Abstract

Objectives: In this study, we aim to determine the frequency of adherence to National Comprehensive Cancer Network follow-up guidelines in a population of head and neck cancer patients who received curative treatment. We will also assess the impact of race, ethnicity, socioeconomic status, and treatment setting on utilization of follow-up care.Methods: This study included patients with biopsy-proven, nonmetastatic oropharyngeal or laryngeal cancer treated with radiotherapy between January 1, 2014, and June 30, 2016, at a safety-net hospital or adjacent private academic hospital. Components of follow-up care analyzed included an appointment with a surgeon or radiation oncologist within 3 months and posttreatment imaging of the primary site within 6 months. Univariable and multivariable analyses were conducted using a logistic regression model to estimate odds ratios and corresponding 95% confidence intervals.Results: Two hundred and thirty-four patients were included in this study. Of those, 88.8% received posttreatment imaging of the primary site within 6 months; 88.5% attended a follow-up appointment with a radiation oncologist within 3 months; and 71.1% of patients attended a follow-up appointment with a surgeon within 3 months. On multivariable analysis, private academic hospital treatment versus safety-net hospital treatment was associated with increased utilization of both surgical and radiation oncology follow-up. Non-Hispanic black (NHB) patients, Hispanic patients, and those with a low socioeconomic status were also less likely to receive follow-up.Conclusion: Safety-net hospital treatment, socioeconomic status, Hispanic ethnicity, and NHB race were associated with decreased follow-up service utilization. Abstract

In this work, a comprehensive numerical model is presented for a multi-quantum well pin photovoltaic device. The model accounts for the fundamental carrier dynamics in the quantum wells without assuming thermal equilibrium between the confined carriers and crystal lattice. It is based on a self-consistent solution of the Poisson, Schrodinger, and current continuity equations. In the quantum well regions, a second set of continuity equations is coupled to these three equations to account for the recombination, generation, escape, and capture in the quantum well. The effect of these quantum well rates on device performance is examined for various well configurations and material parameters. The simulation predicts that efficiency improvements above a baseline Al{dollar}\sb{\rm x}{dollar}Ga{dollar}\sb{\rm 1-x}{dollar}As control cell are possible for low mole fractions of aluminum. Furthermore, with proper design, a multiple quantum well device can exceed the efficiency of a similar quality, bulk GaAs pin device.

Abstract

A new metrology method has been developed for the monitoring of advanced gate dielectric processes associated with 0.1 µm technology. Unlike previous techniques that involved corona or Hg gate based methods, this technique measures all gate dielectric parameters on product as well as monitor wafers. This is accomplished with a specially designed small area elastic pressure probe contacting the gate oxide within the scribe lines of 200 or 300 mm wafers. The elastic probe method utilizes a semiconductor compatible material, which is specially conditioned to produce a nondamaging, highly repeatable contact. Extensive damage evaluations have been performed and indicate that no observable physical or electrical damage is introduced into the silicon by these pressure contacts. Critical parameters such as equivalent oxide thickness (EOT), threshold voltage, channel doping profile, oxide and interface trap charge, and leakage current can be determined with high precision and accuracy. For example, EOT repeatabilities of less than 0.3% (one sigma) have been obtained. The technique is also highly useful for process development, especially in the area of ultra-thin gate oxides and advanced gate dielectrics. [ABSTRACT FROM AUTHOR]

47. (S006) Mission Improvement: Lessons From Initiating a Resident-Led Quality Improvement Project on Smoking Cessation at a County Hospital.

Abstract

BACKGROUND: To ensure that patients receive the highest quality of care, the Accreditation Council for Graduate Medical Education and the American Board of Radiology have made quality improvement projects a required component of training. Quitting smoking has been shown to decrease toxicity and improve outcomes in patients undergoing cancer therapy. In an effort to decrease lack of access related to cost and to improve outcomes for patients undergoing radiation therapy, a protocol to provide free smoking cessation medications to an underserved population was implemented at a county hospital. This report describes the initial implementation of our prospective smoking cessation quality improvement study. METHODS: This study prospectively enrolled a culturally diverse group of currently smoking patients at a safety net hospital and provided them with nicotine replacement or varenicline (if they had previously failed to respond to nicotine replacement) in an effort to help them quit smoking. The primary outcomes were self-reported smoking cessation at the end of the 12-week smoking cessation program and at the 3-month follow-up. RESULTS: We have garnered a $10,000 grant, received institutional review board (IRB) approval, enrolled seven patients (both Spanish- and English-speaking), and completed the 12-week program with three patients. Bringing such a project to fruition can be rather daunting; however, engaging in this project has allowed residents to gain insight into applying for a grant, writing a protocol, going through the IRB approval process, obtaining consent from patients, training coinvestigators, keeping track of patient toxicities/outcomes, and finding ways to successfully enroll patients. CONCLUSION: Forming a successful quality improvement project is challenging at times but ultimately provides an excellent educational experience for residents and a rewarding opportunity to help patients.
