BackgroundSocial integration and mental health are vital aspects of healthy aging. However, close to half of Canadians older than 80 years report feeling socially isolated. Research has shown that social isolation leads to increased mortality and morbidity, and various interventions have been studied to alleviate loneliness among older adults. This proposal presents an evaluation of an intervention that provides one-on-one coaching, is intergenerational, provides both educational and socialization experiences, and increases technology literacy of older adults to overcome loneliness. ObjectiveThis paper describes the protocol of a randomized, mixed-methods study that will take place in Ontario, Canada. The purpose of this study is to evaluate if an intergenerational technology literacy program can reduce social isolation and depression in older adults via quantitative and qualitative outcome measures. MethodsThis study is a randomized, mixed-methods, feasibility trial with 2 conditions. Older adults in the intervention condition will receive 1 hour of weekly technological assistance to send an email to a family member, for 8 weeks, with the assistance of a volunteer. Participants in the control condition will not receive any intervention. The primary outcomes are loneliness, measured using the University of California, Los Angeles Loneliness Scale, and depression, measured using the Center for Epidemiologic Studies Depression scale, both of which are measured weekly. Secondary outcomes are quality of life, as assessed using the Older People’s Quality of Life-Brief version, and technological literacy, evaluated using the Computer Proficiency Questionnaire-12, both of which will be administered before and after the intervention. Semistructured interviews will be completed before and after the intervention to assess participants’ social connectedness, familiarity with technology, and their experience with the intervention. Despite the overwhelming support for the importance of sexual communication to intimate relationships, there is limited information about what motivates someone to engage in or avoid sexual communication. Motivational frameworks have been applied to various aspects of intimate relationships, serving as strong predictors of different behavioural processes and playing a crucial role in facilitating behavioural change. As such, we aimed to elucidate the motivations for sexual communication and explore how they relate to other aspects of the process of sexual communication. A total of 373 participants were recruited from online crowdsourcing websites across two studies, and they completed online questionnaires using a mixed methods approach. In Study 1, open-ended responses regarding participants' motivations for sexual communication were inductively coded and aligned with the Regulatory Focus Theory, which describes two distinct modes of goal pursuit depending on if the person is focused on growth and advancement (i.e., promotion-focused) or safety and security (prevention-focused). This coding structure was replicated in Study 2, and we expanded the results to examine the predictive ability of the coded motivations. We found that those higher in attachment avoidance were more likely to have prevention-focused motivations, and those with relationship-oriented promotion-focused motivations reported more depth of both sexual and nonsexual communication as well as more relationship and sexual satisfaction. We investigated the extent to which individuals deliberately avoid the topic of pornography use with their partner and how such avoidance influences the association between pornography use and sexual satisfaction. We reasoned that such communicative avoidance may account for some of the negative relational effects associated with pornography use. A sample of 191 participants in mixed-sex relationships completed online measures of pornography use, sexual satisfaction, and domain-specific and overall measures of communicative avoidance. Men tended to avoid the topic of pornography use more than other topics that have the potential to cause relationship conflict. Women did not avoid the topic of pornography use more or less than other relationship topics. Controlling for pornography use, men and women who were more likely to avoid the topic of pornography use were less likely to be sexually satisfied; this association remained significant for men (but not women) after controlling their overall communicative avoidance. Lastly, men who were high pornography users and reported high levels of overall communication avoidance were most likely to report lower sexual satisfaction. Introduction Laryngopharyngeal reflux (LPR) is a syndrome caused by reflux of gastric contents into the pharynx or larynx which leads to symptoms of throat clearing, cough, pain, sensation of something stuck in the throat and change in voice. Evaluation of symptoms using the Reflux Symptom Index is considered to be the basic diagnostic procedure. LPR therapy is complex and requires modification of patient’s lifestyle and habits. LPR treatment and management is supposed to reduce the acidity or stomach contents and neutralize acidopeptidic activity in larynx, pharynx and oesophagus. High dosages of PPI (proton pump inhibitors) have shown the best effects in reducing reflux. Aims and objectives: The goal of the current study is 1. To determine the effects of gastric reflux on larynx and pharynx both clinically and endoscopically. 2. To evaluate the effect of proton pump inhibitors along with lifestyle modifications on laryngeal reflux symptom index and reflux finding score. LPR was treated with PPI, alkaline alginates and standard reflux precautions (avoidance of coffee, tea, chocolate, soda, greasy, fried, fatty, and spicy foods, alcohol and smoking). Results: The study constituted of 150 patients coming to ENT OPD with symptoms of laryngopharyngeal reflux and having RSI >13. All patients were examined by 70-degree video-laryngoscope and were treated with proton pump inhibitors, alginates and lifestyle modifications. 