LAD) artery. The median lactate level was 2.5 (0.80-15.3) mmol/L. Malignant arrhythmia, contrast induced nephropathy (CIN), and in-hospital mortality rates were not different between the high and low lactate groups. However, all-cause mortality was significantly higher in the high lactate group during the follow-up (p=0.005). Among all included parameters; lactate level [OR:1.76, (CI: 1.28-2.42); p<0.001] and age [OR:1.10, (CI: 1.03-1.17); p=0.004] were predictors for all-cause mortality. Admission lactate level can predict all-cause mortality in patients with STE-ACS. In addition, high admission lactate levels could help to raise more attention even if the patients have been discharged from the hospital. [ABSTRACT FROM AUTHOR]

Abstract

Background/Introduction: Atrial fibrillation occurs in the setting of ST-elevation myocardial infarction (STEMI) and it is associated with a worse prognosis. Purpose: We aimed to evaluate the effects of de novo AF development on cardiovascular outcomes in STEMI cases. Methods: We screened all of the STEMI cases between January 2018 and January 2021. Patients with de novo AF during hospitalization due to STEMI were compared with 1:1 propensity matched STEMI cases without AF. Results: A total of 68 patients were enrolled. Mean age of the study population was 65.5 ±11.5 years and vast majority of the patients were male (66.2%). Baseline characteristics were similar between the two groups and demonstrated in Table-1. Sustained ventricular arrhythmia rates were similar between the two groups (5.8& vs. 5.8%; p=1.000). Contrast induced nephropathy was more frequent in patients with AF, however it was not statistically significant (14.7% vs. 2.9%; p=0.197). Bleeding episodes were classified into major and minor bleeding according to the need of transfusion and presence of lifethreatening hemorrhage. Major bleeding was occured in only one case (Upper gastrointestinal bleeding) and minor bleeding rates were similar (14.7% vs. 5.8%; p=0.427). In-hospital mortality was distinctly higher in patients with AF, whereas it was not statistically significant (20.5% vs. 2.9%; p=0.054). Outcomes were summarized in Table-1. Conclusion(s): Contrast induced nephropathy, in-hospital mortality and bleeding rates are higher in STEMI patients with AF than counterparts despite it is not statistically significant. Evaluating the Effect of Radiofrequency Catheter Ablation Treatment on the Disease Outcomes of the Patients with Ischemic Ventricular Tachycardia, Hacettepe University Faculty of Medicine, Cardiology Thesis, Ankara, 2019. Scar homogenization based catheter ablation has lower recurrence rates than clinical ventricular tachycardia (VT) catheter ablation in patients with recurrent ischemic VT episodes. The purpose of this study is to define the success and recurrence rates of the scar homogenization based radiofrequency (RF) catheter ablation, complications of this procedure, pre and post-procedural mortality and the predictors of the outcome depending on patient and procedure in patients with ischemic cardiomyopathy and VT. Patients that received catheter ablation between the dates of 01.06.2014 - 30.11.2018 due to ischemic VT were included in the study. The basic characteristic features of the patients such as age, gender, comorbidity and routine pre-procedural laboratory parameters, prescribed drugs and echocardiographic parameters were recorded. As the endpoints of the study, continuous VT attack development, history of treatment of the patients with implantable cardioverter defibrillator (ICD), in-hospital, early and late mortality rates and pre-procedural complications were evaluated. The study population included 128 patients as 120 male (93.8%) and 8 female (6.2%) patients. The mean age of the patients were found as 66±10 years. The ratio of the patients that applied to the hospital with electrical storm before ablation was found as 40.6% (52). Mean left ventricular ejection fraction (LVEF) of the patients was 29.6%±7.2 and LVEF was found as 2 (HR:1,91; %95 CI:1,05-3,66; p=0,045), LVEF (HR:0,95; %95 CI:0,91-0,98; p=0,038) and brain natriuretic peptide (BNP) levels (HR:1,10; %95 CI:1,03-1,18; p=0,004) were found as the most significant predictors of the all-cause mortality; LVEF level (; HR:0,93; %95 CI:0,88-0,98; p=0,013), BNP level (HR:1,07; %95 CI:1,02-1,11; p=0,002) and proper shock amount after first ablation procedure (HR:1,12; %95 CI:1,03-1,22; p=0,011) were found as the most significant predictors of the cardiovascular mortality. Bu çalışmanın amacı; iskemik kardiyomiyopatisi olan ve VT saptanan hastalarda skar homojenizasyonu temelli radyofrekans (RF) kateter ablasyonunun, bu hasta grubunda başarı ve rekürrens oranlarını, işleme bağlı komplikasyonları, peri ve post-prosedürel mortaliteyi ve bu sonlanımların hastaya ve işleme bağlı öngördürücülerini belirlemektir. Çalışmaya 01.06.2014-30.11.2018 tarihleri arasında iskemik VT nedeniyle kateter ablasyonu yapılmış olan hastalar alındı. Hastaların yaş, cinsiyet, komorbidite gibi bazal karakteristik özellikleri, işlem öncesi rutin laboratuvar parametreleri, kullandıkları ilaçlar ve ekokardiyografik parametreleri kaydedildi. Ablasyon öncesi elektriksel fırtına ile başvuran hasta oranı %40,6 (52) olarak saptandı.. Abstract

We aimed to present the long-term prognostic role of coronary computed tomography angiography (CTA) in a cohort of patients with coronary artery disease (CAD) and noncritical stenosis. A total of 1138 patients who underwent coronary CTA for suspected CAD were included in the study. For the categorization of the coronary atherosclerotic plaque (CAP), the coronary system was divided into 16 segments. For each segment, CAPs were categorized as calcified, noncalcified, and mixed. All-cause and cardiovascular (CV) mortality data were collected for prognostic evaluation. Coronary CTA analyses showed that 34.5% of patients had noncalcified CAP, 14.5% of patients had calcified CAP, and 11% of patients had mixed CAP. During a median of 141.5 months follow-up, CV and all-cause mortality was observed in 57 (5%) and 149 (13.1%) patients, respectively. In multivariable Cox regression analysis, calcified CAP morphology and the extent of involved segments were significant predictors of both CV and all-cause mortality. The presence of calcified CAP morphology and the higher number of diseased coronary segments via coronary CTA might help stratify patients at risk for adverse CV outcomes during long-term follow-up. Patients with these features at index coronary CTA may be evaluated more closely with aggressive preventive measures. [ABSTRACT FROM AUTHOR]