80 of them recovered after 2 weeks of lifestyle modification and treatment and 63 recovered after a month while 7 remained symptomatic after 3 months of treatment as well. Conclusion: Laryngopharyngeal reflux (LPR) is common in ENT practice but requires careful diagnosis due to its non-specific symptoms. Up to 32% of adults have experienced some form of childhood maltreatment (CM). Research has shown that these early childhood experiences are associated with a variety of interpersonal difficulties in adult sexual and romantic relationships. Polusny and Follette have suggested that these negative long-term effects are the result of emotional avoidance strategies that individuals use to cope with thoughts, feelings, and memories associated with CM. The present study tested this theorized mediational model with a sample of 150 participants, all of whom were currently in long-term relationships. Participants completed questionnaires designed to assess the severity of CM, fear of their own emotions, and a task designed to assess their accuracy at identifying emotions. Additionally, participants completed a variety of measures assessing the quality of their sexual and romantic relationships. These measures assessed their satisfaction, their communication, and the extent of problems in each domain. Our results showed that higher levels of CM were associated with lower satisfaction with communication and more problems in both the romantic and sexual domains. In regard to our mediational hypothesis, we found partial support showing that fear of one's own emotions and emotion recognition mediated some of these associations. The authors used cross-cultural methodology to examine the demand-withdraw pattern of marital communication. In Western countries, women usually make more demands, whereas men are more likely to withdraw. But the recently advanced marital structure hypothesis suggests that this pattern can be altered by gender roles and beliefs, particularly in traditional marriages. To test such hypotheses, the authors conducted an observational study of marital communication across very different cultures, with varying levels of patriarchy (i.e., 50 White American couples, 52 Pakistani couples in Pakistan, and 48 immigrant Pakistani couples in America). Across cultures, demand-withdraw communication was related to marital distress, extending previous findings to new groups. However, the findings challenge the notion that demanding and withdrawing behaviors are inherently male or female; rather, the results point to the relevance of contextual factors, specifically gender power differences and acculturation, in understanding the demand-withdraw marital interaction pattern. Therapists working with foreign or immigrant couples must consider the cross-cultural generalizability of existing theories of marital communication.

Increasingly, affirmative consent – direct, unambiguous and voluntary agreement to engage in sexual activity (Craig & McKinley, 2015) – is the standard being adopted by educational institutions in North America (Bennett, 2016). Yet, studies show that most individuals continue to communicate consent through nonresistance (Jozkowski et al., 2014a). Given this discrepancy, it is critical to understand what factors prevent individuals from engaging in affirmative consent. Furthermore, a better understanding of the perceived rewards of consent communication could incentivize the adoption of affirmative consent. To understand the range of perceived barriers and rewards, we conducted an online, qualitative study where 231 participants answered two open-ended questions. We used inductive content analysis to categorize participants' perceptions of sexual consent barriers and rewards into four general content areas: (1) Communication Quality, (2) Relational and Emotional Experiences, (3) Sexual Quality and (4) Safety and Coercion. These perceived rewards and barriers were examined through the lens of the Information-Motivation-Behavioural Skills Model. Participants viewed consent communication not only as a means of ensuring safety but also as a way to enhance relational and sexual quality. However, they also perceived barriers in all three of these domains as well as barriers to ensuring that sexual consent communication is fluid and easily understood. These findings provide important avenues for future research investigating how individuals reconcile perceived rewards and costs of affirmative consent communication. This study was conducted to respond the current demand of health-conscious consumers towards reduced-fat and fiber enriched foods. Apricot kernel flour (AKF) had significant amount of total phenol contents and 2, 2-diphenyl-1-picrylhydrazyl (DPPH) free radical scavenging activity (83.89±3.53%). Different concentrations (20%, 40%, 60%, 80% and 100%) of AKF were used to replace the shortening in supplemented cookies. In supplemented cookies, fat content reduced from 23.53±0.23% to 3.82±0.03% by increasing level of AKF. Spread value decreased in high percentage AKF supplemented cookies. Total dietary fibers (TDF) and texture values (by Texture Analyzer) increased significantly (p=0.05) from 2.63±0.09% to 31.82±1.11% and 3.27±0.11 N/kg to 6.02±0.21 N/kg by enhancing AKF supplementation respectively. Total calories reduced due to partially replaced fat content of supplemented cookies. Cookies (supplemented with 60% and 40% AKF) were recommended better for eating quality on the basis of physico-chemical properties, calories count, texture and organoleptic attributes. We used a mixed-method design to examine the attributions women in mixed-gender/sex relationships make for their partner's perceived pornography use and whether such attributions covary with women's relationship and sexual satisfaction. A final sample of 199 women completed measures of relationship satisfaction, sexual satisfaction, and their perceptions of partner's pornography use. Participants also completed three open-ended questions assessing their attributions of their partner's perceived pornography use. Qualitative analyses revealed 11 themes in women's attributions of their partner's perceived pornography use; some of the themes reflected on women themselves (e.g., being open-minded and accepting), whereas other themes reflected on the partner (e.g., partner is sexually bored) or the relationship (e.g., strong and trusting relationship). Furthermore, the women made positive, negative, and neutral attributions. Quantitative analyses showed that positive attributions were significantly more frequent than neutral or negative attributions and the latter two categories did not differ significantly from each other. Also, greater frequency of positive and neutral attributions and lower frequency of negative attributions were associated with higher relationship satisfaction and sexual satisfaction. Our results suggest that women make a range of attributions about their partner's pornography use and that this variation relates meaningfully to indices of relationship functioning. There is consistent empirical evidence to suggest that more open and positive sexual communication is a vital component of sexual relationships, but romantic partners tend to avoid sexual communication. Although clinical and theoretical writing has identified barriers to sexual communication, there is a relative paucity of empirical literature investigating specific barriers to sexual communication and whether these barriers are unique to sexual communication. We developed the Barriers to Communication Questionnaire, a measure that investigates the types of threat that are activated during couples' sexual and nonsexual communication (Study 1) and the degree to which these threats are differentially activated across the two discussions (Study 2). In Study 1, we found that the same categories of threats were activated for both sexual and nonsexual conflict communication: threat to self, threat to partner, and threat to relationship. Study 2 revealed that threat to self is activated to a greater degree during sexual conflict communication compared to nonsexual conflict communication. The differential degree to which threat to self is activated during sexual and nonsexual communication provides a plausible explanation for why romantic partners tend to avoid sexual communication more so than nonsexual communication. The measure developed in the current study could be used clinically to identify the specific threats that are preventing an individual from communicating with the partner about needs and desires. Although much research has described individual sexual dysfunctions, few studies to date have examined the types of problems that couples consider most significant in their sexual relationships. To clarify the types of relational sexual problems that are most common and most severe in the sexual lives of individuals in long-term romantic relationships. A community sample of 117 mixed-sex couples completed this in-lab study. Members of each couple separately completed a demographics questionnaire and a measure of their relational sexual problems, the Sexual Problems Questionnaire (SPQ). Descriptive analyses (eg, examination of means, frequency counts) were conducted to determine the most common and severe sexual problems reported by participants. t -Tests were performed to examine gender differences in mean severity ratings for each SPQ item. Qualitative data were examined by conducting a frequency count on the SPQ items that participants reported to be most important in their sexual relationships. Results of all frequency counts were divided by the total sample size and are reported as percentages. Participants reported on the severity of their sexual problems using the 25-item SPQ. Quantitative analyses revealed that the most common and problematic sexual problems endorsed by both sexes were frequency of sex, sexual initiation, and showing interest. A frequency count of participants' qualitative reports also revealed that frequency of sex (women = 36%; men = 39%), sexual initiation (women = 33%; men = 32%), and showing interest (women and men = 25%) were the most important sexual issues for most individuals. The most pressing relational sexual problems for couples in long-term romantic relationships are consistent between sexes and pertain to the domain of sexual desire. The current study used an expanded measure of sexual problems, which allowed participants to report on a broad range of issues in their sexual relationships. The direction of such relational sexual problems (eg, desiring more or less sexual frequency) was not explored. Our goal was to examine how implicit theories can be applied to understanding women's coping responses to sexual problems. The belief that sexual desire changes over time is a type of incremental theory, while the belief that sexual desire is stable is a type of entity theory (Dweck, 2012 ). We examined how different implicit theories of sexual desire influence how women cope with sexual desire challenges. Also, we sought to determine whether women's perceptions of experiencing a sexual desire challenge moderate the relationship between their implicit beliefs and their coping responses. We randomly assigned women to read an article designed to induce either an entity or incremental theory about sexual desire. We further asked them to indicate how true it is that they have experienced, or are likely to experience, a sexual desire problem (1 = not at all true to 4 = very true). They then completed a measure of coping with desire problems, the Modified Cope Inventory. Results from both studies showed that women primed with entity theories who expected to experience a sexual desire issue reported significantly more negative coping than women primed with incremental theories. Several prominent models of relationship satisfaction and sexual satisfaction imply directional relationships between these constructs (e.g., attachment theory, social exchange models of relationship satisfaction, the interpersonal exchange model of sexual satisfaction). Previous research has demonstrated that sexual satisfaction and relationship satisfaction are distinct but correlated constructs, but relatively few studies have examined how they are related over time. Thus, the purpose of this study was to examine this association. A sample of heterosexual couples (N = 113) completed a longitudinal study spanning 2 years. At Times 1 and 2 they completed measures of relationship satisfaction and sexual satisfaction. Data were analyzed according to the principles of the actor-partner interdependence model using structural equation modeling. Significant actor effects were detected such that, for both men and women, one's own earlier sexual satisfaction predicted one's later relationship satisfaction. In contrast, one's own earlier relationship satisfaction did not significantly predict one's subsequent sexual satisfaction. Sexual satisfaction was a stronger predictor of subsequent relationship satisfaction for men than women. There were no significant partner effects. These results contribute to our theoretical understanding of sexuality and sexual satisfaction in the context of long-term relationships by providing support for theories that conceptualize sexual satisfaction as one factor that contributes to relationship satisfaction. Sexual satisfaction is an important component of relationship well-being within romantic relationships. Yet, relatively little is known about the psychological factors that predict responses to the inevitable sexual challenges couples face. Four studies provide evidence that implicit theories of sexual attraction as either fixed or malleable predict responses to sexual challenges. In Studies 1 and 2, individual differences in these beliefs predicted (above and beyond other implicit theories, relationship beliefs, and measures of sexual desire) perceptions of success for a relationship lacking sexual chemistry. In Study 3, these beliefs predicted actual relationship outcomes in committed couples. Finally, in Study 4, these beliefs predicted willingness to engage in destructive behaviors in response to a sexual challenge-but not in response to a non-sexual challenge-in a hypothetical long-term relationship. Objective The purpose of the present study was to investigate the differential roles of depression and anxiety in intimate relationship satisfaction, both cross-sectionally and longitudinally. Method The cross-sectional sample comprised 70 couples, of which 48 couples also participated at follow-up. All couples completed measures of relationship satisfaction and symptoms of depression and anxiety. Results Cross-sectionally, actor symptoms of depression were the only predictor of relationship satisfaction, after controlling for symptoms of anxiety. Conversely, depressive symptoms did not predict change in relationship satisfaction over time above and beyond the effects of anxiety. Instead, actor symptoms of anxiety at time 1 predicted a decline in relationship satisfaction from time 1 to time 2. For wives, their husbands' levels of anxiety at time 1 also predicted longitudinal change in relationship satisfaction. In this article, we analyze the various types of social support – informational, instrumenmental, and emotional/psychological help – that are provided at a Sufi lodge in southern Punjab, Pakistan. We argue that the lodge has become an important factor in securing the well-being of individuals and families in a context where the state has largely failed in terms of providing social services for its citizens. We conclude that future research in this area could delve deeper into the question of whether, and if so how, such institutions may be a source of social support on a wider basis in Pakistan and elsewhere in South Asia. The purpose of the present study was to compare the emotional reactions of depressed and nondepressed individuals to experiences of romantic rejection versus acceptance. We tested our hypotheses in a sample of 28 depressed and 43 nondepressed undergraduate students. In support of self-consistency theories, the results showed that depressed individuals reported significantly greater negative mood in the romantic acceptance versus rejection condition, while there was no significant difference across the two conditions in the self-reported mood of nondepressed individuals. Further, symptoms of anxiety mediated the interactive effects of depression status and rejection status on mood. In previous batterer typology studies, only 1 study gathered longitudinal data and no research examined whether subtypes continue to differ from one another over time. The present study did so. We predicted that, at 1.5- and 3-year follow-ups, the subtypes identified at Time 1 (A. Holtzworth-Munroe, J. C. Meehan, K. Herron, U. Rehman, G. L. Stuart, 2000; family only, low level antisocial, borderline/ dysphoric, and generally violent/antisocial) would continue to differ in level of husband violence and on other relevant variables (e.g., generality of violence, psychopathology, jealousy, impulsivity, attitudes toward violence and women). Although many group differences emerged in the predicted direction, not all reached statistical significance, perhaps because of small sample sizes. Implications of the findings (e.g., not alt marital violence escalates; possible overlap of the borderline/dysphoric and generally violent/antisocial subgroups) are discussed, as are methodological issues (e.g., need for more assessments over time, the instability of violent relationships). We investigated how depressive symptoms in husbands and wives may affect patterns of interpersonal behavior during marital conflict discussions. Using the Continuous Assessment of Interpersonal Dynamics (CAID) approach, observers rated moment-to-moment levels of dominance and affiliation for each partner, from which dynamic indices were derived, including the slopes for each partner and the degree of rhythmic entrainment between partners. Results supported predictions that the wife's depressive symptoms would be related to alterations in the dynamics of dominance, whereas the husband's depressive symptoms would be related to alterations in the dynamics of affiliation. For example, the higher the husband's depressive symptoms, the less affiliative both the wife and husband became over the interaction and the less entrained the partners were on affiliation.
